Abstract
본 발명은 항암제, 암세포에서 과다분비되는 표적 단백질 분해효소에 의해 특이적으로 분해되는 펩타이드 및 암부위에 특이적으로 축적가능한 고분자로 결합된 표적 단백질 분해효소 감응형 항암제 전구체에 관한 것으로, 비활성 형태의 항암제 전구체를 투여하면, 표적부위에 축적된 후 방사선 조사에 의해 펩타이드가 분해되면서 활성형의 항암제를 표적부위에서 방출할 수 있기 때문에 정상세포의 파괴를 최소화하며 낮은 항암제의 투여 및 방사선량 조사로 높은 항암 치료효과를 얻을 수 있다.

Description
본 발명은 정상세포의 파괴를 최소화하며 낮은 항암제의 투여 및 방사선량 조사로 높은 항암 치료효과를 얻을 수 있는 표적 단백질 분해효소 감응형 항암제 전구체에 관한 것이다.
암은 현재 국내에서 사망원인 1위를 차지하는 질병으로서, 향후 환경문제, 수명의 연장, 식생활의 서구화 등으로 암환자 발생은 더욱 증가할 것이다. 암 치료에 있어서 흔히 사용하는 방법은 기존의 항암치료제를 방사선 치료법 (radiotherapy)과 함께 사용하는 것이다.
많은 항암치료제와 방사선 치료는 암 세포의 세포사멸(cell apoptosis)을 유도하여 암을 치료하는 원리에 기반을 두고 있다. 또한 방사선 치료법에 기존의 화학요법에 사용되던 항암치료제를 병행하여 사용하면 보다 증가된 치료효과를 얻을 수 있다.
그러나 기존의 항암치료 및 방사선 치료요법은 암세포의 성장억제 및 사멸에 효과가 우수할지라도 높은 항암제 투여량 및 방사선 조사량에 기인하여 암조직 뿐 아니라 정상세포에도 높은 독성을 나타내어 심각한 부작용을 야기하는 문제점을 갖고 있다.
이에 최근 새롭게 부각되어 개발되고 있는 분야가 표적 항암 치료제(target therapies)로서 이는 기존의 화학요법제, 호르몬요법제와 다른 작용 메커니즘을 갖는 약물로, 정상세포에는 작용하지 않고 종양세포의 세포성장 신호전달, 수용체 및 유전적 변이를 표적으로 하므로 약물의 효능을 증가시키고, 독성을 최소화할 수 있는 최적의 항암치료제이다.
최근 매우 빠른 속도로 개발되어 지고 있는 분자영상(molecular imaging) 기술을 이용한 정확한 목표종양의 설정에 기인한 항암치료를 병행 사용한다 하여도 항암제 투여량과 방사선 치료시 이용되는 방사선 조사량의 최적화는 항암치료 효과 증진에 매우 중요하다.
모든 암세포는 충분한 항암제 투여 및 방사선을 조사하면 사멸되지만, 암세포 주변의 정상세포도 피해를 입을 수 있으므로 선택적 항암제 치료와 정확한 조사 부위에 효율적인 방사선을 조사하는 물리적인 방법들이 활발히 연구되고 있다.
따라서, 정상세포의 피해를 최소화 할 수 있는 표적 항암 치료제 이용 및 낮은 방사선량 조사로 효율적인 세포 사멸 효과를 얻을 수 있다면 기존 항암제 치료 및 방사선 치료 효과를 획기적으로 증진할 수 있다.
상기 종래기술의 문제점을 해결하기 위하여, 본 발명자는 약물전달 기술, 나노의학 기술 및 방사선 치료 기술 등 다양한 혁신 연구 분야를 융합하여 치료 효율을 극대화하고 부작용을 최소화하는 미래지향적 표적 단백질 분해효소 감응형 항암제 전구체를 개발함으로써 본 발명을 완성하였다.
따라서, 본 발명의 목적은 정상세포의 파괴를 최소화하며 낮은 항암제의 투여 및 방사선량 조사로 높은 항암 치료효과를 얻을 수 있는 표적 단백질 분해효소 감응형 항암제 전구체를 제공하는 데에 있다.
