Abstract
본 발명은 초음파 방식 연무식 흡입기 (nebulizer)를 이용한 이중가닥 올리고뉴클레오티드 구조체 투여용 조성물에 관한 것으로, 상기 방법은 수용액상에서 90 nm 크기의 중성전하를 가지는 자가조립 나노입자를 형성하는 본 발명에 따른 이중가닥 올리고뉴클레오티드를 원액 물질과 동일한 농도, 분자량, 순도, 나노입자 크기, 삼투압몰농도를 유지할 수 있을 뿐만 아니라, 세포독성 없이 타겟 유전자 억제능을 유지하며 비강 및 및 폐 특이적으로 전달할 수 있는바, COVID-19를 포함하는 호흡기 바이러스 감염증, 바이러스 감염에 의한 폐섬유증, 또는 호흡기 질환 예방 또는 치료에 유용하게 활용 가능하다.
Description
본 발명은 초음파 방식 연무식 흡입기 (nebulizer)를 이용한 이중가닥 올리고뉴클레오티드 구조체 투여용 조성물에 관한 것으로, 특히 COVID-19를 포함하는 호흡기 바이러스 감염증, 바이러스 감염에 의한 폐섬유증, 또는 호흡기 질환 예방 또는 치료를 위한, 초음파 방식 연무식 흡입기를 이용한 이중가닥 올리고뉴클레오티드 구조체 투여용 조성물에 관한 것이다.
흡입식 약물 전달(inhalation)은 사용이 간편할 뿐만 아니라 적은 약용량으로 필요한 효과를 나타낼 수 있고, 표적기관 특히 기관지 또는 폐에 직접 도달하여 전신 부작용을 피하거나 최소화할 수 있으며, 빠른 약효를 나타내는 약물전달 방법이다. 흡입기는 약제 성상에 따라 정량분무식 흡입기(metered dose inhaler, MDI), 건조분말 흡입기(dry powder inhaler, DPI), 연무식 흡입기(nebulizer) 등으로 구분된다. 정량분무식 흡입기의 경우 약품 용기 안에 가스가 충전되어 있어 사용시 가스의 압력에 의해 일정량의 약물이 방출되는 방식이고, 건조분말 흡입기의 경우 가스는 충전되어 있지 않고, 흡입자 스스로의 흡입력으로 약물을 흡입하는 형태이다. 연무식 흡입기는 기계적 진동을 이용하여 약물을 분무 가능한 작은 액상 입자로 만든 후 공기 압력을 이용하여 환자에게 흡입튜브를 사용하여 전달하는 방식으로 미세하게 분무하기 때문에 약물의 효과가 기관지와 허파꽈리까지 잘 전달되므로 호흡기 바이러스 감염증, 간질성 폐섬유증, 만성폐쇄성 폐질환, 천식, 기관지 확장증, 기관지염, 폐렴, 폐기종 등에 사용할 수 있는 장점이 있다. 연무식 흡입기는 분무하는 방법에 따라 컴프레셔(compressor) 방식과 초음파(ultrasonic) 방식으로 구분할 수 있다. 컴프레셔 방식의 네뷸라이져는 압축기가 네뷸라이져 약물 챔버로 공기를 통과시키면서 분무 입자를 만들고 진동하는 메쉬의 미세 구멍을 통해 에어로졸이 생성된다. 초음파 방식의 네뷸라이져는 초음파가 약물 챔버를 통과하면서 약물을 진동시키고 에어로졸을 생성한다. 장비에 따라 메쉬의 미세 구멍을 통과시켜 일정한 크기를 지닌 에어로졸을 생성한다.
siRNA 기반 약물도 흡입제로 개발하기 위해 다양한 시도가 이루어지고 있지만, 주로 건조분말형태(dry powder formulation)로써 히알루론산 코팅 리포좀 스프레이(동결건조 형태), 알부민과 PLGA (poly-lactic-co-glycolic acid) 혼합 나노입자 형태, 키토산 폴리머 형태, Catonic lipid 혹은 리포솜 형태, 세포를 투과하는 단백질 (MPG, TAT, CADY, LAH4)을 이용한 나노 입자 형태가 있다. 리포좀 기반 siRNA 약물의 흡입제 개발에 가장 큰 걸림돌은 분무(nebulization) 후 리포좀 절편화 등 안정성이 깨지는 현상이다. 분무 후 리포좀의 안정성 개선을 위해 리포좀에 콜레스테롤, high-phase transition phospholipid 등을 혼합하거나 리포좀에 페길레이션(PEGylation) 변형을 하여 사용하는 등 다양한 형태의 제형이 시도되어지고 있지만 여전히 리포좀 자체 및 리포좀내 약물 안정성 개선이 필요한 실정이다(Mindaugas Rudokas, Med. Princ. Pract. 2016 March; 25:60). 또한 Catonic lipid 혹은 리포솜 형태의 경우 입자 자체 독성의 위험성이 있고 일반적인 RNAi 기반 약물의 경우 선천면역(innate immune response)을 유발하여 염증성 사이토카인(pro-inflammatory cytokine)과 인터페론(interferon) 반응을 비특이적으로 활성화하여 독성을 일으킬 수 있는 위험성이 있으며, 전달 및 약효의 한계를 보이고 있다(Hasan Uludag, front. Bioeng. Biotechnol. 2020 July; 8:916).
1995년 Guo와 Kemphues는 C. elegans에서 antisense를 이용한 유전자 발현을 억제하는데 antisense RNA와 마찬가지로 sense RNA도 유효하다는 것을 발견함으로써 그 원인을 규명하기 위한 연구가 진행되었으며, 1998년 Fire 등이 처음으로 double stranded RNA (dsRNA)를 주입하여 그에 대응하는 mRNA가 특이적으로 분해되어 유전자의 발현이 억제되는 현상을 확인하였고 이를 RNA interference (RNAi) 로 명명하였다. RNAi는 유전자 발현을 억제하기 위해 사용되는 방법으로 간편하면서 적은 비용으로 유전자 억제 효과를 뚜렷하게 볼 수 있기 때문에 그 기술의 응용 분야가 다양해지고 있다.
이러한 유전자의 발현을 억제하는 기술은 특정 유전자의 발현을 조절할 수 있기 때문에 특정 유전자의 과잉 발현이 원인이 되는 암, 유전 질환 등의 표적 유전자를 mRNA 수준에서 제거할 수 있어, 질병치료를 위한 치료제 개발 및 표적 검증의 중요한 도구로 활용될 수 있다. 표적 유전자의 발현을 억제하는 종래의 기술로는 표적 유전자에 대한 전이 유전자를 도입하는 기술이 개시되어 있는데, 프로모터를 기준으로 역방향(antisense)으로 전이 유전자를 도입하는 방법과 프로모터 기준으로 정방향(sense)으로 이식 유전자를 도입하는 방법이 있다.
이러한 RNA를 표적으로 하는 RNA 치료법은 표적 RNA에 대한 올리고뉴클레오티드를 이용하여 해당 유전자의 기능을 제거하는 방법으로 기존의 항체와 화합물(small molecule) 같은 종래의 치료제가 주로 단백질을 표적으로 하는 것과는 다르다고 할 수 있다. RNA를 표적으로 하는 접근법에는 크게 두 가지가 존재하는데, 하나는 이중나선-RNA가 매개된 RNAi이고 다른 하나는 안티센스 올리고뉴클레오티드 (Antisense oligonucleotide, ASO)이다. 현재 다양한 질병에서 RNA를 표적으로 하여 임상이 시도되고 있다.
안티센스 올리고뉴클레오티드는 (ASO, 이하 'ASO'라고 한다)는 왓슨-크릭 염기 반응에 따라 목적 유전자에 결합되도록 디자인된 짧은 길이의 합성 DNA 로서, 유전자의 특정 염기 서열의 발현을 특이적으로 억제할 수 있어 유전자의 기능을 연구하고 암과 같은 질환을 분자 수준에서 치료할 수 있는 치료제를 개발하는데 이용되어 왔다. 이러한 ASO는 유전자 발현을 억제하는 목표를 다양하게 설정하여 용이하게 제작될 수 있는 장점이 있어, 발암 유전자의 발현과 암세포의 성장을 억제하는데 활용하고자 하는 연구가 있어 왔다. ASO가 특정 유전자의 발현을 억제하는 과정은 상보적인 mRNA 서열과 결합하여 RNase H 활성을 유도하여 mRNA를 제거하거나 단백질 번역을 위한 리보좀 복합체의 형성 및 진행을 방해함으로써 이루어진다. 또한 ASO는 게놈 DNA와 결합하여 트리플-헬릭스(Triple helix) 구조를 형성함으로 유전자의 전사를 억제한다고도 보고되었다. ASO는 위와 같은 잠재적 가능성이 있으나, 이를 임상에 활용되기 위해서는 뉴클레아제(nuclease)에 대한 안정성을 향상시키고, 목적 유전자의 염기 서열에 특이적으로 결합하도록 표적 조직이나 세포내로 효율적으로 전달되어야 한다. 또한, 유전자 mRNA의 2차 및 3차 구조는 ASO의 특이적 결합에 중요한 요소로, mRNA 2차 구조가 적은 부분이 ASO가 접근하는데 매우 유리하므로, ASO를 합성하기 전에 mRNA 2차 구조가 적게 생성되는 영역을 체계적으로 분석하여 생체 외뿐 아니라 생체 내에서도 유전자-특이적 억제를 효과적으로 달성하기 위해 노력해왔다. 이러한 ASO는 RNA의 종류인 siRNA 보다 매우 안정적이고 물과 생리 식염수 등에 잘 용해되는 장점을 가지고 있으며, 현재 세 개의 ASO가 Federal Drug Administration (FDA)에 승인 되었다(Jessica, C., J Postdoc Res, 4:35-50, 2016).
간섭 RNA (RNA interference, 이하 'RNAi'라고 한다)는 그 역할이 발견된 이후로, 다양한 종류의 포유동물 세포(mammalian cell)에서 서열 특이적 mRNA에 작용한다는 사실이 밝혀졌다(Barik, S., J Mol. Med. (2005) 83: 764-773). 긴 사슬의 RNA 이중가닥이 세포로 전달되면, 전달된 RNA 이중가닥은 Dicer라는 엔도뉴클라아제(endonuclease)에 의하여 21 내지 23개의 이중가닥(base pair, bp)으로 프로세싱된 짧은 간섭 RNA (small interfering RNA, 이하 'siRNA'라고 한다)로 변환되며, siRNA는 RISC (RNA-induced silencing complex)에 결합하여 가이드(안티센스) 가닥이 타겟 mRNA를 인식하여 분해하는 과정을 통해 타겟 유전자의 발현을 서열 특이적으로 저해한다. siRNA를 이용한 유전자의 발현 억제 기술은 표적 세포에서 표적 유전자의 발현을 억제시키고 이로 인한 변화를 관찰하는 것으로 표적 세포에서의 표적 유전자의 기능을 규명하는 연구에 유용하게 사용된다, 특히, 감염성 바이러스 또는 암세포 등에서 표적 유전자의 기능을 억제하는 것은 해당 질병의 치료 방법을 개발하는데 유용하게 활용될 수 있을 것이며, 생체 외(in vitro)에서의 연구 및 실험동물을 이용한 생체 내(in vivo) 연구를 수행한 결과, siRNA에 의한 표적 유전자의 발현 억제가 가능하다고 보고된 바 있다.
