Description

初代膵島への遺伝子とsiRNAの効率的な送達に使用されてきた(79~84)。しかし、インビボでは、ウイルスベクターの使用における技術的困難に加えて、それらの固有の免疫原性および野生型ウイルスとの可能な組換えは、深刻な安全上の懸念を引き起こす。確かに、ウイルスベクターは、多臓器不全や死さえも含む可能性のある二次的合併症を伴う強力な免疫反応を誘発する可能性がある(85)。レンチウイルスベクターの出現により、免疫原性の懸念の一部が緩和されたが、レンチウイルスは、無傷のヒト膵島の形質導入においてアデノウイルスほど効率的ではない(86,87)。しかし、現在のインビトロプロトコルは最適化されている(88)。それにもかかわらず、ゲノムへのレンチウイルスの統合は、安全性の懸念、挿入型遺伝子変異のリスク、および野生型ウイルスとの組換えを依然として引き起こす。

監視されていないトグルSELEXは、マウス膵島に対するポリクローナルアプタマーライブラリーから始めて、膵島枯渇ヒト腺房細胞と4人の異なる死体ドナーからの厳選されたヒト膵島をそれぞれネガティブセレクターとポジティブセレクターとして使用して実行した。これにより、非特異的(腺房組織結合)RNAアプタマーの枯渇が可能になり、
マウスおよびヒトの膵島に特異的なアプタマーのライブラリーが充実した。
2つの種の間に存在する可能性があることを示唆している。

に示すように、競合アッセイにより、TMED6に対するアプタマー1-717の特異性が確認される。簡単に説明すると、ヒト膵臓の連続切片を、異なる濃度の組換えTMED6タンパク質の存在下でCy-3標識アプタマー1-717で染色した(すなわち、モル比アプタマー/組換えタンパク質範囲=1/1-1/10)。画像は蛍光顕微鏡によって取得され、アプタマー結合はセルプロファイラーによって定量化された。データは、組換えTMED6の添加が用量依存的にアプタマー1-717の結合を阻害することを示しており、TMED6がこのアプタマーの標的であることを強く示唆している。
アプタマー1-717およびm12-3773がインビボでヒト膵島を認識できるかどうかを評価するために、エピジダイマル脂肪パッドにヒト膵島を移植した免疫不全NSGマウスを使用した。さらに、アプタマー1-717およびアプタマーm12-3773の新しい製剤を使用する。この製剤では、各モノクローナルアプタマーがビオチン化され、ストレプトアビジンと複合体を形成して、四量体ナノ粒子(以下、テトラプタマーと呼ぶ)を形成する。この製剤は、対応する単量体アプタマーよりも優れた薬物動態と親和性を持っている。

