Description
본 발명은 간독성을 갖는 제약 화합물의 간독성이 완화된 조성물에 관한 것이다. 보다 특히, 본 발명은 각각이 간독성 성질을 갖는 것으로 공지된 아세트아미노펜, 메토트렉세이트, 스타틴 약물, 니아신, 디발프로엑스 나트륨, 발프로산 또는 플루코나졸과 같은 간독성 화합물의 간독성이 완화된 조성물에 관한 것이다.
아세트아미노펜은 1880년대에 광범위하게 사용된 약물인 페나세틴의 활성 대사물질이다. 아세트아미노펜은 1893년에 진통제 및 해열제로서 첫번째로 사용되었으나, 1949 이후까지 광범위하게 사용되지 않았다. 수년간 아세트아미노펜은 진통제/해열제로서 아스피린에 대한 제2차 선택으로서 사용되었으나, 아스피린 사용과 레이 증후군과의 관계에 대한 해명 및 위장관 출혈을 일으키는 아스피린의 성향에 대한 인식은 아세트아미노펜을 어린이 및 성인 모두에서 제1차 선택의 진통제/해열제로서의 현재 위치로 도약시켰다. 아세트아미노펜은 통상적으로 견딜만하나, 그의 사용은 매우 심각한 부작용 - 잠재적으로 치명적인 간 괴사를 동반할 수 있다. 아세트아미노펜에 의한 간 괴사는 크게 2 군의 환자로 한정된다: 1. 급성으로 아세트아미노펜 과용량을 섭취한 환자 또는 만성적으로 아세트아미노펜 고투여량 요법을 받은 환자. 2. 알콜중독자에 의한 아세트아미노펜 섭취 또는 알콜 섭취와 함께 아세트아미노펜 섭취. 미국에서 매년 26,000 명 초과의 환자들이 아세트아미노펜 유발 간 괴사로 인하여 병원 치료를 받으며, 이들 중 매년 400 명 초과의 환자들이 사망하는 것으로 보고되었다. 이들 과용량의 대다수는 자살 시도의 결과이나, 보고서는 매년 2,000 명 초과의 입원환자 및 100 명의 사망자가 의도되지 않은 아세트아미노펜 과용량에 기인된 것으로 나타낸다. 사실, 미국 식품 의약품 안전청의 자문 위원단은 아세트아미노펜 함유 제품의 표지에 간 괴사의 위험에 대하여 새로운 경고 문구를 삽입할 것을 권고하였다.
세포 대사 억제제인 메토트렉세이트는 류마티스성 관절염 및 건선을 비롯한 여러 상이한 질병들에 널리 사용되는 치료제로서 수십년간 사용되어 왔다. 메토트렉세이트 투여는 여러 다른 부작용과 연관되어 있으나, 심각하고 종종 치명적인 간독성이 치료적 유용성을 제한하는 중요한 인자이다. 아토르바스타틴, 심바스타틴 및 다른 "스타틴" 계열의 콜레스테롤 저하제는 세계적으로 가장 널리 사용되고 있다. 이들의 광범위한 사용에도 불구하고, 간독성은 중요한 문제점이며, 오래된 또는 활성의 간염 병력을 갖는 환자는 이들의 콜레스테롤 저하 작용이 유익할 수 있음에도 불구하고 이들 약물을 피해야 한다. 니아신 (니코틴산 또는 비타민 B3으로도 공지됨), 콜레스테롤 저하제로서 종종 사용되는 다른 약제는 또한 간독성의 높은 발병률과 연관되어 있다. 플루코나졸, 강력한 항진균제, 및 광범위하게 사용되는 항간질제인 디발프로엑스 나트륨 및 발프로산은 그들의 임상적 사용이 간독성으로 인해 제한되는 세가지 다른 약제이다.

따라서, 본 발명의 목적은 아세트아미노펜, 메토트렉세이트, "스타틴", 니아신, 플루코나졸, 디발프로엑스 나트륨, 발프로산 및 관련 약물과 같은 간독성 치료학적 약물의 제약상 허용되는 조성물로서, 심각한 간독성에 대한 잠재성이 현저하게 감소된, 활성 약물의 치료학적 이점을 제공하는 조성물을 제공하는 것이다.