상기 목적을 달성하기 위하여, 본 발명은 항암제, 펩타이드 및 고분자로 결합되어 구성되며, 상기 펩타이드는 암세포에서 과다분비되는 표적 단백질 분해효소에 의해 특이적으로 분해되고, 상기 고분자는 암 부위에 특이적으로 축적가능한 것을 특징으로 하는 표적 단백질 분해효소 감응형 항암제 전구체를 제공한다.
본 발명의 전구체는 항암제와 결합된 펩타이드를 고분자와 결합시키는 링커를 추가로 포함하여 항암제-펩타이드-링커-알부민 복합체인 것이 바람직하다.
상기 링커로는 히드록시숙신이미드(hydroxysuccinimide) 또는 말레이미드(maleimide)를 말단에 가지고 있는 링커라면 어느 것이라도 사용가능하며, 예를들어, 숙신이미딜-4-(N-말레이미도메틸)사이클로헥산-1-카르복실레이트[succinimidyl-4-(N-maleimidomethyl)cyclohexane-1-carboxylate], 비스 N-하이 드록시숙신이미드(bis N-hydroxysuccinimide), 비스 말레이미드(bis maleimide) 등과 같은 화합물을 사용할 수 있다.
상기 전구체는 비활성 항암제 전구체 형태로 표적부위에 투여되어 축적되며, 그후 낮은 투여량의 항암제 및 방사선 조사에 의해 펩타이드가 분해되어 활성성분인 항암제를 표적부위에서 국소적으로 방출한다.
상기 표적 단백질 분해효소로서는 세포사멸 분해효소(caspases), 기질 단백분해효소(matrix metalloproteinases; MMP), 트롬빈, FXIIIa, 카스파제(caspase), 우로키나아제 플라스미노겐 활성제(urokinase plasminogen activator, uPA), HIV 프로테아제, DPP-IV 및 프로테아좀(proteasome)으로 이루어진 군에서 선택된 어느 하나가 사용될 수 있다.
상기 항암제는 독소루비신, 파클리탁셀, 아드리아마이신, 시스플라틴, 5-플루오로우라실, 마이토마이신, 클로모마이신, 블레오마이신, 페플로마이신, 다우노루비신, 아클라루비신, 네오카르치노스타틴, 에피루비신, 이다루비신 또는 피라루비신에서 선택된 어느 하나일 수 있다.
또한, 상기 펩타이드는 서열목록 1로 표시되는 펩타이드를 포함한 아세틸 옥타펩타이드이며, 상기 단백질 분해효소는 카스파제-3 단백질 분해효소이다.
상기 고분자는 생체적합성을 갖는 모든 고분자가 사용될 수 있으며, 특히 암 조직 축적 효율이 높고 항암제의 약물전달체로 사용되는 생체고분자인 콜라젠, 젤라틴, 덱스트란(dextran), 폴리-L-라이신(poly-L-lysine), 폴리아스파르트산(poly-aspartic acid) 등이 있으며, 합성 고분자로서는 폴리(N-2-(하이드록시프로필)메타 아크릴아마이드)[poly(N-2-(hydroxypropyl)methacrylamide], 폴리(디비닐 에테르-코-말레익 언하이드라이드)[poly(divinyl ether-co-maleic anhydride)], 폴리(스틸렌-코-말레익 언하이드라이드)[poly(styrene-co-maleic anhydride)], 폴리(에틸렌 글라이콜)[poly(ethylene glycol)] 등이 있다.
이하, 본 발명을 보다 상세하게 설명한다.
본 발명에 따른 항암제 전구체는 고분자에 항암제가 화학적으로 결합되어 있으므로 비활성 상태로 존재하며, 고분자 나노입자를 이루고 있기 때문에 생체 내 투여시 EPR 효과(enhanced permeability retention: 종양부위에 흡수되는 나노입자의 양과 그 위치에 머무는 시간을 향상시키는 효과)에 의해 암 조직과 같은 염증성 부위에 비침습적 및 선택적으로 축적된다.
