【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は股部（股関節）( hi
p ) のトータル補綴物（ total prosthesis）のための
寛骨臼コップに関する。
【０００２】
【従来の技術】現在知られている股部のトータル補綴物
は大腿補綴部分（これはその末梢端にロッドを備え、そ
の根元端に球形頭部を備えている）と、寛骨臼コップ
（これは前記補綴部分の頭部が自由に回転できる球形空
洞を有しコッタイル（ cotyle ）として働く）とからな
る。
【０００３】これら公知の補綴物の補綴寛骨臼コップは
コップの壁を真っ直ぐに横断するねじ又はスパイクによ
り患者の寛骨に取り付けられる。
【０００４】コップを取り付けるために用いられるこれ
らのねじ又はスパイクの頭は、外科医がコップを患者の
寛骨臼に位置せしめた後に、その取り付けを容易にする
ために、コップの窪んだ内側から接近できる凹所に概ね
位置せしめられている。
【０００５】寛骨臼コップの凹状内面と補綴部分の球形
頭部との間（これらの両者は金属又はセラミック材から
作られている）の望ましくない何らの接触をも阻止する
ために、プラスチック材から作られた球形コップの挿入
体がほとんどの場合設けられる。
【０００６】この挿入体は補綴物の大腿素子の頭部と寛
骨臼コップとの間に、コップの凹状内側金属表面とそこ
に配置されている取り付けねじ又はスパイクの頭との何
らかの直接接触から前記頭部を保護するために、介在せ
しめられている。
【０００７】本件出願人が１９９０年１２月２７日付で
出願した欧州特許第０４０４６８０号に特に述べられて
いる上記構造は優れた結果をもたらした。
【０００８】
【発明が解決しようとする課題】しかしながら、経験よ
り、この補綴物を取り付けた患者が活動することでそれ
らポリエチレン製挿入体は摩耗すること、そして早期摩
耗によるポリエチレン破片はその結果として緩みの原因
となることが明らかになった。
【０００９】本発明の目的はこのような欠点を克服する
ことである。
【００１０】
【課題を解決するための手段】本発明による股部のトー
タル補綴物のための寛骨臼コップは球形キャップの形を
していて、同心状の内面と外面とを有し、これらの間に
壁を形成し、その特徴とする点はその凸状外面に前記壁
の厚みよりも短い軸線方向長さを有するねじ切りされた
盲孔を有することである。
【００１１】盲孔の各々には外ねじを切られたスパイク
が外側より圧力ばめで強固に設けられうる。スパイクは
盲孔の底に達するまで締められる。
【００１２】このようなコップは完全に滑らかな凹状内
面を有するという有利性がある。
【００１３】本発明のコップの外径と同じ外径にコッチ
ロイド（ cotyloid ）空洞を機械加工した後、ねじ切り
された全てのスパイクが取り付けられたコップが外科医
により前記空洞へ直接打ちつけられる。
【００１４】このようにして所定位置に置かれたコップ
の内面は従来のプラスチック製挿入体を受け入れること
ができるし、又大腿骨補綴部分の球形頭部が直接接触す
るようにも置かれうる。
【００１５】コップの内側に現れる孔は何ら存在しない
ので、コップの球形空洞が協働する必要のある素子の望
ましくない如何なる摩耗をも防止している。
【００１６】盲孔の軸線は好ましくはコップの軸線と交
差し、球形キャップにより規定される球の中心を通りう
る。
【００１７】盲孔が現れるオリフィスは二つの列を形成
し、コップの外面に対称的に、互い違いの配列で分布さ
れており、第一列の孔の軸線がコップの軸線となす角度
は、第二列の軸線により形成される角度とは異なってい
る。
【００１８】特定の実施例によれば、本発明の寛骨臼コ
ップは半球形でよい。
【００１９】
【発明の実施の形態】以下、添付の図面を参照して本発
明を具体的に説明する。
【００２０】本発明による寛骨臼コップは符号１で示さ
れており、内面２と同心状の外面３とを有する球形キャ
ップの形をしている。
【００２１】図示の好ましい実施例においては、コップ
１は二つの同心円上に互い違いに対称的に分布されたオ
リフィス５，５′を通して外面３へ出る二列の盲孔４，
４′を有する。
【００２２】各盲孔は内ねじ６，６′を有すると共にコ
ップの壁の厚さより小さい軸方向長さを有する。
【００２３】図面に示す如く、第一列の盲孔４の軸線が
コップの軸線に対してなす角度は約５５°であり、第二
列の盲孔の軸線がなす角度は約３０°である。
【００２４】各盲孔４，４′には各盲孔４，４′中へ外
部から力によって螺入される外ねじを有する着脱自在な
スパイク８，８′を取り付けることができる。
【００２５】植設に必要なスパイクの全てを取り付けた
本発明によるコップは外科医によって患者の寛骨臼に直
接植設することができる。
【００２６】この植設はスパイクが円形の二つの列に互
い違いに配列されていることで容易ならしめられる。
【００２７】植設されたコップは、プラスチック材料か
らなる対応する（相補的）形状の挿入体と協働しうる全
体的に滑らかな凹状の内面２を有する。
【００２８】この挿入体はベアリングとして作用して大
腿補綴部分の球形頭部が最小の摩擦で自在回転するのを
確実ならしめる。
【００２９】コップは一般に骨の材料に対して適合性を
有する金属材料、例えばチタンおよびその合金から形成
されるが、挿入体はポリエチレンまたはポリプロピレン
の如き低摩擦係数のプラスチック材料から形成するのが
好ましい。
【００３０】コップの内面には如何なる孔やコップの内
側から螺入されるねじあるいはスパイクもなく、従って
挿入体に対してこすりつけられる頭部もないので、本発
明によるコップはプラスチック材料からなる挿入体の摩
耗による破片（これは補綴物の安定性を損い、その取り
換えを必要ならしめることがある）の形成を回避するこ
とができる。

Claims (6)

    【特許請求の範囲】
  1. 【請求項１】 同心状の内面（２）と外面（３）とを有
    する球形キャップ状の、股部のトータル補綴物のための
    寛骨臼コップであって、前記内面（２）と外面（３）と
    の間に壁（７）を有するものにおいて、凸状の外面
    （３）が前記壁（７）の厚さより短い軸方向長さを有す
    るねじ切りされた盲孔（４，４′）を有することを特徴
    とする股部のトータル補綴物のための寛骨臼コップ
    （１）。
  2. 【請求項２】 ねじ切りされた盲孔（４，４′）の軸線
    はコップの軸線と交差することを特徴とする請求項１の
    寛骨臼コップ。
  3. 【請求項３】 ねじ切りされた盲孔が現れるオリフィス
    （５，５′）は二つの列を形成し、各盲孔はコップの外
    面（３）に互い違いに対称的に分布され、第一列の孔
    （４）の軸線がコップの軸線となす角度は第二列の孔
    （４′）の軸線により形成される角度とは異なっている
    ことを特徴とする請求項２の寛骨臼コップ。
