Description
Foreliggende oppfinnelse vedrører et farmasøytisk produkt inneholdende en nukleosid analog aktiv mot herpes simplex-viruset (HSV) og et cytotoksisk middel, og anvendelse av nukleosidanalogen for behandling og forebyggelse av herpes simplex virusinfeksjoner
Sykdomsindikasjonen for herpes simplex subtype 1 (HSV-1) er herpes labiahs (forkjø-lelsessår), og sykdomsmdikasjon for herpes simplex subtype 2(HSV-2) er genital herpes
Herpes labialis er en vanlig verdensomspennende sykdom kjennetegnet ved gjentatte angrep med versikulære erupsjoner som vanligvis fremkommer på lepper og penoral hud Mange pasienter har rapportert smerte, svelling og betydelige kosmetiske ulemper assosiert med påfølgende sårskader Til tross for at det generelt er en liten sykdom kan konsekvensen av hyppige alvorlige angrep hos noen pasienter være hemmende Sykdommen er selvfølgelig selvbegrensende i lmmunkompetente individer og tilbakefal-lende episoder varer i 7-10 dager
Den første infeksjon med genital herpes kan være alvorlig (primær første episode) dersom pasienten ikke har tidligere sykehistore med labial eller genital herpesinfeksjon, mens en mindre alvorlig sykdom oppstår dersom en antistoffrespons er utviklet gjennom tidligere eksponering for HSV-ikke-pnmær første episode Det mest vanlige forløpet for primær genital herpes infeksjon er tilbakevendende sykdom Angrepsfrekvensen varie-rer meget, men det er sannsynlig at pasienten vil oppleve i gjennomsnitt 4-5 episoder pr år Symptomatologi i disse episodene er kjennetegnet ved smertefulle lesjoner som ut-vikles fra papuler og vesikler til sår og til slutt skorper Skadene kan være ledsaget av en rekke symptomer som innbefatter smerte, ømhet, kløing og svelling av påvirket område

Anti-herpes simplex virus egenskapene til nukleosidanaloger så som penciklovir/famci-klovu- eller ayclovir forsterkes ved administrenng av forbindelsen sammen med et immunsuppressivt middel Bakgrunnen for dette var at i mus infisert med herpes simplex virus, er behandling med et antiviralt middel for å oppnå fjerning av viruset spesielt ef-fektivt når musene immunsuppnmeres med cyklosponn A
Foreliggende oppfinnelse tilveiebringer følgelig et farmasøytisk produkt, kjennetegnet ved at det omfatter en nukleosid analog aktiv mot herpes simplex viruset, utvalgt fra acyklovir, valaciklovir, penciklovir og famciklovir eller et farmasøytisk akseptabelt salt av disse, og et cytotoksisk middel eller et farmasøytisk akseptabelt salt av denne, som et kombinert preparat for samtidig, separat eller sekvensiell anvendelse for behandling og/eller forebyggelse av herpes simplex virusinfeksjoner
Oppfinnelsen tilveiebnnger videre en anvendelse av en nukleosidanalog ifølge oppfinnelsen eller et farmasøytisk akseptabelt salt av denne for fremstilling av et medikament for administrenng i forbindelse med et cytotoksisk middel, eller et farmasøytisk akseptabelt salt av dette, for behandling og/eller forebyggelse av herpes simplex virusinfeksjoner
Ko-admmistrenng av penciklovir/famciklovir med et immunsuppressivt middel er spesielt nyttig for behandling av alvorlige og/eller langvange herpes simplex virusinfeksjoner
Det antivirale middelet så som penciklovir/famciklovir og det immunsuppressive middelet kan administreres som en enkelt farmasøytisk sammensetning omfattende effektive mengder av de to aktive ingredienser Alternativt kan de to aktive ingrediensene administreres sammen i form av to separate farmasøytiske sammensetninger for samtidig eller sekvensiell anvendelse Normalt vil de aktive ingrediensene administreres separat ifølge normale doserings- og admimstrenngsregimer for ingredienser gitt alene Igangsettelsen av administreringen kan være enten med det immunsuppresive middel eller det antivirale middel
Enhetsdosen av nukleosidanalogen kan for eksempel administreres 1 til 4 ganger pr dag Den nøyaktige dosen vil avhenge av admimstrasjonsmåten og hvor alvorlig tilstanden som skal behandles er, og det er å bemerke at det kan være nødvendig å utføre rutine-messige variasjoner i dosen avhengig av pasientens alder og vekt og lmmunkompromit-terte pasienter kan trenge en økt dosenng
