본 발명은 척추골 사이의 정상적인 디스크 거리를 유지하기 위하여 척추골안에 융합된 지주를 형성하는 인접척추골의 대향된 면의 예정된 곳에서 등뼈안에 횡단방향으로 삽입되는 보철장치에 관한 것이다. 이들장치는 나란하게 디스크 거리를 이격 연결하는 단단한 불활성 플러그의 형태이고 뼈조직의 내부성장을 용이하게 하는 거친 표면을 가지고 있다.
플러그는 인접한 척추들 사이에서 삽입을 용이하게 하기 위하여 공구상에 단부 방향으로 장착되고 정상적인 디스크 거리를 재생할 높이를 가지고 있고 붕괴된 손상 디스크의 잔류 조직을 연신하게 될 것이다. 양호한 플러그는 척추안에 걸리는 미늘과 뼈 식피편 물질을 지지하기 위한 슬롯과 척추들 사이에 삽입을 용이하게 하고 비방사 재료로 형성된 테이펴진 인도 단부를 가지고 있다.
1988년 5월 10일 허여된 미합중국 특허 제4,743,256호에 설명되어 있는 바와 같이, 하부허리 요통은 요추 디스크의 파열 또는 악화로부터 발생한다. 가장 낮은 허리의 요통(좌골신경통)은 척추들 사이의 손상된 디스크에 의해 척추 신경근의 압박에 의해 야기되고 하부 요통은 디스크의 파괴와 손상된 불안정한 척추 관절을 통하여 몸무게의 대부분을 받는 해로운 영향에 의해 야기된다. 좌골 신경통과 하부요통의 제거를 위한 외과적인 치료는 일반적으로 다음 사항을 포함한다.
1) 파괴된 연 디스크의 절제
이 방법은 처추 신경을 압박하는 디스크 부분을 제거하는 것인데 좌골 신경통, 디리통증을 제거하는데 대체적으로 성공적이나, 절반이상이 요통을 재발하게 된다. 시간의 경과에 따라 디스크가 파열함에 따라 높이가 점차적으로 감소하며 높이의 이러한 감소는 척추 세그먼트의 모든 성분에서 관절염의 변화의 결과로 부정확하게 결합되도록 척추의 후면을 결합하는 원인이 된다. 뼈의 침해(척추협착증)에 따른 재발성 신경근 압축이 또한 발전된다. 이러한 원인으로부터의 계속적이고 재발적인 요통은고통과 무력의 인도적인 원인을 발생하게 된다.
2) 후기 융합으로 디스크 절제
횡단법 사이에서 후기-측면결합 또는 뼈의 박층사이에서 종래의 후기 결합과 뼈의 성장의 생성은 인접 척추사이에서 운동을 방지하나 고통을 야기하는 퇴하된 디스크를 통하여 몸무게의 약 90%가 전달되어야 하는 사실을 변경하지 못한다. 더욱이, 후기 결합은 척추 협착증에 의하여 신경근을 가입하도록 인도하는 뼈의 과대 성장을 야기하게 하는 경향이 있다.
3) 전방 내부몸체 융합으로 디스크 절제
내부몸체 융합 기술에서, 연 디스크는 완전히 절제되고 환자 자신의 뼈(자생 뼈) 또는 고체 결합이 인접 척추몸체 사이에서 얻을 수 있다면 대체로 성공적인 이식용 뼈(동종의 뼈)로 대체될 수 있다. 불행하게도, 성공율은 오직 50%정도이다.
4) 후방허리 척추사이 융합으로 디스크 절제(PLIF)
이 방법은 뼈와 신경조직 사이에 정상적인 해부학적 관계를 재구성하며 많은 이점을 가지고 있다. 중량은 종래의 불안정한 퇴화 디스크의 고통을 경감하는 척추몸체 사이의 단단한 뻐의 결합 물체를 통하여 운반되는 장기간의디스크 붕괴 또는 더 퇴보적인 변화를 일반적으로 방지한다. 완전한 디스크의 절제는 동일화 퇴화 디스크의 재발적인 탈장을 방지한다.
그러나, 이러한 PLIF방법은 심각한 여러가지의 단점을 가지며 따라서 기술적으로 매우 어렵고 성공율이 낮거나 널리 사용되지 않을 것이다. 이는 환자에게 생리학적 스트레스를 야기하고 의사에게 생리학적 고민을 야기하는 작고 깊은 구멍에 다량의 혈액의 손실을 일으킨다. 더욱이, 환자 자신의 장골 융기로부터 자생뼈 식피편의 사용은 수술시간을 길게하고 제2의 수술 부위를 낳게 한다. 충분한 강도를 가진 자생골을 많은 양은 획득할 수 어렵기 때문에 동종의 이식뼈가 일반적으로 사용된다.
내부몸체 뼈 이식은 강도의 문제와 뼈와 결합의 문제점을 포함한다. 강한 피질뼈(외부층)는 낮게하면서 디스크 거리의 붕괴를 방지하기 위하여 내부몸체 위치에 지주가 필요하다. 불행하게도 의사는 수술중에 손 공구로 필요한 지주를 설치하여야 하고 피질뼈 지주는 적당한 부하를 운반 하기에 충분하지 않고 그들은 낮는 동안에 그들 자체가 고정되며 몇년에 걸쳐 매우느리게 고정된다. 더욱이, 12개월 내지 18개월에 걸쳐 더욱 신뢰 가능하게 치료되는 연 캘셀로우즈 뼈도 또한 종래의 내부몸체 융합이 필요하다.

