Description
以下の実施例は、当業者に、方法を実行し、本明細書に開示および請求されたプローブを使用する方法の完全な開示および説明を提供するために提示されている。数値(量、温度など)に関して正確さを確保するための努力がなされてきたが、いくつかの誤差と偏差を考慮する必要がある。特に明記されていない限り、パーツは重量部であり、温度は℃で、圧力は大気圧またはその近傍である。標準の温度と圧力は、20℃および1気圧として定義されている。
「抗体」という用語は、標的抗原に選択的に結合する天然または合成の抗体を指す。この用語には、ポリクローナル抗体とモノクローナル抗体が含まれる。無傷の免疫グロブリン分子に加えて、「抗体」という用語には、それらの免疫グロブリン分子のフラグメントまたはポリマー、ならびに標的抗原に選択的に結合する免疫グロブリン分子のヒトバージョンまたはヒト化されたものも含まれる。
本明細書で使用される「特異的に結合する」という用語、例えば、抗体を指す場合、タンパク質および他の生物製剤の不均一な集団におけるタンパク質の存在を決定する結合反応を指す。したがって、指定された条件下(例えば、抗体の場合の免疫アッセイ条件下)では、特定のリガンドまたは抗体は、サンプルに存在する他のタンパク質、またはリガンドまたは抗体が生体内で接触する可能性のある他のタンパク質に対して有意な量で結合しない場合、その特定の「標的」(例えば、抗体はOPNに特異的に結合する)に「特異的に結合する」。一般に、分子を「特異的に結合」する最初の分子の解離定数(Kd)は、10-6M未満、10-7M未満、10-8M未満、10-9未満、10-10未満、10-11M未満、10-12M未満、10-13M未満、10-14M未満、または10-15M未満である。「親和性」は、結合の強さを指し、結合親和性の増加は、より低いKdと相関している。
開示された組成物、システム、キット、および方法は、外傷性脳損傷(TBI)を有する対象を診断および治療するために使用することができる。TBIは、閉鎖性頭部外傷または貫通性頭部外傷から生じる可能性がある。閉鎖性頭部外傷は、頭が突然激しく物体に当たったが、物体が頭蓋骨を突き破らない場合に発生する。穿通性外傷は、物体が頭蓋骨を貫通して脳組織に入るときに発生する。頭蓋骨骨折は、頭蓋骨の骨にひびが入ったり破壊されたりしたときに発生する。頭蓋骨の陥没骨折は、壊れた頭蓋骨の破片が脳の組織に押し込まれると発生する。貫通性頭蓋骨骨折は、弾丸などの何かが頭蓋骨を貫通し、脳組織に明確で局所的な損傷を残すときに発生する。頭蓋骨骨折は脳挫傷を引き起こす可能性がある。
オステオポンチン(OPN)は、当該技術分野で公知の技術を使用して生物学的サンプルから検出することができる。いくつかの実施形態では、生物学的サンプルは、血液、血漿、血清、尿、痰、または汗であり、したがって、髄液用の脊椎穿刺などの痛みを伴う危険な収集方法の必要性を排除する。
OPNのみのROC曲線の下の領域(AUC)、および受信者動作特性(ROC)を使用してマーカーの他のパネルと組み合わせたOPNは、いくつかの実施形態では、TBIの診断および層別化におけるOPNの推測効用および精度を高めるために使用できる。
開示された方法は、対象のTBIを診断するために使用することができる。したがって、本明細書にも開示されているのは、開示された方法によって診断された対象を監視および/または治療する方法である。また、与えられた治療の効果を監視するために使用することもできる。
包装材料およびELISAアッセイに適した抗オステオポンチン抗体を含む製品も本明細書に開示される。キットはさらに、GFAP、UCH-L1、S-110、炎症性サイトカイン、またはそれらの組み合わせに選択的に結合する抗体を含むことができる。キットには、抗アクチン抗体などのローディングコントロール抗体をさらに含めることができる。キットには、検量線を作成するための標準、例えば設定された濃度のOPNタンパク質をさらに含めることができる。包装材料は、抗体を使用して外傷性脳損傷を診断する方法を示すラベルまたは添付文書を含むことができる。
実施例1:血漿オステオポンチンは、神経炎症および小児外傷性脳損傷の重症度を推測する可能性がある。
