Claims (1)
- 【特許請求の範囲】 1. 数個の椎骨レベル間で患者の脊椎に係合するための脊椎固定装置において 、 数個の椎骨レベルを跨いだ状態で患者の脊椎に隣接して配置されるように形状 付けられた細長い脊椎ロッドであって、その長さに沿った長手軸を有する細長い 脊椎ロッド; 椎骨固定素子であって、 椎骨係合部分と、 上記椎骨係合部分から延びた一対のポストにより形成されたロッド係合部分 であって、上記ポスト間に溝穴開口が画定され、当該各ポストが上記脊椎ロッド に係合するように形状付けられた両側の横表面を有するロッド係合部分と、 を含む椎骨固定素子;及び 上記椎骨固定素子を上記脊椎ロッドにクランプするための頂部装填クランプで あって、 上記脊椎ロッドを貫通収容するように形状付けられた細長いボアを有する本 体であって、上記細長いボアが、少なくとも初期の状態で当該脊椎ロッドを上記 ボア内で摺動できるように、該ロッドの直径より大きな長手軸に沿った長さを有 し、かつ上面を備え、この上面から延びて当該細長いボアと交差する第1のネジ ボアを具備した本体と、 上記細長いボアの長手軸に対して実質上平行に上記本体から延びた突出片で あって、上記ロッドが当該細長いボアを通って延びたときに上記脊椎ロッドの長 手軸に対して実質上平行に位置するT字状のバーで終端し、上記T字状のバーが 該細長いボアの方へ内側に向いたクランプ表面を有し、このクランプ表面が上記 椎骨固定素子の上記各ポストの上記両側の横表面の1つに係合するように形状付 けられている突出片と、 上記第1のネジボアに係合するように螺入され、上記ロッドが上記細長いボ アを通って延びたときに当該脊椎ロッドに当接するように形状件けられたチップ を有する止めネジと、 を含む頂部装填クランプ; から成り、上記脊椎ロッドが上記細長いボアを通って延びたときに、上記椎骨固 定素子の上記ポストが、上記突出片を上記溝穴開口に挿通させた状態で、上記T 字状のバーの上記クランプ表面と上記ロッドとの間に位置し; 上記止めネジが上記第1のネジボアを通して上記細長いボア内に螺入されて当 該止めネジの上記チップを上記脊椎ロッドに当接させたときに、当該脊椎ロッド 及び上記椎骨固定素子の上記ポストが上記T字状のバーの上記クランプ表面と当 該止めネジのチップとの間でクランプされる ことを特徴とする脊椎固定装置。 2. 請求の範囲第1項に記載の脊椎固定装置において、上記椎骨固定素子の上 記各ポストの上記各横表面が上記脊椎ロッドを受け入れるように形状付けられた 同軸溝を画定し;上記T字状のバーの上記クランプ表面が上記同軸溝に収容され るように湾曲していることを特徴とする脊椎固定装置。 3. 請求の範囲第1項に記載の脊椎固定装置において、上記椎骨固定素子の上 記各ポストが上記両側の横表面間の厚さを有し;上記頂部装填クランプが上記細 長いボアと上記T字状のバーの上記クランプ表面との間の長さを画定し、この長 さが当該対をなすポストの上記厚さより小さいことを特徴とする脊椎固定装置。 4. 請求の範囲第1項に記載の脊椎固定装置において、上記本体が上記上面に 対向する下面を備え、該下面から延びて上記細長いボアと交差する第2のネジボ アを有し;もって、上記止めネジが上記脊椎ロッドを当該細長いボア内でクラン プするため上記第1のネジボア又は上記第2のネジボア内に螺入できることを特 徴とする脊椎固定装置。 5. 請求の範囲第4項に記載の脊椎固定装置において、上記本体が上記ロッド ボアの長手軸を含む平面に関して対称的となっていることを特徴とする脊椎固定 装置。 6. 請求の範囲第1項に記載の脊椎固定装置において、上記第1のネジボアが 60°から90°までの角度で上記ロッドボアと交差していることを特徴とする 脊椎固定装置。 7. 請求の範囲第1項に記載の脊椎固定装置において、上記各ポストの上記両 側の横表面のうちの少なくとも1つが複数涸の半径方向のスプラインを備え; 上記頂部装填クランプが、 上記脊椎ロッドに係合するように形状付けられた第1表面、及び、当該対をな すポストの上記半径方向のスプラインに相互係合するようになった複数個の半径 方向のスプラインを具備した反対側の第2表面を有するワッシャであって、上記 T字状のバー上へのワッシャの挿入を許容するように形状付けられた貫通開口を 備えたワッシャと、 上記ロッドボアに隣接して上記ワッシャを上記本体に摺動自在に係合させる係 合手段と、 を含み; 上記止めネジが上記脊椎ロッドに当接したときに、上記ワッシャが上記椎骨固 定素子と当該脊椎ロッドとの間に位置してクランプされ、 上記対をなすポストのスプラインとワッシャのスプラインとの相互係合により 上記突出片のまわりでの当該椎骨固定素子の回転を阻止することを特徴とする脊 椎固定装置。 8. 