Abstract

技术领域
在病人身体的手术空间中,可要求为外科医生照明以便在该手术空间中正常地完成手术过程。在最小限度地进入过程中,可提供第二个入口,以便光可通过这第二入口定位在该手术空间。可把由外科医生使用的光通过同一入口引入以进入该空间。还有,可把光固定在入口的上面。
采用这种光具有几个缺点。例如,第二入口的采用增加了过程的进入度。通过共用的入口设置光装置,沿着工作通道占有了空间,对外科医生运用装置造成更多的困难,或需要增加入口的尺寸,从而增加了过程的进入度。此外,可在过程期间把该光固定在该入口,这样,通过入口可阻挠进入到或目测该手术空间。
要在最小限度的进入外科过程中提供手术空间的照明,同时又避免在上面提及的缺点中的任何一个或组合是合乎需要的。本发明的目的在于符合这种需要,这只是其中之一。
发明内容
根据一个方面,一种系统包括收缩器(refractor)和可设置在该收缩器工作管道中的光装置,其中该光装置通过与收缩器的内壁表面啮合被支承在工作管道中。
根据另一方面,一种系统包括连接到光装置的收缩器。该光装置是易形变的,以呈现用于设置在收缩器工作管道中与其内壁表面相接触的许多结构中的任何一种结构的形状。
根据另一方面,一种系统包括连接到光装置的收缩器。该光装置与收缩器工作管道的内壁表面是可啮合的,并可沿在那里相接触的内壁表面相对于收缩器移动以重新设置该光装置。
根据另一方面,一种系统包括收缩器和可在工作管道中啮合到收缩器的光装置。该光装置包括半圆体和在其顶部附近被隔开的多个光传输元件。
根据一个方面,一种系统包括收缩器和可在带有多个光传输元件的收缩器工作管道中设置的光装置,这些光传输元件在邻近内壁表面的收缩器工作管道附近被隔开。

为了促进对本发明原理的理解的目的,现在将参阅示于附图中的实施例并用专业术语来描述它们。尽管如此,应知道,不想因此而限制本发明的范围,在所图示的装置和所描述的方法中,这种变化和进一步修改,以及本发明原理的这种进一步应用,如在这里图示的,会正常地被与本发明有关领域中的技术人员想到。
本发明提供用于完成外科手术的装置和方法,这些外科手术包括最小限度地进入手术,比如讲,椎板切开术,椎板切除术,椎间孔切开术,椎骨关节面切除术,椎间盘切除术,体间移植定位,体内移植定位,骨骼切除和移去,细胞组织切除和移去,以及神经根和细胞组织的收缩的脊椎骨应用。虽然不排除多入口的使用,但是还考虑在病人内单个入口的使用来完成手术过程。提供了用于由外科医生经过这入口提供光进入的手术空间的系统和方法。
参考图1,示出一种手术系统10,它包括光源12和耦合到那里的光装置50。光源12通过光装置50把光提供到收缩器14。来自光源12的光被传输到收缩器14中,来自光装置50的光在这里发出照明手术空间,包括沿收缩器14的工作管道和/或在收缩器14顶端附近的工作空间。设想主体18可由诸如玻璃或塑料可传输光的材料,或不传输光的不透明材料制成。
收缩器14包括在最接近的端部20和顶端22之间延伸的主体18。主体18可限定在最接近的端部20开口处和顶端22之间穿通延伸的工作管道24。最接近的耦合部分34可包括沿着绕收缩器14延伸的圆环部分38。可把工作管道24作成可接受一个或几个手术装置的大小,穿过那里以在邻近收缩器14顶端22的工作空间26处来完成手术过程。
可把收缩器14插入一个或多个细胞组织扩张器(dilator)中的最后一个和/或随后一个围着一个设置的导向金属丝之上,以逐渐收缩病人的细胞组织120和皮肤122。随着收缩器14经过皮肤和细胞组织定位,扩张器被移动以提供通过工作管道24进入工作空间26。在这图示的实施例中,工作空间26是在脊椎骨的柱状段100的附近。工作空间26可包括,例如,平行细胞组织,椎骨102,104,106的一个或多个的骨细胞组织;在椎骨102,104之间的圆盘空间108的环状细胞组织和/或在椎骨104,106之间的圆盘空间110;和/或由操作细胞组织建立的定位。
