【発明の詳細な説明】
レーザビーム眼科手術方法及び装置発明の背景
本発明は眼の屈折度手術に関し、特定的には人の、または動物の眼の角膜を整
形(リシェイプ)するために、複数のレーザビームを使用して眼の角膜組織を切
除する屈折度手術に関する。
角膜は、ほぼ同心の前表面及び後表面を有する薄いシェルであり、中心の厚み
は約 520μmである。角膜は 1.377の屈折率を有し、名目的な曲率半径は 7.86
mmである。角膜の前表面を形成している上皮の中心における厚みは、若い人で
約 70 μmである。上皮の下にはボーマンの層、またはボーマンの膜と呼ばれる
層があり、この層の厚みは約 12 μmである。これは、角膜の大半を作っている
ストロマの前表面を覆い、主としてコラーゲンファイバからなっている。内皮は
角膜の後層を形成し、細胞の単一層である。
眼の屈折力のほぼ 3/4は角膜の前表面の曲率によって決定されるので、角膜の
形状を変化させることは眼の屈折誤差を大幅に減少乃至は排除する一つの方法で
ある。ストロマは十分に厚いので、補正のためにストロマの前領域の部分を切除
してそのプロファイルを変化させ、残余のストロマ組織の大部分はそのままにす
ることによって眼の屈折力を変化させることができる。緑内障、白内障の処置、
及び屈折度手術を含む眼病応用に種々のレーザが使用されてきた。屁折度手術(
即ち角膜の整形)の場合には、紫外(UV)レーザ(193nmにおいてエキシマ
及びNd:YAGの第5高調波の 213nm)を使用して光屈折度角膜切除(PR
K)と呼ばれる大きい領域の表面角膜を切除してきた。角膜の整形は、ファイバ
で結合された接触型、または非接触型プロセスを使用し、現在Ho:YAGレー
ザで行われているレーザ熱凝固によっても遂行することができる。
屈折度手術は、角膜を整形するために角膜組織を光切除するのに使用されてい
るエキシマレーザ(193nm)及びソリッドステートレーザの第5高調波(190
nm−215nm)の開発によって新次元に到達している。マスクまたはダイヤフ
ラムを使用し、このマスクまたはダイヤフラムを移動させてレーザビームをブロ
ックして角膜の外面に所望の曲率を得る方法を含む、角膜の表面にレーザビーム
を送給する幾つかの方法が提唱されている。また、組織を切除して角膜の曲率を
変化させるために、角膜の外面上のレーザビームスポットを移動させるスキャナ
を使用することも提唱されている。角膜の外面に所望の曲率を得るために、レー
ザビームをブロックしたり、運動させたりするようにマスクまたはダイヤフラム
とスキャナとの組合わせも使用されている。マスクまたはダイヤフラム方法は高
エネルギレーザを必要とし、またレーザと角膜との相互作用によって角膜表面が
粗く、即ち段付きになる。レーザが角膜組織と相互作用すると水が生成され、こ
の水が角膜の表面に残る(汗水と同じように)。新しいレーザパルスが角膜に到
達した時に、それが切除率を変化させる。もしこのことを考慮に入れなければ、

したり、非対称レーザビーム空間エネルギ分布の場合の汗水効果の非均一状況を
補償する。
屈折誤差は、2つのカテゴリに分割することができる。即ち、球形及び円筒形
である。球形は、近視または遠視として眼に影響を与え得る。円筒形は、近視性
の、または遠視性の乱視として眼に影響を与え得る。本発明は、遠視性乱視にお
ける角膜の中央部分の切除を回避するようにコンピュータプログラムを使用する
ので、より安全な、より予測が可能な、そしてより高速な手術手順を得ることが
できる。
角膜の遠視と乱視とが組合っている場合は、中央部分には決して触れない。発明の概要
レーザビーム眼科手術方法は、レーザビームを生成する段階と、生成したレー
ザビームを複数のレーザビームに分割し、複数のスキャナ上に同時に合焦させる
段階とを含んでいる。各スキャナは所定のレーザビーム走査パターンを発生し、
