Description
연방정부 지원 연구개발에 대한 진술
본 발명은 미국국립보건원(National Institutes of Health)의 국립암연구소(National Cancer Institute)로부터 Grant No. CA 34200으로 제공받은 기금을 이용하여 이루어졌으며, 미국 정부는 본 발명에 대하여 일부 권리를 가진다.

암 치료에 주목할 만한 발전들이 이루어졌다고 하지만, 암은 여전히 주요한 건강문제로 남아있다. 암은 미국인들 네 명 중 한 명 정도의 사망 원인으로 보고되어 왔다.

존홉킨스 대학 암연구 센터(Johns Hopkins University Oncology Center), 위스콘신 대학 광범위 암 센터(University of Wisconsin Comprehensive Cancer Center) 및 에딘버그의 웨스턴 일반병원(Western General Hospital in Edinburgh)에서 펜클로메딘(penclomedine: PEN)이 1단계 임상시험(Phase I clinical trials)을 통해 평가되었다. 이러한 모든 임상 시험에서, 정맥 내 그리고 구강을 통한 투여의 양자 이후에 복용량-제한 신경독증이 관찰되었고, PEN의 피크 플라스마 레벨(peak plasma levels)과 관련이 있었다(O'Reilly 등; J. Clin Oncol. 12:1974, 전게서 참고).

몇몇 관련 배경기술을 O'Reilly 등, Clinical Cancer Research, 2 (March 1996), 541-548에서 발견할 수 있다. 이 참조문헌은 종양을 가지고 있는 쥐들의 조 직 및 종양에서 14C-펜클로메딘의 분포를 산정하는 연구에 대하여 기술하고 있다. 이 연구는 뇌에서의 주요 방사성 화학종이 펜클로메딘이라는 것을 발견했으며, 이로써 발견된 약제의 신경독증을 설명할 수 있다.
4-디메틸펜클로메딘(DM-PEN)은 환자와 설치류에서 주요 플라스마 대사물질(plasma metabolite)로 확인되었으며(Hartman 등, Clin Cancer Res. 2:953, 전게서 및 O'Reilly 등; Clin Cancer Res. 2:541; 전게서), 쥐에서의 PEN과 DM-PEN의 신경구조 연구들은 PEN-치료 그룹에서만 소뇌 손상이 나타남을 보여주었다(O'Reilly 등, 알킬화제 펜클로메딘은 쥐 소뇌에서 푸르키녜 세포(purkinje cells)의 변질을 야기시킨다; Invest New Drugs 21:269; 2003 참조).
3,5-디클로로-2-메톡시-4-히드록시-6-(트리클로로메틸)피리딘 또는 4-디메틸펜클로메딘이 암 치료를 위한 화합물로 제안된 바 있다. Hartman 등의 WO 97/46531를 참조하라. 또한 Waud 등, 4-디메틸펜클로메딘, 항종양-활성, 펜클로메딘의 잠재적으로 비신경독성 대사산물; Cancer Res, 57:815; 1997 또한 참조하라.

이미 이루어진 암 치료에서의 발전들에도 불구하고, 암 치료에 효과적이면서 동시에 해로운 부작용들이 감소된, 향상된 약제들이 등장할 여지가 남아있다.