は、3’末端をトリヌクレオチドリンカー領域(すなわちGGG)および望ましい治療用RNAのパッセンジャー鎖(パッセンジャーテール)で延長することにより、治療用RNAに容易に結合できる。次に、治療用RNAガイド鎖を70℃で等モル量の2つのRNAを混合し、混合物を室温でゆっくりと冷却することにより、修飾アプタマーに単純にアニーリングする。
た。これは、高血糖が追加の増殖シグナルを提供する可能性があることを示している(93)。これらの発見は、T1D患者の適切なβ細胞量を回復するため、および/または移植中に必要な膵島の数を減らすための新しい治療的介入の可能性を開いた。しかし、これまでのところ、これらの所見の翻訳可能性は、安定したp57Kip2サイレンシングの結果としてのウイルスベクターの使用および新生物形成に関連する安全性の懸念によって妨げられていた。実際、p57Kip2はヒトの癌で頻繁にダウンレギュレートされ(94)、その異所性発現が腫瘍性細胞の増殖を停止させるのに十分であるため、腫瘍抑制遺伝子として提案されている(94)。しかし、アプタマー送達を介したp57kip2の時間的に制御された調節は、時間的に制御された方法でβ細胞増殖を増加させるために糖尿病において重要である可能性がある。これは、安定したサイレンシングをもたらす可能性のある、制御不能な腫瘍性のようなβ細胞の増殖に関する安全性の懸念を回避しながら、時限投与中にβ細胞量を増加させるのに十分である可能性がある。
アポトーシス細胞死は、あらゆる形態の糖尿病におけるインシュリン産生β細胞の喪失の特徴である(99~101)。白血球の浸潤と活性化、および膵島内の高血糖は、炎症性サイトカイン、ケモカイン、活性酸素種などのアポトーシストリガーの局所濃度を高くする(99)。これらのアポトーシス経路のほとんどは、カスパーゼ(CASP)3および7の活性化に収束し、遺伝子の再プログラミング、ホスファチジルセリンの反転、およびアポトーシス小体の形成をもたらす(102)。
AS9システムのgRNAに匹敵するため、DNAエレメントを挿入せずに安全で特異的かつ一時的な遺伝子活性化を提供するが、不可逆的なDNA修飾は誘導されない126。治療用saRNAは癌治療のために調査されているが、私たちの知る限り、T1Dでの研究は行われていない(127~130)。
ラ(CTRLキメラ)またはXIAP-saRNA/アプタマーキメラ(Xiapキメラ)で処理され、後でサイトカイン(CTK)でチャレンジされた、または未処理(CTKなし)のままにされた膵島の単一細胞懸濁液のフローサイトメトリー分析を示す。図16Cは、アプタマーm12-3773またはアプタマー1-717のいずれかで生成されたキメラを使用した、異なる死体ドナーからの膵島を使用した5つの独立した実験からのスパゲッティプロットである。対応のあるT検定値が報告される。データは、Xiap-saRNAアプタマーキメラがベータ細胞をサイトカイン誘導アポトーシスから保護することを示す。
死体ドナーからのヒト膵島は、図17Aに詳述されているようにXiap-saRNAアプタマーキメラまたは対照キメラでトランスフェクトされた。トランスフェクションの24時間後、膵島は免疫不全NSGマウスの前眼房に移植された。膵島細胞のアポトーシスは、インビボアネキシンV(ANXA5)染色およびインビボ顕微鏡検査によって縦方向に評価された。データは、移植前のXiap-saRNA/アプタマーキメラによる治療がアポトーシス(ANXA5)を劇的に減少させ、したがって移植片の細胞喪失を減少させることを示している。
組織特異的耐性の維持におけるPDL1発現の臨床的重要性は、癌におけるPDL1-PD1アンタゴニストの成功によって強調されている(135)。PDL1によるPD1の関与は、エフェクターT細胞の増殖と活性化をダウンレギュレートし、T細胞周期の停止とアポトーシスを誘導し、IL10産生Tregを促進する(136~139)。興味深いことに、新たな副作用の抗PD1治療の1つはT1D140である。これは、PDL1/PD1がβ細胞に対するT細胞の耐性を制御する上で重要な役割を果たしている可能性があることを示唆している。実際、NODマウスでは、PDL1遮断が雌マウスのT1Dを加速し、雄でそれを誘発する(13)。逆に、同系移植された膵島におけるPDL1異所性発現は、NODマウスをT1D再発から保護する(12,13)。インシュリンプロモーターの制御下でPDL1を発現するNODトランスジェニックマウスは、糖尿病の発生率の遅延、T1D発生率の低下、および全身性の膵島特異的T細胞アネルギーを示す(141)。ヒトでは、PDL1多型はT1Dと関連する(OR=1.44)(142)。
ことに、アルファ細胞では変化は観察されず、このアプタマーのベータ細胞への優先的結合が間接的に確認された。これらの原理実証データは、アプタマーを効果的に使用して、機能的なPDL1-saRNAをインビトロでヒトβ細胞に送達できることを示している。
最初の一連の実験では、NSGマウスにACEにヒト膵島を移植する。移植の3週間後、マウスはXiapsaRNA-アプタマーキメラまたは対照キメラで治療される。さまざまな時点で、IL1β、TNF-α、およびIFNγの眼内注射によってヒト膵島移植片にチャレンジし、カスパーゼ3および7の活性化を介してβ細胞にアポトーシスを誘導する。カスパーゼ3および7の活性は、CASP3/7 Green Detection Reagentを使用した眼内イメージングシステムによってインビボで評価される。この細胞透過性試薬は、核酸結合色素に結合した4個のアミノ酸ペプチド(DEVD)で構成されている。活性化されると、カスパーゼ3および7はプローブを切断し、色素をDNAに結合させ、ACE28の細胞レベルで検出できる明るい蛍光発生シグナルを放出する。さらに、抗アネキシンV抗体によるインビボ染色を使用して、インビボで膵島細胞のアポトーシスを直接測定する(図7)。
出し方法(qPCR、ウエスタンブロット、および酵素アッセイ)を使用し、3つの異なる死体ドナーからのヒト膵島を使用して各実験に対して少なくとも3つの独立した繰り返しを実行する。移植研究では、群あたり合計9匹のマウス(各繰り返しで3匹)を使用して、90%のパワー(ANOVA、α=0.05)を確保し、対照に対する1.6SDの差を検出する。
最初の一連の実験では、EFPに500IEQのヒト膵島を移植したNSGマウスを、p57kip2siRNAまたはネガティブ対照としてスクランブルsiRNA(対照キメラ)と結合した膵島特異的アプタマーの異なる用量(6、20、および60pmoles/g)でi.v.またはs.c.で処置する。ポジティブ対照と同じp57kip2shRNAトランスフェクト膵島をコードするアデノウイルスを使用する(14)。事前定義された時点(例えば、投与後1、2、3、4、および5日目)で、移植片が採取され、p57kip2の発現がi)レーザーで捕捉された膵島でのqRT-PCR、およびii)定量的コンピューター支援免疫蛍光分析によって定量化される(95)。どちらの手法も、糖尿病研究所(96,97)とPIs95の研究所で最適化されている。用量依存的な毒性の可能性を評価するために、血清および関心のある臓器(脾臓、肝臓、リンパ節、肺、腎臓、および脳)が収集され、組織病理学的評価のためにマイアミ大学のマウス病理学研究所に送られる。
最適な投与量と投与経路が特定され、p57kip2サイレンシングの動態が評価されたら、実質的な変化(すなわち、≧100%増加)を誘発するために必要なp57kip2siRNA-アプタマーキメラの投与回数を決定する。p57kip2サイレンシングは高血糖マウスでのみβ細胞増殖を誘導することが示されたため(14)、辺縁下のヒト膵島塊(250IEQ)がNSGマウスのEFPまたはACEに移植される。移植の21日後、マウスはストレプトゾトシン(STZ)治療により高血糖になる。ヒトβ細胞はかなり耐性があるため、STZはマウス膵島を選択的に排除する(98)。マウスが高血糖になると(通常、治療後5~6日)、マウスは膵島特異的または対照アプタマーキメラの1、2、3、または4回の投与を受ける。アプタマー投与の頻度は、実施例5で確立された時間経過に基づいて決定される。BrdUは、エクスビボでの増殖測定のために飲料水中で投与される。EPF群のβ細胞量は、IVISによって縦方向(ベースライン、治療中、最後の治療から5日後および10日後)に評価される(図2)。ACE群では、膵島の質量は、膵島の体積のインビボイメージングと定量分析によって評価される(28)。最後の処理から10日後、移植片を採取し、免疫染色によって分析して、(i)BrdUおよびKi76染色によるβ細胞増殖、および(ii)α細胞とβ細胞の比率を決定する。
マウスの正常血糖を回復できるかどうかを判断する
この実施例の目的は、アプタマーを介したp57kip2サイレンシングが、最適以下の数のヒト膵島を移植された糖尿病マウスにおいて正常血糖を回復できるかどうかを試験することである。

