Abstract
NCIMB 기탁번호 42835를 갖는 락토바실러스 루테리(Lactobacillus reuteri)균주는 신규한 계통발생적 클레이드(clade)로부터의 것이며, 인간 장내 독특한 면역 자극 특성 및 향상된 생태학적 성능을 갖는다. 라피노오스(raffinose)와 같은, 균주를 위한 기질(substrate)은 상기 균주와 동일하거나 별도로 투여될 수 있다.

본 발명의 분야
본 발명은 독특한 면역 자극 특성 및 인간 장내에서의 향상된 생태학적 성능을 갖는 신규한 계통발생적 클레이드(phylogenetic clade)로부터의 락토바실러스 루테리(Lactobacillus reuteri(L. reuteri))에 관한 것이다.
본 발명의 배경
장내 미생물군(gut microbiota)은 병리에 직접적으로 기여함으로써 질병에 대한 숙주의 소인에 영향을 미치는 인간 건강의 중요한 결정 인자이다. 현대 생활 방식과 산업화로 인해 장내 미생물군의 박테리아 다양성이 감소했으며, 이는 항생제 사용, 현대 임상 관행, 위생 및 식이 습관의 변화와 같은 요인의 조합 때문일 수 있다는 실질적인 증거가 있다(Blaser & Falkow 2009; Segata, 2015). 전 세계 모든 지역에서 NCDs(non-communicable disease:비전염성 질병) 발병률 증가는 풍요로운 생활 방식과 관련이 있으며, 적어도 부분적으로는 미생물 다양성의 손실로 인한 것으로 생각된다(Haahtela et al., 2015; Hanski et al., 2012; Rook, 2013). 이러한 연결에 대한 메커니즘은 알려져 있지 않으며, '위생 가설(hygiene hypothesis)'과 같은 개념에서 병원성 미생물에 가장 중점을 두었다(Jarchum et al., 2011). 인간 진화 동안 미생물이 우리 면역 체계를 조절하는 데 필수적인 역할로 진화했다는 대안적이고 아마도 더 그럴듯한 가설이 제안되었다. 미생물은 감염이 아니라 우리 인간의 미생물군집(microbiome)을 구성하는 친근하고 공생하는 미생물이다. 이들은 우리 자연 환경에서 다른 인간이나 동물 및 미생물군에 노출되어 획득된다. 연구자들은 이제 제왕절개(C-section) 출산의 증가, 모유 수유 감소, 가족 규모 축소, 및 야외 활동 시간 감소 등 다양한 생활 방식 변화가 친화적인 미생물에 대한 감소된 노출의 근본적인 원인이 되는 반면, 변경된 식단과 항생제는 미생물군집 구성에 악영향을 미치는 것에 동의한다.

락토바실러스 루테리(L. reuteri)는 인간 GI관의 진정한 토착 박테리아 중 하나로 밝혀졌다(Sinkiewicz, 2010). 그것은 자연적으로 인간, 돼지, 햄스터, 생쥐, 쥐, 개, 양, 소 및 다른 종의 새(닭 포함)를 포함한 다양한 척추동물에 서식하며 인간의 비뇨 생식로 및 모유에서도 발견되었다.(Mitsuoka, 1992).

도 2는 시골 파푸아뉴기니인의 장내 미생물군을 보여준다. 오늘날 현대 생활 방식과 산업화로 인해 장내 미생물군의 박테리아 다양성이 상당히 감소했다는 실질적인 증거가 있는데, 이는 아마도 항생제 사용, 현대 임상 관행, 위생 및 식이 습관의 변화와 같은 요인의 조합 때문일 것이다(Blaser & Falkow 2009, Segata, 2015). 도 2a는 파푸아뉴기니인과 북미인 사이에 장내 미생물군이 상당히 다르다는 것을 보여준다. 도 2b는 파푸아뉴기니의 시골 부족 개인의 분변 미생물군에서 더 높은 다양성을 보여준다. 도 3은 47종-유사(like) OTU가 북미인의 장내 미생물군에서 완전히 검출되지 않는다는 것을 보여준다. 이 작업은 서양 사회의 '미생물군집고갈(microbiome depletion)'의 전반적인 전제를 확인한다. 흥미롭게도, 16S rRNA 시퀀싱으로 모든 파푸아뉴기니인 개체에서 검출할 수 있지만 단일 US 대조군에서 검출할 수 없는, 하나의 종은 락토바실러스 루테리이다(도 3). 자폐증 및 IBD와 같은 서양의 비전염성 질병은 파푸아뉴기니인에서 거의 존재하지 않았다(비록 최근에 더 도시화된 생활 방식으로 전파되면서 발생하고 있음). 우리는 시골 파푸아뉴기니인의 분변 샘플에서 얻은 L. reuteri의 분리물을 조사했으며 서양 인간에서 분리된 L. reuteri 균주와 상당히 다른 새로운 L. reuteri 균주를 확인했다. 파푸아뉴기니 부족인의 L. reuteri 균주는 이전에 분리되거나 특성화된 적이 없다.
