Abstract
본 발명은 멤브레인 필터층이 기공크기가 0.04 내지 0.45㎛ 이고 두께가 0.1 내지 1mm 이며, 필터의 기하학적 표면적이 15 내지 300㎠ 이고 피로젠 분리용량이 적어도 500㎍ 임을 특징으로 하는, 뇌척수액의 여과에 사용하기 위한 필터에 관한 것이다.
Description
본 발명은 뇌척수액의 여과, 특히는 염증성 과정(신경염, 예를 들면 다발성 신경근염, 근신경염 또는 귈랑-바레 증후군(Guillain Barre syndrome; GBS)에 의해 수반되거나, 비염증성 과정(신경장애) 뿐 아니라 수액 중독(디프테리아 또는 광견병과 같은) 또는 박테리아(예를 들면 보렐리아증) 또는 바이러스(예를들면 바이러스성 뇌염, 소아마비)에 의한 감염에 의해 수반되고, 신경계에 원인적으로 또는 증상적으로 나타나는 신경계 질환의 치료를 위한 필터(filter)에 관한 것이다.
GBS는, 예를들면 각각 소뇌 신경을 포함하는 상행성 운동 및 신경-관련 마비 및 상행성 신경 마비로서, 처음에는 팔-다리로 확장하여 환자가 더이상 걸을 수 없고 누워만 지냄으로써 모든 근육이 완전 마비되어 환자의 장기에 걸친 흡인치료를 필요로 한다. 장기 결여에 의해 신경이 퇴화될 수도 있다.
상기 언급한 질환은 수액 조성을 변화시킨다. 이들 증상에 대한 원인은 아직까지 확실히 규명되지 않고 있으며, 특히 수액 조성의 변화와 질환 증후간의 관계가 알려져 있지 않다. 귈랑-바레 증후군의 치료는 치료학적 혈장 분리법, 즉 (부분) 혈장 교환으로, 환자로부터 혈액을 채취한 후 소체 성분 및 혈장을 예를들어 원심분리에 의해 기계적으로 분리하는 것을 포함한다. 소체 성분은 혈장을 완전히 또는 부분 교체시킨 후에, 대용 혈장중에 재현탁시켜 환자에게 재주입한다. 그러나, 혈장 분리법은 많은 환자에게서 질환 증상을 거의 개선시키지 못했거나 전혀 개선시키지 못했다.
본 발명에 이르러, 환자의 수액을 특정 필터를 통해 여과시키는 경우에, 환자의 상태가 상당히 개선되고, 증상들이 완전히 완하될 수 있음이 밝혀졌다.
따라서, 본 발명은 멤브레인 필터(membrane filter) 층의 기공폭이 0.04 내지 0.45㎛ 이고, 두께는 0.1 내지 1mm 이며, 필터의 기하학적 표면적은 50 내지 300㎠ 이고, 피로젠(Pyrogen) 분리용량이 적어도 500㎍ 임을 특징으로 하는, 뇌척수액을 여과하는 방법에 사용하기 위한 필터에 관한 것이다.
㎍으로 표시되는 피로젠 분리용량은 필터가 출발물질중에 0.6 내지 6.9ng/㎖ 농도 범위의 표준 대장균 내독소에 기초하여 상기 언급한 양의 피로젠을 보유함을 의미하며, 여과물의 농도는 LAL 시험(리뮬루스-아메보세포-용해물 시험)의 한계치(약 0.006ng/㎖)이하이다.
발열성 화합물(피로젠)(열-발생 화합물)은 주로 극히 소량(1㎍/kg 미만)으로 인체내에 투여되었을때 오한 및 체온상승(발열)을 일으키는 병원성 및 병원성 박테리아, 진균류 또는 바이러스의 투석 가능한 내열성 화합물이다. 화학 용어로 나타내자면, 발열성 화합물은 주로 올리고, 폴리- 및 리포폴리사카라이드 또는 폴리펩타이드이며, 여기에서 가장 효과적인 발열성 화합물은 그람 음성 박테리아의 화합물이다.
