Abstract
본 발명은 키메라 항원 수용체 (CAR) T 세포 요법의 부작용을 치료하는 방법에 관한 것으로서, 상기 방법은 CAR T 세포 요법이 투여되었거나 또는 투여되고 있는 피험체에게 중간엽 줄기세포 (MSC)를 투여하는 단계를 포함한다.
Description
본 발명은 키메라 항원 수용체 (chimeric antigen receptor: CAR) T 세포 요법의 부작용을 치료하는 방법에 관한 것이다.
면역요법 (Immunotherapy)은 질병에 대항하는 피험체의 본래 면역계를 향상시키도록 설계된 생물학적 요법이다. 통상, 면역요법은 암 면역요법을 지칭한다.
암 면역요법의 개발 영역은 입양 세포 전달 (adoptive cell transfer: ACT)이고, 여기서 T 세포가 단리되고, 암 세포를 인식 및 공격하도록 조작되고, 그 다음에 증식시키고 암을 가진 피험체에게 재도입된다. 동종이형 (때로는, 동종이라 함) ACT에 대한 공여자 T 세포의 사용이 또한 연구되고 있기는 하지만, 이는 자가 ACT에 대해 피험체 자신의 T 세포를 단리 및 조작하는 것을 포함할 수 있다.
ACT에 있어서, T 세포 또는 NK 세포는 암 세포에서 발현된 항원을 인식하는 수용체를 발현하도록 조작되었다. 상기 수용체는 T 세포 수용체 (TCR) 또는 키메라 항원 수용체 (CAR)일 수 있다. CAR를 발현하는 T 세포를 CAR T 세포라고 한다.
현재까지, ACT 및 CAR T 세포 요법은 주로 혈액암에서 사용되어 왔고, 소규모 임상 시험으로 제한되었으며, 고형암에 대한 시험은 매우 제한적이었다. 그럼에도 불구하고, CAR T 세포 요법은 진행성 암을 갖는 피험체에서 인상적인 반응을 보여주었다.
통상적으로 B 세포 상에 존재하는 세포 표면 항원인 CD19에 대한 CAR로 CAR T 세포 요법의 치료 대상이 되는 암으로는 급성 림프모구 백혈병 (acute lymphoblastic leukaemia: ALL), 만성 림프구 백혈병 (chronic lymphocytic leukaemia: CLL), 광범위 큰 B 세포 림프종 (diffuse large B cell lymphoma: DLBCL) 및 소포 림프종 (follicular lymphoma)을 포함하는 일부 타입의 비-호지킨 림프종 (non-Hodgkin lymphoma: NHL) 및 다발 골수종 (multiple myeloma)을 포함한다.
이러한 유망함에도 불구하고, CAR T 세포 요법은 부작용 및 상당한 위험이 없는 것은 아니다. 관찰된 부작용으로는 마크로파지 활성화 증후군 (macrophage activation syndrome: MAS)과 관련된 시토킨 방출 증후군 (cytokine release syndrome: CRS), 이식편대 숙주 질환 (graft-versus-host disease: GVHD) 및 B 세포 무형성 (aplasia)과 유사한 결과를 유도하는 on-표적 off-암 효과 (on-target, off-cancer effect), 종양 용해 증후군 (tumour lysis syndrome: TLS), 대뇌 부종 (cerebral oedema)과 같은 신경독성 (neurotoxicity), 및 비-인간 항원을 포함하는 CAR에 대한 피험체의 IgG 반응에 의해 유발되는 아나필락시스 (anaphylaxis)를 포함한다.
CRS는 스테로이드를 포함하는 표준 지지 요법으로 치료되었다. 그러나, 스테로이드는 피험체에서 CAR T 세포의 활성 또는 증식에 영향을 줄 수 있다. CRS에 대한 다른 요법으로는 CRS에서 증가되는 염증촉진성 (pro-inflammatory) 시토킨의 저해제를 투여하는 것이다. 항-IL-6 수용체 항체인 토실리주맙 (Tocilizumab) 및 TNF 저해제인 에타네르셉트 (etanercept)가 CRS를 치료하기 위해 사용되었다.
감소된 항체 생성을 유도하는 B 세포 무형성은 감염을 방지하기 위해 정맥내 면역글로불린으로 치료되었다.
TLS는 수분공급 (hydration), 이뇨 (diuresis), 알로푸리놀 (allopurinol) 및 재조합 유레이트 옥시다제 (recombinant urate oxidase)의 투여, 및 필요에 따라 혈액투석을 포함하는 표준 지지 요법에 의해 관리되었다.
이러한 부작용은 다양한 수준의 성공으로 관리되었지만, 이는 규칙적으로 발생하는 유해 사례, 심지어 다수의 임상 시험에서 발생한 피험체의 사망으로 완전히 성공적이지는 않았다.
분명하게 ACT, 구체적으로 CAR T 세포 요법의 부작용에 대한 예방 및/또는 요법의 개선이 요구된다.

