「キメラペプチド」は、互いに自然状態では会合しない2つのコンポーネントのペプチドが、融合タンパク質として又は化学的連結により互いに連結されたペプチドを意味する。
本発明は、内在化ペプチド連結薬理学的薬剤を送達させる方法であって、内在化ペプチド誘導性の炎症反応を抗炎症剤の共投与、又は内在化ペプチドにビオチン又は類似分子を連結することによって阻害する、方法を提供する。このような方法は、実施例で記載される結果の一部を前提とし、その実施例では、tatに連結された薬理学的薬剤の高用量での投与に続いて直ぐにマスト細胞脱顆粒、ヒスタミン放出、及びヒスタミン放出の典型的な後遺症(発赤、熱、腫脹、及び低血圧)を含む炎症反応が起きる。本発明の方法の実施はメカニズムの理解に依存していないが、マスト細胞脱顆粒は、IgE抗体反応により誘起されているというよりはむしろ、カチオン性のtatペプチドとマスト細胞との直接の相互作用により誘起されると信じられる。炎症反応は、抗炎症剤、特にマスト細胞脱顆粒阻害剤(例えばクロモリン)と、tat又は他の内在化ペプチド連結薬理学的薬剤とを共投与することによって阻害することができる。抗ヒスタミンとコルチコステロイドを含む、広く用いられている種々の抗炎症剤が適している。また、発明者らは、炎症反応を誘導する内在化ペプチドの能力はそれらをビオチン又は類似分子に連結させることにより低下させることができる点を見出した。

内在化ペプチド(別名、細胞膜トランスダクションペプチド又は細胞透過ペプチド) は、多くの細胞又はウイルスタンパク質が膜を横断できるようにする比較的短い(例えば、5-30のアミノ酸)ペプチドの公知のクラスである。かかるペプチドは、通常以上にアルギニン及び/又はリジン残基が(一般のタンパク質と比較して)現れていることから、典型的にはカチオンの電荷を有し、膜を横切ってのそれらの通過を容易にすると考えられている。かかるペプチドのいくつかは、少なくとも5、6、7又は8つのアルギニン及び/又はリジン残基を有する。例としては、アンテナペディアタンパク質(Bonfanti, Cancer Res.57、1442-6(1997))(及びその変異体)、ヒト免疫不全ウイルスのtatタンパク質、タンパク質VP22、単純ヘルペスウイルス1型のUL49遺伝子産物、ペネトラチン(Penetratin)、SynB1及び3、トランスポータン、アンフィパシック(Amphipathic)、gp41NLS、ポリアルギニン、並びにいくつかの植物及び細菌性のタンパク質毒素(例:リシン、アブリン、モデシン(modeccin)、ジフテリア毒素、コレラ毒素、炭疽菌毒素、熱不安定性の毒素及び緑膿菌外毒素A(ETA))が含まれる。その他の例は、次の文献に記載される(Temsamani, Drug Discovery Today, 9(23):1012-1019, 2004;De Coupade, Biochem J., 390:407-418, 2005;Saalik Bioconjugate Chem.15:1246-1253, 2004;Zhao, Medicinal Research Reviews 24(1):1-12, 2004; Deshayes, Cellular and Molecular Life Sciences 62:1839-49, 2005)(すべての文献は参照により組み込まれる)。
本発明者らは、内在化ペプチド(例:tat)が、被験体への投与で炎症反応を誘導する能力があることを見出した。上記炎症反応は、通常、ペプチド投与の1、5、10、20、30、又は60分以内に検出されるが、ペプチド投与の24時間以内には典型的に消失する(上記ペプチドは再投与されないと仮定する)。上記炎症反応は用量依存的である。炎症反応は上記ペプチドの再投与の際に同様の強度で典型的に再発する。炎症反応の一態様は、しばしば、内在化ペプチドを投与した後の約0-30分の期間内で発生する一過性の血圧低下である。
多種多様な抗炎症剤が上述の炎症反応のタイプに関する1又は複数の態様を阻害するのにすぐに利用できる(参照:例えば米国特許番号6,204,245であって、引用により組み込まれる)。
