뼈 대체 재료은 니들, 카테터(20) 또는 다른 형태의 충전 공구를 통해 백(22) 안으로 삽입된다. 뼈 대체 재료는 공동(16)의 범위까지 백을 팽창시킨다.
독창적인 백(22)은 직경이 약 1 내지 4 cm인 작은 직물 백일 수 있으며, 타원 및 다른 기하학적 형상일 수 있지만 대략 구의 형태이다. 백은 내부 공간(21)이 설명될 내용물로 충전되기 이전에는 유연하며 전성을 갖는다. 백(22)의 재료는 백이 위치되는 공동의 형상을 따르도록 구성될 수 있다. 이러한 초기 형상일 때, 백은 직경이 약 3 mm 내지 1 cm인 비교적 작은 튜브 또는 입구를 통해 가압되어 팽창되지 않을 수 있다.
도9에 가장 잘 도시된 바와 같은 백(22)은 특수하고 신규한 방식으로 구성된다. 백(22)은 직물(23)로 구성될 수 있다. 직물(23)은 섬유 세공이 유체 및 용액을 출입시키고 혈관, 섬유 조직, 뼈 결체 조직의 내부 성장과 외부 성장을 허용하지만 뼈 이식편과, 수산화인회석 또는 뼈의 형성을 촉진하는 것으로 공지된 뼈전도성 생체 조직과 잘 교합되는 다른 재료와 같은 뼈 이식편 대용품과 같이 둘러싼 재료의 작은 입자를 보유하기에 충분히 기밀한 밀도로 끈을 꼬거나 성형 형성된 직조물, 편성물일 수 있다. 직물(23)은 복수의 세공(25)을 한정한다. 일반적으로, 직물(23)의 세공(25)은 약 0.25 mm의 직경을 갖거나, 약 5.0 mm 이하의 직경을 갖는다. 이러한 크기는 백 안에 포장된 재료를 함유하지만 조직의 내부 성장을 허용하도록 선택된다. 뼈의 내부 성장을 일으키지 않는 뼈 접합제 또는 다른 재료가 사용되면, 세공(25)은 매체가 백(22)에서 공동(16)으로 누출되는 것을 방지하도록 더욱 기밀할 수 있다. 이는 뼈에서 빠져나올 때, 신경, 혈관 등에 영향을 줄 수 있는 누출을 방지한다.

백(22)이 충전 재료로 완전히 충전되었을 때, 백은 대체로 공동(16)을 충전하는 자가 유지 형상(self-retaining shape)을 형성한다. 충분히 충전되면, 충전 재료를 백 내부(21)에 배치시키는 충전 기구가 개구(27)로부터 제거된다. 개구(27)가 세공(25)이 아니라 백(22) 내의 분리된 별개의 개구일 때, 개구(27)는 백이 충전된 후, 예컨대, 묶음, 체결, 용접, 접착 또는 개구(27)를 폐쇄하는 다른 수단에 의한 밀봉을 요구하는 설정된 직경일 수 있다. 개구(27)가 세공(25)일 경우, 개구(27)로부터의 카테터 또는 충전 기구의 제거 시, 직물(23)은 다른 세공(25)들의 직경과 대체로 유사하게 되도록 개구(27)의 직경을 감소시키기 위해 수축된다.

직물은 가볍고, 생체 친화성이 있고, 가요성이고, 취급이 용이하며, 매우 우수한 인장 강도를 갖기 때문에, 삽입과 팽창 중 잘 찢어지지 않는다. 장치가 팽창되었을 때, 장치는 팽창되어 사전에 굴착된 공동을 충전한다.
본원에서 용어 "직물"의 사용은 그 용어의 통상의 정의를 포함하며 직물과 같은 기능을 하는 임의의 재료를 포함하므로, 본 발명의 "직물"은 재료 및 유체 유동이 설명된 바와 같은 용어 하에서 허용되는 복수의 세공(25)을 구비해야만 하며, "직물"은 좌굴되어 팽창된 백 크기보다 작은 개구로 삽입되기에 충분히 가요성이어야만 한다.

