Abstract
본 발명은 우리딘 트리포스페이트 및 동종의 화합물로 중이염을 치료하는 방법에 관한 것으로서,
상기 치료를 요하는 환자의 막혀진 중이에서 배액을 촉진하는 방법이 기술되어 있으며, 상기 방법은 우리딘 5′-트리포스페이트(UTP), UTP의 유사체 또는 다른 유사체와 같은 우리딘 트리포스페이트로 막혀진 중이에서 배액을 촉진시키는 유효량으로 중이에 투여하여, 중이에서 점액성 분비물을 수화시키고, 중이 또는 유스타키오관에서 섬모 울림주파를 자극시키며, 상기의 방법은 중이염 및 다른 중이의 질병, 외이염 및 메니에르병을 포함하는 내이 질병으로 고생하는 환자를 치료하는데 유용하며, 약학적인 배합물 및 그의 제조방법이 또한 기술되어 있고, 상기의 투여방법은 특정의 액체 현탁액(코용 구약 또는 스프레이를 포함), 경구용, 분무화에 의한 흡입, 외용약, 주사약 또는 좌약의 형태를 포함하는 것을 특징으로 한다.
Description
중이염(OM)은 절대적이지는 않지만 3세이하의 어린이가 많이 걸리며, 중이가 비루스성 또는 세균성 감염된 것이다. 중이내에 막혀진 유체가 존재함에 의한 것을 특징으로 하며, 대개 비인강 및 유스타키오관을 통해 중이로 퍼지는 기도내 감염에 의해 촉진된다. OM의 발병은 증가되고 있으며, 해마다 OM으로 인한 내과 진료실의 방문은 1975년부터 1990까지 150% 증가되었다(L. McCraig 및 J. Hughes, JAMA 273(3), 214-19(1995)). 상기는 아이들이 더 많은 호흡기 병원체에 노출되는 큰-그룹의 낮보호 시설을 사용함에 의해 증가된다. 매년 OM의 치료 및 진단을 위해 대략 25,000,000에서 40,000,000의 사람이 진료실을 방문하며, 이중 3세까지에서 약 75%의 아이들은 적어도 한 번의 급성 OM의 경험을 가지고 있다(6-24개월의 아이들이 최대 발병율을 갖는다)(J. Klein, Clin Infect Dis 19, 823-33(1994)). 해부학상으로, 유아의 유스타키오관은 청소년 및 성인에 비해 더 짧고, 더 넓으며, 좀 더 수평적으로 놓여 있어서, 비인강에서 중이로 병원체의 전개가 용이하다(L. Schwartz 및 R. Brown, Arch Intern Med 152, 2301-04 (1992)). 상기 감염증은 유스타키오관 및 중이의 점막조직에서 염증반응을 일으키며, 중이에서 유체 삼출액을 일으킨다. 생성된 유체는 점성이고, 고름으로 채워져 있으며, 유체 문제의 일반 뮤코실리어리 운동(mucociliary movement)이 만들어지고, 유스타키오관의 가장 좁은 지점인 협부에서의 염증이 비인강으로 유체의 배액이 효과적으로 차단된다(J. Klein, supra (1994)). 중이의 막힘은 환자에게 큰 통증, 현기증 및 듣기 능력의 손상의 원인이 되며; 유체의 축적으로 평균 듣기능력의 손상은 25데시벨이다. 듣기 능력의 손상은 언어 및 사회적 상호작용에 대한 노출에 의존하는 보통의 인식 개발의 측면을 지연시키거나 또는 크게 방해하기 때문에 유아에 있어서는 특히 관심이 있다.(D. Teele, 등의 J. Infect Dis 1621, 685-94(1990)). OM의 다른 잠재적인(그러나 일반적이지는 않은) 후유증으로는 유돌염, 수막염, 경막외 농양, 경막하 축농, 뇌농양 및 측동 혈전증을 포함한다.