内在化ペプチドにより誘導される炎症反応を抑制する代替的な又は付加的な方法は、内在化ペプチドをビオチン又は類似分子に連結して抱合体を形成することである。上記抱合体は、連結された薬理学的薬剤が細胞へ取り込まれることを促進する能力を保持するが、ビオチンを有しない同一の内在化ペプチドに比較して炎症反応の低減を誘導する。抱合された内在化ペプチドは、所望の取り込み及び結果として生じる免疫反応の欠如(低減)を確認するためにスクリーニングされ得る。
表1にリストされた薬理学的薬剤及び関連する状態により例示されるように、広い範囲の患者が本発明の方法により治療可能である。本方法は、内在化ペプチドにより生じる任意の炎症を悪化させるだろう状態の患者(例:高血圧、脈拍の増加、又は炎症の他の兆候若しくは症状に罹患する患者)に特に有用である。本方法は、内在化ペプチド連結薬理学的薬剤を高用量必要とする治療方法に特に有用でもある。厳密には、もし炎症反応があるならば炎症反応の存在と程度を決定づけるのは、連結された薬理学的薬剤というよりもむしろ内在化ペプチドの用量である。しかし、内在化ペプチドの用量は、もちろん連結された薬理学的薬剤の用量により決定される。例えば、炎症反応は、1.5mg/kgより多い内在化ペプチドの用量で、はっきりと分かるようになるかもしれない。いくつかの疾患の治療では、薬理学的薬剤及び結果として連結された内在化ペプチドの有効量が低すぎて、ほとんどの患者に炎症反応を誘導することができない。それにもかかわらず、炎症反応に対する個々の患者の感度はばらつきがあり、軽度の抗炎症剤(例:ヒスタミン)を用いた治療は今でも価値ある予防措置とすることができる。
内在化ペプチド連結薬理学的薬剤が抗炎症剤と共に投与される方法において、その2つの構成要素が十分近接した時間内に投与され、上記抗炎症剤が上記内在化ペプチド誘導性の炎症反応を阻害することができる。上記抗炎症剤は、薬理学的薬剤の前、同時、又は後に投与されてもよいが、好ましくは前に投与される。好ましい時間は、抗炎症剤の薬物動態及び薬動力学に一部依存する。薬理学的薬剤が投与される時に、上記抗炎症剤がほぼ最高血清濃度になるように、抗炎症剤は薬理学的薬剤前の期間において投与される。典型的には、抗炎症剤は、薬理学的薬剤投与の6時間前と1時間後との間に投与される。例えば、抗炎症剤は、薬理学的薬剤の1時間前から30分後までに投与することができる。好ましくは、抗炎症剤は、薬理学的薬剤の30分前から15分後まで、より好ましくは薬理学的薬剤の15分前から同じ時間までに投与される。いくつかの方法において、抗炎症剤は、薬理学的薬剤が投与される前であって、薬理学的薬剤が投与される15分前、10分前又は5分前に投与される。いくつかの方法において、薬剤は、薬理学的薬剤の1-15分前、1-10分前又は1-5分前に投与される。
<a) 治療方法>
内在化ペプチド連結薬理学的薬剤を含むキメラ薬剤は、効果的な投与量、投与回数、及び投与経路にて投与され、治療中の疾患を患っている患者において少なくとも一つの疾患の兆候若しくは症状のさらなる悪化を治癒、低減、又は阻害する。治療的に有効な量とは、本発明のキメラ薬剤で治療していない疾患若しくは状態を罹患している患者(又は動物モデル)のコントロール群における損傷に比べて、本発明のキメラ薬剤で治療された疾患に罹患している患者(又は動物モデル)の群において、治療される疾患若しくは状態の少なくとも一つの兆候若しくは症状のさらなる悪化を治癒、低減、又は阻害するのに十分有意なキメラ薬剤の量を意味する。本発明の方法で治療しない比較対象患者のコントロール群における平均予後よりも、個々の治療を受けた患者の方がより好ましい予後を達成する場合、その量は治療的に有効であるとも考えられる。治療的に有効な投与計画は、意図した目的を達成するのに必要な投与回数及び経路にて治療的に有効な用量を投与することに関する。