최적 및 다양한 태양

적절한 알킬기의 예는 메틸, 에틸 및 프로필을 포함한다. 분기(branched) 알킬기의 예는 이소프로필과 t-부틸을 포함한다. 상기 아릴기는 특히, 페닐 C1 -3 알킬 및 벤질과 같은 페닐 및 알킬 치환 방향족 기(substituted aromatic group)이다.
약학적으로 허용 가능한 산 첨가 염들(acid addition salts)의 예는 예컨대, 염산, 브롬화수소산, 인산, 메타인산, 질산 및 황산과 같은 광산(mineral acids)과 예컨대, 타르타르산, 아세트산, 시트르산, 말산, 젖산, 푸마르산, 벤조익산, 글리콜산, 글루콘산, 숙신산 및 아릴설폰산, 예를 들어 p-톨루엔설폰산과 같은 유기산으로부터 유도된 것들을 포함한다.
본 명세서에 따르면, 개시된 화합물들은 포유류의 암, 특히 인간의 암을 치료하는 데 놀랍고 유익하게 효과적이다. 개시된 화합물들은 4-디메틸펜클로메딘 및 펜클로메딘에 비해 일반적으로 우수한 활성도를 보이는 것으로 나타났다. 더구나, 개시된 화합물들은 디메틸펜클로메딘과 펜클로메딘(PEN) 양자 모두에 비해 감소된 독성을 가지는 것으로 믿어진다.
개시된 화합물들의 합성
상기 개시된 화합물들을 제조하는 일반적인 절차는 다음과 같다: 15 ml의 건조 디클로로메탄(dichloromethane) 속의 4-디메틸펜클로메딘(DM-PEN)(1g)을 염기, 예컨대 0.5 ml의 트리에틸아민(triethylamine)으로 처리하고, 그리고 이어서 1 당량(equivalent)의 클로로티올로카보네이트(chlorothiolocarbonate), 클로로티오노 카보네이트(chlorothionocarbonate), 클로로디티오카보네이트(chlorodithiocarbonate) 또는 티오카바모일 클로라이드(thiocarbamoyl chloride)로 각각 처리하며, 이는 실온에서 5 ml의 건조 디클로로메탄 내에서 한 방울씩 첨가된다. 상기 용액을 실온에서 약 30분간 교반하고 흡수기(water aspirator)를 통해 증발시켜 건조시킨다. 잔여물을 5 ml 아세톤과 함께 분쇄하고 여과하여 트리에틸아민 염산염을 제거한다. 아세톤 여과액을 1 ml로 농축하고 형광 지시기를 포함하는 8 인치, 2 mm 실리카겔 판 위에서 분리시킨다. 주요 UV-가시영역대는 아세톤과 함께 용리되고 용매는 증발되어, 각 제품이 높은 수율로 얻어진다. 특성 평가는 예상되는 구조에 상당하는 적정 질량수 +1을 알려주는 질량분석법(mass spectrometry)과, 단일 UV-가시 성분을 산출하는 박막 크로마토그래피를 통해 이루어진다. 본 명세서의 예시 화합물들을 제조하는 반응 체계는 다음과 같다.
생체 내 항종양 평가
항종양 평가는 기존에 설명된 바(Plowman 등; 전게서 및 Harrison 등; 전게서 참조)와 같이 이루어진다. 무흉선(athymic) NCr-nu/nu 및 CD2F1 쥐를 확보하여 사육 시설 내의 무균, 필터-덮개 미세격리 우리에 넣어둔다. 쥐에 대해 복막 내 주사(i.p. injection)를 하기 위해, DM-PEN과 다양한 유도체들을 수성 히드록시프로필 셀룰로오스(hydroxypropyl cellulose) 내 현탁액으로 제조한다. 체내 통로로부터의 종양 조각들(30-40 mg)이 쥐의 유방 지방 패드로 삽입된다.
종양의 질량이 약 300 mg이 되면 다섯 마리 쥐들의 그룹 각각의 치료를 시작하여 모든 치료 그룹에 대해 5일간 계속한다. 매주 두 번씩 캘리퍼를 이용하여 각 종양을 2차원적으로 측정하여 종양 질량으로 변환한다. 매개체-처리 대조군(vehicle-treated control), 불완전 및 완전 종양 퇴행(tumor regressions), 및 무종양 생존체(tumor-free survivors)와 비교한 종양 성장 지연(delay)을 토대로 항종양 활성도를 산정하고, 대조군 종양들의 크기가 1 g에 이르면 실험을 종료하는데, 이는 일반적으로 57-61일이 소요된다. 예컨대, 두개 내 이식물(i.c. implants)을 위해, 0.03 ml의 MX-1 종양 브라이(brei)(106의 세포들을 포함)를 쥐의 뇌의 우반구에 이식한다.
두개 내 이식물의 치료는 종양 이식을 한 하루 후에 시작하여 5일 동안 계속한다. 쥐들은 매일 생존 여부를 관찰한다. 매개체-처리 대조군, 및 장시간 생존체와 비교한 수명 증가율(ILS)을 토대로 항종양 활성도를 산정한다.
결과
복막 내 주사로 유방 지방 패드에 이식된 MX-1 종양에 대하여 DM-PEN과 함께 각 유도체를 동시에 평가하였다. 최대 복용 가능량(maximum tolerated dose)을 포함하여, 복용당 135, 90 및 60 mg/kg의 단위 복용량 범위를 사용하였다. 모든 티올로카보네이트 유도체들이 DM-PEN 보다 우수한 활성도를 보여주었으며, 다섯 중 하나 또는 둘의 무종양 생존체를 만들어냈다. 결과는 표 1에 나타나 있다.
그러나, 티오노카보네이트와 디티오카보네이트 유도체는 이 종양 모델에서 낮은 활성도만을 보여주었다(데이터 미기재).
DM-PEN의 메틸 티올로카보네이트 유도체, DM-SMTC-PEN를 두개 내 이식된 U251 인간 아교모세포종(glioblastoma) 이종이식(xenograft)에 대하여 평가하여, 이러한 종양에 대해 아실 유도체들에 필적할 정도로 활성인 것을 관찰하였고(Struck 등, 전게서), DM-SMTC-PEN 역시 두개 내 이식된 D54 인간 다형성 아교모세포종(glioblastoma multiforme), 고저항성 두뇌 종양에 대하여 평가하여, 18%의 수명 증가가 나타났는데, 이는 적당한 반응이지만, 미국에서 주요한 두뇌 종양인 악성 신경아교종(gliomas)의 임상 치료를 위해 선택된 현 약제인 BCNU에 비해 크게 다르지는 않은 반응이다.
티오카바메이트의 항종양 활성도는 표 2에 나타나 있다. MX-1 인간 유방 종양 이종이식에 대하여, DM-PEN에서 관찰된 것보다 더 큰 강력한 항종양 활성도가 디메틸 유도체(DM-DMTC-PEN)와 디에틸 유도체(DM-DETC-PEN)에서 관찰되었고, 디메틸 유도체의 경우가 활성이 다소 더 컸다.
일련의 실험에서 DM-SMTC-PEN과 활성도를 비교하기 위한 두개 내 이식된 U251 인간 두뇌 종양 이종이식에 대한 DM-DMTC-PEN의 평가는 수명 증가율(ILS) 44%의 활성도를 보여주었는데, 이는 56%의 ILS를 나타내는 DM-SMTC-PEN보다 약간 떨어지지만, DM-SMTC-PEN의 에틸 및 페닐 유사체(analog)의 그것과는 동일하다.
유도체들의 펜클로메딘(PEN) 시리즈들의 주요 관심사는 일어날 수 있는 신경독증이다. PEN은 복용량을 제한하는 신경독증 때문에 유방암 치료를 위한 잠재력 있는 약제로서의 임상개발에서 제외되었다. 따라서, 신경독증 행동 테스트(behavioral test)에서 DM-SMTC-PEN을 PEN과 동시에 평가하였고, PEN 기와 달리 DM-SMTC-PEN 기에서는 떨림(tremors)의 생성이 나타나지 않아 비-신경독증으로 관찰되었다.

