概要
フェムトリットル以下のサイズの観察体素においてチオフラビンT(ThT)蛍光強度ゆらぎの時間分解検出を使用することによって、見かけ上健常個体及びアルツハイマー病(AD)と診断された患者の血清中の非常に低レベルの小さな構造化アミロイド形成性ナ
ノプラークを単一分子感度で検出することができた。小さな構造化アミロイド形成性ナノプラークに遭遇する頻度及びそのサイズは、対照被験体よりもADと診断された個体の血清中において著しく高かった。本発明者らによる試験の結果は、バルク蛍光ThT分光法とは異なり、時間分解ThT蛍光強度ゆらぎ分析では、血清中の小さな構造化アミロイド形成性ナノプラークの直接検出、定量、及びサイズ決定が可能であることを示す。この方法は、免疫ベースのプローブの使用にも放射性トレーサ、シグナル増幅、又はタンパク質分離技術の使用にも依存せず、末梢血循環(図1~11及び20~22)、脳脊髄液(図12)、唾液(図13)、及び尿(図14)を含む体液中の構造的に修飾されたタンパク質を迅速かつコスト効率よく早期に決定するための低侵襲的試験を提供する。

アルツハイマー病(AD)は、荒廃的神経変性病であり、世界中で認知症の最も一般的な原因である(1、2)。ADは、罹患した個体の生命維持に必要な知能を徐々に破壊し、認知能力を著しく低下させる。ADによって誘導される損傷の程度は、罹患した人が、最終的にはもはや自身の世話をすることができなくなり、残りの人生は常にケア及び支援を必要とするほどである。残念なことに、集中的な研究及び大きな社会的要求にもかかわらず、信頼できかつコスト効率のよいADの早期診断法は未だ揺籃期である。これは、新規薬物の開発の妨げにもなっており、現在ADの治療法は存在していない。
楽観的な材料になるものではなかった。PET及び放射性トレーサを使用する脳イメージングは、>1mmのサイズの脳内におけるアミロイドの原線維沈着物を確実に映すであろう(20~22)。免疫化学的技術を使用した脳脊髄液(CSF)中のAβペプチド(及びTauタンパク質)の含量の測定も信頼できると考えられる(23~25)。しかし、PET及びCSFの方法論は、侵襲的かつ高価である。トピックの概説(13、14)では、著者は、診断法として血漿中のAβを単に測定することの欠点を記載しており、血液検査をPET及びCSFの結果と相互に関連付けようとする試みは今までのところ説得力のあるものではないことを示している。2つの技術によって測定されるAβの構造状態が不均一かつ異なることが、恐らく実験を困難にしている理由の1つである:免疫化学的方法は、Aβペプチド(複数可)の可溶性形態を測定するが、一方、PET法は、脳内の沈降した不溶性ペプチド凝集体を測定する。両方ともがAβペプチドの存在及び挙動に通じるものであったとしても、2種類の測定値の間に明らかな直接的関連は存在しない。追加のバイオマーカー方法論に基づく早期AD診断のための新規、非侵襲的、かつ信頼できる方法が明らかに切実に必要とされている。このような方法が存在しないことは、新規薬物の開発の妨げにもなっており、脳内のアミロイド負荷を低減することを目的とする新規治療法の開発は、経時的に繰り返し同じ個体において構造化アミロイド形成性凝集体のレベルを定期的に同定及び特性評価できる可能性に大きく依存している。
患者コホート及び倫理
見かけ上健常な献血者(対照群)及びプライマリ・ケア設定において検査した後Memory Clinic(Karolinska University Hospital,Huddinge)に付託された個体(患者コホート)から、インフォームドコンセントの際に血液サンプルを得た。血液サンプルの採取及び取り扱いは、ストックホルムの地域倫理委員会によって承認された(許可番号:2012/1019-31/1)。
静脈穿刺用の真空管システム(BD Vacutainer(登録商標)チューブ、Becton,Dickinson and Company)を使用して上肢の浅静脈から血液を採取し、下記の通り処理して血清を得る。時間分解検蛍光強度ゆらぎ分析(FIFA)に供する前にThTをサンプルに添加した。