本発明はまた、疾患のリスクがある被験体において、上記疾患の予防のための方法及び組成物も提供する。通常このような被験体は、コントロール群に比べて、障害(例:状態、病気、障害、又は疾患)を発症する可能性が増大することを示す。例えば、上記コントロール群は、障害と診断されたことがない、又は家族歴を有しない一般群(例えば、年齢、性別、人種、及び/又は民族が適合した群)から無作為に選択される1又は複数の個人を含むことができる。上記障害と関連する「リスク因子」が被験体と関連することが分かった場合、上記被験体は上記疾患のリスクがあると考えてもよい。リスク因子には、例えば被験体群に関する統計学的若しくは疫学的調査を通して所定の障害と関連のある、任意の活性、体質、発症、又は性質が含まれる。従って、その基礎的リスク因子を同定する調査が具体的に被験体を含まなかった場合でさえも、被験体は上記障害のリスクを有するとして分類してもよい。心臓手術を受けていない被験体群に比べて、それを受けた被験体群において一過性脳虚血発作の頻度が増加するため、例えば、心臓手術を受けている被験体は一過性脳虚血発作のリスクがある。
本発明のキメラ薬剤は、医薬組成物の形で投与することができる。医薬組成物は、典型的にはGMP条件下で製造される。医薬組成物は、単位用量形態(即ち、単一の投与のための投与量)で提供することができる。医薬組成物は、混合、溶解、造粒、糖衣錠形成、粉末化(levigating)、乳化、カプセル化、封入(entrappi ng)、又は凍結乾燥工程の通常の手段で製造することができる。例えば、凍結乾燥された本発明のキメラ薬剤は、以下で説明する処方及び組成物中で使用してもよい。
キットが、本発明の方法を実行するために提供される。上記キットは、内在化ペプチドに結合した、1つ又は複数の目的の薬理学的薬剤を含む。上記内在化ペプチドはビオチン化されてもよく、及び/又は上記キットは抗炎症剤を含んでもよい。インスタントキットには、任意ではあるが、薬理学的薬剤、及び/又は抗炎症剤を投与する手段が含まれる。上記キットは、1つ又は複数の緩衝液、添加剤、充填剤、又は希釈剤を含むこともできる。上記キットは、従うべき投与及び用量計画に関する、1つ又は複数の印刷された説明書を提供してもよい。
<A. 内在化活性の測定>
変異体(tat又は他の内在化ペプチドの変異体)は、動物中で透過活性をテストすることができる。内在化ペプチドは、単独で、又は活性薬剤(例えばKLSSIESDV(配列番号:5)のような活性ペプチド)に連結された場合にテストしてもよい。内在化ペプチド(任意ではあるが、活性薬剤(例:ペプチド)に連結したもの)は、好ましくは、蛍光ラベル(例:塩化ダンシル)でラベルされる。内在化ペプチドは、それから動物(例:マウス)の末梢に注射される。例えば、腹腔内、又は静脈注射が適している。注射後約1時間に、マウスは屠殺され、固定液(生理食塩水中で、3%パラホルムアルデヒド、0.25%グルタルアルデヒド、10%ショ糖、10U/mLのヘパリン)で灌流される。その後、脳は取り出され、冷凍され、そして切片にされる。切片は、共焦点顕微鏡を用いて蛍光解析される。任意ではあるが、ポジティブ及びネガティブコントロールと比較して内在化活性を蛍光から測定する。適切なポジティブコントロールは、テストされる内在化ペプチドとして、(もし存在すれば)同一の活性ペプチドに連結された標準tatペプチドである。適切なネガティブコントロールは、内在化ペプチドに連結されていない蛍光ラベルされた活性ペプチドである。ラベルされていない媒体もネガティブコントロールとして使用することができる。
薬剤に連結された内在化ペプチドを含むキメラ薬剤の活性が、脳卒中の様々な動物モデルにおいてテストすることができる。そのようなモデルの一つにおいて、成体雄スプラーグドーリーラットを、90分間、腔内縫合法(36、37)により一過性の中大脳動脈閉塞(MCAO)にさらす。動物を一晩絶食し、硫酸アトロピン(0.5mg/kg IP)を注射する。10分後、麻酔を行う。ラットに口から挿管し、機械的に換気し、臭化パンクロニウム(0.6mg/kg IV)を用いて麻酔する。体温を、加熱灯を用いて36.5~37.5℃で維持する。大腿動脈及び静脈においてポリエチレンカテーテルを用いて、継続的に血圧を測定し、ガスとpH計測のために血液を採取する。