プロピオン酸非ステロイド系抗炎症剤のアミノ酸塩とうっ血除去剤、去痰剤、
抗ヒスタミン剤及び鎮咳剤のうち少くとも1種を含有した組成物

本発明は、ブロピオン酸非ステロイド系抗炎症剤のあるアミノ酸塩を(a)う
っ血除去剤、(b)去痰剤、(c)抗ヒスタミン剤及び(d)鎮咳剤のうち少く
とも1種と共に含んだ組成物の安全有効量を投与することにより、かぜ、かぜ様
及び/又はインフルエンザ症状の改善された治療、管理又は緩和を施すための組
成物及び方法に関する。
発明の背景
感冒は通常重篤な病気ではないが、高度に流行性で不快な苛立つ苦難である。
“感冒”という用語は、様々な異なる呼吸器ウイルスに起因する軽度の呼吸器病
に適用される。ライノウイルスは感冒の主要な既知原因であり、成人ではかぜの
約30%に及ぶが、いくつか他のグループのウイルスも重要である。免疫反応が
起き、したがって一部の気道ウイルスによる感染はワクチンで防止できるが、す
べての可能な病原体をカバーする多型ワクチンの開発は実際的でない。このため
、急性の上部呼吸器疾患をコントロールする問題には複雑なチャレンジを伴い、
感冒に対しての画一的な治療法の長期間望まれていたものの発見は、非現実的な
期待でしかない。
初期症状は軽い倦怠感、咽喉炎及び鼻愁訴だけと最小限である。ライノウイル
ス感染の場合には、鼻水、鼻うっ血及びくしゃみの症状が通常病気の初日に始ま
り、2又は3日目までに最大重篤度に進行する。鼻症状と一緒に、ヒリヒリ、乾
燥又はチクチクしたのど、しわがれ声及び咳も起きることがある。他の症状には
目の軽い灼熱感、嗅覚及び味覚の喪失、副鼻腔又は耳の圧力又は充満感、頭痛及
び声障害もある。発熱も生じうるが、まれである。インフルエンザ感染は通常、
しばしば突然始まり数日間続いてかなり重篤な発熱;全身痛;疲労及び虚弱;胸
部不快感を伴う。
現在、対症治療のみが感冒で行われている。米国で売薬療法によりかぜを治療
する費用は、15億ドル以上の年間費用とみられる。外患クリニックでの治療の
直接費用はほぼ40億ドルとみられる。活動制限による賃金の逸失量に基づく間
接費用は、実質上もっと高い。
咳、かぜ、かぜ様及び/又はインフルエンザ症状とそれに伴う不快感、痛み、
発熱及び全身倦怠感の治療について例示される従来の処方物は、通常鎮痛剤(ア
スピリン又はアセトアミノフェン)と、1種以上の抗ヒスタミン剤、うっ血除去
剤、咳抑制剤、鎮咳剤及び去痰剤を含有している。
炎症及び付随する痛みに抗する上で非ステロイド系抗炎症剤の使用は、容認さ
れている医療行為である。非ステロイド系は、例えば滑液包炎、関節炎、頭痛等
に伴う痛み及び炎症を軽減するために通常用いられる。薬物の非麻薬系鎮痛クラ
スで最も常用される薬物の中にはアスピリン、アセトアミノフェン、イブプロフ
ェン及びナプロキセンがある。アスピリン、アセトアミノフェン及びイブプロフ
ェンは、慣用的な咳/かぜ多症状軽減組成物中の痛み軽減及び解熱成分として今
まで含有されてきた。これらの市販製品は通常、アスピリン、アセトアミノフェ
ン又はイブプロフェンに加えて、1種以上の抗ヒスタミン剤、うっ血除去剤、咳
抑制剤、鎮咳剤及び去痰剤を含有している。
痛み及び炎症から軽減させる上で有用なイブプロフェンのオルニチン、リジン
及びアルギニン塩は、例えば1981年7月21日付で発行されたBruzzeseらの
米国特許第4,279,926号明細書で開示されている。イブプロフェンリジ
ン錠剤の製造方法は、1992年3月19日付で公開されたEP第505,18
0号明細書で開示されている。S(+)形のイブプロフェンの使用は、例えば1
989年7月25日付で発行されたSunshineらの米国特許第4,851,444
号明細書で、1992年4月16日付で公開されたGates らのWO第9,205
,783号明細書では抗ヒスタミン剤と組合せて開示されている。
咳/かぜ医薬組成物の製造におけるナプロキセンと他のそれより新しい非ステ
