Description
-骨伝導支持体および
-挿入補助部。
-治癒の良好な過程の間、骨伝導支持体は、特に有利な方法で、内因性の骨に取り込まれるかまたは細胞に分化転換することができる。このように、骨欠損部の生物学的再建を確保することができる。骨成長の遅い患者では、骨伝導支持体は、好ましくは、少なくとも十分な二次固定が達成されるまで保持される。これは、骨成長が起こらなくなることが多い高齢の患者に関して特に有利である。
-さらなる利点は、骨伝導支持体は、圧縮された、好ましくは嵌入した(impacted)(締め付けられたまたは押し込まれた)状態で、プレースホルダおよび/またはガイド構造体として作用して、組織、特に新生骨組織がインプラント内に、特に治療対象である骨欠損部内に成長するのを可能にすることができる。
-さらなる利点は、特に、骨伝導支持体が、圧縮された、好ましくは嵌入した(締め付けられたまたは押し込まれた)状態にあるとき、インプラントに対し、永久的に、特に均質的に負荷をかけることができるということである。均質なインプラントの負荷は、骨成長に対して基本的な必要条件である。たとえば、特に、圧縮された、好ましくは嵌入した状態にある骨伝導支持体を備えるインプラントは、最大10MPaの圧負荷で長期間負荷をかけることができる。ここで、圧縮、特に嵌入により形成され、かつ骨伝導支持体によって生成される構造体は、特に有利に、特に5%~15%の弾性変形と、特に50MPa~300MPaの低い弾性係数とを示すことができる。
-一般的なインプラント、特に、インプラントの縁領域における中間空間のみに力学的負荷がかかる金属増強材料とは対照的に、骨伝導支持体が圧縮状態にあるとき、存在する中空および/または中間空間に対して、弾性係数が低いため、完全な欠損部充填において、すなわち均質に(微小移動)力学的負荷をかけることができる。これにより、骨成長、特に骨再生の刺激および/または促進がもたらされる。全体的に、これにより、骨欠損部全体の均質な骨形成を達成することができる。
-挿入補助部を用いて、骨伝導支持体の治療対象である骨欠損部内への挿入、したがって特にインプラントの手術での取扱いが、使用者、好ましくは外科医に対して特に有利な方法で容易になる。
-挿入補助部のさらなる利点は、特に、特に外科手術中に、骨伝導支持体を合わせて保持することにより、治療対象である骨欠損部内の骨伝導支持体の望ましくない転位または散乱が防止されるということである。言い換えれば、挿入補助部により、治療対象である骨欠損部内の骨伝導支持体の局所結合を達成することができ、したがって、それらの骨伝導性および支持体に関連して言及した上記利点を、特に好都合な方法で明示することができる。たとえば、挿入補助部は、骨欠損部内の圧縮、特に嵌入中の支持体の少なくとも一部の制御されない散乱なしに、骨欠損部内への挿入後に支持体の可能な限り完全な圧縮、特に嵌入を可能にする。このように、支持体により、特に有効な骨伝導ガイド構造体を構築することができ、それはさらに、十分な二次固定、特に生物学的再建に対して必須である。

- 特に骨欠損部を治療するためのインプラントであって、
骨伝導支持体と、
挿入補助部と、
を備え、
前記挿入補助部が、前記骨伝導支持体を骨欠損部に挿入するように、かつ前記骨伝導支持体を合わせて保持するように設計されており、
前記挿入補助部が、テキスタイル扁平構造体と結合剤との組合せとして構成されており、かつ
前記骨伝導支持体が、ペースト状または捏和可能な調製品の形態での前記結合剤を用いて、互いに結合されており、
さらに下記特徴(a)または(b)を有する、インプラント:
(a)前記結合剤が、前記結合剤の総重量に基づき60wt%~90wt%の量の液体希釈剤を含む、
(b)前記骨伝導支持体が、縦長構造要素を備える成形体である。 - 前記骨伝導支持体が、燐灰石および/またはリン酸三カルシウムを含み、または燐灰石および/またはリン酸三カルシウムからなることを特徴とする、請求項1に記載のインプラント。
- 前記燐灰石および/または前記リン酸三カルシウムが、1%~50%、特に5%~20%、好ましくは10%~15%の多孔率を有することを特徴とする、請求項2に記載のインプラント。
- 前記燐灰石および/またはリン酸三カルシウムが、多孔質であるように構成されていないことを特徴とする、請求項2に記載のインプラント。
