Abstract
プローブを体内に投与後、測定対象となる肝臓に磁気共鳴法を適用して、フリーラジカル濃度のデータを得る工程(1)と、
前記得られたフリーラジカル濃度のデータを処理して画像化情報を得る工程(2)と、
前記画像化情報を経時的に動態計測し、フリーラジカル消費速度を求める工程(3)と、を含み、
前記被験動物に適用して得られたフリーラジカル消費速度情報に基づいて、被験動物がNASHか否かを判定する工程を含むことを特徴とするNASHの判定方法。
Description
NAFLDは、ウィルス性肝疾患、自己免疫疾患性肝疾患、ヘマクロトーシスやWilson病等の代謝性肝疾患などを除外して、飲酒歴のない人に発生する単純脂肪肝から脂肪性肝炎、線維症、肝硬変までの肝障害を含む疾患群と定義され、肝硬変に進行しない単純脂肪肝(SS)と、非アルコール性脂肪肝炎(NASH)に大別される。
NAFLDのうち約80%は非進行性で良性の経過を辿るが、残りの20%はNASHへ進行し、肝硬変、肝癌へと進展する。
単なる脂肪肝がどのようにしてNASHまで進展するかについては、いわゆるtwo hit theoryが広く知られており、まず、first hitによって脂肪肝が発症し、さらにsecond hitとして酸化ストレス、脂質過酸化、エンドトキシンなどにより怠起されるサイトカインの放出などによって肝細胞障害が引き起こされ脂肪性肝炎、肝硬変へと進展していくと考えられている。これらの根底に存在するのがインスリン抵抗性であり、second hitの選択性については遺伝的要因が推定されている。
しかしながら、肝生検は苦痛を伴う侵襲的な検査で、しばしば炎症など検査自体のリスクが指摘されるとともに、採取する組織は肝全体に対し一部(極微量)であるため、採取された肝組織が必ずしも肝臓全体(病変)の状態を反映していないことや、組織診断の評価が診断する医師の間で相違が見られるなど診断の精度が低い点が問題である。
このような背景から、新たなバイオマーカー、画像検査、総合的スコアリングシステムなど非侵襲的診断法の開発が進められているが、NASHの確定診断が可能な判定方法は、これまでに実用化されていなかった。
プローブを被験動物の体内に投与後、測定対象となる肝臓に磁気共鳴法を適用して、フリーラジカル濃度のデータを得る工程(1)と、
前記得られたフリーラジカル濃度のデータを処理して画像化情報を得る工程(2)と、
前記画像化情報を経時的に動態計測し、フリーラジカル消費速度を求める工程(3)と、を含む、前記方法。
<2> 被験動物の非アルコール性脂肪肝炎(NASH)の識別のための指標としてフリーラジカル消費速度の情報を取得する、<1>に記載の方法。
<3> 前記プローブが、ニトロキシルラジカル化合物である、<1>または<2>に記載の方法。
<4> 前記磁気共鳴法が、DNP−MRI法である、<1>から<3>のいずれかに記載の方法。
<5> 被験動物の肝臓全体が対象となるように磁気共鳴法を適用する、<1>から<4>のいずれかに記載の方法。
<6> <1>から<5>のいずれかに記載の方法を、被験動物に適用して得られたフリーラジカル消費速度情報に基づいて、被験動物がNASHか否かを判定する工程を含む、NASHの判定方法。
<7> 被験動物が、NAFLDと診断された被験動物である、<6>に記載の判定方法。
<8> 判定されるNASHが、初期NASHである、<6>または<7>に記載の判定方法。
<9> 前記被験動物がNASHか否かを判定する工程が、既知のNASH罹患動物群及び既知の非NASH罹患動物群のそれぞれ肝臓におけるフリーラジカル消費速度情報に基づいてフリーラジカル消費速度の閾値を設定し、
評価対象の被験動物の肝臓におけるフリーラジカル消費速度が、当該閾値以下になった場合にNASHであると判定する工程である、<6>から<8>のいずれかに記載の判定方法。
<10> コンピュータに、<6>から<9>のいずれかに記載の判定方法を少なくとも実行させることを特徴とするNASH判定用プログラム。
