Abstract
본 발명은 세균에 특이적인 박테리오파지로 감염된 세균에 의해 생성된, 용균성 효소 및 변형된 용균성 효소, 예를 들어 셔플된 용균성 효소 및 키메라 용균성 효소 및 이들의 조합물로 구성된 군에서 선택된 1종 이상의 용균성 효소의 유효량이, 상기 용균성 효소를 환자에게 전달하기에 적절한 담체 중에 존재하는 치료제를 비경구 도입하여 세균 감염을 치료하는 방법 및 이를 위한 조성물을 개시한다. 상기 치료제는 셔플된 홀인 효소 또는 키메라 홀인 효소일 수 있는 홀인 효소를 추가로 포함할 수 있다. 주사는 근육내, 피하 또는 정맥내로 이루어질 수 있다.

과거에는 각종 감염의 치료에 항생제를 사용했다. 스트렙토마이세트 (Streptomycetes)의 도입 및 생산에 관한 셀만 왁스맨 (Selman Waksman)의 업적, 플레밍 (Fleming) 박사의 페니실린 발견 및 기타 수많은 업적이 항생제 분야에 공지되어 있다. 몇 년 동안, "기본" 항생제에 첨가 및 화학적 변형을 가하여 이들을 보다 강력하게 만들거나, 이러한 항생제에 알레르기가 있는 사람들을 치료하려는 시도가 있었다.
몇몇은 이러한 항생제의 신규 용도를 발견했다. 미국 특허 제5,260,292호 [Robinson et al.]는 아미노페니실린을 사용한 여드름 (acne)의 국소 치료법을 개시했다. 여드름 및 여드름성 피부 장애을 국소적으로 치료하기 위한 조성물 및 방법은 암피실린, 아목시실린, 다른 아미노페니실린 및 세팔로스포린, 및 이들의 유도체 및 유사체로 구성된 군에서 선택된 항생제를 여드름 및 여드름성 피부 장애의 치료 유효량으로 적용하는 것을 포함한다. 미국 특허 제5,409,917호 [Robinson et al.]는 세팔로스포린을 사용한 여드름의 국소 치료법을 개시했다.
그러나, 각종 질환의 빈도가 크게 증가함에 따라 더욱 많은 항생제가 처방되거나 사용되어, 항생제에 대한 내성이 발달한 세균의 수가 증가되었다. 현재에는, 훨씬 더욱 내성인 세균 균주를 치료하기 위해 더욱 강한 항생제가 더욱 다량으로 사용되고 있다. 결과적으로, 다수의 항생제 내성 세균이 생겨났다. 더욱 많은 항생제의 사용 및 내성을 나타내는 세균의 수 증가로 인해 필요한 항생제의 사용 시간이 증가되었다. 현재에는, 일부는 환자에게 해로운 영향을 미치는, 넓고 비특이적인 항생제가 더욱 빈번하게 사용되고 있다. 또한, 항생제에 알레르기를 나타내는 사람들의 수가 증가하고 있는 것으로 보인다.
결과적으로, 먼저 세균을 확인한 다음, 그를 사멸시키기 위해 노력해왔다.
박테리오파지를 사용하여 세균 질병을 치료하려는 시도가 있었다. 미국 특허 제5,688,501호 [Merril et al.]는 특정 세균에 특이적인 용균성 또는 비-용균성 박테리오파지를 사용하여 동물에서 그 세균에 의해 유발된 감염성 질병을 치료하는 방법을 개시했다.
미국 특허 제4,957,686호 [Norris]는 타액막 (salivary pellicle)에 쉽게 부착되는 특성이 있는 세균에 기생하는 박테리오파지를 구강에 도입하는 개선된 치과위생 방법을 개시했다.
박테리오파지를 동물에 직접 도입하여 질병을 방지하거나 질병에 대항하게 하는 방법은 어느 정도의 결점이 있음을 염두에 두어야 한다. 특히, 상기 세균은 파지가 부착하기에 적합한 성장 단계에 있어야 한다. 세균 및 파지 둘다 그 성장 주기가 정확하고 동시화 (synchronize)되어 있어야 한다. 또한, 세균에 부착하기에 적합한 수의 파지가 있어야 한다. 또한, 파지는 충분히 활성이어야 한다. 또한, 파지는 공격을 가할 유기체로부터의 세균 부스러기 등과 같은 많은 물질들에 의해 억제된다. 세균 감염의 치료를 위한 박테리오파지의 직접적인 사용을 더욱 어렵게하는 것은 파지를 비기능적으로 만드는 면역학적 반응 가능성이다. 다른 문제점은 박테리오파지 부착을 방지하는, 세균의 표면에 있는 수용체에서의 돌연변이이다.
결과적으로, 세균 감염을 치료 및 방지하기 위한 더욱 안전하고 보다 효과적인 다른 수단을 이용하는 방법이 연구되어 왔다.
더욱 효과적인 치료법이 광범위하게 연구되어 온 세균은 연쇄상구균 (Streptococcus)이다. 연쇄상구균 속은 매우 다양하게 구성되며, 병원성 및 공생 그람-양성 세균 모두가 인간, 말, 돼지 및 소 등을 포함하는 넓은 범위의 숙주에서 살아가는 것으로 밝혀졌다. 숙주 내에서, 연쇄상구균은 종종 구강, 콧구멍 및 인강의 점막 표면에서 콜로니화하는 것으로 나타났다. 그러나, 연쇄상구균은 특정 환경에서 피부, 심장 또는 근육 조직에서도 살 수 있다.
