ように構成される、コンピューティング装置と、

VfAペーシング治療のための1つまたは複数のVfAペーシングされた設定が受容可能であるか否かを判断することは、電気的不均一性の尺度が閾値以下である場合に、1つまたは複数のVfAペーシングされた設定が受容可能であると判断することを含む、実施形態1~7のいずれか1つのシステム。
VfAペーシング治療の送達を行うことなく、監視された電気的活性からベースラインEHIを生成することと、
ベースラインEHIをペーシングされたEHIと比較することと、
ベースラインEHIとペーシングされたEHIとの比較に基づいて、1つまたは複数のVfAペーシングされた設定が受容可能であると判断することと、
を含む、実施形態1~8のいずれか1つのシステム。
調整された1つまたは複数のVfAペーシングされた設定は、VfAペーシング治療が脚ブロック(BBB)を補正することに応答して受容可能であると判断される、実施形態1~11のいずれか1つのシステム。
調整された1つまたは複数のVfAペーシングされた設定は、VfAペーシング治療が患者の心臓のプルキンエ系に完全に連動することに応答して受容可能であると判断される、実施形態1~12のいずれか1つのシステム。
複数の外部電極を用いて患者の組織から電気的活性を監視することと、
1つまたは複数の心房から心室への(VfA)ペーシングされた設定におけるVfAペーシング治療の送達中の監視された電気的活性に基づいて、ペーシングされた電気的不均一性情報(EHI)を生成することであって、ペーシングされたEHIは、機械的心臓機能性および電気的心臓機能性のうちの少なくとも1つを表すことと、
ペーシングされたEHIに基づいて、VfAペーシング治療のための1つまたは複数のVfAペーシングされた設定が受容可能であるか否かを判断することと、
を含む、方法。
VfAペーシング治療のための1つまたは複数のVfAペーシングされた設定が受容可能であるか否かを判断することは、電気的不均一性の尺度が閾値以下である場合に、1つまたは複数のVfAペーシングされた設定が受容可能であると判断することを含む、実施形態14~20のいずれか1つの方法。
VfAペーシング治療の送達を行うことなく、監視された電気的活性からベースラインEHIを生成することと、
ベースラインEHIをペーシングされたEHIと比較することと、
ベースラインEHIとペーシングされたEHIとの比較に基づいて、1つまたは複数のVfAペーシングされた設定が受容可能であると判断することと、
を含む、実施形態14~21のいずれか1つの方法。
調整された1つまたは複数のVfAペーシングされた設定は、VfAペーシング治療が脚ブロック(BBB)を補正することに応答して受容可能であると判断される、実施形態14~24のいずれか1つの方法。
調整された1つまたは複数のVfAペーシングされた設定は、VfAペーシング治療が患者の心臓のプルキンエ系に完全に連動することに応答して受容可能であると判断される、実施形態14~26のいずれか1つの方法。
患者の組織から電気的活性を監視するための複数の外部電極を備える電極装置と、
処理回路を備え、電極装置に結合されたコンピューティング装置であって、
心房から心室への(VfA)ペーシング治療の送達中に複数の外部電極を用いて電気的活性を監視し、
VfAペーシング治療の送達中に電気的不均一性情報(EHI)を生成し、
VfAペーシングされた設定を用いて、電気的活性から生成されたEHIに基づいて、VfAペーシング治療のためのVfAペーシングされた設定が受容可能であるか否かを判断し、
VfAペーシング治療が受容可能であるか否かに基づいてVfAペーシング治療のためのペーシングされた設定を調整する、
ように構成される、コンピューティング装置と、
を備える、システム。
Claims (14)
- システムであって、
患者の組織から電気的活性を監視するための複数の外部電極を備える電極装置と、
処理回路を備え、前記電極装置に結合されたコンピューティング装置であって、
前記複数の外部電極を用いて電気的活性を監視する手段と、
1つまたは複数の心房から心室への(VfA)ペーシングされた設定におけるVfAペーシング治療の送達中の前記監視された電気的活性に基づいて、ペーシングされた電気的不均一性情報(EHI:electrical heterogeneity information)を生成する手段であって、前記ペーシングされたEHIは、機械的心臓機能性および電気的心臓機能性のうちの少なくとも1つを表す、手段と
前記ペーシングされたEHIに基づいて、前記VfAペーシング治療のための前記1つまたは複数のVfAペーシングされた設定が受容可能であるか否かを判断する手段と、
を実装するように構成される、コンピューティング装置と、
を備える、システム。 - システムであって、
複数の外部電極を用いて患者の組織から電気的活性を監視する手段と、
1つまたは複数の心房から心室への(VfA)ペーシングされた設定におけるVfAペーシング治療の送達中の前記監視された電気的活性に基づいて、ペーシングされた電気的不均一性情報(EHI)を生成する手段であって、前記ペーシングされたEHIは、機械的心臓機能性および電気的心臓機能性のうちの少なくとも1つを表す、手段と、