본 발명에 따른 항암제 전구체 투여 후, 부작용을 나타내지 않으나 치료효과가 미비한 낮은 투여량의 항암 치료제를 투여 및 방사선을 조사하면 이는 암 조직 부위에서 미비한 세포 사멸효과를 유발하게 된다.
일반적인 상태에서 유발된 미비한 세포 사멸 효과로는 뚜렷한 항암 치료효과를 나타 낼 수 없으나, 암 조직 부위에 본 발명에 따른 항암제 전구체가 축적된 상태에서 유발된 세포 사멸효과는 비활성 항암 치료제를 활성 상태로 만든다. 이에 암 세포 부위에서 선택적 및 단계적으로 증폭된 활성화된 항암치료제는 높은 세포 독성을 나타내게 되고 궁극적으로 높은 표적 치료효율을 나타내게 된다.
이는 정상세포 파괴를 최소화 하며 획기적으로 낮은 항암 치료제 투여 및 방사선량 조사로 높은 항암 치료효과를 얻을 수 있어 기존의 대표적인 항암제 및 방 사능 병행 치료의 부작용을 혁신적으로 낮출 수 있다. 또한 암세포 표적성을 가지며 세포 사멸 시에만 특이적으로 활성화 되어 암세포에 추가적인 독성을 나타내는 신개념의 항암치료제 개발로 기존의 방사능 치료와 병행되던 화학요법의 치료효과를 혁명적으로 높일 수 있는 새로운 치료기술이다.
도 1은 알부민 단백질에 접합되어 있는 항암제가 비활성 상태로 있다가 세포 사멸효과에 의해 분비된 단백질 분해효소에 특이적으로 분해되면 활성화된 항암제가 방출 가능한 단백질, 펩타이드 기질 및 항암제 나노입자로 이루진 본 발명의 표적 단백질 분해효소 감응형 항암제 전구체의 모식도이다.
또한, 도 2는 본 발명의 표적 단백질 분해효소 감응형 항암제 전구체의 작용기전 원리에 관한 모식도로서, 1)과 같이 본 발명에 따른 항암제 전구체를 투여하게 되면 비활성 항암치료제가 목표 종양에 선택적으로 축적된다. 이후 2)와 같이 부작용과 세포 독성이 낮은 항암 치료제를 투여 및 방사선을 조사하게 되면 3)과 같이 암 세포에서 세포 사멸을 유도하게 되고, 세포 사멸에 기인하여 카스파제 분해효소가 과다 발현된다. 이에 발현된 분해효소에 의하여 알부민에 결합된 비활성 항암제가 방출되고 결과적으로 방출된 활성화된 항암제가 암세포를 선택적으로 사멸시킨다.
도 3은 본 발명에 따른 표적 단백질 분해효소 감응형 항암제 전구체의 합성 예를 나타낸 것으로, 모델 항암제인 독소루비신(Dox)은 카스파제 단백질 분해효소에 의해 특이적으로 분해되는 서열목록 1의 펩타이드(Cys-Asp-Glu-Val-Glu-Ala-Pro-Lys)를 포함한 아세틸 옥타펩타이드(Ac-Cys-Asp-Glu-Val-Glu-Ala-Pro-Lys) 기 질에 화학적으로 결합되어 펩타이드-Dox 복합체(Ac-Cys-Asp-Glu-Val-Glu-Ala-Pro-Lys-Dox)를 이룬다.
상기 펩타이드-항암제를 다양한 링커(linker)를 이용하여 알부민 단백질에 화학적으로 결합시켜 나노입자 형태의 알부민-펩타이드-항암제 형태의 비활성 항암제 전구체를 제조한다. 상기의 비활성 항암제 전구체는 세포 사멸시 분비되는 카스파제-3 단백질 분해효소에 의하여 펩타이드 기질의 Asp-Glu-Val-Glu 부분이 특이적으로 분해되면서 활성화된 Ala-Pro-Lys-Dox를 방출하게 된다.
또한, 도 4는 알부민-펩타이드-Dox의 비활성 세포 독성을 나타낸다.