인간의 병원성 연쇄상구균으로는 화농성연쇄상구균 (S. pyogenes), 폐렴연쇄상구균 (Streptococcal pneumoniae) 및 대변연쇄상구균 (S. faecalis) 등이 있다. A군 연쇄상구균은 인두 또는 피부상에 존재하여 어떠한 증상도 유발하지 않을 수 있으나, 이들은 또한 경미한 정도 내지 심각한 정도의 감염을 유발할 수도 있으며, 심지어는 생명을 위협할 수도 있다. 병원성 용혈성 연쇄상구균 중에서, 화농성연쇄상구균 또는 A군 연쇄상구균이 급성 인두염 ("패혈성 인두염"), 농가진, 류마티스성 열, 성홍열, 사구체신염 및 침습성 (invasive) 근막염의 원인균에 포함된다. 괴사성 근막염 (때때로, 매체에 의해 "살육성 (flesh-eating) 세균"으로 기재되기도 함)은 근육 및 지방 조직에 대한 파괴적 감염이다. 침습성 A군 연쇄상구균 감염은 상기 세균이 감염 환자의 방어를 극복할 때 발생한다. 매년 미국에서는 약 10,000 내지 15,000 건의 침습성 GAS 질병이 발생하여 2,000 여명이 사망한다. CDC는 매년 미국에서 500 내지 1,500 건의 괴사성 근막염 및 2,000 내지 3,000 건의 연쇄상구균 독성 쇼크 증후군이 발생한다고 추정하고 있다. 괴사성 근막염 환자의 대략 20%가 사망하고, 연쇄상구균 독성 쇼크 증후군 환자의 60%가 사망한다. 다른 형태의 침습성 A군 연쇄상구균 질병에 걸린 환자의 약 10 내지 15%가 사망한다.
또한, 봉와직염(蜂窩織炎) (cellulitis)은 통상적으로 황색포도상구균 (Staphylococcus aureus)과 관련이 있으나, C군 연쇄상구균이 피부 상해로부터의 봉와직염을 유발할 수도 있다. 봉와직염은 특히 고령자 또는 이미 약해진 사람에서 사망을 초래할 수 있다.
C1으로 확인된 특정 박테리오파지로 감염된 C군 연쇄상구균 유기체가 생성하는 효소의 특성을 기재한 보고가 있다 ([Maxted, W.R. "The Active Agent in Nascent Page Lywsis of Streptococci," J. Gen Micro., vol 16, pp585-595, 1957], [Krause, R.M., "Studies on the Bacteriophages of Hemolytic Streptococci," J. Exp. Med, vol.108, pp803-821, 1958] 및 [Fischetti, V.A. et al, "Purification and Physical Properties of Group C Streptococcal Phage Associated Lysin," J. Exp. Med, Vol.133 pp.1105-1117, 1971]). 상기 효소는 용해소 (lysin)라고 명명되며 A군, C군 및 E군 연쇄상구균의 세포벽을 특이적으로 절단하는 것으로 밝혀졌다. 본 출원인은 A군 연쇄상구균의 용균 및 세포벽 탄수화물의 분비와 관련하여, 이 효소의 활성 및 특징에 관한 정보를 제공한다.
미국 특허 제5,604,109호 [Fischetti et al.]는 어느 정도 정제된 (semi-purified) C군 연쇄상구균 파지 관련 용해소 효소에 의한 효소 분해를 통해 임상 표본에서 A군 연쇄상구균을 신속하게 검출하는 방법에 관한 것이다. 본 발명은 특이적인 파지로 감염된 세균에 특이적인 파지 용균성 효소가 그 세균의 세포벽을 효과적이고 효율적으로 파괴할 수 있다는 발견에 기초한다. 동시에, 포유동물에는 상기 효소의 기질이 존재하지 않기 때문에, 세균 세포벽을 소화할 때 존재하는 포유동물의 단백질 및 조직에는 비파괴적이다.
미국 특허 제5,985,271호 [Fischetti et al.], 미국 특허 제5,997,862호 [Fischetti et al.] 및 미국 특허 제6,017,528호 [Fischetti et. al.]는 통상적으로 패혈성 인두염으로 알려진 연쇄상구균 A (StreptococcalA) 인두 감염의 예방적 및 치유적 치료를 위한, C1 박테리오파지로 감염된 C군 연쇄상구균 세균에 의해 생성된 용해소 효소를 함유하는 경구 전달 방식, 예를 들면 캔디, 껌, 로젠지제, 트로케제, 정제, 산제, 에어로졸제, 액제 또는 액체 분무제를 위한 조성물 및 사용법을 개시했다. 이는 미국 특허 제5,604,109호의 용해소 효소와 동일하다.
미국 특허 제5,604,109호에서 용해소 효소의 생성 및 정제에 사용된 것과 동일한 통상의 기술을 사용하여 세균에 특이적인 박테리오파지로 감염된 세균에 의해 생성된 다른 용균성 효소를 제작할 수 있다. 세균에 따라, 성장 배지 및 성장 조건에 차이가 있을 수 있다.
세균에 특이적인 파지가 있는 세균의 감염으로 생성된 파지 관련 용균성 효소를 사용한 질병 치료는 수많은 이점이 있다. 용균성 효소와 그의 상보적 파지는 특정 세균에 대해 둘다 표적화되거나 둘다 표적화되지 않는다는 점에서 성질이 유사하고, 선택되는 경우 정상적인 세균성 플로라 (flora)의 기능을 방해하지 않는다. 또한, 용균성 파지는 플라스미드 변이에 의한 영향을 받지 않는 세포벽 구조를 주로 공격한다. 용균성 효소의 작용은 신속하며 세균 성장과 무관하다. 또한, 용균성 효소는 점막 내층으로 지시될 수 있어서, 그곳에서 콜로니화되어 있는 세균을 사멸시킬 수 있을 것이다.
