Abstract
본 발명은 항-VEGFR-2 항체를 포함하는 약학 조성물에 관한 것이다. 본 발명의 약학 조성물은 뇌 부종을 치료, 예방 또는 개선할 수 있다. 또한, 본 발명의 약학 조성물은 뇌 부종을 치료, 예방 또는 개선하기 위해 스테로이드제를 투여받는 환자에게 있어서 스테로이드제의 투여량 및/또는 투여기간을 감소시킬 수 있다. 또한, 본 발명의 약학 조성물은 뇌 부종 또는 교모세포종을 갖는 환자의 생존기간을 유의적으로 연장시킬 수 있다.
Description
본 발명은 항-VEGFR-2 항체를 포함하는 약학 조성물에 관한 것이다. 본 발명의 약학 조성물은 뇌 부종을 치료, 예방 또는 개선할 수 있다. 또한, 본 발명의 약학 조성물은 뇌 부종을 치료, 예방 또는 개선하기 위해 스테로이드제를 투여받는 환자에게 있어서 스테로이드제의 투여량 및/또는 투여기간을 감소시킬 수 있다. 또한, 본 발명의 약학 조성물은 뇌 부종 또는 교모세포종을 갖는 환자의 생존기간을 유의적으로 연장시킬 수 있다.
뇌 부종은 뇌종양, 교모세포종 등에 수반되는 증상 중의 하나로서, 혈액뇌관문(blood brain barrier)이 붕괴되고 세포외 공간에 체액이 축적되어 나타나는 혈관성 부종이다. 또한, 뇌 부종은 외상성 뇌 손상, 뇌내 출혈, 뇌졸중, 허혈성 뇌졸중, 뇌염, 뇌수막염, 바이러스 또는 박테리아에 의한 뇌 감염 등에 기인하여 발생하는 사례도 보고되고 있다.
뇌 부종은 현기증과 두통 같은 다양한 신경학적 증상을 야기하며, 나아가 뇌압을 상승시켜 사망에까지 이르게 하므로 그 심각성은 매우 크다. 또한, 암 환자에게 있어서 암의 전이를 막기 위해 림프절 제거 수술이 행해지면 부종이 발생하는 경우가 많으며, 이는 암 환자의 삶의 질을 크게 저하시키는 결과를 가져온다.
이와 같은 뇌 부종을 완화 또는 감소시키기 위해 주로 사용되는 약물에는 삼투압제제, 이뇨제, 글리세롤, 스테로이드제 등이 있다.
상기 삼투압제제의 대표적인 예는 만니톨이다. 그러나, 만니톨을 장기간 투여할 경우, 전해질 이상, 혈청 삼투농도 증가 등이 발생하고 심폐부전, 신부전 및 다양한 신경계 문제를 야기할 수 있다.
상기 이뇨제는 혈중 삼투농도를 증가시킴으로써 뇌 부종을 호전시킨다는 것이 동물 실험 모델에서 입증되었다는 보고는 있으나, 아직까지 임상적 연구에 대한 보고는 없다.
상기 글리세롤은 뇌혈류를 개선시키고, 뇌경색 동물 모델에서 부종을 감소시킨다는 효과가 보고되었다. 그러나, 글리세롤을 개체에게 빠르게 주입할 경우, 용혈, 혈색소요증, 신부정 등이 야기된다는 문제점이 있다.
상기 스테로이드제와 관련하여, 최근 연구에서 코르티코스테로이드는 VEGF(vascular endothelial growth factor) 경로를 억제하여 종양 혈관의 투과성을 감소시키고 이에 따라 부종을 억제할 수 있다는 점이 보고되고 있다. 이에 따라, 혈관성 부종을 억제하기 위한 스테로이드제로서 코르티코스테로이드가 주로 사용되고 있다.
상기 코르티코스테로이드에는 덱사메타손, 코르티솔, 프레드니손, 프레드니솔론, 메틸프레드니솔론, 트리암시놀론, 코르티코스테론, 데소니드 등이 포함된다. 이 중에서, 덱사메타손은 다른 코르티코스테로이드에 비해 효과가 더 우수하고 반감기가 길다는 측면에서 자주 사용되고 있다. 그러나, 덱사메타손은 고용량으로 투여될 경우 또는 장기간 투여될 경우, 근육량 상실, 골절 위험 증가, 당뇨병과 고혈압 유발, 신체 면역기능 약화 등 다양한 부작용이 용량 의존적으로 나타난다는 측면에서 문제점을 갖고 있다.
상기에서 살펴본 바와 같이, 뇌 부종을 치료 또는 개선하기 위한 적절한 방안이 아직까지 없는 실정이며, 또한 뇌 부종을 완화시키기 위해 주로 사용되는 덱사메타손과 같은 스테로이드제 역시 용량 의존적으로 심각한 부작용을 나타낸다는 점에서 그 적용이 제한된다.
