본 발명은 Fms-유사 티로신 키나아제(Fms-like tyrosine kinase-3: FLT3) 억제제와 약학적으로 허용되는 부형제를 포함하는 백혈병의 치료를 위한 약학적 조성물에 관한 것이다.
키나아제(kinase)는 고에너지분자, 특히 ATP의 인산기를 기질로 옮기는 반응을 매개한다. 키나아제는 인산무수결합을 안정화시키고, 기질과 인산기를 특정한 위치에 자리잡게 하여 반응 속도를 높이는 역할을 담당한다. 음전하를 띤 인산기와 상호작용하여 나타나는 전이상태는 대부분의 경우 양전하를 띤 주변의 아미노산을 통해 정전기적으로 안정화되고, 일부 키나아제는 금속 보조인자를 이용하여 인산기와 배위결합하기도 한다.
키나아제는 기질과 특성에 따라 단백질 키나아제, 지질 키나아제, 탄수화물 키나아제 등 다양한 그룹으로 나뉠 수 있다. 단백질, 지질 또는 탄수화물은 인산화 상태에 따라 활성, 반응성, 다른 분자와 결합할 수 있는 능력 등이 변화할 수 있다. 키나아제는 세포 내 신호전달(signal transduction)에 광범위한 영향을 미치고 세포 내부의 복잡한 생체 메커니즘을 조절한다. 어떤 분자는 인산화를 통하여 활성이 강화되거나 저해되며, 다른 분자와 상호작용하는 능력이 조절될 수 있다. 많은 키나아제가 환경 조건이나 신호에 따라 반응하기 때문에 세포는 키나아제를 통해 상황에 따라 세포 내의 분자를 통제할 수 있다. 따라서 키나아제는 세포의 성장, 분화, 증식, 생존, 물질대사, 신호전달, 세포 수송, 분비 및 그 밖에 수많은 세포반응경로에 매우 중요한 역할을 담당한다.
키나아제는 박테리아에서 곰팡이, 곤충, 포유류에 이르기까지 다양한 종에서 발견되고, 인간에게서는 500개 이상의 키나아제가 현재까지 발견되었다.
단백질 키나아제(Protein Kinase)는 단백질의 활성을 증가시키거나 감소시키고, 안정화하거나 분해를 위한 표식이 되기도 하며, 특정한 세포 구획에 위치시키기도 하고, 다른 단백질과의 상호작용을 개시하거나 교란할 수 있다. 단백질 키나아제는 전체 키나아제의 대부분을 차지하는 것으로 알려져 있으며, 중요한 연구대상이 되어 왔다. 단백질 키나아제는 인산분해효소와 함께 세포 신호전달뿐만 아니라 단백질 및 효소 조절 역할을 담당하는데, 세포 단백질은 수많은 공유결합의 대상이지만 인산화반응처럼 가역적인 공유결합은 많지 않기 때문에 단백질의 인산화가 조절적 기능을 갖는 것으로 설명될 수 있다. 단백질 키나아제는 종종 다수의 기질을 가지기도 하고, 때로는 특정 단백질이 하나 이상의 키나아제에 기질로 작용할 수도 있다. 이러한 이유로 단백질 키나아제는 스스로의 활성을 조절하는 인자를 사용하여 명명한다. 예를 들면, 칼모듈린 의존성 단백질 키나아제는 칼모듈린의 조절을 받는다. 때때로 키나아제는 하부 그룹으로 나뉘기도 한다. 예를 들어 제 1형 및 제 2형 고리형 AMP 의존성 단백질 키나아제는 동일한 효소 소단위로 구성되지만 다른 조절 소단위가 고리형 AMP에 결합하여 조절된다.
단백질 키나아제는 단백질의 티로신, 세린 및 트레오닌 잔기에 위치하는 하이드록시 그룹의 인산화를 촉매하는 효소로서 세포의 성장, 분화 및 증식을 유발하는 성장 인자 신호 전달에 중요한 역할을 담당하며(Melnikova, I. et al., Nature Reviews Drug Discovery, 3(2004), 993), 암세포에서는 특정 키나아제의 비정상적인 발현 또는 돌연변이가 빈번한 것으로 보고되고 있다.
