Abstract
본 발명은 N-말단이 절단된 WT1(Wilm's tumor protein, ΔWT1), 상기 ΔWT1을 포함하는 재조합 아데노바이러스, 상기 재조합 아데노바이러스로 전이된 수지상세포, 상기 수지상세포로 T 세포를 자극하여 ΔWT1 특이 T 세포를 제조하는 방법, 상기 ΔWT1 특이 T 세포를 함유한 종양 치료용 조성물 및 상기 ΔWT1 특이 T 세포가 주입된 수여자에서 ΔWT1 특이 T 세포를 검출함으로써 치료 효능을 검증하는 방법에 관한 것이다.

Description
본 발명은 사람에서 N-말단이 절단된 WT1 에 특이적 CD4+, CD8+ T 세포를 이용한 입양 면역치료법과 특이 효능 검사법에 관한 것으로, 보다 구체적으로는 N-말단이 절단된 WT1(Wilm's tumor protein, ΔWT1), 상기 ΔWT1을 포함하는 재조합 아데노바이러스, 상기 재조합 아데노바이러스로 전이된 수지상세포, 상기 수지상세포로 T 세포를 자극하여 ΔWT1 특이 T 세포를 제조하는 방법, 상기 ΔWT1 특이 T 세포를 함유한 종양 치료용 조성물 및 상기 ΔWT1 특이 T 세포가 주입된 수여자에서 ΔWT1 특이 T 세포를 검출함으로써 치료 효능을 검증하는 방법에 관한 것이다.
동종이형 HSCT(hematopoietic stem cell transplantation)는, 조건화 치료법에서 화학방사선요법의 조합 및 GVL(graft-versus-leukemia) 효과에 기인하여, AML(acute myeloid leukemia)에 최고의 치료 기회를 제공한다(Goldman JM et al. Ann Intern Med. 1988;108:806-814, Horowitz MM et al. Blood. 1990;75:555-562, Goulmy E et al. N Engl J Med. 1996;334:281-285). 동종이형 HSCT는 재발로 인하여 예후가 좋지 않기 때문에 보조적 치료(support care), 화학요법, 이차 HSCT 및 DLI(donor lymphocyte infusion)과 같은 치료 옵션이 수행되어야 한다. 이식후 AML의 치료에서 DLI 치료법을 이용하는 것이 적절함을 입증하는 증거가 있지만, 소수의 환자에서 제한된 임상 효과만을 보였다(Schmid C et al. J Clin Oncol. 2007 Nov 1;25(31):4938-45). DLI에 대한 반응 속도를 높이기 위한 한 전략으로, 구제 화학요법과 DLI의 조합에 대하여 연구되어 왔다(Levine JE et al. J Clin Oncol. 2002 Jan 15;20(2):405-12). 또한, 공조자극 분자의 유도, 타겟 치료법과의 조합(Shimoni A et al. Leukemia. 2003 Feb;17(2):290-7) 또는 CD8+ 세포의 제거와 같은 다른 몇 가지 전략이 DLI에 대한 반응 속도를 높이기 위해 실험되고 있다(Giralt S et al. Blood. 1995 Dec 1;86(11):4337-43).
GVL 반응에 대한 실질적인 타겟이 악성 표현형의 유지에 필수적인 백혈병 특이적인 항원이므로, 궁극적으로는 인입(adoptive) 면역요법을 이용한 백혈병 특이적 DLI가 최소한의 독성으로 재발을 억제하고 치료하는데 가장 효과적인 방법일 수 있다. 조작되지 않은 DLI 대신에 시험관에서 생산한 백혈병 특이적인 CTL로의 치료가 생체내에서 충분치 않은 항원 제시를 극복하고 백혈병 세포에 특이적인 면역반응을 유도할 수 있다. GVHD가 없거나 약하게 T 세포 이식의 충분한 임상적 활성은 이식후 사이토메갈로바이러스 이식 및 EBV-유도 림프구증식의 치료에 대한 연구로부터 제안되어 왔다(Einsele H et al. Blood. 2002 Jun 1;99(11):3916-22, Rooney CM et al. Blood. 1998 Sep 1;92(5):1549-55). 그러나, 백혈병에 특이적이지 않은 백혈병에 반응하는 CTL의 입양 전달이 연구되어 왔고 임상에서 제한적 효과만을 보였다(Falkenburg JH et al. Blood. 1999 Aug 15;94(4):1201-8, Montagna D et al. Exp Hematol. 2003 Nov;31(11):1031-8).