전체 길이의 16S rRNA 유전자 서열을 얻기 위해 BLAST를 사용하여 각 균주의 어셈블리를 DSM 20016T의 16S rRNA 유전자 서열에 정렬하고, 정렬된 서열 단편을 수동으로 추가로 조립했다. 16S rRNA 유전자간의 유사성(similarity) 값은 BLAST를 사용하여 계산되었다. 이 분석의 결과는 PB-W1이 종의 기준주(type strain)를 포함하는 L. reuteri의 다른 균주와 >99.5% 서열 동일성을 공유함을 보여주었으며, 이는 균주가 L. reuteri임을 결정적으로 입증했다.
시골 파푸아뉴기니인에서 분리된 L. reuteri 균주와 서양 인간의 L. reuteri 균주에 대한 추가 게놈 분석 및 비교는 L. reuteri NCIMB 42835의 게놈 서열이 평균 뉴클레오타이드 동일성(Average Nucleotide Identity(ANI))을 기준으로 임의의 공지된 인간 L. reuteri와 >2.5% 상이함을 나타낸다(표 1). 특히, L. reuteri NCIMB 42835는 L. reuteri 계통수 내에서 확립된 계통발생적 혈통 중 어느 하나에도 속하지 않는다(도 1). 따라서 L. reuteri NCIMB 42835는 새로 확인된 클레이드를 나타낸다.
대조적으로, 또한 시골 파푸아뉴기니인에서 분리된 L. reuteri 균주 2는 클레이드 VI에 속한다.

로어리 파이프라인(Roary pipeline)은 신규 및 게시된 L. reuteri 게놈(n = 54)의 주석이 달린 어셈블리를 기반으로 핵심 유전자를 식별하기 위해 적용되었다. BLAST 알고리즘을 사용하여 JSpeciesWS(http://jspecies.ribohost.com/jspeciesws/#analyse/)의 연결된 핵심 유전자를 기반으로 5가지 다른 균주 간의 평균 뉴클레오타이드 서열 동일성(ANI) 값을 계산했다.
결과 요약
종 수준에서 전체 16S 99.5% 유사성은 PB-W1이 L. reuteri 종으로 지정될 수 있음을 나타낸다. ANI 결과는 NCIMB 42835가 알려진 균주와 비교할 때 상당한 균주 수준 변동을 보여준다(전체 게놈의 핵심 유전자에서 >2.5% 평균 차이). 이 차이는 L. reuteri NCIMB 42835가 L. reuteri 계통수 내에서 확립된 계통발생적 혈통 중 임의의 것 내에 속하지 않음을 확인시켜 줌으로, L. reuteri NCIMB 42835는 새로 확인된 클레이드를 나타낸다.
추가 게놈 분석
NCIMB 42835의 16s, ITS 및 IGS 영역은 NCIMB 42835의 전체 게놈에서 분리되었다. 전체 게놈에서 제거된 모든 영역은 균주가 L. reuteri 종의 구성원임을 식별했다.
