Abstract
[과제] 본 발명의 목적은 면역 체크포인트 억제제에 의한 항종양 효과를 증강시키기 위한 제약 조성물을 제공하는 것이다. [해결수단] 5-아미노레불린산(ALA)을 유효 성분으로서 함유하는, 면역 체크포인트 억제제에 의한 항종양 효과를 증강시키기 위한 제약 조성물이 제공된다.
Description
본 발명은 5-아미노레불린산(이하, 간단히 "ALA"라고도 칭함) 또는 그의 유도체의 신규한 용도, 구체적으로는 면역 체크포인트 억제제(immune checkpoint inhibitor)에 의한 항종양 효과를 증강시키기 위한 5-아미노레불린산 또는 그의 유도체의 용도에 관한 것이다.
인체에서 발생한 종양은, 통상 내재하는 면역 메커니즘에 의해 인체에 유해한 영향을 미칠 정도로 증식하기 전에 제거된다. 이 메커니즘으로서 하기 현상이 공지되어 있다. 즉, 암 세포에 특이적인 단백질(종양 항원)은 암 세포 중의 클래스 I MHC 분자로 칭해지는 단백질과 결합하여, 세포 표면에 제시된다. 이 클래스 I MHC와 종양 항원이, 면역을 담당하는 T 세포의 표면 상에 TCR(T 세포 수용체) 단백질과 결합하는 경우, T 세포는 이 세포를 암 세포로서 인식한다. 암 세포를 인식한 T 세포는 증식을 개시하고, 종양 항원을 제시하고 있는 암 세포를 손상시킨다.
그러나, 암 세포가 클래스 I MHC와 함께 PD-L1로 칭해지는 막 단백질을 발현하고 있었던 경우, T 세포에서 발현하고 있는 PD-1로 칭해지는 막 단백질과 결합함으로써, 상기 T 세포의 활성화를 억제하는 것이 공지되어 있다. 정상적으로는 과잉 면역을 억제하기 위한 면역학적 회피 메커니즘을 획득한 이 암 세포는, 증식을 계속하기 때문에, PD-L1과 PD-1의 결합을 억제하는 것이 신규 항암제의 표적으로서 주목되어 왔다(특허문헌 1). PD-L1은 항원의 T 세포에의 제시를 전문으로 행하는 수지상 세포와 같은 "전문적 항원-제시 세포(professional antigen-presenting cells)"에서 또한 발현된다는 점에 주의한다. 최근, 항암제로서 항-PD-1 항체인 니볼루맙(nivolumab) (옵디보(Opdivo)®) 및 펨브롤리주맙(pembrolizumab) (키트루다(Keytruda)®)이 개발되어 실용화되어 있고, 항-PD-L1 항체인 아테졸리주맙(atezolizumab), 두르발루맙(durvalumab), 아벨루맙(바벤시오(Bavencio)™) 등이 또한 개발되어 실용화되어 있다.
다른 한편으로는, T 세포에의 암 세포(또는 전문적 항원-제시 세포)의 항원 제시에 있어서, 면역 반응을 억제하는 다른 메커니즘(면역 체크포인트)도 공지되어 있고, 그 중 대표적인 것이, 항원-제시 측의 B7 패밀리 분자와 T 세포 측의 CTLA4의 조합이며, 이필리무맙(ipilimumab) (여보이(Yervoy)®) 및 트레멜리무맙(tremelimumab)과 같은 약제가 개발되어 있다. 게다가, CD137L-CD137, MHC-LAG-3/KIR, CD48-CD244, GAL9-TIM3, HVEM-BTLA/CD160, CD40L-CD40, OX40L-OX40, GITRL-GITR 등이 공지되어 있다(그 중 모든 것에서 전자가 항원-제시 측의 분자이다) (비특허문헌 1 및 2).
최근, 항-PD-1 항체에 의한 항종양 효과가 T 세포 중의 활성 산소 발생과 관련이 있으며, 상기 항종양 효과가 저농도의 활성 산소 발생제의 첨가에 의한 세포내 신호 전달 및 미토콘드리아 활성화에 의해 증강된다는 보고가 있다. 게다가, 이러한 세포내 신호 전달을 모방하는 약제에서 또한 항-PD-1 항체의 항종양 효과가 증강된다는 것이 동시에 보고되어 있다(비특허문헌 3).
본 발명자들은 ALA 또는 그의 유도체가 암 세포 또는 암 세포가 거의 되어 가고 있는 핵에 이상을 형성한 세포의 미토콘드리아에서 헴 또는 시토크롬을 증강시켜, 전자전달계, TCA 사이클과 같은 미토콘드리아 활성을 향상시키고, 회복될 수 없는 핵에 이상이 있는 경우는 Bax 및 Bak 시스템을 불러일으켜 카스파제 IX-형의 아폽토시스를 야기하는 추정 메커니즘에 의해, 암 치료에 효과적이라는 것을 밝혀냈다(특허문헌 2).