<11> NASHの治療薬の候補物質の投与後の被験動物に対して、<1>から<5>のいずれかに記載の方法を適用して得られたフリーラジカル消費速度情報に基づいて、候補物質のNASHに対する治療作用を判別することを特徴とするNASHの治療薬のスクリーニング方法。
また、本発明によれば、前記NASH判定方法に使用するプログラム、前記NASH判定方法によるNASH治療薬のスクリーニング方法が提供される。

このような生体外物質として、例えば、ヒトに対して有害でない化合物である、ニトロキシルラジカル化合物が挙げられる。ニトロキシルラジカル化合物としては、Carboxy PROXYL、Tempol、Methoxycarbonyl PROXYL、Carbamoyl-PROXYL等が挙げられる。なお、肝組織はニトロキシルラジカルの代謝が非常に早い臓器であり、TempolやMethoxycarbonyl PROXYLなどのプローブでは検出が困難である。またCarboxy PROXYLは細胞膜透過性が無いため、血管内のレドックス解析には有効であるが、肝細胞内の解析は困難で有る。Carbamoyl-PROXYLは、毒性が低く、細胞膜透過性を有し、代謝反応が緩やかであるため好適である。
すなわち、本発明の第4の態様は、NASHの治療薬の候補物質の投与後の被験動物に対して、本発明のフリーラジカル消費情報取得方法を適用して得られたフリーラジカル消費速度情報に基づいて、候補物質のNASHに対する治療作用を判別することを特徴とするNASHの治療薬のスクリーニング方法である。上述の通り、本発明のNASH判定方法によれば、初期NASHについても判別できるため、より効率よく、有益なNASHの治療薬をスクリーニングすることができる。
NASH病態モデルマウスは、以下の手順で作製した。なお、全ての手順および動物の取扱いは、九州大学薬学部動物実験倫理委員会により承認されており、九州大学薬学部動物実験ガイドラインに沿って実験を行った。
C57BL6マウス(雌、5週齢)は、日本チャールズリバー(横浜、日本)から購入し、実験前に1週間順応させ実験を開始した。マウスはケージ当たりに5匹で、温度および概日リズムを調整した室内で、メチオニンーコリン欠乏食または通常食を与えて、飼育した。メチオニンーコリン欠乏食群(NASH病態モデルマウス群)、通常食群(正常マウス群)はそれぞれ5群ずつに分けられ、1,2,4,6,8週毎にDNP−MRIの測定を行い病態評価のため採血、解剖、肝組織採取を行った。
図2にDNP−MRI評価の様子を示す模式図に示す。
マウス(NASH病態モデルマウス)を、イソフルラン(導入4%、維持2%)で麻酔し、胃側を下にして粘着性の皮膚用テープで肝計測用局所検出器(サーフェイスコイル)とホルダーに固定した。実験中のマウスの体温は37±1℃だった。マウスをDNP−MRI共鳴装置に移し、肝臓全体が測定対象となるように設置した後に、NASH病態モデルマウスにCmP(Carbamoyl−PROXYL、レドックス造影剤)を静注し、DRI−MRI測定を開始した。
DRI−MRIの条件は以下の通りである。
TR、1200ms;TE、25ms;TEPR、600ms;number of phase−encoding gradient steps、64;NEX、4;FOV、48mm´48mm;Matrix size、64´64(implane resolution 0.75mm);slice thickness、30mm;number of average、1、scanning time、79s。OMRIデータは、Image J software package(http://rsb.info.nih.gov/ij/)を使用し、解析を行った。
図3に、レドックス造影剤(CmP)を静注により投与した後のマウス上腹部のDNP-MRIによる経時的なDNP-MRI画像(インビボイメージンング)を示す(上段:正常マウス(コントロールマウス)、下段:NASH病態モデルマウス)。