미국 특허 제6,056,954호 [Fischetti et al.]는 세균에 특이적인 박테리오파지로 감염된 세균에 의해 생성된 1종 이상의 용균성 효소의 유효량을 감염 부위에 투여하는 것을 포함하는, 세균 감염의 예방적 또는 치유적 치료 방법 및 이를 위한 조성물을 개시했다. 용균성 효소는 상기 용균성 효소를 감염 부위에 전달하기에 적당한 담체를 포함하는 것이 바람직하다. 이러한 방법 및 치료법은 상부 호흡기감염, 국소 감염, 질 감염, 눈의 감염, 귀의 감염의 치료, 비경구 치료 및 대부분의 다른 세균 감염 치료에 사용될 수 있다.
미국 특허 제6,056,955호 [Fischetti et al.]는 연쇄상구균 감염의 국소 치료법을 개시했다.
세균에 특이적인 파지가 있는 세균의 감염으로 생성된 파지 관련 용균성 효소를 사용한 질병 치료는 수많은 이점이 있다. 파지가 특정 세균에 대해 표적화되어 있기 때문에, 상기 용균성 효소는 정상적인 플로라를 방해하지 않는다. 또한, 용균성 파지는 플라스미드 변이에 의한 영향을 받지 않는 세포벽 구조를 주로 공격한다. 용균성 효소의 작용은 신속하며 세균 성장과 무관하다.
그러나, 세균 감염은 치료되지 않는다면 때때로 더욱 심각한 질환으로 발달한다. 예를 들어, 황색포도상구균 및 폐렴연쇄상구균 등에 의한 피부 감염은 봉와직염으로 발달할 수도 있고, 이를 억제하지 않는다면, 결합 조직의 분해, 패혈증 및 가능하게는 사망을 초래할 수 있다. 기타의 세균 감염은 심부 조직 (deep tissue) 감염 역시 포함할 수 있다. 다른 세균에 의한 기타의 감염은 반드시 피부 감염인 것은 아니며, 신체의 특정 조직을 감염시키고 집중화되어 상기 감염을 치료하기 어렵게 만들 수 있다.
미국 출원 제09/482,992호는 세균에 특이적인 박테리오파지로 감염된 세균에 의해 생성된 1종 이상의 용균성 효소 및 상기 용균성 효소를 환자에게 전달하기 위한 적절한 담체를 비경구 도입하여 세균 감염을 치료하는 방법 및 이를 위한 조성물을 개시하고 있다. 주사는 근육내, 피하 또는 정맥내 수행될 수 있다.
발명의 요약
본 발명은 세균 감염에 의해 유발된 광범위한 각종 질환의 치료 효율을 증가시키기 위해, 용균성 효소 뿐 아니라 각종 세균성 파지 관련 용균성 효소, 홀인 용균성 효소, 키메라 용균성 효소 및 셔플된 용균성 효소의 추출 및 비경구 이용을 개시한다. 용균성 효소는 정맥내, 피하, 피내(皮內) 또는 근육내 투여될 수 있다.
박테리오파지로 감염된 세균에 의해 생성된 용균성 효소를 수득하고 정제하는 방법은 당업계에 공지되어 있다. 최근의 몇몇 증거는 연쇄상구균 유기체를 용균시키는 파지 효소가 실제로는 세포벽 및 파지의 구축에 사용되는 세균 효소일 수 있음을 제안한다. 세균 내에서 복제되는 동안, 파지 유전자 생성물은 박테리오파지의 방출을 목적으로 세균 효소의 상향조절 또는 활성화를 유발할 수 있다. 이러한 세균 효소는 세균 세포에 의해 견고하게 조절될 수 있으며, 세균에 의한 세포벽의 구축 및 조립에 이용된다.
그러나, 세균 질병의 예방적 및 치유적 치료를 위한 상기 용균성 효소의 이용은 본 발명의 출원인에 의해서만 연구되어 왔다. 세균성 파지에 의해 생성된 용균성 효소는 선택된 세균 사멸에 효과적이며 특이적이다.
생물 공학 분야에서는, 부분적으로는 치료용 약물의 디자인을 결정하기 위해 미생물의 게놈 서열분석을 수행하는 것이 최근의 경향이다. 이러한 형태의 모델링은 시간 소모적인 동시에 고가이며, 신규 약물을 발견할 수도 있고 발견하지 못할 수도 있다. 본 발명의 장점은 파지-유도된 용균성 효소가 선택된 세균에 대해 매우 효과적이면서 표적화된 사멸제라는 점이다. 정제된 효소의 1차 구조 및 서열분석은 상기 효소의 효과를 최적화하는 데 수행될 수 있는 각종 화학적 방법들 (예를 들면, 셔플링)의 토대로서 기능한다.
이러한 파지-유도 용균성 효소는 무엇보다도 고전적인 임상 질병과 관련있는 각종 세균성 병원균, 예를 들어 연쇄상구균, 슈도모나스 (Pseudomonas) 등의 사멸 및 세균전 작용제와 관련있는 각종 세균성 병원균, 예를 들어 각각 탄저병 및 페스트 (plague)를 초래하는 바실러스 (Bacillus) 및 예르시니아 (Yersinia) 등의 사멸에 유용하다.
