Abstract
ranulocytter preserveres effektivt under lagring over lange tidsrom uten frysing ved å bruke en ikke-toksisk buffer inneholdende både gelatin og plasma som lagringsmiddel.
Description
Denne oppfinnelse vedrører en fremgangsmåte for lagring av menneske-granulocytter uten frysing, som omfatter oppslemming av cellene i et ikke-giftig lagrings-medium omfattende gelatin og plasma. Etter lagring i et slikt medium viser cellene hovedsaklig det meste av sine opprinnelige fysiologiske egenskaper og er anvendelige ved behandling av sykdommer karakterisert ved en kvalitativ og kvantitativ svikt av disse celler.
Selv om blodbanker har vært utviklet i stor grad for å samle menneskeblod og skille det i sine flere forskjellige bestanddeler som røde celler, hvite celler inneholdende nøytrofiler, blodplater og serum, kan den maksimale fysiologiske eller terapeutiske anvendelse av komponentene bare foretas hvis de kan lagres inntil bruk og/eller transporteres til bruksstedet. Visse bestanddeler så som røde blodceller og serum kan lagres over lange tidsrom etter frysing under bestemte betingelser og deretter rekonstitueres for bruk. Begrenset lagring av blodplater er mulig, men de taper sin evne til å fungere etter maksimalt 5 dagers lagring ved temperaturer over frysepunktet, og frysingen av blodplater for lagring er, selv om det er mulig (Kotelba-Witkowska et al., Transfusion, Vol. 22, 121 ff. (1982), en dyr og lite anvendt teknikk. Nøytrofiler har på den annen side vært gjenstand for agglutinering eller klumping etter lagring ned til 48 timer ved temperaturer over frysepunktet, og har således måttet brukes i løpet av 2 dager fra fremstilling; lagring av nøytrofiler ved frysing har ikke vært vellykket.
Det har vært foreslått i Tullis, Blood, vol.6, 772-773
(1951) å anvende en isotonisk buffer inneholdende gelatin sammen med EDTA, askorbinsyre, natriumacetat og fenolrødt indikator som lagringsmedium for leukocytter. Crowley et al., Transfusion, Vol.14, 574-580 (1974) beskrev dekantering av leukocytt-rikt plasma fra fullstendige blod tatt i buffer inneholdende 0,4% gelatin basert på den totale blod-buffer-blanding og lagret ved 4°C med eller uten tilsetning av forskjellige tilsetningsmidler, men granulocyttenes funksjon avtok markert etter lagring i en uke. Contreras et al., Cryobiology, Vol.17, 243-251 (1980) beskrev lagring av granulocytter i forskjellige plasmaholdige lagringsmedier ved 4°C, men utilfredsstillende reduksjon i funksjonalitet forekom etter 4 dager. Contreras et al., Transfusion, Vol. 18, 46-53
(1978) beskrev lagring av granulocytter i medier som inneholdt enten plasma eller syremodifisert gelatin ved 4°C eller 22°C med utstrakt tap av celler etter 3 dager. Price et al., Transfusion, Vol. 25, 238-241 (1985) beskrev anvendelsen av syremodifisert flytende gelatin som et middel for sedimentering av røde celler i samlingen av granulocytter, men ingen lagring av granulocytter forekom før prøving. Sher et al., Immunology, Vol. 31, 337-341 (1976) beskrev anvendelsen av gelatin som hjelpemiddel for sedimentering av røde celler i samlingen av granulocytter, men gelatinet ble vasket fra granulocyttene før oppslemming av dem i et inkubasjonsmedium uten noen lengre lagring.
Det er nå funnet at granulocytter kan stabileres effektivt for lagring ved temperaturer under 25°C, fortrinnsvis under 8°C over lengre tid på minst 5 dager eller til og med 7 dager eller mer, forutsatt at de er oppslemmet i en hensikts-messig buffer, en som er ikke-giftig og fysiologisk akseptabel, som inneholder gelatin og menneskeplasma oppløst i seg. Etter lagring kan granulocyttene transfuseres uten ytterligere behandling unntatt oppvarming til en ikke høyere temperatur enn 40°C for å få enhver tilstedeværende gel over i væskeform, eller granulocyttene kan rekonstitueres for bruk ved enkelt å vaske ut gelatinet og plasmaet med buffer, som kan være den samme eller forskjellige fra bufferen som brukes for lagring, eller ved å fjerne den gelatinholdige buffer ved sentrifugering etterfulgt av gjenoppslemming av granulocyttene i en ønsket buffer eller medium.
Den anvendte buffer kan være enhver vanlig ikke-toksisk buffer som gir en pH i det ønskede området. Bufferens pH kan variere over et stort område fra ca. pH 6,1 til pH ca. 8,5; fortrinnsvis holdes pH nær nøytralpunktet, for eksempel ca.
6,5 til ca. 7,5 for å preservere funksjonen optimalt. For de beste resultater bør granulocyttene og blodplatene i plasma-og gelatin-holdig buffer lagres ved lav temperatur, for eksempel under 8°C, selv om de kan lagres ved høyere temperaturer opp til 25°C i minst 12 timer med tilfredsstillende resultater.
