Abstract
Den foreliggende oppfinnelse angår nye forløpermedikamenter av MPV-2426, fremgangsmåter for a fremstille nevnte forløpermedikamentformer, farmasøytiske preparater inneholdende slike forløpermedikamentformer og fremgangsmater for a anvende slike forløpermedikamentformer. En forbindelse med den generelle formel (I) eller farmasøytisk akseptable salter og hydrater derav, hvori R representerer usubstituert eller substituert lavere alkyl, usubstituert eller substituert aryl, usubstituert eller substituert sykloalkyl, usubstituert eller substituert heteroaryl, usubstituert eller substituert lavere alkylamin eller en mettet 5- eller 6-leddet heterosyklisk gruppe inneholdende olle eller to nitrogenatomer.
Description
Oppfinnelsens område
Den foreliggende oppfinnelse angår nye bioreversible forløpermedikamenter av MPV-2426 som er en alfa2-adrenerg agonist og spesielt esterderivater, fremgangsmåter for fremstilling av nevnte forløpermedikamentformer, farmasøytiske preparater inneholdende slike forløpermedikamentformer og fremgangsmåter for å anvende slike forløpermedikamentformer.
Bakgrunn for oppfinnelsen
MPV-2426 [4-(6-hydroksyindan-l-ylmetyl)-lH-imidazol-l-iumklorid] er beskrevet i US 6313 311 Bl som en alfa2-agonist anvendelig i behandlingen av hypertensjon, glaukom, migrene, diaré, ischemi, avhengighet av kjemiske forbindelser, angst, f.eks. preoperativ angst og ulike nevrologiske, muskelskjelett-, psykiatriske og kognisjonslidelser så vel som et sedativt og smertestillende middel, nasalt dekongesionsmiddel og som et supplement til anestesi. MPV-2426 tilveiebringer en romlig begrenset og effektiv antinocicepsjon med mindre bivirkninger. Intraspinal, intratekal eller epidural administrering av MPV-2426 er beskrevet i WO 00/18400 Al. Behandlingen av hypertensjon, sjokk og kardiopulmonær gjenopplivning ved administrering av MPV-2426 er beskrevet i WO 01/30347 Al.
Alfa2-agonister er kjent for å redusere intraokulært trykk (IOP). Den første rapporten på IOP-senkende virkninger av disse terapeutiske midlene ble publisert i 1966 [Makabe, R. R Dtsch. Med. Wochenschr, 91 (1966) 1686].
Forløpermedikamenter er farmakologisk inaktive derivater av medikamentmolekyler som etter kjemisk eller enzymatisk omdanning frigjør det aktive medikamentet som utøver den terapeutiske virkningen. Forløpermedikamenter er konstruert for å overvinne ulike farmasøytiske eller biofarmasøytiske problemer assosiert med foreldremedikamentet. Et forløpermedikament med god gjennomtrengning over biologiske membraner bør ha optimal lipofilisitet. I tillegg bør et forløpermedikament være stabilt nok mot kjemisk nedbrytning og vende tilbake til det aktive foreldremedikamentet via enzymatisk hydrolyse i kroppen i løpet av eller etter absorpsjon.
Sammendrag av oppfinnelsen
Den foreliggende oppfinnelse tilveiebringer nye bioreversible esterforløpermedika-menter av MPV-2426 som er kjemisk stabile i ikke-enzymmediet og som har egnet lipofilisitet (i stand til å trenge gjennom biologiske membraner) og som lett hydrolyserer til foreldremedikamentet in vivo.
Oppfinnelsen tilveiebringer også en anvendelse av forbindelser i følge oppfinnelsen for fremstilling av et medikament som kan anvendes i behandlingen av hypertensjon, glaukom, migrene, diaré, ischemi, avhengighet av kjemiske forbindelser, hypotensjon, sjokk, kardiopulmonær gjenopplivning, vannlatingssykdommer, abstinenssyndromer, kongetiv hjertesvikt, angst, f.eks. preoperativ angst eller ulike nevrologiske muskelskjelett-, psykiatriske eller kognisjonslidelser eller som et sedativt eller smertelindrende middel, nasal dekongesionsmiddel eller som et supplement til anestesi. I tillegg tilveiebringer oppfinnelsen farmasøytiske preparater omfattende som et aktivt middel en forbindelse ifølge oppfinnelsen.