비경구 제형들은 일반적으로 약 0.5 중량% 내지 약 25 중량%의 활성 성분을 용액 내에 포함한다. 적절한 보존제 및 완충제가 그러한 제형들에 사용될 수 있다. 주사 현장에서의 짜증을 최소화하거나 제거하기 위해서, 그러한 조성물들은 약 12 내지 약 17의 친수-친유성 밸런스(hydrophile-lipophile balance: HLB)를 갖는 하나 이상의 비이온성 세제들을 포함할 수 있다. 그러한 제형들 내의 계면활성제의 양의 범위는 약 5 중량% 내지 약 15 중량%이다. 적절한 계면활성제들은 폴리에틸렌 솔비탄 지방산 에스테르(polyethylene sorbitan fatty acid esters), 예컨대 산화프로필렌의 프로필렌 글리콜과의 축합(condensation)에 의해 형성되는, 소수성 염기를 가지는 산화에틸렌의 고분자량 부가물과 솔비탄 모노올레이트(sorbitan monooleate)를 포함한다.

제형들은 1회 내지 수회 복용량 단위의 밀봉 용기, 예컨대 앰풀(ampules) 및 비알(vials)로 포장되어 제공될 수 있고, 사용 직전에 주사를 위해 살균 액체 부형제, 예컨대 물의 첨가만을 필요로 하는 동결건조(lyophilize) 조건으로 저장될 수 있다. 즉석 주사 용액들과 현탁액들은 살균된 분말, 과립, 및 정제로부터 준비될 수 있다. 주사 가능한 조성물들을 위한 효과적인 약학적 담체들의 필요 조건들은 해당 기술분야의 숙련된 당업자에게 공지되어 있다. 국부 투여에 적절한 제형들은 향미제 내의 활성 성분들, 일반적으로 수크로오즈 및 아카시아 또는 트래거캔스를 포함하는 로젠지; 불활성 염기 내의 활성 성분, 예컨대 젤라틴 및 글리세린, 또는 수크로오즈 및 아카시아를 포함하는 향정; 및 적절한 액체 담체 내의 활성 성분을 포함하는 구강세정제; 그리고 활성 성분에 추가로, 해당 기술분야에서 공지된 그러한 담체들을 포함하는 크림, 에멀젼, 및 겔을 포함한다.

