Abstract
Det beskrives en fremgangsmåte for formulering av visse epothilone-analoger for parenteral administrering, hvor analogen løses i en blanding av minst 50 vol% tert-butanol i vann, blandingen lyofiliseres, det resulterende lyofiliserte produkt pakkes i et glass med en tilstrekkelig mengde løsningsmiddel som omfatter vannfri etanol og et egnet ikke-ionisk overflateaktivt middel i et andre glass. Alle trinn foretas under beskyttelse mot lys. Ved bruk tilsettes innholdet av det andre glass til det lyofiliserte produkt og blandes for å rekonstituere epothilone-analogen, hvorpå den resulterende løsning fortynnes med et passende fortynningsmiddel for å frembringe en løsning for intravenøs injeksjon som inneholder epothilone-analogen i en konsentrasjon på fra ca. 0,1 mg/mL til ca. 0,9 mg/mL. Et foretrukket overflateaktivt middel er polyetoksylert ricinusolje og et foretrukket fortynningsmiddel er laktatholdig Ringers injeksjon.

Oppfinnelsesområde
Foreliggende oppfinnelse vedrører fremgangsmåte for formulering av en epothilone-analog for parenteral administrering, farmasøytisk blanding omfattende slike samt denne forbindelsen for behandling av kreft.
Bakgrunn for oppfinnelsen
Epothiloner er makrolidforbindelser som kan anvendes på det farmasøytiske området. For eksempel kan Epothilone A og B som har strukturene:
vise seg å utøve lignende mikrotubulus-stabiliserende virkninger som paklitaxel (TAXOL®) og dermed cytotoksisk aktivitet mot hurtig prolifererende celler, som f.eks. tumorceller eller andre hyperproliferative cellesykdommer, se Hofle et al., Angew. Chem. Int. Ed. Engl., Bd. 35, Nr. 13/14, 1567-1569 (1996); WO 93/10121 publisert 27. mai, 1993; og WO 97/19086, publisert 29. mai, 1997.
Derivater og analoger av Epothilone A og B er blitt syntetisert og kan benyttes til å behandle en rekke kreftformer og andre unormale proliferative sykdommer. Slike analoger er omtalt i Hofle et al., Id. ; Nicolaou et al., Angew. Chem. Int. Ed. Engl... Bd. 36, Nr. 19, 2097-2103 (1997); og Su et al., Angew. Chem. Int. Ed. Engl... Bd. 36, Nr. 19, 2093-2097 (1997).
Selv om disse forbindelsene har betydelige terapeutiske egenskaper, representerer de også vanskeligheter for fagmannen innen det galeniske området som følge av visse egenskaper som vil bli detaljert beskrevet nedenfor. I henhold til foreliggende oppfinnelse er det funnet en formulering hvor den beskrevne epothilone-analog trygt kan tilberedes og administreres ved injeksjon, uten vesentlig tap av styrke.
Dessuten har mange anti-cancermedikamenter toksisitetsproblemer. Den terapeutiske profil av mange potente anti-tumormedikamenter er virkelig ugunstig som følge av toksisitet. Det er derfor også behov for fremgangsmåter for administrering og doseringsregimer som reduserer eller omgår den toksisitet som er assosiert med anti-tumormidler. Fremgangsmåtene kan muliggjøre utnyttelse av potente anti-tumormidler som ellers ikke ville bli benyttet klinisk.
Sammenfatning av oppfinnelsen
Oppfinnelsen omfatter et nytt doseringsregime for en epothilone-forbindelse, hvor regimet er anvendelig ved behandling av pasienter som har solide tumorer, særlig fremskredne solide tumorer. Forbindelsen ifølge oppfinnelsen kan videre benyttes til å behandle og/eller forhindre metastatiske, så vel som primære tumorer. I én utførelsesform omfatter oppfinnelsen behandlingen av pasienter som tidligere har fått strålebehandling og/eller kjemoterapi for solide tumorer. Det har også vist seg at epothilone-forbindelsen ifølge oppfinnelsen,
[1 S-[1 R<*>,3R<*>(E),7R<*>, 10S<*>, 11 R*,12R*,16S*]]-7,11 -dihydroksy-8,8,10,12,16-pentametyl-3-[1-metyl-2-(2-metyl-4-tiazolyl)etenyl]-4-aza-7-oksabicyklo[14.1.0]heptadekan-5,9-dion, kan benyttes til å behandle tumorer som er motstandsdyktige overfor strålebehandling eller kjemoterapi. Anvendelsen ifølge oppfinnelsen er anvendelige overfor kreftceller, og således tumorer, som naturlig er, eller blir, ufølsomme overfor paklitaxel.
I én utførelsesform omfatter doseringsregimet ifølge oppfinnelsen den ukentlige administrering av epothilone-forbindelsen ifølge oppfinnelsen fortrinnsvis som én (1) times infusjon ukentlig på en kontinuerlig basis. I en annen utførelsesform skjer administreringen ukentlig i en tre ukers syklus. Doseringsområdet for ukentlig infusjon er fra 1 mg/m2 til 30 mg/m<2>og fortrinnsvis
1 mg/m<2>til 25 mg/m<2>.1 en annen utførelsesform inkluderer doseringsregimet både peroral og intravenøs administrering av den samme epothilone-forbindelse. For eksempel kan den ukentlige infusjon etterfølges eller gå forut for, en peroral administrering av 20 mg/m<2>eller mer. I en bestemt utførelsesform inkluderer administreringsregimet en tre (3) ukers syklus av intravenøs infusjon én gang per uke i ca. én (1) time etterfulgt av, eller forut for, en peroral dose administrert én eller
flere ganger i uken før den første intravenøse administrering av en syklus, eller uken etter den siste intravenøse administrering av en syklus. Andre protokoller omfattes også av foreliggende oppfinnelse, inklusivt: (a) en daglig dosering i 5 til 10 dager etterfulgt av minst 3 dager uten dosering; (b) ukentlig dosering i 2 til 10 uker etterfulgt av minst én uke uten dosering; og (c) dosering én gang hver tredje uke etterfulgt av minst én uke uten dosering.
Det kan også benyttes Hr og H2-antihistaminer før, etter og/eller før og etter en syklus av epothilone-administrering. Det kan også anvendes andre kjemoterapeutika, særlig antitumormidler, med epothilone-syklusen alene, eller med Hr og H2-blokkerne og epothilonent.
I en annen utførelsesform benyttes epothilone-doseringsplanen i henhold til standardregimet for paklitaxel.
Som her omtalt kan et bredt utvalg av kreftformer falle inn under anvendelsen ifølge foreliggende oppfinnelse. I en foretrukket utførelsesform gjelder fremgangsmåtene behandlingen av solide tumorer omfattende bryst, hode og hals, sarkom, kolorektalt, UPT, melanom, øsofagus, nyre, livmorhals, thyroidea, analt, ovarie og kolon.