부작용 및 증상
시토킨-방출 증후군 (Cytokine-release syndrome: CRS)은 CAR T 세포 요법의 중증 부작용이다. CRS는 인터류킨-6 (IL-6), 인터페론-γ (IFN-γ), 종양 괴사 인자 (TNF), IL-2, IL-2-수용체 α, IL-8, IL-10 및 과립구 마크로파지 콜로니-자극 인자 (GMCSF)를 포함하는 시토킨을 다량 방출하는 증식하는 T 세포로부터 기인하는 것으로 사료된다.

2. 1차 인간 T 세포의 배양 및 레트로바이러스 형질도입
건강한 공여자로부터의 말초혈 단핵 세포는 RPMI+10% (vol/vol) 인간 혈청에서 수립되고, 피토헤마글루티닌 (phytohemagglutinin) (2 μg/ml)로 2일 동안 활성화되고, 레트로넥틴 (retronectin) (15 μg/ml; Takara Biomedicals, Shiga, Japan)으로 사전코팅된 비-조직 배양 플레이트 (non-tissue culture plates) (FALCON, Becton Dickinson, Franklin Lakes, N.J.)로 옮겼다. CAR 구조체를 포함하는 긴팔원숭이 (Gibbon ape) 백혈병 바이러스 외피-의사 레트로바이러스 입자 (leukemia virus envelope-pseudotyped retroviral particles)는 (Gallardo, H. F. et al. Blood 90, 952-957 (1997) 및 Riviere, I. et al. Mol . Biotechnol . 15, 133-142 (2000))에 기재된 바와 같이 생성되었다.
미토겐 (mitogen)-활성화 PBL의 형질도입 3일 후에, 유전자 전달 효율은 20% 내지 70% 범위였다. CD4+ 및 CD8+ T-세포 서브세트가 유사한 효율로 형질도입되었다. ζ-사슬 함유 융합 수용체의 발현은 또한 웨스턴 블로팅 (Western blotting)에 의해 분석하고, 동종이량체 형성을 확인하고, 내인성 CD8 또는 CD28에 의한 이종이량체화가 거의 없음을 확인하였다. 3. CD19-특이적
scFv
CD19-특이적 scFv를 제작하기 위해, 중쇄 (VH) 및 경쇄 (VL) 가변 영역은 (Orlandi et. al. 상기 19-28z 융합은 NALM6 T 세포가 주입된 마우스에서 종양 성장을 감소시키고 생존을 증가시키는 그의 능력에 대해 테스트하였다. 6. 인간
PSC를
MSC로
분화시키기 위한 프로토콜
1. 비트로넥틴 코팅된 (0.5 μg/ckg) 플라스틱 웨어 (plastic ware) 상에, E8 완전 배지 (DMEM / F12 기본 배지 + E8 보충제) + 1 μM H1152에서 iPSC를 해동시켰다. 도말된 iPSC를 37 ℃, 5% CO2, 20% O2 (정상 산소)에서 인큐베이션하였다.
2. 비트로넥틴 코팅된 (0.5 μg/ckg) 플라스틱 웨어 상에, E8 완전 배지 (ROCK 저해제 없음)에서 iPSC를 3회 계대로 증식하고, 37 ℃, 5% CO2, 20% O2 (정상 산소)에서 인큐베이션하고, 그 다음에 분화 과정을 개시하였다.