ロイド系抗炎症剤(即ち、アスピリン、アセトアミノフェン及びフェナセチンを
除く)との使用は、例えば1985年11月12日付で発行されたSunshineらの
米国特許第4,552,899号明細書で開示されている。上部呼吸器感染を治
療する上で一部のこれら新しいNSAID単独の使用は、"Therapeutic Utility

本発明者らは、プロピオン酸NSAIDのあるアミノ酸塩を(a)うっ血除去
剤、(b)去痰剤、(c)抗ヒスタミン剤及び(d)鎮咳剤のうち少くとも1種
と共に含む選択された組成物がかぜ、かぜ様及び/又はインフルエンザ症状の改
善された治療、管理又は緩和を施すことを発見した。
したがって、本発明の目的は、治療の必要な啼乳動物生物における咳、かぜ、
かぜ様及び/又はインフルエンザ症状の治療のために、本発明の組成物をこのよ
うな生物に投与することからなる方法を提供することである。ここで用いられる
ようなこのような症状はコリーザ、鼻うっ血、副鼻腔うっ血、副鼻腔痛、上部呼
吸器感染、アレルギー性鼻炎、耳炎、副鼻腔炎等に関する。
発明の要旨
本発明は、プロピオン酸NSAIDのアミノ酸塩を(a)うっ血除去剤、(b
)去痰剤、(c)抗ヒスタミン剤及び(d)鎮咳剤のうち少くとも1種と共に含
んだ組成物の安全有効量を投与することにより、かぜ、かぜ様及び/又はインフ
ルエンザ症状の改善された治療、管理又は緩和を施すための組成物及び方法に関
する。
本明細書で用いられるすべてのパーセンテージ及び比率は、他で指摘されない
かぎり、重量による。
発明の具体的な説明
本発明は、プロピオン酸NSAIDのアミノ酸塩を(a)うっ血除去剤、(b
)去痰剤、(c)抗ヒスタミン剤及び(d)鎮咳剤のうち少くとも1種と共に含
んだ組成物の安全有効量を投与することにより、かぜ、かぜ様及び/又はインフ
ルエンザ症状の改善された治療、管理又は緩和を施すための組成物及び方法に関
する。
“アミノ酸塩”という用語は、一級、二級、三級及び四級アミンの薬学上許容
される有機無毒性塩基、天然置換アミンを含めた置換アミン、環状アミン及び塩
基性イオン交換樹脂、例えばトリエチルアミン、トリプロピルアミン、2‐ジメ
チルアミノエタノール、2‐ジエチルアミノエタノール、リジン、オルニチン、
アルギニン、ヒスチジン、カフェイン、プロカイン、N‐エチルピペリジン、ヒ
ドラバミン、コリン、ベタイン、エチレンジアミン、グルコサミン、メチルグリ
カミン、テオブロミン、プリン類、ピペラジン、ピペリジン、ポリアミン樹脂等
から誘導された塩に関する。
本発明の組成物で有用な非ステロイド系抗炎症剤のプロピオン酸誘導体は当業
者に周知であり、例えば1985年11月12日付で発行されたSunshineらの米
国特許第4,552,899号明細書で開示されており、これは参考のため本明
細書に組み込まれる。非ステロイド系抗炎症剤の化学構造、合成、副作用等の詳
細な開示については、Anti-inflammatory and Anti-Rheumatic Drugs,K.D.Rains
ford,Vol.I-III,CRC Press,Boca Raton(1985) 及びAnti-inflammatory Agents,
Chemistry and Pharmacology,I.R.A.Scherrerら,Academic Press,New York(1974
)を含めた標準テキストが参照され、それら双方とも参考のため本明細書に組み
込まれる。
本発明の組成物で有用な好ましい非ステロイド系抗炎症剤には、イブプロフェ
ン、ナプロキセン、ベノキサプロフェン、フルルビプロフェン、ケトプロフェン
、フェノプロフェン、フェンブフェン、インドプロフェン、ピルプロフェン、カ
ルプロフェン、オキサプロジン、プラノプロフェン、ミロプロフェン、チオキサ
プロフェン、スプロフェン、アルミノプロフェン及びチアプロフェニックのよう
なプロピオン酸誘導体のアミノ酸塩がある。これら非ステロイド系抗炎症剤の混