베르트랑(Bertrand) 연구진에 따르면 동일한 타겟 유전자에 대한 siRNA가 ASO에 비하여 in vitro 및 in vivo에서 mRNA 발현의 저해효과가 뛰어나고, 해당 효과가 오랫동안 지속되는 효과를 가지는 것으로 밝혀졌다. 또한 siRNA의 작용 기작은 타겟 mRNA와 상보적으로 결합하여 서열 특이적으로 타겟 유전자의 발현을 조절하기 때문에, 기존의 항체 기반 의약품이나 화학물질 의약품(small molecule drug)에 비하여 적용할 수 있는 대상이 획기적으로 확대될 수 있다는 장점을 가진다(MA Behlke, MOLECULAR THERAPY. 2006 13(4):664-670).
siRNA의 뛰어난 효과 및 다양한 사용범위에도 불구하고, siRNA가 치료제로 개발되기 위해서는 체내에서의 siRNA의 안정성(stability) 개선과 세포 전달 효율 개선을 통해 siRNA가 타겟 세포에 효과적으로 전달되어야 한다. 체내 안정성 향상 및 siRNA의 비특이적인 선천면역자극(innate immune stimulation) 문제 해결을 위하여 siRNA의 일부 뉴클레오티드 또는 골격(backbone)을 핵산분해효소 저항성을 가지도록 변형(modification)하거나 바이러스성 벡터(viral vector), 리포좀 또는 나노입자(nanoparticle) 등의 전달체를 이용하는 등, 이에 대한 연구가 활발하게 시도되고 있다.
아데노바이러스나 레트로바이러스 등의 바이러스성 벡터를 이용한 전달 시스템은 형질주입 효율(transfection efficacy)이 높지만, 면역원성(immunogenicity) 및 발암성(oncogenicity)이 높다. 반면에 나노입자를 포함하는 비바이러스성(non-viral) 전달 시스템은 바이러스성 전달 시스템에 비하여 세포전달효율은 낮은 편이지만, 생체 내에서의 안정성이 높고, 타겟 특이적으로 전달이 가능하며, 내포되어 있는 RNAi 올리고 뉴클레오타이드를 세포 또는 조직으로 흡수(uptake) 및 내재화(internalization)하고, 세포독성 및 선천면역자극이 거의 없다는 장점을 가지고 있어, 현재는 바이러스성 전달 시스템에 비해 유력한 전달방법으로 평가되고 있다(Akhtar S, J Clin Invest. 2007 December 3; 117(12): 3623-3632).
비바이러스성 전달 시스템 중에서 나노전달체(nanocarrier)를 이용하는 방법은 리포좀, 양이온 고분자 복합체 등의 다양한 고분자를 사용함으로써 나노입자를 형성하고, siRNA를 이러한 나노입자, 즉 나노전달체에 담지하여 세포에 전달하는 방법이다. 나노전달체를 이용하는 방법 중 주로 활용되는 방법은 고분자 나노입자(polymeric nanoparticle), 고분자 미셀(polymer micelle), 리포플렉스(lipoplex) 등이 있는데, 이 중에서 리포플렉스를 이용한 방법은 양이온성 지질로 구성되어 세포의 엔도좀(endosome)의 음이온성 지질과 상호작용하여 엔도좀의 탈 안정화 효과를 유발하여 세포 내로 전달하는 역할을 한다.
또한, siRNA 패신저(passenger; 센스) 가닥의 말단 부위에 화학물질 등을 연결하여 증진된 약동력학(pharmacokinetics)적 특징을 갖도록 하여 생체 내에서 높은 효율을 유도할 수 있다는 것이 알려져 있다(J Soutschek, Nature 11; 432(7014):173-8, 2004). 이 때 siRNA 센스(sense; 패신저) 또는 안티센스 (antisense; 가이드) 가닥의 말단에 결합된 화학 물질의 성질에 따라 siRNA의 안정성이 달라진다. 예를 들어, 폴리에틸렌 글리콜(polyethylene glycol, PEG)과 같은 고분자 화합물이 접합된 형태의 siRNA는 양이온성 물질이 존재하는 조건에서 siRNA의 음이온성 인산기와 상호작용하여 복합체를 형성함으로써, 개선된 siRNA 안정성을 가진 전달체가 된다(SH Kim, J Control Release 129(2):107-16, 2008). 특히 고분자 복합체로 구성된 미셀(micelle)들은 약물 전달 운반체로 쓰이는 다른 시스템인, 미소구체(microsphere) 또는 나노입자 등에 비해 그 크기가 극히 작으면서도 분포가 매우 일정하고, 자발적으로 형성되는 구조이므로 제제의 품질 관리 및 재현성 확보가 용이하다는 장점이 있다.
siRNA의 세포 내 전달 효율성을 향상시키기 위해, siRNA에 생체 적합성 고분자인 친수성 물질(예를 들면, 폴리에틸렌 글리콜을 단순 공유결합 또는 링커-매개(linker-mediated) 공유결합으로 접합시킨 siRNA 접합체를 통해, siRNA의 안정성 확보 및 효율적인 세포막 투과성을 위한 기술이 개발되었다(대한민국 등록특허 제883471호). 하지만 siRNA의 화학적 변형 및 폴리에틸렌 글리콜을 접합시키는 것(PEGylation)만으로는 생체 내에서의 낮은 안정성과 표적 장기로의 전달이 원활하지 못하다는 단점은 여전히 가진다. 이러한 단점을 해결하기 위하여 올리고뉴클레오티드, 특히 siRNA와 같은 이중가닥 올리고 RNA에 친수성 및 소수성 물질이 결합된 이중가닥 올리고 RNA 구조체가 개발되었는데, 상기 구조체는 소수성 물질의 소수성 상호작용에 의하여 SAMiRNA (self-assembled micelle inhibitory RNA) 라고 명명된 자가조립 나노입자를 형성하게 되는데(대한민국 등록 특허 제1224828호), SAMiRNA 기술은 기존의 전달기술들에 비해 매우 사이즈가 작으면서도 균일한(homogenous) 나노입자를 수득할 수 있다는 장점을 가진다.
SAMiRNA 기술의 구체적인 예로서, 친수성 물질로서 PEG 또는 HEG (Hexaethylenglycol)가 사용되는데, PEG는 합성 폴리머(synthetic polymer)로 흔히 의약품 특히 단백질의 수용성(solubility) 증가 및 약물동태학(pharmacokinetics)의 조절을 위해 사용된다. PEG는 다분산계(polydisperse) 물질로, 한 배치(batch)의 폴리머는 다른 개수의 단량체(monomer)의 총 합으로 이루어져 분자량이 가우스 곡선 형태를 나타내며, 다분산지수(polydisperse value, Mw/Mn)로 물질의 동질성 정도를 표현한다. 즉, PEG가 낮은 분자량(3~5kDa)일 때 약 1.01의 다분산지수를 나타내며 높은 분자량(20 kDa)일 때 약 1.2라는 높은 다분산지수를 나타내어, 높은 분자량일수록 물질의 동질성이 상대적으로 낮은 특징을 보인다. 따라서, PEG를 의약품에 결합시킨 경우 접합체에 PEG의 다분산적 특징이 반영되어 단일물질의 검증이 쉽지 않다는 단점이 있어 PEG의 합성 및 정제과정의 개선을 통해 낮은 다분산지수를 가지는 물질을 생산하는 추세이지만, 특히 분자량이 작은 물질에 PEG를 결합시킨 경우 결합이 용이하게 이루어졌는지 확인이 용이하지 않은 불편한 점이 있는 등 물질의 다분산성 특징에 따른 문제점 들이 존재한다(Francesco M. VDRUG DISCOVERY TODAY(2005) 10(21):1451-1458).
이에 따라 최근에는 기존 자가조립 나노입자인 SAMiRNA 기술의 개량된 형태로, SAMiRNA를 구성하는 이중가닥 올리고뉴클레오티드 구조체의 친수성 물질을 일정한 분자량을 갖는 균일한 1~15개의 단량체(monomer)와 필요에 따라 링커(linker)를 포함하는 기본단위를 블록(block)화 하여, 이를 필요에 따라 적절한 개수를 사용함으로써, 기존의 SAMiRNA에 비해 보다 작은 크기를 가지며 또한 다분산성이 획기적으로 개선된 새로운 형태의 전달체 기술이 개발되었다(대한민국 등록특허 제18162349호). 또한 체내로 siRNA를 주입하였을 경우, 혈액 내에 존재하는 여러 다양한 효소들에 의해 siRNA가 급속하게 분해되어 표적 세포 또는 조직 등으로 전달 효율이 좋지 않음이 이미 알려져 있듯이 개량된 SAMiRNA에서도 표적 유전자에 따라 안정성 및 발현 저해율의 편차가 나타났다. 따라서, 본 발명자들은 개량된 자가조립 나노입자인 SAMiRNA를 이용하여 표적 유전자를 보다 안정적이고 효과적으로 발현 저해하기 위해 가이드인 센스 가닥은 ASO인 DNA 서열을, 패신저인 안티센스 가닥은 RNA 서열을 이용하여 DNA-RNA hybrid 형태의 이중가닥 올리고뉴클레오티드를 적용함으로써 표적 유전자에 대한 발현 저해 효과와 안정성을 증진시킬 수 있었다.
그러나, RNAi 약물의 전달에 있어서, 기존의 RNAi 약물의 전달방법은 분말형태가 주를 이루는데, 이는 앞서 설명한 바와 같이, 입자 자체의 독성, 비특이적 면역 반응의 위험성과 더불어 약물 전달 및 약효의 한계를 보이는 바, 본 발명에서는 SAMiRNA의 구조적 특성에 입각하여 이러한 한계를 극복할 수 있는 SAMiRNA에 특히 적합한 약물 전달 방식을 개발하고자 하였으며, 다양한 약물 전달 방식 특히 초음파 방식의 연무식 흡입기를 이용할 때 원액물질과 동일한 농도, 분자량, 순도, 나노입자 크기, 삼투압몰농도를 나타내고, 타겟 유전자 억제능을 유지하면서 세포독성 없이 효과적으로 폐 특이적으로 SAMiRNA가 전달되는 것을 확인함으로써 본 발명을 완성하였다.

