Abstract

본 발명은 추골의 횡폭의 1/2보다 크고 그리고 추골의 깊이보다 큰 길이를 가진 과대한 척추 삽입물을 디스크 공간내로 측방관철 삽입하기 위한 것이다. 본 발명의 측방관철 삽입물은 인접한 추골사이의 디스크 공간의 높이보다 큰 높이를 갖고 있어서, 양 추골을 결합할 수 있다. 삽입물의 폭은 추골 자체의 깊이보다 단지 약간 작다. 본 발명의 측방관철 척추 고정 삽입물은 보다 큰 접촉 표면적을 갖고 있으며, 그에 따라 토크에 견딜 수 있는 큰 안정성을 허용하며, 나사선이 형성된 삽입물의 경우에 나사선이 추골을 관통할 수 있는 깊이를 증가시킨다.

Description

측방관철 척추 삽입물
과거에, 척추 고정 삽입물은 환자의 전방 또는 후방으로부터 전방향 또는 후방향으로부터만 삽입되어 왔다. 이러한 삽입물은 본 기술 분야에 잘 공지되어 있으며, 원통형, 장방형 및 다른 형상을 가질 수도 있다. 과거에, 신체내에 척추 고정을 위해서 척추의 2개의 인접한 추골사이의 디스크 공간을 가로질러 구멍을 드릴링하는 것을 포함한 다양한 방법은 클로워드, 윌터버거, 크록, 비치, 베그비, 브랜티갠(Cloward, Wilterberger, Crock, Viche, Bagby, Brantigan) 등에 의해 공지되어 왔다. 클로워드는 결손부를 연결하기 위해 그리고 고정시키도록 드릴된 구멍내에 뼈의 다월을 위치시키는 것을 개시하고 있다. 비치는 뼈 다월의 나사선을 개시하고 있다. 베그비는 바스켓의 내부에 그리고 뼈 이식조직에 연통하는 반경방향으로 위치된 구멍의 열을 제외하고 그 표면상에 메탈 버켓내로 뼈 이식조직을 위치시키는 것을 개시하고 있다. 베그비는 말에도 사용할 수 있는 것으로 기술하고 있다. 브랜티겐은 금속으로 제조되고 그 외부 표면에서 뼈의 다공성을 모조한 금속을 가진 비활성 블럭의 사용을 개시하고 있다. 브랜티겐은 뼈 다월이 고정시에 금속 플러그로 완전히 교체되고 그 자체가 작동하지 않지만, 그 둘레를 고정하는 동안에 디스크 공간내로부터 척추를 지지하는 작용을 한다.

브랜티갠(Brantigan)의 미국 특허 제 4,743,256 호(1988년 5월 10일)는 상기 미국 특허 제 3,844,601 호에 개시된 바와 같은 장방형 뼈 이식조직을 이용하는 위치에 장방형 형상의 비활성 인조 스페이서를 사용하는 것을 개시하고 있다.
브랜티갠(Brantigan)의 미국 특허 제 4,878,915 호(1989년 11월 7일)는 신체내 척추 고정시에 사용하기 위한 완전히 원통형 비활성 삽입물을 사용하는 것을 개시하고 있다. 이러한 삽입물은 뼈 고정법에 관련있지만, 비활성 스페이서로서 작용하며, 뼈가 삽입물의 외부 표면으로 성장할 수 있게 한다.
브랜티갠(Brantigan)의 미국 특허 제 4,834,757 호(1989년 5월 30일)는 상기 장방형 뼈 이식조직 또는 브랜티갠의 인조 비활성 스페이서 대신에 사용하기 위한 장방형상의 중공 척추 고정 삽입물을 개시하고 있다.
그러나, 이러한 종래의 모든 삽입물은 환자의 전방 또는 후방으로부터 삽입된다. 결과적으로, 과거의 척추 고정 삽입물은 삽입물이 삽입되는 방향에 대한 추골의 치수의 사이즈로 반드시 제한된다. 예를 들면, 환자의 전방 또는 후방으로부터 삽입된 삽입물에 대한 최대 가능한 길이는 추골의 깊이로 제한되며, 상기 추골의 깊이는 추골의 전방 단부로부터 추골의 후방 단부까지 측정한 추골의 치수이다. 따라서, 환자의 큰 손상부내에서 야기된 척추의 전방부 또는 후방부로부터 돌출한 삽입물과 같은 것으로 추골의 깊이보다 큰 길이를 가진 삽입물을 전방으로부터 후방까지 삽입하는 것을 이전에 불가능하였다.