合物も用いてよい。これらのプロピオン酸NSAIDの中では、イブプロフェン
、ナプロキセン及びケトプロフェンが最も好ましい。
本発明で使用上最も好ましいのはこれらNSAID塩のS(+)異性体である
。
本発明の鎮痛剤に適用される“S(+)”という用語は、本発明のアミノ酸塩誘
導体の右旋性又はS(+)異性体を包含した意味である。用語“S(+)”と共
に用いられる“R(−)対掌体を実質上含まない”という表現は、S(+)エナ
ンチオマーが望ましい開始早期化及び高い鎮痛効果を発揮する上でそのR(−)
対掌体を実質上含まないことを意味する。現実的に言えば、これは活性成分が少
くとも90重量%のS(+)エナンチオマーと10重量%以下のR(−)エナン
チオマーを含有していることを意味する。好ましくは、S(+)エナンチオマー
対R(−)エナンチオマーの重量比は20:1以上、更に好ましくは97:3以
上である。最も好ましくは、S(+)エナンチオマーは99重量%以上であって
、R(−)エナンチオマーを含まず、即ちS対Rの重量比は大体99:1又はそ
れ以上である。
イブプロフェン、ナプロキセン、ベノキサプロフェン、フルルビプロフェン、
ケトプロフェン、フェノプロフェン、フェンブフェン、インドプロフェン、ピル
プロフェン、カルプロフェン、オキサプロジン、プラノプロフェン、ミロプロフ
ェン、チオキサプロフェン、スプロフェン、アルミノプロフェン及びチアプロフ
ェニックのアミノ酸塩の安全有効量は、通常約7.5〜約1000mgの範囲であ
り、それらの酸誘導体対応部分と通常同様である。これら薬剤の有用な投与量は
The Physicians' Desk Reference,47th Edition(1993) 及び1985年11月1
2日付で発行されたSunshineらの米国特許第4,552,899号明細書でみる
ことができ、それら双方が参考のため本明細書に組み込まれる。
例えば、本発明の組成物で用いられるイブプロフェンのアミノ酸塩の安全有効
量は、通常約50〜約800mg、好ましくは約50〜約400mg、更に好ましく
は約50〜約200mg、最も好ましくは約50〜約100mgの範囲である。本発
明の組成物で用いられるフルルビプロフェンのアミノ酸塩の安全有効量は、通常
約12.5〜約300mg、好ましくは約12.5〜約200mg、更に好ましくは
約12.5〜約100mg、最も好ましくは約12.5〜約50mgの範囲である。
本発明の組成物で用いられるケトプロフェンのアミノ酸塩の安全有効量は、通常
約5〜約100mg、好ましくは約5〜約75mg、更に好ましくは約5〜約50mg
、最も好ましくは約5〜約25mgの範囲である。通常、これら薬剤のS(+)異
性体の量はラセミ混合物の量の約半分である。
本発明の組成物は、下記クラス:(a)うっ血除去剤、(b)去痰剤、(c)
抗ヒスタミン剤及び(d)鎮咳剤から選択される少くとも1種の他の薬理活性剤
も含有している。本発明の組成物で有用なうっ血除去剤にはプソイドエフェドリ
ン、フェニルプロパノールアミン、フェニレフリン、エフェドリン、それらの薬
学上許容される塩及びそれらの混合物がある。本発明で有用な鎮咳剤にはデキス
トロメトルファン、クロフェジアノール、カルベタペンタン、カラミフェン、ノ
スカピン、ジフェンヒドラミン、コデイン、ヒドロコドン、ヒドロモルホン、ホ
ミノベン、それらの薬学上許容される塩及びそれらの混合物のようなものがある
。本発明で有用な抗ヒスタミン剤にはクロルフェニラミン、ブロムフェニラミン
、d‐クロルフェニラミン、d‐ブロムフェニラミン、トリプロリジン、アザタ
ジン、ドキシルアミン、トリペレナミン、シプロヘプタジン、ヒドロキシジン、
クレマスチン、カルビノキサミン、フェニンダミン、ブロモジフェンヒドラミン
、ピリラミン、それらの薬学上許容される塩のようなものと、アクリバスチン、
AHR‐11325、アステミゾール、アゼラスチン、セチリジン、エバスチン