Gelatin fra enhver av de vanlige kommersielle kilder kan anvendes ved utøvelsen av den foreliggende oppfinnelse. Gelatinmengden kan variere over et stort område, delvis avhengig av den ønskede lengde lagring. Ned til 1,0 vekt% i forhold til totalvekten av hele buffer-plasmablandingen er effektiv for kort lagring; der det er ingen kritisk øvre grense på anvendt mengde, untatt at den må være lav nok slik at blandingen er en væske istedenfor en gel ved en ikke høyere temperatur enn 4 0oC for å lette fjerning av gelatinet etter lagring.
Konsentrasjonen eller tallet på granulocyttene i bufferen kan variere over et begrenset område fra 10<7> til 10<9> pr. ml., fortrinnsvis fra 1 x IO<8> til 5 x IO<8> pr. ml.
Det anvendte plasma kan være normalt menneskeplasma eller autologt plasma. Plasmamengden kan variere fra 25 vekt% av hele blandingen til så meget som 90 vekt%.
Et lokalt anestetikum så som lidocain, er et eventuelt additiv som hjelper til å holde celleantallet på et høyt nivå. Andre lokal anestetika som kan anvendes er procain, mepivacain, bupivacain og lignende.
Forsøk har vist at granulocytter som er blitt lagret i 5 dager eller til og med 7 dager eller mer ifølge foreliggende oppfinnelse, deretter rekonsitituert ved 2 5 gangers fortynning av gelatinet og plasmaet, i stor grad beholder alle de ønskede fysiologiske egenskaper og er hovedsaklig fri for agglutinering eller klumping. Spesielt er de i stand til å fordøye og drepe opsoniserte mikroorganismer.
De følgende spesifike eksempler er ment mer fullstendig å illustrere karakteren til den foreliggende oppfinnelse uten å virke som begrensende på dens omfang.
I hvert av de følgende eksempler ble granulocytt fremstilt fra nytappet menneskeblod som hadde blitt antikoagulert ved tilsetning av 15-18 volum% av en sur eller hypertonisk buffer, en vandig løsning inneholdende natriumsitrat og sitronsyre (0,38 M i sitrat og sitronsyre og 20 mg/ml glykose ved pH 4,8) (ACD). Granulocyttene ble separert ved dextran sedimentasjon og sentrifugering som beskrevet i Curnutte et al., J.Clin.Invest., Vol. 53, pp.1662-1672 (1974). De ble så vasket i Dulbeccos fosfatbufferet saltvann uten kalsium eller magnesium (PBS) og uten å fjerne røde celler ved hypotonisk lysing.
Granulocyttene ble så oppslemmet i den nøytrale histisin-buffer som ble anvendt som lagringsmedium? 0,1 M natriumsitrat, 0,05 M histidin, 0,1 M glukose, 0,05 M natriumpyruvat og 1 fiM lidocain justert til pH 7,4 (HCP) .
Autologt human-plasma ble tatt inn på samme måte som HCP-buffer, idet 10 ml blod ble blandet med 1,4 ml buffer og spunnet i 10 minutter i serie ved 1500 og 2000 opm henholdsvis under seprasjon av plasmafraksjonen etter hver sentrifugering.
De følgende blandinger ble deretter fremstilt idet for-holdende gelatin og plasma er uttrykt som en vektprosendel av det totale:
I hvert tilfelle var konsentrasjonen av granulocytter 2 x IO<8> pr. ml, og pH var 7,4 ved romtemperatur.
Fem prøver av hver av blandingene ble lagret i et kjøle-
skap ved 4°C.
Etter lagring i 7 dager ved 4°C ble hver prøve oppvarmet til romtemperatur; de prøver som var geler ved 4°C ble flytende etter oppvarming. Prøvene ble fortynnet 10 ganger med Dulbeccos fosfatbufferet saltvann uten kalsium og magnesium. Granulocytt funksjonen ble så vurdert. Overlevelse av granulocytter ble målt ved celletelling uttrykt som en prosentdel av begynnelses-celletallet før lagring. Levedyktighet ble bestemt ved prosentdel celler som utelukket tryptofanblått ved oppslemming i 0,4 vekt% i PBS buffer uten kalsium og magnesium. Fra de foregående resultater er det klart at celle-tallene, levedyktigheten og bakteridrepeevnen alle opprett-holdes i meget større grad i blandingene som inneholder gelatin og plasma i kombinasjon, mens to eller selv alle tre funksjoner reduseres sterkt når en eller flere av disse bestanddelene sløyfes.

1. Fremgangsmåte for lagring av menneske-granulocytter uten frysing, som omfatter oppslemming av cellene i et ikke-giftig lagringsmedium omfattende gelatin og plasma, karakterisert ved at cellene lagres i lagringsmediet ved en temperatur under 25°C og ved en pH på 6,1 til 8,0, idet gelatinmengden er minst 1 vekt% av vekten til de samlede celler og lagringsmediet, og mindre enn den nødvendige mengde for å få lagringsmediet som inneholder cellene til å stivne til en gel ved 40°C og høyere.