본 발명자들은 종래 기술의 문제점을 해결하기 위하여 예의 노력한 결과, 본 발명에 따른 항체가 뇌 부종을 유의적으로 치료 또는 개선할 수 있으며, 뇌 부종의 치료, 예방 또는 개선을 위해 투여되는 스테로이드제의 투여량 및/또는 투여기간을 유의적으로 감소시킬 수 있다는 것을 확인하여, 본 발명을 완성하게 되었다.
상기 목적을 달성하기 위해, 본 발명의 일 양태는 항-VEGFR-2 항체를 유효성분으로 포함하는 약학 조성물을 제공한다.
본 명세서에서 사용되는 용어 "항체"는 면역학적으로 특정 항원과 반응성을 갖는 면역글로불린 분자 또는 항원을 특이적으로 인식하는 수용체 역할을 하는 단백질 분자를 의미한다. 따라서, 본 발명에서 "항체"는 광범위한 의미로 사용되며, 다클론항체, 단일클론항체, 전장(whole) 항체(디설파이드 결합에 의해 상호연결된 적어도 2개의 중쇄 및 2개의 경쇄로 이루어진 항체) 및 항체 단편을 모두 포함하는 것으로 해석된다. 상기 전장 항체는 IgA, IgD, IgE, IgM 및 IgG를 포함한다. 또한 상기 IgG는 아형(subtype)으로서 IgG1, IgG2, IgG3 및 IgG4를 포함할 수 있다. 상기 용어 "항체 단편"은 모 항체의 결합 특이성을 적어도 일부 보유하고, 모 항체의 항원 결합 영역의 일부(예를 들면, 하나 이상의 CDR) 또는 가변 영역을 포함하는 항원-결합 단편 또는 항체의 유사체를 의미한다. 상기 항체 단편은 예를 들면 Fab, Fab', F(ab')2, Fv 단편, scFv, 유니바디(unibody), 디아바디, 선형 항체, 나노바디, 도메인 항체 또는 항체 단편으로부터 형성된 다특이적 항체일 수 있다.
본 명세서에서 사용되는 용어 "VEGFR"은 혈관 내피 성장인자(vascular endothelial growth factor)의 수용체를 의미하며, 여기에는 VEGFR-1, VEGFR-2 및/또는 VEGFR-3이 포함된다. 혈관내피세포의 생장 활성을 증진시키는 VEGF의 생물학적 기능은 VEGF에 대하여 고친화성을 가지는 VEGF 수용체를 통하여 매개되는데, 상기 VEGF 수용체는 종양 형성 동안 내피세포에서 선택적으로 발현된다.
본 명세서에서 사용되는 용어 "항-VEGFR-2 항체"는 VEGF 수용체의 서브 타입 중 VEGFR-2에 특이적으로 결합하여 VEGFR-2의 활성을 중화 또는 억제하는 항체를 의미한다. 구체적으로, 상기 항-VEGFR-2 항체는 인간 VEGFR-2(KDR)를 표적으로 하면서 동시에 마우스나 랫트 유래의 flk-1에 대해서도 반응성을 갖는, 인간 항체일 수 있다.
본 명세서에서 사용되는 용어 "인간 항체"는 프레임워크 및 CDR 영역이 인간 이뮤노글로불린 서열로부터 유래된 가변 영역을 갖는 항체를 의미하는 것으로 "완전 인간 항체"라고도 지칭된다. 본 발명에서 인간 항체는 인간 유래 면역글로불린 서열에 의해 코딩되지 않는 아미노산 잔기(예를 들어, 시험관내 무작위 또는 부위-특이적 돌연변이유발에 의해 또는 생체내 체세포 돌연변이에 의해 도입된 돌연변이)를 포함할 수 있다.
본 발명에 따른 일 구체예에서, 상기 항-VEGFR-2 항체는 서열번호 1의 경쇄 CDR1, 서열번호 2의 경쇄 CDR2, 및 서열번호 3의 경쇄 CDR3을 포함하는 경쇄가변영역과 서열번호 4의 중쇄 CDR1, 서열번호 5의 중쇄 CDR2, 및 서열번호 6의 중쇄 CDR3을 포함하는 중쇄가변영역을 포함하는 항체일 수 있다. 또한, 상기 항-VEGFR-2 항체는 바람직하게는 서열번호 7의 경쇄가변영역 및 서열번호 8의 중쇄가변영역을 포함하는 항체일 수 있다. 또한, 상기 항-VEGFR 항체는 보다 바람직하게는 TTAC-0001 항체이거나, 또는 TTAC-0001 항체가 결합하는 에피토프와 동일한 에피토프에 특이적으로 결합하는 항체일 수 있다. 상기 TTAC-0001 항체는 본 출원인의 국제출원 WO 제2008/153237호(그 내용 전부는 본 명세서에 참조로 삽입됨) 등에 기재되어 있으며, 이는 타니비루맵 또는 TANIBIRUMAB라고도 명명된다.