골수성 악성 환자의 면역요법에서 강력한 타겟 항원은 정상 조혈세포에서보다 백혈병 세포에서 풍부한 WT1(Wilm's tumor protein)이다(Menssen HD et al. Int J Cancer. 1997;70:518-523, Baird PN et al. Exp Hematol. 1997;25:312-320). WT1은 백혈병 유발에 관여하는 것으로 보이고, WT1 단백질 발현은 백혈병 세포의 생존을 유지하는데 필수적이다(Algar EM et al. Oncogene. 1996;12:1005-1014, Yamagami T et al. Blood. 1996;87:2878-2884, Svedberg H et al. Oncogene. 1998;16:925-932, Tsuboi A et al. Leuk Res. 1999;23:499-505). 또한, WT1은 백혈병 환자에서 심지어 동종이형 HSCT 후에도 자발적인 면역반응을 유발하는 것에 의하면, 명백하게 면역원성이 있다(Scheibenbogen C et al. Blood. 2002 Sep 15100(6):2132-7, Rezvani K et al. Clin Cancer Res. 2005 Dec 15;11(24 Pt 1):8799-807). WT1 펩타이드를 이용한 백신 전략이 임상 실험에서 연구되어 왔고(Oka Y et al. PNAS USA. 2004 Sep 21;101(38):13885-90) 동종이형 HSCT 세팅(setting)과 조합으로 시도되었다(Mailㅴnder V et al. Leukemia. 2004 Jan;18(1):165-6, Kitawaki T et al. Am J Hematol. 2008 Apr;83(4):315-7). WT1에 특이적인 TCR (T-cell receptor)로 전이된 T 세포는 시험관에서 백혈병 세포를 살상하고, 사람 백혈병 세포를 가진 NOD/SCID(nonobese diabetic-severe combined immunodeficiency) 마우스에서 백혈병을 제거하였다(Xue SA et al. Blood. 2005 Nov 1;106(9):3062-7).
그러나, WT1이 면역원성이 있어, 환자의 체내에서 천연적으로 WT1 특이 T 세포가 생성되기 때문에 환자의 체내에 존재하는 WT1 특이 T 세포가 인공적으로 주입된 WT1 특이 T 세포로부터 유래된 것인지 천연적으로 생성된 것인지 구분하기 어려웠다.
본 발명자는 백혈병 등의 치료 뿐만 아니라 이 치료의 효과를 모니터링하는데 사용할 수 있는 WT1을 제공하기 위해 노력하던 중, WT1의 1부터 147까지의 아미노산이 절단된 WT1의 절편이 백혈병을 치료함과 동시에 상기 치료가 인공적으로 주입된 WT1 특이 T 세포에 기인한 것임을 검증할 수 있다는 것을 확인하고, 본 발명을 완성하기에 이르렀다.
본 발명은 한 관점으로서, 전장 WT1(Wilm's tumor protein)으로부터 1 내지 147의 아미노산이 절단된 WT1(ΔWT)을 제공하고자 한다.
다른 관점으로서, 상기 ΔWT1을 포함하는 재조합 아데노바이러스를 제공하고자 한다.
또 다른 관점으로서, 상기 재조합 아데노바이러스로 전이된 수지상 세포를 제공하고자 한다.
또 다른 관점으로서, 상기 수지상세포로 T 세포를 자극하여 ΔWT1 특이 T 세포를 제조하는 방법을 제공하고자 한다.
또 다른 관점으로서, 상기 ΔWT1 특이 T 세포를 함유한 종양 치료용 조성물을 제공하고자 한다.
또 다른 관점으로서, ΔWT1 특이 T 세포가 주입된 T 세포에서 (a) WT1의 1 내지 147 아미노산내 에피토프 또는 그 에피토프를 포함하는 단백질에 대한 면역반응과 (b) ΔWT1내 에피토프 또는 그 에피토프를 포함하는 단백질에 대한 면역반응을 분석하고 비교하여 ΔWT1 특이 T 세포의 효능을 검증하는 방법을 제공하고자 한다.