본 발명에 따른 항암제 전구체를 약학조성물로 제조할 경우, 약학조성물에 통상적으로 사용하는 적절한 담체, 부형제 또는 희석제를 더 포함할 수 있다.
본 발명에서 사용가능한 담체, 부형제 또는 희석제로는, 락토즈, 덱스트로즈, 수크로스, 솔비톨, 만니톨, 자일리톨, 에리스리톨, 말티톨, 전분, 아카시아 고무, 알지네이트, 젤라틴, 칼슘 포스페이트, 칼슘 실리케이트, 셀룰로즈, 메틸 셀룰로즈, 미정질 셀룰로스, 폴리비닐 피롤리돈, 물, 메틸히드록시벤조에이트, 프로필히드록시벤조에이트, 탈크, 마그네슘 스테아레이트 또는 광물유 등을 들 수 있다.
본 발명에 따른 항암치료제는, 각각 통상의 방법에 따라 산제, 과립제, 정제, 캡슐제, 현탁액, 에멀젼, 시럽, 에어로졸 등의 경구형 제형, 외용제, 좌제 및 멸균 주사용액의 형태로 제형화하여 사용될 수 있다.
제제화할 경우에는 보통 사용하는 충진제, 증량제, 결합제, 습윤제, 붕해제, 계면활성제 등의 희석제 또는 부형제를 사용하여 조제된다. 경구투여를 위한 고형 제제에는 정제, 환제, 산제, 과립제, 캡슐제 등이 포함되며, 이러한 고형제제는 상기 화합물은 적어도 하나 이상의 부형제, 예를 들면, 전분, 칼슘카보네이트(calcium carbonate), 수크로스(sucrose) 또는 락토오스(lactose), 젤라틴 등을 섞어 조제한다.
또한 단순한 부형제 이외에 마그네슘 스테아레이트, 탈크 같은 윤활제들도 사용된다. 경구를 위한 액상 제제로는 현탁제, 내용액제, 유제, 시럽제 등이 해당되는데 흔히 사용되는 단순희석제인 물, 리퀴드 파라핀 이외에 여러 가지 부형제, 예를 들면 습윤제, 감미제, 방향제, 보존제 등이 포함될 수 있다.
비경구 투여를 위한 제제에는 멸균된 수용액, 비수성용제, 현탁제, 유제, 동결건조 제제, 좌제가 포함된다. 비수성용제, 현탁제로는 프로필렌글리콜(propylene glycol), 폴리에틸렌 글리콜, 올리브 오일과 같은 식물성 기름, 에틸올레이트와 같은 주사 가능한 에스테르 등이 사용될 수 있다. 좌제의 기제로는 위텝솔(witepsol), 마크로골, 트윈(tween) 61, 카카오지, 라우린지, 글리세로제라틴 등이 사용될 수 있다.
본 발명에 따른 항암제 전구체를 약학조성물로 제조할 경우, 약학조성물에 함유된 항암제-펩타이드-알부민 복합체의 사용량은 환자의 나이, 성별, 체중에 따라 달라질 수 있으며, 일일 1회 내지 수회 투여할 수 있다. 그리고 이의 투여량은 투여경로, 질병의 정도, 성별, 체중, 나이 등에 따라서 증감될 수 있다. 따라서, 상기 투여량은 어떠한 면으로든 본 발명의 범위를 한정하는 것은 아니다.
상기 약학조성물은 쥐, 생쥐, 가축, 인간 등의 포유동물에 다양한 경로로 투 여될 수 있다. 투여의 모든 방식은 예상될 수 있는데, 예를 들면, 경구, 직장 또는 정맥, 근육, 피하, 자궁내 경막 또는 뇌혈관내(intracerebroventricular)주사에 의해 투여될 수 있다.
또한, 본 발명의 약학조성물은 종래 안전성 데이터가 확보된 항암제의 신규 제형에 관한 것이므로, 안전하게 사용가능하다.