外傷性脳損傷のCCIモデルのための制御された皮質衝撃モデル(CCI)は、電気磁気装置(Impact One、ライカ バイオシステムズ)を使用して幼若(4週齢)の雄C57BL/6またはCX3CR1GFP/+マウスで確立された(Osier ND, et al. Front Neurol 2016 7:134)。簡単に説明すると、マウスを2%イソフルランで麻酔し、手術中は体温を37°Cに維持した。頭蓋骨を露出させるために中央切開を行った。ラムダとブレグマの中心部では、脳の左側に直径4mm、正中線から0.5mm離れた円形の開頭術を行い、無傷の硬膜を露出させた。CCIパラメータは、3m/sでの速度、500msでの圧縮時間、および2mmでの変形深さであった。衝撃後の血液を清浄にし、組織接着剤で傷口を閉じた。シャム群の場合、開頭術は無傷の硬膜で行われた。手術後、麻酔から回復するまでマウスの体温を37℃に維持した。CCIの後、マウスを毎日検査し、神経学的重症度スコア(NSS)を評価した(Shapira Y, et al. Crit Care Med 1988 16: 258-265)。このスコアリングシステムでは、中央に30分間置いたときに直径50cmの円から抜け出せない、仰向けにしたときに立ち直り反射が失われる、シーク行動が失われる、片麻痺または片麻痺がそれぞれ1ポイントにスコアリングされた。CCIの48時間後のNSS値2以下は、低NSSに分類された。NSS値3~4は高NSSに分類された。すべての手順は、施設内動物管理使用委員会(IACUC)および国立衛生研究所の実験動物の管理および使用に関するガイドによって承認された。
対象は0歳から18歳までの患者で、医療専門家によるTBIの診断を受けて、アトランタ小児医療(CHOA)スコティッシュライト病院とエグレストン病院の救急科(ED)に搬送された。グラスゴー昏睡スコア(GCS)のすべてのレベルが適格であり、患者は軽度のTBI(GCS13~15)、中等度のTBI(9~12)、または重度のTBI(GCS3~8)に分類された。除外基準は、年齢パラメータ外の子ども、または非外傷性脳損傷または他のタイプの医学的疾患を持っていた子どもであった。プロトコルは、CHOAの協会レビュー委員会(IRB)によって承認された。インフォームドコンセントを両親から得た。
次の抗体と作業希釈液を免疫蛍光染色に使用した:ヤギ抗OPN(1:100、R&Dシステムズ社)、抗ヤギ588(1:500、バイオレジェンド社)、ラット抗F4/0(1:100、Bio-Rad社)、ビオチン化ラットIgG(1:250、バイオレジェンド社)、Alexa fluor647ストレプトアビジン(1:500、バイオレジェンド社)。次の抗体とワーキング希釈液をイムノブロッティングに使用した:ヤギ抗OPN(1:1000、R&Dシステムズ社)、ヤギ抗MMP9(1:1000,シグマ社)、ウサギ抗GFAP(1:1000、Abcam社)、ウサギ抗アクチン(1:10000、シグマ社)、ウサギ抗トランスフェリン(1:10000、シグマ社)。0.2μgのマウス組換えOPN(R&Dシステムズ社)を陽性対照としてイムノブロットに使用した。
抗凝固剤としてヘパリンを使用してマウス血漿を収集し、収集後30分以内に2~8℃で1000×gで20分間遠心分離した。血漿は、Luminexキット(R&Dシステムズ社)のバッファーで50倍に希釈された。LuminexビーズベースのELISAは、Bio-Plexシステム(Bio-Rad社)を使用してアッセイおよび分析された。
ヒト血漿を適切に希釈し、市販のELISAキット(R&Dシステムズ社)を使用して、OPNおよびGFAPレベルを各サンプルについて2回ずつ分析した。キャリブレータ範囲外のサンプルはすべて再希釈し、再度アッセイした。
すべてのデータは、GraphPadプリズム7分析ソフトウェアを使用して分析された。2つの群の間のOPNまたはGFAPのレベルは、マンホイットニー検定を使用して比較され、p値<0.05は統計的に有意であると見なされた。OPNまたはGFAPと短期的な結果(換気装置の日数およびICU内の日数)との相関は、スピアマンの順位相関検定を使用して分析された。OPNまたはGFAPの予後値は、前述のように(Swets JA. Science 1988 240:1285-1293)、受信者動作特性(ROC)曲線を使用して分析された。p値<0.05およびAUC0.7~0.8は、診断における適切な精度として評価された。