본 발명의 목적은 호흡기 바이러스 감염증, 바이러스에 의한 폐섬유증, 또는 호흡기 질환의 예방 또는 치료에 적합한 이중가닥 올리고뉴클레오티드 구조체를 기관지 및 폐 특이적으로 효과적으로 전달하기 위한 약학 조성물을 제공하는데 있다.
상기 목적을 달성하기 위하여, 본 발명은 하기 구조식 1의 구조를 포함하는 이중가닥 올리고뉴클레오티드 구조체를 포함하는 호흡기 바이러스 감염증, 바이러스에 의한 폐섬유증, 또는 호흡기 질환 예방 또는 치료용 약학 조성물로서, 상기 약학 조성물은 연무식 흡입기를 이용하여 투여되는 것을 특징으로 하는 약학 조성물을 제공한다:

상기 구조식 1에서 A는 친수성 물질, B는 소수성 물질, X 및 Y는 각각 독립적으로 단순 공유결합 또는 링커가 매개된 공유결합을 의미하며, R은 이중가닥 올리고뉴클레오티드를 의미함.
본 발명에 따른 이중가닥 올리고뉴클레오티드 구조체는 수용액상에서 90 nm 크기의 중성전하를 가지는 자가조립 나노입자를 형성하기 때문에 5 μm 이하의 미세입자 형태로 분무되는 연무식 흡입기(nebulizer)에 최적화된 siRNA 플랫폼으로 작용할 수 있으며, 특히 초음파 방식의 연무식 흡입기를 사용하는 경우 투여하고자 하는 원액 물질과 동일한 농도, 분자량, 순도, 나노입자 크기, 삼투압몰농도 뿐만 아니라 타겟 유전자 억제능을 유지함으로써, 투여 과정에서 발생할 수 있는 변형 없이 본 발명에 따른 약학 조성물을 비강 및 폐(허파꽈리)에 효율적으로 전달할 수 있다.
따라서, 본 발명은 호흡기 바이러스 감염증, 바이러스 감염에 의한 폐섬유증이나 폐렴, 또는 기타 호흡기 질환의 치료에 적합하고, 특히 현재 전 세계적으로 폭발적인 감염 및 사망자를 유발하고 있는 신종 호흡기 바이러스인 SARS-CoV-2에 의해 발생하는 COVID-19 치료제에 적용할 때, 환자가 스스로 쉽고 간편하게 이중가닥 올리고뉴클레오티드 구조체를 투약하여 효율적으로 생체내 전달이 가능하다는 장점이 있다.

도 9b는 SAMiRNA-Cy5 나노입자를 초음파 연무식 흡입기를 이용하여 마우스에 투여하여 1 시간 후 적출한 폐 조직의 전달 및 분포되는 것을 콘포컬 이미지 분석한 결과를 나타낸다.
다른 식으로 정의되지 않는 한, 본 명세서에서 사용된 모든 기술적 및 과학적 용어들은 본 발명이 속하는 기술 분야에서 숙련된 전문가에 의해서 통상적으로 이해되는 것과 동일한 의미를 가진다. 일반적으로, 본 명세서에서 사용된 명명법은 본 기술 분야에서 잘 알려져 있고 통상적으로 사용되는 것이다.
본 발명에 따른 이중가닥 올리고뉴클레오티드 구조체를 체내로 전달하는 다양한 방식 중 초음파 방식 연무식 흡입기가 상기 구조체의 물리/화학적 성질에 영향을 주지 않는 최적화된 흡입식 약물전달 방식임을 확인하였다. 구체적으로, 초음파 방식의 연무식 흡입기를 통과시킨 본 발명의 이중가닥 올리고뉴클레오티드 구조체의 물성을 확인한 결과, 농도, 분자량, 순도, 나노입자 크기, 삼투압몰농도가 원액 물질 그대로 유지될 수 있음이 확인되었다. 그러나, 연무식 분무기 중 컴프레서 방식에서는 나노입자 크기, 분자량, 순도는 원액물질과 동일하지만, 물질농도 및 삼투압몰농도가 원물질에 비해 현저하게 감소되는 것으로 확인되었다.
한편, 인간 비강 세포주(nasal epithelial cell) RPMI2650 세포와 인간 폐암 세포주인 A549 세포를 이용하여 음파 방식의 연무식 흡입기에 따른 투여방식의 세포 독성을 확인한 결과 고농도인 50 μM 에서도 세포독성이 나타나지 않음을 알 수 있었다. 또한, 타겟 유전자 억제능을 확인하기 위하여 RPMI2650 및 A549 세포에 각 물질을 농도별로 처리하여 분석해 본 결과 초음파 방식의 연무식 흡입기 통과 후에도 통과 전과 동일한 타겟 억제능을 확인할 수 있었다.
마지막으로, 초음파 방식의 연무식 흡입기를 이용하여 본 발명의 구조체가 In vivo 상에서 폐에 효과적으로 전달되는지를 확인하기 위하여 SAMiRNA에 Cy5를 접합시키고 햄스터에 초음파 방식의 연무식 흡입기를 통해 투여한 후 폐 조직을 적출하여 관찰한 결과, 폐 조직을 구성하고 있는 대부분의 세포에서 형광(SAMiRNA-Cy5)이 관찰되었고, 특히 폐포 및 기관지 등에서 강하게 형광이 관찰되어, SAMiRNA 물질을 초음파 방식의 연무식 흡입기를 이용하여 투여했을 때 효과적으로 폐에 전달됨을 확인하였다. 즉, 본 발명에 따른 이중가닥 올리고뉴클레오티드 구조체 및 이들이 자가조립된 형태의 나노입자의 최적화된 투여는 초음파 방식의 연무식 흡입기에 의할 수 있음을 확인하였다.
따라서, 본 발명은 일 관점에서, 하기 구조식 1의 구조를 포함하는 이중가닥 올리고뉴클레오티드 구조체를 포함하는 호흡기 바이러스 감염증, 바이러스에 의한 폐섬유증, 또는 호흡기 질환 예방 또는 치료용 약학 조성물로서, 상기 약학 조성물은 연무식 흡입기를 이용하여 투여되는 것을 특징으로 하는 약학 조성물에 관한 것이다:

본 발명에 따른 이중가닥 올리고뉴클레오티드는 일반적인 RNAi 작용을 가지는 모든 물질을 포함하는 개념으로, 상기 타겟 유전자 특이적 이중가닥 올리고뉴클레오티드에는 타겟 유전자 특이적 shRNA 등도 포함되는 것임은 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자에게는 자명한 것이다. 즉, 상기 올리고뉴클레오티드는 siRNA, shRNA 또는 miRNA인 것을 특징으로 할 수 있다.
본 발명에 따른 이중가닥 올리고뉴클레오티드는 하나 또는 양쪽 가닥의 3' 말단에 하나 또는 그 이상의 비결합된(unpaired) 뉴클레오티드를 포함하는 구조인 오버행(overhang)을 포함할 수 있다.
본 발명에 따른 이중가닥 올리고뉴클레오티드의 형태로는 DNA-RNA hybrid, siRNA (short interfering RNA), shRNA (short hairpin RNA) 및 miRNA (microRNA) 등이 바람직하지만, 이에 한정되는 것은 아니며, miRNA에 대한 길항제(antagonist) 역할을 할 수 있는 단일가닥의 miRNA 저해제도 포함되는 것이다.