一過性MCAOは、90分間、内頚動脈を介してウィリス動脈輪にポリ-L-リジンコートされた3-0モノフィラメントナイロン糸(Harvard Apparatus)を導入することで、効果的に中大脳動脈を閉塞することにより実現される。これは、大脳皮質及び大脳基底核を取り囲む広範な梗塞を作りだす。動物を、テスト対象のキメラ薬剤又はネガティブ若しくはポジティブコントロールのいずれかを用いて治療する。治療は、虚血の誘導前かその1時間後までのいずれかであってもよい。ネガティブコントロールは媒体であってもよい。ポジティブコントロールは、先に効果があることが示されたtat-NR2B9cペプチド(YGRKKRRQRRRKLSSIESDV(配列番号:6))とすることができる。MCAOの45分前に、単一の静脈内ボーラス投与によりキメラ薬剤を送達する(3nmoles/g)。動物に化合物を投与後、梗塞容積及び/又は障害指数を測定する。梗塞容積は、通常、治療の24時間後に測定する。しかし、より遅延した時期(例:3、7、14、又は60日)に測定してもよい。障害指数は、例えば、治療後2hr、治療後24hr、治療後1週間及び1ヶ月の時期にモニターしてもよい。化合物で治療していないコントロール動物に比べて、梗塞容積及び/又は障害指数が統計学的に有意に減少することを示すキメラ薬剤は、本発明の方法の実施に有用な活性を有するとして同定される。
<Tat-NR2B9cの神経保護効力に関する性差のインパクト>
Tat-NR2B9cの神経保護効力を、脳卒中のインビボ軟膜3血管閉塞(P3VO)モデル(Forder JP, Munzenmaier DH, Greene AS、ラットにおけるアンジオテンシンIIタイプ1受容体遮断を有する局所虚血からの血管形成保護、Am J Physiol Heart Circ Physiol 2005 April;288(4):H1989-H1996)を用いて雄と雌ラットとの両者において評価した。
<動物>
ウィスカーバレル皮質上の中大脳動脈の3終枝に恒常的軟膜血管閉塞(P3VO)を施す前に、成体スプラーグドーリーラット(10~12週齢)(雄~300g、雌~250g)を、12~18時間絶食させた。Tat-NR2B9cの試験を雄ラットと生理食塩水コントロール群(各グループ、n=8)とで行った。Tat-NR2b9c及び生理食塩水コントロールを雌ラット(各グループ、n=8)で試験した。研究者は、手術時から梗塞サイズの解析までの間、治療群に対して盲検で行った。
ラットは、0.5ml/kgの筋中注射(ケタミン(100mg/kg)、アセプロマジン(2mg/kg)及びキシラジン(5mg/kg)をして麻酔し、必要に応じて初期用量の3分の1を補充した。肛門温度プローブを挿入して、動物は37℃に維持された加温パッドの上に動物を置いた。右外頸動脈(ECA)に、色素注射のためにPE10ポリエチレンチューブを用いてカテーテル挿入をした。正中切開を介して頭蓋骨を剥き出しにし、組織を含まないよう表面をこすり、側頭筋は右側頭蓋骨から離断した。解剖顕微鏡と空気圧歯科用ドリルを使用して、硬膜を無傷のままに保ちながら頭蓋骨を4角形にドリルして頭蓋骨のその断片を持ち上げることにより、右体性感覚皮質(ブレグマから尾側2mm及び側方5mm)に6x4mmの頭蓋窓を作製した。人工脳脊髄液で洗った後、皮質の表面にある血管を介しての通過を実証するために、通常の生理食塩水中に溶かした生体用色素パテントブルーバイオレット(10mmol/L;Sigma)の小さなボーラス(10~20μL)を右外頸動脈に注射した。バレル皮質周囲にあるMCAの3つの重要な動脈枝が選択され、そして硬膜を通して電気焼灼した。焼灼後にボーラス注射及び色素通過を繰り返して、焼灼された動脈が遮断されたことを確実にした。上記4角形の頭蓋骨をその窓の上に置き、そして頭蓋骨を縫合した。カテーテルをECAから除去し、ECAを結紮し、そして前頸部を縫合した。局所閉塞の開始1時間後、0.3mlの薬物(3nMol/g体重)又は生理食塩水コントロールを、尾静脈から0.06ml/minの速度で注入した。ラットが十分に回復するまで、体温を維持するために加熱灯下の独立したケージに、各々のラットを戻した。餌と水を自由供給した。
