増殖因子と組織形成抑制剤を含有する組成物発明の背景
:
本発明は眼科学の分野に関する。さらに詳しくは、本発明は、白内障手術の分
野に関し、この手術はヒトの眼の本来の水晶体を外科的に除去して人工水晶体を
移植するものである。
現代の白内障手術は、典型的には、「眼内水晶体」または「IOL」と呼ばれ
る人工水晶体の、眼の後眼房への移植を含む。移植の好ましい部位は、本来の水
晶体を取り囲むカプセル内である。本来の水晶体を外科的に除去すると、水晶体
カプセルの前面の一部も除去される。これにより、水晶体カプセルの残りの部分
内に人工水晶体を据え付ける空きまができ、これはまた「カプセルバッグ」と呼
ばれる。このカプセルバッグは、眼内水晶体の移植に理想的な位置と考えられる
。残念ながら、カプセルバッグへの眼内水晶体の移植に関しては、大きな問題が
ある。
カプセルバッグは、普通は、眼の外科医によって、水晶体上皮細胞と他の組織

本発明に使用される水晶体上皮細胞剌激物質は、DNA合成の開始を剌激する

物質を、本明細書ではまとめて「水晶体上皮細胞増殖刺激物質」と呼ぶ。TGF

TGF−βには、5つの公知のアイソフォームがある。これらの型は、TGF

本発明に使用される組成物はまた、水晶体上皮細胞の増殖を抑制する1つ以上
の代謝拮抗物質を含む。この目的のために種々の代謝拮抗物質が使用される。使
用される代謝拮抗物質は一般に、プリン、ピリミジンおよび葉酸のような代謝活

の1つ以上の上述の代謝拮抗物質を含有する。この目的のために要する代謝拮抗
物質の量は、選択される特定の代謝拮抗物質に依存して変化するが、一般に約0
.01から約500マイクログラム/ミリリットル(「mcg/ml」)である
。
水晶体上皮細胞剌激物質と代謝拮抗物質の上述の組合せは、眼内投与に適切な
種々の薬剤担体に含まれてよい。担体は、好ましくは水性であり、そして眼組織
と化学的および物理的に融和性を有するように処方される。例えば、眼内手術方
法に関連して現在使用されている粘性と弾性を合わせ持つ処方(例えば、ヘアロ
ン(Healon)(登録商標)(ファルマシア社)またはビスコート(Vis
coat)(登録商標)(アルコン・サージカル社(Alcon Surgic
al,Inc.))が、上述の増殖因子と代謝拮抗物質の組合せの
担体として使用される。このような粘性の処方は凝集性であり組織に接着しやす

実である。従って、粘性溶液または半固体組成物の使用は、ある場合には好まし
い。
本来の水晶体の除去の前にカプセルバッグに増殖因子/代謝拮抗物質組合せを
注入するような他の場合には、カプセルバッグからの組成物の漏洩はあまり重大
な問題ではないため、組成物の粘性は第一の関心事ではない。従ってこのような
場合には、増殖因子/代謝拮抗物質組合せのための担体としての水溶液の使用が
好ましい。使用される水溶液は、眼内組織と融和性でなければならず、好ましく
は手術過程で眼内組織の形態と機能を維持する助けとなるべきである。上述の目
的に使用される水溶液は、平衡化生理食塩水(例えば、BSS(登録商標)平衡
化塩溶液(Balanced Salt Solution)、重炭酸、デキス
トロースおよびグルタチオンで強化したBSSプラス(BSS Plus)(登
録商標)平衡化塩溶液(Balanced Salt Solution En
riched with Bicarbonate, Dextrose an

当業者には認識されるように、上述の組成物は無菌でなければならず、敏感な
眼内組織、特に角膜上皮細胞に毒性を有するいかなる物質(例えば、抗菌性の保
存剤)も含むべきでない。上述の組成物は、当業者に公知の技術により処方され