I én utførelsesform av formuleringene ifølge foreliggende oppfinnelse, solubiliseres først epothilone-analogen med en blanding av tert-butanol og vann og lyofiliseres deretter under optimaliserte betingelser. Det lyofiliserte medikament rekonstitueres først med en blanding av en overflateaktiv polyetoksylert ricinusolje og vannfri etanol, og fortynnes deretter med laktat-holdig Ringers injeksjon til en konsentrasjon passende for administrering.
Detaljert beskrivelse av oppfinnelsen
Oppfinnelsen angår en fremgangsmåte for formulering, for parenteral administrering, av en epothilone-analog kjennetegnet ved formel II
og hvilke som helst salter, solvater eller hydrater derav, som omfatter de følgende trinn foretatt under beskyttelse mot lys: a) oppløsning av nevnte epothilone-analog i en blanding av minst ca. 50 vol% tert-butanol i vann for å danne en løsning; b) primær tørking av nevnte løsning ved en temperatur fra -10°C til -40°C under høy-vakuum på fra 50 millitorr til 300 millitorr, i 24 timer til 96 timer,
for å danne et lyofilisert produkt;
c) sekundær tørking av det resulterende lyofiliserte produkt ved en temperatur fra 10°C til 30°C under høy-vakuum på fra 50 millitorr til 300 millitorr, i 24
timer til 96 timer.
Foreliggende oppfinnelse angår en fremgangsmåte for formulering, for parenteral administrering, av en epothilone-analog representert ved som er [1S-
[1 R<*>,3R<*>(E),7R<*>, 10S<*>, 11 R*, 12R*, 16S*]]-7,11 -dihydroksy-8,8,10,12,16-pentametyl-3-[1-metyl-2-(2-metyl-4-tiazolyl)etenyl]-4-aza-7-oksabicyklo[14.1.0]-heptadekan-5,9-dion representert ved formel II:
Forbindelsen representert ved formel II ovenfor, er her også omtalt som «epothilone-forbindelse », og dens fremstilling er beskrevet i US-patentsøknad serie nr. 09/170.582, innlevert 13. oktober, 1998, og US-patentsøknad nr. 09/280.191, innlevert 29. mars, 1999. Forbindelsen representert ved formel II ovenfor kan eksistere som flere optiske, geometriske isomerer og stereoisomerer. Mens de her viste forbindelser viser én optisk orientering, innbefatter foreliggende oppfinnelse alle isomerer og blandinger derav.
Oppfinnelsen angår også en farmasøytisk blanding omfattende en epothilone-analog representert med formel II samt fremgangsmåte for tilbereding av slike.
Forbindelsen representert ved formel II ovenfor er mikrotubulus-stabiliserende midler. Den er anvendelige ved behandlingen av en rekke forskjellige kreftformer og andre proliferative sykdommer inklusivt de følgende: karsinom, inklusivt av blære, bryst, kolon, nyre, lever, lunge, ovarie, pankreas, mavesekk, cervix, thyroidea og hud, inklusivt skvamøst cellekarsinom;

andre tumorer, inklusivt melanom, xeroderma pigmentosum, keratoakantom, seminom, thyroideafollikkel-cancer og teratokarsinom.
Forbindelsen representert ved formel II er anvendelig til behandling av pasienter som tidligere er behandlet for kreft, så vel som de som ikke tidligere er blitt behandlet for kreft. Fremgangsmåtene og blandingene kan faktisk benyttes som førstelinje og andrelinje behandling ved kreft. Forbindelsen representert ved formel II er dessuten anvendelige til behandling av motstandsdyktige cancere.
Forbindelsen representert ved formel II vil også inhibere angiogenese, og derved påvirke veksten av tumorer og føre til behandling av tumorer og tumor-relaterte forstyrrelser. Slike antiangiogene egenskaper ved forbindelsen representert ved formel II vil også være egnet ved behandlingen av andre tilstander som responderer på antiangiogene midler, inklusivt visse former for blindhet relatert til retinal vaskularisasjon, artritt, spesielt inflammatorisk artritt, multippel sklerose, restenose og psoriasis.
Forbindelsen representert ved formel II vil indusere eller inhibere apoptose, en fysiologisk celledødsprosess som er avgjørende for normal utvikling og homeostase. Endringer av apoptoseveier bidrar til patogenesen av en rekke humane sykdommer. Forbindelsen representert ved formel II, som modulatorer av apoptose, vil være egnet ved behandlingen av en rekke humansykdommer med aberasjoner i apoptosen, inklusivt cancer og prekankrøse lesjoner, immunrespons-relaterte sykdommer, virale infeksjoner, degenerative sykdommer i muskelskjelettsystemet og nyresykdom.
Forbindelsen representert ved formel II kan også formuleres eller administreres sammen med andre terapeutiske midler som velges på grunn av deres særlige egnethet ved administrering av terapier assosiert med ovennevnte tilstander. For eksempel kan hver av forbindelsen med formel II formuleres med midler for å forhindre kvalme, overfølsomhet og gastrisk irritasjon, så som antiemetika, og Hr og H2-antihistaminer. De terapeutiske midlene ovenfor kan når de benyttes i kombinasjon med forbindelsen med formel II, benyttes i de mengder som er angitt PDR (Physicians' Desk Reference) eller på annen måte bestemt av fagmannen.
Forbindelsen med formel II kan dessuten administreres i kombinasjon med andre anti-cancer- og cytotoksiske midler og behandlinger egnet ved behandlingen av kreft eller andre proliferative sykdommer. Spesielt anvendelige er anti-cancer- og cytotoksiske medikamentkombinasjoner hvor det andre valgte medikament virker på en forskjellig måte eller i forskjellig fase av cellesyklusen, f.eks. S-fasen, enn den foreliggende forbindelse med formel II, som utøver deres virkninger på G2-M-fasen. Eksempler på klasser av anti-cancer- og cytotoksiske midler innbefatter alkyleringsmidler, så som nitrogensennepsgasser, alkylsulfonater, så som nitrosourea-forbindelser, etyleniminer og triazener; antimetabolitter, så som folat-antagonister, purin-analoger og pyrimidin-analoger; antibiotika, så som antracykliner, bleomyciner, mitomycin, daktinomycin og plicamycin; enzymer, så som L-asparaginase; farnesyl-proteintransferase-inhibitorer; hormonmidler, så som glukokortikoider, østrogen/antiøstrogener, androgen/antiandrogener, progestiner og luteiniserende hormon-frigjørende hormon-antagonister, oktreotidacetat; mikrotubulus-forstyrrende midler, så som ecteinascidiner eller deres analoger og derivater; mikrotubulus-stabiliserende midler, som paklitaxel (Taxol®), docetaxel (Taxotere®); plante-avledede produkter, så som vinkalkaloider, epipodofyllotoksiner, taxaner; og topoisomerase-inhibitorer; prenyl-proteintransferase-inhibitorer; og forskjellige midler som f.eks. hydroksyurea, prokarbazin, mitotan, heksametylmelamin, platina-koordinasjonskomplekser, så som cisplatin og karboplatin; og andre midler benyttet som anti-cancer- og cytotoksiske midler, så som biologisk respons-modifiserende midler, vekstfaktorer; immun-modulatorer og monoklonale antistoffer. Forbindelsen representert ved formel II kan også benyttes i forbindelse med strålebehandling.