본 발명의 약학 조성물은 뇌 부종을 치료, 예방 또는 개선하기 위해 사용된다. 상기 뇌 부종은 뇌종양, 교모세포종, 외상성 뇌 손상, 뇌내 출혈, 뇌졸중, 허혈성 뇌졸중, 뇌염, 뇌수막염, 바이러스 또는 박테리아에 의한 뇌 감염 등 하나 이상의 요인에 기인한 것일 수 있으며, 이에 제한되는 것은 아니다. 상기 뇌 부종은 종양주변 부종(peritumoral edema)일 수 있다.
본 발명에서 사용되는 용어 "치료"는 본 발명에 따른 조성물의 투여에 의해 뇌 부종의 증상이 호전 또는 완치되거나 뇌 부종의 크기가 감소되는 모든 행위를 의미한다.
본 발명에서 사용되는 용어 "예방"은 본 발명에 따른 조성물의 투여에 의해 뇌 부종이 억제 또는 지연되는 모든 행위를 의미한다.
본 발명에서 사용되는 용어 "개선"은 본 발명에 따른 조성물의 투여에 의해 뇌 부종의 크기 또는 정도가 감소되거나 호전되거나 진행이 지연되는 모든 행위를 의미한다.
본 발명에 따른 약학 조성물은 상기 항-VEGFR 항체를 단독으로 함유하거나 또는 하나 이상의 약학적으로 허용되는 담체, 부형제 또는 희석제를 추가로 함유할 수 있다.
상기 약학적으로 허용되는 담체로는 예컨대, 경구 투여용 담체 또는 비경구 투여용 담체를 추가로 포함할 수 있다. 경구 투여용 담체는 락토스, 전분, 셀룰로스 유도체, 마그네슘 스테아레이트, 스테아르산 등을 포함할 수 있다. 또한, 비경구 투여용 담체는 물, 적합한 오일, 식염수, 수성 글루코스 및 글리콜 등을 포함할 수 있으며, 안정화제 및 보존제를 추가로 포함할 수 있다. 적합한 안정화제로는 아황산수소나트륨, 아황산나트륨 또는 아스코르브산과 같은 항산화제가 있다. 적합한 보존제로는 벤즈알코늄 클로라이드, 메틸- 또는 프로필-파라벤 및 클로로부탄올이 있다. 그 밖의 약학적으로 허용되는 담체로는 다음의 문헌에 기재되어 있는 것을 참고로 할 수 있다(Remington's Pharmaceutical Sciences, 19th ed., Mack Publishing Company, Easton, PA, 1995).
본 발명의 약학 조성물은 인간을 비롯한 포유동물에게 임의의 방법으로 투여할 수 있다. 예를 들면, 경구 또는 비경구적으로 투여할 수 있다. 비경구적인 투여방법으로는 이에 한정되지는 않으나, 정맥내, 근육내, 동맥내, 골수내, 경막내, 심장내, 경피, 피하, 복강내, 비강내, 장관, 국소, 설하 또는 직장내 투여일 수 있다. 예컨대, 본 발명의 약학 조성물을 주사형 제형으로 제조하여 이를 30 게이지의 가는 주사 바늘로 피부를 가볍게 단자(prick)하는 방법, 또는 피부에 직접적으로 도포하는 방법으로 투여될 수 있다.
본 발명의 약학 조성물은 상술한 바와 같은 투여 경로에 따라 경구 투여용 또는 비경구 투여용 제제로 제형화 할 수 있다.
경구 투여용 제제의 경우에 본 발명의 조성물은 분말, 과립, 정제, 환제, 당의정제, 캡슐제, 액제, 겔제, 시럽제, 슬러리제, 현탁액 등으로 당업계에 공지된 방법을 이용하여 제형화될 수 있다. 예를 들어, 경구용 제제는 활성성분을 고체 부형제와 배합한 다음 이를 분쇄하고 적합한 보조제를 첨가한 후 과립 혼합물로 가공함으로써 정제 또는 당의정제를 수득할 수 있다. 적합한 부형제의 예로는 락토즈, 덱스트로즈, 수크로즈, 솔비톨, 만니톨, 자일리톨, 에리스리톨 및 말티톨 등을 포함하는 당류와 옥수수 전분, 밀 전분, 쌀 전분 및 감자 전분 등을 포함하는 전분류, 셀룰로즈, 메틸 셀룰로즈, 나트륨 카르복시메틸셀룰로오즈 및 하이드록시프로필메틸-셀룰로즈 등을 포함하는 셀룰로즈류, 젤라틴, 폴리비닐피롤리돈 등과 같은 충전제가 포함될 수 있다. 또한, 경우에 따라 가교결합 폴리비닐피롤리돈, 한천, 알긴산 또는 나트륨 알기네이트 등을 붕해제로 첨가할 수 있다. 나아가, 본 발명의 약학 조성물은 항응집제, 윤활제, 습윤제, 향료, 유화제 및 방부제 등을 추가로 포함할 수 있다.
