Abstract
하이드로젤 패치를 포함하는 추간판 탈출증 또는 이로 인한 통증의 치료용 조성물에 관한 것으로, 디스크 동물 모델에서 시술 전/후 신경학적 등급 개선, 시술 전/후 행동 능력 개선 및 감각 능력 회복의 효과를 나타내므로 최소한의 침습적인 방법으로 추간판 탈출증 또는 이로 인한 통증의 예방 또는 치료에 유용하게 사용될 수 있는 효과가 있다.
Description
하이드로젤 패치를 포함하는 추간판 탈출증 또는 이로 인한 통증의 치료용 조성물에 관한 것이다.
추간판은 척추동물에서 척추의 추골 사이에 존재하는 추간 조직으로, 척추 원반 또는 척추 디스크(intervertebral disc)라고도 한다. 그 구조상 강한 탄력성과 팽창성을 가지고 있으며, 척추의 굴신운동이나 척추를 중심으로 하는 몸의 지지에 중요한 역할을 하고 있다. 개개의 추간판은 중앙부가 가장 두껍다. 원판의 외측부는 결합 조직과 섬유성 연골로 되어 있는 섬유륜(annulus fibrosus)이 있고, 내부의 중앙은 연골세포군과 부드러운 섬유성 연골로 된 수핵(nucleus pulposus)이 있으며 여기에는 다량의 수분이 함유되어 있다.
한편, 나이가 들면서 추간판은 퇴행성 변화를 필연적으로 겪게 되고, 10대 후반이나 20대 초반부터 이런 과정들이 나타날 수 있다. 추간판이 퇴행됨에 따라 추간판의 가장자리를 둘러싸고 있는 섬유륜에 균열(fissure)이 발생하게 되고 추간판 내부의 수핵이 척추 뼈의 경계 넘어로 탈출될 수 있다. 이렇게 추간판이 돌출되어 요통 및 신경 증상을 유발하는 질환을 추간판 탈출증(herniated intervertebral disc, hernia of intervertebral disc), 척추 디스크 질환, 척추 디스크 또는 허리 디스크라고 한다.
요추부 추간판 탈출증 환자의 가장 두드러진 증상은 요통과 함께 다리가 아프고 저린 방사통이다. 탈출된 추간판이 신경근을 자극하게 되어 신경근이 분포하는 다리에 감각 이상이 초래된다. 제5 요추 신경근이 자극되면 안쪽 발등에 감각 이상을 호소하고, 제1 척추 신경근이 자극되면 발등의 외측에 감각 이상을 호소한다. 대개 감각 저하나 무감각을 호소하지만 통각 과민으로도 나타난다. 근력 또한 약해져서 제5 신경근 이환 때에는 족부 신전근이 쇠약해지고 이 때는 발뒤꿈치로 걷는 것이 어려워진다. 드문 경우이나, 돌출된 수핵이 크고 중앙에 위치한 경우 대소변 기능이나 성기능 장애 및 하지 마비가 올 수 있다.
이러한 추간판 탈출증의 치료는 보존적인 방법과 수술적인 방법으로 나눌 수 있으며, 치료 방법을 선택할 때는 증상이 지속된 기간, 통증의 강도, 재발의 횟수, 환자의 직업, 나이, 성별, 작업량 등 여러 가지 조건을 고려해야 한다. 수술적 요법은 6~12주 동안 보존적인 치료를 하여도 효과가 없는 참기 힘든 통증이 있거나, 하지 마비가 초래되어 호전되지 않거나 진행되는 경우, 대소변 장애가 초래되는 경우, 동통이 자주 재발하여 일상 생활이 어렵고 여가 선용에 지장이 있는 경우 시행한다.
하지만, 수술적인 치료 후에도 재발되는 경우가 많아 최소 침습적인 방법으로 지속적인 효과가 있는 추간판 탈출증 치료제의 개발이 요구되는 상황이다.
이에, 본 발명자들은 피브리노겐, 라미닌 및 히알루론산을 포함하는 하이드로젤 패치를 개과(canine) 디스크 모델에 도포한 경우에, 신경학적 등급 개선, 행동 능력 개선 및 감각 능력의 회복 효과를 나타내는 것을 확인하고 본 발명을 완성하였다.