患者の眼の角膜上に導いて角膜組織を切除する。その際、少なくとも2つのビー
ムが角膜組織を同時に切除する。走査は、手術手順を遂行するように、中央処理
ユニットから制御される。レーザ眼科手術装置が提供され、本装置は、レーザビ
ームを生成するレーザと、このレーザビームを複数のレーザビームに分割するビ
ームスプリッタとを含んでいる。複数のスキャナが、それぞれビームスプリッタ
からレーザビームの1つを受け、レーザビームから所定の走査パターンを発生し
、この走査パターンを患者の眼の角膜に衝突させるように位置決めされている。
装置は、各レーザビームを集束させる集束光学系と、走査されたビームを所定の
走査パターンで角膜上に導いて眼の角膜の一部を切除する光学系とを更に含んで
いる。各スキャナに接続されているコンピュータは、同心円形対称パターン、ま
たは線形平行対称ビームパターンの何れかであることができる所望の走査パター
ンを発生させる。図面の簡単な説明

膜切除システム10は、193 nmの紫外波長を有するレーザビーム12を発生す
るレーザ11(エキシマレーザであることができる)を有している。レーザビー
ム12はビームスプリッタ13に衝突し、ビームスプリッタ13はビーム12を
2つのレーザビーム14及び15に分割する。一方のレーザビーム14はガルバ
ノメータスキャナ16(鏡を取付けてあるガルバノメータを使用する典型的なス

18及び23によって導かれたビーム17及び22は、スキャナ16及び21に
よって制御されたパターンを有する平行レーザビームとなり、眼24の角膜20
に同時に衝突する。
本発明においては、マイクロコンピュータ27が、同期制御回路19を通して
レーザ11に接続され、所望のどのような型の走査パターンも発生させるように
プログラム可能であり、またライン30を通してX−Y座標走査ドライバ28に
接続されている。X−Y座標走査ドライバ28は、それからの電気信号がスキャ
ナ16をX−Y座標パターンで駆動するように、スキャナ16に接続されている
ライン31及び32上にX−Y座標走査ドライバ信号を発生する。コンピュータ
27はライン29を通してX−Y座標走査ドライバ33へも同一のX−Y座標走
査ドライバ信号を供給し、X−Y座標走査ドライバ33はスキャナ21に接続さ
れているライン34及び35上にX−Y座標信号を供給してスキャナ21にX−
Y座標走査パターンを発生させる。エキシマレーザの場合には、眼に衝突する走
査ビーム25及び26は 193nmのビーム波長を有しているが、各ビームはビー
ムスプリッタ13の透過特性及び反射特性を使用することによって空間得分布モ
ードで互いに位相ずれの関係を持たせることができる。
2つまたはそれ以上の走査デバイスを使用し、2つまたはそれ以上のレーザビ
ームを角膜の外面上で走査させて各ビームで眼を同時に切除する。これは、図3
に示すようなパターンで達成することができる。図3において、円36は眼の角
膜を表しており、一方向に走査する走査線37は走査レーザビーム25または2
6の一方によって遂行され、同時に走査線38は走査ビーム37とは反対の方向
に走査され、両ビームは同時に、そして重なり合うトレースで走査する。しかし
ながら、図から明らかなように、1a−1dと名付けた走査線が走査線2a−2
dとは逆方向に進み、2つのレーザビームの場合には、レーザビーム25の走査
線37は一つの方向に走査し、一方レーザビーム26は逆の側から、レーザビー
ム走査線37とは逆方向に走査線38を走査する。同様に、図4に示す場合には
角膜36は円形に走査されるようになっており、走査線40は一つの方向に進み
、一方レーザビーム26は眼36の反対側から走査線41に沿って、走査線40
と重なるように走査し、両ビームは同時に走査する。ビーム26は破線に沿って
走査し、一方実線はビーム25による走査を表している。