Vacutainer(登録商標)Plastic Serumチューブに5mlの血液を回収する。血液凝固を活性化するために、チューブを180°反転させ、元に戻すことを5~6回行い、血液を60分間凝固させる。その後、チューブを2500×gで10分間遠心分離し、血餅の上方の上清、すなわち血清を、混濁度及び溶血の証拠について目視検査する。中等度の混濁度及び溶血は結果のアウトカムに影響を与えないことが別の対照実験で証明されてはいたものの(データは示さず)、濁った血清サンプル及び肉眼で確認できるピンク色に変色したサンプルは、更なる分析には供さなかった。
Vacutainer(登録商標)Plastic K3EDTAチューブに5mlの血液を回収して、~7mMのEDTA濃度を得る。高EDTA濃度(10mM以下)が誘導期の長さにもインビトロAβペプチド凝集プロセス中の蛍光成長曲線の傾きにも影響を及ぼさないことを示す(データは示さず)バルクThT蛍光分光測定に基づいて、このEDTA濃度はAβ凝集にもThTアッセイにも干渉しない。チューブを穏やかに180°反転させ、元に戻すことを10回行い、60分間沈降させる。その後、チューブを2500×gで10分間遠心分離し、細胞層の上方の上清、すなわち血漿を、混濁度及び溶血の証拠について目視検査する。上記の通り、濁った血漿サンプル及び肉眼で確認できるピンク色に変色した血漿サンプルは廃棄し、更なる分析には供さなかった。即時使用する場合、200μlの血漿を8ウェルチャンバー付カバーガラスのウェルに分注し、最終濃度が20μM ThTになるように1.6μlの2.5mM ThTと混合し、血清について記載したのと同じ手順に従ってFIFAに供する。
BD Hemogard(商標)Closureを備えるBD Vacutainer(登録商標)Plastic K3EDTAチューブの底部に残存する細胞ペレットに150mM NaCl中1% Triton X-100を0.1ml添加し;その内容物を2500×gで10分間遠心分離し、分析のために細胞溶解物を取り出す。即時分析する場合、200μlの細胞溶解物を8ウェルチャンバー付カバーガラスのウェルに分注し、最終濃度が20μM ThTになるように1.6μlの2.5mM ThTと混合し、血清について記載したのと同じ手順に従ってFIFAに供する。
蛍光相関分光法(FCS)は、希釈水溶液中の明るい蛍光分子を検出するための、最高の単一分子感度を有する定量分析法である(26~32)。FCSは、従来の機器設定において約2×10-16l~10×10-16lである非常に小さな体積における自発的蛍光強度ゆらぎの時系列を高時間分解能で観察することによって、単一分子感度を実現する。このような小さな体積では、1回に見る分子の数が少ないことによって、溶媒分子等の接近しやすい分子から生じるバックノイズが著しく低下する。従って、小さな観察体積を通る明るい蛍光分子の通過によって蛍光強度が顕著に変化し、これは容易に検出することができる。
C-Apochromat 40×、NA=1.2、水浸UV-VIS-IR対物レンズを備える倒立顕微鏡からなるConfoCor 2システム(Carl Zeiss,Jena,Germany)を使用して、時間分解蛍光強度ゆらぎ測定を実施する。アルゴンレーザーの458nmラインを使用してThT蛍光を励起する。対物レンズを通過することによって入射レーザー光がサンプルに強く集束され;対物レンズで測定されるレーザー強度は6μWである。弾性散乱光及びスペクトル的に異なる蛍光は、同じ対物レンズを通して後方に集束され、HFT458主ダイクロイックビームスプリッタを使用して分
離される。発せられた光は、ピンホールを通過してシリコンアバランシェ発光ダイオード検出器(SPCM-AQR-1X;PerkinElmer,USA)に入ることによって空間的に選別される。検出器の前方のピンホールサイズは、70μm(1Airy)に設定した。