다른 한편으로는, ALA은 임의의 시트르산제일철나트륨 (SFC)과 함께, 체내에서 헴이 되고, 이 헴이 헴 옥시게나제 1 (HO-1)이라는 효소에 의해 분해되어, 빌리루빈 및 일산화탄소로 변화한다 (비특허문헌 4). 이 빌리루빈 및 일산화탄소는 높은 항산화 작용을 갖고, 활성 산소 종(ROS)을 직접적/간접적으로 소거할 수 있는 것으로 공지되어 있다(비특허문헌 5).
게다가, ALA는, 임의의 SFC와 함께, 면역을 억제하는 면역 관용 효과(immunological tolerance effect)를 갖는 것으로 공지되어 있다. 이 메커니즘은 항원-제시 세포인 수지상 세포에 있어서, 상기-기재된 HO-1(또는 빌리루빈 및 일산화탄소)이 수지상 세포를 관용원성(tolerogenic) 수지상 세포라 칭해지는 세포로 변화(분화)시키는 것으로 생각된다. 관용원성 세포는 PD-L1을 고도로 발현하고 T 세포에 제시된 항원에 대해 특이적 면역 관용(면역억제)을 수행하는 것으로 공지되어 있으며, 실제로 ALA 및 SFC의 투여에 의해, 수지상 세포로 인식되는 세포 중에서 PD-L1의 mRNA의 상승이 관찰되었다(비특허문헌 6).

상기에 기재된 바와 같이, 항-PD-1 항체 또는 항-PD-L1 항체를 유효 성분으로서 포함하는 암 치료제가 창출되었으며, 추가로, 이들 암 치료제 및 기존의 약제와의 조합 치료법이 주목되고 있다. 실제로, 상기 신호 전달을 모방한 약제(전사 인자 PPAR을 활성화시키는 지질-강하 약물(lipid-lowering drug) 베자피브레이트)와 항-PD-1 항체의 조합에 대한 임상 시험이 계획되어 있다. 즉, 항-PD-1 항체 또는 항-PD-L1 항체와 같은 면역 체크포인트 억제제와 높은 상관관계를 갖고, 면역 체크포인트 억제제에 의한 항종양 효과를 증강시킬 수 있는 의약 또는 제약 조성물의 탐색 또는 창출이 요망되고 있다.
따라서, 본 발명의 목적은 항-PD-1 항체 또는 항-PD-L1 항체와 같은 면역 체크포인트 억제제에 의한 항종양 효과를 증강시키기 위한 제약 조성물을 제공하는 것이다.

본 발명에서, 아연 화합물은 염화아연, 산화아연, 질산아연, 탄산아연, 황산아연, 징크 디암모늄 디에틸렌트리아민펜타아세테이트, 징크 디소듐 에틸렌디아민테트라아세테이트, 징크 프로토포르피린, 및 아연-함유 효모를 포함할 수 있다.
대상체에 대한 금속-함유 화합물의 투여량은, 대상체에의 ALA의 투여량에 대한 몰비로 0 내지 100배, 바람직하게는 0.01 내지 10배, 보다 바람직하게는 0.1 내지 8배일 수 있다.
본 발명의 제약 조성물에 함유된 ALA 및 금속-함유 화합물은 ALA 및 금속-함유 화합물을 포함하는 조성물로서 투여될 수 있거나 각각 단독으로 투여될 수 있지만, 각각 단독으로 투여하는 경우에도 이들은 동시에 투여되는 것이 바람직하다. 여기서 동시는 동시에 투여하는 것뿐만 아니라, 동시에는 아닐지라도, ALA과 금속-함유 화합물의 투여가 상가적 효과, 바람직하게는 상승적 효과를 발휘할 수 있도록, 서로간에 실질적인 간격 없이 투여하는 것도 의미한다.

본 발명의 제약 조성물의 투여 대상체는 면역 체크포인트 억제제가 투여되거나 투여되고 있는 대상체, 전형적으로 암을 앓고 있는 대상체이다. 치료 대상이 되는 암의 유형은, 사용된 면역 체크포인트 억제제에 따라 변할 수 있다.