본 발명에 따른 항암제 전구체는 표적부위인 암 부위에 선택적으로 축적되는 비활성 상태의 고분자 항암치료제가 특정 단백질 분해효소에 특이적으로 반응하여 활성 상태의 항암제를 방출하므로 암세포에만 선택적으로 항암효과를 나타낼 수 있어 치료효과의 극대화 및 항암 치료의 부작용을 최소화할 수 있다.
이하, 하기 실시예를 통해 본 발명을 보다 상세하게 설명한다. 다만, 이러한 실시예에 의해 본 발명이 한정되는 것은 아니다.
<합성예 1> 표적 단백질 분해효소 감응형 항암제 전구체 제조
독소루비신(Dox) 20mg에 비스 (N-히드록시숙신이미드 에스테르[bis (N-hydroxysuccinimide ester; NHS] 27mg와 N-메틸모르폴린(N-methylmorpholine), 4-디메틸아미노피리딘(4-dimethylaminopyridin)을 디메틸포름아미드(dimethylformamide)에서 반응시켜서 NHS기를 가지고 있는 독소루비신(Dox-NHS)을 제조하였다.
제조된 Dox-NHS 10mg와 카스파제 단백질 분해효소에 의해 특이적으로 분해되 는 서열목록 1의 펩타이드를 포함한 아세틸 옥타펩타이드(Ac-Cys-Asp-Glu-Val-Glu-Ala-Pro-Lys) 기질 24mg을 N-메틸모르폴린(N-methylmorpholine)과 디메틸아미노피리딘(dimethylaminopyridine)을 포함한 디메틸포름아미드(dimethylformamide)에서 6시간동안 반응시켜서, 화학적으로 결합된 펩타이드-Dox 복합체(Ac-Cys-Asp-Glu-Val-Glu-Ala-Pro-Lys-Dox)를 제조하였다.
링커로 사용한 숙신이미딜-4-(N-말레이미도메틸)사이클로헥산-1-카르복실레이트[succinimidyl-4-(N-maleimidomethyl)cyclohexane-1-carboxylate]와 알부민을 인산 완충 생리식염수(phosphate buffer saline, pH 7.4)에서 2시간 동안 반응시켰다.
Fast Protein Liquid Chromatography를 이용하여 미반응물을 제거하고 링커가 결합된 알부민을 분리하였다. 링커가 결합된 알부민 7mg와 펩타이드-Dox 복합체(Ac-Cys-Asp-Glu-Val-Glu-Ala-Pro-Lys-Dox) 5mg를 트리(2-카르복시에틸)포스핀[tris(2-carboxyethyl)phosphine]을 포함한 물에서 12시간 동안 반응시켜, 나노입자 형태의 알부민-펩타이드-항암제 형태의 비활성 항암제 전구체를 제조하였다.
<실시예 1> 세포 독성 검토
세포독성은 HeLa 세포에 각기 다른 농도의 독소루비신 및 알부민-펩타이드-Dox를 첨가한 후 MTT([3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide]) 측정법을 사용하여 측정하였다. 도 4와 같이 독소루비신 항암제는 농도에 비례하여 세포 독성을 나타내었으나 알부민-펩타이드-Dox 비활성 항암제는 세포 독성을 나타내지 않음을 확인하였다.
<실시예 2> 항암 효과 검토
합성예 1에서 제조한 항암제 전구체를 암 질환 동물모델에 투여한 후 방사선 조사에 의한 세포 사멸 유도 유무에 따른 항암 효과를 관찰하였다.
암 질환 모델은 편평상피세포 SCC7 암 세포를 C3H/HeN 마우스에 피하 이식하여 만들었다. 먼저 5Gy의 방사선을 암 부의에 조사하여 세포 사멸을 유도하였다. 그 후 2 내지 3일에 합성예 1에서 제조한 항암제 전구체(200 ㎍/마우스)를 투여하였다. 이에 방사선을 암 조직에 조사한 후 합성예 1에서 제조한 항암제 전구체의 투여 유무에 따른 항암 효과를 관찰하였다.