Representative eksempler på disse klasser av anti-cancer- og cytotoksiske midler innbefatter mechlorethamin-hydroklorid, cyklofosfamid, klorambucil, melfalan, ifosfamid, busulfan, carmustin, lomustin, semustin, streptozocin, tiotepa, dacarbazin, metotreksat, tioguanin, merkaptopurin, fludarabin, pentastatin, cladribin, cytarabin, fluoruracil, doksorubicin-hydroklorid, daunorubicin, idarubicin, bleomycin-sulfat, mitomycin C, aktinomycin D, safraciner, saframyciner, quioncarsiner, discodermolider, vinkristin, vinblastin, vinorelbin-tartrat, etoposid, teniposid, paklitaxel, tamoxifen, estramustin, estramustinfosfat-natrium, flutamid, buserelin, leuprolid, pteridiner, diyneser, levamisol, aflacon, interferon, interleukiner, aldesleukin, filgrastim, sargramostim, rituximab, BCG, tretinoin, irinotecan-hydroklorid, betametason, gemcitabin-hydroklorid, altretamin og topoteca og eventuelle analoger eller derivater derav.
Foretrukne medlemmer av disse klasser innbefatter paklitaxel, cisplatin, karboplatin, doksorubicin, carminomycin, daunorubicin, aminopterin, metotreksat, metopterin, mitomycin C, ecteinascidin 743, porfiromycin, 5-fluoruracil, 6-merkaptopurin, gemcitabin, cytosin-arabinosid, podofyllotoksin eller podofyllotoksin-derivater, som f.eks. etoposid, etoposid-fosfat eller teniposid, melfalan, vinblastin, vinkristin, leurosidin, vindesin og leurosin.
Eksempler på anti-cancer- og andre cytotoksiske midler innbefatter følgende: cyklin-avhengige kinase-inhibitorer omtalt i WO 99/24416; og prenyl-proteintransferase-inhibitorer omtalt i WO 97/30992 og WO 98/54966.
Forbindelsene kan også administreres sammen med, eller etter, anti-cancer-og cytotoksiske midler som er neurotoksiske, dvs. giftige for nervesystemet.
Uten ønske om binding til noen teori angående mekanisme eller morfologi, kan forbindelsen representert ved formel II også benyttes til å behandle andre tilstander enn cancer eller andre proliferative sykdommer. Slike tilstander innbefatter virusinfeksjoner, så som herpes-virus, pox-virus, Epstein-Barr-virus, Sindbis-virus og adenovirus; autoimmunsykdommer, så som systemisk lupus erythematosus, immunmediert glomerulonefritt, reumatoid artritt, psoriasis, inflammatoriske tarmsykdommer og autoimmun diabetes mellitus; neurodegenerative forstyrrelser, som f.eks. Alzheimers sykdom, AIDS-relatert demens, Parkinsons sykdom, amyotrofisk lateralsklerose, retinitis pigmentosa, spinalmuskelatrofi og cerebellar degenerering; AIDS; myelodysplastiske syndromer; aplastisk anemi; iskemisk skade assosiert med myokardinfarktasjoner; slag- og reperfusjonsskade; restenose; arytmier; aterosklerose; toksin-induserte eller alkohol-induserte leversykdommer; hematologiske sykdommer, som f.eks. kronisk anemi og aplastisk anemi; degenerative sykdommer i muskelskjelettsystemet, så som osteoporose og artritt; aspirin-følsom rhinosinusitt; cystisk fibrose; multippel sklerose; nyrelidelser og cancersmerte.
Forbindelsen representert ved formel II, er vanskelige å formulere ved at den har meget lav løselighet i vandige medier, hurtig nedbrytes i kontakt med vandige medier, er følsom for lav pH når den er oppløst, er lysfølsom, er «klasse D» cytotoksiske og har eksepsjonelt dårlige fukte-egenskaper. Ett eller to av disse karakteristika vil kunne kompenseres for ved å fremstille en farmasøytisk formulering for intravenøs administrering, men kombinasjonen av alle utgjør en formidabel utfordring for kjemikeren innen området legemiddelfremstilling. I betraktning av at materialer som skal benyttes til fremstilling av en intravenøs formulering må godkjennes for intravenøs administrering, ble formuleringen frembrakt i henhold til foreliggende oppfinnelse, uventet funnet egnet for å løse egenskapene av de angjeldende epothilone-analogene, nevnt ovenfor, som gjør dem vanskelige å formulere. På grunn av at de angjeldende epothilone-analogene er lite løselige i vandige medier og faktisk hurtig nedbrytes i kontakt med slike, ble det først bestemt at de skulle formuleres i lyofilisert form.
Det har vist seg at et passende medium for å danne en løsning av angjeldende forbindelse for lyofilisering, er en blanding av tert-butanol og vann for injeksjon. Denne blandingen må være minst ca. 50 vol%, fortrinnsvis fra ca. 50 til ca. 80 vol% tert-butanol for å forhindre nedbrytning av epothilone-analogen. Som følge av de eksepsjonelt dårlige fukte-egenskapene til angjeldende epothilone-analog, må den initiale løsning fremstilles ved å benytte en blanding av minst ca. 60 vol%, fortrinnsvis fra ca. 60 til ca. 95 vol%, tert-butanol og vann. Etter at løsningen er fremstillet kan den nødvendige mengde vann eller tert-butanol/vann-blanding tilsettes for å oppnå den endelige konsentrasjon for lyofilisering som nevnt ovenfor.
Det har uventet vist seg at stabiliteten av angjeldende epothilone-analog kan forbedres betydelig ved å foreta fremstillingen av løsningen ved en temperatur under romtemperatur, fortrinnsvis fra ca. 5°C til ca. 15°C, mer foretrukket ca. 5°C. Både prosessen ved å blande løsningen og påfølgende lyofilisering må dessuten foretas i beholdere, slik at epothilone-analogen beskyttes mot eksponering for lys. Det er også gunstig å foreta lyofiliseringen i forholdsvis små satser, slik at epothilone-analogen utsettes for et vandig medium i kortest mulig tid.
