EDBフィブロネクチン
インターロイキン-12は、免疫系に対して複数の生物学的効果を有するヘテロ二量体サイトカインである。p35とp40の2つのサブユニットからなり、いずれもIL-12の活性型であるp70の分泌に必要である。インターロイキン‐12は樹状細胞(DC)に作用し、成熟と抗原提示の増加を導き、これは腫瘍特異的抗原に対するT細胞応答の開始を可能にすることができる。また、DCによるIL-12の分泌を促し、応答を増幅するポジティブフィードバック機構を作り出す。いったん応答が開始されると、IL-12は免疫系をTh1サイトカインプロファイルに向かわせ、CD4+T細胞にインターフェロンγ(IFN-γ)を分泌させ、CD8+細胞傷害性T細胞応答を導くという基本的な役割を果たす。

本発明は、とりわけ、がんを含むEDB-フィブロネクチンの発現に関連する疾患または障害を治療するための方法および組成物を提供する。本明細書に記載されるのは、IL-12のがん指向性送達を達成するための標的としてフィブロネクチンを利用する新しい組成物および方法である。このアプローチは、全身毒性を減少させ、IL-12の治療域を増加させる一方で、IL-12の治療可能性を十分に利用することを約束する。
IL-12およびEDBフィブロネクチン標的化ドメインを含む他の構築物は以前に記載されているが、現在開示されている組成物は驚くほど優れている。
別のフォーマットは、図7Bに示されるように、sc(IL-12)-SIP(L19)のホモ二量体であった。SIPフォーマット(「小免疫タンパク質」)は、出願人によりWO2003/076469において開発されており、「ミニ抗体」ともニックネームが付けられている。SIPは、CH4ドメインに結合したscFvを含む2つのサブユニットからなるホモ二量体である。二つのCH4ドメインはジスルフィド架橋によって互いに結合している。L19の腫瘍標的化特性がSIPフォーマットを用いて改善され得るという先行技術の指摘にもかかわらず、このコンジュゲートの腫瘍取り込みの増加は観察されなかった。

「腫瘍性疾患」という用語は、腫瘍および血液疾患を含む、悪性変質およびがんを包含する。

出願人らは驚くべきことに、特定のリンカーを用いてIL-12を一本鎖ダイアボディ(すなわち、WO2013/014149に開示された優れたフォーマット)に結合させると、より優れた腫瘍標的化性能ならびに優れた産生収率が達成され得ることを発見した。
(vi)Alpha3(vii)AP6および(viii)AP7とニックネームを付けられた3つの追加クローンを開発した。これら3つのクローンに関して、Chenら(2013)に報告された教示を検討し、実施した。このレビューは、堅い(rigid)リンカーがより良好な安定性を有し、サイトカインと抗体の間の正しい距離を維持し、従って治療効果を増加する可能性を示唆する。
リンカー(i)~(viii)のいずれも、この特異的免疫サイトカインにおいて以前に試験されていなかった。
材料および方法
実施例で試験された変異体は、以下の一般的な構造を有する:
huIL-12L19L19をコードする配列は、L19抗体およびIL12ペイロードという異なったPCR断片を組み立てることによって作製された。L19遺伝子を、適切なプライマーを用いて、あらかじめ生成された融合タンパク質L19-IL2鋳型からPCR増幅した。第2のL19遺伝子を適当なプライマーでPCR増幅した。
種々のヒトIL-12融合体の産生には、懸濁液中のCHO-S細胞を用いた。huIL‐12L19L19変異体は一過性遺伝子発現を用いて発現した。製造用4×106のCHO-S細胞を懸濁液中で1ml遠心分離し、CHO-Sに適した培地1mLに再懸濁した。次に、100万細胞当たり、プラスミドDNAの0.625μgに続いて、2.5μgのポリエチレンイミン(PEI;pH7.0の1mg/mL水溶液)を細胞に加え、穏やかに混合した。トランスフェクトした培養物を、31℃の振盪インキュベーターで6日間インキュベートした。
融合タンパク質の正しい分子量を、SDS‐PAGE10%およびSDS‐PAGE12%により還元および非還元条件下で分析した。
