Abstract

Description
본 발명은 일반적으로 항암 요법의 분야에 관한 것이다. 보다 구체적으로, 본 발명은 백혈병 및 고형 종양과 같은 암을 치료하는데 사용하기 위한 유기 비소 화합물 및 방법을 제공한다.
백혈병 치료법의 진보에도 불구하고, 대부분의 성인 백혈병 환자는 여전히 질환의 진행으로 인해 사망한다. 무기 화합물인 삼산화비소는 재발성 또는 불응성 급성 전골수구성 백혈병 (APL) 환자의 치료법으로 승인되었고, 다른 백혈병 유형의 치료법으로서 평가중이다. 그러나, 중국으로부터의 예비 자료 및 미국에서의 최근의 경험에 의하면, 다른 혈액암에서도 삼산화비소의 역할이 시사되었다. 따라서, 항백혈병제로서의 삼산화비소의 활성은 현재 수많은 유형의 백혈병에서 조사되고 있다. 그 결과는 조사중인 몇몇 백혈병 유형의 반응률의 측면에서 유리한 것으로 보이지만, 삼산화비소의 전신성 독성이 문제이다 (소이그네트(Soignet) 등의 문헌, 1999; 위어니크(Wiernik) 등의 문헌, 1999; 가이슬러(Geissler) 등의 문헌, 1999; 루셀로트(Rousselot) 등의 문헌, 1999).

무기 비소제의 독성 대 유기 비소제의 독성
삼산화비소의 사용은 그의 독성에 의해 제한된다. 반면에, OA는 OA 내로의 생체내 무기 비소의 메틸화가 해독 반응으로 간주될 정도로 훨씬 덜 독성을 갖는다. 암 치료
본 발명의 유기 비소제를 사용하여, 백혈병, 림프종, 다발성 골수종, 골수이형성증 또는 골수증식 장애를 비롯한 모든 고형 종양 및 모든 혈액암을 포함한 다양한 암을 치료할 수 있다. 또한 다른 형태의 치료에 대하여 불응성으로 된 혈액암을 치료하는데에도 유기 비소제를 사용할 수 있다.
백혈병은 백혈구의 비정상적인 증식을 특징으로 하는 조혈 조직의 악성 신생물이고, 4가지의 주요 암 유형 중 하나이다. 백혈병은 가장 두드러지게 관련된 백혈구의 유형에 따라 분류된다. 급성 백혈병은 주로 미분화된 세포 집단이고, 만성 백혈병은 보다 성숙한 세포 형태를 갖는다 (WO 9924029호).
급성 백혈병은 림프모구성 (ALL) 및 비림프모구성 (ANLL)으로 나누어지고, FAB 분류(French-American-British classification)에 따라 또는 이들의 분화 유형 및 정도에 따라 형태학적 및 세포화학적 외관으로 추가로 세분될 수 있다. 골수 세포 표면 표지/항원은 물론, 특이적 B 세포 및 T 세포 또한 분류에 사용된다. ALL은 주로 소아기 질환이지만, 급성 골수성 백혈병으로도 알려져 있는 ANLL은 성인들 사이에 더 흔한 급성 백혈병이다.

특정 실시양태에서, 주사 조성물의 장기간 흡수는 조성물에 흡수 지연제, 예를 들면 알루미늄 모노스테아레이트, 젤라틴 또는 이들의 조합을 사용함으로써 일어날 수 있다.
병용 요법

화학요법
암 치료법은 또한 화학 및 방사선 기반 치료법 둘다와의 다양한 병용 요법을 포함한다. 병용 화학요법의 예로는 시스플라틴 (CDDP), 카르보플라틴, 프로카르바진, 메클로레트아민, 시클로포스프아미드, 캄프토테신, 이포스프아미드, 멜팔란, 클로르암부실, 부술판, 니트로스우레아, 닥티노마이신, 다우노루비신, 독소루비신, 블레오마이신, 플리코마이신, 미토마이신, 에토포시드 (VP16), 타목시펜, 랄옥시펜, 에스테르젠 수용체 결합제, 탁솔, 젬시타빈, 나벨빈, 파르네실-단백질 트랜스페라제 저해제, 트랜스플라티눔, 5-플루오로우라실, 빈크리스틴, 빈블라스틴 및 메토트렉세이트, 또는 전술한 것들의 임의의 유사체 또는 유도체 변형물이 있다.
