Description
본 발명은 (R)-펄린돌 광학이성질체 또는 그의 약학적으로 허용가능한 염, 및 이를 함유하는, 통증의 치료 및 예방을 위한 조성물에 관한 것이다.
종래 기술 배경
통증 상태의 치료는 의약에 있어 극히 중요하며 전세계적으로 통증의 치료 및 예방을 위한 부가적인 치료법이 요구되고 있다.
통증은 환자들이 그 의학적 주의를 구하는 가장 흔한 증상이다. 비록 이에 대한 정확한 정의는 없지만, 실제 또는 잠재적인 조직 손상과 연관된 불쾌한 감각적 또는 감정적 경험이라 정의될 수 있으며, 또는 이러한 손상 관점에서 기술된다 (International Association for the Study of Pain, IASP).
통증은 복합적인 발병요인을 제시하는 매우 복합적인 증상이다. 통증의 존재는 의학 분야에서 다수의 증후군 및 질병에서 설명되고 있다. 몇몇 형태에서, 통증은 다양한 특성의 불능, 일상 활동을 영위하는데 있어서의 어려움, 분위기, 프로페셔널한 퍼포먼스 및 사회적 관계의 간섭으로 나타난다.

국소해부학적 분포와 관련하여, 통증을 3가지, 즉 말초 통증, 중추 통증 및 심인성 통증으로 분류할 수 있다.
말초 통증은 신경 임펄스를 전달하는 한편, 통증 센세이션의 기원이 되는 말초 신경 섬유에서의 유기적인 질병의 결과로 생긴다. 말초 통증은 또한 표재성(superficial) 말초 통증과 심부(deep) 말초 통증으로 나뉜다. 전자는 급성의 침투성이고 통증의 기원 지점에 위치한다. 심부 말초 통증은 몸통증(somatic)과 내장통증(visceral) 것으로 분류될 수 있을 것이다. 몸의 심부 말초 통증은 국소화되거나(통증이 아픈 자극의 기원점에 위치함) 또는 방사상(통증이 확산되어 아픈 자극의 기원점으로부터 멀어짐)이다. 마지막으로, 심부 내장 말초 통증은 국소해부학적으로 한정짓기가 어렵다.

병심리학적 관점에서 볼 때, 통증은 통각수용적(nociceptive)이거나 신경병성(neuropathic)이다.
통각수용적 통증은 비-신경 조직에 실질적 또는 위협적 손상을 일으키는 통증으로 정의될 수 있으며 이는 통각수용기의 활성화에 기인한다. 통각수용적 통증은 몸통증이거나 내장통증으로 분류될 수 있다. 몸통증은 예컨대 뼈, 관절, 근육 또는 피부와 같은 체세포에 있어서의 통각수용적 수용체의 활성화에 기인한다. 내장통증에서는 내장의 통각수용기가 다른 병인성 메카니즘(예컨대 기계적 손상, 염증, 방사능, 독성 물질)에 의해 활성화된다. 내장 및 몸의 통각수용적 통증 두 가지 모두 급성 또는 만성적일 수 있다. 내장 통증은 특징화하기가 보다 어렵고 보통의 통증 치료보다 덜 민감하다. 몇몇 통증 증후군(예컨대 암 통증)은 내장 및 몸의 통각수용적 통증 두 가지 요소를 모두 포함한다.
신경병 통증은 몸감각(somatosensory) 시스템 (즉 말초 신경, 후근 신경절 또는 후근 또는 중추신경계)에 영항을 미치는 질병이나 병변의 직접적인 결과로서 일어나는 통증으로서 정의할 수 있다.
신경 조직에 대한 손상 또는 장애를 만들어내는 것으로 알려진 병리학적 과정의 대부분은 신경병통증의 잠재적인 원인일 수 있다; 바이러스성, 세균성, 무균성 염증, 신생물이나 기타 구조적 병변으로 인한 압력, 퇴행성, 허혈, 자가면역, 독성, 트라우마 또는 내분비/대사적 대사들 모두가 신경병 통증의 원인과 연관되어 있다.
그러나, 가장 흔하게 연구된 신경병 통증 상태는 당뇨병성 신경병 통증 (당뇨병성 NP), 포진후 신경통 (PHN), 삼차 신경통 및 중추신경병 통증 (척수 손상 (SCI), 중추 뇌졸중후 통증(CPSP) 및 다발경화증 관련 통증)이다.
그 밖의 신경병 통증 상태로는 HIV 감염과 연관된 신경병증(HIV 신경병증), 외상후 또는 수술후 신경병 통증, 만성 신경근병증, 암과 연관된 신경병 통증, 환상 통증 및 다발-병인성 신경병 통증을 들 수 있다.