상기에 설명된 나의 미합중국 특허 제4,743,256호는 디스크의 거리를 유지하는 영구적인 중량지지 지주를 제공하고 척추에 이식체를 통합시키기 위하여 척추의 예정된 지역에 자리잡고 뼈의 안쪽 성장을 촉진하는 표면을 가진 단단한 이식체와 인접한 척추 사이에 디스크의 거리를 유지하여 척수 디스크의 퇴화 또는 파열에 의해 야기되는 척추등의 고통을 제거하기 위한 개선된 외과적인 방법을 설명하고 있다.
본 발명은 예정된 지역에 삽입을 용이하게 하는 공구수용 단부면을 가진 단단한 이식체을 제공하여 더 개선하고 뼈의 내부 성장을 향상하는 이식체의 외주상에 거친 표면의 기하학적인 모양을 가진다. 본 발명은 또한 신규의 공구로서 이식하는 방법을 개선한다.
본 발명에 따라서, 상기에 설명된 나의 미합중국 특허 제4,743,256호에 설명된 형식의 단단한 플러그는 인접 척추의 예정된 지역의 위치에 플러그의 삽입을 용이하게 하는 공구 수용 요홈을 가진 단부면을 제공하여 더욱 개선되고, 척추들 사이에 설치를 용이하게 하는 모양을 가진 거친외주 표면을 가지고, 뼈가 내부로 성장하는 것을 촉진한다. 양호한 플러그는 그 한단부에 내부에 나사이를 가지고 있는 구멍과 단부면 내의 이 구멍으로부터 방사 방향에 날개 또는 슬롯이 구비되어 있다. 삽입 공구는 구멍안으로 나사이가 있고 슬롯 요홈안에 고정된 슬리브에 의해 둘러싸여 있다. 공구 단부에 지지되어 있는 플러그는 적당히 위치될때 인접척추의 예정된 지역에 삽입되고, 슬리브는 회전에 대하여 지지되고 공구는 플러그의 변동없이 플러그로부터 나사이가 나지 않는다.

