Description

一実施形態において、システムは、第1内視鏡のワーキングチャネル内に配置されたニードルをさらに含み、ニードルは、内部を通って延びる管腔を備え、かつ第1標的部位及び第2標的部位の壁を貫通して、消化器官の第2区域内に配置されたニードルの遠位端部がループ内に配置されるまで進められるように構成されており、管腔は第1ガイドワイヤを受容するように構成されている。
一実施形態において、上記の方法は、スネア装置を光学スコープ装置の管腔を通じて、スネア装置の遠位端部が管腔の遠位端部を過ぎて遠位に延びるまで挿入するステップをさらに含み、スネア装置は拡張形態と引き締め形態との間を移行可能に構成された遠位ループを含む。
一実施形態において、上記の方法は、内部を通って延びるワーキングチャネルを備えた第2内視鏡を第1区域に進めるステップと、第2ガイドワイヤを第2ガイドワイヤの遠位端部が第1標的部位に配置されるまで第2内視鏡のワーキングチャネル内に通すステップと、第2内視鏡を患者から抜去するステップと、光学スコープ装置を第2ガイドワイヤ上において第1標的部位まで進めるステップと、第2ガイドワイヤを患者から抜去するステップとをさらに含む。
Claims (13)
- 消化器官の第1区域と第2区域との間に吻合を形成するためのシステムにおいて、
内部を通って延びるワーキングチャネルを有するとともに前記第1区域の標的部位である第1標的部位に延びるように構成されている第1長尺状部材と、
内部を通って延びるワーキングチャネルを有するとともに前記第2区域の標的部位である第2標的部位に配置されるように構成されている第2長尺状部材と、
前記第1長尺状部材のワーキングチャネルを通って延びるとともにループを備えた遠位端部と前記第1区域内での位置決めをするための近位端部とを有する第1可撓性長尺状部材と、
前記第2長尺状部材のワーキングチャネルを通って延びるとともに遠位端部に前記第2区域から前記第1区域に延びるように構成された結合要素を含む第1可撓性要素と、を備え、
前記第1可撓性要素の遠位端部に位置する前記結合要素は前記第1可撓性長尺状部材の遠位端部に位置する前記ループを通過して前記第1可撓性要素を前記第1可撓性長尺状部材に結合し、前記第1区域と前記第2区域とを接続するように構成されており、
前記第1可撓性要素の遠位端部に位置する前記結合要素は、該結合要素が前記第1区域内に位置する前記ループを通過するような大きさにされる第1構成と、前記結合要素がループを通過するのを防止して、前記第1区域において前記第1可撓性要素の結合要素を前記第1可撓性長尺状部材の遠位端部のループに対して取り外し可能にロックして、前記消化器官の第1区域と第2区域との間に接続レールを形成するように構成された第2構成との間を移行可能である、システム。 - 前記第1標的部位及び第2標的部位の壁を貫通して延在するように構成されているステントをさらに備える、請求項1に記載のシステム。
- 前記ステントは、本体部分、近位フランジ及び遠位フランジを備え、前記本体部分は、内部を通って延びる管腔を備え、該管腔が前記第1区域と前記第2区域との間に流体連通を提供するように構成されている、請求項2に記載のシステム。
- ハンドルに結合された近位端部から前記第2区域の標的部位に配置可能な遠位端部まで延びるカテーテルを含むステント送達システムをさらに備え、前記カテーテルは内部に前記ステントを受容するように構成されたワーキングチャネルを含んでおり、前記ステント送達システムは、前記ステントを標的吻合部位内に配置するように構成されている、請求項2に記載のシステム。
- 前記第1可撓性長尺状部材の遠位端部は前記第1区域の標的部位に延びるように構成されており、
前記第1可撓性要素は前記第2区域の標的部位に延びるとともに前記第2区域を通過して前記第1区域に延びて前記第1可撓性長尺状部材のループに結合されるように構成されている、請求項1に記載のシステム。 - 前記第1区域は小腸内にある、請求項1に記載のシステム。
- 前記第2区域は胃内にある、請求項1に記載のシステム。
- 前記第2長尺状部材のワーキングチャネル内に配置されたニードルをさらに備え、該ニードルは、内部を通って延びる管腔を含み、かつ前記第1標的部位及び第2標的部位の壁を貫通して、前記消化器官の前記第2区域内に配置された前記ニードルの遠位端部が前記ループ内に配置されるまで進められるように構成されており、前記管腔は前記第1可撓性要素を受容するように構成されている、請求項1に記載のシステム。
- 前記第1可撓性要素はガイドワイヤであり、前記結合要素は、前記第1構成において、前記結合要素が前記ループを通過することを避けるべく前記ループの直径より大きな直径を有する拡張可能要素である、請求項1に記載のシステム。
- 前記第1長尺状部材は光学スコープ装置である、請求項1に記載のシステム。
- 内部を通って延びるワーキングチャネルを有するとともに前記第1区域の標的部位に延びるように構成されている内視鏡をさらに備え、前記光学スコープ装置は前記内視鏡のワーキングチャネルを通って前記第1区域内の標的部位に延びる、請求項10に記載のシステム。
- 前記第2長尺状部材は内視鏡である、請求項1に記載のシステム。
- 前記第1可撓性要素の遠位端部の前記結合要素はバルーンである、請求項1に記載のシステム。