결국 OM 삼출액의 약 80-90%는 하기의 항생요법으로 자연적으로 해결된다. 상기의 방법은 3개월정도 걸린다. 그러나 중이내의 막힘은 점막-생성 세포의 연속적인 과분비 상태에 의해 항생제로 유체를 멸균하기 위해 몇 주 또는 몇 달동안 지속된다(S. Wintermey 및 M.Nahata, Annals of Pharmocotherapy 28, 1089-99(1994)). 지속적인 과분비 상태의 원인은 잘 이해되지는 않지만, 유스타키오관의 폐색에 근거하여 경감되지 않는 것과 관련이 있다. 치료에 대한 부가적인 장애로, 항생제 요법의 효율성은 항생제에 대해 내성을 갖는 세균이 성장하기 때문에 감소된다(M. Poole, Pediatr Infect Dis J. 14(4), 523-26(1995)). 중이의 막힘이 세달이상 지속된다면, 환자의 중이를 환기시키기위하여 티파노스토미관(typanostomy tube)을 삽입하기위하여 주로 수술을 실시한다(K.Grundfast, Arch Otolaryngol Head Neck Surg, 120, 797-98(1994)). 현재 고막절개술은 (윤절술 후에) 아이들에게 실시하는 두 번째로 빈도가 높은 수술이다(J. Klein, supra (1994)). 상기 관은 귀로부터 유체를 배액시키고, 대부분 만성적인 경우에 질병을 해결하는 방법이다. 그러나 상기의 수술은 비용이 비싸고(>$2,000), 유아 환자의 특정 경우에 일반 마취의 처방을 요구한다. 더욱이, 상기 수술의 가능한 (그러나 일반적이지 않은) 후유증으로는 지속되는 이루, 고막에 영구적인 구멍 또는 흉터 및 담지종(중이를 흡장할 수 있고, 구조 주변에 미란이 형성된 케라틴 파괴 흔적으로 채워진 낭포형 낭)을 포함한다(J. Klein, supra (1995)).
이와같이, 세균 내성에 의한 항생제 요법의 효율성의 감소와 티파노스토미술에 관련된 고비용 및 위험의 결과, 의학 연구자들은 증가되고 있는 유행성 질병에 대해 다른 효율적인 요법의 개발을 추구하고 있다. 프랑스 생물공학회사의 연구소인 신텔라보 프랑스(SYNTHELABO FRANCE)는 활성 화합물로 아데노신 트리포스페이트(ATP)를 사용하는 rhinATP라는 상표로 코에서 점성액의 막힘을 치료하는 방법을 개발하고 있다. 상기의 기술은 미국 특허 제5,420,116호로 특허되었다(출원인은 상기의 문헌과 다른 모든 특허 참고문헌 및 그안의 참고문으로 포함되어 있는 인용된 공보를 사용하였다). 상기의 치료방법은 코용 스프레이 또는 코용 구약을 통해 비강으로 ATP를 투여한다. 미국 특허 제5,292,498호에 기술된 것에 의하면, 우리딘 트리포스페이트(UTP) 및 아데닌 트리포스페이트(ATP)가 또한 사람의 기도 상피세포의 이온 전달활성에 효과적인 것을 보여준다. 특히, UTP 및 ATP는 담낭의 섬유증 환자의 폐 상피세포에 의해 염소 및 물의 분비를 유도하고, 액화를 도와서 상기 질병의 특징인 매우 점액성인 기도표면의 점액의 전달을 용이하게 한다. 또한 UTP 및 ATP는 폐의 상피세포에서 섬모 울림주파(ciliary beat frequency)를 자극하며, 부가적으로 담낭의 섬유증 환자의 폐에서 점액의 전달을 용이하게 한다. Membrane Biol. 147, 83-93(1995)를 참고할 것. 출원인은 OM 환자에서 중이에 고여있는 유체의 높은 점도가 다른 누클레오티드 포스페이트로 가령 유체를 차단하는 자리에 아데노신 5′-트리포스페이트(ATP), 시티딘 5′-트리포스페이트(CTP), 1,N6-에테노아도신 트리포스페이트, 아데노신 1-옥시드 트리포스페이트; 3,N4-에테노시티딘 트리포스페이트, P1,P4-디(아데노신-5′)테트라포스페이트(A2P4), 또는 P1,P4-디(우리딘-5′)테트라포스페이트(U2P4)를 투여할 뿐만아니라 UTP 및 그의 관련된 화합물을 투여함에 의해 경감될 수 있다는 것을 발견하였다.