본 발명은 N-말단이 절단된 WT1(ΔWilm's tumor protein, ΔWT1)에 관한 것이다. 구체적으로, 본 발명의 "ΔWT1"은 전장(full-length) WT1(Wilm's tumor protein)으로부터 N-말단에서 1 내지 147까지의 아미노산이 절단된(제거된) WT1의 절편이다(도 1 참조). 본 발명의 ΔWT1은 에피토프 126(RMFPNAPYL, 서열번호: 3)이 결실되고, 에피토프 187(SLGEQQYSV, 서열번호: 4)과 235(CMTWNQMNL, 서열번호: 5)를 포함하고 있어 종양의 치료 뿐만 아니라 치료 효능의 검증에도 유용하다.
본 발명의 ΔWT1을 코딩하는 핵산은 당업계에 공지된 방법을 사용하여 제조할 수 있으며, 한 양태로는, 전장의 WT1 염기서열을 참조하여 본 발명의 ΔWT1에 특이적으로 결합할 수 있는 프라이머를 작제한 후 이 프라이머를 이용하여 PCR을 수행함으로써 제조할 수 있다.
본 발명은 상기 ΔWT1을 포함하는 재조합 아데노바이러스에 관한 것이다. ΔWT1 항원이 재조합된 아데노바이러스를 사용하는 경우 항원의 내인성 발현으로, 특히 항원제시세포의 MHC class I pathway를 통한 세포독성 T 세포의 면역반응 유도능을 증가시킬 수 있다. 또한, 재조합된 아데노바이러스가 ΔWT1 항원을 수지상세포에 전이함으로써 수지상세포가 ΔWT1 내의 여러 에피토프를 발현할 수 있고, 아데노바이러스 자체가 수지상세포를 활성화시켜 T 세포, 주로는 CD8+ T 세포의 면역반응 유도능을 증가시킬 수 있도록 한다. 본 발명의 ΔWT1을 포함하는 재조합 아데노바이러스는 통상의 재조합 DNA 기술을 참조하여 제조할 수 있다.
본 발명은 상기 ΔWT1로 형질감염된 재조합 아데노바이러스로 전이된(transduced) 수지상세포(dendritic cell)에 관한 것이다. 수지상세포는 수여자의 혈액으로부터 당업계에 공지된 방법(예, 밀도 구배 원심분리 등)을 이용하여 분리될 수 있다. 상기 재조합 아데노바이러스는 통상의 감염(infection) 과정을 통해 수지상세포로 전이될 수 있다.
본 발명은 상기 수지상세포로 T 세포를 자극하여 ΔWT1 특이 T 세포를 제조하는 방법에 관한 것이다. T 세포는 수여자의 골수 또는 혈액으로부터 당업계에 공지된 방법(예, 밀도 구배 원심분리 등)을 이용하여 분리될 수 있다. ΔWT1 특이 T 세포의 증식을 위해 여러 차례 수지상세포로 자극하는 것이 바람직하다. 상기 T 세포는 CD4+ T 세포 및 CD8+ T 세포 중 적어도 하나일 수 있다.
본 발명에서 'ΔWT1 특이 T 세포'는 ΔWT1, 이를 포함한 단백질(예, 전장 WT1) 또는 이들을 발현하는 세포에 대하여 면역반응을 유발하는 T 세포를 의미한다. 여기서, 면역반응은 T 세포의 증식, 사이토카인의 분비 또는 세포독성 등을 의미한다. 본 발명의 ΔWT1은 에피토프 126을 포함하지 않고 에피토프 187 및 235를 포함하므로 본 발명의 ΔWT1 특이 T 세포는 에피토프 126 또는 이를 포함하는 단백질에 대하여 에피토프 187 및 236중 적어도 하나 또는 이를 포함하는 단백질에 비해 낮은 면역반응을 보일 것이다.