Forbindelsene i følge oppfinnelsen kan anvendes for å behandle sykdommer eller tilstander hvor alfa2-agonister er indikert som anvendelige, hvor fremgangsmåten omfatter å administrere en effektiv mengde av forbindelsen ifølge oppfinnelsen til et pattedyr med behov for slik behandling.

Detaljert beskrivelse av oppfinnelsen
Den foreliggende oppfinnelse tilveiebringer nye bioreversible esterforløpermedikamenter med den generelle formel I,
eller farmasøytisk akseptable salter eller hydrater derav, hvori R representerer Ci-C6-alkyl. Forbindelsen kan i følge en utførelsesform være
4- [6-(2,2-dimetylpropanoyloksy)indan-1 -ylmetyl]-1 H-imidazol-1 -iumklorid, 4-(6-acetoksyindan-1 -ylmetyl)-1 H-imidazol-1 -iumklorid eller 4-(6-butyryloksyindan-1 -ylmetyl)-1 H-imidazol-1 -iumklorid.
Forbindelser med formel I kan tilveiebringe adekvat lipofilisitet og stabilitet mot kjemisk hydrolyse og omdannes til det aktive forløpermedikamentet via enzymatisk hydrolyse.
Oppfinnelsen til tilveiebringe videre en anvendelse av forbindelser i følge oppfinnelsen for fremstillingen av et medikament som kan anvendes i behandlingen av en rekke sykdommer som nevnt ovenfor. I en utførelsesform i følge oppfinnelsen anvendes 4-[6-(2,2-dimetylpropanoyloksy)indan-l-ylmetyl]-1 H-imidazol-1-iumklorid til fremstilling av et medikament for behandling av de ovenfor nevnte sykdommer.
Oppfinnelsen angår som nevnt over også farmasøytiske preparater. I henhold til en utførelsesform er det aktivt middel i et farmasøytisk preparat i følge oppfinnelsen 4-[6-(2,2-dimetylpropanoyloksy)indan-1 -ylmetyl]-1 H-imidazol-1 -iumklorid. Forbindelsene ifølge oppfinnelsen kan fremstilles ved hjelp av flere synteseruter som er analoge til eller følger fremgangsmåter som er kjent i litteraturen ved anvendelse av egnede utgangsmaterialer.
Generelt kan forbindelser med formel I eller farmasøytisk akseptable salter eller hydrater derav fremstilles, f.eks. analogt til eller ifølge skjema 1, hvori R er som definert ovenfor.
Hydroksyindanforbindelse II eller et farmasøytisk akseptabelt salt eller hydrat derav, løses i et løsningsmiddel, f.eks. trifluoreddiksyre (TFA) og et karboksylsyre-klorid tilsettes. Blandingen røres f.eks. ved romtemperatur i f.eks. 24 timer. Løsningsmidlet fordampes deretter og produktforbindelse I isoleres fra reaksjons-blandingen på en passende måte. Forbindelsene ifølge oppfinnelsen kan omdannes om ønskelig til deres farmasøytisk akseptable salter eller hydrater ved anvendelse av velkjente fremgangsmåter.
Synteseruten nevnt ovenfor er ment å illustrere fremstillingen av forbindelsene ifølge oppfinnelsen og fremstillingen er ikke ment å være begrenset dertil, dvs. det er også andre mulige syntesemetoder som er innenfor den generelle kunnskapen til fagmannen på området.