발열성 화합물은 소위 박테리아 필터수단에 의해 제거되는, 주입 또는 주사 용액에서 불순물로서 중요하다. 박테리아 필터란 박테리아가 체 또는 흡착 작용에 의해, 예를들어 유리, 멤브레인 또는 체 필터에 의해 기체 또는 액체로부터 제거될 수 있는 미소 다공성 물질로 정의된다(참조예 : Roche Lexikon Medizin, 1st Edition 1984, P. 135). 피로젠 분리용량은 표준 발열성 화합물, 즉 대장균 내독소를 사용하여 결정하고, 분리 능력은 토끼 시험 또는 LAL 시험에 의해 검사한다(참조예: Pharm. Ind. 47 (1981) 407-411). 환자를 보호하기 위한 주입 필터로서 사용되는 구입가능한 박테리아 필터는 피로젠 분리용량이 100㎍ 이하이고, 필터의 기하학적 표면적이 약 10㎠ 이다.
본 발명에 따르는 필터의 용도는 뇌척수액의 여과가 매우 간단한 방법으로 수행될 수 있는 방법이다. 실제로, 본 발명에 따르는 방법은 환자에게 두개의 삼로(three way) 코우크로 이루어진 수막강내 카테터, 10㎖ 시린지 및 수액 필터를 삽입한 후 밀폐 시스템을 장치하는 간단하고 편리한 방법이다. 그후, 각각 10 내지 40㎖의 수액을 손으로 채취하여 필터를 통과시킨 후 재주입한다. 이러한 방법으로, 4명의 급성 환자 및 2명의 만성 환자를 포함하는 6명의 GBS 환자를 치료한다. 3명의 환자에게는 상기 방법을 수행하기 전에 그들의 증상에 영향을 끼침이 없이 격심한 혈장분리를 실시한다. 5일동안 매일 치료하는 중에 1 내지 2일에는 각각 최대 150㎖의 수액을 여과한다. 이 방법은 모든 환자에 대해 합병증없이 내성이 있다. 두통이 잠시 발생할 뿐이며 보통 관찰되는 허리를 찌르는 것 만큼의 심한 정도는 아니다. 모든 환자에게서 수액 여과는 적절하였으며, 때로는 치료하는 동안에 임상적 증상이 명확하게 또는 상당한 정도로 개선된 것으로 관찰되었다. 급성 환자 및 GBS의 초기 단계에 치료된 환자가 만성 환자의 경우보다 더욱 확실하게 더 빨리 개선되었다.
수액의 여과는 또한 수액의 변화에 의해 수반되는 기타 다른 증상, 예를들면 다발성 경화증 또는 ALS에 적용될 수 있다.
본 발명 필터의 멤브레인 필터 층은 기공 크기가 0.04 내지 0.45㎛, 바람직하게는 0.1 내지 0.3㎛ 및 가장 바람직하게는 0.1 내지 0.2㎛ 이다. 이와같은 기공크기를 갖는 멤브레인 필터층은 그 자체가 공지되어 있으며 주사용액을 투여하기 위한 박테리아 필터에 사용되고, 0.1㎛ 이하의 기공크기를 갖는 멤브레인은 바이러스 필터로서 사용된다. 그러나, 일반적으로 사용되는 필터는 수액 여과용으로 사용되는 경우에 상당한 변화를 일으키지 않기 때문에 본 발명에는 적합하지 않다. 또한, 귈랑 바레 증후군의 원인이 불확실하기 때문에, 일반적으로 사용되는 필터로 질병의 증상을 상당히 개선시킬 수 있을 것으로 기대되지는 않는다.
본 발명의 필터는 피로젠 분리용량이 적어도 500㎍, 예를들면 적어도 600 또는 700㎍, 및 바람직하게는 적어도 1000㎍ 이고, 이중에서도 1000 내지 2000㎍의 범위가 특히 바람직하다.
멤브레인 필터 층의 층 두께는 0.1 내지 1mm 이고, 필터의 기하학적 표면적은 50 내지 300㎠ 이다.
멤브레인 필터층의 기공크기가 0.04 내지 0.45㎛ 이고, 기하학적 표면적이 50 내지 300㎠ 이며, 두께가 0.1 내지 1mm 인 경우에, 실제로 사용하기에 충분한 유량 및 적어도 500㎍의 필요한 피로젠 분리용량이 얻어질 수 있다. 일반적으로 유랑은 대략 기공폭 및 표면적에 비례하고 층 두께에는 반비례하며, 피로젠 분리용량은 층 두께 및 표면적에 비례한다.