일반적으로 외부의 자극을 세포가 인지하는 방법 중의 하나로서 세포막에 있는 수용체인 티로신 키나아제를 통한 인지가 알려져 있다. 수용체 티로신 키나아제(Receptor tyrosine kinase: RTK)는 세포 밖에 노출된 세포 외 부분, 세포 내 세포질에 노출된 세포 내 부분 및 그 중간에 위치하는 원형질막을 통과하는 막 통과 부분으로 구성되어 있다. 수용체의 세포 외 부분은 특정 리간드가 결합하는 부분이며, 세포 내 부분은 리간드에 의하여 활성화된 수용체의 활성 신호를 세포 내로 전달하는 기능을 수행한다. 수용체 티로신 키나아제는 세포 내에 노출된 C-말단 부위에 티로신 키나아제 활성을 갖는 도메인이 존재하여, 세로 외 부분에 특정 리간드가 부착하면 수용체 단백질의 세포질 부분에 노출된 C-말단의 티로신 키나아제 도메인의 키나아제 효소가 활성화되어, 이중체 상에서 서로의 C-말단에 있는 티로신을 인산화 시킨다. 이와 같은 티로신의 인산화 과정은 세포 외의 자극에 대한 신호를 세포 내로 전달하는 가장 중요한 과정이 된다. 이러한 기작을 가지고 세포 외 자극을 세포 내로 전달하는 티로신 키나아제 활성을 갖는 수용체는 많이 알려져 있다. 대표적인 예로써 FLT3, VEGFR, SYK 등을 들 수 있다.
이들 중 수용체 티로신 키나아제인 FMS 유사 티로신 키나아제(Fms-like tyrosine kinase 3: FLT3)는 조혈아세포에 의하여 통상적으로 조혈 전구세포에서 발현되고, 통상적인 줄기 세포의 발현 및 면역 시스템에 있어서 중요한 역할을 담당한다. FLT3의 비정상적인 과발현 및 돌연변이는 종종 백혈병 환자에서 관측된다. 특히, FLT3의 D835V, D835Y 및 ITD(internal tandem duplication) 등과 같은 다양한 돌연변이가 급성 골수성 백혈병(Acute myelogenous leukemia: AML)에서 관측된다. 급성 골수성 백혈병(AML)은 골수 및 말초 혈액 내의 아세포의 비정상적인 증식 및 분화를 특징으로 하는 단일성 조혈줄기세포 질환이다. FLT3는 최근 들어 AML의 치료적 관점에서 가장 주요한 타겟 중 하나로 여겨지고 있다.
성인 AML에서 RAS 및 p53 유전자 돌연변이가 성인 AML의 약 20% 및 5%로 보고되는데 반해, FLT3 유전자 돌연변이는 성인 AML의 약 30%에서 발견되고 있다. AML에서 가장 대표적인 문제점은, 좋지 않은 예후를 야기하는 FLT3의 돌연변이가 활성화된다는 것이다. FLT3 돌연변이는 크게 두 종류로 분류된다. 하나는 막근접(juxtamembrane) 영역 내의 내부 순차 중복(internal tandem duplications: ITD)이며, 다른 하나는 티로신 키나아제 도메인(tyrosine kinase domain: TKD) 내의 점돌연변이(point mutation)이다. 초기 AML 환자의 약 23%가 가장 많이 발견되는 돌연변이인 FLT3-ITD가 활성화 되어 있다. ITD 돌연변이가 있는 환자는 나쁜 예후를 보이며, 높은 재발률을 보인다. 또 다른 주요 FLT3 돌연변이는 FLT3 TKD 돌연변이로, 초기 AML 케이스의 약 7%가 이에 해당한다. 다양한 아미노산에 의하여 대체되는 아스파르트산염 835(D835)의 잔기에서의 점돌연변이는 ITD 돌연변이보다는 덜하나 가장 흔하게 일어나는 돌연변이 중 하나이다. 또한, AML에 있어서 FLT3의 또 다른 주요 활성화 방법은 야생형(wild type) FLT3 단백질의 과발현이다.
이와 같이, FLT3에서 ITD(internal tandem duplication) 돌연변이의 활성화는 급성 골수성 백혈병 환자의 약 20%에서 검출되며, 이는 예후 불량과 연관되어 있다. 연구를 통해, FLT3-ITD가 악성 발병에 원인이 되는 역할을 하는 구동 병태 (driver lesion)이며, 인간 AML에서 유효한 치료학적 타겟일 수 있는 것으로 확인되었다(비특허문헌 1). FLT3 유전자의 돌연변이는 AML에서 빈번하게 나타나는 현상으로, 통상적으로 막근접(juxtamembrane) 도메인 코딩 영역의 ITD(internal tandem duplication) 또는 티로신 키나제 도메인 (TKD)의 점 돌연변이를 수반한다.