비경구 투여용 제제의 경우에는 주사제, 크림제, 로션제, 외용연고제, 오일제, 보습제, 겔제, 에어로졸 및 비강 흡입제의 형태로 당업계에 공지된 방법으로 제형화할 수 있다. 이들 제형은 모든 제약 화학에 일반적으로 공지된 처방서인 문헌(Remington's Pharmaceutical Science, 15th Edition, 1975. Mack Publishing Company, Easton, Pennsylvania 18042, Chapter 87: Blaug, Seymour)에 기재되어 있다.
본 발명의 약학 조성물의 총 유효량은 단일 투여량(single dose)으로 환자에게 투여될 수 있으며, 다중 투여량(multiple dose)으로 장기간 투여되는 분할 치료 방법(fractionated treatment protocol)에 의해 투여될 수 있다. 바람직하게는 본 발명의 조성물의 전체 용량은 1일당 환자 체중 1 ㎏ 당 약 0.01 μg 내지 1,000 mg, 가장 바람직하게는 0.1 μg 내지 100 mg일 수 있다. 그러나 상기 본 발명의 약학 조성물의 용량은 투여 경로 및 치료 횟수 뿐만 아니라 환자의 연령, 체중, 건강 상태, 성별, 질환의 중증도, 식이 및 배설율 등 다양한 요인들을 고려하여 환자에 대한 유효 투여량을 고려하여, 당 분야의 통상적인 지식을 가진 자가 적절한 유효 투여량을 결정할 수 있을 것이다. 본 발명에 따른 약학 조성물은 본 발명의 효과를 보이는 한 그 제형, 투여 경로 및 투여 방법에 특별히 제한되지 아니한다. 본 발명의 약학 조성물은 뇌 부종의 증상에 따라 유효성분, 즉 본 발명에 따른 항-VEGFR-2 항체의 함량을 달리 할 수 있다. 바람직하게는, 본 발명에 따른 항-VEGFR-2 항체는 매주 1회 환자 체중 1 kg 당 약 8 mg 내지 24 mg 범위의 양으로 본 발명에 따른 조성물 내에 함유되어 제형화될 수 있다. 매주 1회 환자 체중 1 kg 당 8 mg 또는 12 mg 양의 본 발명에 따른 항-VEGFR-2 항체는 투여받은 대상체의 부종 크기를 유의적으로 감소시킬 수 있는 것으로 후술하는 실시예에서 입증되었으며, 부종 크기 감소 효과를 늘리기 위하여 매주 1회 환자 체중 1 kg 당 24 mg 양의 본 발명에 따른 항-VEGFR-2 항체를 투여하는 것이 가능하다.
또한, 본 발명의 약학 조성물은 개별 치료제로 투여되거나 다른 치료제와 병용하여 투여될 수 있다. 다른 치료제와 병용하여 투여되는 경우, 본 발명의 조성물과 다른 치료제는 동시에, 개별적으로 또는 순차적으로 투여될 수 있다. 상기 다른 치료제는 뇌 부종의 치료 또는 개선 효과를 갖는 것으로 이미 알려져 있는 물질일 수 있으며, 예를 들어 삼투압제제, 이뇨제, 글리세롤, 스테로이드제 등일 수 있으나 이에 한정되는 것은 아니다. 상기 스테로이드제는 코르티코스테로이드가 바람직하며, 여기에는 덱사메타손, 코르티솔, 프레드니손, 프레드니솔론, 메틸프레드니솔론, 트리암시놀론, 코르티코스테론, 데소니드 등이 포함될 수 있다. 또한, 상기 다른 치료제는 방사선 치료 등 약물 요법 이외의 기타 항암 요법을 모두 포함한다.
본 발명의 약학 조성물이 다른 치료제와 병용하여 투여될 경우, 본 발명의 조성물에 함유되는 항-VEGFR-2 항체와 스테로이드제 같은 다른 치료제는 각각 별도의 용기로 분리시켜 제형화되거나, 동일한 용기에서 함께 제형화될 수 있다.