미켈슨(Michelson)의 미국 특허 제 5,015,247 호에는 2개의 인접한 추골사이의 디스크 공간을 가로질러 삽입하기 위한 원통형의 나사형성 삽입물이 기술되어 있다. 이러한 삽입물은 환자의 전방으로부터 또는 후방으로부터 삽입되는 것으로 기술되어 있으며, 인접한 추골을 결합하도록 디스크 공간보다 큰 직경을 갖고 있다.
환자의 전방 또는 후방으로부터 삽입된 원통형 삽입물에서 가능한 최대 직경은 적어도 2개의 요인에 의해 제한된다. 원통형 삽입물의 직경을 제한하는 제 1 요인은 환자의 전방 또는 후방으로부터 중심에 위치된 단일 삽입물을 이용하는 것으로 제조하고자 할 때 나타난다. 이러한 삽입물은 형태 안정성을 증진시키도록 디스크 공간의 횡폭의 충분한 부분을 차지하기에 충분히 커야 한다. 2개의 인접한 추골을 안정화시키기 위해 디스크 공간에 위치된 삽입물을 사용하는 것은 삽입물이 위치될 때 추골이 삽입물과 추골사이를 연결하는 어떠한 뼈도 아니여야 할 필요가 있다. 단일 삽입물이 디스크 공간의 중심에 사용되면, 추골이 지레받침으로서 작용하는 삽입물상에서 전후로 진동하지 않음으로서 본래 안정성이 발생한다. 그러나, 필요한 안정성을 성취하기 위해서, 가능한 가장넓은 삽입물을 사용할 필요가 있으며, 인접한 추골내로의 이러한 큰 단일 삽입물의 편위가 심해져서 2개의 추골이 실질적으로 절반으로 잘려질 수 있다.
원통형 삽입물의 직경 사이즈를 제한하는 제 2 요인은 단일 삽입물의 문제점을 회피하면서 안정성을 얻기 위한 시도시에 2개의 원통형 삽입물이 환자의 전방 또는 후방으로부터 삽입되고, 디스크 공간을 가로질러 그리고 2개의 인접한 추골내로 나란히 위치되는 상황에서 발생한다. 이러한 삽입물은 인접한 추골내로 관통하도록 그리고 인접한 추골의 각각을 충분히 결합하기에 충분히 크지만, 상기 직경은 나란한 이러한 2개의 삽입물을 위치시키고, 척추의 횡폭내에 삽입물을 더 보유할 수가 없게 한다.
다중 삽입물을 사용하는 것은 삽입물이 동일하게 제한된 척추 폭내로 끼워맞춰지기에 충분히 작을 필요가 있다. 다음에, 척추의 폭내에서 하나 이상을 위치시킬 필요성에 의해 제한된 바와 같은 소경인 삽입물은 추골 뼈의 깊이로만 관통한다.

발명의 요약
본 발명은 척추에 측방관철 접근으로 언급한 것으로 환자의 측면으로부터 삽입된 척추 고정 삽입물을 개시하고 있다. 본 발명의 측방관철 척추 고정 삽입물은 고정할 척추의 횡폭을 가로질러 환자의 척추내로 삽입된다. 추골의 횡폭은 척추의 하나의 외측부로부터 대향 외측부까지 측정된 것이다. 추골의 깊이는 척추의 전방부로부터 후방부까지 측정된다.
본 발명의 측방관철 척추 고정 삽입물이 추골의 횡폭을 따라 또는 추골에 대해 적은 각도로 실질적으로 삽입될 때, 환자의 전방 또는 후방으로부터 삽입하는 척추 삽입물과 비교할 때 상이한 구조적 형상을 갖고 있으며, 이러한 삽입물은 추골의 전방으로부터 후방까지 측정된 깊이에 의해 필수적으로 제한된다.
본 발명의 측방관철 척추 고정 삽입물의 일 실시예에서, 삽입물은 디스크 공간을 가로질러 그리고 인접한 추골내로 끼워맞춰지는 치수로 되어 있다. 이러한 삽입물은 실질적으로 원통형이며, 삽입물을 인접한 추골에 결합하기 위한 뼈 결합 수단을 포함하는 외부 표면을 갖고 있다. 이러한 실시예에서, 요추에 사용하기 위해서 본 발명의 측방관철 척추 고정 삽입물의 길이는 추골의 횡폭의 1/2 보다 크고 추골의 깊이 보다 크다. 본 발명의 측방관철 삽입물의 높이는 2개의 인접한 추골사이의 디스크 공간의 높이보다 큰 높이를 가져서 양 추골을 결합한다. 삽입물의 폭은 추골 자체의 깊이보다 단지 약간 작다.