手術24時間後、1mLのペントバルビタールで動物を再麻酔し、そして脳を速やかに回収した。梗塞領域から冠状切片を取り出し、2%塩化トリフェニルテトラゾリウム(TTC)中で15分間37℃でインキュベートした。画像をスキャナーで取り込み、脳切片を-80℃で保管した。梗塞サイズを、この研究では、各ラットの半球に対するパーセントとして測定した。梗塞サイズの測定を行った後、その動物をそれぞれのグループに分けた。治療グループ間で平均±SEとして比較した。
ラットにおける脳卒中のP3VOモデルは、雄と雌の両SDラット中で強くそして再現性のある梗塞を生じた。P3VO手術を行った24時間後に有意に減少した梗塞サイズが観察されるので、tat-NR2B9cペプチドは雄と雌との両者において神経保護的である(図1)。脳卒中の1時間後にTat-NR2B9c(3nM/g)を用いての治療は、両性の動物中で梗塞を劇的に減少させた(図1)。同等濃度のTat-NR2B9cで治療した雌ラットにおいては梗塞の完全な欠如が見られるので、この神経保護反応は雄よりも雌中でより明白に見えた。しかしながら、生理食塩水で治療したコントロールによると、雄ラットよりも雌ラット中で平均梗塞サイズは小さい(71%)ことを示している。
<Tat配列を含むペプチドはインビトロでヒスタミン放出を伴うマスト細胞脱顆粒を誘導する>
<方法>
<細胞培養>
C57マウスを70%エタノールで滅菌し、そして皮膚と結合組織を除いて大腿骨を解離した。骨髄細胞を採取し、そして、OPTI-MEM(Gibco)(5%熱失活化FBS、6%WEHI条件培地(IL-3の供給源として)、及び55μM □-2メルカプトエタノールを含む)に再懸濁した。細胞は約1x106cells/mLで培養された。2日後、細胞を回収及び遠心分離し、そのペレットを新しい培地を用いて新しいプレートに蒔いた。新しいWEHI条件培地を毎週加えた。その細胞が>95%マスト細胞になった後、約4週間、その細胞を培養し、マスト細胞脱顆粒アッセイのために使用した。
マスト細胞脱顆粒アッセイキット(CHEMICON、Temecula、CA)を用いてトリプターゼ活性を測定した。単離後、その細胞を洗浄し、そして1Xアッセイバッファー中に約1x106 cells/mLで再懸濁した。TAT-NR2B9C又は他のペプチドを用いた処理のため、以下の濃度の溶液(0.125mg/mL、1.25mg/mL、12.5mg/mL、又は125mg/mL)50μLをその細胞懸濁液に加えた。500nMのA23187(カルシマイシン)(マスト細胞におけるトリプターゼ放出の誘導因子)をポジティブコントロールとして使用した。細胞を37℃、5%CO2で60分間インキュベートした。細胞懸濁液を700xgで遠心分離し、そしてその上清を注意深く回収した。アッセイ混合物(キット中で提供されるもの)を96穴マイクロタイタープレート中に準備した。20μLのトリプターゼ基質を各実験及びコントロールウェルに加えることにより比色反応を開始した。サンプルを37℃で60分間インキュベートした。マイクロプレートリーダーにおいて吸光度を405nmで測定した。

図2に見られるように、Tatトランスダクションドメインを含むペプチドはすべて、マスト細胞脱顆粒を引き起こしたが、NR2B9cペプチド(Tat配列を含まない)は引き起こさなかった。インビトロのマスト細胞脱顆粒アッセイは、抗体の非存在下で行われた。従って、任意のマスト細胞脱顆粒は免疫現象によるものと言うことはできない。特に、RT-PCR及びウエスタンブロットを使用して、我々はTat-NR2B9cの治療標的であるPSD-95をマスト細胞が含むかどうかを検討した。我々はこれらの細胞中にPSD-95を検出することができなかった(結果は示されない)。この結果からマスト細胞脱顆粒がTat -NR2B9cとPSD-95との相互作用により引き起こされそうにないとする更なる証拠が提供された。
<Tat配列を含むペプチドの抱合体は、インビトロにおけるマスト細胞脱顆粒を誘導できない>