본 발명 (미국 특허 제5,604,109호 전체가 본원에 참고로 도입됨)은 감염되어 이미 병을 앓고 있는 사람을 위한 치유적 치료법으로서, 특정 박테리오파지로 감염된 세균성 유기체에 의해 생성된 용균성 효소를 사용한다. 본 발명은 특이적인 파지로 감염된 세균에 특이적인 파지 용균성 효소가 그 세균의 세포벽을 효과적이고 효율적으로 파괴할 수 있다는 발견에 기초한다. 동시에, 어느 정도 정제된 효소는 단백질분해 효소 활성이 없어서, 세균 세포벽을 소화할 때 존재하는 포유동물의 단백질 및 조직에는 비파괴적이다.
키메라 효소, 셔플된 효소, 용균성 효소 및 홀인 효소의 제작, 정제 및 단리는 당업계에 공지되어 있다. 특히, 미국 특허 제6,132,970호 [Stemmer] (본원에 참고로 도입됨)는 이러한 효소 제작을 위한 수많은 새로운 기술들 및 수립된 방법들의 더 많은 변형법을 개시하고 있다. 본 발명은 이러한 기술들을 이용하며 이들을 구체적으로 기재된 파지 관련 용균성 효소 증진을 위해 적용한다. 용해소 효소의 단리 기술은 미국 특허 제6,056,954호 (역시, 본원에 참고로 도입됨)에 기재되어 있으며, 이 기술을 다른 파지 관련 용균성 효소에 적용할 수 있다. 유사하게, 당업계의 기술의 다른 상태를 사용하여 용균성 효소를 단리할 수 있다.
정의하자면, 셔플된 효소란, 1종 이상의 특정 효소의 1개 이상의 서열이 1군데 이상의 위치에서 절단되고 특별한 순서 또는 랜덤한 순서로 재구축되어 그 효소의 활성 또는 특이성이 증가된 효소이다.
키메라 효소란, 2군데 이상의 활성 부위를 함유하는 2종 이상의 효소들이 조합된 효소로서 이러한 키메라 효소는 동일하거나 상이한 분자 상에 독립적으로 작용할 수 있다. 이는 잠재적으로 2종 이상의 상이한 세균 감염을 동시에 치료할 수 있도록 한다.
홀인 효소는 세포막에 홀 (hole)을 생성한다. 더욱 구체적으로, 홀인은 호흡을 종결시키는 치명적인 막 병변을 형성한다. 용균성 효소처럼, 상기 홀인 효소는 파지 게놈에 의해 코딩되며, 이에 의해 운반된다. 사실, 홀인 효소의 유전적 코드가 파지 내 용균성 효소의 코드 다음에서 발견되거나, 심지어는 그 안에서 발견된다는 것은 매우 통상적이다. 대부분의 홀인 서열은 짧고, 전반적인 성질이 소수성이며, 고도의 친수성 카르복시-말단 도메인을 갖는다. 많은 경우에서, 추정적인 홀인 효소는 파지의 효소적 활성 도메인 내에 있는 상이한 리딩 프레임 상에서 코딩된다. 다른 경우에서, 홀인은 파지를 코딩하는 DNA 다음에 있는 DNA, 또는 그와 근접한 DNA 상에서 코딩된다. 홀인 효소는 파지 감염의 후기 단계 동안에 빈번하게 합성되며, 막 병변을 유발하는 세포질 막에서 발견된다.
홀인 효소는 1차 구조 분석을 기초로 2가지 일반적인 클래스로 분류될 수 있다. 클래스 I 홀인은 통상적으로 95개 이상의 잔기를 갖고, 3개의 잠재적인 막횡단 도메인을 가질 수 있다. 클래스 II 홀인은 통상적으로 더 작아서, 대략 65 내지 95개의 잔기를 갖고, 2개의 TM 도메인을 나타내는 하전되고 소수성인 잔기 분포를 갖는다 [Young et al., Trends in Microbiology v.8, No. 4, March 2000]. 그러나, 적어도 숙주가 그람-양성인 파지의 경우, 2중 성분 용균 시스템은 통상적일 수 없다. 여러 파지에서 홀인이 존재하는 것으로 나타났거나 그렇게 제안되었지만, 그러한 파지 모두의 경우에서 추정적인 홀인 코딩 유전자는 전혀 발견되지 않았다. 홀인은 락토콕칼 (lactococcal) 박테리오파지 Tuc2009, 락토콕칼 NLC3, 폐렴구균성 (pneumococcal) 박테리오파지 EJ-1, 락토바실러스 가세리 (Lactobacillus gasseri) 박테리오파지 Nadh, 황색포도상구균 박테리오파지 Twort, 리스테리아 모노시토제네스 (Listeria monocytogenes) 박테리오파지, 폐렴구균성 파지 Cp-1, 고초균 (Bacillus subtillis) 파지 M29, 락토바실러스 델브루엑키 (Lactobacillus delbrueckki) 박테리오파지 LL-H 용해소 및 황색포도상구균의 박테리오파지 N11 등의 다른 파지에서도 존재하는 것으로 나타났거나 그렇게 제안되었다 [Loessner et al., Journal of Bacteriology, Aug. 1999, p.4452-4460].