신경병증은 신경에 영향을 미치는 기능성 질병 또는 병리학적 변화이다. 이것은 만일 이것이 단일의 신경줄기와 관련될 경우에는 단일신경병증이라 칭해지고, 만일 수 개의 신경줄기와 연속적으로 연관될 경우에는 다중 단일신경병증이라 칭해지며, 만일 확산적으로 그리고 쌍방적으로 수 개의 신경 줄기와 연결될 경우에는 다발신경병증이라 칭해진다.
말초 신경병증은 감각성(sensory), 운동성(motor) 또는 자율성(autonomic)일 수 있다. 가장 흔한 운동성 발현은 근육 연축, 클로누스(clonus), 근다발수축(fasciculations), 근위축증(amyotrophy) 및 근력 및 기민성 상실일 수 있다.
네거티브 감각성 소견에는 통각저하증 및 감각저하증이 포함된다. 한편, 포지티브 감각성 소견에는 자통(pricking), 저림 또는 이명 등의 감각 외에도 감각이상, 감각장애, 통각과민(hyperpathia), 통각과민(hyperalgesia) 및 이질통증이 포함된다. 신경병증의 예로는 말초 다발신경병증을 들 수 있는데, 걷는 것에 의해 악화되고 족부나 손가락 끝에 작열통이나 바늘로 찌르는 듯한 자통 감각, 이질통증, 족부 감각저하증, 및 아킬레스 반응의 부재; 수 초 동안 윗입술과 코에서 쇼크와도 같은 심각한 급성 통증을 일으키고 씹거나 칫솔질에 의해 악화되며 신경학적 검사시 변화를 수반하지 않음을 특징으로 하는 삼차 심경통증; 첫째, 둘째 및 셋째 손가락과 손바닥에서 급성 통증, 저림 감각 및 저하된 민감성에 의해 특징지어지고, 야간에 악화되며 엄지를 벌릴때(thumb abduction) 근력 감소 및 포지티브 팔렌(Phalen) 테스트를 특징으로 하는 손목굴 증후군; 허벅지 측면의 작용통과 자통 감각 및 피부 과민성의 제한된 영역 존재를 특징으로 하는 외측대퇴피신경의 단일신경병증; 소포 출현 후 흉부상에 배너같은 분포와 함께 작열통을 동반하고, 피부 치유 후에도 개선되지 않고, 고색소 피부 반점 및 이질통증을 특징으로 하는 포진후 신경통을 들 수 있다.
개인 및 사회적인 복지에 미치는 통증의 영향은 최근 수십년간 증가일로에 있다.
이와 관련하여, 신경병 통증은 관리하기가 가장 어려운 통증 증후군의 하나로 널리 인식되고 있으며 치료 결과도 대개 만족스럽지 못하다. 신경병 통증은 유럽 전체 인구의 7-8%에 영향을 미칠 정도로 매우 만연해 있다 (European Federation of Neurological Societies guidelines on the pharmacological treatment of neuropathic pain, 2010).
신경병 통증의 관리는 복잡다단한 노력을 필요로 하며 종종 환자와 치료의사를 좌절하게 한다. 기저의 생리병리학적 프로세스 변화에 목적이 있는 것이 아니거나, 또는 치료이하(sub-therapeutic) 투여량으로 투여되는 작용 메카니즘을 갖는 약물들이 통증 및 특히 신경병 통증의 치료에 흔히 이용된다.
이는 약물에 대한 작용 메카니즘의 상대적으로 불완전한 이해와 현재 이용가능한 진통제의 제한된 효과로부터 비롯된 것이다. 치료적 접근법은 의사마다 매우 다르며 다루기 힘든 만성 통증 증후군은 학제간 접근법과 보다 강력한 약물 요법을 보장한다.
진통제 약물 사용은 단계적인 접근법을 따른다. 제1 단계는 약한 통증에 대응하는 것으로, 비-아편유사제 진통제 약물(소염 약물 및 파라세타몰)로 처리하여야 하고; 제2 단계는 중간 정도의 통증에 대한 것으로, 약한 아편유사제가 사용되며 세 번째 단계에서는, 심한 통증에 대한 것으로 강력한 아편유사제가 처방된다. 통증의 강도에 따라, 적절한 투여량과 함께, 대응하는 수준에서 출발하여야 한다. 만일 통증이 지속되거나 악화되면, 약물 투여량을 최적화하여야 한다.
진통제 약물은 2가지로 크게 분류될 수 있는데; 비-아편유사제 및 아편유사제 진통제 약물이 그것이다.