피로젠 분리용량 및 유량을 조절하는 또다른 인자는 총 부피에 기초하여 기공 부피의 퍼센트로서 표시되는 기공부피이며, 분리용량 및 유량의 특정 한계치는 대략 기공 부피에 비례한다. 기공 부피는 필터를 가능한한 작게 유지하기 위하여 가능한한 커야 한다. 그러나, 기공부피는 상한치를 갖는다. 실제로 기공 부피는 일반적으로 50 내지 90% 이며, 상기 언급한 이유로 인해 값이 클수록 바람직하다. 멤브레인 필터 층에 대한 재료로서는 상기 언급한 기공 범위를 갖는 멤브레인이 공지된 방법에 따라 제조될 수 있는 모든 불활성 중합체 물질이 적합하다. 적합한 불활성 중합체 물질의 예로는 폴리올레핀, 예를들어 폴리에틸렌 또는 폴리프로필렌, 폴리아미드, 예를들어 나일론 6,6 또는 폴리카프로락탐, 폴리에스테르 또는 폴리비닐리덴 플루오라이드와 같이 체액에 불활성인 플라스틱이 있다.
바람직한 구체예에서 멤브레인 필터층은 분리용량이 용이하게 달성되기 때문에 전하-변형된 멤브레인이다. 양의 제타 포텐셜 및 음의 제타 포텐셜 모두를 갖는 전하-변형된 멤브레인이 시판되고 있다. 바람직한 전하-변형된 멤브레인은 전하 변형제를 멤브레인이 주조되는 도우프(dope) 용액에 첨가함으로써 제조된 멤브레인이다.
양의 제타 포텐셜을 갖는 전하-변형된 멤브레인은 또한 양이온 멤브레인으로 표시된다. 적합한 멤브레인은 예를들면 폴리아미드, 예를들어 나일론 6,6 으로 구성되며, 여기에서 멤브레인의 전하 변형은 사급 암모늄 그룹을 갖는 열-경화가능한 중합체를 중합체 도우프에 가함으로써 형성된다. 이들 멤브레인의 양전하는 사급 암모늄 그룹이 존재함으로써 형성되기 때문에, 이들은 산성, 중성 및 알카리 pH 범위내에서 그들의 양전하를 유지한다.
음의 제타 포텐셜을 갖는 전하-변형된 멤브레인은 또한 음이온성 멤브레인으로도 표시된다. 음전하 공급원은 카복실산 그룹, 설폰산 그룹, 페놀 아미노 그룹, 설프하이드릴-, 설파이드-, 티오카보닐-, 포스핀-, 포스포릴- 또는 티오포스포릴 그룹과 같은 이온화가능한 작용기의 극성 그룹이다.
또다른 바람직한 구체예에서 본 발명의 필터는 제2 멤브레인 필터 층의 기공 크기가 0.1 내지 5㎛ 이고 두께가 0.1 내지 1mm 인 조합 필터이다. 이 경우에 제1 멤브레인 필터층은 그의 기능이 각각 박테리아 및 바이러스를 분리하는 데에만 있기 때문에 매우 얇게 만들어질 수 있으며, 상기 언급한 제2층에서 대부분의 유해 성분이 흡착된다. 따라서, 본 구체예에서 분리용량의 필수 부분은 상기 언급한 제2층에 의해 제공된다. 제2 필터층의 가장 중요한 점은 기공크기 보다는 흡착 용량에 있으며, 따라서 예를들어 0.8㎛ 또는 1.2㎛ 또는 5㎛ 까지의 큰 기공크기가 적합하다. 그러나, 작은 기공 크기도 또한 제2층에 유리한 결과를 제공하기 때문에 0.1 내지 0.45㎛의 기공크기도 바람직하다.
제2 필터층의 두께는 0.1 내지 1mm 이다. 유량이 많아짐으로 인해 기공크기가 커짐으로써(그리고 기공부피가 늘어남으로써) 층 두께가 증대될 수 있으며, 이는 또 한편으로는 500㎍의 최소 피로젠 분리용량을 유지하면서 50 내지 300㎠ 범위의 필터의 기하학적 표면적을 더 작은 값으로 변경시킨다.
본 발명의 필터는 단층 또는 다중층이다. 단층 구체예의 경우에, 단일 멤브레인 필터층은 바람직하게는 전기한 바와같은 전하-변형된 멤브레인이다.