正常マウス(コントロール)においては、CmPは上腹部に均一に分布した後、時間経過と共に速やかにCmPラジカルがレドックス代謝を受け消失し、CmP投与から5.5分以降はほとんど消失していることがわかる。一方、NASH病態モデルマウス(NASHモデル)においては、CmP投与後、上腹部CmPに由来する高い強度のシグナルが観測され、その後徐々にレドックス代謝が進んでいることがわかる。このように、NASH病態モデルマウスでは、CmPラジカルの減少速度は、正常マウスの場合と比較して明らかに遅く、CmP投与から13分後においてもCmPラジカルが残存していた。
NASH病態モデルマウス群に関して、メチオニンーコリン欠乏食(MCD)開始後1週(MCD1),2週(MCD2),4週(MCD4),6週(MCD6),8週週(MCD8)においてDNP-MRI計測を行い、肝臓におけるCmPラジカルのレドックス代謝速度を算出した。レドックス代謝速度が有意に低下する場合にはNASH病態として判定する。
正常マウス群に関しても、同様に通常食開始後1週(ND1),2週(ND2),4週(ND4),6週(ND6),8週週(ND8)においてDNP-MRI計測を行い、肝臓におけるCmPラジカルのレドックス代謝速度を算出した。
図4に示されるように、正常マウスにおいて、肝臓におけるCmPのレドックス代謝速度は0.17〜0.23min−1を示し、ミトコンドリア電子伝達系におけるレドックス代謝を主として比較的早いレドックス代謝速度を示している。
一方、NASH病態モデルマウス群においては1週(MCD1)から有意にレドックス代謝速度(消失速度)の低下が見られ、時間経過とともにレッドクス代謝速度は減少している。
図4においては、NASHを判別するための閾値を、0.16min−1に設定し、被験マウスのレドックス代謝速度がこの閾値を下回った場合であれば、NASHと判定する。
ヒトの場合も同様に、ただの脂肪肝(NAFLD)をもつヒトとNASH患者に対して本発明のNASH判定方法を用いることで、両者を精密に区別して診断することが可能となる。
Claims (11)
- 非侵襲的に被験動物の肝臓におけるレドックス反応をリアルタイムで検出し、フリーラジカル消費速度の情報を取得する方法であって、
プローブを被験動物の体内に投与後、測定対象となる肝臓に磁気共鳴法を適用して、フリーラジカル濃度のデータを得る工程(1)と、
前記得られたフリーラジカル濃度のデータを処理して画像化情報を得る工程(2)と、
前記画像化情報を経時的に動態計測し、フリーラジカル消費速度を求める工程(3)と、を含む、前記方法。 - 被験動物の非アルコール性脂肪肝炎(NASH)の識別のための指標としてフリーラジカル消費速度の情報を取得する請求項1に記載の方法。
- 前記プローブが、ニトロキシルラジカル化合物である請求項1または2に記載の方法。
- 前記磁気共鳴法が、DNP−MRI法である、請求項1から3のいずれかに記載の方法。
- 被験動物の肝臓全体が対象となるように磁気共鳴法を適用する請求項1から4のいずれかに記載の方法。
- 請求項1から5のいずれかに記載の方法を、被験動物に適用して得られたフリーラジカル消費速度情報に基づいて、被験動物がNASHか否かを判定する工程を含むことを特徴とするNASHの判定方法。
- 被験動物が、NAFLDと診断された被験動物である請求項6に記載の判定方法。
- 判定されるNASHが、初期NASHである請求項6または7に記載の判定方法。
- 前記被験動物がNASHか否かを判定する工程が、既知のNASH罹患動物群及び既知の非NASH罹患動物群のそれぞれ肝臓におけるフリーラジカル消費速度情報に基づいてフリーラジカル消費速度の閾値を設定し、
評価対象の被験動物の肝臓におけるフリーラジカル消費速度が、当該閾値以下になった場合にNASHであると判定する工程である請求項6から8のいずれかに記載の判定方法。 - コンピュータに、請求項6から9のいずれかに記載の判定方法を少なくとも実行させることを特徴とするNASH判定用プログラム。
- NASHの治療薬の候補物質の投与後の被験動物に対して、請求項1から5のいずれかに記載の方法を適用して得られたフリーラジカル消費速度情報に基づいて、候補物質のNASHに対する治療作用を判別することを特徴とするNASHの治療薬のスクリーニング方法。