플러그의 단부 표면내의 나사이를 가진 양호한 구멍은 플러그의 길이의 절반이내에서 종단되고 플러그의 직경의 3분의 1이하의 직경을 가지고 있다. 날개 또는 슬롯은 플러그의 단부 표면내의 구멍으로부터 대각선으로 펼쳐있으나 플러그의 외주로부터 안쪽으로 종단되어 있다.
거친 표면을 형성하기 위한 양호한 중합체 코팅은 나일론, 폴리올레핀, 비닐 또는 이식 환경에서 저하되지 않는 수지등이 양호하다.
본 발명은 사각형의 척추 보철이식 플러그 또는 사각형의 블럭결합 횡단 또는 수직채널 또는 척추를 연결하는 사이에 디스크의 잔류 체환조직을 잡아당길 높이를 가진 척추몸체의 인접면에 전달된 요홈을 제공한다. 편평한 측면을 가진 직사각형 또는 사각형의 블럭 또는 플러그는 이들 사각형의 횡단채널 또는 슬롯에 결합되도록 제공되어 있고, 각각의 블럭 또는 플러그의 상부 및 하부 표면은 척추가 플러그를 파시하기 위하여 신장된 디스크 조직과 슬롯에 단단하게 지지되도록 이격된 척추를 펼치기 위하여 횡단슬롯의 개방단부안에 쉽게 삽입되게 하는 경사진 또는 테이퍼진 인도 단부를 가지고 있다. 이들 플러그는 삽입공구의 단부에 장착되어 있는 동안에 슬롯에 척추칼럼의 측면 또는 횡단적으로 삽입되고, 뼈가 안쪽으로 성장하는 것을 용이하게 하기 위하여 거친 표면을 가지고 있고, 골반의 장골 골즐로부터 절제된 뼈의 스트립과 같은 뼈 식 피편재료로 패킹된 교차수직 및 수평슬롯 또는 그것을 통하여 수직 또는 수평 슬롯을 또한 가지고 있다. 이러한 이식 재료는 척추의 뼈안으로 이식 플러그를 통하여 성장하고 살아있는 뼈의 블럭을 제공한다.

제1도 내지 제5도에서, 도면부호(10)는 본 발명의 보철 이식체상에 지지되어 있는 인접한 척추를 가진 인간의 척추칼럼(10)의 하부부분 또는 예정된 지역에 삽입되어 있는 이식체와 이식을 하기 위한 예정된 위치를 도시한다.

제5도에 도시된 바와 같이, 이식체(1)는 대향 단부에 용이하게 파지되는 핸들(27)에 장착되고 이식체 (11)의 단부면(11a)내의 태핑된 구멍(19)과 짝결합된 나사이를 가진 단부(26)를 가진 스템(25)을 구비한 공구 조립체(24)에 의해 척추(2,3)의 후방 측면으로부터 예정된 위치(15)에 쉽게 삽입된다. 슬리브(28)는 스템(25)상에 습동 장착되고 방사상 슬롯(20)을 결합하는 전방단부에 정반대로 대향된 키이 또는 러그(25)를 가지고 있다. 너얼링된 헤드(30)는 슬리브의 대향 단부에 구비되어 있다.

본 발명의 보철 이식체의 다른 적당한 형태는 제7도에 도시되어 있는데, 장치(31)는 태핑된 구멍(19)과 요홈 또는 슬롯(20)을 가진 단부면(31a)을 가진 직사각형의 형상을 가지고 있다. 장치(31)는 뼈의 안쪽으로의 성장을 촉진하기 위한 작은 구멍의 피라미드를 형성하는 피팅된 외주(31b)를 가지고 있다.

제10도 및 제11도의 이식 플러그(34)는 태핑된 구멍(19)과 슬롯(20)을 가진 다른 플러그와 같이 동일한 단부면(34a)을 가지고 있으나 이는 상방으로 돌출된 강모(34d)를 가진 뚜껑의 외주 표면을 가진 중합체 수지커버(34c)를 가진 단단한 충실원형 로드 코어(34b)을 가지고 있다. 이들 강모는 뼈가 내부로의 성장을 촉진하는 연장된 표면을 형성한다.
다른 많은 형식의 거친 또는 불규칙한 표면을 상기에 설명된 미합중국 특허 제4,743,256호에 설명된 바와 같이, 단단한 금속 기판에 소결된 금속 볼 및 비드로 구성된 다공성 금속 코팅을 포함한 본 발명의 장치에 의해 제공될 수 있다.
보철 이식체는 부하를 운반하기 위하여 단단한 피질뼈에 적절하게 놓인 디스크 거리를 유지하는 한쌍의 지주를 형성하기 위하여 나란히 평행하게 장착된 것으로 도시되어 있다. 이들 이식체는 비교적 짧은 성장기간에 인접한 척추에 이식체를 융화시킬 뼈의 내부로 신속한 성장으로 촉진하는 표면을 가지고 있다.
제12도 내지 제14도에 있어서, 도면부호(100)는 본 발명의 보철 이식체 블럭 또는 플러그(111)에 지지되어 있는 인접한 척추를 가진 인간의 척추칼럼(100)의 하부부분이 총계적으로 도시되어 있다.
제15도는 인접한 척추가 인접한 사각형 횡단 슬롯안으로 측면 방향으로 삽입된 본 발명의 게이지 또는 시도 블럭으로서 붕괴된 중재 디스크 조직을 연신하기 위하여 펼쳐진다.
제12도에서, 척추칼럼(100)은 다섯개의 하부척추(1 내지 5)을 도시한다. 인접척추(2,3)와 인접척추(3,4)는 본 발명의 보철블럭 또는 플러그(111)에 분리되고 지지되어 있다. 척추(1,2)와 척추(4,5)는 인접척추 사이에 정상적인 디스크 거리(113)을 유지하기 위하여 건강한 또는 손상되지 않은 인간 디스크(112)에 의해 지지되고 분리된 것으로 도시되어 있다.
디스크 거리(114)를 가진 인접척추(2,3)와 인접척추(3,4)로부터 절제된 자연적인 인간 디스크(112)의 손상된 부분은 이식체 블럭 또는 플러그(111)에 의해 유지된다. 잔류 디스크 조직(112a)이 이식체를 적어도 부분적으로 둘러싸고 이들 이식체에 의해 인장하에서 지지되도록 척추사이에 디스크(112)의 건강한 체환 조직이 가능한 많이 보유되는 것이 양호하다. 그러나, 약간의 잔류 체환 디스크 조직은 이식체 플러그(111)를 위한 공간에 개방되도록 절제될 수도 있다.