Det første tørketrinn ved lyofiliseringen av løsningen, dannet som beskrevet ovenfor, foretas ved temperaturer fra ca. -10°C til ca. -40°C, fortrinnsvis ca. -25°C, under høy-vakuum, dvs. fra ca. 50 millitorr til ca. 300 millitorr, fortrinnsvis ca.
200 millitorr over et lengre tidsrom, f.eks. fra ca. 24 timer til ca. 96 timer, fortrinnsvis ca. 48 timer. Lyofilisering i dette temperaturområdet fører til et amorft produkt, hvilket er ønskelig for et intravenøst preparat. Fagmannen vil innse at konvensjonelle
prosedyrer, så som røntgen-pulverdiffraksjon, kan benyttes for å bekrefte det lyofiliserte produkts amorfe natur.
Gjenværende løsningsmidler i produktet fjernes gjennom et sekundært tørke-trinn som foretas ved forholdsvis lave temperaturer, dvs. fra ca. 10°C til ca. 30°C, fortrinnsvis fra ca. 25°C, under høy-vakuum, dvs. fra ca. 50 millitorr til ca. 300 millitorr, fortrinnsvis ca. 150 millitorr, i et lengre tidsrom, dvs. fra ca. 24 timer til ca. 96 timer, fortrinnsvis ca. 48 timer.
Det har uventet vist seg at stabiliteten av de her beskrevne lyofiliserte epothilone-analogen ikke forbedres av hjelpestoffer som vanligvis benyttes for slike formål, så som laktose, mannitol, dekstran og lignende. Enkelte av disse hjelpe-stoffene kan faktisk ha en negativ effekt på stabiliteten av det lyofiliserte produkt. Epothilone-analogen formulert i henhold til foreliggende oppfinnelse lyofiliseres følgelig ufortynnet, dvs. uten noe hjelpestoff.
Den lyofiliserte epothilone-analogene representert ved formel II, rekonstitueres med en blanding av like volumdeler dehydratisert alkohol, USP, og et ikke-ionisk overflateaktivt middel, fortrinnsvis en overflateaktiv polyoksyetylert ricinusolje, tilgjengelig fra GAF Corporation, Mount Olive, New Jersey, under varemerket Cremophor EL. Det lyofiliserte produkt og bæremidlet for rekonstituering pakkes separat i passende lysbeskyttede glass. For å minske mengden av overflateaktivt middel i den rekonstituerte løsning benyttes bare en tilstrekkelig mengde av bæreren for å danne en løsning som har en konsentrasjon på ca. 2 mg/mL, til ca. 4 mg/mL av epothilone-analogen. Etter at det er oppnådd oppløsning av medikamentet, fortynnes den resulterende løsning videre før injeksjon med et egnet parenteralt fortynningsmiddel. Slike fortynningsmidler er velkjente for fagmannen. Disse fortynningsmidlene er generelt tilgjengelige i klinikkene. Det ligger imidlertid innenfor rammen av foreliggende oppfinnelse å pakke angjeldende epothilone-analog sammen med et tredje glass som inneholder tilstrekkelig parenteralt fortynningsmiddel til å fremstille den endelige konsentrasjon for administrering. Et foretrukket fortynningsmiddel er laktat-holdig Ringers injeksjon. Den endelige konsentrasjon for administrering inneholder fortrinnsvis fra ca. 0,1 mg/mL til ca. 0,9 mg/mL av epothilone-analogen.
Den endelige fortynning av den rekonstituerte epothilone-analog i formuleringen ifølge oppfinnelsen, kan foretas med andre tilberedninger som har lignende anvendelighet, for eksempel 5% dextrose-injeksjon, laktat-holdig Ringers og dextrose-injeksjon, sterilt vann for injeksjon og lignende. På grunn av dets snevre pH-område, pH 6,0 til 7,5, foretrekkes imidlertid laktat-holdig Ringers injeksjon. Laktat-holdig Ringers injeksjon inneholder natriumklorid USP 0,6 g, natriumlaktat 0,31 g, kaliumklorid USP 0,03 g og kalsiumklorid-2H20 USP 0,02 g, per 100 mL. Osmolariteten er 275 mOsmol/L, som er meget nær isotonitet.
Fremstillingen i henhold til foreliggende oppfinnelse, dvs. løsningen av epothilone-analogen i alkohol-surfaktantbæreren, kan oppbevares i opptil 24 timer før den ytterligere fortynnes for administrering. Det har vist seg at insidensen av allergiske reaksjoner som følge av nærværet av det overflateaktive middel i formuleringen, minskes ved å holde konsentrasjonen ved det minimum som er nødvendig for å bevirke oppløsning av epothilone-analogen. Insidensen av slike reaksjoner er dessuten omtrent den samme som den som man opplever med andre parenteralt administrerte farmasøytika som det inngår i, så som cyklosporin. Denne observerte grad av allergisk reaksjon med foreliggende formulering er betydelige lavere enn det som har vært tilfelle med visse andre onkologiske midler, så som paklitaxel.
Forbindelsen ifølge foreliggende oppfinnelse kan anvendes for behandling av kreft og andre hyperproliferative sykdommer hos pasienter, som omfatter administrering av en terapeutisk effektiv mengde av forbindelsen representert ved formel II, til pasienten. Forbindelsen med formel II kan administreres intravenøst eller peroralt, fortrinnsvis både peroralt og intravenøst. Fortrinnsvis administreres forbindelsen med formel II sammen med ett eller flere ytterligere midler for å forhindre kvalme, overfølsomhet eller gastrisk irritasjon, så som et antiemetikum eller et Hi- eller H2-antihistamin.
Den mengde av en forbindelse representert ved formel II som administreres ved hver IV-infusjon, eller peroralt, eller i begge tilfeller, kan bestemmes av fagmannen, og inkluderer for dosering til et menneske, mengder på fra ca. 0,01 mg/kg/dag til ca. 200 mg/kg/dag, som kan administreres som en enkelt dose eller i form av individuelle avdelte doser, f.eks. fra 1 til ca. 4 ganger per dag. Fortrinnsvis administreres forbindelsene i en dosering på mindre enn
ca. 100 mg/kg/dag, og mer foretrukket mindre enn ca. 25 mg/kg/dag, som en enkelt dose eller i ca. 2 til ca. 4 avdelte doser. Det er klart at dosenivået og hyppigheten av doseringen til et bestemt individ, vil kunne varieres og vil avhenge av en rekke
faktorer, inklusivt aktiviteten av den bestemte forbindelse som benyttes, den metabolske stabilitet og virkningsvarigheten av vedkommende forbindelse, individets art, alder, kroppsvekt, generelle helsetilstand, kjønn og diett, administrasjonsmåte og -tidspunkt, utskillingshastighet, medikamentkombinasjon og alvoret av den angjeldende tilstand. Foretrukne individer for behandling er dyr, mest foretrukket pattedyrarter så som mennesker, og husdyr som hunder, katter og lignende, som er utsatt for ovennevnte lidelser.