特に図2を参照する。この場合もエキシマレーザであってよいレーザ11は紫
外出力ビーム12を発生してシャッタ42へ印加する。シャッタ42は、このレ
ーザビームを集束レンズ43へ印加する。集束レンズはビーム12をビームスプ
リッタ44上に合焦させる。集束レンズ43及びビームスプリッタ44は、図1
の集束及びビームスプリッタ13の部分であり、分割されたビーム14及び15
を生成する。勿論、本発明の思想及び範囲から逸脱することなく、要望に応じて
ビームは2つより多くのビームに分割できることは明白であろう。次いで、ビー
ム14は図1の走査対16(ビームを走査させる第1のスキャナ45を含み、走
査されたビームは第2のスキャナ46へ印加される)に印加される。両スキャナ
45及び46はガルバノメータスキャナであって、それらに取付けられた鏡を有
する電気的ガルバノメータを有し、スキャナ45がビームをX方向に走査させ、
一方スキャナ46がビームをY方向に走査させて鏡に衝突するビーム17をX−
Y方向に制御し、眼24の角膜20上に走査ビーム25を形成できるようになっ
ている。スキャナ45にはライン34を通してY座標信号が印加され、一方スキ
ャナ46にはライン35を通してX座標信号が印加されている。これらの信号は
、中央処理ユニット27からライン29を通して制御信号を受けているX−Y座
標走査ドライバ33において生成されたものである。コンピュータ27はそれに
取付けられた制御画面47を有し、X−Y座標制御信号をも生成してライン30
を通してX−Y座標走査ドライバ28にも供給する。X−Y座標走査ドライバ2
8はライン32上にY座標信号を、またライン31上にX座標信号を生成して1
対のスキャナ48及び50へ供給する。各スキャナは1つの座標走査信号を発生
し、この信号は光学系23で制御される走査ビーム22を発生させて眼走査ビー
ム26を眼24の表面上に形成させる。
以上の説明から、複数のレーザビームを発生する装置が提供されていることが
明白になったであろう。これらのビームは眼の角膜の表面上を同時に走査し、眼
の一部を切除して角膜の屈折度を補正し、またこれらのビームは角膜の一部分の
反対側の、角膜の中心から離れたところに同時に生成され、角膜の制御された走
査切除を行う。
レーザビーム眼科手術を遂行する方法は、1つのレーザから1つのレーザビー
ムを生成する段階と、このレーザビームをビームスプリッタ13を通して複数の
レーザビームに分割する段階と、分割された各レーザビームをX−Yスキャナま
たは1対のスキャナへ印加し、X−Yスキャナまたは1対のスキャナにビームを
制御させ、走査ビームを鏡または光学系を通して眼の表面上へ印加させて眼の角
膜の一部分の切除を遂行させる段階とを含んでいる。各レーザビーム毎の各対の
ビームスキャナを制御するX−Y座標スキャナを制御するコンピュータプログラ
ムに従って複数の走査ビームが同時に走査され、互いに概ね重なり合わされる。
プロセスは、ビームスプリッタ上にビームを合焦させる段階と、複数の走査レー
ザビームを眼の表面上に導き、図3及び4に示すような所定のパターンでビーム
を制御する段階を含む。
図6は 90°及び 180°の両方においてコンピュータ制御によって遠視性乱視
補正を達成した眼の、変化後の表面曲率の典型的な断面図であり、一方図7及び
8はコンピュータ26のコンピュータ制御を通して本装置に従って遂行される乱
視補正のための切除される角膜のアルゴリズムである。

ンピュータ選択を行わせる。1つの選択は、近視、遠視、または乱視補正のため
であり、これを選択するとデュアルビームスキャナ走査パターンが決定される。
必要な信号を発生させるための情報がコンピュータに供給される。コンピュータ
はまた、特定の患者の眼についてのジオプティック補正の量のための入力も有し
ている。コンピュータは、近視、遠視、または乱視の何れかの補正のための、及