<바람직한 실시태양의 상세한 설명>
본 발명에 따라서, 공지된 간독성 성질을 갖는 화합물을 간독성 성질이 완화된 조성물로 제제화한다. 조성물은 상대적으로 낮은 투여량의 니코틴아미드 및 메티오닌과 함께 표준 용량의 간독성 화합물을 포함할 수 있다. 낮은 투여량의 엽산을 간독성 성질을 추가로 완화시키기 위하여 조성물에 첨가할 수도 있다.

아세트아미노펜, 메토트렉세이트, 아토르바스타틴, 심바스타틴, 니아신, 플루코나졸, 디발프로엑스 나트륨 또는 발프로산과 같은 간독성 화합물과 간독성 완화제인 메티오닌, 니코틴아미드 및 엽산과의 조합물에 적합한 제약 비히클, 및 이러한 제제의 제조 방법은 본 발명의 범위내에 있고, 당업자에게 쉽게 명백할 것이며 이해될 것이다.
본 발명의 조성물은 하기 실시예로부터 보다 쉽게 이해될 것이다.
실시예 1
고통스러운 골관절염을 앓는 환자에게 아세트아미노펜 500 mg, 메티오닌 50 mg 및 니코틴아미드 25 mg을 포함하는 정제 2정을 12주간 일일 4회 투여하여 어떠한 간독성 증거 없이 관절 통증의 실질적 경감의 결과를 얻었다.
실시예 2
각각 아세트아미노펜 325 mg, 메티오닌 50 mg, 니코틴아미드 50 mg 및 엽산 500 mcg 중량을 함유하는 캡슐을 제조하였다. 골관절염 또는 근섬유통 통증을 앓는 환자에게 상기 캡슐 1 내지 2개를 6개월간 일일 4회 내지 6회 투여하여 환자의 간 손상에 대한 증거 없이 관절 또는 연조직 통증 경감의 결과를 얻었다.
실시예 3
골관절염을 앓는 환자에게 아세트아미노펜 500 mg, 메티오닌 200 mg 및 니코틴아미드 100 mg을 함유하는 캐플렛 2개를 12주간 일일 4회 투여하여 간독성 증거 없이 골관절염 통증 경감의 결과를 얻었다.
실시예 4
각각 메토트렉세이트 2.5 mg, 메티오닌 100 mg, 니코틴아미드 100 mg 및 엽산 100 mcg을 함유하는 정제를 제조하였다. 피부 건선을 앓는 환자에게 상기 정제 2정을 6개월간 일일 2회 투여하였다. 환자들은 심각한 메토트렉세이트-유도된 간 손상의 증거 없이 건선 병변의 개선을 나타내었다.
실시예 5
편두통을 앓는 환자에게 디발프로엑스 나트륨 250 mg, 메티오닌 250 mg 및 니코틴아미드 100 mg 중량을 함유하는 정제를 일일 2회 투여하여 심각한 간 손상 증거 없이 편두통의 중증도를 예방 또는 감소시킨 결과를 얻었다.
실시예 6
각각 아토르바스타틴 10 mg, 메티오닌 500 mg, 니코틴아미드 100 mg 및 엽산 1.0 mg 중량을 함유하는 캡슐을 제조하고, 이를 일일 1회 환자들에게 투여하였다. 환자들은 간 기능의 유의적 변화의 증거 없이 혈청 콜레스테롤 및 트리글리세리드가 더 낮았다.
실시예 7
플루코나졸 10 mg/ml, 메티오닌 25 mg/ml 및 니코틴아미드 20 mg/ml를 함유하는 경구 현탁액을 3주간 일일 2 내지 12 mg/kg의 투여량으로 구강인두 칸디다증 의 치료를 위하여 어린이들에게 투여하여 표준 플루코나졸 현탁액보다 간독성 위험이 실질적으로 감소된 결과를 얻었다.
Claims (37)
- 제약상 허용되는 담체 중에 1종 이상의 표준 용량의 간독성 화합물을 포함하며, 표준 용량 당 메티오닌 약 5 mg 내지 약 500 mg 및 표준 용량 당 니코틴아미드 약 10 mg 내지 약 500 mg을 추가로 포함하는 조성물.