방사선요법
DNA 손상을 일으키고 광범위하게 사용되어 온 그 밖의 다른 요인으로는 흔히 γ선, X선으로서 알려진 것들 및/또는 방사성 동위원소의 종양 세포로의 유도 전달이 있다. 극초단파 및 자외선 조사와 같은 DNA 손상 요인의 다른 형태도 또한 고려된다. 이들 요인이 모두 DNA, DNA의 전구체, DNA의 복제 및 복구, 및 염색체의 조립 및 유지에 대하여 광범위한 손상을 일으키는 것 같다. X선의 조사량 범위는 장기간 (3 내지 4주) 동안의 50 내지 200 뢴트겐의 1일 조사량부터 2000 내지 6000 뢴트겐의 단일 조사량 범위에 이른다. 방사성 동위원소의 조사량 범위는 광범위하게 변하고, 동위원소의 반감기, 방출되는 방사선의 강도 및 유형, 및 신생물 세포에 의한 흡수에 따라 좌우된다. "접촉" 및 "노출"이란 용어는, 세포에 적용되는 경우, 본원에서 치료적 구성물 및 화학요법제 또는 방사선요법제가 표적 세포에 전달되거나 또는 표적 세포와 병치되는 방법을 기술하는데 사용된다. 세포 사멸 또는 정지를 달성하기 위해, 상기 두 제제는 세포를 사멸시키거나 또는 세포 분열을 방지하는 조합된 유효량으로 세포에 전달된다.
면역요법
면역요법제는 일반적으로 암 세포를 표적으로 삼아 파괴하기 위한 면역 작동 세포 및 분자의 사용에 의존한다. 면역 작동제는, 예를 들어 종양 세포 표면의 몇몇 표지에 대해 특이적인 항체일 수 있다. 항체는 단독으로 치료법의 작동제로서 작용할 수 있거나 또는 세포 사멸을 실제적으로 행하기 위하여 다른 세포를 보충할 수 있다. 항체는 또한 약물 또는 독소 (화학요법제, 방사성뉴클레오티드, 리신 A쇄, 콜레라 독소, 백일해 독소 등)에 콘쥬게이션되어 단지 표적화제로서 작용할 수도 있다. 또 다르게는, 작동제는 종양 세포 표적과 직접적으로 또는 간접적으로 상호작용하는 표면 분자를 갖는 림프구일 수 있다. 다양한 작동 세포로는 세포독성 T 세포 및 NK 세포가 있다.
따라서, 면역요법은 병용 요법의 일부로서 유전자 요법과 함께 사용될 수 있다. 병용 요법에 대한 개괄적인 접근법이 이하에 논의되어 있다. 일반적으로, 종양 세포는 표적화하기 쉬운, 즉 대부분의 다른 세포에는 존재하지 않는 몇몇 표지를 가져야 한다. 수많은 종양 표지가 존재하며, 이들 중 어느 것이라도 본 발명과 관련하여 표적화하기에 적합할 수 있다. 일반적인 종양 표지로는 암배아항원, 전립선 특이 항원, 비뇨기 종양 관련 항원, 태아 항원, 티로시나제 (p97), gp68, TAG-72, HMFG, 시알릴 루이스(Sialyl Lewis) 항원, MucA, MucB, PLAP, 에스트로젠 수용체, 라미닌 수용체, erb B 및 p155가 있다.
유전자 요법
또다른 실시양태에서, 2차 치료는 1차 치료제 전에, 그 후에 또는 그와 동시에 치료적 폴리뉴클레오티드를 투여하는 2차 유전자 요법이다. 유전자 산물을 코딩하는 백터와 함께 치료제의 전달은 표적 조직에 대하여 조합된 항-과증식성 효과를 나타낼 것이다.
수술
암 환자의 대략 60%는 예방적, 진단적 또는 병기결정(staging), 치유적 및 완화적 수술을 포함하는 몇몇 유형의 수술을 받을 것이다. 치유적 수술은 다른 치료법, 예를 들어 본 발명의 치료법, 화학요법, 방사선요법, 호르몬 요법, 유전자 요법, 면역요법 및/또는 대안의 치료법과 함께 사용될 수 있는 암 치료법이다. 치유적 수술은 암성 조직의 전부 또는 일부분을 물리적으로 제거, 절개 및/또는 파괴하는 절제술을 포함한다. 종양 절제란 종양의 적어도 일부를 물리적으로 제거함을 가리킨다. 종양 절제 외에, 수술에 의한 치료법으로는 레이저 수술, 냉동 수술, 전기 수술 및 현미경 제어 수술(모스(Mohs') 수술)이 있다. 또한 본 발명은 표재성 암, 전암, 또는 부수적인 양의 정상 조직의 제거와 함께 사용될 수 있는 것으로 생각된다.
실시예
하기 실시예는 본 발명의 바람직한 실시양태를 나타내기 위하여 포함된다. 당업자라면 하기 실시예에 기재된 기법들이 본 발명의 실시에서 잘 기능하도록 본 발명자에 의해 개발된 기법을 대표하고 따라서 본 발명의 실시에 바람직한 양식을 구성하는 것으로 생각될 수 있음을 알 것이다. 그러나, 당업자는 본 명세서에 비추어, 기재되어 있는 특정 실시양태에서 다수의 변화가 이루어질 수 있고, 본 발명의 취지 및 범주를 벗어남이 없이 비슷하거나 유사한 결과를 얻음을 알 것이다.