본 발명의 다른 실시예에서, 본 발명의 측방관철 척추 고정 삽입물은 2개의 인접한 추골사이의 디스크 물질을 제거함으로써 형성된 디스크 공간내에 끼워맞춰지도록 크기가 정해진다. 이러한 삽입물은 측방관철 접근으로부터 척추까지 삽입되며, 추골의 깊이보다 실질적으로 큰 길이와, 추골의 깊이와 유사한 폭을 갖고 있다. 이러한 삽입물의 높이는 2개의 인접한 추골사이의 디스크 공간의 통상 높이와 동일한 높이이며, 해부 척추전만증을 재생하도록 고정될 수 있다. 이러한 삽입물의 상부 및 하부 표면은 디스크 공간 및 인접한 추골 단부판 표면의 형상과 일치하도록 형성될 수도 있다.
본 발명의 측방관철 척추 고정 삽입물의 치수는 단일 삽입물이 단일 절차에 의해 척추내로 삽입될 수 있고 그리고 인접한 추골을 더 결합할 수 있게 한다. 결과적으로, 본 발명의 측방관철 척추 고정 삽입물은 보다 큰 접촉 표면적을 구비하며, 그에 따라 토크에 견딜 수 있는 보다 큰 안정성을 제공하며, 나사산형성 삽입물의 경우에 어떠한 나사산이라도 추골을 관통할 수 있는 깊이를 증가시킨다.
본 발명의 측방관철 삽입물은 대동맥 대정맥 위치 전방으로서 전방 또는 후방으로부터 척추까지 그리고 경막낭 및 신경 후방에서 삽입된 삽입물보다 사용하기에 안전하며, 모든 구조는 측방 접근에 있어서 단순히 회피된다.
본 발명의 측방관철 척추 고정 삽입물은 측방, 전방 또는 후방측부 절개부를 통해 척추의 외측부에 결합된 중공형 튜브를 통해 디스크 공간내로 삽입되어 안전하고 단순한 수술을 가능하게 할 수 있다.
본 발명의 측방관철 척추 고정 삽입물은 척추 고정법에서 삽입물의 능동적 관여를 위해 적어도 부분적으로 고정 촉진 및/또는 생체영향학적 물질을 포함할 수 있다.
본 발명의 목적
본 발명의 목적은 측방관철 접근으로부터 척추에 삽입할 수 있는 척추 고정 삽입물을 제공하는 것이다.
본 발명의 다른 목적은 과거의 삽입물보다 안전하게 사용할 수 있는 척추 고정 삽입물을 제공하는 것이다.

본 발명은 척추 고정 삽입물(spinal fusion implants)에 관한 것이며, 특히 환자의 측면으로부터(측방관철) 척추의 횡폭을 가로질러 그리고 2개의 인접한 추골사이로 삽입하기 위한 척추 고정 삽입물에 관한 것이다.

도 1 내지 도 5를 참조하면, 본 발명의 측방관철 척추 고정 삽입물의 실시예가 참조부호(10)로 표시되어 있다. 척추 고정 삽입물(100)은 고정 촉진 물질을 보유하기 위한 내부 챔버(114)를 둘러싸는 외부벽(112)을 가진 실질적으로 원통형의 형상이다. 척추 고정 삽입물(100)의 외부는 척추의 추골을 결합하기에 적당한 외부 나사산(116)을 포함하여, 디스크 공간을 가로질로 그리고 일단 외과적으로 삽입되면 인접한 추골내로 척추 고정 삽입물(100)을 안정화시킨다. 척추 추골 삽입물(100)은 내부 챔버(114)로 접근할 수 있게 하는 제거가능한 캡(118)을 일단부에 구비하고 있으며, 삽입 기구를 결합하기에 적합한 삽입 단부(120)를 갖고 있다.

외벽(112)은 폐쇄된 우물형상일 수 있는 개구(122)나, 내부 챔버(114)내로 연통하는 개구를 포함하여, 챔버(114)내로 뼈가 성장할 수 있게 한다.