UTP와 다른 누클레오티드 포스페이트는 P2Y2퓨린 수용기의 활성화를 통해 내강의 상피세포로부터 염소 및 물의 분비를 유도한다. 상기 P2Y2수용기는 7개의 트란스멤브레인 스패닝(transmembrane spanning)중 일부이고, G-단백질이 결합된 수용기는 P2Y 수용기족을 나타낸다. 상기 치료법으로 환자의 중이염을 치료하는 방법이 기술되어 있다. 상기의 방법은 하기의 화학식 1의 화합물 또는 그의 약학적으로 허용가능한 염을 중이에서 배액을 촉진시킬 수 있는 유효량으로 환자의 중이에 투여하여, 중이에서 점액성의 분비물을 수화하고, 중이 및 유스타키오관에서 섬모 울림주파를 증가시킨다:

R1은 O, 이미도, 메틸렌 또는 디할로메틸렌(예를들면 디클로로메틸렌, 디플루오로메틸렌)이 있다. 바람직하게는 R1은 산소 또는 디플루오로메틸렌이다.

상기 화학식 1의 화합물은 바람직한 구체적인 예이지만, 본 발명의 방법은 또한 화학식 2의 화합물(아데노신 5′트리포스페이트[ATP] 또는 1,N6-에테노아데노신 트리포스페이트 또는 아데노신 1-옥시드 트리포스페이트) 또는 화학식 3의 화합물(시티딘 5′트리포스페이트[CTP] 또는 3,N4-에테노시티딘 트리포스페이트) 또는 화학식 4의 화합물(P1,P4-디(아데노신-5′)테트라포스페이트(A2P4) 또는 P1,P4디(우리딘-5′) 테트라포스페이트(U2P4))을 투여하는 것을 포함한다.

본 발명의 두 번째 측면은 중이에서 배액을 촉진시키기위해 유효한 양으로 상기 화학식 1, 2, 3 또는 4의 화합물을 함유하는 약학적 배합물로, 이는 중이에서 점액성 분비물을 수화시키고, 중이 및 유스타키오관에서 약학적으로 허용가능한 담체로 섬모 울림주파를 증가시킨다.
본 발명의 세 번째 측면은 중이에서 점액성 분비물의 요법적인 수화 및 상기 치료법으로 환자의 중이 및 유스타키오관에서 섬모 울림주파의 활성화를 위한 약물의 제조에 여기에 기술되어 있는 활성 화합물을 사용하는 것이다.
본 발명은 특정 우리딘, 아데노신 또는 시티딘 트리포스페이트를 투여함에 의해서 환자의 중이에 축적된 고여있는 점액성 분비물을 제거 또는 예방하는 방법에 관한 것이다.
본 발명의 방법은 고여있는 분비물에 의해 중이 및 외이에 질병을 일으키는, 가령 중이염, 급성 중이염, 지속적인 삼출액을 갖는 중이염, 심한 중이염(해결되지 않은 급성 감염, 알레르기반응 또는 항공기의 급강하로 인한 압력상해에 의해 발생됨) 또는 외이염으로(한정되는 것은 아니지만) 상기 치료법를 요하는 환자의 중이 및 외이에서 막혀진 액의 배액을 촉진시키는데 사용된다. 본 발명의 방법은 (한정되는 것은 아니지만) 메니에르병을 포함하는 내이의 질병을 치료하는데 사용된다. 본 발명은 고여있는 분비물을 수화시키고, 중이 및 유스타키오관의 표면에서 섬모 울림주파를 증가시킴에 의해서 중이의 점액성 분비물의 배액을 유도한다. 점액성 분비물의 수화는 그의 점도를 감소시켜서, 좀 더 쉽게 중이 및 유스타키오관에서 뮤코실리어리 반응을 통해 비인강으로 운반된다. 또한, 본 발명은 상기 뮤코실리어리 반응을 촉진시키고, 중이에 고여있는 분비물을 비인강으로 배액시키는 것이 용이하다.