특히, 친수성 물질 단량체 중에서도 특히 화합물 (1)로 표시되는 단량체는 다양한 기능기를 도입할 수 있으며, 생체 내 친화성이 좋고 면역반응을 적게 유도하는 등 생체적합성(bio-compatibility)이 우수할 뿐 아니라, 구조식 6 또는 구조식 7 따른 구조체 내에 포함된 이중가닥 올리고뉴클레오티드의 생체 내 안정성을 증가시키고, 전달 효율을 증가시킬 수 있다는 장점을 가지고 있어 본 발명에 따른 구조체의 제조에 매우 적합하다.

상기 구조식 4 내지 구조식 7에서의 친수성 물질은 총 분자량이 1,000 내지 2,000에 범위 내인 것이 특히 바람직하다. 따라서, 예를 들어, 구조식 6 및 구조식 7에서 화합물 (1)에 따른 헥사에틸렌 글리콜(Hexaethylene glycol), 즉 G가 O이고, m이 6인 물질이 사용되는 경우 헥사에틸렌 글리콜 스페이서(spacer)의 분 자량이 344이므로, 반복 횟수(n)는 3 내지 5인 것이 바람직하다. 특히, 본 발명은 필요에 따라 상기 구조식 4 및 구조식 5에서 (A'm-J)n으로 또는 (J-A' m)n 표시되는 친수성 그룹의 반복단위, 즉 친수성 물질 블록(block)이 n으로 표시되는 적절한 개수로 사용될 수 있음을 특징으로 한다. 상기 각 친수성 물질 블록 내에 포함되는 친수성 물질 단량체인 A와 링커인 J은 독립적으로 각 친수성 물질 블록간에 동일할 수도 있고, 상이할 수도 있다. 즉, 친수성 물질 블록이 3개 사용될 경우(n=3), 첫 번째 블록에는 화합물 (1)에 따른 친수성 물질 단량체가, 두 번째 블록에는 화합물 (2)에 따른 친수성 물질 단량체가, 세 번째 블록에는 화합물 (3)에 따른 친수성 물질 단량체가 사용되는 등, 모든 친수성 물질 블록별로 다른 친수성 물질 단량체가 사용될 수도 있고, 모든 친수성 물질 블록에 화합물 (1) 내지 화합물 (3)에 따른 친수성 물질 단량체에서 선택된 어느 하나의 친수성 물질 단량체가 동일하게 사용 될 수도 있다. 마찬가지로, 친수성 물질 단량체의 결합을 매개하는 링커 역시 각 친수성 물질 블록별로 모두 동일한 링커가 사용될 수도 있고, 각 친수성 물질 블록 별로 상이한 링커가 사용될 수도 있다. 또한, 친수성 물질 단량체의 개수인 m 역시 각 친수성 물질 블록간에 동일할 수도 있고, 상이할 수도 있다. 즉, 첫 번째 친수 성 물질 블록에서는 친수성 물질 단량체가 3개 연결(m=3)되고, 두 번째 친수성 물 질 블록에서는 친수성 물질 단량체가 5개(m=5), 세 번째 친수성 물질 블록에서는 친수성 물질 단량체가 4개 연결(m=4)되는 등, 서로 다른 개수의 친수성 물질 단량 체가 사용될 수도 있고, 모든 친수성 물질 블록에서 동일한 개수의 친수성 물질 단 량체가 사용될 수도 있다.

상기 구조식 내지 구조식 3' 및 구조식 6 및 구조식 7에서의 소수성 물질(B)은 소수성 상호작용을 통해 이중가닥 올리고뉴클레오티드 구조체로 구성된 나노입자를 형성하는 역할을 수행한다. 상기 소수성 물질은 분자량이 250 내지 1,000인 것이 바람직하며, 스테로이드 유도체, 글리세라이드 유도체, 글리세롤 에테르, 폴리프로필렌 글리콜, C12 내지 C50의 불포화 또는 포화탄화수소, 디아실포스파티딜콜린(diacylphosphatidylcholine), 지방산, 인지질, 리포폴리아민(lipopolyamine), 지질(lipid), 토코페롤(tocopherol) 및 토코트라이에놀(tocotrienol)로 구성된 군에서 선택된 어느 하나가 사용될 수 있지만, 이에 한정되는 것은 아니며, 본 발명의 목적에 부합하는 것이라면 어떠한 소수성 물질도 사용 가능하다는 점은 본 발 명이 속하는 기술분야에서 통상의 지식을 가진 자에게는 자명한 것이다.
상기 스테로이드(steroid) 유도체는 유도체는 콜레스테롤, 콜리스탄올, 콜산, 콜리스테릴포르메이트, 코테스타닐포르메이트 및 콜리스타닐아민으로 구성된 군에서 선택될 수 있으며, 상기 글리세라이드 유도체는 모노-, 디- 및 트리-글리세라이드 등에서 선택될 수 있는데, 이때 글리세라이드의 지방산은 C12 내지 C50의 불포화 또는 포화 지방산이 바람직하다.
특히, 상기 소수성 물질 중에서도 포화 또는 불포화 탄화수소나 콜레스테롤이 본 발명에 따른 이중가닥 올리고뉴클레오티드 구조체의 합성 단계에서 용이하게 결합시킬 수 있는 장점을 가지고 있다는 점에서 바람직하며, C24 탄화수소, 특히 disulfide bond를 포함하는 형태가 가장 바람직하다.
상기 소수성 물질은 친수성 물질의 반대쪽 말단(distal end)에 결합되며, 센스가닥 또는 안티센스 가닥의 어느 위치에 결합되어도 무방하다.
본 발명에 따른 구조식 1 내지 구조식 3', 구조식 6 및 구조식 7에서의 친수성 물질 또는 소수성 물질과 이중가닥 올리고뉴클레오티드는 단순 공유 결합 또는 링커가 매개된 공유결합(X 또는 Y)에 의해 결합된다. 상기 공유결합을 매개하는 링커는 친수성 물질, 또는 소수성 물질과 이중가닥 올리고뉴클레오티드의 말단에서 공유결합하며, 필요에 따라 특정 환경에서 분해가 가능한 결합을 제공하는 한 특별히 한정되는 것은 아니다. 따라서, 상기 링커는 본 발명에 따른 이중가닥 올리고뉴클레오티드 구조체의 제조과정에서 이중가닥 올리고뉴클레오티드 및/또는 친수성 물질(또는 소수성 물질)을 활성화하기 위해 결합시키는 어떠한 화합물도 사용될 수 있다. 상기 공유 결합은 비분해성 결합 또는 분해성 결합 중 어느 것이어도 무방하다. 이때, 비분해성 결합으로는 아미드 결합 또는 인산화 결합이 있고, 분해성 결합으로는 이황화 결합, 산분해성 결합, 에스테르 결합, 안하이드라이드 결합, 생분해성 결합 또는 효소 분해성 결합 등이 있으나, 이에 한정되는 것은 아니다.
본 발명은 또한 본 발명에 따른 이중가닥 올리고뉴클레오티드를 포함하는 구조체에 있어서, 상기 구조체 내의 친수성 물질의 올리고뉴클레오티드와 결합된 반대편 말단 부위에 아민기 또는 폴리히스티딘(polyhistidine) 그룹이 추가적으로 도입될 수 있다.
이는 본 발명에 따른 이중가닥 올리고뉴클레오티드를 포함하는 구조체의 전달체의 세포내 도입과 엔도좀 탈출을 용이하게 하기 위한 것으로, 이미 Quantum dot, Dendrimer, liposome 등의 전달체의 세포내 도입과 엔도좀 탈출을 용이하게 하기 위해서 아민 그룹의 도입과 폴리히스티딘 그룹이 이용할 수 있다는 점 및 그 효과가 보고된 바 있다.
구체적으로 전달체의 말단 혹은 바깥쪽에 수식된 일차 아민기는 생체 내 pH에서 양성자화 되면서 음전 하를 띠는 유전자와 정전기적 상호작용에 의해 결합체를 형성하며, 세포내 유입 후에 엔도좀의 낮은 pH에서 완충 효과를 갖는 내부 삼차 아 민으로 인해 엔도좀의 탈출이 용이해짐에 따라 라이소좀의 분해로부터 전달체를 보호할 수 있다고 알려져 있고(고분자 기반 하이브리드 물질을 이용한 유전자 전달 및 발현 억제. (Polymer Sci. Technol., Vol. 23, No.3, pp254-259),
비필수 아미노산의 하나인 히스티딘은 잔기(-R)에 이미다졸링(pKa3 6.04)을 가지므로 엔도좀과 라이소좀에서 완충능력(buffering capacity)을 증가시키는 효과가 있어, 리포좀을 비롯한 비바이러스성 유전자전달체(non-viral gene carrier)들 에서 엔도좀 탈출효율을 높이기 위해서 히스티딘 수식을 이용될 수 있다는 점이 알 려져 있다 (Novel histidine-conjugated galactosylated cationic liposomes for efficient hepatocyte selective gene transfer in human hepatoma HepG2 cells. 상기 아민기 또는 폴리히스티딘 그룹은 하나 이상의 링커를 통해 친수성 물질 또는 친수성 물질 블록과 연결될 수 있다.