본 발명의 또 다른 일 양태는 항-VEGFR-2 항체를 개체에게 투여하는 단계를 포함하는 뇌 부종의 치료 또는 예방 방법을 제공한다.
본 발명에 따른 뇌 부종의 치료 또는 예방 방법에서 각 용어는 특별히 언급하지 않는 한 상기 뇌 부종의 치료 또는 예방용 약학 조성물에서 기술한 바와 동일한 의미를 갖는다.
용어 "개체"는 임의의 인간 또는 비인간 동물을 포함한다. 용어 "비인간 동물"은 척추동물, 예컨대 비인간 영장류, 양, 개, 및 설치류, 예컨대 마우스, 래트 및 기니 피그일 수 있다. 상기 개체는 바람직하게 인간일 수 있다. 용어 "개체"는 본 명세서에서 "대상체" 및 "환자"와 상호교환적으로 사용 가능하다.
상기 뇌 부종 치료는 예를 들면 비치료 개체에 비해 뇌 부종이 더 성장 또는 확장되는 것을 억제하는 것이거나, 뇌 부종의 크기를 유지하는 것이거나, 뇌 부종의 크기를 감소시키는 것이거나, 뇌 부종을 소멸시키는 것 일 수 있다. 예를 들어, 치료 전 뇌 부종의 크기를 치료 후에 약 10% 이상, 약 20% 이상, 약 30% 이상, 약 40% 이상, 약 50% 이상, 약 60% 이상, 약 70% 이상, 약 80% 이상, 약 90% 이상 축소시킬 수 있는 것을 의미한다.
본 발명에 따른 뇌 부종의 치료 또는 예방 방법에서, 상기 항-VEGFR-2 항체는 스테로이드제와 같은 다른 치료제와 동시에, 순차적으로, 또는 개별적으로 개체에 투여될 수 있다. 상기 "동시" 투여는 항-VEGFR-2 항체와 다른 치료제를 동일한 주입방법을 통해 한번에 투여되는 것을 의미한다. 상기 "순차적" 투여는 별개의 주입방법을 이용하여 항-VEGFR-2 항체와 다른 치료제를 투여하되, 비교적 연속적으로 투여하는 것을 의미하는 것으로, 투여 간격에 소모되는 시간으로 가능한 최소한의 시간을 허락한다. 상기 "개별적" 투여는 일정 시간 간격을 두고 항-VEGFR-2 항체와 다른 치료제를 투여하는 것을 의미한다. 상기 항-VEGFR-2 항체 및 다른 치료제의 투여 방법은 환자의 치료 효능 및 부작용을 고려하여 당 분야의 통상의 지식을 가진 자가 적절하게 선택할 수 있다.
본 발명의 또 다른 일 양태는 뇌 부종을 치료, 예방 또는 개선하기 위해 스테로이드제를 투여받는 환자에게 있어서 스테로이드제의 투여량 및 투여기간 중 하나 이상을 감소시키기 위한 약학 조성물을 제공한다. 상기 약학 조성물은 항-VEGFR-2 항체를 유효성분으로 포함한다.
본 발명에 따른 스테로이드제의 투여량 및 투여기간 중 하나 이상을 감소시키기 위한 약학 조성물에서 각 용어는 특별히 언급하지 않는 한 상기 뇌 부종의 치료 또는 예방용 약학 조성물에서 기술한 바와 동일한 의미를 갖는다.
상기 스테로이드제의 투여량은 뇌 부종을 갖고 있는 환자가 뇌 부종을 치료 또는 개선하기 위해 투여받는 양, 또는 뇌 부종을 가질 위험이 있거나 뇌 부종을 가질 것으로 의심되는 대상체가 뇌 부종의 발생을 예방하기 위해 투여받는 양을 의미한다. 상기 스테로이드제의 투여량은 단일 투여량(single dose) 또는 다중 투여량(multiple dose)일 수 있으며, 뇌 부종의 증상에 따라 달라질 수 있다. 본 발명에 따른 약학 조성물은 상기 스테로이드제의 투여량을 유의적으로 감소시키거나 최소화시킬 수 있다.
상기 스테로이드제의 투여기간은 뇌 부종을 갖고 있는 환자가 뇌 부종을 치료 또는 개선하기 위해 투여받는 전체 기간, 또는 뇌 부종을 가질 위험이 있거나 뇌 부종을 가질 것으로 의심되는 대상체가 뇌 부종의 발생을 예방하기 위해 투여받는 전체 기간을 의미한다. 상기 스테로이드제의 투여기간은 뇌 부종의 증상에 따라 달라질 수 있다. 본 발명에 따른 약학 조성물은 상기 스테로이드제의 투여기간을 유의적으로 감소시키거나 최소화시킬 수 있다.
