Abstract
Inventia se refera la medicina, in special la chirurgie si poate fi utilizata pentru hernioplastia herniilor postoperatorii inferior-inghinale oblice, pararectale mari si gigante. Metoda revendicata se realizeaza in modul urmator. Se efectueaza douaincizii semiovale oblic-longitudinale cutanate si ale tesutului celuloadipos subcutanat deasupra herniei, se excizeaza un lambou cutanat cu tesutul celuloadipos subcutanat, din care se prepara un transplant autodermic. Se mobilizeaza aponevroza muschiului oblic extern si teaca anterioara a muschiului drept in jurul defectului herniar. Sacul herniar se deschide, se elibereaza de organele interne, la baza se excizeaza, peritoneul se sutureaza.Marginile defectului herniar pararectal se aduc in atingere sub tensiune prin suturi cu fire separate. La distanta de 1,3…2,0 cm de la sutura aplicata anterior se efectueaza o incizie marginata a aponevrozei muschiului oblic extern si foitei anterioare a tecii muschiului drept. Se sutureaza prin suturi cu fire separate marginea mediala a aponevrozei muschiului oblic extern si marginea laterala atecii muschiului drept si se aduc in atingere libera muschiul drept si muschii care s-au intretaiat anterior oblic intern si transvers. Transplantulautodermic se plaseaza oblic-longitudinal deasupra muschilor si se fixeaza din interior prin suturicu fire separate de aponevroza muschiului oblic extern si teaca muschiului drept. Se efectueaza suturarea marginii laterale a aponevrozei muschiului oblic extern si marginii mediale a tecii muschiului drept prin suturi cu fire separate capat-in-capat deasupra transplantului autodermic. Plaga cutanat-subcutanata se dreneaza si se sutureaza.The invention relates to medicine, especially to surgery and can be used for herniaplasty of postoperative oblique, large pararectal and giant hernias. The claimed method is performed as follows. Cutaneous oblique-longitudinal skin lesions and subcutaneous celluloadipose tissue are performed above the hernia, a cutaneous flap is excised with subcutaneous celluloadipose tissue, from which an autodermal transplant is prepared. The aponeurosis of the external oblique muscle and the anterior sheath of the right muscle around the herniated defect are mobilized. The hernia sac is opened, it is released from the internal organs, at the base it is excised, the peritoneum is sutured. The margins of the pararectal hernia defect are affected under tension by sutures with separate threads. At a distance of 1.3 ... 2.0 cm from the previously applied suture, a marginal incision of the aponeurosis of the external oblique muscle and the anterior leaf of the right muscle sheath is made. It is sutured by sutures with separate threads the medial margin of the aponeurosis of the external oblique muscle and the lateral edge of the ataxia of the right muscle and the free muscle is straight and the muscles that have previously been interlaced internally and transversely. Transplantulautodermic is placed oblique-longitudinally above the muscles and is fixed from the inside by suturic threads separated by the aponeurosis of the external oblique muscle and the sheath of the right muscle. The lateral margin of the aponeurosis of the external oblique muscle and the medial margin of the right muscle sheath are sutured by separate sutures end-to-end above the autodermal transplant. Skin-subcutaneous wound is drained and sutured.