양호한 편평한 측면을 가진 직사각형 채널(115)은 이식체(111)에 의해 인접할 단부(116)를 가지고 있다.

외과 수술에서, 게이지 블럭(119)은 약 25mm의 길이와 약 11mm의 폭을 가지고 높이를 13 내지 17mm로 변경할 수도 있고,이들변수는 수용자의 척추칼럼의 크기에 따라 다르다. 블럭의 경사진 전면단부(119c)는 게이지 블럭(119)을 수용하기 위한 디스크의 섬유를 연신하여 잔류 섬유를 통하여 절단 구멍에 의해 채널(115)의 입구를 덮는 디스크 조직(112a)의 어떤 부분을 통하여 통과할 것이다.
제16도에 도시된 바와 같이, 채널척추 사이의 디스크(112)의 건강한 잔류 디스크 조직(112a)은 채널(115)에 게이지 블럭(119)의 접근을 허용하는 슬럿(121)에 개방되도록 트림된다. 이들 슬롯은 채널(115)에 등록되고 전면개방 단부(121a)와 후면단부(121b)를 가질 수 있다. 삽입된 블럭을 둘러싸고 인접한 척추를 연결하는 섬유의 체환을 떠나는 손상된 디스크(112)로부터 신경핵 펄포서스를 제거하는 것이 양호하다.