본 발명의 목적은, 최소한의 침습적인 방법으로 지속적인 효과가 있는 추간판 탈출증 또는 이로 인한 통증의 치료용 약학적 조성물을 제공하는데 있다.
다른 목적은, 추간판 탈출증 또는 이로 인한 통증의 예방 또는 치료 효과가 있는 하이드로젤 또는 하이드로젤 패치를 포함하는 약학적 조성물을 제공하는데 있다.
일 양상은, 피브린 및/또는 피브리노겐; 라미닌 또는 라미닌 유래 펩티드 또는 단백질; 및/또는 히알루론산 또는 그의 염을 유효성분으로 포함하는 추간판 탈출증 또는 이로 인한 통증의 예방 또는 치료용 약학적 조성물을 제공한다.

용어 "약학적 유효량"이란 추간판 탈출증 또는 이로 인한 통증을 예방 또는 치료하는데 유효한 조성물의 양으로서, 의학적 치료에 적용 가능한 합리적인 수혜/위험 비율로 질환을 치료하기에 충분하며 부작용을 일으키지 않을 정도의 양을 의미한다. 상기 유효량의 수준은 환자의 건강상태, 질환의 종류, 중증도, 약물의 활성, 약물에 대한 민감도, 투여 방법, 투여 시간, 투여 경로 및 배출 비율, 치료 기간, 배합 또는 동시 사용되는 약물을 포함한 요소 및 기타 의학 분야에 잘 알려진 요소에 따라 결정될 수 있다.
용어 "투여하는", "도포하는", "도입하는" 및 "이식하는"은 상호교환적으로 사용되고 일 구체예에 따른 패치 또는 조성물의 원하는 부위로의 적어도 부분적 국소화를 초래하는 방법 또는 경로에 의한 개체 내로의 일 구체예에 따른 패치 또는 조성물의 배치를 의미할 수 있다.

또한, 상기 조성물은 의약외품 조성물일 수 있다. 용어 "의약외품"은 사람이나 동물의 질병을 치료, 경감, 처치 또는 예방할 목적으로 사용되는 섬유, 고무제품 또는 이와 유사한 것, 인체에 대한 작용이 약하거나 인체에 직접 작용하지 아니며, 기구 또는 기계가 아닌 것과 이와 유사한 것, 감염 예방을 위하여 살균, 살충 및 이와 유사한 용도로 사용되는 제제 중 하나에 해당하는 물품으로서, 사람이나 동물의 질병을 진단, 치료, 경감, 처치 또는 예방할 목적으로 사용하는 물품 중 기구, 기계 또는 장치가 아닌 것 및 사람이나 동물의 구조와 기능에 약리학적 영향을 줄 목적으로 사용하는 물품 중 기구, 기계 또는 장치가 아닌 것을 제외한 물품을 의미하며, 피부 외용제 및 개인 위생 용품도 포함할 수 있다. 상기 하이드로젤, 하이드로젤 패치 또는 조성물을 추간판 탈출증 또는 이로 인한 통증의 예방 또는 개선을 목적으로 의약외품 조성물에 첨가할 경우, 상기 하이드로젤, 하이드로젤 패치 또는 조성물을 그대로 첨가하거나 다른 의약외품 성분과 함께 사용할 수 있고, 통상적인 방법에 따라 적절하게 사용할 수 있다. 유효 성분의 혼합양은 사용 목적 (예방, 건강 또는 치료적 처치)에 따라 적합하게 결정될 수 있다.
또 다른 양상은, 피브리노겐; 라미닌 또는 라미닌 유래 펩타이드; 및 히알루론산 또는 그의 약학적으로 허용가능한 염을 포함하는 졸(Sol) 상태의 조성물에 대하여 트롬빈을 첨가하여 하이드로젤 패치를 제조하는 방법을 제공한다.
상기 방법은 또한, 상기 졸 상태의 조성물에 세포 성장인자를 배합하는 단계를 추가로 포함할 수 있다.
상기 방법은 또한, 트롬빈을 첨가하여 젤화시킨 후, 다시 트롬빈을 첨가하여 2차 젤화시키는 단계를 포함할 수 있다. 상기 젤화는 10 내지 40℃에서 5분 내지 3시간 동안 수행될 수 있다. 상기 하이드로젤 패치는 틀의 모양에 따라서 다양한 형태 또는 크기로 제작할 수 있다.
