Description
本出願は、2018年5月1日に出願された米国仮特許出願62/665,046号、2019年1月31日に出願された米国仮特許出願第62/799,473号、および、2019年2月11日に出願された米国仮特許出願62/803,708に対する優先権を主張するものであり、これらは参照により本明細書に完全に援用される。
Claims (25)
- 患者の血管管腔内または心腔内で介入装置の位置調整をするための操縦装置であって、
血管管腔内または心腔内で広がり、かつ、周囲の組織に対して周方向の力をかけるように制御可能な一組の拡張可能構造体と、
1組の拡張可能構造体をアンカーポイントとして利用する1組の紐と、
環状の外周部と中央開口部とを有し、一組の拡張可能構造体によって囲まれ、環状の外周部の周りに固定された一組の紐の遠位端によって支持されるアイレットとを備え、アイレットの中央開口部に介入装置を通せるようになっており、
一組の紐を利用することによって、2つの自由度でアイレットを操作でき、血管管腔または心腔に対して幾何学的に規定された領域において、1組の拡張可能構造体の場所で、介入装置の位置を制御できることを特徴とする操縦装置。 - 請求項1に記載の操縦装置であって、介入装置がガイドワイヤ、カテーテルまたは針であることを特徴とする操縦装置。
- 請求項1に記載の操縦装置であって、一組の拡張可能構造体が一組の拡張可能な枝部であることを特徴とする操縦装置。
- 請求項1に記載の操縦装置であって、内部カテーテルを利用して、通路を介して内部カテーテルの長さに沿って延びる内腔を通過させて患者の体内に介入装置を最初に案内し、内部カテーテルが患者の体外でアクセス可能な近位端部と、アイレットの環状の外周部と一直線をなす遠位端部とを有することを特徴とする操縦装置。
- 請求項4に記載の操縦装置であって、鞘部が内部カテーテルの少なくとも一部を囲んでおり、一組の拡張可能構造体が周方向に広がるのを抑制できることを特徴とする操縦装置。
- 請求項1に記載の操縦装置であって、ハンドルを使って内部カテーテルの近位端部から紐を操作することを特徴とする操縦装置。
- 請求項6に記載の操縦装置であって、ハンドルによって介入装置を操縦するための7つのDOF調整を可能にすることを特徴とする操縦装置。
- 請求項1に記載の操縦装置であって、エンコーダが紐に機械的に連結されて、アイレットと介入装置の位置を追跡することを特徴とする操縦装置。
- 請求項8に記載の操縦装置であって、ユーザーインタフェースが、追跡したアイレットの位置を、操縦装置の機構の作業空間のナビゲーションマップ上に重ね合わせることによって、介入装置の位置を描写できることを特徴とする操縦装置。
- 請求項1に記載の操縦装置であって、標的組織のボリューメトリックイメージングの再構築のために、患者の組織に対して一つ以上のエネルギー源、センサ、治療装置および診断装置を配置しかつ追跡できることを特徴とする操縦装置。
- 請求項1に記載の操縦装置であって、介入装置の位置がモニターセンサで計測されることを特徴とする操縦装置。
- 患者の血管管腔内または心腔内で介入装置の位置調整をするための操縦装置であって、
近位端部と遠位端部との間に延びる中央管腔を有する柔軟で細長い構造の内部カテーテルを備え、
内部カテーテルの近位端部に連結された、ユーザーの操作および操縦制御のためのハンドルを備え、
内部カテーテルをその長さに沿って少なくとも部分的に囲む細長い鞘部を備え、
内部カテーテルの遠位端部に配置される一組の拡張可能な枝部を備え、一組の拡張可能な枝部と細長い鞘部とを互いに対して操作することによって、一組の拡張可能な枝部が身体の血管内または体腔内で広がり、かつ、周囲の組織に対して周方向の力を加えるように制御でき、