FLT3-ITD 돌연변이와 FLT3-TKD 돌연변이는 모두 구성 요소의 이량체화와 FLT3 수용체의 활성화로 인해 리간드-비의존적인 증식을 야기한다. FLT3-ITD의 야생형 대립유전자 대비 높은 돌연변이 비율은 성인과 어린이 모두에서 매우 불량한 예후와 연관되어 있다(비특허문헌 2). 다른 유형의 백혈병, 예컨대 만성 골수단핵구성 백혈병(Chronic myelomonocytic leukemia: CMML) 또한 FLT3의 활성화 돌연변이를 보유할 수 있다. 따라서, 활성화 돌연변이를 갖는 FLT3은 여러 암 유형에 대한 중요한 표적이다(비특허문헌 3 및 비특허문헌 4).
특히나, 이미 승인된 약물 또는 개발중인 FLT3 저해제의 불응성/재발성 급성 골수성 백혈병 환자에 대한 분석에서 다양한 획득 내성 (acquired resistance)로서 다양한 TKD 변이가 보고 되었다. 대표적인 예로서, receptor domain (RD)의 K429A 변이, JM 도메인의 Y572C, TK1 도메인의 A627P, N676K/D/I/S, F691L과 TK2 도메인의 D835Y/F/I/H/V/A, Y842C/H/N등이 있다 (비특허문헌 5). FLT3 저해제 임상에서의 내성 환자 분석에서 가장 중요한 획득 내성으로 F691L이 있으며, F691L의 경우, 약물의 활성을 저해할 수 있는 gatekeeper 변이이다. 따라서 이러한 F691L 단독 또는 ITD-F691L의 이중변이를 타겟팅 할 수 있는 약물의 개발이 미충족 의료로 간주되고 있다(비특허문헌 6).
혈액 세포에 주로 발현이 되어 있는 비장 티로신 키나아제(Spleen tyrosine kinase: SYK)은 B 세포 수용체(B-cell receptor) 및 마스트 세포와 같은 다른 면역 수용체들의 신호 전달 경로에서 중요한 역할을 한다. 비장 티로신 키나아제는 또한 신경 세포 및 혈관 내피 세포와 같은 비조혈 세포에서도 발현된다. 최근 연구에서는 IL-1, TNF-α, ITGB1을 포함하는 다양한 세포 자극의 산화화에서 중요한 역할을 하는 것이 증명 되었다. 본 명세서에서 FLT3는 조혈 간세포의 표면에서 통상 발현되는 클래스 III 수용체 티로신 키나아제(TK) 패밀리의 멤버이다. FLT3 및 그 리간드는 다능 줄기 세포의 증식, 생존 및 분화에 중요한 역할을 한다. FLT3은 다수의 AML 케이스에서 발현된다. 또한, 막근접 도메인에 그리고 그 주위에 유전자내 종렬 중복(ITD)을 갖는 활성화된 FLT3 및 활성화 루프에서 D835 근처에 티로신 키나아제 도메인(TKD) 돌연변이는 각각 AML 케이스의 28% 내지 34% 및 11% 내지 14%로 존재한다. FLT3에서 이들 활성화된 돌연변이는 종양원성이고 세포에서 변형 활성을 나타낸다. FLT3-ITD 돌연변이를 갖는 환자는 임상 연구에서 불량한 예후를 나타내고, 재발률이 더 높으며, 처음 치료로부터 완화 지속기간이 더 짧고(FLT3-ITD 돌연변이가 없는 환자의 11.5달에 대하여 6달), 무병 생존율이 감소되고 및 OS가 감소된다. 조혈 줄기세포 이식(HSCT) 후 재발 발생도 또한 FLT3-ITD 환자에 대해서 더 높다. 1차 치료에 대한 예후와 비슷하게, 재발된/불응성 FLT3-돌연변이 양성 AML 환자는 구제 항암 요법에 의한 완화율이 더 낮고, 2차 재발까지의 완화 기간이 더 짧으며 FLT3-돌연변이 음성 환자에 대하여 감소된 OS를 가진다.