び特定の患者の眼に必要な補正のジオプティックスのためのアルゴリズムに基づ
いて信号を供給する。スキャナからコンピュータへフィードバックされる信号も
、複数のレーザビームを同時に操作するために多重スキャナ対の駆動の補正を可
能にする。このようにしてコンピュータは、遠視性乱視を補正するために図7及
び8に示すアルゴリズムに従ってレーザビーム対で角膜を切除するための走査を
発生し、補正の量はジオプタ入力によって決定される。
以上の説明から本発明が、乱視、または近視、または遠視、もしくは近視と乱
視の組合わせ、または遠視と乱視の組合わせを補正するために、2つまたはそれ
以上のレーザビームを同時に使用することによって角膜の外面、または角膜内で
眼科手術を行う方法及び装置の両方が提供され、これらの複数のレーザビームは
同一のレーザ源から形成されるが位相ずれの関係にあり、各ビームはビームスプ
リッタを通して得ており、これらのビームは患者の眼の屈折度手術を遂行するよ
うに所定のコンピュータ制御レーザスキャナ上で走査されることが明白になった
であろう。本発明による複数のレーザビームを用いる切除は、遠視性乱視を補正
する場合に角膜組織の中央部分を対称的に切除して眼の屈折度補正を行うので、
遠視性乱視を補正するためのより安全で、より予測可能な手術手順が得られる。
しかしながら、本発明は、単なる例示のために図示した形状に限定されるもので
はないことを理解されたい。

Claims (1)
- 【特許請求の範囲】 (1) 角膜の一部分を切除するレーザビーム眼科手術方法であって、 レーザビーム(12)を生成する段階と、 上記生成したレーザビーム(12)を複数のレーザビーム(14、15)に 分割する段階と、 上記複数の各レーザビーム(14、15)をスキャナ(16、21、45、 46、48、50)上に合焦させる段階と、 患者の眼(24)の角膜(20)を切除するために上記複数の各レーザビー ム(14、15)を所定の走査パターンで走査させる段階と、 互いに平行な上記複数の走査レーザビーム(25、26)を患者の眼の角膜 (20)の表面上に直接導き、上記複数の走査レーザビーム(25、26)を互 いに概ね重畳する平行パターンで走査させる段階と、 上記各スキャナ(16、21、45、46、48、50)を中央処理ユニッ ト(27)から制御し、それによって上記眼の上記角膜を手術的に整形する段階 と、 を備えていることを特徴とするレーザビーム眼科手術方法。 (2) 上記複数のレーザビーム(14、15)を走査させる段階は、2つの各レー ザビームを走査させ、各レーザビームの位相を他方のレーザビームの位相からず らせる段階を含んでいる請求項(1)に記載のレーザビーム眼科手術方法。 (3) 上記生成したレーザビームを複数のレーザビームに分割する段階は、上記レ ーザビームを2つのレーザビーム(14、15)に分割し、各レーザビームを別 々のスキャナ(16、21、45、46、48、50)に衝突させる段階を含ん でいる請求項(2)に記載のレーザビーム眼科手術方法。 (4) 上記各走査レーザビームは概ね平行に、そして他方のレーザビーム(25、 26)から分離したビーム(25、26)で走査し、各平行ビームは第2の平行 ビームと重畳するパターンを形成している請求項(3)に記載のレーザビーム眼科 手術方法。 (5) 上記走査を制御する段階は、同一の上記中央処理ユニット(27)を用いて 2つのスキャナ(16、21、45、46、48、50)を制御する段階を含ん でいる請求項(4)に記載のレーザビーム眼科手術方法。 (6) 上記レーザビームを生成する段階は、193 nm乃至215nmの紫外波長を有 するレーザビーム(12)を生成する段階を含んでいる請求項(5)に記載のレー ザビーム眼科手術方法。 (7) 上記レーザビームを生成する段階は、193 nmの波長を有するエキシマレー ザからレーザビームを生成する段階を含んでいる請求項(6)に記載のレーザビー ム眼科手術方法。 (8) 上記各レーザビームを走査させる段階は、各レーザビームをガルバノメータ スキャナ(16、21、45、46、48、50)の対を用いて走査させる段階 を含んでいる請求項(5)に記載のレーザビーム眼科手術方法。 (9) 上記走査ビームの各対を走査させる段階は、上記2つのビームの上記各レー ザビーム(14、15)を、各々が他方に対して平行の複数の概ね直線状に、且 つ各ビームが他方とは反対方向に走行するように走査させる段階を含んでいる請 求項(8)に記載のレーザビーム眼科手術方法。 (10) 上記走査ビームの対を走査させる段階は、上記2つのビームの上記各レー ザビーム(14、15)を、上記角膜の中央部分の周囲を互いに平行の概ね円形 のビームパターン(図4及び5)で走査させる段階を含んでいる請求項(9)に記 載のレーザビーム眼科手術方法。 (11) 角膜の一部分を切除するレーザ眼科手術装置(10)であって、 レーザビーム(12)を生成するレーザ(11)と、 上記レーザ(11)からのレーザビーム(12)を複数のレーザビーム(1 4、15)に分割するビームスプリッタ(13、44)と、 上記ビームスプリッタ(13、44)からの上記レーザビーム(14、15 )の1つ受けるようにそれぞれ位置決めされ、それに衝突する上記レーザビーム (14、15)から所定の走査パターンを発生させる複数のスキャナ(16、2 1、45、46、48、50)と、 上記レーザ(11)と上記ビームスプリッタ(44)との間に位置決めされ 、上記各レーザビームを上記スキャナの1つの上に合焦させる集束光学系(43) と、 上記複数のスキャナ(16、21、45、46、48、50)と上記角膜と の間に位置決めされ、上記各レーザビームを平行パターンで患者の眼の角膜上に 同時に導き、上記眼の角膜の一部分を切除させる導ビーム手段(23、18)と 、 上記各スキャナに接続され、上記各スキャナ(16、21、45、46、4 8、50)を所定のパターンで制御し、それによって複数のレーザビームが患者 の眼に対して手術手順を遂行できるようにするコンピュータ(27)と、 を備えていることを特徴とするレーザビーム眼科手術装置(10)。 (12) 上記ビームスプリッタは、複数の位相のずれたレーザビームを発生するよ うになっている請求項(11)に記載のレーザビーム眼科手術装置(10)。 (13) 上記ビームスプリッタ(13、44)は、上記生成されたレーザビームを 2つの位相のずれたレーザビームに分割し、各レーザビームを別々のビームスキ ャナ(16、21、45、46、48、50)上に衝突させるようになっている 請求項(12)に記載のレーザビーム眼科手術装置(10)。 (14) 上記導ビーム手段(18、23)は、上記各走査レーザビームを概ね平行 に、且つ互いに離間させて患者の眼上に導くようになっている請求項(13)に記載 のレーザビーム眼科手術装置(10)。 (15) 上記コンピュータ(27)は、上記各ビームスキャナ(16、21、45 、46、48、50)を同時に制御するようになっている請求項(14)に記載のレ ーザビーム眼科手術装置(10)。 (16) 上記レーザ(11)は、193 nm乃至215nmの紫外波長を有するレーザビ ーム(12)を生成するようになっている請求項(15)に記載のレーザビーム眼科 手術装置(10)。 (17) 上記レーザ(11)は、193 nmの波長を有するエキシマレーザであるよ うな請求項(16)に記載のレーザビーム眼科手術装置(10) (18) 上記各スキャナ(16、21、45、46、48、50)は、ガルバノメ ータに取付けられている鏡を有するガルバノメータスキャナであるような請求項 (17)に記載のレーザビーム眼科手術装置(10)。