본 발명은 ΔWT1 특이 T 세포를 함유한 종양 치료용 조성물에 관한 것이다. 상기 조성물은 T 세포를 보호 및 유지하고, 주입시 도움을 주는 하나 이상의 희석제를 포함하는 것이 바람직하다. 상기 희석제는 생리식염수, PBS(Phosphate Buffered Saline), HBSS(Hank's balanced salt solution) 등의 완충용액, 혈장 또는 혈액성분 등이 있을 수 있다.
상기 '종양'은 WT1 단백질을 발현하는 종양을 포함하며, 예를 들면, 백혈병(예, 급성 골수성, 급성 림프성 및 만성 골수성) 및 암(예, 유방, 폐, 갑상선 또는 위장관 암 또는 흑색종)을 포함하지만, 이에 제한되는 것은 아니다.
본 발명은 ΔWT1 특이 T 세포가 주입된 T 세포에서 (a) WT1의 1 내지 147 아미노산내 에피토프 또는 그 에피토프를 포함하는 단백질에 대한 면역반응과 (b) ΔWT1내 에피토프 또는 그 에피토프를 포함하는 단백질에 대한 면역반응을 분석하고 비교하여 ΔWT1 특이 T 세포의 효능을 검증하는 방법에 관한 것이다. 상기 'WT1의 1 내지 147 아미노산내 에피토프'는 바람직하게는 WT1의 에피토프 126을 의미한다. 상기 'ΔWT1내 에피토프'는 바람직하게는 에피토프 187과 236 중 적어도 하나를 의 미한다. 앞서 설명된 바와 같이 본 발명의 ΔWT1 특이 T 세포는 에피토프 126에 대하여 에피토프 187 및 236에 비해 낮은 면역반응을 보인다. 따라서, 상기 ΔWT1 특이 T 세포를 주입받은 수여자에서 본 발명에 따른 ΔWT1 특이 T 세포의 치료 효능을 검증할 수 있다.
상기에서 ΔWT1 특이 T 세포를 분석하는데 적용할 수 있는 방법으로는 FACS(fluorescence-activated cell sorting)또는 ELISPOT(enzyme-linked immuno spot) 측정법이 있다.
본 발명의 WT1의 1부터 147까지의 아미노산이 절단된 WT1(ΔWT1)은 백혈병, 유방암, 폐암, 갑상선암, 위장관암 및 흑색종을 포함한 WT1을 발현하는 종양의 입양면역치료 뿐만 아니라 상기 치료가 본 발명의 ΔWT1 특이 세포독성 T 세포에 의한 것인지 여부를 면역학적으로 검증하는데 이용될 수 있다.
이하, 실시예는 오로지 본 발명을 보다 구체적으로 설명하기 위한 것으로서, 본 발명의 요지에 따라 본 발명의 범위가 이들 실시예에 의해 제한되지 않는다는 것은 본 발명이 속하는 기술분야에서 통상의 지식을 가진 자에게 있어서 자명할 것이다.
재료 및 방법
<실시예 1: 병력>
이차 AML 48세 환자에게 HLA 매칭 동기(HLA-A*0201/-, B*1507/5101, Cw*010/1402, DRB1*0901/1201, DQB1*0301/0303, DPB1*0202/0501)로부터 감소된 강도 조건(플루다라빈 및 부설판)을 이용하여 HSCT를 수행하였다. 5개월째 면역억제제의 중지 후에도 GVHD는 발병하지 않았으나, 10개월째 재발이 확인되었다. 구제 요법(플루다라빈-사이타라빈-이다루비신) 및 조혈모세포이식과 증가된 용량 스케줄의 DLI가 6주 동안 일시적인 완화를 유도하였다. 인터페론-α와 과용량 스케줄의 DLI가 시도되었지만 질병은 GVHD 없이 계속되었다. 환자의 백혈병 블라스트(leukemic blast)에서 WT1의 발현이 면역조직화학 염색에 의해 확인되어 동종이형 WT1에 특이적인 T 세포로의 면역요법이 계획되었다.