특히 도 2를 참조하면, 척추 고정 삽입물(100)이 척주(S)의 외측부로부터 디스크 공간(D)을 가로질러 인접한 추골(V1및 V2)내로 뚫린 구멍내로 삽입된 척주(S)의 일부분의 전방부의 정면도이다. 척추 고정 삽입물(100)은 인접한 추골(V1및 V2)의 횡폭(W)을 따라 삽입되어, 척추 고정 삽입물(100)은 추골의 하나의 전방부로부터 추골의 대향 외측부까지의 방향에서 측방관철식으로 연장한다.

본 발명의 측방관철 삽입물은 1995년 2월 7일자로 출원되고 발명의 명칭이 척추의 외측부로부터 사람의 흉부 및 요추 질병의 수술 교정을 위한 방법 및 기구인 미국 특허 출원 제 08/394,836 호에 기술된 측방관철 방법에 의해 삽입된다. 상기 특허 출원의 내용은 참고로 본원에 인용한다.
도 4를 참조하면, 삽입물(100a) 및 추골(V1)의 접촉 영역을 도시하기 위해 도 3의 4-4 선을 따라 취한 상부 단면도가 도시되어 있다. 추골(V1)은 척추의 전방으로부터 후방부까지 측정한 깊이(D)와, 추골(V1)의 하나의 외측부로부터 대향 외측부까지 측정된 횡폭(W)을 갖고 있다. 삽입물(100a)은 추골(V1)의 깊이(D)보다 실질적으로 큰 길이(L)를 갖고 있어서, 삽입물(100a)은 추골(V1)의 횡폭(W)을 가로질러 실질적으로 연장할 수 있다. 바람직한 실시예에 있어서, 삽입물(100a)은 추골의 횡폭(W)의 1/2 보다 크며, 추골(V1)의 깊이(D)와 유사한 충분히 큰 사이즈의 직경을 갖고 있다. 삽입물(100a)의 큰 길이 및 직경으로 인해서, 삽입물(100a)과 추골(V1)사이의 큰 접촉 표면적은 매우 안정된 구조를 형성할 수 있다. 삽입물(100a)은 척추의 전방 또는 후방으로부터 삽입된 삽입물에서 이전에 가능한 것보다 매우 큰 추골(V1)과의 접촉 표면적을 갖고 있다.
본 발명의 배경에서 상술한 바와 같이, 환자의 전방 또는 후방으로부터 중심에 위치된 단일 삽입물은 추골의 횡폭(W)의 충분한 부분을 차지하기에 충분히 커서 형태 안정성을 증진시킬 수 있어야 한다. 그러나, 이러한 삽입물과 인접한 추골내로의 연장부의 수직 높이는 절단될 수 있어서, 어떤 2개의 연속적인 디스크 공간이 영향을 미친다면, 디스크 공간사이의 추골은 절반으로 절단될 것이다. 따라서, 추골의 중심선(중간-봉합축)의 각 측면상에 하나씩 다중 삽입물을 사용할 수 있다. 도 5를 참조하면, 요추(S)의 일부분의 전방 정면도가 도시되어 있으며, 2개의 원통형 삽입물(150, 152)은 척추(S)의 전방부로부터 동일한 디스크 공간(D)을 가로질러 드릴된 구멍내로 그리고 2개의 인접한 추골(V1및 V2)내로 삽입되어 있다.
도 6을 참조하면, 척추 및 추골(V1)의 전방부로부터 삽입된 2개의 삽입물(150, 152)사이의 접촉 영역을 도시하는 것으로 도 5의 6-6 선의 단면도가 도시되어 있다. 도 6에서 볼 수 있는 바와 같이, 추골(V1)과의 접촉하는 2개의 척추 삽입물(150, 152)의 표면적은 추골(V1)의 횡폭(W)을 가로질러 삽입된 단일 측방관철 척추 고정 삽입물(100)의 표면적보다 실질적으로 작다. 결과적으로, 본 발명의 측방관철 척추 고정 삽입물(100)은 환자의 전방 또는 후방으로부터 삽입된 이전에 가능한 삽입물보다 보다 안정한 구조를 이루고 있어서 고정 구조체의 안정성을 증진시킨다.
바람직한 실시예에서, 본 발명의 척추 고정 삽입물(100)이 요추에 삽입될 때에 있어서 삽입물의 전장의 범위는 35㎜ 내지 50㎜이며, 보다 바람직하게는 38㎜ 내지 44㎜이며, 삽입물의 최대 직경 범위는 22㎜ 내지 30㎜이며, 보다 바람직하게는 24㎜ 내지 26㎜이다. 흉추에 있어서, 이러한 삽입물의 길이 범위는 12㎜ 내지 30㎜이며, 삽입물의 최대 직경 범위는 14㎜ 내지 26㎜이며, 보다 바람직하게는 20㎜이다.