또한, 상기 조성물은 스테로이드제의 투여 경로를 정맥 투여에서 경구 투여로 변경하더라도 목적하는 뇌 부종의 치료, 예방 또는 개선 효과를 얻을 수 있다. 이는 환자로 하여금 스테로이드제의 복용 편의성, 복용 순응도를 높일 수 있으므로 매우 유리하다.
본 발명의 또 다른 일 양태는 뇌 부종을 치료, 예방 또는 개선하기 위해 스테로이드제를 투여받는 개체에게 항-VEGFR-2 항체를 투여하는 단계를 포함하는, 스테로이드제의 투여량 및 투여기간 중 하나 이상을 감소시키는 방법을 제공한다.
상기 방법은 스테로이드제의 투여 경로를 정맥 투여에서 경구 투여로 변경시킬 수 있다.
상기 방법에서 각 용어는 특별히 언급하지 않는 한 상기 뇌 부종의 치료 또는 예방용 약학 조성물, 상기 뇌 부종의 치료 또는 예방 방법, 및 상기 스테로이드제의 투여량 및 투여기간 중 하나 이상을 감소시키기 위한 약학 조성물에서 기술한 바와 동일한 의미를 갖는다.
본 발명에 따른 항-VEGFR-2 항체는 뇌 부종을 치료, 예방 또는 개선할 수 있다. 보다 구체적으로, 뇌 부종이 확장, 팽창 또는 성장을 억제하거나, 또는 뇌 부종의 크기를 유지하거나, 또는 뇌 부종의 크기를 유의적으로 감소시키거나, 또는 뇌 부종을 소멸시킬 수 있다. 이와 함께 본 발명에 따른 항체는 뇌 부종으로 인해 야기되는 뇌의 midline shift를 개선하는 효과를 나타낸다.
뇌 부종을 치료 또는 개선하기 위해 종래 스테로이드제와 같은 약물을 투여받고 있는 환자는 종래 투여받던 스테로이드제와 같은 약물을 더 이상 투여받지 않거나 또는 투여량을 감소시키더라도, 본 발명에 따른 항-VEGFR-2 항체로 인해 뇌 부종이 유의적으로 감소되는 효과를 가질 수 있게 된다. 그 결과, 목적하는 뇌 부종 치료 또는 개선 효과를 달성하면서, 스테로이드제와 같은 약물의 고용량 또는 장기 투여에서 나타나는 부작용, 예컨대 근육량 상실, 골절 위험 증가, 당뇨병과 고혈압 유발, 신체 면역기능 약화 등 다양한 부작용을 최소화시키거나 회피할 수 있게 된다.
또한, 스테로이드제의 투여량 및/또는 투여기간을 감소시킴과 동시에 투여 경로를 정맥 투여에서 경구 투여로 변경하더라도 뇌 부종의 치료, 예방 또는 개선 효과를 얻을 수 있으므로, 환자로 하여금 스테로이드제의 복용 편의성, 복용 순응도를 높일 수 있다는 점에서 매우 유리하다.
또한, 본 발명에 따른 항체를 뇌 부종을 가진 환자, 특히 재발성 교모세포종 환자에게 투여하였을 때 전체생존기간을 유의적으로 연장할 수 있다.
도 1a 및 1b는 실시예 3 환자의 뇌를 촬영한 MRI 사진이다. 구체적으로, 도 1a는 스테로이드제만 투여될 때 촬영한 사진이며, 도 1b는 본 발명에 따른 항체를 투여하면서 스테로이드제의 투여량을 50% 이하로 감소시킨 후 촬영한 사진이다.
도 2a 및 2b는 실시예 7 환자의 뇌를 촬영한 MRI 사진이다. 구체적으로, 도 2a는 본 발명에 따른 항체를 투여하기 전에 촬영한 사진이며, 도 2b는 본 발명에 따른 항체를 투여한 이후에 촬영한 사진이다.
도 2a 및 2b는 실시예 7 환자의 뇌를 촬영한 MRI 사진이다. 구체적으로, 도 2a는 본 발명에 따른 항체를 투여하기 전에 촬영한 사진이며, 도 2b는 본 발명에 따른 항체를 투여한 이후에 촬영한 사진이다.
이하 본 발명을 실시예에 의해 보다 상세하게 설명한다. 그러나 이들 실시예는 본 발명을 예시적으로 설명하기 위한 것으로, 본 발명의 범위가 이들 실시예에 의해 제한되는 것은 아니다.
본 실험을 위해 재발성 교모세포종(Recurrent Glioblastoma)을 가지면서 12주 이상의 기대 수명을 갖는, 뇌 부종 상태가 심각한 환자들을 모집하였다. 본 발명에 따른 항체의 효과를 명확하게 확인하기 위하여, VEGF를 타겟하는 제제로 치료된 적이 있는 환자는 배제하였다. 이에 따라 선별한 환자들의 정보를 하기 표 1에 나타내었다.