種々のIL-12融合物の正しい結合を確認するために、Elisaプレートを50ug/mlのフィブロネクチンドメイン7B89で一晩コーティングした(参照、WO2001/062800A1、その内容は参照により本明細書に援用される)。免疫サイトカインを10ug/mlおよび1ug/mlで試験した。二次試薬として、タンパク質A西洋ワサビペルオキシダーゼを用いた。このアッセイはBMBlue POD可溶性基質を用いて発色させた。333mMのH2SO4を加えて比色反応を停止し、マイクロタイタープレートリーダーを用いて波長450nmと650nmで吸光度を測定した。
Superdex200増加カラムを用いたAKTA FPLCシステムでサイズ排除クロマトグラフィーを行った。表面プラズモン共鳴実験親和性測定を、フィブロネクチン7B89ドメインコーティングCM5チップ上、精製huIL‐12L19L19クローンで、Biacore X100装置により行った。試料は連続希釈として注入され、濃度範囲は1μMから250nMであった。
様々なhuIL-12融合体のがん細胞への結合能を確認するため、凍結同系F9奇形腫検体の凍結切片(8um)上に免疫蛍光法を実施した。腫瘍切片は氷冷アセトン(5分)で固定した。固定後、カバースリップを洗浄し、PBS中の20%ウシ胎児血清で45分間ブロッキングした。濃度5μg/mlのHuIL‐12L19L19クローンを室温で1時間2% BSA/PBS溶液中に添加した。次に、カバースリップを、PBSで2回洗浄し、最終希釈1:1000の二次抗体マウス抗ヒトインターロイキン-12を2%BSA/PBS溶液中に室温で1時間添加した。その後、カバースリップを再度PBSで2回洗浄し、最終希釈1:500の三次抗体ヤギ抗マウスを添加した。DAPIを用いて核を対比染色した。
種々のIL-12融合体のin vivo腫瘍への結合能を確認するために、それらの標的化能を生体内分布分析により評価した。100μgの各IL-12L19L19クローンを125IおよびクロラミンT水和物で放射性ヨウ素化し、PD10カラムで精製した。放射性標識したタンパク質をF9マウス奇形腫を皮下移植した免疫適格マウスの側尾静脈に注射した。マウス1匹あたりの注射用量は4~9μgであった。注射24時間後にマウスを屠殺した。臓器の重量を測定し、Packard Cobra γカウンターを用いて放射能を計数した。代表臓器の放射能含有量は、組織1g当たりの注入量に対する割合(%ID/g ±標準誤差)で表した。
クローニング、発現およびSDS-PAGE
huIL‐12L19L19融合タンパク質の8つの変異体は、サイトカインとL19一本鎖ダイアボディの間の各々異なるポリペプチドリンカーでクローニングに成功した。SDS‐PAGEキャラクタリゼーションは、全ての変異体について約120kDaの分子量を示し、これは予想されるタンパク質サイズ(約109kDa、非グリコシル化)を確認した。(CHO-S細胞における一過性遺伝子発現による)発現は、すべての変異体について、3.5~5mg/Lの範囲であった。驚くべきことに、「SAD」とニックネームを付けられたクローンは、9mg/Lの収量を示し、これは、他の7つのクローンの収量よりも著しく高い。結果を図1に示す。
ELISAでは、8つのクローン、Alpha3、AP6、AP7、DDS、SES、AKKAS、(G4S)3およびSADはすべてについて、ヒトフィブロネクチンのドメイン7B89に対する結合(10μg/mlおよび1μg/ml濃度の両方)を確認した。結果を図2に示す。
ヒトフィブロネクチンのドメイン7B89で被覆したチップについて、Biacore分析により、より正確な親和定数の決定を行った(図3)。試料は連続希釈として注入し、濃度は1000nM、750nM、500nMおよび250nMに等しかった(図3)。見かけのKDはBiacore X100 Evaluation Softwareにより推定した。
8つのクローン、Alpha3、AP6、AP7、DDS、SES、AKKAS、(G4S)3およびSADを、サイズ排除クロマトグラフィー(SEC-200increase)で特徴付けしたところ、全てのクローンは、単量体免疫サイトカインに対応する主要ピークと同等のプロファイルを示した(図4)。
