Abstract

<발명의 분야>
본 발명은 종양에 대한 입양 면역 유전자 요법 분야에서 유용한, 다발성 골수종 종양 성장을 특이적으로 감소시키는 인간화 BCMA 항체 (PMC306) 및 BCMA-CAR-T 세포에 관한 것이다.
<발명의 배경>
면역요법은 암 치료를 위한 매우 유망한 접근법으로 부상하고 있다. 우리의 면역 체계의 무장 군대인 T 세포 또는 T 림프구는 지속적으로 외래 항원을 찾고 정상 세포와 비정상 세포 (암 또는 감염된 세포)를 구별한다. BCMA

<발명의 상세한 설명>
<정의>
본원에서 사용된 바와 같은 "키메라 항원 수용체 (CAR)"는 특정 단백질을 표적으로 하는 새로운 능력을 T 세포에 부여하도록 조작된 수용체 단백질이다. 수용체는 항원-결합 및 T-세포 활성화 기능 둘 모두를 단일 수용체로 조합하기 때문에 키메라이다. 본 발명자들은 마우스 모노클로날 항체, 클론 4C8A로부터 유래된 마우스 모노클로날 항체의 중쇄 및 경쇄 가변 영역으로부터 시작하여 인간화 BCMA scFv를 조작하였다. 마우스 4C8A 항체는 인간 BCMA에 대해 강력하고 선택적인 결합을 나타낸다 [6]. 본 발명자들은 BCMA 종양 항원을 과다발현하는 암 세포를 표적으로 하기 위해 인간화 BCMA 항체에 기초한 BCMA-CAR-T 세포를 생산하였다. 본 발명의 BCMA-CAR-T 세포는 여러 암 세포주에 대해 높은 세포독성 활성을 갖는다.

본 발명은 BCMA-CAR을 코딩하는 핵산을 제공한다. CAR-T 세포는 CAR을 통해 특이적인 항원에 결합하여, 이에 의해 신호가 세포로 전달되고, 그 결과 세포가 활성화된다. CAR을 발현하는 세포는 질환을 위한 치료제로서 사용될 수 있다. 치료제는 활성 성분으로서 CAR을 발현하는 세포를 포함하고, 적합한 부형제를 추가로 포함할 수 있다.
본 발명자들은 BCMA를 특이적으로 표적으로 하는 인간화 BCMA ScFv 서열에 기초하여 CAR-T 세포를 생산하였다. 본 발명자들은 BCMA 종양 항원을 과다발현하는 암 세포를 표적으로 하기 위해 인간화 BCMA-CAR-T 세포를 생산하였다. 본 발명의 인간화 BCMA-CAR-T 세포는 다발성 골수종 암 세포에 대해 높은 수준의 시토카인을 분비하였고, CHO-BCMA-양성 표적 세포를 사멸시키지만, 대조군 부모 CHO 세포를 사멸시키지 않는다.
상응하는 마우스 ScFv에 비해 본 발명의 인간화 BCMA-ScFv의 장점은 인간화 BCMA scFv 서열로 인해 인간에 대한 더 적은 면역원성을 포함한다. 그러므로, 본 발명의 인간화 BCMA 항체는 많은 임상 적용에서 치료제로서 매우 강력하고 유리하다.

실시예 1. 인간화 BCMA VH 및 VL 및 scFv 서열
BCMA scFv는 하이브리도마 클론 4C8A (WO2019/195017)로부터 유래되었다. 마우스 클론 4C8A의 중쇄 및 경쇄 가변 영역의 서열을 결정하고, 인간화 scFv를 구축하는데 사용하였다.
인간화 BCMA (PMC306) scFv의 구조는 VH-링커-VL이다. 링커는 G4Sx3이다.

