Abstract
치료제의 국소적 전달을 위한 방법 및 장치는 허혈에 의한 심근조직의 손상을 감소시킨다. 국소적 전달 장치는 허혈성 심근 조직에 양분을 공급하는 관상동맥에 치료제를 전달하기 위해 사용된다. 일 실시예에 따르면, 심근 조직에 국소적으로 인슐린을 전달하기 위한 이식 가능한 의료 장치는 스텐트에 고정된 생체 적합성 폴리머 중에 치료학적 투여량의 인슐린을 포함한다. 치료학적 투여량의 인슐린은 심근조직의 허혈성 상해를 감소시키는데 효과적인 투여 기간에 걸쳐 치료학적 투여량만큼 스텐트로부터 방출된다.

혈관상(vascular bed)으로의 혈류의 감소 또는 중지는 위태로운 기관 또는 조직에 대한 즉각적인 개입 및 적절한 관류가 요구되는 즉각적인 다양한 임상적 조치를 필요로 한다. 서로 다른 조직은 서로 다른 정도로 허혈성 상해의 정도를 견디어 낼 수 있다. 그러나, 조직은 재관류되지 않을 경우 비가역적인 조직 손상 및 세포 괴사로 진행될 수 있다.
심장 조직의 관류 부전(허혈)은 조직에서 산소 및 에너지가 고갈되기 때문에 적절히 작용하는 심장의 능력의 손실을 일으킨다. 영구적인 손상은 직접적으로 심장근육이 겪게 되는 산소 결핍 지속시간과 관련이 있다. 허혈은 세포 부위로의 혈류가 정상적인 대사 활성을 지탱할 정도로 충분하지 않을 경우 일어난다. 급성 심근경색(MI) 후의 외과적 그리고 경피 혈관재형성 기술은 허혈성 심근 조직을 치료하는데 매우 효과적이다. 급성 MI의 경우에, 주요 혈류는 관상 동맥의 차단에 의해 멈춰지며, 조직은 오직 측부동맥을 통해서 관류된다. 재관류는 허혈성 조직으로의 혈류 또는 산소 공급을 다시 안정화하는 것을 행위를 기술하는데 사용되는 용어이다. 재관류는 일반적으로, 관상동맥 혈관성형술, 혈전용해제의 투여, 관상동맥 우회로 수술(bypass surgery) 등을 포함하는 혈류 혈관재형성 기술에 의해 이루어질 수 있다.
허혈성 심근을 적절한 시기에 재관류함으로써 경색 크기를 제한할 수 있으며, 혈관성형술 또는 혈전용해 치료를 이용한 이른 재관류는 심근 손상이 감소되는 이점을 제공하고, 심실의 기능을 향상시키며, 급성 MI를 갖는 환자의 치사율을 감소시킨다. 그러나, 심근 구제 치료는 관상동맥 재폐색 및 심각한 잔여 관상동맥 협착증과 같은 부작용에 의해 효과가 없게 될 수도 있다.
허혈성 심근의 재관류는 그 단독으로 심근의 기능을 총분히 회복시키지 않는다. 실제로, 재관류 그 자체는 허혈 상황에서 살아남은 많은 세포에 손상을 일으킬 수 있다는 것이 잘 알려져 있다. 연구결과 재관류는 비가역적으로 손상된 심근의 파괴를 가속화할 수 있으며, 위태롭지만 아직 생존해 있는, 재관류에 의해 구조된 심장 세포의 생존을 위협할 수도 있는 것으로 나타났다. 이러한 소위 재관류 손상은 최종적인 경색 크기의 50% 이상을 나타낼 수 있다. 수 많은 세포 기전이 허혈-유도 재관류 손상에 책임이 있는 것으로 여겨진다. 따라서, 허혈 이후의 심근을 보호하고 관상동맥 재관류의 이점을 최대화하기 위한 아주반트(adjuvant) 치료의 개발은 현대 심혈관 연구의 주요 타겟이 되어 왔다.
허혈 또는 재관류 손상을 최소화할 수 있는 화합물은 중요한 치료제이다. 지난 수년간, 심근 허혈 및 재관류 이후의 치사율은 허혈 후 심장조직으로의 에너지 전달을 최적화하는 약물의 전달에 의해 더욱 향상될 수 있다는 것이 입증되었다. 예를 들어, 급성 심근경색 및 재관류 이후에 글루코오스, 인슐린, 및 칼륨(GIK)의 조합을 동맥 주입하는 것은 손상되었지만 살아 있는 심근 조직에 대한 효과를 제공하는 것으로 나타났으며 치사율을 감소시키는 것으로 나타났다.
GIK의 동맥 주입에 의해 발생되는 높은 수준의 인슐린은 관상 동맥의 혈관 이완을 향상시킬 뿐만 아니라, 허혈 및 허혈 후의 심근 수축 및 이완 작용을 향상시키는 것으로 나타났다. 인슐린의 제공은 또한 심근의 글리코겐 저장을 보존 및회복시킨다. GIK는 또한 동맥 유리 지방산(FFAs) 순환 수준 및 심근 FFA 흡수를 감소시킨다. 높은 FFA 수준은 허혈성 심근에 대해 독성이 있으며 막 손상의 증가, 부정맥, 및 심근 기능의 감소와 관련이 있다. 따라서, 인슐린이 허혈성 손상을 줄일 수 있는 많은 기전이 있다. 그러나, 인슐린을 동맥 주입에 의해 전신 투여할 경우, 인슐린은 신체 전체에서의 글루코오스 및 칼륨 흡수를 촉진시켜 저혈당 및 저칼륨혈증을 일으킬 수 있어 안전하지 않은 수준으로 혈액에서의 글루코오스 및 칼륨의 수준을 감소시킨다. 따라서, GIK 치료는 전신 인슐린 투여의 바람직하지 않은 전신 부작용을 약화시키기 위해 글루코오스 및 칼륨을 함께 투여하며, 글루코스 및 칼륨의 수준을 주의깊게 모니터링하는 것을 필요로 한다.