상기 방법은 또한, 하이드로젤 패치를 용액 내에서 4 내지 -210℃로 저온보존 또는 동결 보존하는 단계를 포함할 수 있다. 상기 용액은 DMSO(Dimethyl sulfoxide)를 포함할 수 있으나, 하이드로젤 패치의 화학적 또는 물리적 성질을 실질적으로 변화시키지 않는 것이라면 어떠한 것이든 사용될 수 있다. 상기 하이드로젤 패치는 저온 보존 또는 동결 보존되어도 그 형태 또는 활성이 실질적으로 변하지 않는다.
또 다른 양상은 상기 하이드로젤 패치를 용액(예를 들면, DMSO)에서 4 내지 -210℃에서 저온보전 또는 동결보존하는 방법을 제공한다.
상기 하이드로젤 패치, 피브린 및/또는 피브리노겐, 라미닌, 히알루론산 또는 세포 성장인자에 대해서는 상기한 바와 같다.
중복되는 내용은 본 명세서의 복잡성을 고려하여 생략하며, 본 명세서에서 달리 정의되지 않은 용어들은 본 발명이 속하는 기술분야에서 통상적으로 사용되는 의미를 갖는 것이다.
일 양상에 따른 하이드로젤 패치 또는 조성물에 의하면, 디스크 동물 모델에서 시술 전/후 신경학적 등급 개선, 시술 전/후 행동 능력 개선 등의 효과가 있으므로 추간판 탈출증 또는 이로 인한 통증의 치료에 유용하게 사용될 수 있는 효과가 있다.
또한, 일 양상에 따른 디스크 치료용 하이드로젤 패치 또는 조성물은 디스크 환자의 마비된 운동 능력 및 감각 능력의 회복을 촉진시키므로 수술만으로 치료가 어려운 추간판 탈출증의 치료 효과를 가지고 있다.

이하, 본 발명을 실시예에 의해 보다 상세하게 설명한다. 그러나 이들 실시예는 본 발명을 예시적으로 설명하기 위한 것으로 본 발명의 범위가 이들 실시예에 의해 제한되는 것은 아니다.
실시예 1. 하이드로젤 패치의 제조
실시예 1.1 하이드로젤 배합
피브리노겐(Sigma, F8630) 20 mg/ml, 히알루론산(Sigma, 53747) 5 mg/ml 및 트롬빈 200 U/ml을 각각 PBS에 졸(sol) 상태로 용해하였다. 이후에, 졸 상태의 피브리노겐, 라미닌(Thermofisher, 23017-015) 및 히알루론산을 각각 10 mg/ml, 30 μg/ml, 1.5 mg/ml의 농도로 배합하여 졸 상태의 하이드로젤을 제작하였다.
실시예 1.2 하이드로젤 패치의 제조
멸균된 파라필름에 원형으로 0.3 내지 0.8 mm의 원틀을 찍어내고, 졸 상태의 13.3 U/ml의 트롬빈(Sigma, T4648)을 분주한 후 실시예 1.1에서 배합한 졸 상태의 하이드로젤 200 ㎕를 원틀에 맞추어 분주하고 트롬빈과 혼합하였다. 이후에, 1시간 동안 37℃에서 가교제 반응 젤화의 졸-젤 상전이를 통하여 디스크 치료용 하이드로젤 패치를 제작하였다(도 1).
실험예 1. 하이드로젤 패치의 디스크 치료 효과 확인
실시예 1에서 제조한 하이드로젤 패치의 디스크 치료 효과를 확인하기 위하여, 다음과 같은 4가지 케이스의 개과(Canine) 디스크 모델을 사용하였다.