実施例2に記載された方法を使って、マスト細胞脱顆粒に対する、Tat含有ペプチドへのある種の修飾(例:抱合)の効果を調べた。修飾されたペプチドには、Tat-NR12B9c、Tat-NR2B9cのD-異性体(D-Tat-NR2B9cと名付けられる)、ビオチン抱合Tat-NR2B9c、ビオチン抱合AMP-KLSSIESDV(配列番号:5)が含まれる。図5に示されるように、ビオチン抱合Tat又はAMPペプチドは、マスト細胞脱顆粒を誘導できなかった。
<ビーグル犬を用いた動物試験において、Tat-NR2B9cはヒスタミンレベルとヒスタミン反応の増加を誘発する>
GLPでの14日間静脈内投与毒性試験を未処置のビーグル犬(3/性別/グループ)(CRM試験番号:501448)で実施した。その中で、動物に0、0.25、1.0、又は10mg/kgのTat-NR2B9cを毎日注射した。血液サンプル(約1mL)を投与の1、6、及び12日前、並びに注射の5及び15分後にすべての動物から採取した。血液サンプルを、静脈穿刺(頸静脈、伏在静脈、及び橈側皮静脈)によって、EDTAを含むチューブに回収した。サンプルを、それから冷却遠心分離器(約4℃)中で、2700rpm、10分間遠心分離(回収の30分以内に)した。適切なラベルを付した第2のチューブに血漿を分離して、CRMでの解析まで-80℃で保管した。血漿サンプルをヒスタミンレベルの検討に用いた。Tat-NR2B9cを静脈内に投与された動物からのサンプルを、正当と認められる方法を使用して解析した。
非拘束覚醒ビーグル犬におけるGLP循環器系遠隔計測試験(CRM試験番号:691106)において、投薬の段階と段階の間に3日間の休薬期間を設けて、Tat-NR2B9cの用量を増加させながら(0.25、1.0、又は5.0mg/kgペプチド総量)、6匹の動物(雄3匹、雌3匹)に投与した。0.25又は1.0mg/kgでは、血圧への作用は観察されなかった。5mg/kgでは、6匹のイヌ中4匹で一過的な血圧降下が観察され、約30分持続した。血圧の降下が用量に関連している可能性があるかもしれないとする所見は、その降下が、アレルギー(抗体媒介)免疫応答によるものではなかったことを示す。更に、低用量(0.25mg/kg)と高用量(5.0mg/kg)との間の期間は約6日間である。即ち、免疫応答を発生可能にするには不十分な期間である。従って、その作用は、ヒスタミン放出を引き起こすマスト細胞の直接の脱顆粒により引き起こされる。
一回の低速静脈注射によりビーグル犬に投与されるTat-NR2B9cの用量範囲を調べるために、非GLP試験を実施した。2匹の動物(ビーグル犬雄一匹及び雌一匹)に、Tat-NR2B9cを7回静脈内投薬した。その投薬と投薬との間には、3~4日間の休薬期間があった。第一回目の用量は2.5mg/kgで与えた。その動物は任意の毒性兆候を示さなかったので、2回目の用量は7.5mg/kgで投与された。雄動物は、頭部軟部組織の血管神経性浮腫、及び特に腹部の腹側面のじん麻疹型反応を呈した。雌イヌでは反応がなかった。バイタルサイン(心拍数、血圧、呼吸数、及び体温)は、両動物において正常な生理学的範囲内に留まった。3回目の用量は、12.5mg/kgで与えた。投薬後、血管神経性浮腫及びじん麻疹が、両動物で観察された。雄イヌの反応は中程度と評価され、雌動物では反応は軽度であった。その次の用量は20.0mg/kgにセットした。投薬後、雄動物はショックに落ち入り、血圧(BP)及び脈拍は検出されなかった。ベネドリルとデキサメタゾンのi.v.投与でその動物を治療した。投薬後5分のBPは、37/13mm Hg(イヌにおける正常なBPは~160/90)として記録された。雌動物に投薬しないよう決定した。
<抗ヒスタミンを用いた治療は、イヌにおけるTat-NR2B9cにより誘導される症状を阻止する>