본 발명의 제약 조성물에 의해 투여될 ALA의 투여량은, 대상체의 신장, 체중, 연령 및 증상에 따라, 대상체 체중의 kg당, ALA로 환산(즉, 화학식 (I)에서 R1 및 R2가 수소 원자인 경우의 질량 환산)할 경우, 1 mg - 1,000 mg, 바람직하게는 5 mg - 100 mg, 보다 바람직하게는 10 mg - 30 mg, 추가로 바람직하게는 15 mg - 25 mg의 범위로 투여될 수 있다.

본 발명의 또 다른 실시양태에서, 본 발명의 제약 조성물의 대상체에의 투여는 면역 체크포인트 억제제의 투여와 동일한 날에 개시된다. 예를 들어, 본 발명의 제약 조성물은 면역 체크포인트 억제제의 투여가 개시된 날부터 매일 투여될 수 있다. 본 발명의 제약 조성물 및 면역 체크포인트 억제제가 동시에 투여되는 경우, 이들은 단일 제제로서 제조되고 투여될 수 있거나, 별개의 투여 경로로 동시에 투여될 수 있다.

실시예
<실시예 1> ALA와 PD-L1 항체의 병용에 의한 항종양 효과의 증강
실험 방법
C57BL/6 마우스에게, 3 x 105개의 마우스 흑색종 세포주 B16F10을 옆구리에 접종하고, 종양을 28일 동안 성장시켰다. 접종으로부터 10일째 및 15일째, 항-마우스 PD-L1 항체(클론: MIH5)를 PD-L1 항체 단독 투여 군, 항-PD-L1 항체 + ALA 투여 군, 및 항-PD-L1 항체 + ALA + SFC 투여 군에 대해 200 ㎍/헤드(head)로 복강내 투여하였다(도 1). 항-PD-L1 항체 + ALA + SFC 투여 군에 대해, 100 mg/kg의 ALA 히드로클로라이드 염(네오 알라 콤파니, 리미티드(neo ALA CO., LTD.)로부터) 및 157 mg/kg의 시트르산제일철나트륨(SFC; 코마쓰야 코포레이션(KOMATSUYA CORPORATION)으로부터)을 접종으로부터 10일째부터 16일째까지 매일 1회 경구 투여하였다. 또한, 항-PD-L1 항체 + ALA 투여 군에 대해, 100 mg/kg의 ALA 히드로클로라이드 염(네오 알라 콤파니, 리미티드로부터)을 접종으로부터 10일째부터 16일째까지 매일 1회 경구 투여하였다. 접종 후 무처리 군을 대조군으로서 두었다.
종양 직경을 2일마다 측정하고, v = (단축, mm)2 x (장축, mm)/2인 경우의 v를 종양 부피(mm3)로서 기록하였다. N의 값은 대조군에 대해 13마리의 마우스(16 일째 8마리의 마우스가 생존하였음), 항-PD-L1 항체 단독 군에 대해 10마리의 마우스(16일째 9마리의 마우스가 생존하였음), 항-PD-L1 항체 + ALA 투여 군에 대해 4마리의 마우스(16일째 모든 마우스가 생존하였음), 및 항-PD-L1 항체 + ALA + SFC 투여 군에 대해 9마리의 마우스(16일째 모든 마우스가 생존하였음)였다.
결과
접종으로부터 16일째, 항-PD-L1 항체 + ALA 투여 군은 항-PD-L1 항체 단독 군과 비교하여 종양 부피가 분명히 더 작았다(도 2). 게다가, 항-PD-L1 항체에 의한 효과는, ALA가 첨가되고 SFC가 추가로 투여된 경우 보다 현저하였다(항-PD-L1 항체 + ALA + SFC 투여 군). 이 결과는 항-PD-L1 항체의 항종양 효과가 ALA + SFC의 병용에 의해 증강된다는 것을 나타내고, 마우스 생존율의 추가 연장에 기여할 것으로 기대된다.
<실시예 2> ALA와 PD-1 항체의 병용에 의한 항종양 효과의 증강
실험 방법
C57BL/6 마우스에게, 3 x 105개의 마우스 흑색종 세포주 B16F10을 옆구리에 접종하고, 종양을 16일 동안 성장시켰다. 접종으로부터 8일째 및 12일째, 항-마우스 PD-1 항체(클론: RMP1-14)를 PD-1 항체 단독 투여 군, 항-PD-1 항체 + ALA 투여 군, 및 항-PD-1 항체 + ALA + SFC 투여 군에 대해 200 ㎍/헤드(head)로 복강내 투여하였다(도 3). 항-PD-1 항체 + ALA + SFC 투여 군에 대해, 100 mg/kg의 ALA 히드로클로라이드 염(네오 알라 콤파니, 리미티드로부터) 및 157 mg/kg의 시트르산제일철나트륨(SFC; 코마쓰야 코포레이션로부터)을 접종으로부터 8일째부터 16일째까지 매일 1회 경구 투여하였다. 접종 후 무처리 군을 대조군으로서 두었다.