실시예 2. 인간화 BCMA-CAR 서열

실시예 3. CAR 렌티바이러스 생산
[7]에 기재된 바와 같이 293T 세포를 사용하여 표준 절차에 의해 렌티바이러스를 생산하였다. 본 발명자들은 렌티바이러스 벡터의 Xba I 및 Eco R I 부위로 클로닝된 렌티바이러스 벡터 내부에 인간화 BCMA-ScFv-CAR 구축물을 생성하였다. Xba I 및 Eco RI 클로닝 부위 사이에 인간화 BCMA ScFv-CD28-CD3제타 삽입물을 함유하는 pCD510-FMC63-28z 렌티바이러스 CAR 구축물
렌티바이러스를 293T 세포에서 생성시키고, 역가를 RT-PCR에 의해 확립하였다. 그 후, T 세포의 형질도입을 위해 동일한 용량의 렌티바이러스를 사용하였다.
실시예 4. 전혈로부터의 말초 혈액 단핵 세포 (PBMC) 단리
전혈 (스탠포드 병원 혈액 센터(Stanford Hospital Blood Center), 미국 캘리포니아주 스탠포드)을 10 mL 헤파린 진공채혈기 (벡톤 디킨슨(Becton Dickinson))에서 개인 또는 혼합 공여자 (필요한 혈액량에 따라)로부터 수집하였다. 대략 10 ml의 항응고 전혈을 50 ml 원심분리 튜브 (PBS, pH 7.4, Ca+2 및 Mg+2 없음)에서 20 ml의 총 부피로 멸균 포스페이트 완충 염수 (PBS) 완충액과 혼합하였다. 혈액/PBS (20 ml)를 코니칼 원심분리 튜브에 15 mL의 피콜-파큐 플러스(Ficoll-Paque PLUS) (지이 헬스케어(GE Healthcare))의 상단에 완만하게 층을 이루게 하고, 샘플을 실온에서 30-40분 동안 400xg에서 원심분리하였다. 희석된 혈장/피콜(Ficoll) 계면에 말초 혈액 단핵 세포 (PBMC)를 함유하는 세포층을 제거하고, 세척하고, 실온에서 10분 동안 200xg에서 원심분리하였다. 세포를 혈구계측기로 계수하였다. PBMC를 CAR-T 배지 (AIM V-AlbuMAX(BSA) (라이프 테크놀로지스(Life Technologies)), 5% AB 혈청 및 1.25 μg/mL 암포테리신 B (제미니 바이오프로덕츠(Gemini Bioproducts), 미국 캘리포니아주 우드랜드), 100 U/mL 페니실린, 및 100 μg/mL 스트렙토마이신 함유)로 1회 세척하고, 실험에 사용하거나 -80℃에서 동결하였다.
실시예 5. PBMC로부터의 T-세포 활성화
단리된 PBMC 세포를 300 U/mL huIL2 (1000x 스톡으로부터; 인비트로젠(Invitrogen))를 갖는 CAR-T 배지에 재현탁하고, 1:1 비드-대-세포 비율로 CD3-CD28 비드와 혼합하였다. 세포를 바이러스 형질도입 전에 24시간 동안 CO2의 존재하에 37℃에서 인큐베이션하였다.
실시예 6. T-세포 형질도입 및 확장
PBMC의 활성화 후, 세포를 24시간 동안 37℃, 5% CO2에서 인큐베이션하였다. 1x106개 세포의 각 웰에 5x106개 렌티바이러스 및 2 μL/mL의 트랜스플러스(Transplus) 배지 (알스템(Alstem), 미국 캘리포니아주 리치몬드) (1:500의 최종 희석)를 첨가하였다. 바이러스 첨가를 반복하기 전에 세포를 추가 24시간 동안 인큐베이션하였다. 그 후, 12-14일의 기간 동안 IL-2를 갖는 300 U/ML IL-2 신선한 배지의 계속된 존재하에 세포를 성장시켰다 (총 인큐베이션 시간은 필요한 CAR-T 세포의 최종 수에 따라 달라짐). 세포 농도를 2-3일마다 분석하였으며, 이 때 배지를 첨가하여 세포 현탁액을 1x106개 세포/mL로 희석하였다.
실시예 7. 인간화 BCMA-CAR-T 세포는 BCMA scFv를 발현하였다