凍結同系F9奇形腫検体凍結切片(8um)上にクローンのAlpha3,AP6,AP7,DDS,SES,AKKAS,(G4S)3およびSADを用いて免疫蛍光法を行った。すべてのクローンは、陰性対照と比較して、血管系に特異的な結合を示した(図5)。
in vivo標的化は生体内分布解析により評価した。Alpha3、AP6、AP7、DDS、SES、AKKAS、(G4S)3およびSAD、ならびに陽性対照(マウスIL-12に結合したL19一本鎖ダイアボディ)の8クローンを、125Iで放射性ヨード化し、皮下移植したF9マウス奇形腫を有する免疫応答性マウスに注射した(4~9μgタンパク質/動物)。注射24時間後に計数した放射能は、すべてのクローンで腫瘍に集積を示したが、「SAD」クローンは他の7クローンと比較して腫瘍に優れた集積を示した。(図6)
さらなる一連の比較実験において、「SAD」リンカーもまた、結合能、単量体プロファイルおよび腫瘍標的化能に関して、WO2013/014149に開示された古い(およびより短い)GSADGGリンカー(配列番号26)よりも優れていることが驚くべきことに見出された。
材料および方法
この実施例で検証された変異体は、以下の一般的な構造を有する:
一本鎖ダイアボディフォーマット中のL19抗体(すなわち、それぞれhuIL-12L19L19「Old」およびhuIL-12L19L19「SAD」変異体)に6または15アミノ酸リンカーを介して融合されたhuIL-12を含む融合タンパク質を、上記の株に沿ってクローン化した。
一本鎖ダイアボディフォーマットにおけるL19抗体(すなわち、それぞれhuIL-12L19L19「Old」およびhuIL-12L19L19「SAD」変異体)に6または15アミノ酸リンカーを介して融合されたhuIL-12を含む融合タンパク質を、懸濁液適応CHO細胞培養における一過性遺伝子発現によって産生した。トランスフェクション後、細胞を、ProCHO-4培地(4mM超グルタミンを添加)中で、振盪条件下で31℃で6日間維持し、その後、培養上清を遠心分離によって回収し、さらに処理して、融合タンパク質を精製した。
トランスフェクトされたCHO細胞懸濁培養物を、4℃で5000rpmで30分間遠心分離した。上清は0.45umフィルターを用いた濾過によりさらに精製にした。タンパク質A樹脂を濾過した上清に添加し、混合物を振盪条件下で約1時間インキュベートした。次に樹脂を液体クロマトグラフィーカラムに回収し、「緩衝液A」(100mM NaCl、0.5mM EDTA、0.1% Tween 20 in PBS pH 7.4)で洗浄した後、「緩衝液B」(500mM NaCl 0.5mM EDTA in PBS pH 7.4)で2回目の洗浄を行った。huIL‐12を含む融合タンパク質を100mM TEAを用いて重力流によって溶出した。アリコートを採取し、融合タンパク質を含む画分をUVスペクトロメトリーによって確認して、プールし、PBSに対して一晩透析した。
融合タンパク質のサイズ排除クロマトグラフィーを、AKTA-FPLCシステム上の泳動緩衝液としてPBSを用いてSuperdex 200増加10/300 GLカラムを用いて実施した。100μlのタンパク質溶液をループに注入し、カラムに自動的に注入した。280nmでのUV吸光度を経時的に評価した。融合タンパク質のSECプロファイルをUNICORNソフトウェアのピーク積分関数を用いて分析し、全%面積またはピーク%面積に対する単量体画分の割合を定量した。試料負荷によるピークアーチファクトまたは試料緩衝液中に存在する塩を除外するために、保持容量5~17.5mLの間隔のみを定量に考慮した。
表面プラズモン共鳴実験親和性測定を、フィブロネクチン7B89ドメイン新鮮被覆CM5チップ上の精製「Old」および「SAD」クローンを有するBiacoreX100装置により実施した。試料を、250nM、125nMおよび62.5mMに等しい濃度の連続希釈として注入した(図10)。見かけのKDはBiacore X100 Evaluation Softwareにより推定した。