한편, 본 발명에 따른 타겟 유전자 특이적 이중가닥 올리고뉴클레오티드를 포함하는 이중가닥 올리고뉴클레오티드 구조체 및 이로부터 형성된 나노입자에 타겟팅 모이어티가 구비된다면, 효율적으로 타겟 세포로의 전달을 촉진하여, 비교적 낮은 농도의 투여량으로도 타겟 세포로 전달되어 높은 타겟 유전자 발현 조절 기능을 나타낼 수 있다.
이에 따라 본 발명은 상기 구조식 1 내지 구조식 3', 구조식 6 및 구조식 7에 따른 구조체에 리간드(L), 특히 수용 체 매개 내포작용(receptor-mediated endocytosis, RME)을 통해 타겟 세포 내재화(internalization)를 증진시키는 수용체와 특이적으로 결 합하는 특성을 가진 리간드(ligand)가 추가적으로 결합된 이중가닥 올리고 RNA, 구조체를 제공하며, 예를 들어 구조식 1에 따른 이중가닥 올리 고 RNA 구조체에 리간드가 결합된 형태는 하기 구조식 14과 같은 구조를 가진다.

상기 구조식 14에서, A, B, X 및 Y는 상기 구조식 1에서의 정의와 동일하며, L은 수용체 매개 내포작용(receptor-mediated end ocytosis, RME)을 통해 타겟 세포 내재화(internalization)를 증진시 키는 수용체와 특이적으로 결합하는 특성을 가진 리간드를 의미하며, i는 1 내지 5의 정수, 바람직하게는 1 내지 3의 정수이다.
상기 구조식 14에서의 리간드는 바람직하게는 타겟 세포 특이적으로 세포 내재화 (internalization)을 증진시키는 RME 특성을 가진 타겟 수용체 특이적 항체 나 앱타머, 펩타이드; 또는 엽산(Folate, 일반적으로 folate와 folic acid는 서로 교차 사용되고 있으며, 본 발명에서의 엽산은 자연 상태 또는 인체에서 활성화 상 태인 folate를 의미한다), N-아세틸 갈락토사민(N-acetyl Galactosamine, NAG) 등 의 헥소아민(hexoamine), 포도당(glucose), 만노스(mannose)를 비롯한 당이나 탄수 화물(carbohydrate) 등의 화학물질 등에서 선택될 수 있지만, 이에 한정되는 것은 아니다.
또한, 상기 구조식 14에서의 친수성 물질 A는 구조식 4 및 구조식 5에 따른 친수성 물질 블록의 형태로 사용될 수 있다.
본 발명에 있어서, 상기 구조식 1 내지 구조식 3', 구조식 6 및 구조식 7에서의 R(또는 S 및 AS)로 표시되는 이중가닥 올리고뉴클레오티드는 앰피레귤린 (Amphiregulin), RelA/p65 및 SARS-CoV-2로 구성된 군에서 선택되는 유전자의 발현을 특이적으로 억제하는 것이라면 모두 제한 없이 사용 가능할 것이다. 구체적으로, 상기 R(또는 S 및 AS)로 표시되는 이중가닥 올리고뉴클레오티드는 앰피레귤린 (Amphiregulin) 유전자의 발현을 특이적으로 억제할 수 있고, 앰피레귤린을 타겟하는 서열을 포함할 수 있다. 상기 앰피레귤린 타겟 서열은 예를 들어, 서열번호 5 및/또는 6의 서열을 포함할 수 있다. 구체적으로, 서열번호 5의 센스 서열 및 서열번호 6의 안티센스 서열을 포함할 수 있다.
상기 R(또는 S 및 AS)로 표시되는 이중가닥 올리고뉴클레오티드는 SARS-CoV-2를 타겟하는 서열을 포함할 수 있다. 상기 SARS-CoV-2 타겟 서열은 예를 들어, 서열번호 11 내지 30으로 구성된 군에서 선택되는 서열을 포함할 수 있다.
본 발명은 다양한 호흡기 질환의 예방 또는 치료에 적합하며, 특히 호흡기 바이러스 감염증, 바이러스 감염에 의한 폐섬유증, 또는 호흡기 질환 예방 또는 치료용 조성물을 투약 방법으로 특히 적합하다.
본 발명에 있어서, 상기 호흡기 바이러스는 COVID-19인 것을 특징으로 할 수 있으나, 이에 한정되지는 않는다. 본 발명에 있어서, 상기 바이러스 감염에 의한 폐섬유증은 바이러스 감염에 의한 후유증에 일 예로서, 본 발명은 폐섬유증 이외에도 바이러스 감염, 특히 호흡기 바이러스 감염에 의한 후유증의 예방 또는 치료용 조성물을 투약하는데 활용될 수 있다.
본 발명에 있어서, 상기 호흡기 질환은 간질성 폐섬유증(Interstitial lung disease), 만성폐쇄성질환(COPD), 폐렴, 천식, 급만성기관지염, 알레르기 비염, 기관지염, 세기관지염, 인후염, 편도염 및 후두염으로 구성된 군에서 선택되는 것을 특징으로 할 수 있으나, 이에 한정되지는 않는다
본 발명에 있어서, 상기 이중가닥 올리고뉴클레오티드 구조체는 투여용 수용액 상에서 10-100 nm 크기의 중성전하를 가지는 자가조립 나노입자를 형성하는 것을 특징으로 할 수 있다.
본 발명에 있어서, 상기 나노입자는 서로 다른 서열을 포함하는 이중나선 올리고뉴클레오티드를 포함하는 이중나선 올리고뉴클레오티드 구조체가 혼합되어 구성되는 것을 특징으로 할 수 있다.
본 발명의 조성물에는 투여를 위하여 상기 기재된 유효 성분 이외에 추가로 약학적으로 허용되는 가능한 담체를 1종 이상 포함하여 제조될 수 있다. 약제학적으로 허용 가능한 담체는 본 발명의 유효성분과 양립 가능하여야 하며, 식염수, 멸균 수, 링거액, 완충 식염수, 덱스트로즈 용액, 말토덱스트린 용액, 글리세롤, 에탄올 및 이들 성분 중 한 성분 또는 둘 이상의 성분을 혼합하여 사용할 수 있고, 필요에 따라 항산화제, 완충액, 정균제 등 다른 통상의 첨가제를 첨가할 수 있다. 또한 희석제, 분산제, 계면활성제, 결합제 및 윤활제를 부가적으로 첨가하여 수용액, 현탁 액, 유탁액 등과 같은 주사용 제형으로 제제화 할 수 있다. 특히, 동결건조(lyophilized)된 형태의 제형으로 제제화하여 제공하는 것이 바람직하다. 동결건조 제형 제조를 위해서 본 발명이 속하는 기술분야에서 통상적으로 알려져 있는 방 법이 사용될 수 있으며, 동결건조를 위한 안정화제가 추가될 수도 있다. 더 나아가 당 분야의 적정한 방법으로 또는 레밍톤 약학 과학(Remington's pharmaceutical Science, Mack Publishing company, Easton PA)에 개시되어 있는 방법을 이용하여 각 질병에 따라 또는 성분에 따라 바람직하게 제제화 할 수 있다.
본 발명의 약학 조성물은 비경구 투여가 바람직하고, 특히 기관지내 흡입을 통한 폐로의 투여가 바람직하다. 본 발명에 따른 조성물의 투여량은 환자의 체중, 연령, 성별, 건강상태, 식이, 투여시간, 방법, 배설율 또는 질병의 중증도 등에 따 라 그 범위가 다양하며, 본 기술분야의 통상의 전문가가 용이하게 결정할 수 있다.
실시예
이하, 실시예를 통하여 본 발명을 보다 상세히 설명하고자 한다. 이들 실시 예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로, 본 발명의 범위가 이들 실시예에 의해 제한되지 않는다는 것은 당해 기술분야에서 통상의 지식을 가 진 자에게 있어 자명한 것이다. 따라서 본 발명의 실질적인 범위는 첨부된 청구항 들과 그것들의 등가물에 의하여 정의된다고 할 것이다.
실시예 1. 이중가닥 올리고뉴클레오티드 구조체의 합성
본 발명에서 제조된 이중가닥 올리고뉴클레오티드 구조체 SAMiRNA는 하기 구조식과 같은 구조를 갖는다.