Forbindelsen med formel II administreres i alminnelighet inntil pasienten viser en respons, for eksempel en reduksjon i tumorstørrelse eller inntil den dosebegrensende toksisitet nås. Fagmannen vil lett innse når en pasient viser en respons eller når den dosebegrensende toksisitet er nådd. De vanlige dosebegrensende toksisiteter som er assosiert med forbindelsen med formel II innbefatter tretthet, artralgi/myalgi, anoreksi, overfølsomhet, neutropeni, trombocytopeni og neurotoksisitet.
Ved intravenøs administrering administreres forbindelsen med formel II fortrinnsvis ved å benytte formuleringene ifølge oppfinnelsen. Generelt administreres forbindelsene med formel II ved IV-infusjon over et tidsrom på fra ca. 10 minutter til ca. 3 timer, fortrinnsvis fra ca. 30 minutter til ca. 2 timer, mer foretrukket ca. 45 minutter til 90 minutter, og mest foretrukket ca. 1 time. Forbindelsene administreres i alminnelighet intravenøst i en dose på fra ca. 0,5 mg/m<2>til 65 mg/m<2>, fortrinnsvis ca. 1 mg/m2 til 50mg/m<2>, mer foretrukket ca. 2,5 mg/m<2>til 30 mg/m<2>, og mest foretrukket ca. 25 mg/m<2>.
Fagmannen vil uten videre kjenne til hvorledes doser fra mg/kg omregnes til mg/m<2>når både høyden og vekten av pasienten er kjent (se f.eks. http://www.fda.gov/cder/cancer/animalframe.htm).
Ved peroral administrering administreres forbindelsen med formel II fortrinnsvis i kombinasjon med en farmasøytisk akseptabel syrenøytraliserende buffer. Bufferen nøytraliserer syre i pasientens mave, slik at hastigheten av dekomponeringen av forbindelsen med formel II nedsettes tilstrekkelig til at de forblir i tilstrekkelig tid i gastrointestinaltrakten til å absorberes. Forbindelsen med formel II kan også administreres med en antisyre, som f.eks. hydroksyder av aluminium og magnesium; karbonater, som f.eks. natriumkarbonat og kalsiumkarbonat; silikater; og fosfater for å nøytralisere syren i mavesekken før, under eller etter administrering av forbindelsen med formel II.
I denne sammenheng refererer uttrykket «farmasøytisk akseptabel syrenøytraliserende buffer» seg til en kombinasjon av en farmasøytisk akseptabel ugiftig syre og et farmasøytisk akseptabelt ugiftig salt av en syre som, når det tilsettes til en løsning, gir en løsning som er mer resistent overfor pH-endringer sammenlignet med en løsning uten bufferen, når det tilsettes syre eller alkali til løsningen. Uttrykket
«farmasøytisk akseptabel syrenøytraliserende buffer» inkluderer også forbindelser f.eks. basiske forbindelser som, når de tilsettes til en sur løsning, nøytraliserer syren og øker løsningens pH.
I én utførelsesform av oppfinnelsen tilberedes forbindelsen med formel II og den farmasøytisk akseptable syrenøytraliserende buffer i en enkelt peroral doseringsform og administreres samtidig. Forbindelsen med formel II, kan administreres som en fast peroral doseringsform (f.eks. en tablett, kapsel eller pulver) eller en flytende peroral doseringsform (f.eks. en løsning, suspensjon eller mikstur). Løsningen eller suspensjonen kan tilberedes umiddelbart før administrering ved å benytte de riktige løsningsmidler eller med-løsningsmidler for å løse epothilone- og buffer-komponentene.
For eksempel kan forbindelsen med formel II og den farmasøytisk akseptable syrenøytraliserende buffer administreres peroralt samtidig som en løsning av epothilone med formel (II) løst i en væske som omfatter propylenglykol:etanol:fosfatbuffer (for eksempel ved 1M, ca. pH 8) i et forhold på henholdsvis ca. 58:12:30.
Forbindelsen med formel II og den farmasøytisk akseptable syrenøytraliserende buffer kan også tilberedes som separate distinkte farmasøytiske blandinger og administreres hver for seg. Hver av disse administreres som en fast peroral doseringsform eller en flytende peroral doseringsform. Når forbindelsen med formel II og den farmasøytisk akseptable syrenøytraliserende buffer administreres hver for seg, kan den farmasøytisk akseptable syrenøytraliserende buffer administreres peroralt før, etter, eller både før og etter at forbindelsene med formel II er administrert. Fortrinnsvis administreres den farmasøytisk akseptable syrenøytraliserende buffer både før og etter peroral administrering av forbindelsen med formel II, i en mengde som er tilstrekkelig til å nøytralisere mavesyren. Når den farmasøytisk akseptable syrenøytraliserende buffer administreres før forbindelsen med formel II, administreres den innen ca. 5 timer, fortrinnsvis innen ca. 3 timer, mer foretrukket innen ca. 1 time og mest foretrukket innen ca. 10 minutter før forbindelsen med formel II administreres. Når den farmasøytisk akseptable syrenøytraliserende buffer administreres etter forbindelsen med formel II, administreres den innen ca. 5 timer, fortrinnsvis innen ca. 3 timer, mer foretrukket innen ca. 1 time og mest foretrukket innen ca. 10 minutter etter at forbindelsen med formel II er administrert.
Forbindelsen med formel II kan også administreres som en enterisk drasjert pille eller kapsel, for å forsinke frigjøringen av epothilone inntil den farmasøytisk effektive syrenøytraliserende buffer er administrert. Enterisk drasjerte tabletter og kapsler er kapsler som er dekket med substanser som motstår oppløsning i mavesaften, men faller sammen i tarmen.
I alminnelighet administreres den farmasøytisk akseptable syrenøytraliserende buffer i en mengde tilstrekkelig til å avgi minst ca. 20 milliekvivalenter syrenøytraliserende kapasitet, fortrinnsvis minst ca. 30 milliekvivalenter syrenøytraliserende kapasitet, mer foretrukket minst ca. 40 milliekvivalenter syrenøytraliserende kapasitet og mest foretrukket minst ca. 50 milliekvivalenter syrenøytraliserende kapasitet. Den farmasøytisk akseptable syrenøytraliserende buffer administreres i alminnelighet som en vandig løsning som har en pH på mellom ca. 5 og 9, fortrinnsvis ca. 6 til 8,5, og mer foretrukket ca. 7 til 8. Enhver farmasøytisk akseptabel syrenøytraliserende buffer som gir en løsning med en pH i det ønskede området, kan benyttes for fremgangsmåtene ifølge oppfinnelsen. Fortrinnsvis er den farmasøytisk akseptable syrenøytraliserende buffer en dibasiskfosfat-monobasiskfosfat-buffer eller en dibasisk fosfatbuffer-sitronsyre-citratbuffer.