- 제1항에 있어서, 경구 섭취에 적합한 조성물.
- 제2항에 있어서, 용액제, 현탁액제, 정제, 캡슐 및 캐플렛으로 이루어진 군으로부터 선택되는 조성물.
- 제1항에 있어서, 주사에 적합한 조성물.
- 제4항에 있어서, 멸균 용액제 또는 현탁액제로 이루어진 군으로부터 선택되는 조성물.
- 제5항에 있어서, 피부내, 피하, 근육내, 정맥내 또는 척수강내 주사에 적합한 조성물.
- 제1항에 있어서, 엽산을 표준 용량 당 약 50 mcg 내지 약 5 mg의 양으로 추 가로 포함하는 조성물.
- 제7항에 있어서, 경구 섭취에 적합한 조성물.
- 제8항에 있어서, 용액제, 현탁액제, 정제, 캡슐 및 캐플렛으로 이루어진 군으로부터 선택되는 조성물.
- 제7항에 있어서, 주사에 적합한 조성물.
- 제10항에 있어서, 멸균 용액제 또는 현탁액제로 이루어진 군으로부터 선택되는 조성물.
- 제11항에 있어서, 피부내, 피하, 근육내, 정맥내 또는 척수강내 주사에 적합한 조성물.
- 제1항에 있어서, 상기 간독성 화합물이 아세트아미노펜, 메토트렉세이트, 아토르바스타틴, 심바스타틴, 니아신, 플루코나졸, 디발프로엑스 나트륨 및 발프로산으로 이루어진 군으로부터 선택되는 것인 조성물.
- 제13항에 있어서, 상기 간독성 화합물이 아세트아미노펜인 조성물.
- 제14항에 있어서, 상기 아세트아미노펜이 표준 용량 당 약 80 내지 1000 mg의 양으로 존재하는 것인 조성물.
- 제13항에 있어서, 상기 간독성 화합물이 메토트렉세이트인 조성물.
- 제16항에 있어서, 상기 메토트렉세이트가 표준 용량 당 약 25 내지 250 mg의 양으로 존재하는 것인 조성물.
- 제13항에 있어서, 상기 간독성 화합물이 아토르바스타틴인 조성물.
- 제18항에 있어서, 상기 아토르바스타틴이 표준 용량 당 약 5 내지 100 mg의 양으로 존재하는 것인 조성물.
- 제13항에 있어서, 상기 간독성 화합물이 심바스타틴인 조성물.
- 제20항에 있어서, 상기 심바스타틴이 표준 용량 당 약 5 내지 100 mg의 양으로 존재하는 것인 조성물.

- 소정량의 간독성 화합물, 소정량의 니코틴아미드 및 소정량의 메티오닌을 포 함하는 조성물을 제제화하는 것을 포함하는, 간독성 화합물의 간독성을 완화시키는 방법.
- 제30항에 있어서, 조성물 중 간독성 화합물 각각의 표준 용량에 대하여 니코틴아미드가 약 5 내지 500 mg의 양으로 존재하고, 메티오닌이 약 25 내지 500 mg의 양으로 존재하도록 조성물을 제제화하는 방법.
- 제31항에 있어서, 상기 니코틴아미드가 간독성 화합물의 표준 용량 당 약 25 내지 200 mg의 양으로 존재하는 것인 방법.
- 제31항에 있어서, 상기 메티오닌이 간독성 화합물의 표준 용량 당 약 10 내지 100 mg의 양으로 존재하는 것인 방법.
- 제30항에 있어서, 상기 조성물이 소정량의 엽산을 추가로 포함하는 것인 방법.
- 제34항에 있어서, 상기 엽산이 간독성 화합물의 표준 용량 당 약 50 mcg 내지 5 mg의 양으로 존재하는 것인 방법.
- 제35항에 있어서, 상기 엽산이 간독성 화합물의 표준 용량 당 약 500 mcg 내 지 1 mg의 양으로 존재하는 것인 방법.
- 제30항에 있어서, 상기 간독성 화합물이 아세트아미노펜, 메토트렉세이트, 아토르바스타틴, 심바스타틴, 니아신, 플루코나졸, 디발프로엑스 나트륨 및 발프로산으로 이루어진 군으로부터 선택되는 것인 방법.