본 명세서에서 암은 백혈병 예컨대 급성 골수성 백혈병(Acute myelogenous leukemia: AML), 만성 골수성 백혈병(Chronic myelogenous leukemia: CML), 급성 림프구성 백혈병(Acute lymphocytic leukemia: ALL), 만성 림프구성 백혈병(Chronic lymphocytic leukemia, CLL), 급성전 골수성 백혈병(Acute promyelocytic leukemia: APL), 모상세포 백혈병(hairy cell leukemia), 만성 호중구성 백혈병(chronic neutrophilic leukemia: CNL) 등을 포함한다.
일 구체예에서, 상기 암은 백혈병일 수 있다.
일 구체예에서 상기 백혈병은 급성 골수성 백혈병, 급성 림프구성 백혈병, 또는 만성 골수성 백혈병을 포함할 수 있다.
본 명세서에서 급성 골수성 백혈병(AML)은 FLT3 돌연변이를 갖는 급성 골수성 백혈병을 포함한다. 일 구체예에서, 급성 골수성 백혈병은 돌연변이체 FLT3 폴리뉴클레오티드-양성급성 골수성 백혈병, FLT3 유전자내 종렬 중복(ITD) 양성 급성 골수성 백혈병, 또는 FLT3 점 돌연변이를 갖는 급성 골수성 백혈병을 포함한다.
상기 FLT3 점 돌연변이는 FLT3 아미노산 서열의 티로신 키나아제 도메인(tyrosine kinase domain: TKD)(FLT3-TKD) 영역 내에 돌연변이일 수 있다.
본 명세서에서 "약학적으로 허용 가능한"은 일반적으로 비독성, 불활성, 및/또는 생리학적으로 양립가능한 조성물 또는 조성물의 구성요소를 지칭한다. "약학적 부형제(excipient)" 또는 "부형제"는 보조제, 담체, pH-조절 및 완충제, 등장성 조절제, 습윤제, 보존제 등과 같은 물질을 포함한다. "약학적 부형제"는 약학적으로 허용 가능한 부형제이다. 상기 약학적 조성물은 통상적인 약학적으로 허용되는 부형제 또는 첨가제를 포함할 수 있다. 본 발명의 약학적 조성물은 통상적인 방법에 따라 제제화할 수 있으며, 정제, 환제, 산제, 캅셀제, 시럽, 에멀젼, 마이크로에멀젼 등의 다양한 경구 투여 형태 또는 근육내, 정맥내 또는 피하 투여와 같은 비경구 투여 형태로 제조될 수 있다.
상기 약학적 조성물이 경구 제형의 형태로 제조되는 경우, 사용되는 담체 또는 첨가제, 부형제의 예로는 희석제, 붕해제, 결합제, 활택제, 계면활성제, 현탁제, 또는 유화제 등을 들 수 있다. 본 발명의 약학적 조성물이 주사제의 형태로 제조되는 경우 상기 담체 또는 첨가제, 부형제로는 물, 식염수, 포도당 수용액, 유사 당수용액, 알콜, 글리콜, 에테르(예: 폴리에틸렌글리콜 400), 오일, 지방산, 지방산에스테르, 글리세라이드, 계면활성제, 현탁제, 또는 유화제 등을 들 수 있다. 이러한 제제화 방법는 당해 제제학 분야에서 통상의 지식을 가진 자에게 널리 알려져 있다.
상기 약학적 조성물에 포함되는 활성성분으로서 화학식 A 또는 화학식 1의 화합물은 개체 또는 환자의 치료 또는 예방에 유효한 양으로서, 목적하는 바에 따라 경구 또는 비경구 투여할 수 있으며, 경구 투여시는 활성성분을 기준으로 하루에 체중 1 kg당 예를 들어 0.01 내지 1000 mg, 0.01 내지 500 mg, 0.1 내지 300 mg, 또는 0.1 내지 100mg의 양으로 투여되도록, 비경구 투여시는 활성성분을 기준으로 하루에 체중 1 kg당 예를 들어, 0.01 내지 100 mg 또는 0.1 내지 50 mg의 양으로 투여되도록 포함될 수 있으며, 상기 조성물은 1 내지 수회에 나누어 투여할 수 있다. 특정 개체 또는 환자에 대한 투여 용량은 환자의 체중, 연령, 성별, 건강 상태, 식이, 투여 시간, 투여 방법, 질환의 중증도 등의 여러 관련 인자에 비추어 결정되어야 하는 것이고 전문가에 의해 적절히 가감될 수 있는 것으로 이해되어야 하며, 상기 투여량은 어떠한 면으로든 본 발명의 범위를 한정하기 위한 것은 아니다.