종래의 많은 실시예가 백(22)의 사용을 척추 본체와 같은 조직의 복구용으로 개시되었지만, 도16 내지 도18에서는 대퇴 두부의 무혈관괴사증을 치료하는데 백(22)이 어떻게 사용될 수 있는지를 도시한다. 도16에서, 복구가 필요한 상태의 대퇴 두부(40)가 도시된다. 도17은 전술된 바와 같이, 리머(14)에 의해 리밍된 대퇴 두부를 도시한다. 리머(14)는 공동(16)을 형성한다. 도18에서, 백(22)은 대퇴 두부(40) 내부에 형성된 공동(16) 내에 도시된다. 백(22)의 개구(27)는 뼈 대체 재료에 의해 충전되고 팽창된 후에 폐색된다.
대체 실시예에서, 또한 전술된 바와 같은 스크리브너 등의 벌룬은 대퇴 두부(40)의 복구용 백(22)으로써 사용될 수 있다.
도19 내지 도21에 도시된 본 발명의 실시예를 참조하면, 골절부(50)를 가지는 정강이 고원부(48)가 도시된다. 골절부(50)는 도20에 도시된 바와 같이 리머(14)로 공동(16)을 형성함으로써 복구된다. 도21에 도시된 바와 같이, 일단 공동(16)이 적절하게 리밍되면, 백(22)이 그 내부로 삽입되어 뼈 복구 매체(19)로써 충전된다.
또한, 전술한 본 발명의 방법 및 백(22)으로 다른 조직 및 뼈 이상이 치료될 수 있다. 본 발명은 척추 본체, 대퇴 두부 및 정강이 고원부의 치료에만 한정되지 않는다. 본 명세서에 설명된 백(22) 및 치료 방법은 포유류 신체 전체를 통하여 다른 것들은 물론 본 명세서에 설명된 것을 포함하는 많은 유형의 뼈 및 조직 이상을 치료하기 위해 사용될 수 있다.
이하에 청구된 특징의 특정 조합으로 지시된 것에 부가하여, 본 발명은 또한이하에 청구된 종속적인 특징의 다른 조합 및 전술한 특징의 다른 조합을 가지는 실시예를 개시한다.
상기 개시 내용은 예시를 위한 것이며 모든 것을 개시하려고 한 것이 아니다. 본 설명은 당 기술 분야의 통상적인 기술자에게 많은 변경 및 대체예를 제시할 것이다. 이러한 모든 대체예 및 변경예는 "포함하는"이라는 용어가 "포함하지만 이에 한정되지 않는"의 의미로 사용된 청구범위의 범주 내에 포함되는 것을 의도한다. 당 기술 분야에 정통한 자들은 등가물이 또한 청구범위에 의해 포함되는 본 명세서에 설명된 구체적인 실시예에 대한 다른 등가물을 인지할 것이다.

Claims (24)