앙호한 영구 이식체 블럭 또는 플러그(11)가 제18도 및 제19도에 도시되어 있다. 이러한 플러그는 선택된 게이지 블럭과 같이 동일한 편평한 측면 치수를 가지고 있으나 이들 편평한 상부, 하부 및 측면벽으로부터 돌출되어 있고, 융기된 환형 너브(122)의 모양은 직사각형 채널(115)의 측면벽과 하부를 파지하고 물리게 하기 위한 거친 표면을 구비한다. 이들 너브는 각각의 너브(122)가 편형한 수직 후면벽(122a)과 한쌍의 편평한 수직 측면벽(112b)과 경사진 전면표면(122c)을 가지도록 환상요홈(123)과 종방향 채널(123a)에 의해 분리되어 있다.
플러그(111)는 노우즈(119c)와 같은 동일한 경사진 측면벽 (111b)과 게이지 블럭(119)의 경사진 측면벽(119e)에 의해 둘러싸인 감소된 동일한 노우즈(111a)를 가지고 있다. 더욱이, 수직후면벽(111c)은 후면벽(119d)과 동일하고 게이지 블럭(119)의 후면벽(119d)과 같은 내부에 나사이를 가진 동일한 구멍(111d)을 포함한다.
또한, 이식체 플러그(111)는 플러그의 상부와 하부를 연결하는 수직 슬롯을 가지고 있다. 직사각형인 이러한 수직 슬롯(124)은 블럭의 폭의 3분의 1의 폭을 가지고 플러그의 전방 및 후방 단부에 인접하게 연장된 길이를 가지고 있다.
이러한 슬롯(124)은 슬롯(125)을 통하여 수평으로 중앙에서 교차된다. 블럭(111)은 단일 수평 또는 수직 슬롯만을 가질 수도 있다.
슬롯(124,125)은 수용자의 골반뼈로부터 양호하게 얻어진 뼈이식체(126)의 스트립으로 채워진 블럭 또는 플러그(111)내에 공동을 제공한다. 이식체 플러그(111)내에 내장된 물질은 플러그(111)를 둘러싸는 너브(122)사이에 방사상 및 종방향 채널안으로 효홈 또는 채널(124,125) 밖으로 빨리 성장하고 인접한 척추의 뼈 조직안으로 성장할 것이다.
이식체 플러그가 척추 사이의 놓인 위치에 가압될때, 너브(122)의 경사진 전면 표면은 채널(115)의 단부(116)에 플러그를 전방으로의 이동을 수용하나, 너브의 날카로운 정점은 그들이 척추뼈를 물기 때문에 플러그의 수축을 방지한다. 그러므로 플러그가 일단 적당한 위치에 위치되면 그들은 그 위치로부터 변경되지 않을 것이다.
플러그(111)의 높이는 디스크 조직에 약 9.08kg(20파운드) 내지 13.62kg(30파운드)의 인장 부하를 유지하기에 충분할 것이다. 이러한 인장 부하는 플러그에 대하여 척추를 탄탄하게 인장 할 뿐만 아니라 뼈의 내부성장을 가속시킬 것이다.
본 발명에 의한 양호한 보철 플러그 또는 블럭(111)은 외과적 방법을 용이 및 단순화 할 뿐만 아니라 플러그의 척추의 융합을 가속시킬 것이다. 너브에 으해 구비된 거친 표면은 척추 안으로의 앵커의 다수의 목적을 달성하고 뼈의 내부성장을 위한 채널을 제공한다.

Claims (13)