본 발명의 다른 일 양상은 상기 약학적 조성물을 이용하여 암을 예방 또는 치료하는 방법을 제공한다.
일 구체예에서 상기 치료방법에 사용되는 화합물의 투여량, 투여횟수 또는 투여방법은 처리되는 대상, 질병 또는 상태의 심각도, 투여의 속도 및 처방 의사의 판단에 따라 다를 수 있다. 통상 체중 70 kg의 사람에 대한 투여량은 하루 0.1 내지 2,000 ㎎, 예를 들어 1 내지 1,000 ㎎ 또는 10 내지 2,000 ㎎의 양으로 투여될 수 있다. 투여 횟수는 1회 내지 수회 예컨대 1 내지 4회 또는 온/오프(on/off) 스케줄로 투여될 수 있고, 투여방법은 경구 또는 비경구적 경로를 통해 투여될 수 있다. 일부 경우에 있어서, 상기 언급된 범위 보다 적은 투여량이 보다 적합할 수도 있고, 해로운 부작용을 일으키지 않으면서도 보다 많은 투여량이 사용될 수도 있으며, 보다 많은 투여량의 경우는 하루에 걸쳐 수회의 적은 투여량으로 분배될 수 있다. 관련 기술 분야의 통상의 기술을 갖는 의사는 사용되는 화합물의 투여량을 필요에 따라 용이하게 결정 및 처방할 수 있다. 예를 들어, 의사는 제약 조성물에 사용되는 본 발명의 화합물의 용량을 목적하는 치료 효과를 달성하는데 요구되는 것보다 낮은 수준에서 출발하여, 목적하는 효과가 달성될 때까지 투여량을 점진적으로 증가시킬 수 있다.
일 구체예에서 상기 치료 방법은 본 발명의 일 양상에 따른 화합물을 활성 성분으로서 단독으로, 또는 암, 종양 또는 백혈병을 치료하기 위한 알려진 하나 이상의 다른 약제 또는 약학적 담체와 조합하여 사용할 수 있다. 일 구체예에 따른 화학식 A 또는 화학식 1의 화합물, 이의 입체 이성질체, 호변 이성질체, 용매화물, 및 약제학적으로 허용 가능한 염 중에서 선택되는 화합물은, 다른 FLT3 키나아제 활성 저해용 약제 또는 FLT3 키나아제 활성 저해의 효능을 높이거나 상승 작용을 나타내는 다양한 기전의 다른 약제와 함께 병용 투여함으로써 FLT3 활성의 저하 또는 FLT3 매개 질환의 치료효과를 강화시킬 수 있다.
본 명세서에서 용어 “치료”는 질병의 치료(treatment), 개선(improvement), 완화(amelioration) 또는 관리(management)를 모두 포함하는 개념으로서 사용된다. 본 명세서에서 용어 “치료하는" 또는 "치료"는 질환을 저해하는 것, 예를 들어, 질환, 병태 또는 장애의 병리 또는 징후를 경험하거나 또는 나타내는 개체에서 질환, 병태 또는 장애를 저해하는 것, 병리 및/또는 징후의 추가적인 발생을 막는 것, 질환을 개선시키는 것, 또는 병리 및/또는 징후를 반전시키는 것, 예컨대 질환 중증도를 감소시키는 것을 말한다.
본 명세서에서 용어 "예방하는" 또는 "예방"은 질환을 예방하는 것, 예를 들어 질환, 병태 또는 장애의 성향이 있을 수 있지만 질환의 병리 또는 징후를 아직 경험하지 않았거나 나타내지 않는 개체에서 질환, 병태 또는 장애를 예방하는 것을 말한다.
본 명세서에서 용어 “개체” 또는 “환자”는 포유류, 예를 들어, 마우스, 래트, 기타 설치류, 토끼, 개, 고양이, 돼지, 소, 양, 말 또는 영장류 및 인간을 포함하는 임의의 동물을 말한다.
본 명세서에서 용어, "가진다", "가질 수 있다", "포함한다", 또는 "포함할 수 있다" 등의 표현은 해당 특징(예: 수치, 또는 성분 등의 구성요소)의 존재를 가리키며, 추가적인 특징의 존재를 배제하지 않는다.