monoSAMiRNA 이중가닥 올리고 구조체의 센스가닥은 3,4,6-트리아세틸-1-헥사(에틸렌글리콜)-씨피지(1-Hexa(Ethylene Glycol) CPG를 지지체로 하여, 친수성 물질 단량체인 DMT 헥사에틸렌 글리콜 포스포아미다이트(Demethoxytrityl hexaethylene glycol phosphoramidate) 3개를 상기 반응을 통해 연속하여 결합한 후, DNA 합성을 진행한 다음, 추가적으로 5' 말단 부위에 소수성 물질인 이황화 결합이 포함되어 있는 C24 (C18-S-S-C6)을 결합시켜, 3' 말단에 헥사에틸렌글리콜-(-PO3 - 헥사에틸렌글리콜)3이 결합되어 있고, 5' 말단에 C24 (C18-S-S-C6) 이 결합되어 있는 센스가닥을 만들었다.
합성이 완료되면 60℃의 온탕기에서 28% (v/v) 암모니아(ammonia)를 처리하여 합성된 DNA 단일가닥 및 DNA-고분자 구조체를 CPG로부터 떼어낸 뒤, 탈보호(deprotection) 반응을 통해 보호 잔기를 제거하였다. 상기 반응물들로부터 고속 액체 크로마토그래피(High Performance Liquid Chromatography, HPLC)로 DNA 단일 가닥, DNA-고분자 구조체 및 리간드가 결합된 DNA-고분자 구조체를 분리하고, 이를 MALDI-TOF 질량 분석기(MALDI TOF-MS, SHIMADZU, 일본)로 분자량을 측정하여, 합성하고자 하는 염기서열 및 DNA-고분자 구조체와 부합하는지 확인하였다
안티센스가닥의 합성은 링커 (UnyLinker™)가 결합된 지지체로 하여 센스가닥과 상보적인 서열의 RNA 합성을 진행한 다음 5' 말단 부위에 인산기(PO4)가 결합되어 있는 안티센스가닥을 만들었다.
합성이 완료되면 60℃의 온탕기에서 28% (v/v) 암모니아(ammonia)를 처리하여 합성된 RNA 단일가닥 및 RNA-고분자 구조체를 CPG로부터 떼어낸 뒤, 탈보호(deprotection) 반응을 통해 보호 잔기를 제거하였다. 보호 잔기가 제거된 RNA 단일가닥 및 RNA-고분자 구조체는 70℃의 오븐에서 엔-메틸피롤리돈(N-methylpyrolidon), 트리에틸아민(triethylamine) 및 트리에틸아민트리하이드로플로 라이드(triethylaminetrihydrofluoride)를 부피비 10:3:4의 비율로 처리하여 2ㅄTBDMS (tert-butyldimethylsilyl)를 제거하였다. 상기 반응물들로부터 고속 액체 크로마토그래피(High Performance Liquid Chromatography, HPLC)로 RNA 단일 가닥, RNA-고분자 구조체 및 리간드가 결합된 RNA-고분자 구조체를 분리하고, 이를 MALDI-TOF 질량 분석기(MALDI TOF-MS, SHIMADZU, 일본)로 분자량을 측정하여, 합성하고자 하는 염기서열 및 RNA-고분자 구조체와 부합하는지 확인하였다. 그 후, 각 각의 이중가닥 올리고 구조체를 제조하기 위하여 센스가닥과 안티센스 가닥을 동량 혼합하여 (1X)PBS(Phosphate buffer saline, 30mg/1ml(1x)PBS)에 넣고, 90 ℃ 항온수조에서 5분 반응시킨 후 온도를 상온까지 천천히 내리면서 목적하는 SAMiRNA를 제조하였다. 제조된 이중가닥 올리고 구조체들은 고속 액체 크로마토그래피(High Performance Liquid Chromatography, HPLC, Non Denaturation) 통해 어닐링을 확인하였다. 효능평가를 위해 합성된 SAMiRNA-AREG의 서열정보는 하기 표 2와 같다

실시예 2. 연무식 흡입기를 사용하여 SAMiRNA 나노입자 포집
본 발명에서는 각각 초음파 방식과 컴프래셔 방식 연무식 흡입기를 사용하여 SAMiRNA를 포집하였다.
초음파 연무식 흡입기(메쉬넵2, China)의 약물챔버에 SAMiRNA 1ml를 넣고 챔버에 마우스피스를 연결하였다. 마우스피스의 끝부분을 포집을 위한 시험관에 넣은 상태로 연무식 흡입기를 가동하였다. 분무된 SAMiRNA를 포집하기 위하여 아이스박스에 얼음을 넣어 시험관 주위에 두었으며, 연무식 흡입기 가동은 10분간 진행하였으며, 분무된 SAMiRNA의 액화되는 시간을 고려하여 총 20분간 포집을 진행하였다.
컴프래셔 연무식 흡입기(필립스 비가열식 흡입기, US)의 약물챔버에 SAMiRNA 1ml를 넣고 챔버에 고무 호스를 연결하고 고무 호스 말단에 포집을 위한 시험관을 결합시켰다. 분무된 SAMiRNA를 포집하기 위하여 아이스박스에 얼음을 넣어 시험관 주위에 두었으며, 연무식 흡입기 가동은 10분간 진행하였으며 분무된 SAMiRNA의 액화되는 시간을 고려하여 총 20분간 포집을 진행하였다.
실시예 3. 연무식 흡입기를 사용하여 포집한 SAMiRNA 나노입자의 물성 분석
포집된 SAMiRNA는 고속 액체 크로마토그래피로 RNA 단일 가닥, RNA-고분자 구조체 및 리간드가 결합된 RNA-고분자 구조체를 분리하고 이를 MALDI-TOF 방식의 질량 분석기를 사용하여 의도한 서열의 존재 여부를 판별 및 확인하였다. 포집된 SAMiRNA의 삼투압몰농도를 Osmomat 3000 (gonotec, Germany) 장비로 측정하여 혈관 내에 직접 들어와도 삼투압의 변화를 일으키지 않는 생리식염수와 같은 삼투압몰농도를 확인하였다. 포집된 SAMiRNA의 입자 크기는 동적 광산란(DLS) 장치인 Zetasizer NANO-ZS (Malvern, UK)와 조정 가능한 저항성 펄스 감지(TRPS)를 사용하는 The qNano Gold (IZON, New Zealand) 장비를 제조업자의 프로토콜에 따라 사용하여 분석하였다.
그 결과, 초음파 방식의 연무식 흡입기를 이용하여 포집한 SAMiRNA 나노입자는 연무식 흡입기 사용 전 물질과 동일한 농도, 분자량, 순도, 나노입자 크기, 삼투압 몰농도를 확인할 수 있었던 반면, 컴프래셔 방식의 연무식 흡입기를 이용하여 포집한 SAMiRNA의 경우 나노입자 크기, 분자량, 순도는 동일하였지만, 농도가 8.3mg/ml로 3.6배 감소되었고 삼투압 몰농도 또한 생리식염수 농도 보다 현저하게 낮아지는 결과가 확인되었다 (도 1).
실시예 4. 초음파 연무식 흡입기를 사용하여 포집한 SAMiRNA 나노입자의 세포독성 평가
초음파 연무식 흡입기를 사용하여 포집한 SAMiRNA 나노입자의 세포독성 평가를 위해 인간 폐암 세포주인 A549 (CCL-185, ATCC, US)와 인간 비강 상피 세포주인 RPMI2650 (한국세포주은행, KR)를 이용하였다. A549 세포주는 10% Fetal Bovine Serum (Hyclone, US) 및 1% Penicillin-Streptomycin (Hyclone, US)가 포함된 F12K 배지 (Gibco, US)를 이용하여 37℃, 5% CO2 조건에서 배양하였다. RPMI2650 세포는 10% Fetal Bovine Serum (Hyclone, US) 및 1% Penicillin-Streptomycin (Hyclone, US)가 포함된 RPMI1640 배지 (Hyclone, US)를 이용하여 37℃, 5% CO2 조건에서 배양하였다. 96 well plate (Falcon, US)에 A549세포를 3X103 cells/well의 조건으로 분주, RPMI2650 세포는 1X104 cells/well의 조건으로 분주하고 다음날 초음파 연무식 흡입기를 이용하여 포집한 SAMiRNA를 농도별(0, 1, 5, 10, 20, 50 μM)로 처리하였다. 96 시간 배양 후, 세포독성 측정을 위해 WST assay kit (DOGEN, KR)를 사용하여 제조업자의 프로토콜에 따라 실험을 진행하여 분석하였다.
그 결과, 두 종류의 세포주에서 초음파 연무식 흡입기 통과 전, 후 물질 모두 20 μM 농도까지는 세포독성이 관찰되지 않았고, RPMI2650 세포의 경우 50 μM 농도에서만 일부 세포독성이 관찰되었다 (도 2).
실시예 5. 초음파 연무식 흡입기를 사용하여 포집한 SAMiRNA 나노입자의 타겟 유전자 발현 억제능 평가
5-1. 초음파 연무식 흡입기를 사용하여 포집한 SAMiRNA 나노입자의 세포 처리
초음파 연무식 흡입기를 사용하여 포집한 SAMiRNA 나노입자가 타겟 유전자 발현을 효율적으로 억제하는지 분석하기 위하여 A549 (CCL-185, ATCC, US)와 RPMI2650 (한국세포주은행, KR) 세포주를 이용하였다. A549 세포주는 10% Fetal Bovine Serum (Hyclone, US) 및 1% Penicillin-Streptomycin (Hyclone, US)가 포함된 F12K 배지 (Gibco, US)를 이용하여 37℃, 5% CO2 조건에서 배양하였다. RPMI2650 세포는 10% Fetal Bovine Serum (Hyclone, US) 및 1% Penicillin-Streptomycin (Hyclone, US)가 포함된 RPMI1640 배지 (Hyclone, US)를 이용하여 37℃, 5% CO2 조건에서 배양하였다. 12well plate (Falcon, US)에 A549세포를 5X104 cells/well의 조건으로 분주, RPMI2650 세포는 1.2X105 cells/well의 조건으로 분주하고 다음날 초음파 연무식 흡입기를 이용하여 포집한 SAMiRNA를 농도별로 (0, 0.1, 0.5, 1, 5, 10μM)로 처리하였다.
5-2. 초음파 연무식 흡입기를 사용하여 포집한 SAMiRNA 나노입자의 타겟 유전자 발현 분석
실시예 5-1의 방법으로 초음파 연무식 흡입기를 이용하여 포집한 SAMiRNA 나노입자를 각 세포주에 처리하였으며 24시간 배양 후 Universal RNA extraction kit (Bioneer, KR)을 이용하여 세포 용해물로부터 총 RNA를 추출하고, 이 RNA를 주형으로 AccuPower GreenStar™ RT-qPCR Master Mix (Bioneer, KR)를 이용하여 제조업자의 프로토콜에 따라 인간 AREG 및 RPL13A (Human reference qPCR primer set, Bioneer, KR)의 mRNA 발현 수준을 qRT-PCR로 분석하였다. qPCR array 후 도출되는 두 유전자의 Ct값을 2(- Delta Delta C(T)) Method [Livak KJ, Schmittgen TD. 2001. Analys is of relative gene expression data using real-time quantitativ e PCR and the 2(-Delta Delta C(T)) Method. Methods. Dec;25(4):4 02-8]을 통하여 상대 정량 분석으로 대조군 대비 실험군인 AREG mRNA의 상대적인 양(fold change)을 계산하였다. 각 유전자에 대한 프라이머 서열은 하기와 같다 (표 3).