본 발명은 사람을 치료하는데 우선 관심이 있지만, 개, 고양이와 같은 가축 병치료를 목적으로 다른 포유류를 치료하는데도 사용될 수 있다.
상기 화학식 1의 화합물은 (가) 우리딘 5′-트리포스페이트(UTP); (나) 우리딘 5′-O-(3-티오트리포스페이트)(UTPγS); 및 (다) 5-브로모-우리딘 5′-트리포스페이트(5-BrUTP)를 포함한다. 상기 화합물은 공지되어 있거나, 또는 공지된 방법에 따라 제조될 수 있고, 또는 당분야의 통상의 지식을 가진 자에 의해 변형이 이루어질 수 있다. 간단히 하면, 여기서 화학식 1은 자연적으로 나타나는 D-형태의 우리딘 트리포스페이트 활성 화합물을 나타내지만, 본 발명은 또한 L-형태의 화합물과 특별히 명시하지 않는 한 D 및 L-형태의 화합물의 혼합물도 포함할 수 있다.

상기 화학식 4의 화합물의 예로는 (P1,P4-디(아데노신-5′)테트라포스페이트(A2P4) 또는 P1,P4-디(우리딘-5′)테트라포스페이트(U2P4)를 포함한다. 상기 화합물은 공지된 방법에 의해 제조될 수 있으며, 또는 하기에 기술된 것을 변형시킬 수 있다: P. Zamecnik 등의 Proc. Natl. Acad. Sci. USA 89, 838-42(1981); 및 K. Ng 및 L.E. Orgel, Nucleic Acids Res. 15 (8), 3572-80(1987).
또한 UTP, ATP, CTP, A2P4, 3,N4-에테노시티딘 트리포스페이트, 1,N6-에테노아데닌 트리포스페이트, 아데노신 1-옥시드 트리포스페이트, ATPγS, ATPβS, ATPαS, AMPPCH2P, AMPPNHP, N4-에테노시티딘 및 1,N6-에테노아데노신이 예를들면 시그마 케미칼 컴퍼니, PO Box 14508, St. Louis, MO 63178에서 상업적으로 이용할 수 있다.
상기 화학식 1-4의 활성 화합물은 그 자체 또는 나트륨 또는 칼륨과 같은 알카리금속염, 망간, 마그네슘 및 칼슘과 같은 알카리토금속염 또는 암모늄 및 테트라알킬 암모늄염(NX4 +)(상기에서 X는 C1-4이다.)과 같은 그들의 약학적으로 허용가능한 염의 형태로 투여될 수 있다. 약학적으로 허용가능한 염은 모재 화합물의 소망의 생물학적 활성을 유지할 수 있고, 소망하지 않는 독성효과가 부여되지 않는 염이다.
여기에 기술된 활성 화합물은 적당한 형태로 중이염에서 배액을 촉진하기위하여 환자의 중이에 투여되지만, 바람직하게는, 활성 화합물로 이루어진 액체/액체 현탁액(환자가 흡입할 수 있는 호흡용 입자인 액체 배합물의 코용 스프레이 또는 코용 구약)이 투여되도록 처방된다. 코용 스프레이 또는 코용 구약을 제조하기위한 활성 화합물의 액체 약학적 조성물은 활성 화합물을 적당한 운반체로 가령 당분야의 통상의 지식을 가진 자에게 공지된 기술에 의한 물이 없는 무균 피로겐 또는 무균 식염수로 조합하여 제조될 수 있다.
배액을 촉진시키기위한 활성 화합물의 투여량은 치료되는 조건 및 환자의 상태에 의존하여 다양하며, 대개 유효량은 환자의 중이 표면에서 활성화합물이 약 10-7내지 약 10-2mole/ℓ, 바람직하게는 약 10-6내지 약 3×10-4mole/ℓ의 농도가 충분하다.
