【発明の詳細な説明】

【発明の属する技術分野】本発明は、人工補装具、例え
ば全股関節人工補装具を移植するための装置及び方法に
関する。

【従来の技術】現在の人工補装具移植技術における問題
点は、接合剤固定される移植体の周りの接合剤マントル
の位置及び厚さが適切であることを保証するのが困難で
あるということである。

【発明が解決しようとする課題】本発明は、この困難性
を解決するための手段を提供しようとするものである。

【課題を解決するための手段】本発明によれば、人工補
装具移植手術を実施するための装置は、型移植体構成要
素と、前記型移植体構成要素を骨のキャビティに対して
正確に且つ動かないように保持するための手段とを有
し、前記型構成要素は、装着されるべき挿入体構成要素
の挿入体部分と同じ寸法を持つテーパ形状の挿入体部分
を有する、装置である。

構成要素の挿入体部分よりも小さい第２移植構成要素を
提供でき、又は第２移植体構成要素を第１移植体構成要
素の代わりに提供できる。

入する工程と、接合剤を少なくとも部分的に硬化させる

挿入する工程とを有する、上述の装置を使用して人工補

【発明の実施の形態】図１は、大腿骨１５内に配置され
た、直線状のテーパを持ち且つカラーのない光沢のある
設計の理想的な主股関節骨髄内人工大腿骨補装具１を示
す。この人工補装具は、ヘッド２、ネック３、及びステ
ム４を有し、参照番号１６を附した骨接合剤で所定位置
に保持されている。大腿骨１５の骨皮質１７が海綿状骨
組織１８を或る程度保持する。ステム４を周知の種類の
中心決め装置１９即ちセントラライザーで管の中心に置
き、管を骨栓２０で塞ぐ。

何が起こるのかを示す。ステム４がセメント等の接合剤
１６とともに骨から離れ、矢印２１で示すような振り子
効果が生じる。これは、骨内に深刻な損傷をもたらし、
そのため、薄い皮層２２しか残らない。空間２３が形成
され、この空間は液体組織及び繊維状組織で満たされ
る。

は、損傷した関節を修復する移植体及び方法が示してあ
り、この方法が本発明でどのように使用できるのかを示
すため、この方法を以下に詳細に説明する。修正手順
は、図３に示すように、移植体を接合剤とともに完全に
取り出し、骨栓３０及び案内ワイヤ３１を先ず最初に装
着し、繊維質組織を取り出すことによって開始する。次
に、骨チップ３２を加え、インパクタ即ちラム３３を使
用してこれを押し固める。骨チップを欧州特許出願第０
５５５ ００４号に記載された方法で層をなして形成
し、次いで、ステム型を図５に示すように導入して骨チ
ップを押し固め、キャビティ３５を形成する。これを図
６に最もよく示す。

ひとたび接合剤が硬化すると、即ち安定した状態になる
と、型５０をキャビティから取り出す。これを行うこと
ができるようにするため、型５０は、全体に研磨された
表面を持つか或いは硬化中の骨接合剤に付着しない材料
でコーティングされており、劣化させることがない。案
内ワイヤシステムを使用した場合、案内ワイヤを骨髄内
栓３０から取り外して引っ込める。外科医は、次いで、
形成された接合剤キャビティを検査し、接合剤マントル
が不完全な特定の領域があるかどうかを確認し、又はこ
の他の欠陥があるかどうかを確認する。
【００１５】ここで、外科医には二つの選択肢がある。
形成されたマントル３６が完全に適切である場合には、
図８で参照番号３８を附した最終的な移植体構成要素を
固定するためにこれを使用できる。
【００１６】しかしながら、接合剤マントルに欠陥があ
る場合には、既に形成された接合剤キャビティ内に接合
剤３９を更に導入し、欠陥を埋め、次いで別の人工補装
具を導入する。この人工補装具４０の挿入体部分４１の
大きさは、型５０の挿入体部分５１よりも小さい。かく
して、これは、最終的な移植体として使用される。
【００１７】この技術は、キャビティ内に接合剤で固定
される最終的な挿入体の形態に応じて使用される。これ
は、型には、型を接合剤に係止した状態にする劣化特性
（retrograde feature）がないためである。
【００１８】ステムの形状は、負荷をステムとこのよう
に形成された接合剤マントルとの間で伝達するための適
切な機構を形成しなければならず、この技術の使用につ
いての理想的な股関節ステムは、エクスター股関節ステ
ム（Exeter Hip Stem ）である。この種のステムは、二
重テーパ−研磨ステム形態を有し、これは接合剤マント
ルと効果的に結合し、ステムからの圧縮による負荷を接
合剤に、そして骨に伝達する。
【００１９】この選択は、外科医が、型５０で形成した
元来の接合剤マントルを、確定された移植ステムととも
に使用することを選択する場合、重要である。必然的に
種々のステムを製作しなければならないため、マントル
と確定されたステムとの間に不整合が生じる場合があ
る。二重テーパステムを使用すると、テーパ部分が、体
温において比較的柔軟で且つ粘弾性の接合剤と再び係合
でき、そのため、製造誤差にも関わらずテーパによる負
荷を効果的に伝達できる。
【００２０】従来の技術を用いると、移植体が骨のキャ
ビティ内に不適切に位置決めされる場合があり、接合剤
マントルの厚さを均等に制御して接合剤の機械的性能を
優れたものにすることができない。これが新たな挿入方
法の特別の利点である。
【００２１】更に、本発明の方法は、開口部を成形する
ためのブローチのカニューレシステムについても使用で
きる。ブローチは、通常の大きさの移植体の上に周知の
キャビティの形状を形成するのに使用でき、更に、マン
トル形状を保証する。
【００２２】深さ表示器のシステムは、例えば、米国特
許第５，１９２，２８３号に説明された技術に示されて
いるように使用でき、更に、深さ表示システムは、この
ようなブローチで形成したキャビティ内に型挿入体を位
置決めするのにも使用できる。
【００２３】かくして、新たな方法は、器具、型挿入体
の使用を必要とし、これにより、多かれ少なかれ、接合
剤で固定された移植体の周りに所定厚さの適切で且つ完
全な接合剤マントルが形成されることが保証される。