OVEを通るThT反応性構造化アミロイド形成性ナノプラークの偶発的通過を反映しているThT蛍光強度の稀なバーストを同定するために自動化単一事象計数分析を実施しすることにより、構造化アミロイド形成性ナノプラークがOVEを通過する頻度を求める。この目的のために、Matlabソフトウェアを使用してプログラムを書いた。蛍光ピーク、すなわち「単一事象」は、全時系列の標準偏差(SD)の5倍大きい値だけ全時系列の平均蛍光強度<F(t)>と異なる蛍光強度の増加ΔF(t)として定義される、ΔF(t)=(F(t)-<F(t)>)>5×SD。次いで、観察された蛍光ピーク数を全測定時間(ここでは3000s)に対して正規化し、それによって得られた単一事象発生(fSEO)の頻度をs-1で表し、サンプル間で比較する。
断について知らされなかった。同様に、診断は標準的な臨床試験を使用して確定され、時間分解ThT血清蛍光強度ゆらぎ分析の結果は、試験が完了し、コードが明らかになるまで、臨床家には知らされなかった。対照及び患者のコホートにおける平均fSEO値が有意に異なるかどうかを評価するために、オンラインで利用可能なGraphPad Softwareツール(http://www.graphpad.com/quickcalcs/ttest1.cfm?Format=SD)を使用して独立t検定分析を実施した。
全ての化学物質は、更に精製することなく使用し、以下の通り商業的供給元から購入した:ローダミン6G(Sigma-Aldrich);チオフラビンT(Sigma-Aldrich);カルボキシラート官能化量子ドット名のクリスタル、d=20nm、525nmで発光極大(Qdot(登録商標)525 ITK(商標)Carboxyl Quantum Dots)、及び直径d=100nmの黄色-緑蛍光(Ex/Em:505/515)カルボキシラート修飾ポリスチレンナノ/ミクロスフィア(FluoSpheres(登録商標)Size Kit #2)(Molecular Probes製)。使用前に、フルオスフィア懸濁液を30~60分間超音波処理した。
時間的自己相関分析では、シグナル自己相似性分析を実施してデータにおける非ランダム性を検出し、そして、データがランダムではない場合には適切な時系列モデルを同定する。このプロセスの第1の工程として、いわゆる正規化強度自己相関関数G(τ)を計算する:
既知の濃度1fM≦c≦50nMのフルオスフィア(d=100nm)の希釈水懸濁液を使用して拡散時間決定手順を検証し(図S2)、単一事象をカウントすることによって、すなわち単一事象発生fSEOの頻度を求めることによって濃度を検証した(図S3)。
ThTを含まない血清を、得られた結果に対する確率的誤差の影響を評価するための対照として使用した(図S4)。分かる通り、ThTを含まない血清中でも時折蛍光強度ピークが検出されるが(薄灰色及び縞模様の薄灰色)、その平均発生頻度はThTを含む血清よりも有意に低く(黒色及び濃灰色);対照及び患者群においてこれらピークが発生する頻度に差はなく;標準偏差は平均よりも大きく、このことは、確率的誤差の平均値がゼロと有意には異ならないことを示唆している(図S4)。
血清自己蛍光及び血清ThT蛍光
血清は、様々なクラスの化学化合物の複雑な水性混合物であり、そのうち脂質(トリグリセリドの形態)及びタンパク質(アルブミン及び免疫グロブリン)が最も大量に存在する。血清を構成する化合物の多くは、天然に蛍光性であり、458nmで光を吸収し、例えば、フラビン(FAD)及びピリジン性(NADH)の補酵素及び脂肪色素である。結果として、血清は自己蛍光を示し、また、振幅A血清=(0.003±0.002)及び単一の特徴的な減衰時間τ血清=(55±15)μsの明確なtACCを生じさせる(図2A、黒色)。これら実験データにフィッティングさせることができた最も単純な理論的モデルは、三重項なしの自由3D拡散についての自己相関関数であった(27におけるAF1)。Rh6Gを用いた機器の較正によって求められた値に固定した構造パラメータ、Sp=5を保持すると、フィッティングによって導出されるパラメータ値は、N=300及びτD=55μsである。
ThT血清蛍光の時間分解検出から、時系列の標準偏差(SD)よりも5倍よりも大きい値だけ平均蛍光強度と異なる蛍光強度に非常によく同定可能なピークが稀に発生することが明らかになる、(F(t)-<F(t)>)>5×SD(図3)。既に示されている通り、本発明者らはこれらピークを「単一事象」と呼ぶ。