본 실험에서 항-VEGFR-2 항체는 서열번호 7의 경쇄가변영역 및 서열번호 8의 중쇄가변영역을 포함하는 TTAC-0001 항체를 사용하였다. TTAC-0001 항체의 투여량과 투여간격은 각각 하기 표 2에 나타내었다.

실시예 1 내지 12의 환자에게 TTAC-0001 항체를 투여하는 기간 동안 일부 환자에게서 피로감, 미약한 두통 등과 같은 현상이 관찰되었으나, 이들은 임상의의 소견에 따라 심각한 부작용으로 여겨지지 않았다. 또한, 체중의 급격한 감소처럼 TTAC-0001 항체 투여로 인해 나타나는 부작용 역시 관찰되지 않았다. 이는 본 발명에 따른 항체가 인체에 대해 독성을 나타내지 않고 안전하게 사용될 수 있다는 것을 보여주는 것이다.
MRI 촬영은 8주마다 수행하여 MRI 이미지를 획득하였다. MRI 이미지에서 뇌 부종과 종양에 대해 각각 최장 직경과 최단 직경을 측정하였으며, 또한 뇌 부종과 종양의 체적을 산출하였다. 그 결과 획득한 정량데이터를 정성 분석 내용과 함께 하기 표 3에 정리하여 나타내었다.

상기 표 3에서 보는 바와 같이, 본 발명에 따른 항체로 처리된 12명의 뇌 부종을 갖고 있는 환자 중에서, 5명은 뇌 부종의 크기가 감소되었으며, 4명은 뇌 부종의 크기가 유의적인 변화 없이 유지되었으며, 나머지 3명은 종양이 진행되거나 크기가 커짐에 따라 부종의 크기도 커졌다. 즉, 거의 절반 수에 가까운 환자에 대해 뇌 부종의 크기를 유의적으로 감소시켰으며, 이에 수반하여 뇌 부종으로 인해 나타나는 부작용들, 예컨대 midline shift 등이 개선되는 것이 관찰되었다.
하기에서 대표적인 실시예들에 대해 보다 구체적으로 설명하고자 한다.
실시예 1 및 8의 환자는 뇌 부종의 크기가 증가되는 결과가 관찰되었다. 이와 관련하여, 실시예 1의 환자는 종양의 크기가 1차 MRI 촬영에 대비하여 2차 MRI 촬영에서 267 cm3에서 385 cm3로 약 44.2% 증가하였다. 또한, 실시예 8의 환자는 종양의 크기가 1차 MRI 촬영에 대비하여 2차 MRI 촬영에서 735 cm3에서 1743 cm3로 약 137.1% 증가하였다. 이와 같이 실시예 1과 8의 환자에게서 뇌 부종의 크기가 증가한 것은 임상의의 소견에 따를 때 뇌 종양 진행(tumor progression)에 따라 종양주변 부종(peritumoral edema)의 크기가 증가하는 결과가 수반되어 나타나는 것으로 확인되었다.
실시예 7의 경우, 본 발명에 따른 항체의 투여에 의해 부종의 크기가 감소되면서 뇌의 midline shift가 개선되는 결과가 특히 관찰되었다. 이는 도 2a 및 도 2b의 비교를 통해 확인이 가능하다. midline shift는 정중편위, 중심선 이동, 중간선 전위 등으로 불리워지며, 이는 높은 두개내압(intracranial pressure, ICP), 외상성 뇌손상, 뇌졸중, 뇌출혈, 뇌종양, 혈종(hematoma) 등에 의해 뇌의 중간 부분(center line)을 가로지르는 선이 이상이 없는 부위 쪽으로 치우친 것을 가리킨다. midline shift는 나쁜 예후로서 여겨지는데, 이는 비정상적 자세(abnormal posturing) 또는 빛에 반응하는 동공 수축 실패 등과 같은 심각한 기능장애(dysfunction)를 야기할 수 있는 뇌간(brain stem)의 왜곡(distortion)과 일반적으로 연관되기 때문이다. 통상적으로 midline shift가 5 mm를 초과할 경우 즉시 수술이 권장된다. 이러한 뇌의 midline shift는 본 발명에 따른 항체에 의해 비침습적으로 유의적인 개선 효과를 얻을 수 있다.