본 발명의 일 양상에 따른 약학적 조성물은 FLT3 억제 활성이 우수하므로, 암, 예컨대 백혈병 등의 비정상적 FLT3 활성에 의한 세포 증식 질환의 예방 또는 치료에 효과적으로 사용될 수 있다. 또한, 상기 약학적 조성물은 F691L 획득 돌연변이를 지닌 백혈병을 포함한 암의 치료에 사용될 수 있다.
도 1은 MOLM-14 FLT3-ITD/F691L 세포주로 이종 이식된 누드 마우스에서 대조군, 화합물 A 및 길테리티닙을 투여하였을 때의 항종양 효과를 나타낸 것이다.
이하, 본 발명을 보다 상세하게 설명한다.
본 발명에서 사용되는 모든 기술용어는, 달리 정의되지 않는 이상, 본 발명의 관련 분야에서 통상의 당업자가 일반적으로 이해하는 바와 같은 의미로 사용된다. 또한, 본 명세서에는 바람직한 방법이나 시료가 기재되나, 이와 유사하거나 동등한 것들도 본 발명의 범주에 포함된다. 또한, 본 명세서에 기재된 수치는 명시하지 않아도 “약”의 의미를 포함하는 것으로 간주한다. 본 명세서에 참고문헌으로 기재되는 모든 간행물의 내용은 전체가 본 명세서에 참고로 통합된다.
이하, 본 발명을 하기 실시예 및 실험예에 의해 더욱 구체적으로 설명한다. 그러나, 이들 실시예 및 실험예는 본 발명에 대한 이해를 돕기 위한 것일 뿐, 어떤 의미로든 본 발명의 범위가 이들에 의해 제한되는 것은 아니다.
[실시예 1] 키나아제 저해 시험법
FLT3 억제제인 5-클로로-N-(3-사이클로프로필-5-(((3R,5S)-3,5-디메틸피페라진-1-일)메틸)페닐)-4-(6-메틸-1H-인돌-3-일)피리미딘-2-아민 (이하 화합물A)에 대하여 야생형 혹은 돌연변이형 FLT3 및 SYK에 대한 저해 활성을 측정하였다.
활성 측정은 Thermo Fisher Scientific사에 의해 개발된 란타스크린(LanthaScreen) 시험법 (야생형 및 돌연변이형 FLT3) 혹은 Z'-LYTE 시험법 (SYK)을 이용하였다. 란타스크린(LanthaScreen) 시험법은 Alexa Fluor 647-라벨된, ATP-경쟁적 키나아제 저해제(kinase tracer-236)의 키나아제에 대한 결합(binding)을 기반으로, 유로피움-결합항체(europium-conjugated antibody)의 존재 하에 FRET(fluorescence resonance energy transfer) 신호(signal)를 측정함으로써 단백질의 활성을 측정하는 방법이다. Z'-LYTE 시험법은 비인산화된 기질을 절단할 수 있는 효소를 이용한 방법으로 펩타이드 기질의 각 끝에 달린 두개의 형광의 FRET 신호를 측정 함으로써 키나아제 단백질의 활성을 측정하는 방법이다. 두 실험은 모두 384웰 플레이트(well plate)에서, 50 mM HEPES pH 7.5, 0.01% BRIJ-35, 10 mM MgCl2, 1 mM EGTA, 1% DMSO 조건 하에서 수행되었다. 기저신호(Background signal)를 각 키나아제가 없는 상태에서 측정하고, 비저해신호로서 용매(1% DMSO)만을 첨가하여 측정한 후에, 평가하고자 하는 화합물A 를 설정된 농도(예를 들면, 50 ~ 0.05 nM, 1/10 희석)로 계단식 희석하여 처리하였고, 화합물A의 야생형 FLT3, FLT3-ITD, FLT3 D835Y 및 SYK에 대한 50% 활성 저해값(IC50)은 GraphPad Prism 소프트웨어를 이용하여 산출하였다. 그 결과는 하기 표 1에 나타내었다.

[실시예 2] 야생형 혹은 돌연변이형 FLT3에 대한 결합능력 측정 시험법
화합물 A에 대하여 야생형 혹은 돌연변이형 FLT3에 대한 결합 능력은 DiscoverX사의 KINOMEscan 스크리닝 플랫폼을 이용하여 측정 되었다. KINOMEscan 시험법은 DNA가 연결된 키나아제 효소의 활성 부위에 대해 경쟁결합측정법(competitive binding assay)을 이용해 시험하고자 하는 물질과 키나아제(kinase)간의 결합을 정량 PCR(quantitative PCR)을 이용하여 정량적으로 측정하는 방법이다. 시험은 DiscoverX사에 의뢰하여 진행 되었으며, 각 야생형 혹은 돌연변이형 FLT3에 대한 억제제의 결합 능력은 Kd 값으로 산출 되었다. 그 결과는 하기 표 2에 나타내었다.