材料及び方法に記載の通り計算した単一事象発生の頻度:データ解析の小区分は、患者群fSEOp=(2.3±1.7)×10-3s-1と対照群fSEOc=(1.5±1.2)×10-3s-1との間で異なる。独立t検定分析によって求められた両側P値、P=0.0418は、患者群と対照群との間の単一事象発生頻度の差が統計的に有意であることを示唆している(図4A)。コードを明らかにし、臨床診断に基づいて患者をサブグループに分類した後も、AD患者fAD SEOp=(2.5±1.5)×10-3s-1と対照群fSEOc=(1.5±1.2)×10-3s-1との間の差は統計的に有意なままである、P=0.0420(図4B)。
材料及び方法:データ解析に記載の通り実施した時間的自己相関分析は、2つの群について記録されたtACC間で特徴的な減衰時間の明らかな差を示し、患者群では減衰時間が顕著に長い(図6)。正規化されたtACCの評価は、分析される系が複雑であり、サイズが均一な粒子を含有するのではなく、非常に異なるサイズの粒子の混合物を含有することを示す。対照群におけるThT活性粒子の平均拡散時間は約5msであり、患者群における粒子の平均拡散時間は約15msであり、これは、球状分子の場合、2つの群間の分子量の差が約27倍であることに相当する。明らかに、患者群は、より多数のナノプラークに加えて、血清中により大きなThT活性構造化凝集体を保有する。
本発明者らは、血清における時間分解ThT FIFAによって得られた結果を、アミロイドベータペプチドの42アミノ酸形態(Aβ42)のCSFレベル及びtauタンパク質の全CSFレベル(t-Tau)を含む脳脊髄液(CSF)における確立されている生化学的バイオマーカーを使用して得られた結果と比較した。この比較は、時間分解ThT血清FIFAによって得られた結果が、CSF生化学的分析によって得られた結果と明らかに一致することを示す(図7)。CSF中のAβ42レベルに基づくAD診断につい
ての一般的に許容されている限度は、<550ngl-1である(36)。図7に提示するデータに基づいて、本発明者らは、血清におけるfSEO>1.5×10-3s-1の限度がADの指標であることを示唆する。
アミロイド原線維及び構造化アミロイド形成性オリゴマーの存在下において、ThTは、遊離色素とは顕著に異なる蛍光励起及び発光スペクトルを取得する(図1、挿入図)(8~12)。この蛍光変化は、一般的に、ThTのアミロイド形成性構造との相互作用に特異的であると考えられ、ネイティブなタンパク質モノマー又は非晶質凝集体への結合時には誘導されない(6、7)。この特性に起因して、蛍光顕微鏡によって組織アミロイド沈着物を可視化するためにThTが広く使用されており(37~39);ThT同族体は、陽電子放射断層撮影法(PET)及び単一光子放射断層撮影(SPECT)を使用するインビボイメージングのための基剤であり(40~42);ThTは、インビトロにおける精製タンパク質及びペプチドからのアミロイド原線維形成の時系列をモニタリングするために、古典的なバルク蛍光分光法によるタンパク質凝集の基礎研究において不可欠である(5、43、44)。
出用の物質が本質的に不安定であり、酵素の非存在下でさえもシグナルを生成する場合がある(48)。ドデシル硫酸ナトリウム-ポリアクリルアミドゲル電気泳動(SDS-PAGE)、2D-PAGE、液体クロマトグラフィー(LC)、質量分析(MS)、マトリクス支援レーザー脱離イオン化-飛行時間型(MALDI-TOF)質量分析法、エレクトロスプレーイオン化(ESI)質量分析法、液体クロマトグラフィー-タンデム質量分析法(LC-MS/MS)、及び関連技術を使用するプロテオミクスアプローチは、高い感度及び特異性で対象となる分子を直接特性評価し、それによって、貴重な情報を提供する(55)。しかし、ダイナミックレンジの制限から生体液等の複雑な混合物中の低含量のタンパク質を検出することが困難である、インタクトなより大きなタンパク質を測定することができない、及びサンプル調製手順が複雑であることからこれら技術は複雑である(56)。結果として、プロテオミクス研究で発見されたCSFバイオマーカーで、これまでに臨床に到達したものはない(57)。