본 발명자들은 실시예 2에 나타낸 인간화 BCMA-CAR 구축물로 인간화 BCMA-CAR-T 세포를 설계하였다. 본 발명자들은 관련되지 않은 ScFv와 함께 모의 scFv를 사용하였으며, 음성 대조군으로 모의-CAR-T 세포를 생성하였다. 렌티바이러스 인간화 BCMA CAR의 T 세포로의 형질도입 후 인간화 BCMA-CAR-양성 세포가 검출되었다 (도 4).
실시예 8. 인간화 BCMA-CAR-T 세포는 CHO-BCMA 세포를 사멸시켰지만, CHO 세포를 사멸시키지 않았다
본 발명자들은 인간화 BCMA-CAR-T 세포를 표적 CHO-BCMA 표적 세포 및 CHO (BCMA-음성) 대조군 세포와 함께 인큐베이션하였다. 인간화 BCMA-CAR-T 세포는 CHO-BCMA 세포를 특이적으로 사멸시켰지만 (도 5a), CHO 세포를 사멸시키지 않았다 (도 5b). 결과는 BCMA 항원을 표적으로 하고 BCMA-양성 세포를 사멸시키는 인간화 BCMA-CAR-T 세포의 높은 특이성을 나타낸다.
실시예 9. 인간화 CAR-T 세포는 표적 CHO-BCMA 세포에 대해 IFN-감마를 유의하게 분비하였지만 CHO 세포에 대해서는 분비하지 않았다
본 발명자들은 인간화 BCMA-CAR-T 세포 및 표적 CHO-BCMA 및 부모 CHO 세포의 공동-인큐베이션 후 상등액을 수집하고, IFN-감마 검정을 수행하였다. BCMA-CAR-T 세포는 CHO-BCMA 세포에서는 IFN-감마를 분비하였지만, 음성 대조군 CHO 세포에서는 분비하지 않았다 (도 6). 결과는 인간화 BCMA-CAR-T 세포의 특이성을 확인한다.
실시예 10. 인간화 CAR-T 세포는 BCMA-양성 RPMI8226 다발성 골수종 세포에 대해 높은 수준의 IFN-감마를 분비하였지만, BCMA-음성 K562 백혈병 세포에 대해서는 분비하지 않았다.
본 발명자들은 BCMA-CAR-T 세포를 다발성 골수종 암 세포 RPMI8266, 및 BCMA-음성 K562 세포 (만성 골수성 백혈병 세포)와 함께 인큐베이션하고, 제조사의 프로토콜에 따라 피셔(Fisher)로부터의 키트를 사용하여 IFN-감마로 ELISA를 수행하였다. 인간화 BCMA-CAR-T 세포는 BCMA-양성 다발성 골수종 암 세포에 대해 높은 수준의 IFN-감마를 분비하였지만, BCMA-음성 K562 세포에 대해서는 분비하지 않았다 (도 7). IFN-감마의 사멸 및 분비의 수준은 T 세포 및 모의 CAR-T 세포보다 BCMA-CAR-T 세포에서 유의하게 더 높았다. 이는 혈액학적 BCMA-양성 세포에 대한 인간화 BCMA-CAR-T 세포의 특이성을 확인한다.
실시예 11. 인간화 BCMA-CAR-T 세포는 생체내 마우스 모델에서 RPMI8226 이종이식편 종양 성장을 유의하게 감소시켰다.
다발성 골수종 RPMI8226 세포를 NSG 마우스에 피하로 주사한 다음 (1x10^7개 세포/마우스), 인간화 BCMA-CAR-T 세포를 i.v.에 의해 2회 주사하였다 (1x10^7개 CAR-T 세포/마우스). 인간화 BCMA-CAR-T 세포는 마우스에서 RPMI8226 종양 성장을 유의하게 감소시켰다 (도 8a). 인간화 BCMA-CAR-T 세포로 처리된 마우스는 마우스 체중 감소를 유발하지 않았으며, 이는 CAR-T 세포가 마우스에 독성이 없었음을 시사한다 (도 8b). 연구 동안 행동 또는 시각적 변화는 관찰되지 않았다.
인간 T (CD4+/CD8+) 세포에 대해 특이적인 인간 BCMA 단백질 및 항체에 대한 결합에 대해 연구가 끝날 때 말초 혈액 세포를 유동 세포계측법에 의해 분석하였다. CD4 mAb에 결합하는 세포의 백분율은 도 8c의 왼쪽 패널에 나타내고, BCMA 단백질에 또한 결합한 인간 T 세포의 백분율은 도 8c의 오른쪽 패널에 나타낸다. 결과는 인간화 BCMA-CAR-T 세포 (그러나 모의-Car-T-세포는 아님)가 BCMA 재조합 단백질을 사용한 FACS에 의해 마우스 혈액에서 검출되었음을 나타낸다.