용균 시스템은 홀인 및 세균 세포벽을 분해할 수 있는 1종 이상의 펩티도글리칸 히드롤라제 또는 "용해소"로 구성된다. 용해소는 당 잔기에 작용하는 엔도-N-아세틸글루코사미니다제 또는 N-아세틸무라미다제 (리소자임) 또는 펩티드 가교결합에 작용하는 엔도펩티다제이거나, 더욱 통상적으로는 당과 펩티드 잔기를 연결하는 아미드 결합을 가수분해하는 N-아세틸무라모일-L-알라닌 아미다제일 수 있다.전형적으로, 상기 홀인은 세포막에 공극 (pore)을 형성하는 파지 감염의 마지막 단계에 발현되어 용해소(들)이 세포벽 펩티도글리칸에 접근할 수 있게 하므로, 프로제니 파지의 방출을 초래한다. 중요한 것은, 외인성 첨가된 용해소는 세균 세포의 세포벽을 용균시켜, 무(無)로부터의 용균"이라 알려진 현상을 일으킬 수 있다는 점이다. 그러나, 외막을 보유하는 그람-음성 세균의 경우, 용균성 효소와 조합된 외인성 첨가된 홀인의 존재로 인해 용균성 효소가 펩티도글리칸에 더욱 효율적으로 접근할 수 있어서 용균이 더욱 잘 일어날 수 있다.
더욱 구체적으로, 정제된 효소의 서열은 통상의 기술로 결정될 수 있고, 셔플링 등과 같은 당업계의 기술을 사용하여 1차 구조를 재배열하여 효소(들)의 활성 및 안정성을 증가시킬 수 있다. 또한, 셔플링은 복합 효소 ("키메라 효소")가 1종 이상의 활성을 보유하도록 한다.

본 발명의 한 실시양태에서, 연쇄상구균 파시아애 (Streptococcal fasciae) 및 황색포도상구균에 의해 유발된 각종 질환의 치료법을 개시한다.
본 발명의 다른 실시양태에서, 황색포도상구균을 용균시키는 효소인 리소스타핀을 치료제에 포함시킬 수 있다.
본 발명의 추가의 실시양태에서, 통상의 항생제를 용균성 효소와 함께 리소스타핀의 존재 또는 부재하에 치료제에 포함시킬 수 있다.
본 발명의 다른 실시양태에서, 1종 이상의 용균성 효소, 1종 이상의 키메라 용균성 효소, 1종 이상의 셔플된 용균성 효소, 1종 이상의 홀인 효소 또는 이들의 조합물을 치료제에 포함시킬 수도 있다.

본 발명의 한 실시양태에서, 폐렴연쇄상구균, 연쇄상구균 파시아애 및 황색포도상구균에 의해 유발된 각종 질환의 치료법이 개시되어 있다.

본 발명의 바람직한 실시양태에서, 1종 이상의 용균성 효소, 1종 이상의 키메라 용균성 효소, 1종 이상의 셔플된 용균성 효소, 1종 이상의 홀인 효소 또는 이들의 조합물을 사용하여 세균 감염을 치료함으로써, 세균 사멸의 속도 및 효율을 증가시킨다.
키메라 효소를 사용하여 세포벽을 1군데 이상의 위치에서 절단함으로써 세균의 감염을 치료할 수도 있다.
수많은 키메라 용균성 효소가 생성되어 연구되어 왔다. 박테리오파지 phi X174 및 MS2 각각의 용균 단백질 E 및 L로부터 구축된 키메라 용해소 유전자 E-L을 내부 결실시켜 용균 또는 사멸시키는 특성이 변경된 일련의 신규 E-L 클론을 제작했다. 이 연구에서, 모 유전자인 E, L, E-L 및 E-L의 내부 말단절단 (truncated) 형태의 용균 활성을 상이한 막 스패닝 (spanning) 도메인마다의 차이점을 기초로 조사하여 상이한 용균 메카니즘을 특성화했다. 전자 현미경을 사용하여 세포질 및 세포질주변 공간에 대한 마커 효소의 방출을 밝혀낸 결과, 2가지 상이한 용균 메카니즘이 대장균의 내막 단백질, 또는 내막과 외막 단백질 관통 여부에 따라 구별될 수 있음을 밝혀냈다 [FEMS Microbiol. Lett. 1998 Jul 1, 164 (1); 159-67].
다른 실험에서, 락토콕칼 파지 Tuc2009 용해소의 N-말단 절반을 코딩하는 영역과 주요 폐렴구균성 자가용해소의 C-말단 도메인을 코딩하는 영역을 융합시켜 활성 키메라 세포벽 용균성 효소 (TSL)를 구축했다. 상기 키메라 효소는 콜린-함유 폐렴구균의 세포벽을 가수분해할 수 있는 글리코시다제 활성을 나타냈다.
본 발명의 바람직한 실시양태는 2종의 감염성 세균을 동시에 치료하거나 세균의 세포벽을 2군데의 상이한 위치에서 절단하기 위한, 키메라 용균성 효소의 사용을 개시한다.
본 발명의 다른 실시양태에서, 홀인 효소를 상기 용균성 효소와 접합하여 사용함으로써 세균 사멸 속도 및 효율을 가속화 한다. 또한, 홀인 효소는 키메라 효소 및(또는) 셔플된 효소의 형태일 수 있다. 또한, 홀인 효소를 세균 감염 치료에 단독으로 사용할 수도 있다.
본 발명의 다른 실시양태에서, 홀인 효소는 용균성 효소와 조합되거나 용균성 효소와 독립적으로 셔플된 홀인 효소 또는 키메라 홀인 효소일 수 있다.
본 발명은 적절한 담체와 블렌딩한 용균성 (lytic) 효소, 홀인 (holin) 용균성 효소, 셔플된 (shuffled) 용균성 효소 및(또는) 키메라 용균성 효소를 환자에게 비경구 도입하여 세균 감염을 치료하는 방법 및 이를 위한 조성물을 개시한다. 주사는 근육내, 피하 또는 정맥내로 이루어질 수 있다.