如在图2中进一步示出的光装置50包括带有耦合组件53的可耦合到光源12的连杆52。在连杆52的对面端部处是可设置在收缩器14的工作管道24中的照明元件60。耦合组件53可包括耦合到连杆52端部的端部构件54,并通过它延伸出一个或多个光传输元件。可把配件56啮合到端部构件54,和固定在配件56上的光源连接器58,并紧扣到端部构件54。把光源连接器58啮合到光源12,以致可把来自光源12的光导入由连杆52容纳的光传输元件中。还考虑到耦合组件53可包括适于把连杆52紧扣到光源12的任何其它结构,使得光能从光源12通过连杆52传输。
图3提供照明元件60的顶端视图,而图4提供通过收缩器14的横截面,并在设置于其中的光装置50的照明元件60处最接近地观看。照明元件60包括具有内壁构件62和外壁构件64的主体。通道68形成在内壁构件62和外壁构件64之间。可把内壁构件62和外壁构件64在它们的横向边缘74,76处相互连接起来,而在通道68之中包括多个光传输元件66。
光传输元件66从照明元件60的最接近端70邻近的连杆52延伸,并如图1所示输出到通道68。以在邻近顶端72处沿通道68把传输元件66隔开的方式,相对于照明元件60来具体构筑光传输元件66。通道68沿顶端72中的至少一部分是开口的,以把光传输元件66的顶部暴露到手术空间。光传输元件66从它们的顶部发出光束照亮这手术空间。由于光传输元件66沿收缩器14的内壁表面30分布的,所以从光传输元件66发出的光对工作空间26提供有效的照明,并把在工作管道24中由手术装置阻止即荫蔽的光减到最小或消除掉。
收缩器14的主体18包括内壁表面30和外壁表面32。照明元件60被设置在工作管道24中,使得外壁构件64被设置在内壁表面30的邻近。可把光传输元件66的顶端至少部分地围绕主体18的内壁表面30分散以给手术空间提供较多的照明并改变发射光的方向。在一实施例中,光传输元件66在收缩器主体18的约50%或更多的内周长上延伸。在一型式中,光传输元件66在沿由内壁表面30形成的收缩器18的约50%到约75%内周长上被隔开配置。所考虑的其它用于光传输元件66的配置有从收缩器主体18的约1%到100%内周长延伸,从收缩器主体18的约25%到约75%内周长延伸,和从收缩器主体18的约40%到约60%内周长延伸。
照明元件60通过如箭头78所指出的来移动横向边缘74,76是易变形的,即操作到各种结构。用这方法,如果需要可改变照明元件60以与主体18的内壁表面30相一致。例如,照明元件60可在尺寸上减小,并通过把横向边缘74,76向着彼此移动来设置在工作管道中。一旦插入工作管道24中,如果必要,可把边缘74,76回到它们的原有结构,随即造成外壁构件64以摩擦啮合与内壁表面30相接触。
照明元件60可被轴向地移动,如图1中箭头80所指,来调节在工作管道24中照明元件60的定位,同时保持与收缩器14内壁表面30的摩擦啮合。照明元件60还可绕内壁表面30作如箭头82所指的转动,来调节绕工作管道24光传输元件68的定位,同时保持与内壁表面30的摩擦啮合。通过提供在调节期间保持与收缩器14啮合的易于可调的照明元件,外科医生可在需要时,在不从工作管道24移动照明元件60,或在没有使照明元件落进手术空间危险的情况下重新设置发光的位置和方向。其它实施例考虑了照明元件60可固定地设置在工作管道24中。
照明元件60可具有与工作管道24的形状相应的形状,因此工作管道基本上保持不被照明元件60阻挡的状态。手术装置,植入管及诸如此类之物可通过由内壁构件62所限定的通道被设置在适当的位置,同时照明元件60仍在工作管道24中保持与主体18啮合以照亮这手术空间。此外,径向分布的光传输元件66提供许多可发出光的方向和位置,在手术过程期间,减少全部或基本上阻挡发射光的机会。