도 7을 참조하면, 본 발명의 측방관철 고정 삽입물의 다른 실시예이며 단일 추골(V1)(199)의 전방 사시도가 도시되어 있다. 척추 고정 삽입물(199)은 디스크 공간을 가로질러 드릴된 구멍내로 그리고 추골(V1))의 횡폭을 따라 추골(V1)내로 삽입된 다월이다. 척추 고정 삽입물(199)의 치수는 상술한 척추 고정 삽입물(100)과 치수가 동일하다. 척추 고정 삽입물(199)은 사람에게 이식하기에 적당한 물질로 제조될 수 있으며, 척추 고정법에 능동적으로 관여하는 고정 증진 및/또는 생체에 작용하는 물질을 포함할 수 있다. 삽입물(199)은 다공성, 및/또는 메시형 및/또는 망상조직 물질, 또는 상술한 목적에 적당한 어떠한 다른 물질로 제조될 수 있다.
도 8을 참조하면, 본 발명의 측방관철 척추 고정 삽입물(200)의 다른 실시예가 도시되어 있다. 척추 고정 삽입물(200)은 내부 챔버(214)를 둘러싸는 얇은 외부 벽(212)을 가진 실질적으로 원통형 형상을 갖고 있다. 척추 고정 삽입물(200)의 외부는 디스크 공간을 가로질러 그리고 일단 외과적으로 삽입되면 인접한 추골내로 척추 고정 삽입물(200)을 안정화하도록 추골의 뼈를 결합하기에 적당한 표면을 제공하는 거친 표면을 포함한다. 거친 표면은 척추 고정 삽입물(200)의 원주를 따라 다수의 래치(220)가 형성되어 있다. 다수의 래치(220)의 각각은 뼈 결합 에지(222) 및 각진 세그먼트(224)를 갖고 있다.
척추 고정 삽입물(200)은 디스크 공간을 가로질러 시작된 원통형 뼈내로 그리고 2개의 인접한 추골내로 이식된다. 척추 고정 삽입물(200)은 나사선이 없고 삽입물을 척추를 통해 전방으로 밀기 때문에 직접 선형 전진에 의해 디스크 공간을 가로질러 원통형 뼈내로 밀려질 수 있다. 척추 고정 삽입물(200)을 전진시키는데 토크가 필요없을 경우는 최소 필수 높이의 표면 거칠기도 없다.
래치(220)는 척추 고정 삽입물(200)이 삽입되는 한 방향에서 접하며, 일단 2개의 인접한 척추사이에 삽입되면 척추 고정 삽입물(200)이 삽입 방향에 반대 방향으로 디스크 공간을 벗어나 빠지는 것을 방지하는 기능을 한다. 래치(220)는 추골의 비제거된 뼈에 대항하여 척추 고정 삽입물(200)을 전방으로 가압한다. 삽입물은 이들이 삽입된 동일한 경로를 따라 다시 빠져나갈려고 하기 때문에, 래치(220)는 원통형 뼈의 단부에서 입방의 비제거된 뼈에 대항하여 척추 고정 삽입물(200)을 가압하는 경향이 있으며, 제거되는 것에 저항하며, 대단히 안정된 이식시에 발생되는 운동을 제어한다.
척추 고정 삽입물(200)은 삽입물(200) 및 드라이버 기구를 함께 밀접하게 결합하고 묶기 위해 드라이버 기구를 결합하기 위한 맞물림 수단을 일 단부에 구비하고 있다. 삽입물 드라이버 기구에 일단 고정되면, 척추 고정 삽입물(200)은 중공 원통형 튜브를 통해 도입되고, 디스크 공간을 가로질러 드릴된 원통형 구멍내로 박힌다. 삽입물 드라이버 기구는 멜릿(mallet) 또는 유사한 장치에 의해서 충돌되어 디스크 공간을 가로질러 척추 고정 삽입물(200)을 선형으로 전진시킬 수 있다. 일단 척추 고정 삽입물(200)이 디스크 공간을 가로질러 삽입되면, 래치(220)는 추골의 뼈와 맞물리고, 삽입물 드라이버 기구는 척추 고정 삽입물(200)로부터 분리된다.