Claims (9)

    【特許請求の範囲】
  1. 【請求項１】 人工補装具移植手術を実施するための装
    置において、型移植体構成要素、前記型移植体構成要素
    を骨のキャビティに対して正確に且つ動かないように保
    持するための手段を有し、前記型構成要素は、装着され
    るべき挿入体構成要素の挿入体部分と同じであるか或い
    はこれよりも大きい寸法を持つテーパ形状の挿入体部分
    を有する、装置。
  2. 【請求項２】 装着されるべき移植体構成要素を有し、
    この移植体構成要素は、前記型構成要素の挿入体部分と
    同じ寸法のテーパ形状の挿入体部分を持つ、請求項１に
    記載の装置。
  3. 【請求項３】 装着されるべき移植体構成要素を有し、
    この移植体構成要素は、前記型構成要素の挿入体部分よ
    りも小さい寸法のテーパ形状の挿入体部分を持つ、請求
    項１又は２に記載の装置。
  4. 【請求項４】 前記型構成要素を正確に且つ動かないよ
    うに保持するための手段は、案内ワイヤを含む、請求項
    １、２、又は３に記載の装置。
  5. 【請求項５】 前記型構成要素は、研磨仕上げが施して
    あるか或いは硬化中の骨接合剤に付着しない材料でコー
    ティングされている、請求項１乃至４のうちのいずれか
    一項に記載の装置。
  6. 【請求項６】 挿入体部分には劣化特性がない、請求項
    １乃至５のうちのいずれか一項に記載の装置。
  7. 【請求項７】 前記移植体構成要素は、股関節全交換手
    術に用いられる、請求項１乃至６のうちのいずれか一項
    に記載の装置。
  8. 【請求項８】 請求項１、２、及び４乃至７のうちのい
    ずれか一項に記載の装置を使用して人工補装具移植手術
    を実施する方法において、接合剤を用いて骨に開口部を
    形成する工程と、型構成要素の前記挿入体部分を挿入す
    る工程と、接合剤を少なくとも部分的に硬化させる工程
    と、前記型構成要素を取り出す工程と、接合剤を検査
    し、次いで挿入体部分と寸法が同じ移植体構成要素を挿
    入する工程とを有する、方法。
  9. 【請求項９】 請求項１、及び３乃至７のうちのいずれ
    か一項に記載の装置を使用して人工補装具移植手術を実
    施する方法において、骨に接合剤で開口部を形成する工
    程と、型構成要素の前記挿入体部分を挿入する工程と、
    接合剤を少なくとも部分的に硬化させる工程と、前記型
    構成要素を取り出す工程と、接合剤を検査する工程と、
    キャビティ内に更に多くの接合剤を充填する工程と、前
    記型構成要素よりも寸法が小さい挿入体部分を持つ移植
    体構成要素を挿入する工程とを有する、方法。