<실시예 2-1: 실험 설계>
말초 혈액은 표준 백혈구성분채집 프로토콜에 의해 수여자로부터 수득하였고, ΔWT1 특이 T 세포는 후술되는 방법에 따라 생산하였고 입양 전이(adoptive transfer)를 위해 동결보존하였다. 구제 화학요법과 조혈모세포이식이 추가된 거대 용량의 DLI를 수행하여 면역요법에 적합한 조건을 유도하였다. 호중구감소증(neutropenia)으로부터 회복된 시점에 ΔWT1 특이 T 세포를 일주일 간격으로 총 4회 2×107 세포/m2/회의 용량으로 주입하였다. 18million Unit 용량의 재조합 IL- 2를 2주마다 주별 3회 투여하였다. (Exp Hematol) GVHD를 주기적으로 모니터링하였고, 수여자의 키메라증을 AmpF/STR PCR Amplification Kit (PE Applied Biosystems, Foster City, CA, USA) 및 ABI Prism 310 Genetic Analyzer (PE Applied Biosystems)로 측정하였다. 림프구 아집단의 분석 및 인터페론-γ를 분비하는 세포의 검출을 위해 환자로부터 말초 혈액을 연속적으로 회수하였다.
<실시예 2-2: 면역조직화학>
포르말린으로 고정되고 파라핀으로 포매된 조직 블록을 4㎛ 두께의 연속 절편으로 박절하고 실란처리된 유리 슬라이드에 표본제작하였다. 그 후 조직 절편을 탈파라핀 처리하고, 수화한 후 증류수로 씻어주었다. 다음 WT1에 대한 면역조직화학 측정법을 연속 파라핀 절편에 대해 자동화된 면역조직화학 염색기 (Lab Vision Autostainer LV-1; LabVision/Neomarkers, Fremont, CA)를 사용하여 수행하였다. 그 다음 항원 재생은 0.01M 시트레이트 완충용액(pH 6.0)을 사용하여 15분 동안 121℃의 조절된 최종 온도에서 microwave vacuum histoprocessor (RHS-1, Milestone, Bergamo)에서 시료를 가열함으로써 수행하였다. 내인성 퍼옥시다제 활성은 10분 동안 메탄올에 녹인 3% 과산화수소에서 시료를 정치함으로써 차단하였다. 일차 항체는 바탕 감소 물질(background-reducing components)이 보충된 Dako Antibody Diluent (Dako, Carpinteria, CA, USA)로 1:200으로 희석하여 이용하였다. 그 다음 일차 항체를 실온에서 30분 동안 정치한 후 Envision plus System (Dako)을 사용하여 검출하였다. 면역반응은 5분 동안 DAB (diaminobenzidine; Dako)로 발색한 후 헤마톡실린으로 대조염색하였다.

DC와 ΔWT1 특이 T 세포의 생산에 이용될 수여자의 말초 혈액을 표준 백혈구성분채집 프로토콜을 사용하여 입수하였고, 단핵세포는 Ficoll-Hypaque (Amersham Pharmacia Biotech, Piscataway, NJ, USA) 밀도 구배 원심분리를 사용하여 분리하였다. 밀도 분리 후 CD14+ 단핵구를 MACS System (Miltenyi Biotec, Bergisch Gladbach, Germany)을 이용하여 제조업자의 설명문에 따라 분류하였다. 분리된 세포의 순도는 유세포분석에 의하면 95% 이상이었다. 또한, 환자의 PBMC 유래 CD4+ 및 CD8+ T 세포를 MACS System (Miltenyi Biotec)으로 입수하여 환자의 T 세포 특성을 측정하는데 이용되었다.
<실시예 3: 절단된 WT1을 코딩하는 재조합 아데노바이러스(Adv-ΔWT1)의 생산>
사람 WT1 발현 벡터는 ATCC (American type culture collections)로부터 입수된 pBluscriptR-human WT1을 기반으로 센스 프라이머 5'-GCGCTCTAGAATGGAGAAGGGTTACAGC-3'(서열번호: 1) 및 안티센스 프라이머 5'-ATCGTCTAGATCAAAGCGCCAGCTGGAG-3'(서열번호: 2)를 이용하여 pShuttle vector (Clontech, Palo Alto, CA, USA)로 서브클론되었다. 이어서, WT1 발현 카세트를 pAdeno-XTMviral vector (Clontech, Palo Alto, CA, USA)로 삽입하였다. pAdeno viral vector 작제물을 Pac I로 절단하고, Lipofectamine (Life Technologies Inc., Rockville, Maryland, USA)에 의해 HEK 293 세포로 형질감염시켰다. 재조합 아데노바이러스의 DNA를 패키징한 후 절단된 WT1 DNA를 포함하는 아데노바이러스를 증폭하고 CsCl 밀도 구배로 두 차례 밴딩함으로써 세포 용균물로부터 정제하였다. 바이러스 생산물을 탈염하고 10% 글리세롤을 함유한 PBS(phosphate-buffered saline, v/v)에 -80℃에서 보관하였다. 바이러스 스톡의 역가를 tissue culture infectious dose (TCID50) 방법을 사용하여 결정하였다.