Tat-NR2B9cの30分前に投与される1mg/kgのベネドリルを用いた前投与の後、実施例4の両動物に、12.5mg/kgのTat-NR2B9cを投与した。雄動物中に、耳の真皮の軽微な発赤があった。Tat-NR2B9c投与の15~20分後に、雄動物は嘔吐もした。雌イヌには反応が観察されなかった。従って、抗ヒスタミン薬ベネドリルの前投与は、以前観察された、両動物における同一用量レベルのTat-NR2B9cでの血管神経性浮腫及びじん麻疹反応を阻止した。この結果は、抗ヒスタミン(例:ベネドリル)、及びコルチコステロイド(例:デキサメタゾン)が、マスト細胞脱顆粒の有害事象を効果的に治療することを示している。
<Tat-NR2B9cがヒトにおいて血中ヒスタミンの上昇を誘発するとする直接証拠>
<方法>
我々は、ヒトにおけるTat-NR2B9cの安全性、許容性、及び薬物動体試験を実施した。被験体は、18歳以上の正常、健康、非喫煙男性、又は閉経後若しくは外科的に生殖不能な女性被験体のいずれかであった。被験体は、点滴静脈内投与(10±1分)で、Tat-NR2B9c(ロット番号:124-134-001B)を投与されるか、又はプラセボ(リン酸緩衝生理食塩水)(ロット番号:124-134-001A)を与えられるかのいずれかであった。4人の被験体がコホート1から3の各々において投薬され、そして、10人の被験体がコホート4から8の各々において投薬された。62人の被験体のすべてが試験を完了した。各コホートの治療期間は以下である:コホート1:2006年9月14日、コホート2:2006年9月26日、コホート3:2006年10月6日、コホート4:2006年10月20日、コホート5:2006年11月6日、コホート6:2006年12月4日、コホート7:2006年12月17日、コホート8:2007年2月25日。
試験期間中、薬物動体解析のため、11の血液サンプルを各被験体から以下のタイムポイントで回収した:投薬後0.00(投薬前)、0.08(5分)、0.17から0.25(各々の独立した薬物注入の終了から正確に10から15分)、0.33(20分)、0.50、0.75、1.00、2.00、6.00、12.00、及び24.00時間。それに加え、ヒスタミン分析のために、各被験体から以下のタイムポイントで8つの血液サンプルを回収した:投薬後0.00(投薬前)、0.08(5分)、0.17(10分)、0.25、0.50、1.00、2.00、及び24.00時間。
試験経過中少なくとも一回投薬を受けたすべての被験体において、安全性試験を行った。すべての有害事象(AE)の出来事を治療と被験体番号により集計した。バイタルサインの絶対値、心電図(ECG)パラメータ、実験パラメータ及び身体診察も文書化され、正常範囲外の値に印をつけた。ベースライン値からシフトしたものを集計した。AEは、研究者及び国際医薬用語集 (MedDRA)の用語を使って文書化した。
<パート1:血中ヒスタミンレベルへのTat-NR2B9cの作用>
投薬による異常ヒスタミン結果の要約が、表7で説明される。3.75mg/kg用量群での8被験体中7人は、NA1投与開始から10分後に10nmol/Lよりも高い(平均24.3nmol/L;最大39.8nmol/L)ヒスタミンレベルを有していた。上記被験体のうち3人は、NA1投与開始から15分後に10nmol/Lよりも高い(平均15.3nmol/L;最大20.3nmol/L)のヒスタミンレベルをまだ有していた。
試験に参加した40人の被験体は、上記試験中に総数168個の有害事象(AE)を経験した。AEの大半は重症度が軽度なものであった。プラセボ治療を受けた16人中6人の被験体(37.5%)は少なくとも1つのAEを経験した一方で、実薬治療を受けた46人中34人の被験体(73.9%)は少なくとも1つのAEを経験した。2.60及び3.75mg/kg用量群の被験体は、それより低い用量群の被験体よりも有意に多くのAEを経験した。重度の有害事象(SAE)は報告されなかった。Tat-NR2B9cを施された被験体が経験した最も共通する有害事象は、熱感(46分の13; 28.3%)、そう痒症(46分の12; 26.1%)、潮紅(46分の10; 21.7%)、及び口渇(46分の9; 19.6%)であった。すべてのAEは、血中グルコースが増加した2例を除いて消散したため、被験体を経過観察することはできなかった。
<材料>