한편, 실시예 3의 경우, 재발성 교모세포종 환자가 갖고 있는 뇌 부종을 완화시키기 위하여, 2016년 3월 29일에 스테로이드제로서 덱사메타손 8mg을 정맥 투여하였다. 2016년 4월 27일에 MRI로 촬영한 뇌 사진을 도 1a에 나타내었으며, 도 1a의 좌측에 보이는 백색 부분이 뇌 부종을 가리키는 것이다. 2016년 5월 4일부터 TTAC-0001 항체를 2 cycle에 걸쳐 12 mg/kg의 용량으로 투여를 시작하였다. 이와 함께, 2016년 5월 12일부터 2016년 6월 15일까지는 덱사메타손의 투여량을 4mg으로 감소시켰으며 투여 경로는 정맥 투여에서 경구 투여로 변경하였다. 이어서, 2016년 6월 16일부터 2016년 6월 29일까지 덱사메타손의 투여량을 3mg으로 더 감소시켰으며 투여 경로는 여전히 경구 투여를 이용하였다. 2016년 6월 29일에 MRI로 촬영한 뇌 사진을 도 1b에 나타내었으며, 도 1b의 좌측에 보이는 백색 부분이 뇌 부종을 가리키는 것이다.
그 결과, 실시예 3 환자의 뇌 부종의 체적은 706.5 cm3 에서 415.6 cm3로 당업계의 통상의 지식을 가진 자가 예측할 수 없는 정도로 현저하게 감소한다는 것이 확인되었다. 이는 뇌 부종의 감소를 위해 종래에 주로 사용되어오던 덱사메타손을 고용량으로 투여받고 있음에도 불구하고 축소되지 않던 뇌 부종의 크기가 본 발명의 항-VEGFR-2 항체가 투여된 후에 유의적으로 감소되었다는 것을 보여준다.
더욱이, 본 발명의 항-VEGFR-2 항체를 투여하기 시작하면서 덱사메타손의 투여량을 50% 이상 줄이는 동시에, 목적하는 효과를 더 빠르게 얻게 할 수 있는 정맥 투여 대신 환자 편의성을 높인 경구 투여를 하였음에도 불구하고, 목적하는 뇌 부종 크기 감소 효과를 얻을 수 있었다. 이는 본 발명에 따른 항체를 스테로이드제와 함께 사용할 때, 스테로이드제를 고용량 및/또는 장기간 사용함에 따라 수반되는 부작용, 예컨대 근육량 상실, 골절 위험 증가, 당뇨병과 고혈압 유발, 신체 면역기능 약화 등 부작용을 줄이거나 회피할 수 있게 되는 것을 의미하며, 또한 투여경로의 변경에 따른 환자편의성이 개선될 수 있게 된다는 것을 의미한다.
실시예 3과 마찬가지로, 실시예 10에서도 환자는 2016년 8월 5일 덱사메타손 6mg 경구투여, 2016년 8월 17일 덱사메타손 8mg 경구투여의 순으로 투여량을 증가시키다가, TTAC-0001 항체를 투여한 2016년 8월 18일에는 덱사메타손의 경구투여량을 4mg으로 감소시켰다. 이어서, 2016년 8월 25일, 2016년 8월 31일, 2016년 9월 7일, 2016년 9월 14일, 2016년 9월 21일, 2016년 10월 5일에 각각 덱사메타손 4mg을 경구투여하였다. 이와 같이 뇌 부종을 완화시키기 위해 투여되던 덱사메타손의 양을 본 발명에 따른 항체를 투여하면서 감소시켰음에도 불구하고, 실시예 10 환자의 뇌 부종의 체적은 1102.2 cm3 에서 930.7 cm3로 감소하는 것이 확인되었다.
한편, 재발성 교모세포종 환자에 대한 회귀적 분할 분석법에 기초하여 상기 실시예 환자들에 대해 전체생존기간 중간값(mOS)을 예측하고(Prognostic factors for survival in adult patients with recurrent glioma enrolled onto the new approaches to brain tumor therapy CNS consortium phase I and II clinical trials, Carson et al., 2007), 실시예 환자들의 실제 전체생존기간(OS)를 기록하였다.
그 결과, 실시예 5의 환자는 전체생존기간 중간값(mOS)이 4.9월로 예상되며, 실시예 9의 환자는 전체생존기간 중간값(mOS)이 4.9월로 예상되며, 실시예 2의 환자는 전체생존기간 중간값(mOS)이 10.4월로 예상되었다.
이에 대응하여, 실시예 5 환자의 실제 전체생존기간은 17.3월이었으며, 실시예 9 환자의 실제 전체생존기간은 12.9월이며, 실시예 2 환자의 실제 전체생존기간은 15.9월이었다.
이와 같이 본 발명에 따른 항체를 뇌 부종을 가진 재발성 교모세포종 환자에게 투여하였을 때 연장되는 전체생존기간은 회귀적 분할 분석법에 기초하여 예측된 전체생존기간에 비해 매우 유의적이라고 판단된다.