세균에 의한 전신성 또는 조직 감염의 치료 방법은 세균에 특이적인 박테리오파지로 감염된 세균에 의해 생성된 1종 이상의 용균성 효소, 1종 이상의 키메라 용균성 효소, 1종 이상의 셔플된 용균성 효소 또는 이들의 조합물의 유효량 및 적절한 담체를 포함하는 치료제를 사용하여 감염을 비경구 치료하는 것을 포함한다. 본 발명의 바람직한 실시양태에서, 홀인 용균성 효소 역시 단독으로 또는 바람직하게는 세포벽을 용해하는 1종의 다른 용균성 효소와 조합하여 사용할 수 있다.
세균 감염의 치료 방법은 세균에 특이적인 박테리오파지로 감염된 세균에 의해 생성된 1종 이상의 용균성 효소의 변형된 버전의 유효량을 포함하는 치료제를 사용하여 감염을 치료하는 것을 포함하며, 이때, 용균성 효소의 변형된 버전은 용균성 효소, 셔플된 용균성 효소, 키메라 용균성 효소 및 이들의 조합물로 구성된 군에서 선택된다. 상기 용균성 효소는 이를 활성이도록 하는 pH를 갖는 환경 중에 있는 것이 바람직하다. 홀인 효소는 단독으로 사용하거나 상기 변형된 용균성 효소의 투여와 접합하여 사용할 수 있다. 홀인 효소는 "천연" 상태이거나, 셔플된 홀인 효소이거나 키메라 용균성 효소일 수 있다.
셔플된 용균성 효소 및 키메라 용균성 효소는 효소적으로 또는 재조합 DNA 수단을 통해 생성할 수 있다. 임의의 방법을 사용하여 이 효소를 생성할 수 있다. 셔플된 용균성 효소 및 키메라 용균성 효소는 변형된 또는 조작된 용균성 효소라 지칭할 수 있다.
용균성 효소를 사용하여, 헤모필루스 인플루엔자 (Hemophilus influenza), 슈도모나스, 폐렴연쇄상구균, 연쇄상구균 파시아애, B군 연쇄상구균, 리스테리아 (Listeria), 살모넬라 (Salmonella), 대장균 및 캄필로박터 (Campylobacter) 및 기타 세균 및 이들의 임의의 조합을 치료 또는 방지할 수 있다. 이러한 용균성 효소는 키메라 및(또는) 셔플된 용균성 효소로 보충되거나, 그 자체가 키메라 및(또는) 셔플된 용균성 효소일 수 있다. 유사하게, 홀인 효소를 포함시킬 수 있으며, 이 또한 키메라 및(또는) 셔플된 용균성 효소일 수 있다.
수많은 상이한 세균을 치료할 수 있다. 세균 중에서, 심부 조직 및 더욱 구체적으로 결합 조직을 가장 빈번하게 감염시키는 세균은 A군 연쇄상구균, 포도상구균 (Staphylococcus), 슈도모나스 및 클로스트리듐 (Clostridium)이다. 1종 이상의 용균성 효소 및 1종 이상의 홀인 용균성 효소, 셔플된 용균성 효소, 키메라 용균성 효소 또는 이들의 조합물을 한꺼번에 감염된 신체에 도입할 수 있다.
수많은 상이한 방법을 사용하여 용균성 효소(들)을 도입할 수 있다. 이러한 방법들은 변형된 용균성 효소를 정맥내, 근육내, 피하 및 피내 도입하는 것을 포함한다.
본 발명의 한 바람직한 실시양태에서, 심부 조직 감염은 적절한 홀인 용균성 효소, 키메라 용균성 효소, 셔플된 용균성 효소(들), 용균성 효소 또는 이들의 조합물 및 상기 효소를 위한 담체를 포함하는 치료제를 환자의 감염 조직에 주사함으로써 치료될 수 있다. 사용되는 용균성 효소는 키메라 및(또는) 셔플된 용균성 효소로 보충되거나, 그 자체가 키메라 및(또는) 셔플된 용균성 효소일 수 있다. 유사하게, 홀인 효소를 포함시킬 수 있으며, 이 또한 키메라 및(또는) 셔플된 용균성 효소일 수 있다. 담체는 증류수, 염수 용액, 알부민, 혈청 또는 이들의 임의의 조합물로 구성될 수 있다. 더욱 구체적으로, 주입 또는 주사용 용액은 통상적인 방식, 예를 들어 방부제, 예를 들어 p-히드록시벤조에이트 또는 안정화제, 예를 들어 에틸렌-디아민 테트라아세트산의 알칼리 금속염을 첨가하여 제조할 수 있으며, 이후에 주입 용기, 주사 바이알 또는 앰플에 넣을 수 있다. 별법으로, 주사하기 위한 화합물을 기타의 성분이 있거나 없는 상태로 동결건조시킬 수 있고, 경우에 따라 사용시에 완충 용액 또는 증류수 중에 용해시킬 수 있다. 비-수성 비히클, 예를 들어 고정된 오일 및 에틸 올레에이트 역시 본원에 유용하다. 투여 방식으로서 근육내 주사를 선택한 경우, 등장성 제제가 바람직하게 사용된다. 통상적으로, 등장성을 위한 첨가제는 염화나트륨, 덱스트로스, 만니톨, 소르비톨 및 락토스를 포함할 수 있다. 몇몇 경우에는, 등장성 용액, 예를 들어 인산염 완충 염수가 바람직하다. 안정화제로는 젤라틴 및 알부민 등이 있다. 몇몇 실시양태에서, 제제에 혈관수축제를 첨가한다. 본 발명에 따른 제약상 제제는 멸균되고 발열원이 없는 상태로 제공된다.