<실시예 4: 생체외에서 ΔWT1 특이 사람 T 세포의 생산>
미성숙 DC는, CD14+ 단핵구로부터 10% FBS, 100 ng/mL GM-CSF (Endogen, Woburn, MA, USA) 및 50 ng/mL IL-4 (Genzyme, Cambridge, MA, USA)로 보충된 RPMI 1640 배지에서 6 내지 7일 동안 습한 37℃, 5% CO2 인큐베이터에서 배양함으로써 생산하였다. 미성숙 DC를 회수한 후 MOI 500의 Adv-ΔWT1로 3시간 동안 37℃에서 감염시켰다. 감염 후 DC는 24시간 동안 100 ng/mL TNF-α 및 100 ng/mL LPS로 처리하여 성숙시켰다.
그 다음 CD14 음성 분획의 PBLs (peripheral blood lymphocytes)를 상기 DC와 24구판에서 2.0×106 세포/구 및 1.0×105 세포/구의 최종 농도로 10% FBS(Gibco- BRL), 2 mM L-글루타민 및 1% 페니실린-스트렙토마이신 (Cambrex)으로 보충된 RPMI 1640에서 공동 배양하였다. 7일째에 세포를 회수하여 재 자극하였다. DC를 1.0×105 세포/구의 농도로 플레이트하고 그 후 T 세포를 1.0×106 세포/구의 농도로 첨가하였다. 20 U/mL IL-2 (Endogen)를 8일째부터 3일마다 첨가하였다. 3회 자극한 후 T 세포를 ΔWT1에 특이적인 면역반응에 대해 평가하였다.
<실시예 5: 크롬 방출 측정법>
크롬 방출 측정을 위해, 효과기 세포(effector cell)를 한차례 세척한 후 10% FBS를 함유한 RPMI에 재현탁하였다. unloaded LCL(Lymphoblastoid cell line), ΔWT1을 코딩하는 RNA로 형질감염된 LCL 및 K562를 100 μCi Na2[51Cr]O4/106 세포와 함께 1시간 동안 37℃에서 정치하고 4차례 세척하고 타겟 세포로 사용하였다. 타겟 세포를 T 세포와 함께 지정된 효과기 세포 대 타겟 세포(E/T) 비율로 3회 공동 배양하였다. 4시간 동안 정치한 후 상층액을 회수하고 [51Cr] 양을 측정하였다. 특이 삽입율은 실험에서의 방출양(cpm)으로부터 자발적 방출양(cpm)을 뺀 값을 최대 방출양(cpm)으로부터 자발적 방출양(cpm)을 뺀 값으로 나눈 후 100을 곱하여 계산하였다.
<실시예 6: ELISPOT 측정법>
인터페론-γ(IFN-γ)를 분비하는 세포의 검출을 위해, ELISPOT 측정법이 BD Elispot assay kit로 수행되었고, 과정은 제조업자의 설명문에 따라 진행되었다. HLA-A0201에 제한된 WT1 유래 펩타이드인 WT1 126(RMFPNAPYL, 서열번호: 3), 187(SLGEQQYSV, 서열번호: 4) 및 235(CMTWNQMNL, 서열번호: 5) 펩타이드를 AnyGen Co. Ltd. (Kwangju, South Korea)에 의뢰하여 합성하였다. T 세포는 un-pulsed, 10 ㎍/mL의 WT1 펩타이드로 pulsed, 20 ㎍/mL의 ΔWT1을 코딩하는 RNA로 10:1의 비율로 3 반복 형질감염하였다. IFN-γ를 분비하는 세포에 상응하는 스팟의 개수를 AID-ELISPOT-Reader (AID)를 사용하여 계수하였다.