도 9를 참조하면, 본 발명의 척추 고정 삽입물(300)의 다른 실시예가 도시되어 있다. 척추 고정 삽입물(300)은 추간 공간(D)내의 삽입물(300)을 안정화시키기 위한 거친 표면을 가진 실질적으로 원통형 형상을 갖고 있다. 거친 표면은 이에 의해 제한되지는 않지만 도 9에 도시된 다이아몬드형상 뼈 결합 패턴과 같은 널링가공된 표면(320)을 포함한다. 척추 고정 삽입물(300)은 본 발명의 정신을 벗어남이 없이 척추 고정 삽입물(300)의 전체 외부 표면에 걸쳐, 외부 표면중 단지 일부분에 걸쳐 또는 이들의 조합을 통해 널링가공된 표면(320)을 구비할 수 있다. 삽입물을 더 전방으로 전진시키는데 저항하도록 척추 고정 삽입물(300)의 전방의 디스크 공간내에 비드릴된 뼈가 없는 상황에서, 널링가공된 표면(320)은 추골의 뼈와 대단히 높은 간섭 끼워맞춤을 형성하며, 모든 방향에서 그리고 그 자체를 전방으로 가압하는 경향이 없이 동일하게 운동에 저항한다.

도 13을 참조하면, 본 발명의 척추 고정 삽입물(700)의 다른 실시예가 도시되어 있다. 척추 고정 삽입물(700)은 평평한 측면(706)상의 수평 슬롯(728)과, 척추 고정 삽입물(700)의 원통형 부분상의 수직 슬롯(729)의 형태인 개구부를 구비하는 것을 제외하고는 상술한 척추 고정 삽입물(400)과 유사하다. 척추 고정 삽입물(700)은 상술한 래치(220)와 유사한 추골의 뼈를 결합하기 위한 래치(72)를 갖고 있다.
본 발명의 척추 고정 삽입물은 삽입물 및 추골의 표면적 또는 간섭 끼워맞춤을 향상시키는 어떤 그리고 모든 거친 표면을 포함할 수 있다. 또 본 발명의 척추 고정 삽입물의 변형 실시예에 대한 상술한 래치는 래치와 조합하는 널링가공된 또는 다른 거친 표면을 포함하여, 일단 삽입되면 디스크 공간을 가로질러 척추 고정 삽입물의 억제를 더 향상시킬 수 있다.

도 16을 참조하면, 본 발명의 척추 고정 삽입물(900)의 다른 실시예가 도시되어 있다. 척추 고정 삽입물(900)은 척추의 인접한 추골사이의 디스크 공간(D)내로 삽입되지만 디스크 공간을 가로질러 형성된 원통형 보어내로는 삽입되지 않은 것이 상술한 삽입물과는 상이하다. 따라서, 척추 고정 삽입물(900)은 척추 고정 삽입물(900)이 인접한 추골사이의 본래 디스크 공간내에 끼워맞춰질 때 인접한 추골로부터 뼈의 일부분을 제거할 필요가 없다. 그러나, 인접한 추골사이에 존재하는 디스크 물질의 적어도 일부분을 제거하는 것은 적당한 삽입을 위해 필요하다.
척추 고정 삽입물(900)은 인접한 추골을 결합하기 위한 상부 표면(902) 및 바닥 표면(904)을 가진 장방형 블럭(901)을 포함하며, 적어도 부분적으로 평평하거나 같은 모양이 될 수 있다. 상부 표면(902) 및 바닥 표면(904)은 인접한 추골의 뼈를 결합하기 위한 설명한 어떠한 거친 표면이라도 포함하여 형태 안정성을 증진시킨다. 척추 고정 삽입물(900)은 입방체이거나 적어도 부분적으로 중공일 수 있으며, 다수의 개구(906)를 구비하여 뼈의 내방성장을 허용한다. 개구(906)는 삽입물(900)의 모든 표면상에 존재할 수 있으며, 전체 삽입물(900)을 통해 통과하거나 고정 촉진 물질을 보유하기 위한 폐쇄된 바닥 우물형상일 수 있다.
도 17을 참조하면, 척추의 외측부로부터 인접한 추골(V1및 V2)의 횡폭을 따라 2개의 인접한 추골(V1및 V2)사이의 디스크 공간(D)내에 삽입된 척추 고정 삽입물(900)이 도시되어 있다. 척추 고정 삽입물(900)은 디스크 공간(D)의 높이와 실질적으로 동일한 높이와, 추골의 횡폭(W)의 1/2보다 큰 길이와, 추골의 깊이와 유사한 폭을 갖고 있다.