iPSC-MSC의 T 세포 억제에 있어서 PDGF-BB (10 ng/mL) 및/또는 LiCl (1 mM)을 M-CFM에 보충하는 영향을 조사하기 위해 2개의 실험 (TC-A-96 및 DAD-V-90)을 수행하였다. T 세포 억제는 Waisman Biomanufacturing의 면역역가 분석 (IPA)을 사용하여 평가하였다.
하기 표 7에 요약된 바와 같이, 혈소판-유래 성장 인자 (PDGF) 및 LiCl의 하기 조합을 평가하였다: PDGF+/LiCl+, PDGF-/LiCl-, PDGF+/LiCl- 및 PDGF-/LiCl+. 2개의 상이한 Dneg1 시드 밀도 (5x103 세포/ckg 및 1x104 세포/ckg) 및 분화 배지 중 2개의 상이한 악티빈 A (activin A: AA)의 농도 (1X AA = 15ng/mL 및 0.1X AA = 1.5ng/mL)가 TC-A-96 실험에서 비교되었다. 단일 Dneg1 시드 밀도 (5x10e3 세포/ckg) 및 악티빈 A 농도 (1.5 ng/mL)가 DAD-V-90 실험에서 사용되었다. 또한, 단일 leukopak (LPK7)은 제1 IPA (IPA 1)에서 사용되고, 2개의 leukopak (LPK7 및 LPK8)은 제2 IPA (IPA 2)에서 사용되었다.
상기 분석은 각 MSC 계통이 T 헬퍼 (CD4+) 림프구의 증식을 억제할 수 있는 정도를 평가하기 위해 설계되었다. 냉동보존된 MSC는 건강한 개체의 말초혈 (Leucopaks (LPK) 유래의 말초혈 단핵구 세포 (PBMC))로부터 정제된 냉동보존된 백혈구를 사용하여 시험되었다. 따라서, 공여자에서 공여자로 LPK 세포 집단 변이가 예상된다. 각 MSC 시험 시료는 임상 등급 물질에 대해 2명의 상이한 개인으로부터의 PMBC에 대해, 연구 등급 물질에 대한 단일 PMBC 시료로 시험을 제한하는 옵션을 사용하여 시험되었다. 각 MSC 시험 시료에 대한 분석은 참조 기준 MSC 계통과 함께 수행하여 분석 무결성/재현성을 보장하고 시험 시료를 정규화하였다. 상기 분석은 Bloom et al. 요약하면, 시험 MSC를 21 Gy의 감마선 조사에 노출시켰다. 48-웰 조직 배양 플레이트에서, 4x10e5, 2x10e5, 4x10e4 및 2x10e4 조사된 MSC를 개별 웰로 플레이트하였다. PMBC는 카르복시플루오레세인 숙신이미딜 에스테르로 개별적으로 표지하였다. 표지된 PMBC 세포를 상기 MSC 함유 웰에 대해 4x105 세포로 플레이트하였다. 이는 1:1, 1:0.5, 1:0.1 및 1:0.05의 적정된 PBMC:MSC 비율을 초래하였다. 추가의 웰에 자극된 PBMC 단독, 다른 것에 MSC 단독, 및 또 다른 것에는 자극 없이 1:0.05 비율로 플레이트하고, 이들 모두는 대조군으로 제공되었다. 후속하여, T 세포-자극 모노클로날 항체, 항-인간 CD3-엡실론 및 항-인간 CD28 (R&D Systems, Inc., Minneapolis, MN)을 각 웰에 첨가하였다.

방출 증후군의 예방 또는 치료

0일 차에, 8- 내지 12-주령 암컷 NOD.Cg-Prkdc scid Il2rg tm1Wjl /SzJ (NSG) 마우스에게 20x106 인간 PBMC(huPBMC)를 꼬리 정맥을 통해 정맥내로 주사하였다. 실험 설계의 개략도는 도 3에 제공되었다.
동결된 인간 PBMC를 StemCell Technologies로부터 구입하고, NSG 마우스는 The Jackson Laboratory로부터 구입하였다.
동결된 huPBMC 시료를 저장하고, 제조자 지침에 따라 해동하였다. 간략하게, 동결된 세포의 바이알을 37℃ 수조에서 해동하고, 상기 바이알의 외부를 70% 에탄올로 세척하고, 상기 세포를 37℃로 예열된 RPMI 10% FBS 10 mL 함유 15mL 원추형 튜브로 옮기고, 1500 rpm으로 10분 동안 원심분리하고, 10 mL의 PBS로 한번 세척하고, Trypan Blue dye exclusion에 의한 세포 계수를 위해 1 mL의 PBS에 현탁하였다. 150 μl의 PBS 중 20x106 huPBMC 알리코트 (aliquots)를 제조하고, 얼음에서 유지하면서 주사를 위해 마우스를 준비하였다.