Foreksempel kan peroral administrering av forbindelsen med formel II innebære at pasienten peroralt først administreres den farmasøytisk akseptable syrenøytraliserende buffer som ca. 150 ml_ av en vandig løsning omfattende vannfritt dibasisk natriumfosfat (ca. 0,2M), natriumcitrat-dihydrat (0.07M) og vannfri sitronsyre (ca. 0.008M) ved en pH på ca. 7,4; etterfulgt av peroral administrering av forbindelsen med formel II som en flytende doseringsform i et propylenglykol:etanol-system i et forhold på ca. 80:20; etterfulgt av peroral administrering av en annen ca. 150 ml_ vandig løsning omfattende vannfritt dibasisk natriumfosfat (ca. 0,2M), natriumcitrat-dihydrat (ca. 0.07M) og vannfri sitronsyre (ca. 0.008M) ved en pH på ca. 7,4.
Som omtalt ovenfor kan forbindelsen med formel II administreres peroralt, intravenøst eller både og. Spesielt kan anvendelsen ifølge oppfinnelsen tilpasses doseringsprotokoller, som f.eks. én gang per dag i 2 til 10 dager, fortrinnsvis hver 3. til 9. dag, mer foretrukket hver 4. til 8. dag og mest foretrukket hver 5. dag. I én utførelsesform inngår en periode på 3 dager til 5 uker, fortrinnsvis 4 dager til 4 uker, mer foretrukket 5 dager til 3 uker, og mest foretrukket 1 uke til 2 uker, mellom sykluser hvor ingen behandling gis. I en annen utførelsesform kan forbindelsen med formel II administreres peroralt, intravenøst eller både og, én gang per dag i 3 dager, med en periode på fortrinnsvis 1 uke til 3 uker mellom sykluser hvor det ikke gis noen behandling. I nok en annen utførelsesform kan forbindelsen med formel II administreres peroralt, intravenøst eller både og, én gang per dag i 5 dager, med en periode på fortrinnsvis 1 uke til 3 uker mellom sykluser hvor det ikke gis noen behandling.
I en foretrukket utførelsesform er behandlingssyklusen for administrering av forbindelsen med formel II én gang per dag i 5 påfølgende dager, og perioden mellom behandlingssykluser er fra 2 til 10 dager, fortrinnsvis én uke.
Forbindelsen med formel II kan også administreres peroralt, intravenøst eller både og, én gang hver 1. til 10. uke, fortrinnsvis hver 2. til 8. uke, mer foretrukket hver 3. til 6. uke og enda mer foretrukket hver 3. uke.
I henhold til en annen anvendelse administreres forbindelsen med formel II i en 28 dagers syklus, hvor forbindelsen med formel II administreres intravenøst på dagene 1, 7 og 14, og peroralt på dag 21. Alternativt administreres forbindelsen med formel II etter en 28 dagers syklus, hvor forbindelsen med formel II administreres peroralt på dag 1 og intravenøst på dagene 7,14 og 28.
I henhold til anvendelsen over administreres forbindelsen med formel II inntil pasienten viser en respons, for eksempel en reduksjon i tumorstørrelse eller til den doseringsbegrensende toksisitet nås.
Mange anti-cancermidler er neurotoksiske, f.eks. er de kjent for å gi bivirkninger i det sentrale og perifere nervesystem. Denne oppfinnelse omfatter anvendelse av forbindelsen med formel II til pasienter som tidligere har opplevd neurotoksisitet med andre anti-cancermidler. Selv om forbindelsene over også kan gi neurotoksisitet i visse doser, kan de her angitte fremgangsmåter benyttes til å redusere eller unngå slik toksisitet.
Eksempler
Det følgende eksempel tjener til å illustrere gjennomføringen av foreliggende oppfinnelse.
Eksempel 1: IV-doseringsform
[1 S-[1 R*,3R*(E),7R*, 1OS*, 11 R*, 12R*, 16S*]]-7,11 -dihydroksy-8,8,10,12,16-pentametyl-3-[1-metyl-2-(2-metyl-4-tiazolyl)etenyl]-4-aza-17-oksabicyklo[14.1.0]-heptadekan-5,9-dion, 9,86 g, ble fuktet/delvis oppløst med 600 ml_ av en 9:1 blanding tert-butanol og vann for injeksjon USP som på forhånd var avkjølt til 5°C. Etter at medikamentpulveret var fullstendig fuktet, ble oppløsningen fullført ved tilsetning av 600 ml_ av en 1:9 blanding tert-butanol og vann for injeksjon og 766 mL av en 1:1 blanding tert-butanol og vann for injeksjon som likeledes på forhånd var avkjølt til 5°C, for derved å danne den endelige løsning av en 1:1 blanding. Opp-løsningen ble foretatt under beskyttelse mot lys.
Den ovenfor dannede løsningen ble umiddelbart lyofilisert i en Virtis INOTOP frysetørker ved -16°C under lysbeskyttelsesbetingelser over et 48 timers tidsrom. Det resulterende lyofiliserte produkt ble deretter tørket ved 15°C under høyvakuum i 48 timer. Det ble ikke observert noen påvisbar nedbrytning av medikamentet under disse prosesser. Det frysetørkede produkt ble pakket under sterile betingelser i 30 ml_ glass, som hvert inneholdt 10 mg medikament og et standard-overskudd for å ta hensyn til tap i glass/nål/sprøyte.
Det frysetørkede produkt rekonstitueres med 5,5 mL av en 1:1 volumblanding av dehydratisert alkohol USP og Cremophor EL®, som i alminnelighet tilsettes medikamentet i et separat glass, for å oppnå en endelig medikamentkonsentrasjon på 2 mg/mL. Etter at oppløsning har skjedd ved forsiktig omdreining av glasset, fortynnes den resulterende løsning for å oppnå en konsentrasjon på 0,2 mg/mL ved tilsetning av 9 mL laktat-holdig Ringers for injeksjon for hver milliliter rekonstituert medikamentprodukt.