  1. 인접 척추 사이의 디스크 거리(15,114)와 인접한 척추(2,3;3,4)의 면에 대향된 예정된 지역(15,115)을 횡단하는 척추칼럼(10,100)용 보철에 있어서, 각각의 플러그는 척추로부터 뼈의 내부 성장을 용이하게 하기 위하여 상기 예정된 지역을 상호 로킹하는 거친 표면(11b,31b,32b,34d,122)을 가지고, 각각의 플러그는 예정된 지역에 플러그를 단부 방향으로의 삽입을 용이하게 하기 위하여 공구상에 플러그를 단부 방향으로 고정 장착하기 위하여 플러그(19,111d)의 내부로 연장된 공구수용 수단과 단부면을 가지고, 인접한 척추 사이에서 요구되는 디스크 거리를 유지하고 인접한 척추의 예정된 지역에 자리잡고 있는 횡단지주를 형성하기 위하여 크기와 형상이 정해진 나란한 불활성의 강성 플러그(11,31 내지 34,111)를 구비한 것을 특징으로 하는 척추칼럼을 위한 보철.
  2. 제1항에 있어서, 플러그의 거친 표면은 강성 금속으로부터 방사하는 미늘(32b,122)이고 뼈의 내부 성장을 수용하는 연장된 지역을 구비하고 척추에 물리도록 플러그의 공구수용 단부쪽으로 경사진 인도면(32c,122c)을 가진 것을 특징으로 하는 척추칼럼을 위한 보철.
  3. 제1항 또는 제2항에 있어서, 공구수용 수단은 공구의 단부를 가진 나사이 결합을 위하여 플러그의 단부면에 내부에 나사이를 가진 구멍(19,111d)이 있는 것을 특징으로 하는 척추칼럼을 위한 보철.
  4. 제3항에 있어서, 내부에 나사이를 가진 구멍으로부터 방사상의 단부면내에 방사상 슬롯(20)을 포함한 것을 특징으로 하는 척추칼럼을 위한 보철.
  5. 제1항에 있어서, 각각의 상기 플러그는 레이디오루슨트 재료로 형성된 것을 특징으로 하는 척추칼럼을 위한 보철.
  6. 제1항에 있어서, 각각의 상기 플러그는 평행육면체의 형태인 것을 특징으로 하는 척추칼럼을 위한 보철.
  7. 제1항에 있어서, 플러그는 피질뼈로부터 멀리 돌출됨이 없이 척추의 연약한 캔셀로우즈뼈를 펼치고 척추의 단단한 피질뼈(18,118)를 결합하기에 충분한 길이를 가진 충실로드(11,31 내지 34,111)인 것을 특징으로 하는 척추칼럼을 위한 보철.
  8. 제1항에 있어서, 플러그가 뼈식피편 재료(126)를 수용하도록 채용된 적어도 하나의 슬롯(124,125)을 가진 것을 특징으로 하는 척추칼럼을 위한 보철.
  9. 인접한 척추몸체를 연결하는 조직(112a)을 가진 붕괴된 손상척추 디스크의 대향 측면상에 이격된 대향 측면을 가지고 상기 척추몸체의 대향된 면은 그안에 절단된 횡단채널(15,115)을 가지고, 상기 디스크 조직은 채널과 정렬된 구멍(121a)을가지고, 인접한 척추(2,3;3,4)를 함께 융합하도록 채용된 보철 장치에 있어서, 손상되지 않은 본래의 디스크 거리를 유지하기 위하여 디스크 조직을 연신하도록 채널에 가압 결합하기 위한 정렬된 채널의 하부 사이의 손상된 디스크 거리보다 큰 높이를 가진 강성의 볼활성 플러그(11,31내지 34,111)와, 몸체가 고정되고 융합되도록 채널내에 기초한 척추몸체로부터 뼈조직의 내부 성자을 용이하게 하기 위하여 채널(15,115)의 표면을 결합하기 위하여 상기 플러그상의 표면 (11b,31b,32b,32d,122)을 구비한 것을 특징으로 하는 척추칼럼을 위한 보철장치.
  10. 제9항에 있어서, 각각의 상기 플러그는 레이디오루슨트 재료로 형성된 것을 특징으로 하는 척추칼럼을 위한 보철장치.
  11. 제9항에 있어서, 각각의 상기 플러그는 평행육면체의 형태인 것을 특징으로 하는 척추칼럼을 위한 보철장치.
  12. 디스크 공간을 유지하면서 그 사이에 디스크 공간(14,114)을 가진 이격 대향된 면을 가진 이웃 척추몸체(2,3;3,4)를 함께 융합하는 방법에 있어서, 공구(24,120)의 단부상에 거친표면(11b,31b,32b,34d,122)을 가진 불활성 강성 플러그(11,31 내지 34,111)를 장착하고, 척추의 이웃하는 측면으로부터 안쪽으로 이격 대향된 면에서 대향 횡단 채널을 절단하고 척추(16,116)의 대향 측면의 짧은쪽을 종단하고, 척추를 지주로 지지하기 이하여 상기 채널안으로 공구를 단부 방향으로 플러그를 삽입하고, 플러그를이동시키지 않고 플러그로부터 공구를 제거하고, 결합된 상기 거친 표면 근처의 척추로부터 뼈의 내부 성장을 용이하게 하는 것을 특징으로 하는 이웃하는 척추몸체 융합방법.
  13. 붕괴된 손상 중재 디스크 조직에 의해 연결된 이격된 이웃하는 척추몸체를 함께 융합하기 위하여 제1항 내지 제12항중의 어느 한항에 있어서의 보철 또는 보철장치 사용 방법에 있어서, 이격된 척추의 이웃 면안으로 정렬된 횡단채널(15,115)을 절단하고, 정렬된 채널과 연통하는 디스크 조직(112a)안에 구멍(121a)을 제공하고, 손상되지 않은 디스크의 디스크 공간을 유지하기 위하여 디스크 조직을 잡아당기고 이격된 이웃 척추를 연신하기 위하여 디스크 조직내의 구멍과 채널안에 불활성 강성 플러그(11,31 내지 34,111)를 가압하고 척추에 플러그를 융합하기 위하여 뼈의 내부 성장을 용이하게 하고 채널의 표면을 파지하기 위하여 상기 플러그에 불규칙한 표면(11b,31b,32b,34d,122)을 제공하는 것을 특징으로 하는 보철 또는 보철장치 사용방법.