실시예 1
16-(디메틸아르시노)티오헥사데칸산의 합성
디클로로메탄 (50 mL) 중의 16-머캅토헥사데칸산 (0.002 mol, 0.576 g)의 용액을 디메틸클로로아르신 (0.0033 mol, .046 g)으로 적가 처리한 후에 트리에틸아민 (0.4 g)을 적가하였다. 혼합물을 교반하고 4시간 이상 동안 환류시키며, 그 동안 혼합물을 물로 2회 세척하고 황산나트륨 상에서 건조시켰다. 용매를 감압하에 제거하고 잔류물을 에틸 아세테이트 및 석유 에테르 (1:10 v/v)로 용출하는 컬럼 크로마토그래피로 정제하여 목적 생성물을 제조하였다 (IC10 < 1 μM, IC50 < 1.5 μM, IC90 < 2.5 μM, DOX의 억제율(%) (1 μM) = 100%).
실시예 2
11-(디메틸아르시노)티오운데칸산의 합성
11-(디메틸아르시노)티오운데칸산을 16-(디메틸아르시노)티오헥사데칸산의 합성에 대해 상기 기재한 절차를 사용하여 제조하였다 (IC10 < 1 μM, IC50 < 1.5 μM, IC90 < 2.5 μM, DOX의 억제율(%) (1 μM) = 100%).
실시예 3
생물학적 분석법
세포
HL-60 세포주를 아메리칸 타입 컬쳐 콜렉션 (ATCC; 미국 메릴랜드주 록빌 소재)로부터 입수하여, 20% 우 태 혈청 (FBS; 애틀란타 바이올로지컬즈(Atlanta Biologicals); 미국 조지아주 로렌스빌 소재) 및 2 mM L-글루타민 (메디아테크(Mediatech); 미국 버지니아주 헌든 소재)을 보충한 이스코브 변형된 둘베코 배지 (IMDM; ATCC), 또는 "완전 배지" 중에 37℃에서 5% CO2 가습 인큐베이터에서 생장시켰다.
시험 화합물
모든 화합물 (A-F)을 이리시스, 인크. (IriSys, Inc.; 미국 캘리포니아주 샌디에고 소재)로부터 동결건조된 분말로서 수령하였다. 이들을 DMSO 중에 10 mM로 가용화하고 완전 배지 중에서 희석한 후에 DMSO를 함유하는 IMDM에서 추가로 희석하여 최종 DMSO 농도가 모든 배양물에서 0.2%이도록 하였다. 공지된 세포독성 화합물 독소루비신 (시그마 케미컬 컴퍼니(Sigma Chemical Co.); 미국 미주리주 세인트 루이스 소재)을 양성 대조군으로서 사용하였다.
세포독성 분석
분석을 위해 편평한 바닥의 96-웰 마이크로타이터 조직 배양 플레이트 (코스타(Costar); 미국 매사추세츠주 캠브리지 소재)를 사용하였다. 배양을 2벌의 8개 웰에서 수행하고; 각각의 웰은 1 x 104개의 세포를 함유하였다. 세포를 플레이팅한지 1일 후에, 웰에 배지 또는 시험 화합물을 투여하였다. 각 웰에서의 최종 부피는 200 ㎕이었다. 각 화합물을 한 플레이트 상에서 시험하고; 독소루비신을 각각의 플레이트 상에서 양성 대조군으로서 사용하였다. 웰은 완전 배지만 함유하거나 (세포 없음; 백그라운드), 또는 세포와 배지 (대조군의 100%), 1 μM 내지 78.1 nM (2배 희석으로서 수행됨)을 함유하였다. 플레이트를 3일 동안 37℃에서 가습 5% CO2 인큐베이터에 두었다. 웰에 1 μCi의 삼중수소 티미딘 (3H-TdR)을 최종 18시간의 인큐베이션 동안 더하고, 브란델(Brandel) 96-웰 세포 수거기 (미국 메릴랜드주 게이더스버그 소재)에서 플레이트를 수거한 다음 왈락 1450 마이크로베타 트리룩스 신틸레이션 카운터 (Wallac 1450 Microbeta TriLux scintillation counter) (핀란드 투르쿠 소재)의 필터 메이트(filter mate)에서 방사성을 측정함으로써, 3H-TdR의 흡수에 의해 증식을 측정하였다. 시험 화합물로 처리한 세포의 결과를 대조군 세포 (완전 배지만으로 처리한 세포; 대조군의 100%)와 비교하여 시험 화합물의 효과를 측정하였다.