투여된 활성 화합물의 특정 배합물의 용해도에 의존하여, 배액을 촉진시키기 위한 하루 투여량은 한번 또는 몇 번의 단위로 나누어 투여된다. 바람직하게 하루당 투여는 일당 두 번이하이다.
배액을 촉진시키기위하여 환자의 중이에 활성 화합물을 투여하는 또 다른 방법은 활성 화합물이 특정 경구용의 형태인 것을 포함하며, 환자의 입에 부어서 먹을 수 있는 활성 화합물의 액체 현탁액 또는 알약의 형태로 환자가 삼키는 방법으로 투여된다.
중이 및 내이에 대해 활성 화합물의 유효량을 투여하는 또 다른 방법은 기도로 활성 화합물을 분사하여 흡입할 수 있으며, 상기 활성 화합물은 비인강으로 들어가고, 연속적으로 환자의 내이 및 중이로 들어간다.
중이로 활성 화합물을 투여하는 또 다른 방법으로 활성 화합물이 외용약의 형태인 것을 포함하며, 외이에 크림 또는 겔로서 투여되며, 환자의 중이로 고막을 통해 연속적으로 침투된다.
중이로 활성 화합물을 투여하는 또 다른 방법으로 활성물질이 주사약의 형태인 것을 포함하며, 외이에서 고막을 통해 중이로 직접 주사하거나, 상부 목부분을 통해 중이로 직접 주사한다.
중이로 활성 화합물을 투여하는 또 다른 방법으로 활성물질이 좌약의 형태인 것을 포함하며, 체흡수를 통해 요법적으로 유효량의 화합물이 중이에 도달할 수 있다.
활성 화합물을 투여하는 부가적인 방법으로 활성 화합물이 겔, 크림 또는 액체 현탁액의 형태로 중이, 내이 또는 외이로 활성 화합물의 정맥내 주사하는 것으로 요법적으로 유효량이 중이, 내이 또는 외이로 도달한다.
UTP 및 상기 화학식 1-4의 화합물은 또한 중이염을 치료하기위하여 다른 제제와 함께 조합하여 사용할 수 있으며, 가령 이에 한정되는 것은 아니지만, 페니실린, 페니실린+베타-락탐, 에리트로마이신+술리속사졸, 에팔로스포린, 트리메토드프림, 트리메토드프림+설파메톡사졸, 마크로리드 및 옥사졸리디논의 항생제; 백신; 안티히스타민, 충혈제거제, 점액분해제; 비스테로이드계 항염제; 및 코르티코스테로이드계가 있다. UTP는 또한 NE-1530으로 천연적으로 나타나는 기도의 올리고사카라이드(Neose Technologies, Inc.) 및 유전자 요법과 같이 개발된 제제와 조합하여 사용할 수 있다.
본 발명은 하기의 실시예에서 더 자세히 설명될 것이다. 하기의 실시예는 본 발명을 설명하기 위한 것으로 이를 한정하지는 않는다.
실시예 1
사람의 유스타키오관에서 P2Y2의 압출
P2Y2수용체에 대한 mRNA의 분석은 사람의 유스타키오관 상피 부분에서 실시되었다. 상기의 과정은 L. Burch 등의 Am. J. Physiol., 269(2), C511-C518(1995)에 기술되어 있다. 동결부분(8㎛)이 슬라이드에 놓이고, 2시간동안 포스페이트 완충된 식염수(PBS)내 4% 파라포름알데히드로 고정시킨다. 고정후, 상기 슬라이드를 PBS로 두 번 세척하고, 탈수시키고, 공기로 건조시키며, 사용할 때까지 -80℃에서 저장한다. 선잡종화처리(prehybridization treatments)가 프로테나아제 K소화로 구성되며, 아세틸화한다. RNase 조절부분은 200mg/ml RNase A로 처리한다. 몇 개의 부분을 포함하는 슬라이드는 106개/분의 안티센스 또는 센스 프로브를 함유하는 잡종화 완충액내 54℃에서 한밤동안 잡종화한다.35S-UTP-레벨된 RNA 프로브는 시험관내 Ambion MAXIscript 전사 시스템으로 합성된다. 잡종화 후, 상기 슬라이드가 실내온도에서 4×SSC로 세척되고, RNase A 소화(20㎎/㎖)로 세척되며, 실내온도에서 2×SSC/1mM 디티오트레이톨(DTT)로 세척되고, 54℃(3×15분)에서 0.5×SSC/1mM DTT로 강하게 세척되고, 에탄올로 탈수된다. 건조된 슬라이드가 0.6M 암모늄 아세테이트와 1:1로 희석시킨 Kodak NTB2 광유화액에 침지시킨다. 상기 슬라이드가 1.5에서 2.5wk의 간격으로 현상되고, 헤마톡실린 및 에로신으로 대비염색시킨다.