  1. 신체 내의 뼈 이상을 치료하기 위한 방법이며,
    이상을 갖는 뼈의 영역을 노출시키는 단계와,
    뼈에 적어도 하나의 공동을 형성하는 단계와,
    상기 개구를 통해 적어도 하나의 공동 내로 팽창성 빈 직물 백을 삽입하는 단계와,
    직물 벽을 통해 뼈의 성장을 지지 또는 증진시키는 재료로 충전 개구를 통해 백을 채워지는 단계와,
    백의 내부로부터 재료의 손실을 방지하도록 상기 충전 개구를 폐쇄하는 단계를 포함하고,
    상기 적어도 하나의 공동의 일부는 개구를 한정하고,
    상기 백은 대략 0.25 내지 대략 5.0 ㎜ 사이의 직경을 갖는 복수개의 백 개구를 포함하는 직물 벽을 형성하고, 상기 백은 내부를 한정하고 외부를 갖고,
    상기 채워지는 단계는 백과 재료의 결합이 자체 보유 강성 형상을 형성할 때까지 상기 백이 팽창하도록 하고, 백의 외부는 공동의 뼈와 사실상 접촉하고, 복수개의 백의 개구들은 백의 내부로부터 백의 외부로 재료가 통과하는 것을 방지하도록 구성되고 배열되는 방법.
  2. 제1항에 있어서, 상기 뼈 이상은 골 종양, 물혹, 대퇴 두부 무혈관괴사증,정강이 고원부 골절, 척추의 압박 골절 및 이들의 임의의 조합으로 구성된 군 중에서 하나의 요소로부터 선택되는 방법.
  3. 제1항에 있어서, 상기 뼈의 영역은 척추뼈이고, 상기 척추뼈는 적어도 하나의 압박 골절을 포함하는 방법.
  4. 제1항에 있어서, 상기 뼈의 영역은 대퇴 두부이고, 상기 대퇴 두부는 무혈관괴사증을 포함하는 방법.
  5. 제1항에 있어서, 상기 뼈의 영역은 정강이 고원부이고, 정강이 고원부는 적어도 하나의 골절을 포함하는 방법.
  6. 제1항에 있어서, 상기 백이 재료로 채워질 때, 상기 충전 개구는 대략 0.25 내지 대략 5.0 ㎜ 사이의 직경을 갖는 것을 특징으로 하는 방법.
  7. 제1항에 있어서, 상기 백이 재료로 채워질 때, 충전 개구는 복수개의 백 개구의 직경보다 큰 직경을 갖고, 상기 백이 충전 재료로 채워진 후에, 충전 개구의 직경은 적어도 복수개의 백 개구의 직경보다 작은 방법.
  8. 제7항에 있어서, 상기 충전 개구는 대략 3 ㎜ 내지 대략 1 ㎝ 사이의 직경을갖는 방법.
  9. 제1항에 있어서, 상기 충전 개구는 자체 폐쇄식인 것을 특징으로 하는 방법.
  10. 제1항에 있어서, 상기 공동을 형성하는 단계는,
    상기 개구를 형성하기 위해 뼈의 영역 내로 천공하는 단계와,
    상기 개구 내로 리머 공구를 삽입하는 단계와,
    리머 공구로 상기 영역으로부터 뼈의 양을 절제하는 단계를 포함하고,
    상기 뼈의 양은 공동을 형성하기에 충분한 것인 방법.
  11. 제1항에 있어서, 상기 공동을 형성하는 단계는,
    상기 개구를 형성하기 위해 뼈의 영역 내로 천공하는 단계와,
    상기 개구 내로 팽창성 압밀 부재를 삽입하는 단계와,
    압밀 부재를 팽창하게 하여 뼈의 양을 압밀하는 단계를 포함하고,
    상기 뼈의 양은 공동을 형성하기에 충분한 것인 방법.
  12. 제11항에 있어서, 상기 팽창성 압밀 부재는 직물 백인 방법.
  13. 제1항에 있어서,
    백 내로 전개하기 위해 붕괴된 형상을 갖도록 본체에 대해 직물 백을 배치하는 단계와,
    상기 백 내에서 본체를 팽창시키는 단계를 더 포함하고,
    상기 본체는 해면 뼈를 압밀하기 위한 팽창된 형상으로 팽창되고,
    상기 본체의 팽창은 백을 팽창시키는 방법.
  14. 제13항에 있어서, 본체가 팽창된 형상으로 팽창된 후에 백으로부터 본체를 제거하는 부가 단계를 포함하는 방법.
  15. 척추뼈의 하나 이상의 압박 골절을 치료하기 위한 방법이며,
    하나 이상의 압박 골절을 갖는 척추뼈의 영역을 노출시키는 단계와,
    척추뼈 내로 적어도 하나의 개구를 천공하는 단계와,
    척추뼈의 적어도 하나의 공동을 형성하는 단계와,
    상기 개구를 통해 적어도 하나의 공동 내로 팽창성 빈 직물 백을 삽입하는 단계와,
    직물 벽을 통해 뼈의 성장을 증진시키는 재료로 충전 개구를 통해 백을 채워지는 단계와,
    백의 내부로부터 재료의 손실을 방지하도록 상기 충전 개구를 폐쇄하는 단계를 포함하고,
    상기 적어도 하나의 공동의 일부는 개구를 한정하고,
    상기 백은 대략 0.25 내지 대략 5.0 ㎜ 사이의 직경을 갖는 복수개의 백 개구를 포함하는 직물 벽을 형성하고, 상기 백은 내부를 한정하고 외부를 갖고,
    상기 채워지는 단계는 백과 재료의 결합이 자체 보유 강성 형상을 형성할 때까지 상기 백이 팽창하도록 하고, 백의 외부는 공동의 척추 뼈와 접촉하고, 복수개의 백의 개구들은 백의 내부로부터 백의 외부로 재료가 통과하는 것을 방지하도록 구성되고 배열되는 방법.
  16. 제15항에 있어서, 상기 공동은 척추뼈의 해면 뼈를 리밍 또는 압밀함으로써 형성되는 방법.
  17. 대퇴 두부의 무혈관괴사증을 치료하기 위한 방법이며,
    무혈관괴사증을 갖는 대퇴 두부의 영역을 노출시키는 단계와,
    대퇴 두부 내로 적어도 하나의 개구를 천공하는 단계와,
    대퇴 두부에 적어도 하나의 공동을 형성하는 단계와,
    상기 개구를 통해 적어도 하나의 공동 내로 팽창성 빈 직물 백을 삽입하는 단계와,
    직물 벽을 통해 뼈의 성장을 증진시키는 재료로 충전 개구를 통해 백을 채워지는 단계와,
    백의 내부로부터 재료의 손실을 방지하도록 상기 충전 개구를 폐쇄하는 단계를 포함하고,
    상기 적어도 하나의 공동의 일부는 개구를 한정하고,
    상기 백은 대략 0.25 내지 대략 5.0 ㎜ 사이의 직경을 갖는 복수개의 백 개구를 포함하는 직물 벽을 형성하고, 상기 백은 내부를 한정하고 외부를 갖고,

