Description

【発明の詳細な説明】 本発明は全股関節補綴のための寛骨臼コツプおよび該コ
ツプとその支持骨部材との間に均一の空間を形成するた
めのその寛骨臼への装着に関する。
種々のタイプの補綴が股関節の修正または代用に利用で
き、これらの補綴は一反的に凹状球形表面の寛骨臼コツ
プとコツプに適合する球状の関節頭をもった太腿茎を使
用している。最近の復元手術の際の寛骨臼コツプの主要
問題として、自然の寛骨臼内にコツプを不均斉に設置す
る点が挙げられる。この不均斉な設置はコツプのまわり
に不均一な骨のセメント層を形成することにあり、その
結果、コツプのまわりに局部的な圧力を与え、次の使用
中に寛骨臼からコツプを早くゆるめることになる。
従って、本発明の目的は、補綴のまわり全体に等間隔の
セメントを設けて寛骨臼の中心に補綴を容易かつ堅実に
装着させるための器具を提供するとともに、改良された
安定性のある寛骨臼コツプの補綴を提供することにある
。
補綴装着部と支持部材との間にセメントスペーサを使用
することによってセメントの厚みを制御状のうねを有す
る従来の寛骨臼コツプは、例えば、アメリカ合衆国特許
第３．６９８．０１７号、第４．０３１．．５７０号お
よび第４．１４１．０８８号に開示されている。金属製
外側シェル部材に収納されたコツプ部材を含む寛骨臼コ
ツプはアメリカ合衆国特許第４．１７２．２９６号に開
示されている。
望ましい客体は外側金属シェルと内側弾性プラスチック
装着部を含む寛骨臼コツプによって達成され、該装着部
が太腿頭に適合するようにされた球形の内側面を有し、
外側面が球状クラウン内の上方端部で円錐台形になって
おり、該シェルが装着部の外側面で実質的に適合する内
側面を有し、かつ実質的に半球状の外側面が寛骨臼に埋
まるようにされた環状フランジを有する。シェルの外側
面はフランジに平行に離間された均−高さの環状隆起を
有し、フランジと隆起は寛骨臼の壁から均等間隔で離間
したシェル外側面を持つ寛骨臼内にコツプをセンタリン
グする働きをするようにされている。
好ましくは、装着部の外側面とシェルの内側面とがシェ
ル内に装着部を固定するための適合する嵌合回転止め手
段を有する。この回転止め手段は装着部の外側面に少な
くとも１つの突起を含み、シェルの内側面内の適合する
回転止め手段と係合し、突起は装着部の基板に平行に隣
接する複数の同間隔に離間した実質的に矩形の隆起を構
成して側面内の適合する回転止め手段と係合し、突起は
円錐台の軸に平行の半球状の突出部を含み、シェル内の
装着部の回転を防止する。コツプは、更に、コツプの口
にある回腸筋の切欠部を含んでいて、コツプによって回
腸筋の鍵の剥離を防止する。
同様に、本発明の寛骨臼コツプを設置するために寛骨臼
のリーマ−を有し、寛骨臼を露出するためにカッティン
グ歯を持った従来の半球形のり−マーを含む。第２のリ
ーマ−は寛骨臼の底部の半球状の頂点との接触によって
限定された深みにコツプのフランジを受けるために寛骨
臼の周囲に埋まる歯を持ったカッティング・フランジに
よって基板が囲まれた平滑な半球体を有する。第３のリ
ーマ−は平滑なフランジによって基板が囲まれた半球体
を含み、該半球体は平滑なフランジに合致する寛骨臼を
広げるカッティング歯を有する。
本発明の上記の目的、特徴、および利点は、次に図面を
参照して説明する。
第１図から第９図は外側の金属シェル１２と内側の弾性
プ゛ラスチック装着部１４から成る股全体の代用品の寛
骨臼のコツプＩＯを示す。コツプ１０は自然の寛骨臼の
コツプの代用品として働き、骨盤１８内の寛骨臼■６に
導入するために構成されている。コツプｌＯは骨盤１８
の左右いずれにも使用でき、適当な準備後に寛骨臼１６
に挿入される。準備として、寛骨臼の拡大が必要であり
、本発明においては、更に下で詳述するような穴の口を
広げる工程を含む。
装着部１４は高密度のポリエチレンのようなものから作
ることができ、大腿骨補綴２４０球状の頭部と関節で接
合する球状の内側面２ｏを有する。
装着部１４の外側面２６は球状の王冠３ｏの上方端で切
れる円錐台２８を構成する。このような装着部の外側面
２６の形状は身体に力弓る圧力荷重を分散させ、かつ、
また装着部１４の加工を簡単にする。
シェル１２は、例えばコバルト、クロミウム、モリブデ
ンの合金から作られ、装着部の外側面２６と実質的に合
致する内側面３２と寛骨臼１６に埋まるようになった環
状フランジ３６内で終る実質的に半球状の外側面３４と
を有する。シェルの外側面３４もフランジ３６から均等
の高さで離間した環状隆起３８を有し、フランジ３６と
隆起３Ｂは後述するようにされていて、シェルの外側面
３４が寛骨臼１６の壁４ｏがら均等に離間するように寛
骨臼１６内にコツプ１０のセンタリングを行なわせる働
きをする。隆起３８は地続きであるが寛骨臼１６にコツ
プＩＯを設置する間、骨のセメントを均一に流すために
は分かれている方がよい。
シェルの外側面３４も２つの同心の離間したアンダーカ
ット（下から切−り取ｐた部分、）　４；２を含む。
装着部の外・側面２６とシェルの内側面３２は装着部１
４をシェル内に固定するために対応して嵌合する回転止
め手段を有する。この手段は装着部１４の基板４６に平
行かつ隣接する実質的に矩形の隆起４４の形状をした３
つの等間隔の突起を有し、シェルの内側面３２において
適合する隆起と係合する。装着部１４は、更に、各々の
隆起４４の後のアンダーカット５０を有し、アンダーカ
ット５０は第９図に示されている。装着部１４がシェル
１２の内側に設置されるとき、アンダーカット５０は隆
起４４を装着部１４の中央に向って内方に変形させてシ
ェル１２により滑り込む。装着部１４が完全にシェル１
２内に入ると、隆起４４はくぼみ４８と嵌合し、それに
より装着部がシェル１２内に確実に係止する。
シェル１２はまた装着部の外側面２６上で対応するくぼ
み５４と係合する円錐台２８の軸に平行の半円形の突出
部５２を有する。突出部５２と装着部のくぼみ５４０組
合によるくぼみ手段は装着部１４をシェル１２に挿入す
る際の調節ガイドを提供するだけでなく挿入後の装着部
１４のシェル内での回転な防止する。
寛骨臼１６内へのコツプ１ｏの適正配置は寛骨臼１６の
注意深い準備を必要とする。この準備は第１Ｏ図から第
１２図を参照に説明する。
第１に、第１０図に示すように、寛骨臼１６は従来から
ある半球状のり〜マー５６で露出される。
次に、第１１図に示すように、歯６４を有するカッティ
ング・フランジ６２によりその基板が囲まれているなめ
らかな半球体６０を構成する第２リーマ−５８が寛骨臼
を取り囲む骨盤１８の骨を平坦にし、それによってさら
穴６６を作り、コツプ１０のフランジ３６を座らせる。
このさら穴の深さは寛骨臼１６の底部７０を持つ半球体
６００頂点６８の接触によって制限される。第１２図に
示す第３リーマ−７２は半球体７４を構成し、なめらか
なフランジ７６によって基板が囲まれ、歯７８を有し、
最後に、フランジ７６と同心の寛骨臼の穴を広げる。
この穴を広げる準備はシェル１２の形状を利用して行な
われ、寛骨臼内にコツプ１ｏの対称的な装着を確実にす
る簡単な方法を提供し、装着の深さは埋めこみ領域６６
に接触するフランジ３６によって限定され、横方向の移
動は壁４０をもつ隆起３８との接触によって防止される
。その結果、コツプ１０と代表的には約２胴の壁４０と
の間に均等の空間が形成され、コツプ１０のまわりの骨
セメントの均一の適用を保証し、それによって集中圧迫
を最少限にする。
寛骨臼のコツプＩＯは寛骨臼１６からの骨の除去を最少
限にするような低い側面設計になっている。このコツプ
は、更に回腸筋切欠部８２を含み、コツプＩＯに対して
回腸筋の鍵（図示せず）の剥離を防止する。
以上、本発明は好ましい態様として記載されているにす
ぎず、特許請求の範囲およびその精神の範囲内の発明の
変更、改良等を含む。