請求の範囲第7項に記載の脊椎固定装置において、上記本体が上記ロッド ボアと上記突出片との間にハブを備え、このハブが、上記ワッシャを同ハブ上に 配置したときに、同ハブのまわりでの当該ワッシャの回転を阻止するように該ワ ッシャの上記開口に相補する形状を有し;上記ワッシャを摺動自在に係合させる 係合手段が上記ロッドボアの長手軸に平行な方向における該ワッシャの摺動を許 容するように該ワッシャを上記ハブ上に装着する装着手段を含むことを特徴とす る脊椎固定装置。 9. 請求の範囲第8項に記載の脊椎固定装置において、上記装着手段が上記開 口と一体で上記第1表面に形成された少なくとも1つの拘束くぼみと、上記ワッ シャを上記ハブ上に配置したときに、上記拘束くぼみ内に係合するように当該ハ ブから外方に突出した該ハブ上の被拘束部分とを含むことを特徴とする脊椎固定 装置。 10. 椎骨固定素子を脊椎ロッドにクランプするための頂部装填クランプにお いて、 脊椎ロッドを貫通収容するように形状付けられた細長いボアを有する本体であ っ て、上記細長いボアが、少なくとも初期の状態で上記脊椎ロッドを当該ボア内で 摺動できるように、当該ロッドの直径より大きな長手軸に沿った長さを有し、か つ上面を備え、この上面から延びて当該細長いボアと交差する第1のネジボアを 具備した本体と; 上記細長いボアの長手軸に対して実質上平行に上記本体から延びた突出片で あって、上記ロッドが当該細長いボアを通って延びたときに当該脊椎ロッドの長 手軸に対して実質上平行に位置するT字状のバーで終端し、上記T字状のバーが 該細長いボアの方へ内側に向いたクランプ表面を有し、このクランプ表面が該脊 椎ロッドと当該T字状のバーとの間に配置された椎骨固定素子の表面に係合する ように形状付けられている突出片と; 上記第1のネジボアに係合するように螺入され、上記ロッドが上記細長いボ アを通って延びたときに当該脊椎ロッドに当接するように形状付けられたチップ を有する止めネジと; から成り、上記止めネジが上記第1のネジボアを通して上記細長いボア内に螺入 されて当該止めネジの上記チップを上記脊椎ロッドに当接させたときに、当該脊 椎ロッド及び上記椎骨固定素子が上記T字状のバーの上記クランプ表面と該止め ネジの当該チップとの間でクランプされる ことを特徴とする頂部装填クランプ。 11. 請求の範囲第10項に記載の頂部装填クランプにおいて、上記T字状の バーの上記クランプ表面が上記椎骨固定素子の溝内に収容されるように凸状に湾 曲していることを特徴とする頂部装填クランプ。 12. 請求の範囲第10項に記載の頂部装填クランプにおいて、上記本体が上 記上面と反対側の下面を備え、該下面から延びて上記細長いボアと交差する第2 のネジボアを有し;もって、上記止めネジが上記脊椎ロッドを当該細長いボア内 でクランプするため上記第1のネジボア又は上記第2のネジボア内に螺入できる ことを特徴とする頂部装填クランプ。 13. 請求の範囲第12項に記載の頂部装填クランプにおいて、上記本体が上 記ロッドボアの長手軸を含む平面に関して対称的となっていることを特徴とする 頂部装填クランプ。 14. 請求の範囲第10項に記載の頂部装填クランプにおいて、上記第1のネ ジボアが60°から90°までの角度で上記ロッドボアと交差していることを特 徴とする頂部装填クランプ。 15. 請求の範囲第10項に記載の頂部装填クランプにおいて、 上記脊椎ロッドに係合するように形状付けられた第1表面、及び、上記椎骨固 定素子の半径方向のスプラインに相互係合するようになった複数個の半径方向の スプラインを具備した反対側の第2表面を有するワッシャであって、上記T字状 のバー上へのワッシャの挿入を許容するように形状付けられた貫通開口を備えた ワッシャと、 上記ロッドボアに隣接して上記ワッシャを上記本体に摺動自在に係合させる係 合手段と、 を更に含み; 上記止めネジが上記脊椎ロッドに当接したときに、上記ワッシャが上記椎骨固 定素子と当該脊椎ロッドとの間に位置してクランプされ、上記スプラインの相互 係合により上記突出片のまわりでの当該椎骨固定素子の回転を阻止することを特 徴とする頂部装填クランプ。 16. 請求の範囲第15項に記載の脊椎固定装置において、上記本体が上記ロ ッドボアと上記突出片との間にハブを備え、このハブが、上記ワッシャを同ハブ 上に配置したときに、同ハブのまわりでの当該ワッシャの回転を阻止するため該 ワッシャの上記開口に相補する形状を有し;上記係合手段が上記ロッドボアの長 手軸に平行な方向における該ワッシャの摺動を許容するように該ワッシャを上記 ハブ上に装着する装着手段を含むことを特徴とする脊椎固定装置。 17. 請求の範囲第16項に記載の脊椎固定装置において、上記装着手段が上 記開口と一体で上記第1表面に形成された少なくとも1つの拘束くぼみと、上記 ワッシャを上記ハブ上に配置したときに、上記拘束くぼみ内に係合するように当 該ハブから外方に突出した該ハブ上の被拘束部分とを含むことを特徴とする脊椎 固定装置。