精製タンパク質サンプルhuIL-12L19L19「SAD」(リンカーGSADGGSSAGGSDAG、配列番号4)およびhuIL-12L19L19「Old」(リンカーGSADGG、配列番号26)(100g)を125IおよびクロラミンT水和物で放射性ヨウ素化し、PD10カラムで精製した。タンパク質はタンパク質A親和性クロマトグラフィー後に放射性ヨード化した。タンパク質を皮下移植したF9マウス奇形腫を有する免疫適格(129/Sv)マウスの外側尾静脈に注射した。マウス1匹あたりの注射用量は10~11gであった。注射24時間後にマウスを屠殺した。臓器サンプルの重量を測定し、Packard Cobra γカウンターを用いて放射能を計数した。異なる臓器におけるタンパク質取り込みを計算し、組織1g当たりの注射用量のパーセンテージ(%ID/g ±標準誤差)で表した。腫瘍へのタンパク質取り込みはTarliら(1999)に従い腫瘍成長により調整した。
融合タンパク質の発現と精製、サイズ排除クロマトグラフィー
huIL-12L19L19「SAD」とhuIL-12L19「Old」の2種類の変異体は、CHO細胞での一過性遺伝子発現によって作製された。実験は2回行われ、2組の産生実験が異なる日に行われ、それぞれバッチAおよびBが生成された。タンパク質-Aアフィニティークロマトグラフィーによる一段階精製、およびPBSによる透析に続いて、タンパク質試料の均一性をサイズ排除クロマトグラフィーによって評価した(図9)。両タンパク質変異体は、初期保持容積で溶出する高分子量変異体の存在によって強調されるタンパク質凝集を示した。どちらの場合も、huIL-12L19「SAD」は、UNICORNソフトウエアのピーク積分機能を用いたタンパク質の単量体部分の定量によって確認されたように、より良好なプロファイルを示した。実際、huIL-12L19「SAD」変異体は、huIL-12L19「Old」と比較して、より低い凝集傾向を示した。これは、ベースラインより上の曲線下総面積のパーセンテージとして単量体のピーク面積を考慮するか(平均値:それぞれ54.57%対46.69%)、または統合されたすべてのピークの合計のパーセンテージとして単量体のピーク面積(平均値:それぞれ58.83%対52.74%)を考慮した(表1)。
見かけのKDは、Biacore X100 Evaluation Softwareにより、「Old」クローン(huIL-12L19L19「Old」とリンカーGSADGG)では6.7nM、「SAD」クローン(huIL-12L19L19「SAD」とリンカーGSADGGSSAGGSDAG)では3.8nMと推定された(図10)。
in vivo腫瘍標的化
注射24時間後に計数した放射能は、「SAD」クローンが「Old」クローンと比較して予想外に優れた腫瘍取り込みを有することを示した(図11)。
huIL-12L19L19「SAD」変異体の有効性は、悪性黒色腫、非小細胞肺がん(NSCLC)、腎細胞がん、尿路上皮がん、頭頸部扁平上皮がん(HNSCC)、高頻度マイクロサテライト不安定性(MSI-H)又はミスマッチ修復機構欠損(dMMR)を有する転移性結腸直腸がん、肝細胞がん、胃がん、皮膚の扁平上皮がん、子宮頸がん、びまん性大細胞型B細胞リンパ腫(DLBCL)を有するヒト患者において評価される。少なくとも1つの患者コホートでは、直前の治療として投与された免疫チェックポイント遮断療法をベースとしたレジメンで、疾患の進行が実証されている。
治療開始から6ヵ月間、または同意の撤回または進行性疾患がみられるまで患者を追跡する。
実施形態の第1のセットによれば、以下が提供される:
1.a)第1および第2サブユニットを有するヘテロ二量体IL-12タンパク質、
b)一本鎖ダイアボディ、および
c)IL-12タンパク質と一本鎖ダイアボディとの間のリンカーを含むコンジュゲートであって、リンカーはSADを含むアミノ酸モチーフを含む、コンジュゲート。