<실시예 7: FACS 분석>
FACS 분석을 위해 세포를 회수하고 세척한 후 PBS에 재현탁하고 어두운 곳에서 30분 동안 얼음 위에서 다양한 플루오로크롬 콘주게이션된 항체와 정치하였다. 세척한 후 FACSCalibur apparatus (Becton-Dickinson, Franklin Lakes, NJ, USA) 및 BD CellQuest software를 이용하여 분석하였다. T 세포의 표현형을 결정하기 위해 이용된 mAbs는 FITC-labeled anti-CD4와 anti-CD8, PE-labeled anti-CD8과 anti-CCR7 및 PE-cy5 labeled anti-CD45RA이었다. 상기 항체는 Becton-Dickinson로부터 구입하였다.
결과
1. 시험관내에서 유도된 ΔWT1 특이 T 세포의 특성
ΔWT1에 특이적인 T 세포를 생산하기 위해, T 세포를 3주 동안 Adv-ΔWT1로 전이된 DC로 자극하였다. 도 2a에 나타낸 바와 같이 T 세포의 증식은 21일째에 3배 이상 증가되었다. 생산된 T 세포에서 CD4+ 및 CD8+ 세포의 비율은 각각 30.4% 및 65.7% 이었다. 분리된 CD8+ 세포에서 메모리 T 세포 아집단을 분석한 결과, 대부분의 CD8+ T 세포가 즉각적인 효과기 기능을 갖는 효과기 메모리 표현형 [CD45RAlowCCR7low]을 95% 보여주었다(도 2b).
수여자에서 유도된 T 세포의 면역반응은 크롬 방출 측정법과 IFN-γ ELISPOT으로 측정되었다. ΔWT1에 특이적인 T 세포는 ΔWT1을 코딩하는 RNA로 형질감염된 자가 LCL(LCL/WT1 RNA)에 대해 특이적인 세포독성을 보였다(도 2c). ΔWT1에 특이적인 IFN-γ를 분비하는 T 세포의 상당한 비율이 도 2d에 나타낸 바와 같이 CD4+ 및 CD8+ 세포 모두에서 검출되었다. 본 발명에서 이용된 ΔWT1에 대한 특이성을 확인하기 위해, CD8+ T 세포를ΔWT1 단백질 내에 존재하는 에피토프 펩타이드(WT1-187, 235) 및 존재하지 않는 WT1 펩타이드(WT1-126)로 자극하고, IFN-γ ELISPOT 측정법을 수행하였다. 도 2e에 나타낸 바와 같이 WT1-187 및 WT-235에 특이적인 IFN-γ를 분비하는 T 세포는 WT1-126에 비하여 더 높은 비율로 검출되었다. 이러한 결과는 Adv-ΔWT1로 전이된 DC로 자극된 T 세포는 ΔWT1에 특이적인 CD4+ 및 CD8+ T 세포를 모두 포함한다는 것을 의미한다.
2. T 세포 주입 및 임상학적 반응
플루다라빈 계통 화학요법과 후속된 DLI (1.6×108/kg의 CD3+ 세포) 및 조혈모세포이식 (4.0×106/kg의 CD34+ 세포) 후 3주에 BM에서 백혈병 블라스트(leukemic blast)가 사라짐이 관찰되었다. 먼저 ΔWT1 특이 T 세포가 2×107/m2의 용량으로 즉각적인 독성 없이 정맥내에 주입되었다. 그러나, 이 용량(two doses)의 IL-2는 황달과 비정상적인 간 효소에 의해 확인된 바와 같이 심각한 급성 간염을 유발하였다. 간 기능은 IL-2의 중지와 보조적 치료에 의해 정상화되었고, 2차 WT1 특이 T 세포의 주입이 1차 후 3주에 진행되었다. 3회 및 4회 ΔWT1 특이 T 세포가 6주와 8주에 각각 주입되었다. 수여자의 키메라증은 화학면역요법 전에는 17%이었고 T 세포 주입 완료 후 1주째에 97%이었다. T 세포 이식 후 56일째에 피부, 구강 및 눈에서 GVDH가 발생하였다. 스테로이드 치료가 시작되었고 그 후 81개월 동안 지속되었다. GVDH에 대한 스테로이드로의 면역억제 치료가 백혈병의 재방을 일으키지는 않아, ΔWT1 특이 T 세포 치료의 지속적인 효과를 입증하였다. 뒤이어 합병증으로 폐 아스퍼질로시스(pulmonary aspergillosis) 및 대상 포진을 보였다. 2008년 6월 환자는 GVDH로부터 자유로워졌고 완전 완화가 2.7년 동안 유지되었다.