16s rRNA 식별
NCIMB 42835의 16s rRNA 식별은 인실리코(in silico) 방법을 사용하여 수행되었다. NCIMB 42835의 게놈은 나노포아(Nanopore) 기술 https://nanoporetech.com/과 함께 일루미나(Illumina) 시퀀싱 플랫폼 https://www.illumina.com/science/technology/next-generation-sequencing/sequencing-technology.html을 사용하여 얻어져 게놈의 22배 적용 범위(coverage)가 되었다. NCIMB 42835 균주는 유니버설 16s rRNA 프라이머 세트 CO1 5' AGTTTGATCCTGGCTCAG 3'(서열 식별 번호 1) 및 CO2 5' TACCTTGTTACGACT 3'(서열 번호 2)의 존재에 대해 조사(mined)되었다. 16s rRNA 영역의 식별을 용이하게 하기 위해 Carver et al이 설명한 대로 게놈 시각화 도구인 아르테미스(Artemis)가 사용되었다. 두 프라이머 모두 NCIMB 42835의 게놈 내에서 식별되었다. 그런 다음 서열 데이터를 NCBI 뉴클레오타이드 데이터베이스에 대해 검색하여 뉴클레오타이드 상동성에 의한 균주 종의 동일성을 결정했다. NCIMB 42835는 인간 면역 세포(human immune cells)에서 다양한 사이토카인 반응을 유도한다
우리는 인간 면역 세포(PBMC)에 의한 사이토카인 생산에 대한 L. reuteri 균주의 효과에 대한 기능적 데이터로 이러한 게놈 비교를 보완했다. 분석은 동일한 계통발생적 혈통의 균주가 유사한 반응을 보여 L. reuteri 균주의 진화 역사가 이전 연구 결과와 일치하여 숙주와의 상호 작용에 영향을 미친다는 것을 나타낸다(Spinler et al., 2014). 이 연구 결과는 추가로 NCIMB 42835와 인간 유래 프로바이오틱 L. reuteri의 다른 두 하위-집단(sub-population)으로부터의 균주 사이의 분명한 차이점을 입증한다.
파푸아뉴기니(Papua New Guinea(PNG))와 같이 미생물 부담(burden)이 매우 높은 개발도상국의 지역에서 태어난 신생아에 대한 연구는, 호주와 같이 고도로 발달된 지역의 신생아와 비교하여 신생아 면역 기능에서 광범위한 차이를 보여준다(Lisciandro et al., 2012a ; 2012b). 임신 말기에 PNG 신생아의 항원 제시 세포(antigen-presenting cells(APC))는 이미 호주 신생아에 비해 활성화(HLA-DR 및 CD86) 및 억제(면역글로불린 유사 전사체 3 및 4) 마커(marker)의 훨씬 더 높은 기준선 발현을 보여준다. PNG 환경에서 미생물 부담의 면역 자극 특성은 APC의 활성화 마커의 기준선 수준이 증가한 이유 중 하나일 수 있다. 그러나 활성화 시 PNG 제대혈로부터의 APC는 활성화 및 항원 처리가 감소하여 상대적으로 진행이 휴지(quiescent)되고 호주인 신생아에 비해 약화된 T 세포 반응을 유발한다. 이러한 설정 사이에 많은 환경적 차이가 있지만 미생물 부담이 가장 두드러진 것 중 하나이며, PNG 신생아의 더 강한 면역 관용 반응(tolergenic response)은 초기 생애에 유해한 염증 반응의 발병을 예방할 수 있다. 이와 관련하여, L. reuteri 균주가 설치류 연구에서 면역 조절 및 조절 T 세포를 유도하는 것으로 나타났음을 강조하는 것이 중요하다(Liu et al., 2013; 2017). 이러한 및 기타 유형(nature) 실험은 모두 조기 면역 프로그래밍에서 산전 산모 환경의 역할을 강력하게 지지하며, 알레르기 및 기타 만성 염증성 질환에 대한 경향이 전 세계적으로 증가하는 상황에서 이를 활용하여 면역 건강을 개선하는 방법에 대한 도전적인 질문을 제기한다.
다양한 균주를 검사하는 연구에 따르면 임상 효과는 균주-특이적이라는 것을 보여준다(Wickens et al., 2008). 이것은 파푸아뉴기니 원주민의 신규한 NCIMB 42835 L reuteri를 고갈된 서양 미생물군집에 보충하는 것과 같은 개입이 건강 관련 면역 반응을 가질 수 있음을 의미한다(Abrahamson et al., 2012; Van Nimwegen et al. 2011; Bisgard et al., 2011 ).