바람직한 실시예에서, 척추 고정 삽입물(900)은 8㎜ 내지 16㎜ 범위, 바람직하게는 10㎜ 내지 12㎜ 범위의 높이와, 24㎜ 내지 32㎜ 범위, 바람직하게는 26㎜의 폭과, 32㎜ 내지 50㎜ 범위, 바람직하게는 42㎜의 길이를 갖고 있다.
도 18을 참조하면, 본 발명의 척추 고정 삽입물(1000)의 다른 실시예가 도시되어 있다. 척추 고정 삽입물(1000)은 척추 고정 삽입물(900)과 유사하지만, 폭이 보다 좁으며, 하나 이상의 척추 고정 삽입물(1000)이 인접한 추골사이의 디스크 공간(D)내에 삽입시키기 위해 모듈 형상으로 조합될 수 있다.
도 19를 참조하면, 다수의 척추 고정 삽입물(1000)은 추골(V1및 V2)의 횡폭을 따라 척추의 외측부로부터 디스크 공간(D)내에 삽입된 모듈 형상으로 조합되어 도시되어 있다.
도 20을 참조하면, 본 발명의 척추 고정 삽입물(1100)의 다른 실시예가 도시되어 있다. 척추 고정 삽입물(1100)은 추골의 횡폭을 따라 척추의 외측부로부터 2개의 인접한 추골사이의 디스크 공간내로 삽입된다. 삽입물(1100)은 2개의 인접한 추골사이에 존재하는 천연 디스크 물질을 교체하기 위한 크기로 되어 있다. 삽입물(1100)은 구부러진 측면(1102, 1104)을 가진 대체로 장방형 본체를 갖고 있다. 상부 및 바닥 표면(1106, 1108)은 상술한 스플라인(810)과 구조 및 기능이 유사한 다수의 스플라인(1110)을 구비하고 있다. 삽입물(1100)이 디스크 공간내에 삽입될 때, 스플라인(1100)은 인접한 추골의 뼈를 결합한다.
삽입물(1100)은 삽입물(1100)의 외부 표면에 개구(1112) 및 슬롯(1114)을 구비한 중공형이여서, 삽입물(1100)의 내부내로 뼈의 내방성장을 허용하게 도시되어 있다. 그러나, 삽입물(1100)은 입방일 수 있으며, 개구(1112) 대신에 채널 또는 우물형상을 가질 수 있어서, 척추 고정 부분내로 뼈의 내방성장 및 삽입물(1100)의 합체를 허용한다. 삽입물(1000)의 내부는 스냅 끼워맞춤 커버로 폐쇄될 수 있는 구멍(1120)을 통해 접근할 수 있다.
본 발명은 바람직한 실시예와 관련해서 상세하게 설명하였지만, 본 발명의 다른 변형예가 본 발명의 개념을 벗어남이 없이 이루어질 수 있다.

Claims (25)

  1. 척추의 외측부로부터 2개의 인접한 추골사이의 디스크 공간내로 삽입하기 위한 측방관철 척추 고정 삽입물(a translateral spinal fusion implant)로서, 상기 삽입물은 추골의 횡폭의 1/2보다 실질적으로 크고 그리고 추골의 깊이보다 실질적으로 큰 길이와, 상기 디스크 공간과 실질적으로 동일한 높이를 갖고 있는 측방관철 척추 고정 삽입물.
  2. 제 1 항에 있어서,
    상기 삽입물은 디스크 공간보다 큰 높이를 갖고, 상기 추골을 결합할 수 있는 측방관철 척추 고정 삽입물.
  3. 제 2 항에 있어서,
    상기 삽입물은 형상에 있어서 적어도 부분적으로 원통형인 측방관철 척추 고정 삽입물.
  4. 제 1 항에 있어서,
    상기 삽입물은 상기 2개의 인접한 추골을 결합하고, 상기 삽입물을 제위치에 유지하기 위한 거친 표면을 구비하며, 상기 거친 표면은 상기 삽입물의 외부의 적어도 일부분상에 존재하는 측방관철 척추 고정 삽입물.
  5. 제 4 항에 있어서,
    상기 거친 표면은 다수의 래치를 포함하는 측방관철 척추 고정 삽입물.
  6. 제 5 항에 있어서,
    상기 래치는 한 방향을 향한 측방관철 척추 고정 삽입물.
  7. 제 4 항에 있어서,
    상기 거친 표면은 널링가공된 측방관철 척추 고정 삽입물.
  8. 제 4 항에 있어서,
    상기 거친 표면은 다수의 종방향 스플라인을 포함하는 측방관철 척추 고정 삽입물.