본 발명의 또 다른 측면에 따르면, 심근 경색 및 재관류 이후의 조직 손상을 감소시키기 위해 인슐린을 심근 조직에 전달하는 방법은
혈관 내의 폐색(occlusion) 부위를 확인하는 단계;
폐색 부위를 치료하여 재관류가 이루어지도록 하는 단계; 및

본 발명이 또 다른 측면에 따르면, 인슐린을 심근조직으로 국소적으로 전달하기 위한 이식 가능한 의료 장치는
관상동맥 내에 이식될 수 있는 형태의 이식 가능한 의료 장치; 및
치료학적 투여량만큼 그리고 심근 조직의 허혈성 상해를 감소시키는데 효과적인 투여기간에 걸쳐, 인슐린을 심근 조직으로 치료학적 투여량만큼 방출시키는 이식 가능한 의료 장치에 고정된 생체 적합성 폴리머 중의 치료학적 투여량의 인슐린을 포함한다.

발명의 상세한 설명
본 발명은 급성 심근경색을 포함한 급성 허헐성 증후군의 치료를 위한 방법 및 장치에 관한 것이다. 방법 및 장치는 허혈성 상해 후의 괴사 부위를 제한하기 위해 치료제를 국소적으로 심근 조직에 전달하기 위한 것이다. 치료제의 국소전달은 전신 전달의 필요성 및 비표적 조직에 대한 손상을 예방하기 위한 추가적인 보호제를 투여할 필요성을 없앤다.
우선, 본 명세서에 사용된 다음 용어를 하기와 같은 의미를 갖는다:
용어 "약물" 및 "치료제"는 원하는 통상의 유익한 효과를 생성시키기 위해 생체의 도관에 전달되는 임의의 치료학적 활성 물질을 언급하기 위해 서로 혼용하여 사용된다.

