Claims (19)
- 電気穿孔を提供するためのシステムであって、前記システムは、
患者の腎臓領域に配置されるように構成された第1電極および第2電極;
センサ;
前記第1電極、前記第2電極、および前記センサに結合されたパルス発生器;
を備え、
前記パルス発生器は、
コンピュータ実行可能命令を記憶することができるメモリ;
前記メモリに記憶された実行可能命令を実行にするように構成されたプロセッサ;
を備え、
前記命令は、前記プロセッサに、
前記パルス発生器を介して、前記第1電極と前記第2電極との間において前記腎臓領域に対して刺激を引き起こす刺激電流を発生させるステップ;
前記センサを介して、前記刺激電流によって生じる前記患者の血圧の変化を検出するステップ;
前記患者の血圧の前記変化を検出したことに応じて、電気穿孔電流を生成して、前記第1電極と前記第2電極との間において前記腎臓領域に対して可逆または不可逆の電気穿孔を生じさせるステップ;
を実施させる、
システム。 - 前記腎臓領域は、腎静脈、腎動脈、または腎盂のうちの少なくとも1つを含む、
請求項1記載のシステム。 - 前記命令は、前記患者の血圧の変化が検出されないとき、前記プロセッサに電極構成を変更するステップを実施させる、
請求項1記載のシステム。 - 前記電極構成を変更するステップは、
前記第1電極の位置または前記第2電極の位置、
前記第1電極の極性または前記第2電極の極性、
前記刺激電流のパラメータ、
のうち少なくとも1つを変更するステップを有する、
請求項3記載のシステム。 - 前記命令は、前記プロセッサにさらに、
前記電気穿孔電流を生成した後に、前記パルス発生器を介して、第2刺激電流を生成して、前記第1電極と前記第2電極との間において前記腎臓領域に対して刺激を生じさせるステップ;
前記センサを介して、前記第2刺激電流に対する生理学的応答を検出するステップ;
を実施させる、
請求項1記載のシステム。 - 前記腎臓領域の外側に配置されるように構成された第3電極をさらに備える、
請求項1記載のシステム。 - 前記命令は、前記プロセッサにさらに、
前記第1電極と前記第2電極の両方をアノードまたはカソードの一方に変えるステップ;
前記第3電極をアノードまたはカソードの他方に変えるステップ;
前記電気穿孔電流を生成した後に、前記パルス発生器を介して、第2刺激電流を生成して、前記第1電極と前記第2電極との間において前記腎臓領域に対して刺激を生じさせるステップ;
前記センサを介して、前記第2刺激電流に対する第2生理学的応答を検出するステップ;
を実施させる、
請求項6記載のシステム。 - 前記命令は、前記第2刺激電流に対する生理学的応答が検出されたことに応じて、前記プロセッサに、第2電気穿孔電流を生成するステップを実施させる、
請求項7記載のシステム。 - 前記第1電極と前記第2電極は、前記患者の前記腎臓領域の自律神経組織の近傍に配置されるように構成されている
請求項1記載のシステム。 - 前記第1電極と前記第2電極は、前記患者の下大静脈、下行大動脈、または尿管に隣接する神経にわたって電場を分布させるように構成されている
請求項9記載のシステム。 - 前記電気穿孔電流は、ナノ秒のパルス幅を有するパルスで伝搬されるように構成されている
請求項1記載のシステム。 - 前記生理学的応答は、
心拍数、血圧、経皮インピーダンス、または末梢神経の神経連絡
のうち少なくともいずれかにおける変化である
請求項5記載のシステム。 - 電気穿孔を提供するためのシステムを制御する方法であって、
前記システムは、
第1電極および第2電極;
センサ;
前記第1電極、前記第2電極、および前記センサに結合されたパルス発生器;
を備え、
前記パルス発生器は、プロセッサを備え、
前記方法は、前記プロセッサに、
前記パルス発生器を介して、前記第1電極と前記第2電極との間において刺激電流を発生させるステップ;
前記センサを介して、前記刺激電流によって生じる患者の血圧の変化を検出するステップ;
前記患者の血圧の前記変化を検出したことに応じて、電気穿孔電流を生成して、前記第1電極と前記第2電極との間において可逆または不可逆の電気穿孔を生じさせるステップ;
を実施させる、
方法。 - 前記方法はさらに、前記患者の血圧の変化が検出されないとき、前記プロセッサに電極構成を変更するステップを実施させる、
請求項13記載の方法。 - 前記電極構成を変更するステップは、
前記第1電極の位置または前記第2電極の位置、
前記第1電極の極性または前記第2電極の極性、
前記刺激電流のパラメータ、
のうち少なくとも1つを変更するステップを有する、
請求項14記載の方法。 - 前記方法は、前記プロセッサにさらに、
前記電気穿孔電流を生成した後に、前記パルス発生器を介して、第2刺激電流を生成して前記第1電極と前記第2電極との間に刺激を発生させるステップ;
前記センサを介して、前記第2刺激電流に対する生理学的応答を検出するステップ;
を実施させる、
請求項13記載の方法。 - 前記システムは、第3電極をさらに備える、
請求項13記載の方法。 - 前記方法は、前記プロセッサにさらに、
前記第1電極と前記第2電極の両方をアノードまたはカソードの一方に変えるステップ;
前記第3電極をアノードまたはカソードの他方に変えるステップ;
前記電気穿孔電流を生成した後に、前記パルス発生器を介して、第2刺激電流を生成するステップ;
前記センサを介して、前記第2刺激電流に対する第2生理学的応答を検出するステップ;

請求項17記載の方法。 - 前記方法は、前記第2刺激電流に対する生理学的応答が検出されたことに応じて、前記プロセッサに、第2電気穿孔電流を生成するステップを実施させる、