개시된 화합물들과 조성물들은 백혈병(leukemias) 및 림프종(lymphomas), 예컨대 급성 림프구 백혈병(lymphocytic leukemia), 급성 비림프구 백혈병(nonlymphocytic leukemias), 만성 림프구 백혈병, 만성 골수 백혈병(myelogenous leukemia), 호지킨 병(Hodgkin's Disease), 비호지킨 림프종(non-Hodgkin's lymphomas), 및 다발 골수종(multiple myeloma), 소아 고형 종양(childhood solid tumors), 예컨대 뇌종양(brain tumors), 신경모세포종(neuroblastoma), 망막모세포종(retinoblastoma), 윌름즈종양(Wilms Tumor), 골종양(bone tumors), 및 연부조직육종(soft-tissue sarcomas), 성인들의 온고형종양(common solid tumors), 예컨대 폐암(lung cancer), 결장(colon) 및 직장암(rectum cancer), 유방암(breast cancer), 전립샘암(prostate cancer), 요암(urinary cancers), 자궁암(uterine cancers), 구강암(oral cancers), 췌장암(pancreatic cancer), 흑색종(melanoma) 및 기타 피부암(skin cancers), 위암(stomach cancer), 난소암(ovarian cancer), 뇌종양(brain tumors), 간암(liver cancer), 후두암(laryngeal cancer), 갑상샘암(thyroid cancer), 식도암(esophageal cancer), 및 고환암(testicular cancer)을 포함하는 수많은 암을 치료하기 위해 투여될 수 있다.
본 명세서에 개시된 방법은 특히 뇌, 결장, 신장, 및 유방 종양, 바람직하게는 결장, 뇌 및 유방 종양의 치료에 특히 효력이 있다. 당해 방법은 포유류, 특히 인간에게 실시될 수 있다.
본 발명의 문맥에서 동물, 특히 인간에게 투여되는 단위 복용량은 적당한 시간에 걸쳐서 동물에 치료 반응을 가져오기에 충분해야 한다. 해당 기술분야의 숙련된 당업자는 단위 투약량이 동물의 상태, 동물의 체중, 그리고 암의 심각성과 단계를 포함하는 다양한 요인들에 의해 결정된다는 것을 이해할 것이다.

알킬화 화학요법제의 예들은 카무스틴(carmustine), 클로람부실(chlorambucil), 시스플라틴(cisplatin), 로무스틴(lomustine), 시클로포스파미드(cyclophosphamide), 멜팔란(melphalan), 메클로레타민(mechlorethamine), 프로카바진(procarbazine), 티오테파(thiotepa), 우라실 머스타드(uracil mustard), 트리에틸렌멜라민(triethylenemelamine), 부설판(busulfan), 피포브로만(pipobroman), 스트렙토조신(streptozocin), 이포스파미드(ifosfamide), 다카바진(dacarbazine), 카보플라틴(carboplatin), 및 헥사메틸멜라민(hexamethylmelamine)을 포함한다.
대사길항물질인 화학요법제의 예들은 시토신 아라비노시드(cytosine arabinoside), 플루오로우라실(fluorouracil), 젬시타빈(gemcitabine), 수산화요소(hydroxyurea), 메르캅토퓨린(mercaptopurine), 메토트렉세이트(methotrexate), 티오구아닌(thioguanine), 플록수리딘(floxuridine), 플루다라빈(fludarabine), 클라드리빈(cladribine) 및 L-아스파라기나제(L-asparaginase)를 포함한다.
천연산물인 화학요법제의 예들은 악티노마이신 D(actinomycin D), 블레오마이신(bleomycin), 캠토테신(camptothecins), 다우노마이신(daunomycin), 독소루비신(doxorubicin), 에토포시드(etoposide), 미토마이신 C(mitomycin C), 탁솔(파클리탁셀), 탁소티어(taxotere), 테니포시드(teniposide), 빈크리스틴(vincristine), 비노렐빈(vinorelbine), 이다루비신(idarubicin), MITHRACINTM(플리카마이신), 및 디옥시코포마시인(deoxycoformycin)을 포함한다.
호르몬 화학요법제의 예는 타목시펜(tamoxifen)을 포함한다. 전술한 잡화학요법제의 예는 미토탄(mitotane), 미톡산트론(mitoxantrone), 빈블라스틴(vinblastine), 및 레바미졸(levamisole)을 포함한다.