심근 세포 생존을 보존하기 위해 약물을 국소적으로 전달하는 방법

본 발명의 일 측면에 따르면, 스텐트 또는 다른 국소 전달 장치는 급성 MI 및 재관류 후에 인슐린을 국소적으로 전달하기 위해 사용된다. 인슐린은 당분해 대사 및 ATP 생성을 향상시키는 호르몬이다. 인슐린은 또한 혈관확장제, 항염증제, 및 항혈소판제로서 작용할 수 있다. 따라서, 인슐린은 경색 크기를 감소시키기 위해, 염증의 감소, 허혈성 괴사의 속도 감소, 순환 FFA 수준 및 심근 FFA의 흡수 감소, 심근 글리코겐 저장의 회복 그리고 수축 기능의 향상과 같은 여러 기전에 의해 작용한다.
본 발명에 사용하기 위한 인슐린은 인간, 비-인간, 또는 합성일 수 있으며, 완전하거나 조각일 수 있다. 바람직하게는, 인슐린은 방사선에 안정한 단시간 작용 형태이다. 결정형의 인슐린은 방사선에 대한 내성 증가를 위해 사용될 수 있다. 인슐린을 폴리머와 결합시킬 경우, 생활성을 보존하기 위해 다른 물질을 부가할 수 있다. 인슐린은 폴리(락타이드-코-글리콜라이드)(PLGA) 중에 함께 전달될 때 24 시간 이상의 투여기간 동안 그 활성을 유지하는 것으로 밝혀졌다. 실질적으로 더 긴 투여기간동안, 제산제 또는 다른 약물이 PLGA 매트릭스의 생활성을 유지하기 위해 요구되는 pH를 유지하기 위해 사용될 수 있다.
일 실시예에서, 인슐린은 하이드로겔 또는 프로토-하이드로겔 매트릭스와 함께 결합될 수 있다. 인슐린/하이드로겔은 스텐트의 구멍에 로딩하고, 탈수시킨다. 하이드로겔의 재수화는 하이드로겔의 팽윤을 유발시켜 인슐린이 하이드로겔로부터 방출될 수 있도록 한다.
스텐트로부터의 인슐린의 전달이 허혈성 상해를 감소시키기 위해 주로 혈관의 관강으로 전달하기 위한 것으로 기재하였다고 하더라도, 인슐린은 또한 재협착을 치료하기 위해 스텐트로부터 벽면으로 전달될 수도 있다.
본 발명의 또 다른 측면에 따르면, 스텐트 또는 국소적 전달 장치는 MI 또는 재관류 후에 VIP의 국소적 전달을 위해 이용된다. VIP는 심장에 의해 자연적으로 방출되는 뉴로펩티드이며, 심장에 대한 보호 효과를 발휘한다. VIP는 혈관확장제, 혈소판 억제제, 및 항증식제로서 작용한다. VIP는 염증 촉진 물질의 생성을 억제하고 활성화된 마크로파지에서 항염증 시토킨의 생성을 촉진시킴으로써 작용한다.

두가지 약물(dual agent)의 예에서, 허혈성 상해의 감소에 적절한 약물은 주로 스텐트의 관강으로부터 전달되는 반면, 재협착 치료를 위한 약물은 주로 스텐트의 벽면으로부터 전달된다. 유사한 일 실시예에서, 허혈성 상해의 감소를 위한 제 1 약물은 약 1 내지 약 24 시간의 기간에 걸쳐 제 1 투여기간동안 제 1 전달속도로 전달하며, 반면에 재협착 치료를 위한 제 2 약물은 2일 이상, 바람직하게는 약 3일 이상, 그리고 보다 바람직하게는 약 10일 이상과 같은 제 2 투여기간동안 제 2 전달속도로 전달한다.

도 1은 동맥 (100)의 관강 (116)에 이식된 스텐트 형태의 팽창 가능한 의료 장치 (10)를 나타낸다. 동맥 (100)의 벽은 세 개의 서로 다른 조직층: 내막 (110), 중간막 (112), 외막 (114)을 포함한다. 팽창 가능한 의료 장치 (10)을 동맥의 폐색 부위에 이식할 때, 팽창 가능한 의료 장치로부터 동맥 (100)의 관강 (116)에 전달되는 치료제는 폐색 부위 및 혈류의 하류의 조직에 국소적으로 분포된다.
도 1-3에 나타낸 팽창 가능한 의료 장치 (10)의 일 예는 버팀목의 기계적 특성을 손상시키는 것 없이 구멍 (14)를 함유할 수 있는 큰 비변형 버팀목 (12) 또는 전체 장치를 포함한다. 비변형 버팀목 (12)는 전체가 참고로 본 명세서에 통합되어 있는 미국특허 6,241,762에 상세하게 기재되어 있는 연성의 힌지 (20)를 사용함으로써 이루어질 수 있다. 구멍 (14)는 다양한 유익한 약물을 의료 장치의 이식 부위에 전달하기 위한 큰 보호된 용기로서 작용한다.

