본 출원은 미국 가출원 제60/400,562호(출원일: 2002년 8월 2일)를 우선권으로 주장하여 출원한 것이다.
본 발명은 환자의 수술 공간을 조명하기 위한 시스템 및 기술에 관련한다.
환자의 수술 공간은 수술 공간 내에서 의사가 적절하게 수술 절차를 수행할 수 있도록 조명(照明)하는 것이 필요할 수 있다. 최소한으로 침습된 절차에서, 광(光)이 수술 부위에 배치될 수 있는 제2 입구가 제공될 수 있다. 의사가 그 공간에 접근하기 위해 사용되는 입구와 동일한 입구를 통해 광이 들어갈 수도 있다. 또한, 광은 입구 위로 위치할 수도 있다.
그러한 광의 사용은 몇 가지 단점을 갖는다. 예를 들면, 제2 입구의 사용은 수술 절차의 침습성을 증가시킨다. 통상의 접근 입구를 통한 광의 배치는 작업 채널을 따라 공간을 채우므로 의사가 수술 공간 작업 채널 내에서 장치를 다루기 어렵도록 하거나, 또는 입구의 크기를 증가시킬 필요를 발생시켜서 수술 절차의 침습성을 증가시킨다. 또한, 광은 절차 중의 입구를 확보할 수도 있는데, 그것은 입구를 통한 수술 공간에 대한 접근 또는 관찰을 차단할 수 있다.

발명의 요약

본 발명의 개념에 대한 이해를 증진시키기 위해, 도면으로 도시된 구현예가 참조로 제공되고, 이를 설명하기 위해 구체적인 용어들이 사용될 것이다. 그럼에도 불구하고 도시된 장치 및 설명된 방법 내에서 변형 및 추가적인 개조, 및 도시된 본 발명의 개념으로부터 당해 기술 분야의 통상의 당업자가 보통 예견할 수 있는 추가적인 응용 내로 본 발명의 범위를 제한하고자 하는 것은 아님이 이해되어야 할 것이다.
본 발명은 추궁판절개술(laminotomy), 척추후궁절제술(laminectomy), 척수감압술(foramenotomy), 관절면절제술(facetectomy), 척추원반절제술(discectomy), 체간 임플란트의 배치, 체내 임플란트의 배치, 뼈 절단 및 제거, 조직 절단 및 제거, 및 신경근 및 조직 리트랙션(retraction)과 같은 척수 적용에서 최소한으로 침습한 수술을 포함하는 수술 장치 및 방법을 제공하는 것이다. 다수 입구의 사용이 제외되는 것은 아니지만, 수술 절차를 수행하기 위해 환자 내에서 단일 입구가 사용되는 것을 예측할 수 있다. 입구를 통해 의사가 접근하는 수술 공간을 조명하기 위한 시스템 및 방법이 제공된다.
도 1을 참고하면, 광원(12) 및 광원에 연결된 광 장치(50)를 포함하는 수술 시스템(10)이 도시되어 있다. 광원(12)은 광 장치(50)를 통해 리트랙터(14)에 광을 제공한다. 광은 광원(12)으로부터 전달되어 리트랙터(14)로 들어가는데, 여기서 리트랙터(14)를 따라 존재하는 작업 채널 및/또는 리트랙터(14)의 말단에 인접한 작업 공간을 포함하는 수술 공간을 조명하기 위하여, 광 장치(50)로부터 방출된다. 본체(18)는 유리 또는 플라스틱과 같은 광 전달 물질, 또는 빛을 투과하지 않는 불투명 재료로 만들어질 수 있다.

리트랙터(14)의 본체(18)는 내벽 표면(30)과 외벽 표면(32)을 포함한다. 외벽 부재(64)는 내벽 표면(30)에 인접하여 위치하도록 광 요소(60)는 작업 채널(24) 내에 위치한다. 광전달 요소(66)의 원위 말단부는 본체(18)의 내벽 표면(30) 주위에 적어도 부분적으로 분산되고, 수술공간의 보다 우수한 조명과 방사된 빛의 다양한 방향을 제공한다. 하나의 구현예에서, 광전달 요소(66)는 리트랙터 본체(18)의 내측 둘레의 50% 이상 확장된다. 하나의 형태에서, 광전달 요소(66)는 내벽 표면(30)에 형성된 리트랙터 본체(18)의 내측 둘레를 따라 약 50 % 내지 75% 정도 공간을 점하고 있다. 다른 구현예에서는 리트랙터 본체(18)의 내측 둘레의 약 1% 내지 약 100%, 리트랙터 본체(18)의 내측 둘레의 약 25% 내지 약 75%, 그리고 리트랙터 본체(18)의 내측 둘레의 약 40% 내지 60% 확장된 광전달 요소(68)의 배열이 고려된다.

- 환자 체내의 공간에 접근하여 조명(illuminating)하기 위한 수술 시스템으로서,환자의 신체에 위치 가능하고, 길이를 따라 작업 채널을 한정하는 내벽 표면을 포함하는 리트랙터(reatractor) 및하나 이상의 벽 부재(wall member) 및 하나 이상의 벽 부재를 따라 하나 이상의 광 전달 요소를 포함하는 광 요소를 포함하며, 하나 이상의 벽 부재가 내벽 표면을 따라 위치 가능하고, 내벽 표면과 마찰 체결 가능하며, 마찰 체결이 리트랙터에 대한 광 요소의 위치를 유지하기에 충분한 수술 시스템.
- 제1항에 있어서, 하나 이상의 광 전달 요소가 하나 이상의 벽 부재를 따라 확장하여, 벽 부재 주위에 위치된 복수 개의 광 전달 요소들을 포함하고 있는 수술 시스템.