담체는 소량의 첨가제, 예를 들어 등장성 및 화학적 안정성을 증진시키는 물질을 적절하게 함유한다. 상기 물질들은 이용되는 투여량 및 농도에서 수용자에게 독성이 없으며, 이들로는 완충액, 예를 들어 인산염, 시트르산염, 숙신산염, 아세트산 및 기타의 유기 산 또는 이들의 염; 항산화제, 예를 들어 아스코르브산; 저분자량 폴리펩티드 (약 10개 미만의 잔기), 예를 들어 폴리아르기닌 또는 트리펩티드; 단백질, 예를 들어 혈청 알부민, 젤라틴 또는 이뮤노글로불린; 친수성 중합체, 예를 들어 폴리비닐피롤리돈; 글리신; 아미노산, 예를 들어 글루탐산, 아스파르트산, 히스티딘 또는 아르기닌; 단당류, 이당류 및 기타의 탄수화물, 예를 들어 셀룰로스 또는 그의 유도체, 글루코스, 만노스, 트레할로스 또는 덱스트린; 킬레이팅제, 예를 들어 EDTA; 당 알콜, 예를 들어 만니톨 또는 소르비톨; 카운터 이온, 예를 들어 나트륨; 비이온성 계면활성제, 예를 들어 폴리소르베이트, 폴옥사머 또는 폴리에틸렌 글리콜 (PEG); 및(또는) 중성 염, 예를 들어 NaCl, KCl, MgCl2, CaCl2등이 있다. 글리세린 또는 글리세롤 (1,2,3-프로판트리올)은 제약상 용도를 위해 시판된다. 주사용 멸균수 또는 염화나트륨 주사액 또는 기타의 제약상 허용가능한 수성 주사액 중에 희석하여 0.1 내지 100% (v/v), 바람직하게는 1.0 내지 50%, 더욱 바람직하게는 약 20%의 농도로 사용할 수 있다. DMSO는 많은 국소 적용된 약물의 투과력을 현저하게 증진시키는 능력이 있는 비양성자성 용매이다. DMSO는 주사용 멸균수 또는 염화나트륨 주사액 또는 기타의 제약상 허용가능한 수성 주사액 중에 희석하여 0.1 내지 100% (v/v)의 농도로 사용할 수 있다.
특히 정맥내 용액의 제조시에는, 담체 비히클이 링거 용액, 완충 용액 및 덱스트로스 용액을 포함할 수도 있다.
변형된 용균성 효소를 포함하는 용균성 효소(들)을 담체 시스템 또는 경구 전달 방식에 도입할 때, 또는 그 이전에, 상기 효소는 pH 범위를 약 4.0 내지 약 9.0 사이, 더욱 바람직하게는 약 5.5 내지 약 7.5 사이, 및 가장 바람직하게는 약 6.1로 유지하기 위한 안정화 완충액 환경 중에 있는 것이 바람직하다. 상기 범위가 연쇄상구균을 위한 용해소 효소에게 가장 적당한 pH 범위이다.
상기 안정화 완충액은 변형된 용균성 효소가 최적 활성이도록 해야 한다.상기 완충액은 디티오트레이톨 등의 환원 시약일 수 있다. 또한, 상기 안정화 완충액은 에틸렌디아민테트라아세트산 이나트륨 염 등의 금속 킬레이팅 시약이거나 이를 포함할 수 있고, 또는 인산염 또는 시트르산염-인산염 완충액을 함유할 수도 있다. 담체 중의 상기 완충액들이 변형된 용균성 효소들의 환경을 안정화시킬 수 있다.
감염 치료에 효과적인 용균성 효소의 투여 빈도 또는 투여량 및 치료 기간은 부분적으로 감염의 심각성, 수용자의 감염성 세균에의 노출 기간, 감염된 피부 또는 조직 1 cm2당 수, 감염 정도, 감염의 심각성 및 각종 수많은 변수에 따라 달라질 것이다. 상기 조성물은 하루에 1회 내지 여러회에 걸쳐 어디에나 적용될 수 있고, 단기간 또는 장기간 적용될 수 있다. 이의 사용은 수일 또는 수주 동안 지속될 수 있다. 이용되는 임의의 투여 형태는 최소의 시간 동안 최소의 유닛을 제공해야 한다. 효소의 활성 유닛의 농도 범위는 효소의 유효량 또는 유효 투여량을 조성물의 약 100 유닛/mL 내지 약 500,000 유닛/mL, 바람직하게는 약 1000 유닛/mL 내지 약 100,000 유닛/mL, 및 가장 바람직하게는 약 10,000 내지 100,000 유닛/mL으로 제공하는 것이라 여겨진다. 1 mL 당 활성 유닛의 양 및 노출 기간은 감염의 성질, 및 변형된 용균성 효소(들)을 함유할 수 있도록 하는 담체와의 접촉량에 따라 달라진다. 변형된 용균성 효소(들)은 체액 환경 중에 있을 때 가장 잘 작동한다는 것을 기억해야 한다. 그러므로, 변형된 용균성 효소(들)의 효과는 부분적으로는 담체 중에 보유된 수분의 양과 관련이 있다. 패혈증 치료를 위해서는, 치료제를 정맥내 연속 유동시켜 혈류로 들어가게 해야 한다. 패혈증을 치료하기 위한 변형된 용균성 효소(들)의 농도는 감염의 심각성에 따라 달라진다.