도 4는 하나 이상의 유익한 약물이 구멍 (14)에 여러 층으로 로딩된 의료 장치 (10)의 일부의 횡단면을 나타낸다. 비록 여러 개의 분리된 층이 설명의 용이를 위해 나타낸 것이라고 할지라도, 층들은 독립된 조성을 갖는 분리된 층이거나, 연속적인 폴리머 매트릭스 및 약물 인레이(inlay)를 형성할 수도 있다. 예를 들어, 층들은 약물, 폴리머, 용매 조성물의 층들이 별도로 침적된 다음, 용매의 작용에 의해 구멍에서 함께 혼합될 수 있다. 약물은 균일하게 인레이 내에 분포되거나 농도 구배 중에 분포될 수 있다. 그러한 층을 생성시키는 방법 및 그러한 층의 배열의 예는 전체가 참고로 본 명세서에 통합되어 있는 미국출원 공개 2002/0082680(공개일: 2002. 6. 27)기재되어 있다. 구멍 (14) 내에 폴리머와 결합된 약물을 이용함으로써 의료 장치 (10)이 원하는 특정 약물 전달 프로파일로 맞춘 약물 방출 동태학을 갖도록 디자인할 수 있다.
일 실시예에 따르면, 구멍 (14)의 총 깊이는 약 50 내지 140 미크론이며, 전형적인 층 두께는 약 2 내지 50 미크론이며, 바람직하게는 12 미크론일 것이다. 따라서, 각각의 전형적인 층은 표면 코팅 스텐트에 적용되는 전형적인 코팅 두께의 약 2 배이다. 전형적인 구멍에는 그러한 층이 2 이상, 바람직하게는 약 5 내지 12개가 존재하며, 전형적인 유익한 약물층의 두께는 전형적인 표면 코팅보다 약 4 내지 28 배 더 두껍다. 본 발명의 일 구현예에 따르면, 구멍들은 5 x10-6 평방인치 이상, 바람직하게는 10 x10-6 평방인치 이상의 면적을 갖는다.
도 4의 예에서, 구멍의 벽면에는 장벽층 (18)이 구비되어 있으며, 그것은 치료제층 (16)에 있는 치료제 모두가 실질적으로 장벽층의 완전한 침식 전에 구멍의 관강면으로부터 전달되기에 충분히 낮은 침식속도를 갖는 폴리머 또는 다른 물질의 층이다. 장벽층 (18)은 수송, 저장, 및 스텐트의 이식 과정 동안 유익한 약물의 손실을 억제한다. 그러나, 장벽층 (18)은 약물의 벽면 및 관강 전달이 허용가능한 곳에서는 빠질 수 있다.

도 5에서, 항재협착 약물 (32)가 장치 (10)의 벽면에 하나 이상의 층으로 구비되며, 허혈성 상해를 감소시키기 위한 치료제 (36)이 하나 이상의 층으로 장치의 관강 면에 구비된다. 분리층 (34)는 상기 약물층의 사이에 구비될 수 있다. 분리층 (34)는 두 약물의 투여기간이 실질적으로 차이가 있고 약물중 하나의 약물의 전달이 계속되고 있는 동안 다른 약물의 전달이 완전히 종료될 경우에 특히 유용할 수 있다. 분리층 (34)는 바람직하게는 두가지 약물의 투여기간 이상의 속도로 분해 가능한 임의의 생체 적합성 물질일 수 있다.

Claims (6)
- 원기둥형 장치의 몸체; 및
상기 장치의 몸체 상의 구멍(through-opening)내에 침적된, 급성 심근경색 후 허혈성 상해 또는 재관류 손상 치료를 위한 치료제
를 포함하는 의료 장치로서, 이때 상기 치료제가 혈관 벽으로 전달되지 않고 혈관의 관강으로 전달될 수 있도록 구멍의 벽면 또는 그에 인접한 곳에 장벽층이 구비되어 있으며, 상기 치료제가 투여 기간에 걸쳐 방출되도록 하는 약물 전달 프로파일을 나타내는 방식으로 생체 적합성 폴리머와 함께 구멍 내에 침적되는 것인, 관상동맥 내에 이식되도록 배열되어 심근조직으로 상기 단일 치료제를 국소적으로 전달하기 위한, 이식가능한 의료 장치. - 제1항에 있어서, 재관류 손상을 치료하기 위한 치료제가 항염증 약물, 프로테아제 억제제, 항산화제, 또는 혈관확장제 중에서 선택된 것임을 특징으로 하는 장치.
- 제1항에 있어서, 허혈성 상해를 치료하기 위한 치료제가 아데노신 또는 이의 유도체, 혈관확장제, 니코란딜을 포함하는 산화질소 공여체, 항-TNF 화합물, 덱사메타손, 아스피린, 피르페니돈, 메클로페남산, 및 트라니라스트를 포함하는 항염증제, 시롤리무스 및 에버롤리무스를 포함하는 면역억제제, 티로피반을 포함하는 항혈소판제 및 항트롬빈제; COX-1 및 COX-2 억제제를 포함하는 사이클로옥시게나아제 억제제; 혈관활성제 또는 인슐린 중에서 선택된 것임을 특징으로 하는 장치.
- 제1항에 있어서, 치료제가 아데노신 또는 이의 유도체인 것을 특징으로 하는 장치.
- 제1항에 있어서, 구멍의 관강면 상에, 장치를 이식하기 전에 치료제가 전달되는 것을 방지하는 분해될 수 있는 캡층이 제공되어 있음을 특징으로 하는 장치.
- 제1항에 있어서, 이식 가능한 의료 장치는 관상동맥 내에서 팽창 가능한 스텐트인 것을 특징으로 하는 장치.