在一特殊的实例中,照明元件60是由沿光传输元件66的对面两侧设置的一对不锈钢板组成,这传输元件是用塑料光纤的型式来提供的。在另一实施例中,壁构件62,64由不透明材料构成,以防止经过那里的光通过,故它通过光传输元件66的顶端被聚焦。其它实施例考虑了由其它诸如,好比说,塑料材料或其它金属和金属合金来制作照明元件60。还考虑过照明元件60的形状可以是圆柱形的,并适于与收缩器的内壁表面摩擦地啮合。也考虑过可经过似钩形的构件,成套螺钉,或其它耦合装置把照明元件60耦合到收缩器的最接近端。
光源12可以是能产生光和/或把光传输到连杆52的任何装置。连杆52可以是光纤缆的任何一种或组合,包括塑料光纤缆,金属丝或其它传输装置或能在光源12和收缩器14之间传输光的装置。可把光纤捆扎到保护外套中。
收缩器14作为具有圆形横截面的圆柱收缩器被示。但是考虑过其它收缩器实施例,包括具有非圆形横截面的收缩器,且工作管道没有全部主体18密封。在2001年3月23日申请的第09/815,693号美国专利申请系列(代理案宗号4002-2642);在2002年4月25日申请的第10/117,440号美国专利申请系列(代理案宗号4002-2896);和在2002年6月26日申请的第10/180,658号美国专利申请系列(代理案宗号4002-3006)中提供其它收缩器的示例,各案宗通过引用全文结合在此。
尽管已经在上面的描述和附图中详细地图示说明和描述了本发明,但是,应把这相同的内容看作对特性的图示说明而不是限制,应理解只示出和描述了较佳的实施例,并希望保护包括在本发明的精神范围内的所有变化和修改。
Claims (15)
1.一种用于进入和照亮在病人体内的空间的手术系统,包括:
收缩器,可设置在病人的体内,并包括沿着那里限定工作管道的内壁表面;以及
照明元件,包括内壁构件和外壁构件,和沿着所述内壁构件和外壁构件的至少一个光传输元件,所述内壁构件和外壁构件可沿着所述内壁表面设置在合适的地方,并且所述外壁构件与所述内壁表面摩擦地啮合,所述摩擦啮合足以维持所述照明元件相对于所述收缩器的位置。
2.根据权利要求1所述系统,其特征在于,其中所述至少一个光传输元件包括沿着所述内壁构件和外壁构件延伸,并在所述内壁构件和外壁构件附近被隔开的多个光传输元件。
3.根据权利要求2所述系统,其特征在于,其中所述多个光传输元件被设置在所述内壁构件和外壁构件之间的通道中。
4.根据权利要求2所述系统,其特征在于,其中所述多个光传输元件由光纤组成。
5.根据权利要求1所述系统,其特征在于,其中所述内壁构件和外壁构件是可弯曲的以与所述内壁表面相一致。
6.根据权利要求1所述系统,其特征在于,其中所述收缩器的所述内壁表面基本上密封所述工作管道,并且所述照明元件的所述内壁构件和外壁构件至少在所述内壁表面的约50%上延伸。
7.根据权利要求1所述系统,其特征在于,其中所述照明元件沿着所述内壁表面在轴向上是可移动的,而所述外壁构件在那里仍维持摩擦啮合。
8.根据权利要求1所述系统,其特征在于,其中所述照明元件沿着所述内壁表面可环绕地移动,而所述外壁构件在那里仍维持摩擦啮合。
9.根据权利要求1所述系统,其特征在于,其中所述外壁构件包括可沿所述收缩器的所述内壁表面设置的呈凸出形曲面的表面,所述内壁构件包括在所述呈凸出形曲面的壁表面对面的呈凹入曲面的壁表面。
10.根据权利要求9所述系统,其特征在于,其中所述内壁构件和外壁构件在相对面的横向边缘之间延伸。
11.根据权利要求10所述系统,其特征在于,其中所述内壁构件和外壁构件沿所述相对面的横向边缘相互接合。
12.根据权利要求10所述系统,其特征在于,其中所述内壁构件和外壁构件在所述相对面的横向边缘之间沿超过所述内壁表面的50%延伸。
13.根据权利要求10所述系统,其特征在于,其中所述内壁构件和外是由不透明材料组成。
14.根据权利要求9所述系统,其特征在于,其中所述呈凹入曲面的壁表面形成与所述工作管道平行的通道。
15.根据权利要求1所述系统,其特征在于,其中所述收缩器是一根管子。