  18. 제17항에 있어서, 상기 공동은 대퇴 두부의 해면 뼈를 리밍 또는 압밀함으로써 형성되는 방법.
  19. 하나 이상의 골절을 갖는 정강이 고원부를 치료하기 위한 방법이며,
    정강이 고원부 내로 적어도 하나의 개구를 천공하는 단계와,
    정강이 고원부에 적어도 하나의 공동을 형성하는 단계와,
    상기 개구를 통해 적어도 하나의 공동 내로 팽창성 빈 직물 백을 삽입하는 단계와,
    직물 벽을 통해 뼈의 성장을 증진시키는 재료로 충전 개구를 통해 백을 채워지는 단계와,
    백의 내부로부터 재료의 손실을 방지하도록 상기 충전 개구를 폐쇄하는 단계를 포함하고,
    상기 적어도 하나의 공동의 일부는 개구를 한정하고,
    상기 백은 대략 0.25 내지 대략 5.0 ㎜ 사이의 직경을 갖는 복수개의 백 개구를 포함하는 직물 벽을 형성하고, 상기 백은 내부를 한정하고 외부를 갖고,

  20. 제19항에 있어서, 상기 공동은 정강이 고원부의 해면 뼈를 리밍 또는 압밀함으로써 형성되는 방법.
  21. 해면 뼈를 압밀하기 위한 장치이며,
    내부층과 외부층을 포함하고,
    상기 외부층은 내부층을 적어도 부분적으로 둘러싸는 가요성 재료를 한정하고,
    상기 내부층은 엘라스토머 본체를 한정하고,
    상기 외부층은 이를 통해 팽창하는 복수개의 세공을 포함하고,
    상기 내부층과 외부층의 결합은 뼈 내로 전개하기 위해 붕괴된 형상과 공동을 형성하도록 해면 뼈를 압밀하기 위한 팽창된 형상을 갖도록 팽창성 본체를 한정하고,
    상기 내부층은 사실상 중공 챔버를 한정하고,
    상기 내부층과 외부층은 적어도 하나의 개구를 한정하고,
    팽창성 본체는 소정량의 충전 재료가 적어도 하나의 개구를 통해 사실상 중공 챔버 내에 위치될 때 붕괴된 형상에서 팽창된 형상으로 팽창되도록 구성되고 배열되고,
    상기 외부층은 상기 팽창성 본체의 팽창 동안 마주치는 해면 뼈에 의해 내부층이 관통되는 것을 방지하기 위해 구성되고 배열되는 장치.
  22. 제21항에 있어서, 상기 내부층은 의료용 벌룬인 장치.
  23. 제22항에 있어서, 상기 의료용 벌룬은 라텍스, 열가소성 엘라스토머, 우레탄 및 이들의 임의의 조합으로 구성된 군 중에서 적어도 하나의 부재로부터 적어도 부분적으로 구성되는 장치.
  24. 제21항에 있어서, 상기 외부층은 팽창성 메쉬 백인 장치.