Eksempel 1
4-(6-acetoksyindan-l-ylmetyl)-lH-imidazol-l-iumklorid
4-(6-hydroksyindan-l-ylmetyl)-1 H-imidazol-1-iumklorid (100 mg, 0,399 mmol) ble løst opp i 1 ml trifluoreddiksyre (TFA) og eddiksyreklorid (0,510 mmol) ble tilsatt. Blandingen ble rørt ved romtemperatur i 24 timer. TFA ble fordampet og resten ble løst i vann og gjort basisk med 2 M NH3(aq.). Den vandige fasen ble ekstrahert med diklormetan (DCM). De kombinerte ekstraktene ble tørket (Na2SC>4) og fordampet. Resten ble løst i dietyleter og løsningen ble mettet med tørr HCl-gass. Presipitatet ble filtrert og tørket under vakuum. Utbytte: 74 mg (56 %) som et hvitt fast stoff. Smp.: 176-177°C. EKSPERIMENTER
Et foreløpermedikament med god gjennomtrengelighet over biologiske membraner bør ha optimal lipofilisitet (vanligvis beskrevet som oktanol-vann-fordelingskoeffisient, og Papp). I tillegg bør et forløpermedikament være stabilt nok mot kjemisk nedbrytning og vende tilbake til det aktive foreldremedikamentet via enzymatisk hydrolyse i kroppen i løpet av eller etter absorpsjon.
Eksperiment 1. Lipofilisitet
Lipofilisiteten ble vurdert ved å bestemme tilsynelatende fordelingskoeffisient (log Papp) mellom 1-oktanol og fosfatbuffer pH 5,0 eller 7,4 ved romtemperatur. Vanligvis betraktes log Papp-verdier i området 2-3 som optimalt for absorpsjon og membranpenetrering generelt.
Fremgangsmåte
Den tilsynelatende fordelingskoeffisienten (Papp) ble vurdert fra fordelingen av testforbindelsene mellom 1-oktanol og fosfatbuffer (0,16 M, pH 5,0 eller pH 7,4, u. = 0,5). Buffer- og 1-oktanolfasene ble mettet før anvendelse ved å røre kraftig i 24 timer ved romtemperatur. En kjent konsentrasjon av forbindelsen i fosfatbuffer ble ristet med et egnet volum 1-oktanol for å oppnå likevekt. Etter 1 times risting ble fasene separert ved hjelp av sentrifugering og konsentrasjonen av forbindelsen i bufferfasen ble bestemt ved hjelp av HPLC.
Resultater
Log Papp-verdiene til forbindelsene er gitt i tabell 1. Lipofilisiteten til forbindelsene var vesentlig høyere enn for foreldremedikamentet. Økt lipofilisitet kan resultere i forsterket membrangjennomtrengelighet så vel som lengre virknings varighet som en konsekvens av endrede farmakokinetiske egenskaper (lengre oppholdstid i kroppen).
Eksperiment 2. Kjemisk stabilitet
Metode
En passende mengde av testforbindelsen (startkonsentrasjoner var 0,1-0,4 mM) ble løst i en på forhånd varmet fosfatbuffer (0,16 M, u. = 0,5, pH 7,4 eller pH 5,0). Løsningen ble plassert i et termostatvarmebad ved 37°C og prøver ble tatt ved egnede intervaller og analysert ved hjelp av HPLC for å bestemme nedbrytnings-hastigheten til forbindelsen. Pseudo-førsteordens halveringstid (ti/2) ble beregnet fra de lineære kurvene til semi-logaritmiske plott av restforbindelse over tid.
Resultater
Nedbrytningen av forbindelsene i vandig løsning ved pH 5,0 og 7,4 fulgte pseudo-førsteordens kinetikk. Halveringstiden (ti/2) for degraderingen er vist i tabell 2. Stabiliteten til forbindelsene var vesentlig høyere ved pH 5,0 enn ved pH 7,4.
Eksperiment 3. Enzymatisk hydrolyse
For å utøve den farmakologiske virkningen i kroppen bør et forløpermedikament degraderes enzymatisk til foreldremedikamentet. Forbindelsenes følsomhet for serumesteraser ble vurdert.