2.上記SADリンカーが、アミノ酸モチーフモチーフGSADGGSSAGGSDAG(配列番号4)を含む点1に記載のコンジュゲート。
3.ヘテロ二量体IL-12タンパク質の第1のサブユニットがp40であり、第2のサブユニットがp35である、点1~2のいずれかに記載のコンジュゲート。
4.一本鎖ダイアボディが単一特異性または二重特異性である、点1~3のいずれかに記載のコンジュゲート。
5.一本鎖ダイアボディがフィブロネクチンのエクストラドメインB(ED-B)に結合する、点1~4のいずれかに記載のコンジュゲート。
6.一本鎖ダイアボディが2つのL19 VHドメインおよび2つのL19 VLドメインを含む、点1~5のいずれかに記載のコンジュゲート。
7.完全長構造[p40]-[リンカー1]-[p35]-[SADリンカー]-[L19VH]-[リンカー3]-[L19VL]-[リンカー4]-[L19VH]-[リンカー3]-[L19VL]を有する点1~6のいずれかに記載のコンジュゲート。
8.配列番号16による全長配列を有する点1~7のいずれかに記載のコンジュゲート。
9.・腫瘍性疾患と診断されているか、
・腫瘍性疾患に罹患しているか、または
・腫瘍性疾患を発症するリスクがある
ヒト又は動物対象の治療におけるか、またはそのような状態の予防のための、上記点のいずれかに記載のコンジュゲートの(医薬品の製造のための)使用。
10.ヒトまたは動物の対象における血管新生の抑制における上記点のいずれかに記載のコンジュゲートの(医薬品の製造のための)使用。
11.点1~8のいずれかに記載のコンジュゲートを少なくとも含み、および任意に1種以上の薬学的に許容される賦形剤を含む医薬組成物。
12.(i)点1~8のいずれか1つに記載のコンジュゲート、または点11に記載の医薬組成物、および(ii)1つ以上の治療的活性化合物を含む組合せ。
13.ED-Bフィブロネクチンの発現または過剰発現に関連する障害または状態を治療または予防する方法であって、それを必要とする対象に、点1~8のいずれか1項に記載のコンジュゲートの有効量、点11に記載の医薬組成物、または点12に記載の組合せを投与することを含む、方法。
14.a)点1~8のいずれか1つに記載のコンジュゲート、点11に記載の医薬組成物または点12に記載の組合せ、
b)前記コンジュゲート、組成物または組合せを投与するための装置、および
c)使用説明書を含むパーツの治療用キット。

12.ED-Bフィブロネクチンの発現または過剰発現に関連する障害または状態を治療または予防する方法であって、それを必要とする対象に、点1~7のいずれか1項に記載のコンジュゲートの有効量、点10に記載の医薬組成物、または点11に記載の組合せを投与することを含む、方法。

6.一本鎖ダイアボディが、配列番号7および5に記載の抗体L19のVHおよびVLドメインを含む、点1~5に記載のコンジュゲート。
7.一本鎖ダイアボディが、a)点6の重鎖/軽鎖可変ドメイン配列対の少なくとも1つを含み、前記ドメインの少なくとも1つが、それぞれ、配列番号7または配列番号5に対して≧80%の配列同一性を有する、および/またはb)点6の重鎖/軽鎖可変ドメイン配列対を有する、ただし、ドメインの少なくとも1つは、フィブロネクチンのエクストラドメインB(ED-B)に結合する能力を維持しながら、それぞれ、配列番号7または配列番号5に対して≧10のアミノ酸置換を有する、点1~6のいずれかに記載のコンジュゲート。
8.一本鎖ダイアボディにおける少なくとも1つのアミノ酸置換が保存的アミノ酸置換である、点1~7のいずれか一項に記載のコンジュゲート。
9.一本鎖ダイアボディが、点5、または点6の抗体の1つと比較して、フィブロネクチンのエクストラドメインB(ED-B)に対して≧50%の標的結合親和性を有し、かつ/または点5、または点6の抗体の1つとフィブロネクチンのエクストラドメインB(ED-B)への結合について競合する、点1~8のいずれか一項に記載のコンジュゲート。
10.一本鎖ダイアボディが2つのL19 VHドメインおよび2つのL19 VLドメインを含む、点1~9のいずれか一項に記載のコンジュゲート