添加剤を全く使用せずにポリプロピレンチューブ内において-80℃で保存した脳脊髄液(CSF)アリコート0.5mlを使用した。サンプルを室温で60分間解凍した。その後、各ウェルにThT水溶液を10μl含有する8ウェルチャンバー付カバーガラスにアリコート200μlを移した。数回分注/吸い込みを行うことによってサンプルを穏やかに混合し、分析に供した。分析はデュープリケートで実施した。
見かけ上健常なドナー由来の朝起きてすぐの中間尿試料を、尿保存剤を全く使用せずに滅菌容器に回収した。試料を室温で60分間沈降させた。その後、各ウェルにThT水溶液を10μl含有する8ウェルチャンバー付カバーガラスにアリコート200μlを移した。数回分注/吸い込みを行うことによってサンプルを穏やかに混合し、分析に供した。分析はデュープリケートで実施した。
朝、朝食の2時間後、及び朝の衛生行動後に見かけ上健常なドナーから唾液サンプルを採取した。この時間中、ドナーは水しか摂取しなかった。サンプル採取の直前に脱イオン水で流すことによって口内をおおまかに清浄化した後、自然に流れる唾液2mlを受動的にサンプリングし(吸引なし)、氷浴に浸漬している滅菌容器に回収した。回収後、各ウェルにThT水溶液を10μl含有する8ウェルチャンバー付カバーガラスにアリコート200μlを移した。数回分注/吸い込みを行うことによってサンプルを穏やかに混合し、15分間加温にし、分析に供した。分析はデュープリケートで実施した。

- 前記血液のサンプルが、フラビンアデニンジヌクレオチド(FAD)酵素及び/若しくはFAD脂肪色素;並びに/又はピリジン性(NADH)酵素及び/若しくはNADH脂肪色素からなる群のうちの1つ以上を含まない、請求項4~7のいずれか一項に記載の方法。
- 前記検出剤が、個々のアミロイド凝集体と結合及び/又は会合及び/又は反応したときに蛍光を発光する蛍光部分を含む、請求項1~8のいずれか一項に記載の方法。
- 前記検出剤が、チオフラビンT(ThT)、及び/又はARCAM1、及び/又は五量体ホルミルチオフェン酢酸(pFTAA)、及び/又は4-(ジシアノビニル)-ジュロリジン(DCVJ)、及び/又は1-アミノ-8-ナフタレンスルホナート(ANS)、及び/又はビス-ANS、及び/又は2-[N-ビス-(3-ジメチルアミノプロピル)-アミノ]-4-[2,3-ジヒドロ-3-メチル-(ベンゾ-1,3-チアゾール-2-イル)-メチリデン]-1-フェニル-キノリニウム(PicoGreen)からなる群のうちの1つ以上である、請求項1~9のいずれか一項に記載の方法。
- 前記個々のアミロイド凝集体が、アミロイドオリゴマー;アミロイドナノ凝集体;及び/又はアミロイド老人性凝集体;及び/又はアミロイド原線維;及び/又はアミロイド前原線維を含む群から選択される、請求項1~10のいずれか一項に記載の方法。
- 前記個々のアミロイド凝集体が、20以上のペプチド;例えば:30以上;40以上;50以上;60以上;70以上;80以上;90以上;又は100以上のペプチドを含む、請求項1~11のいずれか一項に記載の方法。
- 前記アミロイドナノ凝集体が、20のタンパク質又はペプチド~100,000のタンパク質又はペプチドを含む、請求項11又は12に記載の方法。
- 前記老人性アミロイド凝集体が、1,000,000超のタンパク質又はペプチドを含む、請求項11に記載の方法。
- 前記個々のアミロイド凝集体が、1枚以上のベータ(β)シートを含み、前記検出剤が、1枚以上のベータ(β)シートを含む個々のアミロイド凝集体と結合及び/又は会合及び/又は反応する、請求項1~14のいずれか一項に記載の方法。