前記ペーシングされたEHIに基づいて、前記VfAペーシング治療のための前記1つまたは複数のVfAペーシングされた設定が受容可能であるか否かを判断する手段と、
を含む、システム。 - 前記1つまたは複数のVfAペーシングされた設定は、電圧、パルス幅、固有の(intrinsic)またはペーシングされた心房タイミングに対するVペーシングのタイミング、およびペーシングレートのうちの少なくとも1つを含む、請求項1または2に記載のシステム。
- 前記1つまたは複数のVfAペーシングされた設定は、少なくとも1つの植込み型の電極のロケーションを含み、前記ロケーションは、深さおよび角度のうちの少なくとも1つを含む、請求項1~3のいずれか一項に記載のシステム。
- 前記1つまたは複数のVfAペーシングされた設定は、ペーシング極性、ペーシングベクトル、および使用されるペーシング電極の数のうちの少なくとも1つを含む、請求項1~4のいずれか一項に記載のシステム。
- 前記VfAペーシング治療の前記送達は、VfAペーシング治療装置によって行われ、前記VfAペーシング治療装置は、右心房のコッホ三角領域(triangle of Koch region)から右心房心内膜(endocardium)および中心線維体(central fibrous body)を通って植込み可能な組織穿孔電極であって、前記患者の左心室心筋の基部領域および/または中隔領域において左心室に前記VfAペーシング治療を送達する、組織穿孔電極を備える、請求項1~5のいずれか一項に記載のシステム。
- 前記VfAペーシングされた設定は、前記患者の前記左心室の高位の後壁基部(high posterior basal)および/または中隔エリア(septal area)の近接した少なくとも1つの植込み型の電極のロケーションを含む、請求項6に記載のシステム。
- 前記複数の外部電極は、前記患者の胴部の皮膚に近接して位置するように構成されたアレイ内に位置決めされた表面電極を含む、請求項1~7のいずれか一項に記載のシステム。
- 前記ペーシングされたEHIは、電気的不均一性の尺度(metric)を含み、
前記VfAペーシング治療のための前記1つまたは複数のVfAペーシングされた設定が受容可能であるか否かを判断する手段は、前記電気的不均一性の尺度が閾値以下である場合に、前記1つまたは複数のVfAペーシングされた設定が受容可能であると判断する手段を含む、請求項1~8のいずれか一項に記載のシステム。 - 前記VfAペーシング治療のための前記1つまたは複数のVfAペーシングされた設定が受容可能であるか否かを判断する手段は、
前記VfAペーシング治療の送達を行うことなく、前記監視された電気的活性からベースラインEHIを生成する手段と、
前記ベースラインEHIを前記ペーシングされたEHIと比較する手段と、
前記ベースラインEHIと前記ペーシングされたEHIとの比較に基づいて、前記1つまたは複数のVfAペーシングされた設定が受容可能であると判断する手段と、
を含む、請求項1~9のいずれか一項に記載のシステム。 - 前記電気的不均一性情報は、電気的不均一性の少なくとも1つの尺度を含み、前記電気的不均一性の少なくとも1つの尺度は、前部中隔活性時点(ASAT:anterior-septal activation times)を含み、ASATは、前記患者の心臓の前部中隔領域に対応する前記複数の外部電極のうちの外部電極から監視された活性時点を含む、請求項1~10のいずれか一項に記載のシステム。
- 前記電気的不均一性情報は、電気的不均一性の少なくとも1つの尺度を含み、前記電気的不均一性の少なくとも1つの尺度は、前部中隔活性時点(ASAT)、活性時点の標準偏差(SDAT:standard deviation of activation times)、複合左心室活性時点(LVAT:left ventricular activation time)および複合右心室活性時点(RVAT:right ventricular activation time)を含む、請求項1~11のいずれか一項に記載のシステム。
- システムは、前記VfAペーシング治療が受容可能であるか否かに基づいて、前記VfAペーシング治療のための前記1つまたは複数のVfAペーシングされた設定を調整する手段を実装するように更に構成され、
前記調整された1つまたは複数のVfAペーシングされた設定は、前記VfAペーシング治療が脚ブロック(BBB:bundle branch block)が補正されたという決定に応答して受容可能であると判断される、請求項1~12のいずれか一項に記載のシステム。 - システムは、前記VfAペーシング治療が受容可能であるか否かに基づいて、前記VfAペーシング治療のための前記1つまたは複数のVfAペーシングされた設定を調整する手段を実装するように更に構成され、
前記調整された1つまたは複数のVfAペーシングされた設定は、前記VfAペーシング治療が前記患者の心臓のプルキンエ系(Purkinje system)に完全に連動(engaging)されたという決定に応答して受容可能であると判断される、請求項1~13のいずれか一項に記載のシステム。