上述の組成物は、種々の技術により水晶体カプセルに投与される。例えば、こ
の組成物は、水晶体の除去後にシリンジによりカプセルバッグの内側に投与され
るか、あるいは、水晶体超音波吸引術または他の方法により水晶体の除去の前に
カプセル水晶体中に注入される。この組成物の投与方法についての唯一決定的な
条件は、この組成物が水晶体カプセル全体に分布され、水晶体上皮細胞の細胞分
裂促進的活性化に十分な長さの時間、休眠中のこれらの細胞と接触を保つことで
ある。この目的を達成するために要する時間は、使用される特定の水晶体上皮細
胞剌激物質と代謝拮抗物質や、この水晶体上皮細胞剌激物質/代謝拮抗物質組合
せがカプセルバッグに投与される方法(すなわち、水晶体超音波吸引術の前の注
入または白内障除去後の潅注)のような情況に依存して多少変化する。しかし、
一般にこの組成物は、約5から10分間またはそれ以上、カプセルバッグの内側
と接触を保つ必要がある。上述の組成物は、従来法の灌注と吸引により除去され
てよい。
以下の実施例は、水晶体上皮細胞に及ぼす、上述の水晶体上皮細胞剌激物質と
代謝拮抗物質の効果を説明するために与えられる。
実施例1
培養したウサギ水晶体上皮細胞の細胞分裂促進能力に及ぼす、特定の代謝拮抗
物質であるマイトマイシンCの効果を試験するために検討が行われた。この実験
のために、標準的なインビトロの細胞分裂促進測定法を行った。継代数6と13
の間の、正常なウサギ水晶体上皮細胞(コリエル・インスティテュート・フォー
・メディカル・リサーチ・セル・レポジトリー(Coriell Instit
ute for Medical Research Cell Reposi
tory)から得た名称AG04676)を96ウェルの組織培養プレートに播
種した。この細胞は、約80%コンフルエントになるまで、3から
4日毎に、10%正常ウサギ血清、2mMの1−グルタチオン、および0.05
mg/mlのゲンタマイシンを補充したアールの塩(Earle’ssalts
)を含む新鮮なMEMイーグル培地を与えられた。細胞がそれらの最大細胞分裂
促進能力を有することを確信するために、この実験は最後の培地置換の3から5
日後に開始した。
無血清培地に処方された試験化合物(増殖因子混合物と代謝拮抗物質)を、3
重測定でウェル中の細胞に10分間暴露した。次に試験物質は、細胞から吸引し
て、無血清培地に置き換えられた。18時間インキュベーション後、トリチウム
化チミジンを細胞に6時間暴露した。次にトリクロロ酢酸と一緒にDNAを沈殿
させて;DNAに取り込まれなかったチミジンを洗浄により除去し、そして標識
されたDNAをドデシル硫酸ナトリウムで可溶化した。活発に分裂している細胞
のDNAに取り込まれたトリチウム化チミジンは、ベータ・シンチレーション・
カウンターを使用して定量して、3重測定の平均と標準偏差を計算した。
この特定の検討で増殖因子の混合物を使用した時に、これらの細胞の細胞分裂
促進性を最も大きく刺激した。この混合物は、30ng/mlの上皮増殖因子と
、各々20ng/mlの血小板由来増殖因子(−BBホモダイマー)、塩基性繊
維芽細胞増殖因子、およびトランスフォーミング・グロース・ファクター−アル
ファよりなる。図1に示されるように、20ng/mlのトランスフォーミング
・グロース・ファクター−ベータの添加により、増殖因子混合物の刺激活性は著
しく上昇した。増殖因子混合物の存在下で、マイトマイシンCを種々の濃度で細
胞に添加した。
図1に示される結果は、増殖因子混合物に暴露された細胞は、対照群(すなわ

Claims (1)
- 【特許請求の範囲】 1. カプセル外白内障手術後の続発性(二次性)白内障の形成を低下させる 眼用薬剤組成物であって、DNA合成の開始を剌激することにより水晶体上皮細 胞を活性化するのに十分な量の水晶体上皮細胞増殖剌激物質;水晶体上皮細胞の 増殖を抑制するのに十分な量の代謝拮抗物質;および薬剤として許容されるその 担体よりなる、上記組成物。 2. 水晶体上皮細胞増殖剌激物質は増殖因子よりなる、請求項1に記載の組 成物。 3. 増殖因子はヒト由来である、請求項2に記載の組成物。 4. 水晶体上皮細胞増殖剌激物質はTGF−βよりなる、請求項1に記載の 組成物。 5. 水晶体上皮細胞増殖剌激物質は、TGF−βを含む増殖因子の混合物よ りなる、請求項1に記載の組成物。 6. 混合物はTGF−β1よりなる、請求項5に記載の組成物。 7. 混合物は、EGF、b−FGF、TGF−αおよびPDGF−BBと組 合せたTGF−β1よりなる、請求項6に記載の組成物。 8. 代謝拮抗物質は、マイトマイシンC、5’−フルオロウラシル、アラビ ノシトシン、タキソール、アクチノマイシンC、およびメトトレキサートよりな る群から選択される、請求項1に記載の組成物。 9. 代謝拮抗物質はマイトマイシンCよりなる、請求項8に記載の組成物。 10. 組成物は、0.01から10,000ng/mlの水晶体上皮細胞増 殖剌激物質と、0.01から500mcg/mlの代謝拮抗物質よりなる、請求 項1に記載の組成物。 11. 治療的に有効量の組成物を手術時に手術部位に投与することにより、 カプセル外白内障手術後の続発性白内障の形成を低下させるための、眼用薬剤組 成物の使用であって、この組成物が、DNA合成の開始を剌激することにより水 晶体上皮細胞を活性化するのに十分な量の水晶体上皮細胞増殖剌激物質;水晶体 上皮細胞の増殖を抑制するのに十分な量の代謝拮抗物質;および薬剤として許容 されるその担体よりなる、上記使用。