  9. 제 3 항에 있어서,
    상기 삽입물은 상기 추골을 결합하기 위한 외부 나사산을 구비하는 측방관철 척추 고정 삽입물.
  10. 제 3 항에 있어서,
    상기 삽입물은 중공형인 측방관철 척추 고정 삽입물.
  11. 제 7 항에 있어서,
    상기 삽입물은 상기 삽입물의 상기 중공형 부분을 제거가능하게 폐쇄하기 위해 적어도 일단부상에 캡을 구비하고 있는 측방관철 척추 고정 삽입물.
  12. 제 1 항에 있어서,
    고정 촉진 물질을 보유할 수 있는 다수의 개구를 구비하는 측방관철 척추 고정 삽입물.
  13. 제 1 항에 있어서,
    상기 삽입물의 일단부는 상기 삽입물을 삽입하기 위한 기구를 결합하기 위한 결합 수단을 포함하는 측방관철 척추 고정 삽입물.
  14. 제 1 항에 있어서,
    상기 삽입물은 고정 촉진 물질을 포함하는 측방관철 척추 고정 삽입물.
  15. 제 1 항에 있어서,
    상기 삽입물은 적어도 부분적으로 생체흡수성인 측방관철 척추 고정 삽입물.
  16. 제 1 항에 있어서,
    상기 삽입물은 내부 챔버와, 이 내부 챔버에 접근하기 위한 접근 개구를 구비하는 측방관철 척추 고정 삽입물.
  17. 제 16 항에 있어서,
    상기 삽입물은 상기 접근 개구를 폐쇄하기 위한 수단을 측방관철 척추 고정 삽입물.
  18. 제 16 항에 있어서,
    상기 내부 챔버는 고정 촉진 물질을 보유할 수 있는 측방관철 척추 고정 삽입물.
  19. 제 16 항에 있어서,
    상기 삽입물은 상기 내부 챔버를 적어도 부분적으로 둘러싸는 벽을 포함하는 측방관철 척추 고정 삽입물.
  20. 제 19 항에 있어서,
    상기 벽은 상기 내부 챔버와 연통시에 이를 통해 통과하는 다수의 개구를 구비하는 측방관철 척추 고정 삽입물.
  21. 제 1 항에 있어서,
    상기 삽입물은 2개의 인접한 추골사이의 디스크 공간내에 상기 삽입물을 삽입하기 위한 구동 기구를 결합하기 위한 구동 결합 수단을 포함하는 측방관철 척추 고정 삽입물.
  22. 제 1 항에 있어서,
    상기 삽입물은 그 형상에 있어서 대체로 장방형인 측방관철 척추 고정 삽입물.
  23. 제 1 항에 있어서,
    상기 삽입물은 그 형상에 있어서 대체로 사각형인 측방관철 척추 고정 삽입물.
  24. 제 1 항에 있어서,
    상기 삽입물은 다수의 모듈 부재를 포함하며, 상기 모듈 부재의 각각은 추골의 횡폭의 1/2보다 실질적으로 크고 그리고 추골의 깊이보다 실질적으로 큰 길이와, 2개의 인접한 추골사이로부터 제거된 디스크 물질에 의해 형성된 공간의 높이와 실질적으로 동일한 높이와, 상기 추골의 깊이보다 실질적으로 작은 폭을 갖고 있는 측방관철 척추 고정 삽입물.
  25. 척추의 외측부로부터 2개의 인접한 추골사이의 디스크 공간내로 삽입하기 위한 측방관철 척추 고정 삽입물에 있어서,
    다수의 모듈 부재를 포함하며, 상기 모듈 부재의 각각은 상기 추골의 횡폭의 1/2보다 실질적으로 크고 그리고 추골의 깊이보다 실질적으로 큰 길이와, 상기 추골의 깊이보다 실질적으로 작은 폭과, 상기 디스크 공간과 실질적으로 동일한 높이를 갖고 있으며,
    상기 모듈 부재의 각각은 상부벽, 하부벽 및 측벽을 포함하며, 상기 상부벽 및 하부벽은 2개의 인접한 추골의 단부 플레이트에 대해 지지하기 위해 상기 상부벽 및 하부벽의 내부 표면중 적어도 일부분을 포함하는 지지 구조체를 형성하며,
    이에 의해 상기 다수의 모듈 부재는 상기 2개의 인접한 추골사이에 삽입될 수 있는 측방관철 척추 고정 삽입물.
