Claims (1)
- 【特許請求の範囲】 1.心肺蘇生術を遂行するときの胸部圧縮及び減圧により誘起される心肺循環を 増大させる方法において、 患者の胸部の減圧中に、負の胸腔内圧の大きさ及び持続期間を増大させて、 周囲の静脈血管系から心臓及び肺内に流入する静脈血流を増大させるため、患者 の肺内への空気流入を制限することを特徴とする方法。 2.心肺蘇生術を遂行するときの心肺循環を増大させる方法において、 患者の胸郭から血液を送り出すため患者の胸部を圧縮し、 周囲の静脈血管系から心臓及び肺への静脈血の流れを誘起するため患者の胸 部を減圧し、 患者の胸部の減圧中に、負の胸腔内圧の大きさ及び持続期間を増大させて、 周囲の静脈血管系から心臓及び肺内に流入する静脈血流を増大させるため、患者 の肺内への空気流入を制限することを特徴とする方法。 3.能動的圧縮−減圧心肺蘇生術を遂行する方法において、 胸部を圧縮するため患者の胸部に対してアプリケータ本体を押しつけ、 患者の胸部を能動的に拡大するためアプリケータを持ち上げ、 持上げステップ中に、負の胸腔内圧の大きさ及び持続期間を増大させて、周 囲の静脈血管系から心臓及び肺内に流入する静脈血流を増大させるため、患者の 肺内への空気流入を制限し、 予め選択したリズムに従って前記押しつけステップ及び持上げステップを変 化させ、心肺循環を増大させることを特徴とする方法。 4.心肺蘇生術を遂行するときの胸部圧縮及び減圧により誘起される心肺循環を 増大させる方法において、 患者の気道内に換気チューブを導入し、換気チューブは、チューブの管孔を 通る流入を制限する手段を備えているか該手段に連結され、 患者の狭角から血液を送り出すため患者の胸部を圧縮し、 周囲の静脈血管系から心臓及び肺内に静脈血が流入できるようにするため患 者の胸部を減圧し、換気チューブが患者の肺内への空気流入を制限して、患者 の胸部の減圧中に負の胸腔内圧の大きさ及び持続期間を増大させ、 換気チューブを介して患者を周期的に換気して、心肺循環を増大させること を特徴とする方法。 5.前記制限ステップは患者の気道内に換気チューブを配置することからなり、 換気チューブは、実質的に流出を制限することなくチューブの管孔を通る流入を 制限するため、換気チューブの管孔内に配置されるか該管孔と直列に連結される 流量制限オリフィスを備えているか、該オリフィスに連結されていることを特徴 とする請求の範囲第1項〜第3項のいずれか1項に記載の方法。 6.前記流入制限手段は、換気チューブの管孔内に配置されるか該管孔と直列に 、永久的に又は着脱可能に連結される圧力応答弁を備え、前記弁は前記弁は開放 するように付勢されており、胸腔内圧が0〜−100cmH2O の範囲内の限界値レ ベル以下に低下すると空気の流入を許容し、換気チューブを介して患者を周期的 に換気するステップを更に有し、患者は患者の胸部を2〜10回圧縮する度毎に 換気され、患者は患者の胸部の圧縮中又は減圧中のいずれの場合にも換気される ことを特徴とする請求の範囲第5項に記載の方法。 7.周期的換気中に患者に供給される吸引されたガスは、制限ステップ中に流入 制限手段を通るか制限ステップ中に流入制限手段をバイパスして供給されること を特徴とする請求の範囲第4項又は第6項に記載の方法。 8.胸腔内圧が5〜50cmH2O の範囲内にあるとき、患者の胸部の圧縮中に空気 が肺を出ることを制限するステップを更に有することを特徴とする請求の範囲第 1項〜第3項のいずれか1項に記載の方法。 9.前記減圧ステップは、胸部の弾性に応答して患者の胸部が拡大することを許 容すること又は患者の胸郭を拡大すべく患者の胸部を能動的に拡大させることか らなることを特徴とする請求の範囲第2項又は第4項に記載の方法。 10.前記胸部は、1回の圧縮につき約3.5 〜5cmの範囲内で圧縮され、胸部は1 分間につき60〜100回の速度で圧縮されることを特徴とする請求の範囲第3 項又は第4項に記載の方法。 11.気管内への挿入に適したチューブを備え且つガスを搬送するための少なくと も1つの第1管孔を備えた形式の改善された気管内チューブにおいて、 チューブの管孔を通るガスの流入を制限するための第1管孔内の手段を有し 、心肺蘇生術を遂行するときに、患者の胸部の減圧中に負の胸腔内圧の大きさ及 び持続期間を増大して周囲の静脈血管系から心臓及び肺に流入する静脈血を増大 させるため、患者の肺に流入する空気が一定量又は可変量に制限されることを特 徴とする気管内チューブ。 12.前記流量制御手段は、換気チューブの管孔内に配置されるか又は該管孔と直 列に連結される流量制限オリフィスを更に有することを特徴とする請求の範囲第 11項に記載の気管内チューブ。 13.前記流入制限手段はチューブの管孔内の圧力応答弁を有し、該圧力応答弁は 、胸腔内圧が0〜−100cmH2O の範囲の限界値以下に低下すると空気の流入を 許容すべく開くように付勢されていることを特徴とする請求の範囲第11項に記 載の気管内チューブ。 14.前記流入制限手段の回りに空気をバイパスさせる手段を更に有することを特 徴とする請求の範囲第11項に記載の気管内チューブ。 15.圧縮可能構造が、第1開口及び第2開口と、第1開口内に配置されるか第1 開口内に取り付けられる空気導入用一方向弁と、予め選択した体積の空気を供給 するための、第2開口に配置された手段とを有する形式の予め選択した体積の空 気を供給する改善された装置において、最低胸腔内圧を超えると患者の肺内に空 気流を供給でき、前記装置が、 前記圧縮可能構造を永久的な態様又は着脱可能な態様で患者に連結する手段 と、 最低胸腔内圧を超えるまで患者の肺へのガスの流入を制限するための、前記 圧縮可能構造の第2開口内に配置されるか第2開口内に取り付けられる流入制限 手段とを有し、心肺蘇生術を遂行するときに周囲の静脈血管系から心臓及び肺内 に流入する静脈血を増大させるため、患者の胸部の減圧中は胸腔内圧の上昇が緩 和され且つ負の胸腔内圧の大きさ及び持続期間が増大されることを特徴とする装 置。 16.前記流入制限手段は、圧縮可能構造内に配置されるか又は該圧縮可能構造と 直列に永久的な態様又は着脱可能な態様で連結される流量制限オリフィスを更 に有することを特徴とする請求の範囲第15項に記載の装置。 17.前記流入制限手段は圧力応答弁を有し、該圧力応答弁は、胸腔内圧が0〜− 100cmH2O の範囲の限界値以下に低下すると空気の流入を許容すべく開くよう に付勢されていることを特徴とする請求の範囲第15項に記載の装置。 18.前記流入制限手段は、気管内チューブの管孔内の圧力応答弁を有し、該圧力 応答弁は、胸腔内圧が0〜−100cmH2O の範囲の限界値以下に低下すると空気 の流入を許容すべく開くように付勢されていることを特徴とする請求の範囲第1 5項に記載の装置。 19.前記連結手段は顔面マスク又は喉頭マスクを更に有することを特徴とする請 求の範囲第15項に記載の装置。 20.前記連結手段は気管内チューブ又は口腔咽頭気道を更に有することを特徴と する請求の範囲第15項に記載の装置。