Claims (1)

  1. 【特許請求の範囲】 １、外側金属シェルと内側弾性プラスチック装着部とか
    ら成り、 該装着部が大腿骨頭と球状の王冠内の上方端で切れる円
    錐台から成る外側面とに関節が接合するようにされた球
    形の内側面を有し、 該シェルが該外側面と実質的に合致する内側面と寛骨臼
    に埋めこまれるようにされた環状フランジで終る実質的
    に半球状の外側面とを有し、該シェルの外側面が該フラ
    ンジから均等の高さで離間しかつそれに平行の環状隆起
    を有し、該フランジと該隆起が寛骨臼の壁から均等に離
    間した該シェルの外側面とともに寛骨口内に該コツプの
    センタリングを促進するようにされていることを特徴と
    する寛骨臼へ装着するための寛骨臼コツプの補綴。 ２、上記装着部の外側面と上記シェルの内側面とが該シ
    ェル内に該装着部を固定するための適合する場合回転止
    め手段を有する特許請求の範囲第１項に記載のコツプの
    補綴。 ３、上記回転止め手段が上記シェルの内側面にお特許請
    求の範囲第２項に記載のコツプの補綴。 ４、上記実記が上記装着部の基板と平行に隣接する複数
    の等間隔に離間した実質的に矩形の隆起ケ含む特許請求
    の範囲第３項に記載のコンブの補綴。 ５、上記回転止め手段が上記装着部の外側面において適
    合する回転止め手段と係合するために該シェルの内側面
    上に少なくとも１つの突起を有する特許請求の範囲第２
    項に記載のコツプの補綴。 ６、上記突起が上記シェル内の上記装着部の回転を防止
    するために上記円錐台の軸に平行の半球状の突出部を含
    む特許請求の範囲第５項に記載のコツプの補綴。 ７、上記コツプによって回腸筋の朧の剥離を防止するた
    めに該コツプの口に回腸筋の切欠部を有する特許請求の
    範囲第１項に記載のコツプの補綴。 ８．半球体の頂点と寛骨臼の底部との接触によって限定
    された深みにコツプのフランジを受けるために寛骨臼の
    周囲を埋めこむ歯を有するカッティングフランジによっ
    て基板が囲まれている平滑な半球体を含むことを特徴と
    する特許請求の範囲第１項に記載の寛骨臼のコツプの補
    綴を設置するために使用する寛骨臼のリーマ−８ ９、平滑なフランジによって基板が囲まれた半球体を含
    み、該半球体が該平滑なフランジと同心の寛骨臼を広げ
    るためのカッティング歯を有することを特徴とする特許
    請求の範囲第１項に記載のコツプの補綴を設置するため
    に使用する寛骨臼のリーマ−０