종양 직경을 2일마다 측정하고, v = (단축, mm)2 x (장축, mm)/2인 경우의 v를 종양 부피(mm3)로서 기록하였다. N의 값은 대조군에 대해 18마리의 마우스(16 일째 10마리의 마우스가 생존하였음), 항-PD-1 항체 단독 군에 대해 10마리의 마우스(16일째 모든 마우스가 생존하였음), 및 항-PD-1 항체 + ALA + SFC 투여 군에 대해 10마리의 마우스(16일째 9마리의 마우스가 생존하였음)였다.
결과
접종으로부터 16일째, 항-PD-1 항체 + ALA + SFC 투여 군은 항-PD-1 항체 단독 군과 비교하여 종양 부피가 분명히 더 작았다(도 4). 이 결과는 항-PD-1 항체의 항종양 효과가 ALA + SFC의 병용에 의해 증강된다는 것을 나타내고, 마우스 생존율의 추가 연장에 기여할 것으로 기대된다.
산업상 이용 가능성
본 발명에 따르면, 면역 체크포인트 억제제에 의한 항종양 효과를 증강시키기 위한 제약 조성물이 제공된다. 따라서, 면역 체크포인트 억제제 및 본 발명에 따른 제약 조성물을 병용함으로써, 암 환자의 수명 연장 및 관해율의 증가를 야기할 것으로 기재된다.
게다가, 본 발명에 따른 제약 조성물의 유효 성분인 ALA는 동물, 식물, 및 진균 등에 널리 존재하는, 생체내 함유된 천연 아미노산의 일종이므로, 생채내에서 안전하게 사용 가능하다는 이점을 갖는다.
Claims (14)
- 제1항에 있어서, 상기 면역 체크포인트 억제제가 항-PD-1 항체, 항-PD-L1 항체, 항-CTLA4 항체, 항-B7 항체, 항-C27 항체, 항-KIR 항체, IDO 억제제, 항-CD137 항체, 및 항-TIM3 항체로 이루어진 군으로부터 선택된 적어도 1종인, 제약 조성물.
- 제2항에 있어서, 면역 체크포인트 억제제가 항-PD-L1 항체 또는 항-PD-1 항체임을 특징으로 하는, 제약 조성물.
- 제1항에 있어서, 상기 면역 체크포인트 억제제가 아테졸리주맙, 두르발루맙, 아벨루맙, 니볼루맙, 펨브롤리주맙, 피딜리주맙, BMS-936559, 이필리무맙, 트레멜리무맙, 에노블리투주맙, 바를리루맙, 리릴루맙, 에파카도스타트, 우토밀루맙, 우렐루맙, 및 TSR-022로 이루어진 군으로부터 선택된 것인 제약 조성물.
- 제1항 내지 제4항 중 어느 한 항에 있어서, 면역 체크포인트 억제제와 동시에 또는 상이한 시간대에 투여됨을 특징으로 하는, 제약 조성물.
- 제1항 내지 제5항 중 어느 한 항에 있어서,
R1이 수소 원자, 탄소수 1 내지 8개의 알카노일 기, 및 탄소수 7 내지 14개의 아로일 기로 이루어진 군으로부터 선택되고,
R2가 수소 원자, 탄소수 1 내지 8개의 선형 또는 분지형 알킬 기, 탄소수 3 내지 8개의 시클로알킬 기, 탄소수 6 내지 14개의 아릴 기, 및 탄소수 7 내지 15개의 아르알킬 기로 이루어진 군으로부터 선택된 것임
을 특징으로 하는, 제약 조성물. - 제1항 내지 제6항 중 어느 한 항에 있어서, R1 및 R2가 수소 원자인 제약 조성물.
- 제1항 내지 제7항 중 어느 한 항에 있어서, 1종, 또는 2종 이상의 금속-함유 화합물을 추가로 함유하는 제약 조성물.
- 제8항에 있어서, 금속-함유 화합물이 철, 마그네슘, 아연, 니켈, 바나듐, 구리, 크롬, 몰리브덴, 또는 코발트를 함유하는 화합물인 제약 조성물.
- 제9항에 있어서, 금속-함유 화합물이 철, 마그네슘, 또는 아연을 함유하는 화합물인 제약 조성물.
- 제10항에 있어서, 금속-함유 화합물이 철을 함유하는 화합물인 제약 조성물.
- 제11항에 있어서, 상기 철을 함유하는 화합물이 시트르산제일철나트륨(sodium ferrous citrate)인 제약 조성물.