、ケトチフェン、ロドキサミド、ロラチジン、レボカバスチン、メキタジン、オ
キサトミド、セタスチン、タジフィリン、テメラスチン、テルフェナジン、それ
らの薬学上許容される塩及びそれらの混合物を含めた非鎮静抗ヒスタミン剤があ
る。本発明で有用な去痰剤(粘液溶解剤としても知られる)にはグアヤコール酸
グリセリル、抱水テルピン、塩化アンモニウム、N‐アセチルシステイン、ブロ
ムヘキシン、アンブロキソール、それらの薬学上許容される塩及びそれらの混合
物が
ある。これらすべての成分とそれらの許容される投与量範囲は以下で記載されて
いる:1988年11月8日付で発行されたSunshineらの米国特許第4,783
,465号、1986年10月28日付で発行されたSunshineらの米国特許第4
,619,934号明細書;これらは参考のため本明細書に組み込まれる。
好ましくは、本発明の医薬組成物は、約200:1〜約1:1、好ましくは約
50:1〜約1:1、最も好ましくは約10:1〜約1:1の鎮痛剤:薬理活性
剤の比率で、鎮痛剤及び他の薬理活性剤を含んでいる。
錠剤、カプセル、顆粒、ロゼンジ及びバルク粉末のような固体形と、シロップ
及び懸濁液のような液体形を含めた様々な経口剤形が使用できる。これらの経口
形は安全有効量、通常少くとも約5%の活性成分を含有している。固体経口剤形
は、好ましくは約5〜約95%、更に好ましくは約10〜約95%、最も好まし
くは約25〜約95%の活性成分を含有している。液体経口剤形は、好ましくは
約1〜約50%、更に好ましくは約1〜約25%、最も好ましくは約3〜約10
%の活性成分を含有している。
錠剤は、適切な結合剤、滑沢剤、希釈剤、崩壊剤、着色剤、香味剤、保存剤及
び流動誘導剤を含有させて、圧縮、すりこみ、腸溶性コート、糖衣化、フィルム
コート又はマルチ圧縮することができる。軟質ゼラチンカプセルも有用である。
液体経口剤形には、適切な溶媒、保存剤、乳化剤、懸濁剤、希釈剤、甘味剤、
味覚遮断剤、着色剤及び香味剤を含有した、水性及び非水性溶液、エマルジョン
、偽エマルジョン、懸濁液と、非沸騰性顆粒から再調製される溶液及び/又は懸
濁液がある。経口剤形を処方するために用いられる薬学上許容されるキヤリヤ及
び賦形剤の具体例は、1975年9月2日付で発行されたRobertの米国特許第3
,903,297号明細書で記載されており、これは参考のため本明細書に組み
込まれる。固体経口剤形を作るための技術及び組成物はMarshall,"Solid Oral D
osage Forms",Modern Pharmaceutics,Vol.7(Banker and Rhodes,editors),359-
427(1979) で記載されており、これは参考のため本明細書に組み込まれる。錠剤
(圧縮及び成形)、カプセル(硬質及び軟質ゼラチン)及び丸剤を作るための技
術及び組成物はRemington's Pharmaceutical Sciences(Arthur Osol,editor),1
553-1593(1980) で記載されており、これは参考のため本明細書に組み込まれる
。
液体経口剤形を製造する上で、活性成分は慣用的調剤実務に従い水性ベースで
経口上許容される製剤キヤリヤ中に配合される。“水性ベースで経口上許容され
る製剤キヤリヤ”とは、全部又は主要溶媒分が水であるものをいう。典型的キヤ
リヤには単純な水性溶液、シロップ、分散液及び懸濁液と、水中油型のような水
性ベースエマルジョンがある。最も好ましいキヤリヤは、適切な懸濁剤を含有し
た水性ビヒクル中における医薬組成物の懸濁液である。適切な懸濁剤にはアビセ
ル(Avicel)RC‐591(FMC市販の微結晶セルロース/カルボキシメチルセ
ルロースナトリウム混合物)、グアーガム等がある。このような懸濁剤は当業者
に周知である。本発明の組成物中における水の量は活性成分の全重量及び容量と
他の任意非活性成分に応じてかなり広範囲にわたるが、最終組成物の重量に基づ