OPN、GFAP、およびMMP-9レベルは、TBI後のOPN誘導の可能性を調べるために、制御された皮質衝撃(CCI)モデルの脳と血液で比較された。CCIは、最初に生後1か月のオスのCX3CR1GFP/+マウスに適用された。このマウスでは、ミクログリアと単球/マクロファージが緑色蛍光タンパク質(GFP)でタグ付けされている(Osier ND, et al. Front Neurol 2016 7:134)。イムノブロットは、回復時間48時間で神経学的重症度スコア(NSS)について低い(2以下)または高い(3~4)のいずれかを示したCCI損傷マウスの同側半球において、OPNおよびMMP-9、および程度は低いがGFAPの顕著な誘導を示した。(Shapira Y、etal。CritCareMed 1988 16:258-265)。重要なことに、反対側の半球ではOPNまたはMMP-9の誘導はなかった(図1B、各グループでn>6)。同様に、免疫染色は、CCI損傷CX3CR1GFP/+マウスの対側半球ではなく、同側半球でOPNの選択的誘導を示した(図1C,1D;n>3)。さらに、抗OPN免疫シグナルは、GFAP陽性アストロサイトではなく、GFPおよびF4/80二重陽性活性化ミクログリア/マクロファージに主に局在していた(図1C,1Dの矢印)。これらのデータは、脳のOPNとMMP-9およびGFAPの増加がすべて幼若マウスのCCIについてのマーカーであることを示唆している。
この仮説の検証を開始するために、入院時の頭蓋内病変のCTの証拠のある場合とない場合がある、66人のTBI損傷児において、血漿OPNおよびGFAPレベルを比較した。コホートには、50例の重度のTBI(GCS:3~8;7.3±0.7歳)、5例の中等度のTBI(GCS:9~12;5.9±3.1歳)、および11例の軽度のTBI症例(GCS:13~15;7.2±1.4歳)を含む(表1)。この分析は、OPNの初期入院時の血漿レベルが、頭蓋内病変のある子ども(n=46、うち1例の重度のTBI症例ではCTスキャンを受けなかった)で、CT所見が陰性の子ども(n=19)よりも有意に高いことを示した(図3A、t検定によるp=0.006)。対照的に、初期血漿GFAPレベルは、入院時の頭蓋内病変のCTの形跡がある場合とない場合で同等であり(p=0.07)、最近の報告(Mondello S, et al. ScienceReport2016 6:28203)の所見と一致している。さらに、初期血漿OPNレベルは、軽度TBIの子どもよりも重度TBIの子どもで高かったが(図3B、t検定によるp=0.02)、重度TBI群と軽度TBI群の間の血漿GFAPレベルの有意差はなかった(p=0.75)。受信者動作特性(ROC)グラフ分析では、入院時の重度のTBI(GCS:3~8)を推測するために、血漿GFAPレベルと比較して、初期血漿OPNレベルを使用する方が精度が高いことも示された(図3B)。これらのデータは、小児TBIの急性期において血漿OPNがGFAPよりも優れた診断バイオマーカーであることを示唆している。
TBIの現在の管理は、重症度を推測し、脳損傷の進行を監視するために、X線画像と神経学的検査のみに依存している。臨床的重症度およびTBI脳損傷の進展と相関する血液ベースのバイオマーカーテストにより、急性期治療、合併症の早期介入、およびフォローアップリハビリテーション計画における適切なトリアージが可能になる(Au AK, et al. Curr Opin Neurol 2017 30:565-572; Adrian H, et al. eNeuro 2016 e0294-16 2016 1-13 )。TBIに関連する救急科の訪問率が年齢層別に最も高いのは、0~4歳、5~14歳、および15~24歳であり、25~44歳の群の発生率よりも2~4倍 高いため、TBI血液バイオマーカーを開発する必要性は特に子どもにとって緊急である。さらに、GFAPなどの成人TBIで有用性が証明されている血液バイオマーカーは、小児には適用できない可能性がある(Okonkwo DO, et al. J Neurotrauma 2013 30:1490-1497; Mondello S, et al. Science Report 2016 6:28203)。
結論として、小児TBIの血液バイオマーカーとしてのOPNのメリットは、神経炎症時のミクログリア/マクロファージの強力な誘導と、生体液中における非常に効率的な輸送と安定性とから生じ得る。これらの固有の属性を備えたOPNは、小児TBIにおける有望な血液バイオマーカーである。
<目的>