MOLM-14 FLT3-ITD/F691L 세포주가 피하 이식된 마우스 모델
MOLM-14 FLT3-ITD/F691L 세포주가 피하 이식된 마우스 모델에서 FLT3 저해제인 화합물 A와 또다른 FLT3 저해제인 6-에틸-3-[3-메톡시-4-[4-(4-메틸피페라진-1-일)피페리딘-1-일]아닐리노]-5-(옥산-4-일아미노)피라진-2-카르복스아드 (이하 길테리티닙)의 비교 효능 시험을 진행하였다.
MOLM-14 FLT3-ITD/F691L 세포주를 3×10^6 세포/0.15mL/마우스로 누드 마우스에 피하 접종(subcutaneous injection)하고 자라게 두었다.
대조군은 DMSO/PEG400/DW (비율=0.5/2/7.5, v/v) 혼합 용액을 1일 1회 경구로 투여 받았고, 화합물 A군은 30 mg/kg/day 용량으로 1일 1회 경구 투여 받았으며, 길테리티닙군은 30 mg/kg/day 용량으로 1일 1회 경구 투여 받았다. 대조군은 13일 동안, 약물 투약군은 18일동안 개별 약물을 투여 받았다.
실험 결과를 도 1에 나타내었다. 도 1은 MOLM-14 FLT3-ITD/F691L 세포주로 이종 이식된 누드 마우스에서 대조군, 화합물 A 및 길테리티닙을 투여하였을 때의 항종양 효과를 나타낸 것이다. Y축은 각군에서의 생존마우스의 종양 부피(mm3)을 나타내고, X축은 투약 일수를 나타낸다. 약물 투여에 따른 효과를 평가하기 위해 종양이 완전히 소실되는 완전 반응(CR)의 결과를 확인하였다. 도 1에서와 같이, 약물 투여에 따른 종양 부피를 측정하여 확인한 결과, 화합물 A의 완전 반응이 투여 12일차에 나타났다. 또한, 도 1에서와 같이, 상기 결과는 길테리티닙 투여군보다 화합물 A의 투여군에서의 종양 부피가 크게 감소 되어 보다 우수한 항종양 효능을 보였다. (* p <0.05, ** p <0.01, *** p <0.001, and **** p <0.0001 이원분산분석 비교 후 Sidk's test수행)
이제까지 본 발명에 대하여 그 구체예들을 중심으로 살펴보았다. 본 발명이 속하는 기술 분야에서 통상의 지식을 가진 자는 본 발명이 본 발명의 본질적인 특성에서 벗어나지 않는 범위에서 변형된 형태로 구현될 수 있음을 이해할 수 있을 것이다. 그러므로, 상기 개시된 구체예들은 한정적인 관점이 아니라 설명적인 관점에서 고려되어야 한다. 본 발명의 범위는 전술한 설명이 아니라 특허청구범위에 나타나 있으며, 그와 동등한 범위 내에 있는 모든 차이점은 본 발명에 포함된 것으로 해석되어야 할 것이다.
Claims (3)
- Fms-유사 티로신 키나아제(Fms-like tyrosine kinase-3: FLT3) 억제제와 약학적으로 허용되는 부형제를 포함하는 약학적 조성물로서,
상기 FLT3 억제제는 하기 화학식 1의 화합물, 이의 입체 이성질체, 이의 호변 이성질체, 및 이들의 조합으로 이루어진 군으로부터 선택하는 적어도 하나의 화합물이거나;
이 선택한 적어도 하나의 화합물의 약제학적으로 허용되는 염 또는 이 선택한 적어도 하나의 화합물의 용매화물이며;
상기 약학적 조성물은 백혈병의 치료를 위한 약학적 조성물로서, 상기 백혈병 암세포는 FLT3 유전자내 종렬 중복(internal tandem duplication; ITD) 변이를 지니며,
F691L, D835Y, D835F, D835I, D835H, D835V와 D835A 중에서 선택하는 적어도 하나의 FLT3 돌연변이를 더 포함하는 약학적 조성물:
