Description
Det er kjent å anvende fluorider for profylaktisk behandling av tennene mot karies. Slik behandling foregår vanligvis ved at tennene pensles med en vandig NaF-oppløsning en eller flere ganger i året hos tannlege. Det er vanlig å anvende oppløsninger som inneholder ca. 2% (vekt/volum) NaF, dvs. ca. 1% F .
Videre er det kjent å anvende et fluorholdig harpikspreparat ("Duraphat"), som inneholder ca. 2,5% F . Endelig er det foreslått å anvende et såkalt surgjort fosforfluorid i form av en vandig oppløsning som inneholder ca. 1,2% F med pH ca. 2,9.
Det er også kjent å anvende forskjellige fluorider i tann-pasta .
Det har vært vanlig antatt at fluor opptas av tennene for å
gå inn som en bestanddel av tannemaljen og å forsterke denne.
Det er nu funnet at man med fordel kan anvende et vandig tannbehandlingspreparat som inneholder fluoridioner og som har en pH på max. 2. Ved en slik behandling reagerer fluoridet øyeblikke-lig med tannemaljen, og det er praktisk talt ikke nødvendig for pasienten å vente med å lukke munnen efter at penslingen er foretatt, mens det ved behandling med de tidligere anvendte midler er nødvendig for pasienten å sitte med munnen åpen i lengre tid f.ek?. 4-5 minutter efter påføringen. Ved å anvende preparatene ifølge foreliggende oppfinnelse oppnås dessuten en høyere grad av fluov-opptak i emaljen og følgelig en bedre beskyttelse av tennene enn hva som ble oppnådd med de tidligere anvendte preparater.
Det antas at den forbedrede virkning av preparatet ifølge oppfinnelsen skyldes at det ved tilsetning av fluorider i en sur oppløsning (max. pH = 2) utfelles CaF^ i meget finkornet form. Denne utfelling er langsom fra nøytrale oppløsninger, muligen;.-p.gr.a. mangel på tilgjengelig kalsiumioner. CaF2 er noe °PP~ løselig i det orale miljø og antas å utgjøre et fluordepot son1, avgir små mengder fluor, som delvis gir fluor til r eminer a 1 i s e r i n r; av emalje (dannelse av fluorapatitt) og delvis påvirker metabolisme til bakteriene i tannbelegget (reduserer syredannelsen.) Videre er CaF£ i seg selv syreresistent i vesentlig grad, og CaF2 avsatt som et homogent lag på større deler av tannflaten kan gi en direkte beskyttelse mot karies. Som nevnt er det funnet at behandling av tenner med de sure fluorpreparater ifølge oppfinnelsen fører til en høyere og raskere avsetning av CaF^ på emalje, sement og dentin enn hva tilfellet er med de kjente preparater.
Preparatet kan være et fluorid, f.eks. natriumfluorid opp-løst i en mineralsyre, f.eks. saltsyre, men det enkleste er å be-nytte en vandig flussyre, idet man da har et preparat med det ønskede fluorinnhold og den ønskede pH. Et egnet konsentrasjons-område er 0,5 - 5% (vekt/volum) HF, dvs. 0,5-5 g HF pr. 100 ml oppløsning. Man vil da få en pH ca. 1,5, som er funnet å være særlig hensiktsmessig.
Når pH er lav, vi], det orale miljø normalt inneholde til-strekkelig kalsium til at kalsiumfluorid dannes, men det er mulig å pensle tennene med en kalsiumoppløsning før eller efter penslingen méd det fluorholdige preparat for å øke utfellingen ytterligere.
Ved bruk av skanderende elektronmikroskopi (SEM) er det funnet at dåt ved anvendelse av preparatet ifølge foreliggende oppfinnelse dann.es et kontinuerlig, finkornet belegg, mens det med nøytrale
fluoridoppløsninger dannes grove belegg med store korn. Det acidulerte fosforfluorid (APF) danner en mellomting mellom disse ytterligheter.
Klinisk er det funnet at behandling i et minutt med et preparat ifølge oppfinnelsen i form av en vandig flussyre med ca.
i i -
2% HF, medfører en god kariesbeskyttelse sammenlignet med en ubehandlet tann. Dette er vist in vivo hvor karies ble fremkalt på forsøkspersoner ved at et løst, ortodontisk bånd ble lagt på kontralaterale tenner som skulle ekstraheres av kjeveortopediske gruniYer-.. På den ene side ble tannen behandlet med HF, mens den kontralaterale tann var ubehandlet. Forsøket er foretatt på flere individer med samme resultat, dvs. den HF-behandlede tann var karies-rfri, mens den ubehandlede tann hadde karies. Det er kjent at behandling med 2% nøytral NaF på tilsvarende måte gir liten eller ingen beskyttelse.
Ekstraherte tenner ble behandlet i 5 minutter med forskjellige fluoro.ppløsninger, og fluorinnholdet (vekt%) ble undersøkt med en analytisk elektronisk mikrosonde.
.Resultatene var som følger
Foruten å bli anvendt til en generell profylaktisk behandling av tennene, er preparatet ifølge oppfinnelsen særlig egnet til såkalt fissurforsegling, dvs. tetning av sprekker (fissurer)
i tannemaljen. Videre er preparatet velegnet til behandling av tannhalser som ellers er meget utsatt for karies og der konven-sjonelle metoder for profylakse har liten virkning.
Preparatet ifølge oppfinnelsen medfører som ovenfor omtalt
en rekke fordeler, nemlig at virkningen er momentan og at virkning en er vesentlig bedre enn den som er oppnådd med de tidligere kjente preparater. Dessuten er f.eks. en vandig flussyreopp-løsning forholdsvis billig. Preparatet egner seg også til å redusere følsomheten på eksponerte tannhalser, såkalt "ising".
Det skyldes sannsynligvis at det utfelte lag av CaF^ beskytter tannhalsen mot mekaniske og kjemiske irritasjoner.

1. Tannbehandlingspreparat særlig for pensling av tenner, inneholdende fluoridioner i vandig oppløsning, karakterisert ved at det har en pH på max. 2.
2. Tannbehandlingspreparat ifølge krav 1, karakterisert ved at det har en pH på ca. 1,5. 0 0
3. Tannbehandlingspreparat ifølge et av kravene 1 og 2, karakterisert ved at det er basert på vandig flussyre (HF).
4. Tannbehandlingspreparat ifølge et av kravene 1-3, karakterisert ved at det hovedsaklig består av 0,5 5% (vekt/volum) vandig flussyre.