子どもの虐待的頭部外傷(AHT)は、重大な罹患率と死亡率に関連しているが、有意な研究がなされていない。さらに、これらのケースは、特にこれらの子どもたちを永続的な敵対的な環境に戻すことの潜在的に悲惨な結果を考えると、診断上の課題である。したがって、AHTの診断を迅速に、具体的かつ客観的に行うための緊急かつ未だ需要を満たしていない必要性がある。現在まで、虐待による頭部外傷を検出し、回復の道筋を決定し、管理計画の策定を支援するために十分な感度を備えたバイオマーカーはない。この研究は、AHTをTBIの他のメカニズムから区別するOPNレベルの能力を評価することを目的としている。この研究では、AHTとリハビリテーションの結果との関係をさらに調査している。
79人の小児TBI患者(0~4歳)であり、このうち24人にAHTが確認され、55人が偶発的な外傷を負っていた。このうち、59人が入院患者のリハビリテーションを完了した。OPNレベルを測定するために、入院から6時間以内、24時間、48時間、72時間に採血した。WeeFIMの評価は、入院時および入院患者のリハビリテーションからの退院時に収集された。
時間の経過に伴うOPNの平均値を図5と表2に示す。グループ間でグラスゴー昏睡スコア(GCS)に違いはない(6.25対6.56)。AHT群は相対的に若かった(平均年齢0.65歳対2.36歳)。偶発的な外傷の群と比較して、AHTでは入院時(p=0.008)および72時間(p=0.044)により高いOPNレベルが観察された。AHT群は、WeeFIMスコア(t(30)=2.406、p=.02)で測定した場合、入院のリハビリテーション中に改善が少なかった(平均WeeFIM変化4.73対18.48)。
OPNは、虐待的な頭部外傷の診断をサポートするための客観的な指標として役立つ可能性がある。さらに、AHTのより正確な診断は、理学療法士が回復パターンをよりよく推測し、生存者の間で治療の決定を知らせるのに役立つ可能性がある。
ProTECT III に登録された中等度から重度のTBI患者のサブセットにおける血清OPNレベル:血清OPNレベルは、中等度(n=10)、中等度から重度(n=11)、または重度のTBI(n=9)の30人の登録患者で比較された。TBI後24時間および48時間で、中等度の群と比較して、中等度から重度および重度の群でOPNレベルが増加した(図6Aおよび6B)。初期血清OPNレベル(65.70±45.00)も、健康な被験者の報告された平均血漿OPNレベル(23.56±19.73ng/mL)よりも高いことも観察された。
本開示に含まれる技術的思想を以下に記載する。

前記参照値の少なくとも1.5倍のOPN濃度は、対象がAHTを有することを示している、方法。

前記第2の生物学的サンプル中のOPN濃度が、前記第1の生物学的サンプルのOPN濃度より少なくとも30%高いことは、対象がAHTを有することを示している、方法。

Claims (12)
- 頭部外傷を有する対象の診断または予後を与えるための方法において、
(a)被験者からの生物学的サンプルを提供する工程と、
(b)前記生物学的サンプル中のオステオポンチン(OPN)の濃度を決定する工程と、
(c)決定されたOPN濃度を少なくとも1つの参照値と比較する工程とを備え、
前記少なくとも1つの参照値が、患者群の生物学的サンプルからの受信者動作特性曲線(ROC)分析によって決定されるOPNカットオフ値であり、前記OPNカットオフ値が、対照対象の群の対照サンプルの値の少なくとも2倍であり、
OPN値の上昇は、前記対象が外傷性脳損傷(TBI)を有することを示す、方法。 - 前記対象が小児の対象である、請求項1に記載の方法。
- 前記対象が成人の対象である、請求項1に記載の方法。
- 前記少なくとも1つの参照値が、前記対照対象の群の前記対照サンプルの中央値OPN濃度である、請求項1に記載の方法。
- 前記OPNカットオフ値が100ng/ml血漿よりも高い、請求項1に記載の方法。
- 前記サンプル中のGFAP、UCH-L1、S-110、炎症性サイトカイン、またはそれらの組み合わせの濃度を決定する工程をさらに有する、請求項1に記載の方法。
- 前記サンプルが、血液、血漿、血清、尿、痰、および汗からなる群から選択される、請求項1に記載の方法。
- 前記サンプルが血漿または血清である、請求項7に記載の方法。
- 前記OPN濃度の決定は、免疫検出法を使用して実施される、請求項1に記載の方法。
- 頭部損傷を有する対象のOPN濃度が、前記少なくとも1つの参照値と比較して上昇し、さらに対象をTBIのために治療する工程をさらに有する、請求項1に記載の方法。
- 小児の前記対象における上昇したOPN値が、前記対象が虐待による頭部外傷(AHT)を有することを示す、請求項1に記載の方法。
- 前記OPN値がTBIの重症度を推測するために使用される、請求項1に記載の方法。