비-아편유사제 진통제 약물로는 파라세타몰 및 비스테로이드계 소염 약물(NSAIDs)을 들 수 있다. 파라세타몰은 그 작용 메카니즘이 잘 알려져 있지 않지만 중추 작용에 관여하는 것으로 추정된다. 대량을 즉시 또는 만성적으로 투여하면 설프히드릴 래디칼의 고갈과 연계된 간독성 위험이 있다.
비스테로이드계 소염 약물은 사이클로옥시게나제 (COX) 억제, 염증 프로세스 및 통증과 연관된 프로스타글란딘 (PG)의 합성 저해에 의해 작용한다. 이들은 아세틸살리신산 (AAS), 이부프로펜 또는 디클로페낙과 같이 비선택적으로 COX1 및 COX2를 억제하거나, 또는 셀레콕시브, 로페콕시브 또는 에토리콕시브와 같이 COX2만을 선택적으로 저해할 수 있다. 비스테로이드계 소염 약물은 위장관 (구역질, 상복부통증, 궤양화, 출혈), 신장 (급성 신부전, 간질 신장염), 혈액학적 (혈소판 항응집, 출혈) 또는 과민성 반응 등을 비롯하여 몇가지 부작용을 나타낸다. 이러한 효과는 비선택적 NSAIDs의 경우 가장 악명이 높다. 소위 약한 아편유사제 약물로는 트라마돌, 하이드로코돈 및 덱스트로프로폭시펜과 같은 약물을 들 수 있다. 이 약물들은 높은 투여량에서 유해 반응 빈도가 증가하기 때문에 최대 약량학을 갖는다. 트라마돌은 아편유사제 μ 수용체에 결합하여 세로토닌의 신경 재흡수를 억제함으로써 작용하는 코데인의 합성 유도체이다. 트라마돌은 간에서 대사되어, 신장에서 배출되므로, 신부전을 앓는 환자에 있어서는 투여량 감소 또는 투여 간격 연장이 요구될 수 있다.
하이드로코돈은 코데인과 덱스트로프로폭시펜의 합성 유도체로서 메타돈과 유사한 구조를 나타낸다. 약한 통증 내지 보통 통증을 갖는 환자들은 파라세타몰 또는 NSAID와 연관된 약한 아편유사제로 치료되어야 한다. 만일 통증을 치료적 투여량으로도 제어할 수 없을 경우에는, 이 그룹 내의 다른 약물로 갈아탈 것을 고려해서는 아니되며 보다 강력한 아편유사제를 사용하여야 한다.
아편유사제는 수용체 μ, κ, 및 δ에 결합함으로써 작용하며 이들의 고유한 활성에 따라, 작용제 (모르핀, 펜타닐, 메타돈, 옥시코돈, 하이드로모르폰 및 페티딘), 부분 작용제/길항제 (부프레노르핀) 및 작용제/길항제 (펜타조신)으로 분류된다. 순수한 작용제는 μ 수용체 및 그 밖의 수용체에 대해 영향력을 발휘하는 약물이다. 부분 작용제/길항제는 수용체에 대해 부분적인 작용제 효과를 나타내고 적어도 1종의 수용체에서 길항제 효과를 나타내는 것이고, 작용제/길항제는 수용체에 대해서는 순수한 작용제 효과를, 그리고 적어도 1종의 수용체에 대해 길항제 효과를 발휘한다.
아편유사제는 예측가능한 바람직하지 못한 부작용을 유도하는데, 이러한 부작용이 최소화되지 않을 경우, 투여량 적정 및 환자의 치료 순응성을 해칠 수 있다. 부작용으로는 호흡곤란, 구역질 및 구토, 변비, 뇨축적, 이상황홀감, 진정, 축동, 진해 작용, 고혈압, 서맥, 인지 변화 (환각, 망상 관념), 통각과민, 간대성근경련증, 소화불량, 가려움증, 내성 및 의존성을 들 수 있다.
모르핀은 경구 또는 비경구 경로(피하 및 정맥내)로 투여될 수 있다. 정맥내 투여는 특별한 주의를 요하며 입원 환자에 대해서만 이용할 수 있다는 제약이 있다. 피하 투여를 위해 주입 펌프가 이용된다.
트리시클릭 항우울제(아미트립틸린, 노르트립틸린 및 데시프라민)가 진통제 약물과 조합되어 사용되며, 특히 아편유사제는 대부분 신경병 통증 치료시 조합적으로 사용된다. 항콜린 작용으로 인해, 유해 반응에는 변비, 구강건조증, 뇨축적 및 빈맥이 포함된다.