하기의 도 1은 600bp 안티센스 프로브를 사용하여 사람의 P2Y2수용기를 잡종화한다. 하기의 도 2는 대조군으로서 600bp 센스 프로브를 사용하여 사람의 P2Y2를 접종화한 것이다. 안티센스와 센스 프로브의 비교는 유스타키오관내 P2Y2수용기의 압출을 나타내고 있는, 안티센스 프로브로 더 많은 방사선자동사진법 신호를 낸다.
Claims (15)

- 제 1 항에 있어서,상기 화합물은 환자의 중이에 경구용 형태로 투여함에 의해 도출되며, 상기 화합물의 치료적으로 유효한 양은 체흡수 및 순환을 통해 환자의 중이 또는 유스타키오관과 접촉되도록 하는 것을 특징으로 하는 방법.
- 제 1 항에 있어서,상기 화합물은 환자의 비인두의 기도로 상기 화합물의 에어로졸 현탁액으로 투여함에 의해 도출되며, 상기 화합물의 치료적으로 유효한 양은 환자의 중이 또는 유스타키오관과 접촉되도록 하는 것을 특징으로 하는 방법.
- 제 1 항에 있어서,상기 화합물은 환자의 고막을 통해 중이로 상기 화합물이 외용약의 형태로 투여함에 의해 도출되며, 상기 화합물의 치료적으로 유효한 양은 환자의 중이 또는 유스타키오관과 접촉되도록 하는 것을 특징으로 하는 방법.
- 제 1 항에 있어서,상기 화합물은 주사의 형태로 투여함에 의해 도출되며, 상기 화합물의 치료적으로 유효한 양은 환자의 중이 또는 유스타키오관과 접촉되도록 하는 것을 특징으로 하는 방법.
- 제 1 항에 있어서,상기 화합물은 좌약의 형태로 투여함에 의해 도출되며, 상기 화합물의 치료적으로 유효한 양은 체흡수 및 순환을 통해 환자의 중이 또는 유스타키오관과 접촉되도록 하는 것을 특징으로 하는 방법.
- 제 1 항에 있어서,상기 화합물은 약 10-7내지 약 10-2mole/ℓ의 농도가 되도록 환자의 중이 또는 유스타키오관에 투여하는 것을 특징으로 하는 방법.
- 제 1 항에 있어서,상기 X2및 X3은 OH인 것을 특징으로 하는 방법.
- 제 1 항에 있어서,상기 R1은 산소인 것을 특징으로 하는 방법.
- 제 1 항에 있어서,상기 R2는 H인 것을 특징으로 하는 방법.
- 제 1 항에 있어서,상기 화학식 1의 화합물은 우리딘 5′-트리포스페이트, 우리딘 5′-O-(3-티오트리포스페이트), 5-브로모-우리딘 5′-트리포스페이트 및 그의 약학적으로 허용가능한 염으로 이루어진 군에서 선택되는 것을 특징으로 하는 방법.
- 제 1 항에 있어서,상기 화학식 2의 화합물은 아데노신 5′트리포스페이트, 1,N6-에테노아데노신 트리포스페이트, 아데노신 1-옥시드 트리포스페이트 및 그의 약학적으로 허용가능한 염으로 이루어진 군에서 선택되는 것을 특징으로 하는 방법.