Immunsuppressive midler administreres ifølge konvensjonelle administrasjonsmåler for det immunsuppressive middel som anvendes
Når de to aktive ingrediensene administreres som separate preparater, administreres de fortrinnsvis enteralt, så som oralt eller parenteralt (for eksempel lntramuskulært eller spesielt intravenøst)
Det farmasøytiske produkt ifølge oppfinnelsen kan formuleres på konvensjonell måte ved anvendelse av en eller flere fysiologiske akspetable bærere eller eksipienter Pro-duktene kan følgelig for eksempel formuleres for oral, bukkal, parenteral eller rektal administrenng Produkter for oral administrenng er fortrinnsvis i form av for eksempel
tabletter eller kapsler
Produkter for oral anvendelse så som tabletter og kapsler kan fremstilles ved konvensjonelle midler med farmasøytiske akseptable eksipienter så som bindemidler (for eksempel pregelatinisert maisstivelse, polyvinylpyrrohdon eller hydroksypropylmetylcellu-lose), fyllstoffer (for eksempel laktose, mikrokrystallinsk cellulose eller kalsiumhydro-genfosfat), smøremiddel (for eksempel magnesiumstearat, talk eller silika), oppløs-ningsmidler (for eksempel potetstivelse eller natnumsnvelsesglykollat), eller fuktemid-del (for eksempel natnumlaurylsulfat) Tablettene kan belegges ifølge fremgangsmåter som er velkjente innenfor fagområdet Væskepreparater for oral administrenng kan være i form av for eksempel oppløsninger, siruper eller suspensjoner, eller de kan være som et tørt produkt for gjenopprettelse med vann eller annen egnet bærer før anvendelse derav Slike flytende preparater kan fremstilles ved konvensjonelle midler med farmasøytiske akseptable additiver så som suspendenngsmidler (for eksempel sorbitol sirup, cellulosedenvater eller hydrogenerte spiselige fettstoffer), emulgenngsmidler (for eksempel lecitin eller akasie), ikke-vandige bærere (for eksempel mandelolje, oljeholdige estere, etylalkohol eller fraksjonerte vegetabilske oljer), og konserveringsmidler (for eksempel metyl eller propyl-p-hydroksybenzoater eller sorbinsyre) Preparatene kan også inneholde buffersalter, smaksstoffer, farvestoffer og søtmngsmidler etter behov
Preparater for oral administrenng kan formuleres for å tilveiebnnge kontrollert frigjø-ring av en eller begge aktive ingredienser
For parenteral administrenng kan preparatene være i en form egnet for bolus-injeksjon eller kontinuerlig infusjon Formulennger for injeksjon kan være i enhetsdosenngsform, for eksempel i sprøyter, ampuller eller i flerdose beholdere, tilsatt konservenngsmiddel
Sammensetningene kan være i form av suspensjoner, oppløsninger eller emulsjoner i olje eller vandig bærer, og kan inneholde formulenngsmidler så som suspendenngsmidler, stabilisenngsmidler og/eller dispergenngsmidler Alternativt kan de aktive ingrediensene være i pulverform for konstitusjon med en egnet bærer, for eksempel stenlt pyro-genfhtt vann, før anvendelse derav
For rektal administrenng kan sammensetningene formuleres som suppositoner eller re-tensjonsenemer, for eksempel inneholdende konvensjonelle suppositonske baser så som kakaosmør eller andre glycender
Topisk admimstrasjonsmåte er foretrukket ved behandling herpes labiahs og kremfor-mulennger, så som de beskrevet for penciklovir i WO 91/11187 (Beecham Group p 1 c ) er velegnet
De farmasøytiske sammensetningene ifølge oppfinnelsen kan fremstilles ifølge konvensjonelle teknikker som er velkjente innenfor farmasøytisk industn Slik at penciklovir/famciklovir og det immunsuppressive middel for eksempel kan blandes, om ønskelig, med egnede eksipienser Tabletter kan for eksempel fremstilles ved direkte kompresjon av en slik blanding Kapslene kan fremstilles ved fylling av blandingen sammen med egnede eksipienser på gelatinkapsler ved anvendelse av en egnet fyllema-skin Kontrollerte frigivelsesformer for oral eller rektal administrenng kan formuleres på konvensjonell måte for kontrollerte frigivelsesformer, kremer og andre formulennger for topisk administrenng formuleres på konvensjonell måte