다중층 구체예의 경우에, 조합 필터는 적어도 두개의 층으로 구성되며, 이중 제2 필터층(상기 언급한 것)은 바람직하게는 전하-변형된 멤브레인이다. 이러한 조합 필터의 특정 구체예에서 제1 필터층은 예를들어 나일론 6,6 으로부터 전하-중성 멤브레인이며, 제2 필터층은 전하-변형된 멤브레인이고, 여기에서 전하는 양 또는 음(양 또는 음의 제타 포텐셜)일 수 있다. 두개의 층으로 구성된 필터의 또다른 특정 구체예에서 제1층은 양으로 전하-변형된 멤브레인이고 제2(상기 언급한 것) 층은 음으로 전하-변형된 멤브레인이다. 세개의 층으로 구성된 필터의 추가의 특정 구체예에서 제2층은 양으로 전하-변형된 멤브레인이고 제3층은 음으로 전하-변형된 멤브레인이다.
실제로 사용하는 경우에, 본 발명의 필터는 층의 수 및 종류에 관계없이, 필터에 그의 용도 특성을 변화시키지 않으면서 기계적인 강도를 부여하는 하나 또는 그 이상의 지지 또는 운반층을 갖는다. 또한, 실제로 사용하는 경우에 있어 필터는 바람직하게는 대응 접속부가 있는 필터 하우징(filter housing) 내에 배열되며, 상술한 지지층은 필터층에 합체될 수 있다.
또다른 바람직한 구체예에서 본 발명의 필터는 배기 수단이 장치된다. 가스 기포는 수액 필터의 작동 동안에 발생할 수 있으며, 이 가스 기포로 인해 필터가 차단될 수 있다. 배기는 무균상태를 유지하면서 가스 기포를 필터 하우징으로부터 배출시킨다. 적합한 배기수단은 예를 들면 통상 사용되는 배기 밸브이다. 바람직하게는 상기의 목적은 수액은 빠져나오지 못하게 하고 가스 기포만을 여과압을 통해 통과시킬 수 있는 기공크기를 갖는 소수성 멤브레인인 배기 멤브레인에 의해 이루어진다. 배기 멤브레인은 필터 하우징의 유입면, 즉 여과되는 수액이 필터 하우징을 통과하는 면상에 배열된다.
첨부된 도면은 본 발명을 설명하기 위한 것이고, 제1도 내지 4도는 하기한 바와같이 본 발명 수액 필터의 단층 및 다중층 구체예를 도시한 것이다.
제1도는 본 발명의 수액 필터에 대한 단층 멤브레인 필터층이다.
제2도는 이중층 구체예이다.
제3도 또한 이중층 구체예이다.
제4도는 삼중층 구체예이다.
각각의 층은 지지층 및 하우징이 표시되지 않은 채로 도시되었다.
제1도는 층 두께가 0.2mm 이고 기공 크기가 0.2㎛ 인 멤브레인 필터층(11)을 도시한다. 재료는 나일론-6,6 이며, 멤브레인은 사급 암모늄 그룹을 도입시켜 양의 제타 포텐셜을 이룸으로써 전하-변형되었다. 유사한 멤브레인이 예를들어 Pall Corporation에서 포시딘(Posidyne)상품명으로 시판되고 있다.
상기의 멤브레인 필터층은 필터의 기하학적 표면적이 160㎠ 이고, 피로젠 분리용량이 560㎍인 수액 필터를 만드는데 사용된다.
제2도는 이중층 멤브레인을 도시한다. 제1층(21)은 나일론-6,6 으로 구성되며, 두께가 0.1mm, 기공크기가 0.04㎛ 이다. 제1층(21) 위에 층 두께가 0.2mm 이고, 기공크기가 0.2㎛ 인 제1도에 도시된 포시딘 재료의 제2층(22)이 위치한다.
기하학적 표면적이 160㎠ 인 필터는 피로젠 분리용량이 560㎍ 이다. 제1층(2)의 기공크기가 0.04㎛ 이기 때문에, 조합 필터는 바이러스 보유력이 있다.
제3도에서 제1 멤브레인 필터층(31)은 두께가 0.2mm 이고 기공 크기가 0.2㎛ 인 포시딘으로 구성된다. 제2층(32)은 음전하- 변형된 멤브레인, 즉 음의 제타 포텐셜을 갖는 멤브레인이다. 이러한 멤브레인은 예를들어 Pall Corporation에서 카복시딘(Carboxydyne)상품명으로 시판되고 있다. 제2층(32)은 두께가 0.2mm 이고 기공 크기가 0.2㎛ 이다.