AnaSpec Inc (San Jose, CA)によって化学的に合成されたTAT-NR2B9c。Sickkids Hospital Advanced Protein Technology Centre (Toronto, ON, Canada)によって化学的に合成されたRv-Tat-NR2B9c。全てのペプチドは、高性能液体クロマトグラフィーによって、>95%まで精製した。ペプチドストック(3mM)を無菌の生理食塩水にて調製し、一定量に分割して-80℃で保存した。クロモリン、ピリラミン、ラニチジン、オキサトミド及びデキサメタゾンは、Sigma-Aldrich (St. Louis, MO)から購入した。
Rv-Tat-NR2B9cを虚血モデルにおけるTat-NR2B9cと比較した。Rv-Tat-NR2B9cは、ペプチドのtat部分におけるN-C由来のアミノ酸順序がRv-Tat-NR2B9cにおいて逆転していることを除いては、Tat-NR2B9cと同様である。
上述のロドキサミドとTat-NR2B9cとの共製剤はTat-NR2B9cとの比較において試験し、媒体コントロールは上述のラット3PVO脳卒中モデルにて試験した。結果として生じる梗塞の領域は、図15に示される。Tat-NR2B9cは、媒体コントロールと比較して梗塞サイズを著しく阻害した。しかしながら、ロドキサミドとTat-NR2B9cとの組合せは、Tat-NR2B9c単独と比較して統計的に有意に低減させるという驚くべき結果になった。したがって、ロドキサミドの末梢への共投与は、Tat-NR2B9cを起因とする炎症を低減させるだけでなく、梗塞を低減させるというその有効性も増加させる。
Claims (9)
- 内在化ペプチド連結薬理学的薬剤を含む医薬組成物であって、
前記医薬組成物は、マスト細胞脱顆粒阻害剤と組み合わせて脳卒中を患っているかそのリスクがある被験体における前記脳卒中の治療又は効果的な予防に使用するためのものであり、
前記薬理学的薬剤は、NMDAR 2BにPSD95が結合することを阻害し、
前記マスト細胞脱顆粒阻害剤は、前記内在化ペプチドによって誘導されるマスト細胞脱顆粒を阻害することができる、及び/又は、前記マスト細胞脱顆粒阻害剤は、前記薬理学的薬剤の30分前から15分後までに投与され、
前記薬理学的薬剤は、ペプチドであって、そのC末端に配列番号38のC末端モチーフを有するペプチドであり、
前記内在化ペプチドは、配列番号:51、配列番号:1、配列番号:2、配列番号:3、配列番号:4又は配列番号:70の1-11番目のアミノ酸を含む配列を有し、
前記内在化ペプチドは、インベルソ型であり、
前記マスト細胞脱顆粒阻害剤は、トラニラスト、ロドキサミド、アゼラスチン、ベポタスチン、クロルゾキサゾン、エピナスチン、イソプロテレノール、ケトチフェン、ネドクロミル、オロパタジン、ペミロラスト、ピメクロリムス、ピルブテロール、クロモリンビス(アセトキシメチル)、クロモリン二ナトリウム、又はクロモリンのいずれかである、医薬組成物。 - 前記マスト細胞脱顆粒阻害剤は、ロドキサミドである、請求項1記載の医薬組成物。
- 前記マスト細胞脱顆粒阻害剤は、末梢ルートにより投与される、請求項1に記載の医薬組成物。
- 前記マスト細胞脱顆粒阻害剤は、前記薬理学的薬剤の投与の30分前から15分後までに投与される、請求項1に記載の医薬組成物。
- 前記マスト細胞脱顆粒阻害剤は、前記薬理学的薬剤の投与の15分前から同じ時間までに投与される、請求項1に記載の医薬組成物。
- 前記被験体は、疾患のエピソードを患っており、
前記薬理学的薬物及び前記マスト細胞脱顆粒阻害剤は、前記疾患のエピソード中に一回投与される、請求項1記載の医薬組成物。 - 前記マスト細胞脱顆粒阻害剤の投与は、前記薬理学的薬剤なしに前記マスト細胞脱顆粒阻害剤を患者に投与するという反復投与計画には適合しない、請求項1に記載の医薬組成物。
- 前記マスト細胞脱顆粒阻害剤は、経口的に、又は静注で投与された時に脳で薬理効果を発揮するのに充分な量で血液脳関門を通過しない、請求項1に記載の医薬組成物。
- 前記マスト細胞脱顆粒阻害剤は、経鼻的に、経口的に、又は静注で投与される、請求項1に記載の医薬組成物。