- 治療用RNAに結合したアプタマーを含む構築物であって、ここで、前記アプタマーが、ランゲルハンス島で発現するクラステリン(配列番号414)に特異的であるか、またはランゲルハンス島で発現する「膜貫通emp24ドメイン含有タンパク質6」(TMED6、Genbankアクセッション番号NM14476.1)に特異的である、構築物。
- 前記クラステリンに特異的であるアプタマーが、配列番号264に記載のヌクレオチド配列を含み、前記TMED6に特異的であるアプタマーが、配列番号259に記載のヌクレオチド配列を含む、請求項3に記載の構築物。
- 請求項3または4に記載の構築物を含む、ベータ細胞の増殖を調節するための医薬組成物。
- 請求項3または4に記載の構築物を含む、組織移植片のアポトーシスの阻害を必要とする対象における前記組織移植片のアポトーシスを阻害するための医薬組成物であって、前記組織移植片が、クラステリンまたはTMED6を発現している組織移植片である、医薬組成物。
- 請求項3または4に記載の構築物を含む、ベータ細胞に対するT細胞の細胞毒性から前記ベータ細胞を保護するための医薬組成物。
- 請求項3または4に記載の構築物を含む、糖尿病の治療を必要とする対象における糖尿病を治療するための医薬組成物。