3. 면역 재구성에 따른 WT1 특이 T 세포 반응
말초 혈액에서 CD4+ T 세포의 절대적인 개수는 ΔWT1 특히 T 세포 주입 완료 후 9개월까지 정상 수치 이하에서 유지된 반면, CD8+ 세포는 주입 중 및 후에 점 진적으로 상승하였다(도 3a). CD4:CD8 비율은 ΔWT1 T 세포 주입 완료 후 13개월에 정상화되었다. 말초 혈액에서 ΔWT1에 특이적으로 IFN-γ를 분비하는 T 세포는 주입 전에는 관찰되지 않았으나, 초기 주입 후 3주째에 처음 관찰되었고 그의 비율은 연속적인 ΔWT 특이 T 세포의 주입 동안에 점진적으로 증가되었다. 이러한 패턴은 9개월까지 지속되었고 ΔWT1에 대한 특이적인 T 세포 반응은 최종 실험까지 2년 동안 지속되어(도 3b) 질병의 완전 완화를 유지하는데 ΔWT1 특이적인 면역이 기여함을 입증하였다.
4. WT1 특이 T 세포가 주입된 환자에서 2년 후 T 세포의 특성 동정
2년째에 WT1에 특이적인 T 세포가 입양 전달 전에 주입된 폴리클로날 림프구 또는 조혈줄기세포, 또는 주입된 ΔWT1 특이 T 세포로부터 유래된 것인지 확인하기 위하여 WT1 특이 T 세포의 특성을 분석하였다. unpulsed DCs (DC/Mock) 또는 ΔWT1 RNA로 형질감염된 DC (DC/ΔWT1 RNA)가 IFN-γ ELISPOT 측정법에서 항원 제시 세포로서 이용되었다. IFN-γ 분비 T 세포는 PBMC로부터 CD4+ 및 CD8+ T 세포 모두에서 검출되었다(도 4a). WT1-187 및 WT1-235 특이적인 IFN-γ를 분비하는 CD8+ T 세포는 WT1-126에 비하여 높은 비율로 나타났다. 이러한 결과는 생산된 T 세포의 특이성(도 2D 및 E)과 유사하여, 향상된 WT1 특이적인 T 세포는 주입된 T 세포로부터 유래되고 2년 동안 확장 및 유지되었다는 것을 입증한다.

도 2a는 3주 동안 Adv-ΔWT1로 전이된 수지상세포로 자극받은 T 세포의 증식 정도를 나타낸 것이다.
도 2b는 본 발명에 따른 ΔWT1 특이 T 세포에서 CD4+, CD8+ 및 메모리 T 세포의 비율을 유세포분석으로 확인한 결과이다.
도 2c는 ΔWT1 RNA로 형질감염된 LCL, unpulsed LCL 및 K562에 대한 본 발명에 따른 ΔWT1 특이 T 세포의 세포독성을 크롬 방출 측정법으로 확인한 결과이다.
도 2d는 unpulsed LCL 및 ΔWT1 RNA로 형질감염된 LCL에 대하여 IFN-γ를 분비하는 ΔWT1 특이 T 세포의 비율을 ELISPOT 측정법으로 확인한 결과이다.
도 2e는 WT1의 3개 에피토프에 대하여 IFN-γ를 분비하는 ΔWT1 특이 T 세포의 비율을 ELISPOT 측정법으로 확인한 결과이다.
도 3a는 본 발명의 ΔWT1 특이 T 세포를 주입받은 수여자에서 CD4T 세포와 CD8 세포의 비율을 나타낸 것이다.