く全水分含有率は通常約20〜約75重量/容量%、好ましくは約20〜約40
%の範囲である。
水自体でキヤリヤ全体を構成してもよいが、典型的液体処方物では、組成物中
への香油等のような非水溶性成分の溶解及び配合を助けるために、共溶媒、例え
ばプロピレングリコール、グリセリン、ソルビトール溶液等を含有していること
が好ましい。したがって、一般的には、本発明の組成物は、好ましくは約5〜約
25容量/容量%、最も好ましくは約10〜約20容量/容量%の共溶媒を含有
している。
本発明の組成物は、場合により1種以上の他の公知の治療剤、特に咳/かぜ薬
で常用されるもの、例えばテルブタリン、アミノフィリン、エピネフリン、イソ
プレナリン、メタプロテレノール、ビトテロール、テオフィリン及びアルブテロ
ールのような気管支拡張剤と、アセトアミノフェン及びアスピリンのような他の
鎮痛剤を含有していてもよい。高度に好ましい任意成分はカフェインである。薬
剤師業界で周知の他の任意成分、例えば嗜好に合う快適な外観の最終製品を提供
するための天然又は人工甘味剤、香味剤、着色剤等と、貯蔵寿命を伸ばして高め
るための酸化防止剤、例えばブチル化ヒドロキシアニソール又はブチル化ヒドロ
キシトルエン、及び保存剤、例えばメチルもしくはプロピルパラベン又は安息香
酸ナトリウムも、これらの成分について通常知られる量で含有させてよい。
治療方法
投与される医薬組成物の量はその処方物内にある活性成分の%に依存しており
、1回分当たりに要求されるうっ血除去剤、咳抑制剤、去痰剤及び/又は抗ヒス
タミン剤のような任意成分とナフタレン誘導体の量、安定性、放出特性と他の薬
学的パラメーターの関数である。
通常約1〜約50mg/kg/日、好ましくは約2〜約30mg/kg/日、最も好ましく
は約3〜約20mg/kg/日の医薬組成物が本明細書で記載されたように投与される
。この量は1回で、又は好ましくは複数回(2〜6回)に分けて反復して投与し
ても、あるいは治療クールにわたり徐放的に投与してもよい。通常、本発明の医
薬組成物の各個別投与量は約1〜約25mg/kg、好ましくは約2〜約15mg/kg、
最も好ましくは約3〜約10mg/kgの範囲である。上記より多い投与量も咳、か
ぜ様、インフルエンザ及びインフルエンザ様症状を軽減する上で有効であるが、
有害な副作用を防止するためにいかなる薬物に関しても一部の個体で注意が払わ
れねばならない。
下記例は必須及び任意双方の成分が混合された本発明の態様について示してい
る。

経口投与用の硬質ゼラチンカプセル組成物は、下記成分を混合することにより
製造する:成分

経口投与用の硬質ゼラチンカプセル組成物は、下記成分を混合することにより

の活性剤を連続して加え、撹拌しながら溶解させる。次いでグリセリン及び液体
糖を加える。別の容器で、着色剤を精製水(最終バッチ容量の約0.5%)に加
える。次いでこの着色剤溶液を最初のバッチ容器に加える。別の容器で、ケトプ
ロフェンリシネートを撹拌しながらアルコールに加える。プロピレングリコール
、他の活性剤及びフレーバーをこのアルコールプレミックスに加え、得られた混
合液を均一になるまで撹拌し、その後最初の容器に加える。残りの精製水を得ら
れた混合液に加えて、撹拌する。
治療の必要なヒトへの10〜20ml(2〜4さじ)の投与で、咳、かぜ様、イ
ンフルエンザ及びインフルエンザ様症状の改善された軽減を示す。

の約0.5%)に加える。次いでこの着色剤溶液を最初のバッチ容器に加える。
別の容器で、イブプロフェンアルギニネートを撹拌しながらアルコールに加える
。プロピレングリコール及びフレーバーをこのアルコールプレミックスに加え、
得られた混合液を均一になるまで撹拌し、その後最初の容器に加える。残りの精
製水を得られた混合液に加えて、撹拌する。
治療の必要なヒトへの10〜20ml(2〜4さじ)の投与で、改善された鎮痛
及び/又は抗炎症効果を示す。