상기의 세포 독성 시험에서와 같이 방사선 조사를 하지 않은 합성예 1에서 제조한 항암제 전구체는 항암효과를 나타내지 않았다. 암 조직에 5Gy의 방사선을 조사한 결과 항암 효과를 관찰할 수 있었다. 특히, 합성예 1에서 제조한 항암제 전구체를 투여한 암 질환 모델에 세포 사멸을 유도할 수 있는 방사선을 조사한 결과 방사선 조사만 시행한 것과 비교하여 뚜렷이 증가한 항암효과를 관찰 할 수 있었다.
이하, 본 발명의 항암제 전구체를 함유하는 약학조성물의 제제예를 설명하나, 본 발명은 이를 한정하고자 함이 아닌 단지 구체적으로 설명하고자 함이다.
<제제예 1> 주사제의 제조
합성예 1에서 제조한 항암제 전구체 3 mg, 소디움 메타비설파이트 3.0 ㎎, 메틸파라벤 0.8 ㎎, 프로필파라벤 0.1 ㎎ 및 주사용 멸균증류수 적량을 혼합하고 통상의 방법으로 최종 부피가 2㎖이 되도록 제조한 후, 2㎖용량의 앰플에 충전하고 멸균하여 주사제를 제조하였다.
<제제예 2> 정제의 제조
합성예 1에서 제조한 항암제 전구체 20 ㎎, 유당 100 ㎎, 전분 100 ㎎ 및 스테아린산 마그네슘 적량을 혼합하고 통상의 정제 제조방법에 따라 타정하여 정제를 제조하였다.
<제제예 3> 캡슐제의 제조
합성예 1에서 제조한 항암제 전구체 10 ㎎, 유당 50 ㎎, 전분 50 ㎎, 탈크 2 ㎎ 및 스테아린산 마그네슘 적량을 혼합하고 통상의 캡슐제 제조방법에 따라 젤라틴 캡슐에 충전하여 캡슐제를 제조하였다.

- 항암제, 펩타이드 및 고분자로 결합되어 구성되며, 상기 펩타이드는 암세포에서 과다분비되는 표적 단백질 분해효소에 의해 특이적으로 분해되고, 상기 고분자는 암 부위에 특이적으로 축적가능한 것을 특징으로 하는 표적 단백질 분해효소 감응형 항암제 전구체.

- 제 1항 또는 제 2항에 있어서, 상기 항암제는 독소루비신, 파클리탁셀, 아드리아마이신, 시스플라틴, 5-플루오로우라실, 마이토마이신, 클로모마이신, 블레오마이신, 페플로마이신, 다우노루비신, 아클라루비신, 네오카르치노스타틴, 에피루비신, 이다루비신 또는 피라루비신에서 선택된 어느 하나인 것을 특징으로 하는 항암제 전구체.
- 제 1항 또는 제 2항에 있어서, 상기 펩타이드는 서열목록 1로 표시되는 펩타이드인 것을 특징으로 하는 항암제 전구체.
- 제 1항 또는 제 2항에 있어서, 상기 고분자는 콜라젠, 젤라틴, 덱스트란(dextran), 폴리-L-라이신(poly-L-lysine), 폴리아스파르트산(poly-aspartic acid), 폴리(N-2-(하이드록시프로필)메타아크릴아마이드)[poly(N-2-(hydroxypropyl)methacrylamide], 폴리(디비닐 에테르-코-말레익 언하이드라이드)[poly(divinyl ether-co-maleic anhydride)], 폴리(스틸렌-코-말레익 언하이드라이드)[poly(styrene-co-maleic anhydride)] 및 폴리(에틸렌 글라이콜)[poly(ethylene glycol)]로 이루어진 군에서 선택된 어느 하나인 것을 특징으로 하는 항암제 전구체.
- 제 1항 또는 제 2항에 있어서, 상기 표적 단백질 분해효소는 세포사멸 분해효소(caspases), 기질 단백분해효소(matrix metalloproteinases; MMP), 트롬빈, FXIIIa, 카스파제(caspase), 우로키나아제 플라스미노겐 활성제(urokinase plasminogen activator, uPA), HIV 프로테아제, DPP-IV 및 프로테아좀(proteasome)으로 이루어진 군에서 선택된 어느 하나인 것을 특징으로 하는 항암제 전구체.