그 결과, 두 세포주 모두에서 초음파 연무식 흡입기 전, 후 SAMiRNA 나노입자 모두 농도 의존적으로 AREG mRNA 억제 효능을 보이는 것을 관찰하였다 (도 3). 종합해보면, 초음파 연무식 흡입기를 이용하여 포집한 SAMiRNA 나노입자는 물리 화학적 성질이 그대로 유지되면서, 세포독성은 나타나지 않고, 동일한 타겟 유전자 억제능이 유지되는 것을 확인하였다.
실시예 6. 동물모델에서 초음파 연무식 흡입기를 이용하여 투여한 SAMiRNA 나노입자의 전달 효능 평가
6-1. 초음파 연무식 흡입기 통과 후 포집한 형광 라벨된 SAMiRNA (SAMiRNA-Cy5) 나노입자 물성 분석
실시예 2의 방법으로 초음파 연무식 흡입기를 이용하여 포집된 SAMiRNA-Cy5 나노입자를 실시예 3의 방법으로 분석하였다. 그 결과, 초음파 방식의 연무식 흡입기를 이용하여 포집한 SAMiRNA-Cy5 나노입자는 연무식 흡입기 사용 전 물질과 동일한 형광값, 농도, 분자량, 순도, 나노입자 크기, 삼투압몰농도를 확인하였다(도 4).
6-2. 햄스터 동물모델에서 초음파 연무식 흡입기 이용하여 투여한 SAMiRNA-Cy5 나노입자 전달 효능평가
실험동물은 햄스터를 사용하였고, 중앙실험동물로부터 햄스터 (5주령, male) 구입하여 1주간 순화기간을 거쳐 실험을 진행하였다. 초음파 연무식 흡입기 마우스피스를 햄스터 안면부에 장착한 후, 약물챔버에 SAMiRNA-Cy5 (5 mg/ml) 또는 PBS 1ml을 넣고 2분 30초 동안 초음파 연무식 흡입기를 가동하여 햄스터에 노출시켰다. 초음파 연무식 흡입기 노출 24시간 경과 후 폐, 간, 비장, 신장을 적출하여 무게를 측정하였다. 각 장기의 형광 이미지 및 형광값 분석을 위해 Davinch-InvivoTM imaging system (Davinch-K, KR)을 이용하여 진행하였다.
그 결과, PBS를 초음파 연무식 흡입기를 이용하여 노출시킨 햄스터의 장기와 비교하여, SAMiRNA-Cy5를 초음파 연무식 흡입기를 통해 노출시킨 햄스터에서 적출한 폐, 비장, 간, 신장의 장기 무게 및 비율의 변화는 관찰되지 않았다(도 5a). 또한 SAMiRNA-Cy5를 초음파 연무식 흡입기를 통해 노출시킨 햄스터에서 적출한 폐, 비장, 간, 신장 조직 형광 이미지를 Davinch-InvivoTM 장비를 이용하여 관찰한 결과, 간, 비장, 신장에서는 형광이 거의 나타자니 않았지만, 폐에 강하게 형광이 관찰되어 (도 5b), SAMiRNA 나노입자는 초음파 연무식 흡입기를 통해 기도를 통해 폐에 효과적으로 전달되는 것을 확인하였다.
6-3. 햄스터 동물모델에서 초음파 연무식 흡입기를 통해 투여한 SAMiRNA 나노입자의 폐 조직 분포(biodistribution) 분석
실시예 6-2의 PBS 및 SAMiRNA-Cy5를 초음파 연무식 흡입기를 통해 노출시킨 햄스터에서 적출한 폐 조직을 면역형광염색(immunofluorescence)을 수행하였다. 조직 고정을 위해 10% neutral buffered formalin (Sigma, US)에 하루 동안 보관한 후 10%, 20%, 그리고 30%의 수크로스(Sigma, US) 용액에 순차적으로 넣어 탈수를 진행하였다. 각 폐 조직 샘플을 OCT compound (Sakura Finetek, US)가 담겨있는 base mold (Thermo Scientific, US)에 넣고, 액체 질소가 담겨있는 용기에 스테인레스 플레이트를 넣고, 그 위에 올려 OCT compound를 완전히 얼려주었다. 얼린 조직은 -70 ℃에 보관하였으며, 조직 절편기로 자르기 전에 조직 절편이 용이하도록 -20 ℃에 30분 놓아 두었다. 절편 조직은 14 μm 두께로 슬라이드에 올려 1시간 동안 건조시켰다. 세포 투과를 위해 0.1%의 Triton-X100 (Sigma, US) 용액에 10분간 반응시킨 후, 5% normal goat serum (Abcam, GB)과 1% BSA (Sigma, US)가 포함된 용액에 1시간 동안 blocking 하고, 알파-액틴-2 항체(Sigma, US)를 4 ℃에서 하루 간 반응시켰다. PBS 세척 후 2차 항체 anti-mouse-Alexa Fluor 488 (Invitrogen, US)에 1시간 동안 반응 후 세척을 진행하였다. 1 μM DAPI (Sigma, US) 용액에 10분간 반응시켜 세척한 후, mounting solution (Thermo Scientific, US)을 떨어뜨린 후 cover glass (VWR, US)로 덮어 마운팅 과정을 진행하였다. 염색된 조직의 형광 분석을 위해 스피닝 디스크 콘포칼(Dragonfly high-speed confocal image platform, Andor, GB)을 이용하여 분석하였다.
그 결과, 결과 초음파 연무식 흡입기를 이용하여 SAMiRNA-Cy5를 노출시킨 햄스터의 폐 전체에서 형광(SAMiRNA-Cy5)이 관찰되었다. 각 부위별로 확대(고배율 관찰)하여 분석한 결과, 폐를 구성하고 있는 대부분의 세포에 SAMiRNA-Cy5가 잘 전달되어 있는 것을 확인하였고, 특히 폐포 및 기관지에서 강하게 형광이 관찰되는 것을 확인하였다 (도 6). SAMiRNA 나노입자를 초음파 연무식 흡입기를 이용하여 투여했을 때 효과적으로 폐에 전달되는 것을 통해, 이중가닥 올리고뉴클레오티드 구조체 및 나노입자는 초음파 방식의 연무식 흡입기에 최적화된 siRNA 플랫폼임을 확인할 수 있었다.
6-4. 마우스 동물모델에서 초음파 연무식 흡입기 이용하여 투여한 SAMiRNA-Cy5 나노입자의 시간별 조직 형광 이미지 분석
실험동물은 C57BL/6 마우스를 사용하였고, 대한바이오링크부터 마우스 (6주령, male) 구입하여 1주간 순화기간을 거쳐 실험을 진행하였다. 형광 라벨링된 SAMiRNA-Cy5는 실시예 6-1에서 분석된 물질을 사용하였다. 초음파 연무식 흡입기 마우스피스를 마우스 안면부에 장착한 후, 약물챔버에 SAMiRNA-Cy5 (2 mg) 또는 PBS를 넣고 30초(15초 노출-15 휴식-15초 노출 조건) 동안 초음파 연무식 흡입기를 가동하여 투여하였다 (도 7a). 투여 후 시간별(0, 1, 24, 48, 96, 168 시간)로 비강, 폐, 간, 비장, 신장, 심장 등을 적출하여 무게를 측정 후 Davinch-InvivoTM imaging system (Davinch-K, KR)를 이용하여 형광 이미지 분석을 진행하였다. 시간별로 적출한 모든 조직에 대해 동일한 조건(형광세기, 노출시간 등)으로 형광 이미지 분석을 진행하였다.
그 결과, 초음파 연무식 흡입기 투여 1시간 경과 후 적출한 마우스 비강 및 폐 조직에서 형광이 가장 강하게 관찰되어 비강 및 폐 조직으로 SAMiRNA가 효과적으로 전달되는 것을 확인하였다. 반면에 간, 신장 심장, 비장, 뇌, 혈액 등에서는 형광신호가 관찰되지 않는 것을 확인하였다. 24시간 경과 후 비강 및 폐 조직의 형광강도가 1시간에 비해 현저하게 낮아지는 것을 관찰하였고, 48시간 이후 비강 및 폐 조직에서의 형광이 점차적으로 줄어들어 168시간 경과 후에는 폐 조직에서도 형광이 거의 관찰되지 않는 것을 확인하였다(도 7b).
6-5. 마우스 동물모델에서 초음파 연무식 흡입기를 통해 투여한 SAMiRNA 나노입자의 비강 및 폐 조직 정량 분석
실시예 6-4에서 PBS 및 SAMiRNA-Cy5를 초음파 연무식 흡입기를 통해 노출시킨 마우스에서 적출한 비강 및 폐 조직을 이용하여 형광 정량 및 PK 분석을 수행하였다. 적출한 각 조직의 무게를 측정하고 round bottom tube (SPL, KR)에 전체 조직을 넣은 다음 tissue lysis buffer (Bioneer, KR) 1ml을 넣고 homogenizer (IKA, DE)를 사용하여 조직을 분쇄하였다. 15분간 ice에서 incubation한 뒤 14,000rpm, 4℃로 15분간 원심 분리한 후 상층액을 1.5ml amber micro-centrifuge tube (Axigen, US)로 옮겨 보관하였다. PBS 처리군 조직 lysate에 SAMiRNA-Cy5를 농도별(0.1, 0.5, 0.25, 0.125 ug/ml)로 spiking하여 형광정량 분석을 위한 standard sample을 준비하였다. Black 96 well microplate (Corning Costar, US)에 standard 및 sample을 100ul씩 분주한 후, microplate reader (TECAN, CH)로 fluorescence intensity (Excitation Wavelength 645nm, Emission Wavelength 675nm)를 측정하고 산출한 standard curve에 대입하여 조직 내 SAMiRNA-Cy5 잔존량을 분석하였다. SAMiRNA PK 분석을 위해 용해된 조직 샘플들을 QIAshredder (QIAGEN)를 이용하여 한번 더 분쇄하였다. 조직 용해물로부터 anti-mouse AGO2 antibody (Sigma, US)가 부착된 MagListo™ Protein G Kit (Bioneer, KR)를 이용하여 Ago2 immunoprecipitation을 수행하였다. 수득한 RISC-loaded siRNA는 stem-loop RT-qPCR로 절대정량 분석하였다. Taqman MicroRNA Reverse transcription kit (Applied Biosystem, US) 와 Mygenie™ 96 (Bioneer, KR) 그리고 SAMiRNA-mRelA antisense RT 프라이머 (Bioneer, KR)를 이용하여 cDNA로 변환한 다음 AccuPowerㄾ Plus DualStar™ qPCR Master Mix (Bioneer, KR), 600 nM qPCR 프라이머 (Bioneer, KR), 300 nM 프로브, 그리고 Exicycler™ 96 (Bioneer, KR)를 이용하여 qPCR을 수행하였다. 정량에 사용된 표준 물질은 비처리 샘플에 스파이크 후 동일한 과정을 거쳐 분석에 사용되었다. 분석에 사용된 RT 및 qPCR 프라이머 서열은 하기와 같다(표 4).

