Przedmiot wzoru przemyslowego, którego istota przejawia sie w ukladzie elementów oraz ich kolorystyce, uwidoczniony jest na zalaczonym rysunku przedstawiajacym widok calosci formularza i usytuowanie wszystkich elementów prostokatnych oraz ich wzajemne polaczenia kolorowymi strzal- kami, a takze opisy poszczególnych etapów postepowania zaznaczone kolorystycznie prostokatnymi ramkami, umieszczonymi w dolnej czesci formularza. Poszczególne ramki dolnej czesci formularza, umieszczone poziomo kolejno jedna za druga, zawieraja opisy: WYBIERZ PRODUKT, OKRESL KWOTE, PODAJ SWOJE DANE OSOBOWE, DECYZJA, JAK CHCESZ OTRZYMAC PIENIADZE, WYPLATA GOTÓWKI. W poszczególnych mniejszych prostokatnych ramkach, wypelniajacych srodkowa czesc formula- rza, umieszczony jest tekst o tresci informacyjnej, odpowiadajacy tematycznie etapowi formularza i czynnosci która nalezy wykonac. Kazda ramka posiada kontur w kolorze niebieskim oraz biale tlo, natomiast umieszczony wewnatrz tekst napisany jest pismem drukowanym w kolorze czarnym. The essence of the industrial design, the essence of which is reflected in the arrangement of elements and their colors, is shown in the attached drawing, which shows a view of the entire form, the location of all rectangular elements and their interconnections with colored arrows, as well as descriptions of the individual stages of the procedure, marked with colored rectangular frames at the bottom of the form. Individual frames in the lower part of the form, arranged horizontally, contain the descriptions: SELECT A PRODUCT, SPECIFY THE AMOUNT, PROVIDE YOUR PERSONAL DATA, DECIDE HOW YOU WANT TO RECEIVE THE MONEY, AND WITHDRAW CASH. In the upper left corner of the form is a rectangular field intended for the logo of the banking product vendor, while below, in the lower corner, is a field with a titular description of the banking product type and general information useful when completing the form. On the right side of the form is a rectangular field labeled "Funds have been disbursed" and "The agreement will be sent to the correspondence address." On the inside, diagonal lines align with the field's outline, their other ends connecting to the outline of the fields arranged in columns and thematically related to the final step of the procedure. Individual smaller rectangular frames, filling the center of the form, contain informative text that thematically corresponds to the form's stage and the action to be performed. Each frame has a blue outline and a white background, while the text inside is printed in black capital letters. Frames grouping a procedure are connected by straight or diagonal blue arrows, pointing to the next related field or checkbox generating a different procedure, represented by a horizontal arrangement of rectangles below.