1번 케이스의 닥스훈트 디스크 모델의 신체 검사를 수행한 결과, 도 2에 나타난 바와 같이, i) Paw replacement test에서 양발 모두 자세 반응이 나타나지 않았고, ii) 허리 통증을 나타냈으며, iii) Ambulatory test에서 양발 모두 보행이 불가한 부전마비 증상을 나타냈다.2번 케이스의 프렌치 불독 디스크 모델의 경우에는 MRI를 촬영하여 디스크 발생 부위 및 중증도를 확인한 결과, 도 3에 나타난 바와 같이, 요추(lumbar) 3-5에 Type I 디스크의 심한 척수 압박이 존재하고, 요추(lumbar) 1-3에 Type I 디스크의 중간 정도의 척수 압박이 존재하는 것을 확인하였다. 또한, 2번 케이스의 프렌치 불독 디스크 모델의 신체 검사를 수행한 결과, 도 4에 나타난 바와 같이, i) Paw replacement test에서 왼발 자세 반응이 나타나지 않았고, ii) Extensor Postural Thrust를 통하여 발이 내려가면서 지면에 닿는 순간까지 뒷다리가 쭉 펴지며 지면에 동시에 닿는지 여부와 다리가 지면에 닿은 후 자연스럽게 뒤로 가려는 뒷다리의 움직임, 속도, 간격 등을 평가하였는데, 뒷다리에서도 자세 반응이 나타나지 않았으며, iii) Ambulatory test에서 왼발의 보행이 불가한 부전마비 증상을 나타냈다.다음으로, 도 5에 나타난 바와 같이, 4가지 케이스의 개과 디스크 모델에 각각 반쪽고리판절제술(Hemilaminectomy)을 수행하고 실시예 1에서 제조한 200 ㎕의 디스크 치료용 하이드로젤 패치를 압박된 척수 부위에 도포한 후, 신경학적 등급이 0이 되기까지 걸리는 기간(Recovery period, day)을 측정하였다. 그 결과는 하기 표 2에 기재된 바와 같다. 표 2에 기재된 바와 같이, 4가지 케이스의 개과 디스크 모델 모두 14일 이내에 신경학적 등급이 0이 되는 것을 통하여, 하이드로젤 패치의 도포가 디스크 치료 효과가 있음을 확인하였다.

실시예 1에서 제조한 디스크 치료용 하이드로젤 패치를 도포한 후 14일 동안 1번 케이스의 닥스훈트 디스크 모델의 신체 검사를 수행한 결과, 도 6에 나타난 바와 같이, i) Paw replacement test에서 양발 모두 자세 반응이 나타났고, ii) 허리 촉각 통증이 완화되었으며, iii) Ambulatory test에서 양발 모두 보행이 가능한 것을 확인하였다. 또한, 실시예 1에서 제조한 디스크 치료용 하이드로젤 패치를 도포한 후 14일 동안 2번 케이스의 프렌치 불독 디스크 모델의 신체 검사를 수행한 결과, 도 7에 나타난 바와 같이, i) Paw replacement test에서 왼발 자세 반응이 나타났고, ii) Extensor Postural Thrust에서 발이 내려가면서 지면에 닿는 순간까지 뒷다리가 쭉 펴지며 지면에 동시에 닿은 후 자연스럽게 뒤로 가려는 뒷다리의 움직임이 나타났으며, iii) Ambulatory test에서 왼발의 보행이 가능한 것을 확인하였다.추가로, 실시예 1에서 제조한 디스크 치료용 하이드로젤 패치를 도포한 지 14일 후에 4가지 케이스의 개과 디스크 모델에서 뒷다리 압박 센싱 능력을 다음과 같이 측정하였다. 구체적으로, 로드셀이 부착된 포셉으로 개과 디스크 모델의 뒷발에 천천히 압력을 가하며 로드셀의 컴퓨터 프로그램을 통하여 압력을 측정하여 최대 5 kg까지 압력을 가하였다. 5 kg까지 압력을 가하는 도중에 개과 디스크 모델에서 통증을 느끼는 조짐을 나타내는 경우에 압력을 중단하고, 중단 시의 압력 값을 측정하여 개과 디스크 모델의 압박 센싱 능력을 측정하였다. 압력 값은 로드셀(iLoad Mini, Loadstar Sensors)을 핸들에 부착한 포셉을 사용하여 포셉의 누르는 힘을 측정하였다([Gorney A.M. et al., 2016, J Vet Intern Med] 참조). 그 결과는 하기 표 3에 기재된 바와 같이, 하이드로젤 패치를 도포함으로써 개과 디스크 모델의 압박 센싱 능력이 현저하게 회복된 것을 확인하였다.

이를 통하여, 실시예 1에서 제조한 하이드로젤 패치는 디스크 동물 모델의 마비된 운동 능력 및 감각 능력의 회복을 통하여, 디스크 및 이로 인한 통증의 치료 효과가 있음을 알 수 있었다.