- 前記アミロイド関連疾患が、アルツハイマー病;及び/又はパーキンソン病;及び/又はハンチントン病;及び/又は筋萎縮性側索硬化症(ALS);及び/又は2型糖尿病;及び/又は全身性アミロイドーシス;及び/又はプリオン病(例えば、クロイツフェルト・ヤコブ病(CJD));及び/又はアミロイド軽鎖アミロイドーシス;及び/又は多発性骨髄腫;及び/又は炎症;及び/又は腎不全;及び/又は白血球走化性因子2(LECT2)アミロイドーシス;及び/又は多発ニューロパシーを伴うトランスサイレチン関連遺伝性アミロイドーシス(家族性アミロイド多発ニューロパシー(FAP)又はSkelleftesjukanとしても知られている);及び/又は血液透析関連アミロイドーシス;及び/又は脳アミロイドアンギオパシー;及び/又は家族性内蔵アミロイドーシス;及び/又は原発性皮膚アミロイドーシス;及び/又は脳アミロイドアンギオパシー;及び/又はプロラクチノーマ;及び/又は家族性角膜アミロイドーシス;及び/又は心房の老人性アミロイド;及び/又は甲状腺髄様がん;及び/又は医薬品インスリン誘発性アミロイドーシスを含む群から選択される1つ以上である、請求項1~15のいずれか一項に記載の方法。
- 前記アルツハイマー病が、主観的認知障害;軽度認知障害;アルツハイマー病;及び血管性認知症を伴うアルツハイマー病を含む群から選択される、請求項16に記載の方法。
- 前記アミロイド関連疾患が、アルツハイマー病であり、前記個々のアミロイド凝集体が、アミロイドベータ(β)ペプチド(βペプチド)、例えば、β40及び/又はβ42及び/又はβ43を含む、請求項16又は17に記載の方法。
- 前記個々のアミロイド凝集体の存在及び/又は量及び/又は濃度及び/又はサイズの決定が、個々のアミロイド凝集体分解能において行われる、請求項1~18のいずれか一項に記載の方法。
- 被験体(ヒトを除く)におけるアミロイド関連疾患を治療する方法であって、アミロイド関連疾患が個々のアミロイド凝集体に関連し、
前記方法が、
・ 前記被験体由来の体液のサンプルを提供する工程と、
・ 前記サンプルに検出剤を添加し、前記検出剤が、個々のアミロイド凝集体と結合及び/又は会合及び/又は反応する工程と、
・ 前記サンプル中の個々のアミロイド凝集体の存在及び/又は量及び/又は濃度及び/又はサイズを決定する工程と、
・ 前記決定に基づいて、前記被験体を前記アミロイド関連疾患を有すると同定する工程と;
を含み、
前記サンプル中の個々のアミロイド凝集体の存在及び/又は量及び/又は濃度及び/又はサイズの決定が、蛍光相関分光法(FCS)及び蛍光強度ゆらぎ分析(FIFA)を含む時間分解検出に基づく分析を含み、前記方法が、個々のアミロイド凝集体の増幅を含まない、方法。 - 前記治療が、前記アミロイド関連疾患の治療を前記被験体に施すことを含む、請求項20に記載の方法。
- アミロイド関連疾患を治療するための剤を同定する方法であって、
・ 試験被験体由来の体液の第1のサンプルを提供する工程と;
・ 前記第1のサンプル中の個々のアミロイド凝集体の存在及び/又は量及び/又は濃度及び/又はサイズを決定する工程と;
・ 剤を投与された前記試験被験体由来の体液の第2のサンプルを提供する工程と;
・ 前記第2のサンプル中の個々のアミロイド凝集体の存在及び/又は量及び/又は濃度及び/又はサイズを決定する工程と;
・ 前記第1のサンプル中の個々のアミロイド凝集体の存在及び/又は量及び/又は濃度及び/又はサイズの決定を、前記第2のサンプル中の個々のアミロイド凝集体の存在及び/又は量及び/又は濃度及び/又はサイズの決定と比較する工程と;
・ 前記比較に基づいて、前記剤を前記アミロイド関連疾患を治療するためのものと同定する工程と;
を含み、アミロイド関連疾患が個々のアミロイド凝集体に関連し、前記方法が、前記サンプルに検出剤を添加し、前記検出剤が個々のアミロイド凝集体と結合及び/又は会合及び/又は反応することを更に含み、前記サンプル中の個々のアミロイド凝集体の存在及び/又は量及び/又は濃度及び/又はサイズの決定が、蛍光相関分光法(FCS)及び蛍光強度ゆらぎ分析(FIFA)を含む時間分解検出に基づく分析を含み、前記方法が、個々のアミロイド凝集体の増幅を含まない、

・ 前記比較に基づいて、前記剤を前記アミロイド関連疾患を治療するためのものと同定する工程と;