OKT3 항체를 투여하고 5시간 후에, 무균 4 mm Goldenrod Animal 란셋을 사용한 볼 정맥 천자 (cheek vein puncture)를 통해서 또는 꼬리 정맥으로부터 혈액을 채취하여 말초혈 시료를 수득하였다.
마우스를 임상 점수 및 체온을 고려하여, OKT3 투여하고 5 또는 24시간 후에 희생시켰다. 말초혈 시료는 심장 천자를 통해 안락사 직후에 입수하고, 그 다음에 비장을 수집하였다. 순환계 및 비장에서 발견되는 인간 CD45, CD4 및 CD8 T 세포 퍼센트는 표준 유세포 측정 염색 및 분석에 의해 결정되었다. 순환계 및 비장 CD4 및 CD8 T 세포에서 CD69 발현은 표면 염색 및 유세포 측정 분석에 의해 평가되었다.
OKT3 투여하고 5 및 24시간에 수집된 혈장 시료는 IL-1β, IL-2, IL-6, IFNγ, TNF, IL-10, 및 선택적으로 IL-4 및 IL-5의 발현에 대해 평가될 것이다.
본 CRS 모델에서, 시험 마우스는 대조군 마우스와 비교하여 더 높은 직장 온도를 나타내었다 (도 4). 또한, 시험 마우스는 본 CRS 모델에서 시험 마우스와 비교하여 감소된 임상 점수를 나타내었다 (도 5).
말초혈 (도 6) 또는 비장 (도 9)에서 CD45+ 세포 퍼센트에 있어서 대조군과 시험 마우스 사이에 차이는 관찰되지 않았다.
그러나, CD69 발현은, 말초혈 (도 7) 및 비장 (도 10) 중에 인간 CD4+ 세포에 있어서 및 말초혈 (도 8) 및 비장 (도 11) 중에 인간 CD8+ 세포에 있어서 대조군 마우스와 비교하여 시험 마우스에서 감소되었다.
대조군 마우스에 비해 시험 마우스의 인간 CD4+ 세포 및 인간 CD8+ 세포에서 감소된 CD69 발현의 관점에서, IL-1β, IL-2, IL-6, IFNγ, TNF, IL-10, IL-4 및 IL-5 중 하나 이상의 발현은 대조군 마우스에 비해 시험 마우스에서 감소되는 것으로 기대된다.

- 청구항 1 내지 9 중 어느 한 항에 있어서, 상기 CAR T 세포 요법은 하기를 치료하기 위한 것인 방법 또는 용도: 광범위 큰 B 세포 림프종 (diffuse large B cell lymphoma: DLBCL); 비-호지킨 림프종 (non-Hodgkin lymphoma: NHL); 원발성 종격동 B 세포 림프종 (primary mediastinal B cell lymphoma: PMBCL); 만성 림프구 백혈병 (chronic lymphocytic leukaemia: CLL); 형질전환된 소포 림프종 (transformed follicular lymphoma: TFL); 다발 골수종 (multiple myeloma: MM); 외투 세포 림프종 (mantle cell lymphoma: MCL); 급성 골수성 백혈병 (acute myeloid leukaemia: AML); 또는 급성 림프모구 백혈병 (acute lymphoblastic leukaemia: ALL).
- 청구항 1 내지 10 중 어느 한 항에 있어서, 상기 CAR는 항-CD19 CAR인 것인 방법 또는 용도.
- 청구항 1 내지 11 중 어느 한 항에 있어서, 상기 피험체는 포유동물, 선택적으로 인간인 것인 방법 또는 용도.
- 포유동물 피험체에서 CAR T 세포 요법의 부작용을 치료, 개선 또는 감소시키기 위한 치료적 조성물로서, 상기 치료적 조성물은 중간엽 줄기세포 (MSC)를 포함하고, 상기 MSC는 하기 단계를 포함하는 방법에 의해 제조되며:
(a) LiCl 및 FGF2를 포함하지만, PDGF는 배제된 중간엽-콜로니 형성 배지 (M-CFM)에서 원시 중배엽 세포를, 중간엽 콜로니가 형성되기에 충분한 시간 동안 정상산소군 (normoxic) 조건하에 배양하는 단계; 및
(b) (a)의 중간엽 콜로니를 부착 배양하여 MSC를 생성하는 단계,
상기 (b)의 MSC는 miR-145-5p, miR-181b-5p 및 miR-214-3p를 발현하지만, miR-127-3p 및 miR-299-5p는 발현하지 않고, 및/또는 표현형 CD73+CD105+CD90+CD146+CD44+CD10+CD31-CD45-을 갖는 것인 치료적 조성물. - 청구항 13의 치료적 조성물을 포함하는 용기 (container).