Eksempel 2: IV-administrering av forbindelse II
Totalt 24 kreftpasienter (12 menn og 12 kvinner) fikk forbindelse II ved IV-administrering for å bestemme den maksimalt tolererbare dose (MTD), den dosebegrensende toksisitet (DLT), farmakokinetikk og farmakodynamikk, og for å vurdere antitumoraktiviteten av forbindelse II. Medianalderen (område) til pasientene var 57 (34-74). 5 pasienter hadde brystkreft, 5 pasienter hadde hode- og halscancer, 2 pasienter hadde sarkom, 2 pasienter hadde kolorektal cancer, 2 pasienter hadde UPT-cancer, 2 pasienter hadde melanom, 2 pasienter hadde spiserørskreft, 1 pasient hadde nyrekreft, 1 pasient hadde livmorhalskreft, 1 pasient hadde thyroideacancer og 1 pasient hadde analcancer. 21 pasienter hadde tidligere fått kjemoterapi (18 pasienter hadde fått neurotoksiske midler og 18 pasienter hadde fått strålebehandling). Det mediane antall tidligere kjemoterapilinjer som inkluderte adjuvans (område) var 2 (1-3).
Pasienter ble peroralt administrert forbindelse II på dag 1 (for dosenivåer på 20 mg/m2 og høyere), etterfulgt av en 30 minutters IV-infusjon av forbindelse II hver uke ved å starte på dag 7. Pasienter ble administrert forbindelse II i doser på 1, 2,5, 5, 10, 20, 25 og 30 mg/m<2>. Pasienter ble overvåket under behandlingsforløpet med henblikk på den dosebegrensende toksisitet (DLT). Resultatene av studien viste at forbindelse II kan administreres ukentlig i doser opp til 30 mg/m<2>uten at det observeres alvorlig toksisitet.
I en andre studie ble totalt 12 kreftpasienter (5 menn og 7 kvinner) peroralt administrert forbindelse II på dag 1, etterfulgt av en 30 minutters IV-infusjon av forbindelse II hver uke ved å starte på dag 7 med en dose på 25 mg/m<2>, for å vurdere neurotoksisitet av forbindelse II. Medianalderen (område) til pasientene var 51 (30-65). 4 pasienter hadde kolorektal cancer, 3 pasienter hadde brystkreft, 2 pasienter hadde melanom, 1 pasient hadde nyrekreft, 1 pasient hadde sarkom og 1 pasient hadde ovariecancer. 10 pasienter hadde tidligere fått kjemoterapi (6 pasienter hadde fått neurotoksiske midler og 18 pasienter hadde fått strålebehandling). Medianantallet tidligere kjemoterapilinjer som inkluderte adjuvans (område) var 2 (0-3). Denne studie viste at forbindelse II kan benyttes til pasienter som har hatt tidligere kjemoterapilinjer som inkluderer anvendelsen av neurotoksiske anti-cancermidler. For pasienter som har hatt tidligere kjemoterapilinjer som gjør bruk av neurotoksiske anti-cancermidler, er det imidlertid fordelaktig at den kumulative dose av forbindelse II ikke overskrider ca. 200 mg/m<2>per syklus.
Studien viste dessuten at brysttumorer og kolontumorer hos pasienter som tidligere var gitt kjemoterapi, responderte på behandling med forbindelse II. Nærmere bestemt responderte brystkreftpasienter som tidligere var behandlet med adriamycin og taxotere med cyklofosfamid, 5-fluoruracil, metotreksat-adjuvansterapi; adriamycin og taxotere med cyklofosfamid, 5-fluoruracil, metotreksat-adjuvansterapi; eller adriamycin, cyklofosfamid, 5-fluoruracil, mot metastatisk cancer, på behandling med forbindelse II. Pasienter med metastatisk koloncancer tidligere behandlet med taxol og carboplatin; 5-fluoruracil og leucovorin; eller irinotecan, responderte på behandling med forbindelse II.
Eksempel 3: Farmakokinetikk for forbindelse II administrert peroralt til kreftpasienter
Pasienter med fremskredne maligniteter ble ukentlig administrert forbindelse II som en 30-minutters infusjon (en behandling = 3 intravenøse ukentlige administreringer). Pasientene fikk doser på 1, 2,5, 5, 10, 20, 25 eller 30 mg/m<2>. Ved å starte med 20 mg/m<2>doseringsnivået, ble det gitt en enkelt peroral dose av forbindelse II på dag 6 i en bærer av 80% propylenglykol og 20% etanol (volumdeler) etterfulgt av administrering av en citrat/fosfatbuffer (22,5 g) før behandling 1 for å bestemme den absolutte biotilgjengelighet av forbindelse II. Dosen av forbindelse II administrert peroralt på dag 6 stemte overens med dosen av forbindelse II administrert IV på dag 1. En rekke plasmaprøver ble tatt på dag 6 og dag 1 av behandling 1 for å bestemme farmakokinetiske forhold ved LC/MS/MS.
Prøver ble analysert ved å tilsette en intern standard til 0,2 mL plasmaprøve, utfelle med aceton og deretter ekstrahere supernatanten med 1-klorbutan. Det organiske lag ble fraskilt og inndampet til tørrhet. Residuet ble rekonstituert og injisert i LC/MS/MS-systemet. Kromatografisk separasjon ble oppnådd, isokratisk, på en YMC ODS-AQ kolonne (4,6 x 50 mm, 3 um) med en mobilfase av aceto-nitril:0,01 M ammoniumacetat, pH 5,0 (65:35). Deteksjon skjedde ved negativ elektrospray tandem-massespektrometri. Standardkurven som strakte seg fra 2 til 500 ng/mL for alle analytter, ble tilpasset en 1/x vektet kvadratisk regresjonsmodell.
Forbindelse II for peroral administrering, 25 mg/glass, ble levert som «medikament på flaske». Bæreren (buffer) for konstitusjon av forbindelse II, 25 mg/glass, var en blanding av 80 vol% propylenglykol og 20 vol% etanol. Propylenglykol/etanol-blandingen ble fremstillet ved å blande 80 volumdeler propylenglykol og 20 volumdeler etanol i en egnet beholder og forsiktig omdreie beholderen inntil løsningen var fullstendig blandet.
Citrat/fosfat-bufferen for peroral administrering etter forbindelse II, ble levert på en separat flaske. Buffer for bruk med forbindelse II besto av vann for injeksjon
(WFI).
Forbindelse II ble fremstillet for administrering til pasienter ved å benytte en egnet sprøyte for langsomt å injisere 2,5, 5 eller 10 mL av propylenglykol/etanol-blandingen inn i glasset på 20 mL som inneholdt 25 mg/glass av forbindelse II, for å gi konsentrasjoner på henholdsvis 10,5 eller 2,5 mg/mL avhengig av dosen som skulle administreres til pasienten. Sprøyten ble fjernet og glasset ristet kraftig i 10 sekunder. Glasset ble anbragt i et ultralydbad og ultralydbehandlet inntil løsningen ble klar. Glass ble slått sammen avhengig av dosen.