그 결과, 투여 1시간 뒤에 적출한 폐의 경우 144.18 μg/g (1.3 %), 24시간의 경우 1.27 μg/g (0.01%), 48시간의 경우 0.77 μg/g (0.006%)의 SAMiRNA-Cy5가 확인되었다. 비강의 경우 1시간 경과 후 661.8 μg/g (8.2%), 24과 48시간 경과 후 0.7 μg/g (0.01%) SAMiRNA-Cy5가 확인되었다. 초음파 연무식 흡입기 이용하여 비강 및 폐 조직에 전달된 SAMiRNA는 1시간에 가장 많이 검출되었고, 시간이 지남에 따라 점차적으로 감소되는 것을 확인하였다(도 8a). 폐 조직 PK 결과의 경우에도 폐 조직의 형광정량 분석결과와 유사하게 1시간 경과 후 가장 높은 SAMiRNA antisense copy number (3.E+11)가 확인되었고, 24시간의 경우 2.E+08, 48시간의 경우 6.E+07로 확인되어 시간이 지남에 따라 점차적으로 감소되는 것을 확인하였다(도 8b).
SARS-CoV-2 타겟을 포함하는 SAMiRNA에 대하여도 위와 같은 동일한 방법을 통해 이용하여 비강 및 폐 조직에 전달되는지 여부를 확인할 수 있으며, SARS-CoV-2 타겟 서열은 다음 표 5에 기재된 바와 같다.

6-6. 마우스 동물모델에서 초음파 연무식 흡입기를 통해 투여한 SAMiRNA 나노입자의 폐 조직 분포(biodistribution) 분석
실시예 6-4에서 PBS 및 SAMiRNA-Cy5를 초음파 연무식 흡입기를 통해 노출시킨 마우스에서 적출한 폐 조직을 이용하여 SAMiRNA 분포 분석을 수행하였다. 조직 고정을 위해 10% neutral buffered formalin (Sigma, US)에 하루 동안 보관한 후 10%, 20%, 그리고 30%의 수크로스(Sigma, US) 용액에 순차적으로 넣어 탈수를 진행하였다. 각 폐 조직 샘플을 좌엽과 우엽으로 분리한 후 OCT compound (Sakura Finetek, US)가 담겨있는 base mold (Thermo Scientific, US)에 넣고, 액체 질소가 담겨있는 용기에 스테인레스 플레이트를 넣고, 그 위에 올려 OCT compound를 완전히 얼려주었다. 얼린 조직은 -70 ℃에 보관하였으며, 조직 절편기로 자르기 전에 조직 절편이 용이하도록 -20 ℃에 30분 놓아 두었다. 절편 조직은 8 μm 두께로 슬라이드에 올려 1시간 동안 건조시켰다. 세포 투과를 위해 0.1%의 Triton-X100 (Sigma, US) 용액에 10분간 반응시킨 후, 5% normal goat serum (Abcam, GB)과 1% BSA (Sigma, US)가 포함된 용액에 1시간 동안 blocking을 진행하였다. 이후 1 μM DAPI (Sigma, US) 용액에 10분간 반응시켜 세척한 후, mounting solution (Thermo Scientific, US)을 떨어뜨린 후 cover glass (VWR, US)로 덮어 마운팅 과정을 진행하였다. 염색된 조직의 형광 분석을 위해 스피닝 디스크 콘포칼(Dragonfly high-speed confocal image platform, Andor, GB)을 이용하여 분석하였다. 정확한 시간별 폐 조직내 SAMiRNA 분포를 분석하기 위해 모든 폐 조직 샘플에 대해 동일한 조건(형광세기, 노출시간 등)으로 confocal microscope 분석 진행을 진행하였다.
그 결과, 폐 조직의 형광정량 및 PK 분석결과와 유사하게 1시간 경과 후 가장 높은 SAMiRNA-Cy5 형광이 관찰되었고, 시간이 지남에 따라 점차적으로 감소하는 것을 확인하였다. 폐 조직에서 형광이 관찰되는 1시간, 24시간, 48시간 폐 조직 전반에 골고루 분포되어 있는 것을 관찰하였다(도 9a). 투여 1시간 경과 후 좌·우 폐 조직을 관찰한 결과 SAMiRNA가 좌·우 폐 전체에 골고루 분포되어 있어 효과적으로 전달되는 것을 확인하였다(도 9b). 또한 투여 1시간 경과 폐 조직을 고배율로 관찰한 결과 폐 조직을 구성하고 있는 대부분의 세포에서 SAMiRNA-Cy5 가 관찰되었고, 세기관지 및 세기관지 상피세포, 폐포 상피세포, 폐포 주머니 등에 골고루 분포되어 있는 것을 관찰하여 효과적으로 전달된 것을 확인하였다(도 9b).
이상으로 본 발명 내용의 특정한 부분을 상세히 기술하였는 바, 당업계의 통상의 지식을 가진 자에게 있어서 이러한 구체적 기술은 단지 바람직한 실시 양태일 뿐이며, 이에 의해 본 발명의 범위가 제한되는 것이 아닌 점은 명백할 것이다. 따라서 본 발명의 실질적인 범위는 첨부된 청구항들과 그것들의 등가물에 의하여 정의된다고 할 것이다.

Claims (25)
- 하기 구조식 1의 구조를 가지는 이중가닥 올리고뉴클레오티드를 포함하는 자가조립 나노입자(SAMiRNA)를 포함하는 호흡기 바이러스 감염증, 바이러스 감염에 의한 폐섬유증, 또는 호흡기 질환 예방 또는 치료용 약학 조성물로서, 상기 자가조립 나노입자는 초음파에 의해 에어로졸을 생성하는 연무식 흡입기를 이용하여 투여되는 것을 특징으로 하는 약학 조성물:

상기 구조식 1에서 A는 폴리에틸렌 글리콜(polyethylene glycol) 또는 헥사에틸렌 글리콜(hexaethylene glycol)을 포함하는 친수성 물질이고;

X 및 Y는 각각 독립적으로 단순 공유결합 또는 링커가 매개된 공유결합을 의미하며; 그리고
R은 타겟 유전자를 표적하는 안티센스 가닥(anti-sense strand) 및 안티센스 가닥에 상보적인 센스 가닥(sense strand)을 포함하는 이중가닥 올리고뉴클레오티드를 의미함.

PNA(peptide nucleic acid), LNA(locked nucleic acid) 및 UNA(unlocked nucleic acid) 형태로의 변형;으로 구성된 군에서 선택된 어느 하나 이상인 것을 특징으로 하는 약학 조성물.