감염의 치료를 가속화하기 위해, 상기 치료제는 변형된 용균성 효소(들)의 살균 활성을 상승시킬 수도 있는 보완제 1종 이상을 추가로 포함할 수 있다. 상기 보완제는 변형된 용균성 효소(들)의 치유 효과를 상승적으로 증진시키기에 효과적인 양의 페니실린, 합성 페니실린 바시트라신, 메티실린, 세팔로스포린, 폴리믹신, 세파클로르. 세파드록실, 세파만돌 나페이트, 세파졸린, 세픽심, 세프메타졸, 세포니오이드, 세포페라존, 세포라니드, 세포탄메, 세포탁심, 세포테탄, 세폭시틴, 세프포독심 프록세틸, 세프타지딤, 세프티족심, 세프트리악손, 세프리악손 목살락탐, 세푸르옥심, 세팔렉신, 세팔로스포린 C, 세팔로스포린 C 나트륨 염, 세팔로틴, 세팔로틴 나트륨 염, 세파피린, 세프라딘, 세푸르옥심악세틸, 디히드레이트세팔로틴, 목살락탐, 로라카르베프. 마페이트, 킬레이팅제 및 이들의 임의의 조합물일 수 있다.
또한, 상기 치료제는 황색포도상구균 세균의 치료를 위한 효소 리소스타핀을 추가로 포함할 수 있다. 리소스타핀 등의 점막용해성 (mucolytic) 펩티드가 황색포도상구균 감염된 인간의 치료 [Schaffner et al., Yale J. Biol. & Med., 39:230 (1967)] 및 황색포도상구균에 의해 유발된 소 유선염의 치료 [Sears et al., J. Dairy Science, 71 (Suppl.1):244 (1988)]에 효험이 있는 것으로 제안되었다. 스타필로콕쿠스 시뮬란스 (Staphylococcus simulans)의 유전자 생성물인 리소스타핀은 황색포도상구균의 세포벽의 폴리글리신 가교결합을 효소적으로 분해함으로써 황색포도상구균에 대한 정균 및 살균 효과를 발휘한다 [Browder et al., Res. Comm., 19:393-400 (1965)]. 미국 특허 제3,278,378호는 후에 스타필로콕쿠스 시뮬란스라 명명된 에스. 스타필로리티쿠스 (S. staphylolyticus)의 배양 배지로부터 리소스타핀을 생성하는 발효 방법을 기재하고 있다. 리소스타핀을 생성하는 다른 방법은 미국 특허 제3,398,056호 및 제3,594,284호에 추가로 기재되어 있다. 그 후, 리소스타핀 유전자가 클로닝되어 서열결정되었다 [Recsei et al., Proc. Natl. Acad. Sci. USA, 84:1127-1131 (1987)]. r-리소스타핀 등의 재조합 점막용해성 살균 단백질은 이의 표적화된 특이성, 낮은 독성 및 생물학적 활성 잔기의 감소 가능성 때문에 현행 항생제 요법과 관련된 문제점을 잠재적으로 우회시킬 수 있다. 또한, 리소스타핀은 비분열 세포에 대해서도 활성인 반면, 대부분의 항생제는 이들의 효과 매개에 활발하게 분열하는 세포가 요구된다 [Dixon et al., Yale J. Biology and Medicine, 41:62-68 (1968)]. 변형된 용해소 효소(들)과 조합된 리소스타핀은 상기 나열한 항생제의 존재 또는 부재하에 사용될 수 있다. 리소스타핀 및 용해소 효소 모두를 동일한 치료제에 사용하는 것은 더욱 중요하다. 빈번하게, 신체가 세균에 감염된 경우, 세균의 한 속에 의한 감염이 신체를 약화시키거나 신체의 세균 플로라를 변화시킴으로써 이 감염은 다른 잠재적인 병원성 세균이 신체를 감염토록 할 수 있다. 신체를 때로 동시감염하는 세균 중 하나가 황색포도상구균이다. 황색포도상구균의 많은 균주가 페니실린아제를 생성하므로 포도상구균, 연쇄상구균 및 기타의 그람-양성 세균 균주는 표준 항생제로 사멸되지 않을 것이다. 결과적으로, 변형된 용균성 효소(들) 및 리소스타핀의, 가능하게는 항생제와 조합한 사용은세균 감염에 대한 가장 신속하고 효과적인 치료법으로 기능할 수 있다. 다른 바람직한 실시양태에서, 본 발명은 뮤타노라이신 및 리소자임을 포함할 수 있다.
홀인 용균성 효소, 키메라 용균성 효소, 셔플된 용균성 효소 및 이들의 조합물을 포함하나, 이에 제한되지 않는 용균성 효소를 사용하여 세균 세포를 신속하게 용균시킨다. 도 1에서 얇은 절편의 전자현미경 사진은 A군 연쇄상구균 (1)에 용해소를 15초 동안 처치한 결과를 나타낸다. 상기 전자현미경 사진 (배율: 25,OOO)은 용해소 효소에 의해 세포벽 (4)에 생성된 홀 (3)을 통해 세포 내용물 (2)이 배출된다는 것을 나타낸다.
상기 논의한 바와 같이, 홀인 용균성 효소, 키메라 용균성 효소 및 셔플된 용균성 효소의 사용은 미국 특허 제6,132,970호의 방법 또는 당업계의 유사한 방법에 의해서는 변형된 적이 없는 "천연" 용균성 효소의 사용과 병행될 수 있다.
상기 교시 내용에서 본 발명의 많은 변형과 변이가 가능하다. 그러므로, 본 발명은 구체적으로 기재된 것 이외에도 보호될 수 있으며, 첨부된 청구의 범위에 속함을 이해해야 한다.