Buffer for administrering med forbindelse II ble levert i en 8 oz. klar glass-flaske, og ble tilsatt vann for injeksjon (WFI). Barnesikringskapselen ble fjernet fra bufferflasken, og ca. 140 mL vann for injeksjon (WFI) ble tilsatt. Flasken ble ristet kraftig eller ultralydbehandlet med risting en gang iblant inntil det var oppnådd en klar løsning.
Etter peroral administrering på dag 6, ble 7 mL blodprøver oppsamlet i Becton Dickinson Vacutainer-rør med K3EDTA som antikoagulant (lavendelblå topp) i henhold til følgende program (uttrykt som timerminutter fra starten av den perorale administrering): predose, 00:15, 00:30, 00:45, 1:00, 1:30, 2:00, 3:00, 4:00, 6:00, 8:00, 24:00, 48:00 og 72:00. Etter IV-administrering på dag 1, ble 7 mL blodprøver oppsamlet i Becton Dickinson Vacutainer-rør med K3EDTA som antikoagulant (lavendelblå topp) i henhold til følgende program (uttrykt som timerminutter fra starten av IV-infusjonen): predose, 00:15, 00:30 (infusjonsslutt), 00:45, 1:00, 1:30, 2:00, 3:00, 4:00, 6:00, 8:00, 24:00, 48:00 og 72:00.
Umiddelbart etter blodprøven ble Vacutainer-rørene snudd opp-ned flere ganger for å sikre blanding med antikoagulanten og deretter umiddelbart anbragt på knust is. I løpet av 30 minutter etter oppsamling ble prøvene sentrifugert i 5 minutter ved ca. 2000 x g og 0 til 5°C. Plasmaet ble deretter overført til separate forhånds-merkede polypropylenrør med skrukork og oppbevart ved -70°C inntil bioanalyse. Plasmakonsentrasjonene av forbindelse II ble analysert ved å benytte LC/MS/MS-metoden.
Plasmakonsentrasjonen versus tid-data ble analysert ved å benytte «non-compartmental» metoder. De farmakokinetiske parametere bestemt for forbindelse II inkluderte den maksimalt observerte plasmakonsentrasjon (Cmax), tid for å oppnå Cmax (Tmax), areal under plasmakonsentrasjons-tidskurven fra tidspunktet null til tidspunktet for siste prøvetaking T (AUC(O-T)).
Totalt 18 pasienter fikk forbindelse II peroralt som en løsning på dag 6 og IV på dag 1. Sammenfatningen av de farmakokinetiske resultatene fra disse pasientene er angitt i Tabell 1.
De utførelsesformer av oppfinnelsen som er beskrevet ovenfor er ment utelukkende som eksempler, og fagmannen vil være i stand til uten å benytte annet enn rutinemessig eksperimentering, å finne frem til mange tilsvarende spesifikke forbindelser, materialer og fremgangsmåter.
Claims (12)
1. Fremgangsmåte for formulering, for parenteral administrering, av en epothilone-analog,karakterisert vedformel II
og hvilke som helst salter, solvater eller hydrater derav, som omfatter de følgende trinn foretatt under beskyttelse mot lys: a) oppløsning av nevnte epothilone-analog i en blanding av minst ca. 50 vol% tert-butanol i vann for å danne en løsning; b) primær tørking av nevnte løsning ved en temperatur fra -10°C til -40°C under høy-vakuum på fra 50 millitorr til 300 millitorr, i 24 timer til 96 timer, for å danne et lyofilisert produkt; c) sekundær tørking av det resulterende lyofiliserte produkt ved en temperatur fra 10°C til 30°C under høy-vakuum på fra 50 millitorr til 300 millitorr, i 24 timer til 96 timer.
2. Fremgangsmåte ifølge krav 1,karakterisert vedati trinn a) nevnte analog først fuktes med en blanding av minst 60% tert-butanol i vann, hvorpå tilstrekkelig vann eller en blanding av tert-butanol og vann, tilsettes til denne slik at den resulterende løsning inneholder fra 2 mg/mL til 30 mg/mL av nevnte analog i en blanding av fra 50 vol% til 80 vol% tert-butanol i vann.
3. Fremgangsmåte ifølge krav 2,karakterisert vedat nevnte analog i trinn a) først fuktes med en blanding av fra 60 vol% til 95 vol% tert-butanol i vann.
4. Fremgangsmåte ifølge krav 1,karakterisert vedat nevnte primære tørking i trinn b) foretas ved en temperatur på -25°C til -40°C og et trykk på fra 200 millitorr til 300 millitorr.
5. Fremgangsmåte ifølge krav 1,karakterisert vedat nevnte sekundære tørking i trinn c) foretas ved en temperatur på fra 25°C til 30°C og et trykk på fra 150 millitorr til 300 millitorr.
6. Fremgangsmåte ifølge krav 1,karakterisert vedat den videre omfatter følgende trinn: d) pakking av nevnte lyofiliserte produkt oppnådd i trinn c) i et første glass i kombinasjon med et andre glass som inneholder en tilstrekkelig mengde av samme volum av et egnet ikke-ionisk overflateaktivt middel og vannfri etanol for å bevirke oppløsning.
7. Fremgangsmåte ifølge krav 6,karakterisert vedat nevnte overflateaktive middel er polyetoksylert ricinusolje.
8. Fremgangsmåte ifølge krav 7,karakterisert vedat nevnte andre glass inneholder en mengde av nevnte blanding som er tilstrekkelig til å danne en løsning med fra 2 mg/mL til 4 mg/mL av nevnte analog i dette.
9. Farmasøytisk blanding,karakterisert vedat den omfatter, i separate glass, en lyofilisert epothilone-analog representert ved formel II som er oppnådd ved fremgangsmåte ifølge krav 1-8, og en løsningsmiddelmengde av denne, slik at når innholdet i nevnte glass kombineres, inneholder den resulterende løsning fra 2 mg/mL til 4 mg/mL av nevnte analog, hvor nevnte løsningsmiddel omfatter en blanding av tilnærmet like volumdeler dehydratisert etanol og et egnet ikke-ionisk overflateaktivt middel.
10. Fremgangsmåte for tilberedning av en farmasøytisk blanding for parenteral administrering,karakterisert vedat den omfatter blanding av innholdet i glassene med den farmasøytiske blanding fremstillet ifølge krav 9 for å danne en løsning av nevnte lyofiliserte epothilone-analog og fortynning av den resulterende løsning med en mengde av et egnet parenteralt fortynningsmiddel, slik at konsentrasjonen av nevnte analog i dette vil være fra 0,1 mg/mL til 0,9 mg/mL.
11. Fremgangsmåte ifølge krav 10,karakterisert vedat nevnte fortynningsmiddel er laktat-holdig Ringers injeksjon.
12. Epothilone-analog representert ved formel II som definert i krav 1 og hvilke som helst salter, solvater eller hydrater derav, for anvendelse ved behandling av kreft.