ハンドルを介してユーザーが操作するために近位端部にてハンドルに連結される一組の紐を備え、一組の紐は、内部カテーテルに沿って鞘部内に延びており、一組の紐の遠位端部より近位側に位置する一組の拡張可能な枝部のアンカーポイントに係合しており、
中央開口部を形成する環状の外周部を有するアイレットを備え、アイレットは一組の拡張可能な枝部によって囲まれ、かつ、アイレットの環状の外周部まわりに固定された一組の紐の遠位端部によって支持されており、アイレットは内部カテーテルの遠位端部に連結され、かつ、遠位端部と一直線をなしており、内部カテーテルを貫通する介入装置がアイレットの中央開口部を通過できるようになっていることを特徴とする操縦装置。 - 請求項12に記載の操縦装置であって、ハンドルで一組の紐を操作することによって、アイレットおよびアイレットを通過する介入装置の対応する部分を、拡張可能な枝部のアンカーポイントによって規定される幾何学的に規定された領域に配置可能であることを特徴とする操縦装置。
- 請求項12に記載の操縦装置であって、介入装置がガイドワイヤ、カテーテルまたは針であることを特徴とする操縦装置。
- 請求項12に記載の操縦装置であって、エンコーダが紐に機械的に連結されて、アイレットと介入装置の位置を追跡することを特徴とする操縦装置。
- 請求項15に記載の操縦装置であって、ユーザーインタフェースが、追跡したアイレットの位置を、操縦装置の機構の作業空間のナビゲーションマップ上に重ね合わせることによって、介入装置の位置を描写できることを特徴とする操縦装置。
- 請求項12に記載の操縦装置であって、一組の紐が4本の紐を含むことを特徴とする操縦装置。
- 患者の血管管腔内または心腔内で介入装置の位置調整をするための操縦装置であって、
内部を貫通する中央管腔を形成する細長い装置アセンブリを備え、そのアセンブリは近位端部にハンドルを有し、遠位端部に拡張可能構造体を有し、
その拡張可能構造体は、一組の拡張可能な枝部と一組の紐とアイレットとを備え、
一組の拡張可能な枝部は、血管管腔内または心腔内で広がり、かつ、周囲の組織に対して周方向の力をかけるように制御可能であり、
一組の紐は、一組の拡張可能な枝部をアンカーポイントとして利用し、
アイレットは、細長い装置アセンブリの遠位端部に近接しており、一組の紐によって支持されるとともに、その内部に介入装置を通せる大きさとされており、
一組の紐を利用することにより、アイレットを2つの自由度で操作できるとともに、拡張可能な枝部の場所で幾何学的に規定された領域において、介入装置の先端部の位置を制御可能にすることを特徴とする操縦装置。 - 請求項18に記載の操縦装置であって、介入装置がガイドワイヤまたはカテーテルであることを特徴とする操縦装置。
- 請求項18に記載の操縦装置であって、エンコーダが紐に機械的に連結されて、アイレットと介入装置の位置を追跡することを特徴とする操縦装置。
- 請求項20に記載の操縦装置であって、ユーザーインタフェースが、追跡したアイレットの位置を、操縦装置の機構の作業空間のナビゲーションマップ上に重ね合わせることによって、介入装置の位置を描写できることを特徴とする操縦装置。
- 請求項18に記載の操縦装置であって、一組の紐が4本の紐を含むことを特徴とする操縦装置。
- 請求項18に記載の操縦装置であって、ハンドルが上部ローラと下部ローラとを有し、スライドしてアイレットを操縦することで、拡張可能な枝部を引き出すことを特徴とする操縦装置。
- 請求項18に記載の操縦装置であって、エネルギー源が細長い装置アセンブリの管腔内に統合されていることを特徴とする操縦装置。
- 請求項18に記載の操縦装置であって、紐の移動を測定することによって拡張可能構造体に対する位置を測定および追跡するように構成されている一組の位置センサが使用されることを特徴とする操縦装置。