Lactobacillus reuteri DSM 20016은 락토바실러스 루테리의 기준주이고 Lactobacillus reuteri ATCC PTA 6475는 상업적으로 이용 가능한 균주이며 둘 다 서양 개체로부터 분리되었으며 Human II 클레이드에서 유래했다(도 1). Lactobacillus reuteri DSM 17938은 페루 인디언의 모유에서 분리된 상업적으로 이용 가능한 균주이며 이 균주는 가금류/인간 클레이드 VI에 속한다. 우리는 산업화되지 않은 생활 방식을 살고 있는 파푸아뉴기니 토착인 개체, Lactobacillus reuteri NCIMB 42835에서 신규한 균주를 분리했다. 이 연구의 목적은 시험관 내에서 건강한 지원자의 PBMC와 함께 배양할 때 이러한 균주가 다른 사이토카인 프로파일을 갖는지 결정하는 것이었다.
PBMC 분리(isolation)
3명의 건강한 기증자(30세에서 45세 사이의 남성)의 말초 정맥 전혈(whole blood)을 멸균 EDTA 튜브에 수집하고, 역전으로 몇 번 혼합하고 멸균 PBS로 1/1 희석했다. 새로운 멸균 50mL 원추형 튜브에 20mL의 피콜(Ficoll)을 각 기증자에 대해 분취(aliquote) 했다. 각 기증자에 대해 희석된 EDTA 전혈 25ml를 2단계에서 분취한 피콜 위에 적층했다. 튜브는 브레이크 오프로 4°C에서 30분 동안 800g에서 원심분리했다. 혈장 아래의 백색 버피 코트(buffy coat)(PBMC 층)를 수집하고 세포를 멸균 PBS로 2회 세척했다. PBMC를 신선한 따뜻한 RPMI 10% FBS + 1% 페니실린/스트렙토마이신에 재현탁하고 자동 세포 계수기를 사용하여 계수했다. 각 기증자에 대한 세포 수는 ml당 1*106으로 조정되었고 24개의 웰 플레이트에 분취되었다.
균주 제조
냉동 및 동결건조된 L. reuteri 균주를 간단히 원심분리하고 멸균 조건에서 따뜻한 RPMI 10ml로 재현탁하여 스톡 농도(stock concetration)를 만들었다. 최고, 중간 및 낮은 농도가 제조되었다(100:1) 최고 투여량: 1ml의 스톡 농도를 4.5ml의 완전한 RPMI에 첨가했다. (50:1) 중간 투여량: 최고 투여량을 완전한 RPMI로 1:1 희석했다. (10:1) 낮은 투여량을 만들기 위해: 최고 투여량을 1:9로 희석했다.
배양 및 제어
500㎕ 중 5x105 세포의 PBMC 현탁액을 24개 웰에 분취했다. PBMC는 5% CO2(37°C, 95% 공기, 100% 습도)의 가습 인큐베이터에서, 24시간 동안 50㎕의 균주와 함께 배양되었다. 상청액을 수집하고 원심분리하여 펠릿(pellet) 세포로 만들고 깨끗한 무세포 조건 배지를 새로운 에펜도르프(Eppendorf)로 옮기고 즉시 -80℃에 보관했다. 대조군은 PBMC + 비히클(균주 동결 보호제)로 표시된다.
루미넥스(Luminex) 다중 면역 분석
샘플을 분석 직전에 분석 희석제에 1/100로 희석했다. 각 웰에 대해, 50μL의 표준 또는 희석 샘플을 웰 + 50μL에 분배했다.
측정하고자 하는 분석물질은 3가지 사이토카인 IL-10, TNF-α, IL-6)이었다. 플레이트를 제조사의 프로토콜에 따라 배양하고 Luminex MAGPIX 분석기에서 판독했다.
Lactobacillus reuteri NCIMB 42835는 다른 모든 L. reuteri보다 PBMC에서 더 많은 TNF-α, IL-10, IL-6을 유도했다(도 4, 5, 6 및 7). L. reuteri NCIMB 42835의 모든 3회 투여 후 PBMC에서 TNF-α 유도를 분석할 때, 이 사이토카인의 투여량 의존적 유도를 나타내는 독특한 프로파일을 갖는다(도 4, 5).