Fremgangsmåte
En passende mengde av testforbindelsen ble løst i ett volum (f.eks. 1 ml) fosfatbuffer (0,16 M, u. = 0,5, pH 7,4) ved 37°C. Fire volum (f.eks. 4 ml) av på forhånd varmet humant serum ble tilsatt og løsningene ble blandet i et vannbad ved 37°C (startkonsentrasjoner var 0,2-0,5 raM). Ved egnede intervaller med 300 u.1 prøver tatt og dcproteinert med 600 u.1 acetonitril. Etter blanding og sentrifugering ble 600 ul av supernatantene fordampet til tørrhet under en luftstrøm ved 40°C. Resten ble igjen løst i 300 ul av mobilfasen og analysert ved hjelp av HPLC. Pseudo-førsteordenshalveringstiden (ti/2) ble beregnet fra de lineære kurvene til semi-logaritmiske plott av restforbindelse over tid. Også dannelsen av foreldremedikamentet ble bestemt.
Resultater
Hydrolysen av forløpermedikamentene i 80 % humant serum følger pseudo-første-ordenskinetikk. Halveringstiden (ti/2) for degraderingen er vist i tabell 2. Alle forløpermedikamentene til MPV-2426 frigjorde foreldremedikamentet, MPV-2426, kvantitativt via enzymatisk hydrolyse i 80 % humant serum (pH 7,4). Dannelsen av MPV-2426 ved hydrolyse av dets pivaloylester i 80 % humant serum ved 37°C er illustrert i fig. 1.
Eksperiment 4. IOP-studie i kaniner
IOP-studien ble utført for å vise at de foreliggende forløpermedikamenter kan frigjøre foreldremedikamentet og derved farmakologisk aktivitet in vivo. IOP-senkende effekt av forløpermedikamentet ble også sammenlignet med effekten av MPV-2426.
Fremgangsmåte
For å utføre IOP-testen ble en kanin plassert i en tilbakeholdende plastikkboks lokalisert i et stille rom. En enkel dråpe (25 u.1) av testløsningen ble dryppet unilateralt inn i venstre øye på den øvre korneosklerale limbus. Ved inndryppingen ble det øvre øyelokket trukket forsiktig tilbake fra øyeeplet. IOP ble målt ved anvendelse av et BioRad (Cambridge, MA) Digilab Modular One-pneumatonometer. Før hver måling ble én eller to dråper oksybuprokain (0,06 %) påført på kornea før tonometri for å eliminere ubehag. De øvre og nedre øyelokk ble deretter forsiktig trukket tilbake og applanasjonssensoren ble brakt i kontakt med hornhinnesenteret. For hver bestemmelse ble minst to avlesninger tatt for hvert behandlet (ipsilateralt) og ubehandlet (kontralateralt) øye og gjennomsnittet av disse avlesningene ble anvendt. IOP til kaninene ble målt ved 2, 1 og 0 timer før og ved 0,5, 1, 2, 3, 4 og 5 timer etter øyedråpeadministrering. IOP ved tidspunktet for øyedråpeadministrering (0 h) ble anvendt som basi sl inje verdi. Alle studiene ble satt opp ved anvendelse av en maskert og randomisert kryssoverdesign. I det minste 72 timer utvaskingstid ble tillatt for hver kanin mellom hver dose.

Resultater
Endringen i intraokulært trykk (IOP) etter topisk unilateral administrering av tre forskjellige doser (0,1 u,g, 1,0 u.g og 2,5 u.g) av pivaloylesteren av MPV-2426 er vist i tabell 3. Den IOP-reduserende virkningen varte fra 1 time gjennom varigheten til det 5 timer lange eksperimentet. Mindre signifikante endringer ble observert i de ubehandlede øynene.
Pivaloylesteren til MPV-2426 viste økende IOP-reduksjonspotensiale sammenlignet med en tilsvarende dose MPV-2426 (tabell 3). Også en forlenget virkningsvarighet ble observert med pivaloylesteren til MPV-2426.

7. Farmasøytisk preparat omfattende som et aktivt middel en forbindelse med formel I,
eller farmasøytisk akseptable salter eller hydrater derav, hvori R er som definert i krav 1, og eventuelt en farmasøytisk akseptabel eksipient.
8. Farmasøytisk preparat ifølge krav 7, hvori forbindelsen er 4-[6-2,2-dimetylpropanoyloksy)indan-l-ylmetyl]-lH-imidazol-iumklorid.
