PURPOSE:To form a spiral groove on a base plate in a simple process by applying the base plate made of polymethyl methacrylate with methyl methacrylate or its prepolymer by spin coating, by pressing a mask pattern with the spiral groove against it, and then by performing irradiation with ultraviolet rays. CONSTITUTION:A base plate 1 made of polymethyl methacrylate is applied with methyl methacrylate containing a light initiator or its prepolymer 2 by spin coating by utilizing a rotary system 3 (a). Then, a mask pattern 4 with a spiral groove is pressed against it to bring one groove-side surface of the mask pattern 4 into close contact with the base plate 1 (b). The side of the base plate 1 is irradiated with ultraviolet rays 5 to fix the methyl methacrylate or its prepolymer 2 on the base plate 1 (c). The mask pattern 4 is removed to obtain a disk base plate 6 with the spiral groove (d).