주성분 분석(Principle component analysis(PCA))은 데이터의 주요 구성 요소를 분석하여 서로 가장 많이 분산되고 분리된 데이터의 기본 구조를 시각화한다. L. reuteri 균주의 1회 투여량 또는 3회 투여량의 모든 사이토카인 데이터에 대한 PCA는 NCIMB 42835가 플롯의 오른쪽에서 가장 먼 점이므로 사이토카인의 가장 높은 유도를 나타내는 것으로 나타났다. 그것은 두 개의 서양 균주에서 가장 멀리 떨어져 있으며 서양 균주와 NCIMB 42835의 중간인 것처럼 보이는 다른 두 개의 비-서양 균주와도 분명히 분리된다(도 8, 9). 사이토카인 데이터는 NCIMB 42835가 인간 세포의 주요 사이토카인에 대한 균주 특이적 효과를 갖는 독특한 분리주임을 보여주는 계통발생적 데이터와 일치한다. 우리는 PNG의 기증자로부터 분리된 NCIMB 42835가 세계의 다른 지역에서 분리된 다른 L. reuteri 균주에 비해 증가된 사이토카인 수준에 의해 입증되는 바와 같이 면역 자극을 나타낸다는 것을 발견했다.

인간 시험. 참가자는 앨버타 대학(University of Alberta) 캠퍼스에서 모집되었으며 사전 동의를 받았다. 3부문 시험(three-arm trial) 설계에서, L. reuteri의 두 가지 균주(NCIMB 42835 및 DSM 20016T)의 효과를 위약(placebo) 그룹과 비교했다. 비-서양식 식단의 영향은 모든 연구 부문에서 교차 설계로 테스트된다. 섭취를 위한 L. reuteri 균주의 생산. L. reuteri 균주는 식품 등급 조건하에 Walter lab에서 섭취를 위해 제조되었으며, 세포 수는 수확 전에 설정되었으며 투여량은 Duar et al.(2017)에 의해 설명된 대로 ml당 약 109개 세포로 표준화되어 샘물(spring water)로 피험자에게 제공되었습니다.
분변 샘플에서
L. reuteri
세포 수의 측정.
분변 샘플은 2일마다 수집되었다(L. reuteri 또는 위약 섭취 전과 섭취 후 2주 동안 2회). LRIM 한천을 사용한 세균 배양에 의한 분변 샘플의 L. reuteri 균주의 절대 정량화. 이 한천은 인간 분변 샘플에서 L. reuteri를 정량적으로 분리하기에 충분히 선택적인 것으로 나타났다(Duar et al., 2017).
결과:

본 발명의 균주는 캡슐, 마이크로캡슐, 정제, 과립, 분말, 트로치(troche), 환제, 좌약, 현탁제 및 시럽과 같은 통상적인 제조의 경구 섭취가능한 형태로 동물(인간 포함)에게 투여될 수 있음이 이해될 것이다. 적합한 조성물(formulation)은 통상적인 유기 및 무기 첨가제를 사용하여 일반적으로 사용되는 방법에 의해 제조될 수 있다. 의약 조성물 중 활성 성분의 양은 원하는 치료 효과를 발휘할 수준일 수 있다.

장내 미생물군은 장 면역계의 발달과 적절한 기능에 중요하다. 장내 미생물군이 없는 경우, 무균 동물 모델에서 입증된 바와 같이, 장 면역계가 저개발되고 대식세포 식세포 능력 및 면역글로불린 생산과 같은 특정 기능적 파라미터(parameter)가 감소한다. 손상되지 않는 면역 반응을 자극하는 장내 미생물군의 중요성이 점점 더 분명해지고 있다. 서양 세계에서 알레르기 발병률 및 중증도의 증가는 보건 및 위생의 증가와 관련되어 있으며, 동시에 숙주가 직면하는 감염 문제의 수 및 범위의 감소와 관련이 있다. 이러한 면역 자극의 결여는 숙주가 비병원성이지만 항원성 물질에 반응하도록 하여 알레르기 또는 자가면역을 야기할 수 있다. 일련의 비병원성 면역조절 박테리아의 고의적인 섭취는 면역 기능의 적절한 발달과 제어를 위해 필요하고 적절한 교육적 자극을 숙주에게 제공할 것이다.

기타 활성 성분