항경련제 (카르바마제핀, 옥스카르바제핀, 페니토인, 소듐 발프로에이트, 클로나제팜, 라모트리진, 가바펜틴, 프레가발린) 역시도 통증, 특히 신경병 통증에 이용된다. 카르바마제핀 및 페니토인은 간 독성, 백혈구감소증 및 저혈소판증을 일으킬 수 있기 때문에, 혈청 농도를 모니터링할 필요가 있다.
코르티코스테로이드 중에서는 덱사메타손이 통증 치료에 가장 흔히 이용되는데, 이 약물은 위장관 질환, 당뇨병, 신경-정신 질환 및 근위(proximal) 근육병증의 개시와 연관이 있다.
그러므로 본 발명이 속한 기술 분야에서는 질병의 한 증상 뿐 아니라 질병 자체로서의 통증을 치료하는데 적합한 새로운 치료적 접근이 여전히 필요한 실정이다.
본 발명이 해결하고자 하는 문제는 단독으로 또는 다른 진통제와 조합하여 부작용은 감소되면서 목적하는 진통 활성은 유지하는, 효과적인 치료를 제공하는 통증 치료를 위한 대안적이고도 신규한 그리고 효과적인 물질을 제공하는 것이다.
발명의 간단한 설명
본 발명은 광학 활성적인 화합물 (R)-펄린돌 광학이성질체가 유리한 진통 작용을 나타내고, 통증의 치료 및 예방에 유용하다는 발견에 기초한 것이다.
본 발명의 발명자들은 놀랍게도 광학 활성적인 (R)-펄린돌 광학이성질체가 α2A 아드레날린 수용체에 대해 길항제 효과를 나타냄을 발견하였는데, 이는 통증, 특히 신경병 통증의 치료 및 예방에 있어서 (R)-펄린돌 광학이성질체의 예기치 못한 활성을 설명해주는 것이다.
(R)-펄린돌은 유일한 활성 성분으로서 단독으로 사용되거나 또는 부가적인 진통제와 함께 조합 치료시 통증, 특히 신경병 통증의 치료 및 예방을 위한 효과적인 물질인 것으로 나타났는데, 조합 사용되는 경우 (R)-펄린돌은 부가적인 진통제의 효과를 증강시키는 것으로 입증되었다.

(R)-펄린돌 광학이성질체의 α2A 아드레날린 수용체 길항제 활성의 테스트
α2A 아드레날린 수용체를 발현하는 CHO (차이니즈 햄스터 난소: Chinese hamster ovary) 세포의 형태학적 반응을 평가하는 플랫폼으로 기능적인 α2A 아드레날린 수용체 길항성 분석을 실시하였다. 이 기능성 분석은 테스트 대상으로서 (R)-펄린돌 및 (rac)-펄린돌을 이용하여 수행하였다.
인간의 α2A 아드레날린 수용체를 발현하는 CHO 세포들을 실험 전날에 적절한 배양 배지에 플레이팅하였다. 화합물들을 웰에 첨가하고 6분 후 판독하였다. 샘플로부터의 반응을 대조군 반응과 비교하여 작용제 활성 또는 길항제 활성을 평가하였다. 최대 활성 백분율 (% 최대 활성)은 Sigma-Aldrich사가 시판하는 α2A 작용제인, UK-14304 (5-브로모-N-(2-이미다졸린-2-yl)-6-퀴녹살린아민, 5-브로모-N-(4,5-디히드로-1H-이미다졸-2-일)-6-퀴녹살린아민, 브리모니딘)에 의해 유도된 작용제 활성의 백분율로서 나타내었다.
(R)-펄린돌 광학이성질체와 (rac)-펄린돌은 α2A 아드레날린 수용체에 미치는 UK-14304의 작용제 활성에 반대작용하였다. 예기치 않게, (R)-펄린돌은 α2A 수용체 길항제 활성을 나타내었다. 예를 들어, 10 μM의 농도에서, (R)-펄린돌의 존재 하에서 UK-14304의 작용제 활성 (59.72%)은 (rac)-펄린돌의 존재 하에서의 UK-14304의 작용제 활성 (89.02%)보다 더 낮다. 따라서, (R)-펄린돌은 작용제 활성을 40.28% 저하시키는 반면, (rac)-펄린돌은 작용제 활성을 단지 10.98% 저하시킬 따름이다. 그러므로, (rac)-펄린돌은 α2A 아드레날린 수용체에 대한 길항제 활성을 (R)-펄린돌에 비해 약 4배(4x) 덜 나타낸다.
이러한 예기치 못한 결과는 통증, 특히 신경병 통증 증후군에 대한 (R)-펄린돌의 장점을 뒷받침하는 것이다.