Sammensetningene som kan anvendes ifølge oppfinnelsen kan om ønskelig være i en pakke eller dispergenngsinnretmng som kan inneholde én eller flere enhetsdoseformer inneholdende de aktive ingrediensene Pakken kan for eksempel omfatte metall eller plastfohe, så som bhsterpakker Pakken eller dispensenngsinnretmngen kan inneholde admimstrenngsinstruksjoner Når penciklovir/famciklovir og et immunsuppressivt middel skal administreres som to separate sammensetninger kan de være i form av for eksempel en dobbeltpakning
Det er å bemerke at en alternativ anti-herpes simplex virus nukleosidanalog som ganci-klovir kan anvendes istedenfor acyklovir/valaciklovir eller penciklovir/famciklovir i foreliggende oppfinnelse, ved et hensiktsmessig dosenngsmvå ifølge dets aktivitet

Referanser til en anti-herpes simplex virus nukleosidanalog, inkludert forbindelser beskrevet i ovennevnte patentreferanse og spesifikke forbindelser nevnt ovenfor og salter derav, innbefatter solvater så som hydrater
Eksempler på farmasøytiske akseptable salter er som beskrevet i ovennevnte patentreferanse i navnet Beecham Group p 1 c og referansene sitert den, og innholdet er inkorpo-rert hen som referanse
Anti-herpes simplex virus nukleosidanaloger kan identifiseres ved standardmetoder Følgende resultater fra dyrestudier illustrerer oppfinnelsen
EKSPERIMENTER I MUS INFISERT MED HSV-1 VIRUS
En kutan infeksjon ble dannet ved inokulenng i øremushngen til mus med HSV-1 (SC 16) og effektene av oral famciklovir og valaciklovir på latent virusinfeksjon ble undersøkt
BALB/c hunnmus (Bantin and Kingman, Kingston, Hull, UK) ble levert 3-4 uker gamle og inokulert 1 uke senere Virussuspensjon (10 ul) inneholdende 5xl0<4> p f u ble inokulert i huden i venstre øremushng Hudtykkelsen ble daglig målt i de enkelte mus ved hjelp av et skyvelær (ref Nash et al, 1980, J Gen Virol 48,351-357) Mus ble daglig avlivet og vev fjernet for analysering av virus Andre mus ble 4 måneder gamle og ble deretter avlivet Trlgeminale ganglia og cervical dorsal rotgangha ble fjernet og dyrket sammen De kulturene som viste virusreplikasjon ble registrert som positive

Resultatene viser at i det andre ekspenmentet har effekten av behandling med famciklovir eller valaciklovir en betydelig større effekt på virusreplikasjon og sykdom
Antiviral/immunsuppressiv kombinasjonsterapi
Famciclovir er et eksempel på et antivirusmiddel Effektiviteten av antivirusterapien ved anvendelse av famciclovir i kombmasjon med det immunsuppressive middel cyklosponn A sammenlignes ved behandling av enten cyklosponn A eller famciclovir alene Fremgangsmåte
Mus med eller uten lmmunsuppresjon forårsaket av cyklosponn A (CyA) ble inokulert med herpes simplex-virus type 1 i øremushngen Behandlingen ble deretter utført med famciclovir (FCV) fra dagene 1 til 5 eller dagene 5 til 10 (etter infeksjon)
Resultater
Uten lmmunsuppresjon døde 20% av musene Med CyA-immunsuppresjon økte de kliniske tegnene, infeksjonen vedvarte og 50% av musene døde Behandling med FCV fra dagene 1-5 eller 5-10 etter infeksjon modererte sykdommen hos både immunkompetente og immunsuppnmerte mus, men oppløsning av kliniske tegn var mer markert hos immunsuppnmerte mus
ØRETYKKELSE
Det er tydelig ut i fra resultatene ovenfor at CyA-behandhngen øker øretykkelsen Dersom det antas at FCV vil være like effektiv i disse mus som de ubehandlede kontrolldyr og at FCV og CyA virker via uavhengige mekanismer, kan en predikert AUC i nærvær av CyA beregnes (AUC hos behandlede kontrollmus x % av kontroll for FCV alene) og dette kan sammenlignes med den observerte verdien
ØREVIRUSTITER
En tilsvarende analytisk prosess til den anvendt ovenfor for øretykkelsesdataene ble utført på ørevirustiterdataene
Konklusjon
For både øretykkelse og ørevirustiter er de observerte AUC betydelig lavere enn predikert Dette antyder at FCV er overraskende mer aktivt hos CyA-behandlede (immunsuppnmerte) mus enn hos immunkompentente mus