상기 언급한 조합 재료로 제조된 수액 필터는 160㎠ 의 기하학적 표면적에서 피로젠 분리용량이 560㎍ 이다.
제4도에서 제1층(41)은 두께가 0.1mm 이고, 기공크기가 0.04㎛ 이며, 나일론-6,6 으로 구성된다. 제1층(41)은 제2도의 조합 필터와 유사한 바이러스 보유력을 나타낸다.
제2층(2)은 두께가 0.2mm 이고 기공크기가 0.2㎛ 인 포시딘 멤브레인이다.
제3층(43)은 두께가 0.2mm 이고 기공크기가 0.04㎛ 인 카복시딘 멤브레인이다.
상기 언급한 조합 재료로 제조된 수액 필터는 160㎠ 의 표면적에서 피로젠 분리용량이 560㎍ 이다.
Claims (16)
- 두개의 삼로 코우크를 포함하는 수막강내 카테터, 10㎖의 시린지, 및 기공크기가 0.04 내지 0.45㎛ 이고 층 두께가 0.1 내지 1mm 인 멤브레인 층을 포함하며 기하학적 표면적이 50 내지 300㎠ 이고 기공부피가 50 내지 90% 인 필터를 포함하는 뇌척수액의 여과에 사용하기 위한 장치.
- 제1항에 있어서, 멤브레인 층의 기공크기가 0.1 내지 0.3㎛ 임을 특징으로 하는 장치.
- 제2항에 있어서, 멤브레인 층의 기공크기가 0.1 내지 0.2㎛ 임을 특징으로 하는 장치.
- 제1항 내지 3항중의 어느 하나에 있어서, 멤브레인 층의 피로젠 분리 용량이 1000 내지 2000㎍ 임을 특징으로 하는 장치.
- 제1항에 있어서, 멤브레인 층이 폴리아미드로 구성됨을 특징으로 하는 장치.
- 제1항에 있어서, 멤브레인 층이 전하-변형된 멤브레인인 장치.
- 제1항에 있어서, 필터가 멤브레인 층상에 기공크기가 0.1 내지 5㎛ 이고 두께가 0.1 내지 1mm 인 제2 멤브레인 층이 위치한 조합 필터로 조립됨을 특징으로 하는 장치.
- 제7항에 있어서, 제2 멤브레인 층의 기공크기가 0.1 내지 0.45㎛ 임을 특징으로 하는 장치.
- 제7항 또는 제8항에 있어서, 제2 멤브레인 층(22, 32, 42)이 양의 제타 포텐샬을 갖는 전하-변형된 멤브레인임을 특징으로 하는 장치.
- 제7항 또는 제8항에 있어서, 제1 멤브레인 층(31)이 양의 제타 포텐샬을 갖는 전하-변형된 멤브레인이고, 제2 멤브레인 층(32)이 음의 제타 포텐샬을 갖는 전하-변형된 멤브레인임을 특징으로 하는 장치.
- 제7항 또는 제8항에 있어서, 제2 멤브레인 층(42)이 양의 제타 포텐샬을 갖는 전하-변형된 멤브레인이고, 제3 멤브레인 층(43)이 음의 제타 포텐샬을 갖는 전하-변형된 멤브레인임을 특징으로 하는 장치.
- 제9항에 있어서, 바이러스 보유력을 제공하는 제1 멤브레인 층(21, 41)의 기공크기가 0.1㎛ 이하임을 특징으로 하는 장치.
- 제1항에 있어서, 여과 특성을 변화시킴이 없이 필터에 기계적 강도를 제공하는 하나이상의 지지층이 존재함을 특징으로 하는 장치.
- 제1항에 있어서, 필터를 보유하기 위한 접속피스를 갖는 필터 하우징이 존재함을 특징으로 하는 장치.
- 제14항에 있어서, 필터 하우징의 유입면 쪽에 배기 멤브레인이 존재함을 특징으로 하는 장치.
- 기공크기가 0.04 내지 0.45㎛ 이고 층 두께가 0.1 내지 1mm 인 멤브레인 층을 포함하며 기하학적 표면적이 15 내지 300㎠ 이고 기공부피가 50 내지 90% 인 필터를 통해 뇌척수액을 통과시킴을 특징으로 하여 뇌척수액을 여과하는 방법.
